Abstract:
The incarceration of persons found guilty of various crimes who are also chronic substance abusers presents an important opportunity for treatment. It is an important opportunity because they would be unlikely to seek treatment on their own, without treatment they are very apt to continue their drug use and criminality after release, and cost effective drug abuse treatment methods are now available to treat them while in custody (both during incarceration and aftercare) and significantly alter their lifestyles. Correctional authorities should now feel optimistic that chronic heroin and cocaine users with predatory criminal histories can be treated effectively. This article shares the success of the Stay'n Out and Cornerstone Programs that have been successful with serious drug abusing offenders, and the factors that make for success. It is the proper program components joined by thoughtful leadership in the right setting. These principles are generalizable and transferable to many locations.

BACKGROUND: The 2013 American College of Cardiology (ACC)/American Heart Association (AHA) cholesterol protocols recommend the use of Pooled Cohort Equations to estimate 10-year and life time Atherosclerotic cardio-vascular disease (ASCVD) risk as a guide for primary prevention treatment options. Many providers underutilize this important tool.OBJECTIVES: To observe resident physicians’ HMG COA inhibitor (statins) prescribing pattern, with particular attention to appropriate dosing as per 2013 ACC/AHA Cholesterol Guidelines, at the University of Toledo Family Medicine Residency Program and to increase Resident Physicians’ awareness of the ASCVD risk calculator as a tool to improve appropriate statin dosing.METHODS: A retrospective, observational, cross-sectioned chart review was performed to analyze pre-existing data collected from a patient population within a defined time period. The study included 237 patient charts, who received care from among 12 Family Medicine Residents.RESULTS: The success rate for correct statins prescriptions for first year residents was 63%, including 24 correct dose prescriptions out of 38 patients total. Second year residents’ success rate increased to 73%, representing 58 correct dose prescriptions out of 80 patients total. Third year residents success rate was 63% with 75 correct dose prescriptions out of 119 patients total.CONCLUSION: Out of 237 chart reviewed, 157 patients received appropriately dosed statin prescriptions, representing a success rate of 66%. This suggests that across all levels of resident training, there is room of improvement in the utilization of the 2013 ACC/AHA lipid lowering guidelines.

Abstract:
For the past few years, Soviet historians have fixed their attention on the problem of "alternatives," a shorthand for wide-ranging attempts to free historical thinking from the overly determinist schemes of Stalinist orthodoxy. The question is posed most often as the possibility of a more humane alternative to Stalin and the political order associated with his name. Some historians and publicists, however, have gone beyond the Stalin period to reflect on, for example, how even the 1917 revolutions might have been avoided. The leadership under Mikhail Gorbachev and reformist allies among the intelligentsia have singled out the New Economic Policy, or NEP, as the legitimate socialist forerunner to the present reformist programs. In so doing, they approach consensus with an influential group of western historians, including Stephen Cohen, Moshe Lewin, and Robert Tucker, who have kept alive the memory of Nikolai Bukharin in particular, but a non-Stalinist path to socialism as well.

Abstract:
This article examines the state-sponsored rise of local patriotism in the post- 1961 period, interpreting it as part of the effort to strengthen popular support for and the legitimacy of the Soviet regime during the second phase of de- Stalinization. It shifts the analytical focus away from the Secret Speech of 1956, the time of Nikita Khrushchev's full-scale assault on Iosif Stalin and his legacy, to the Twenty-Second Party Congress of 1961, the inauguration of a Utopian and pioneering plan to build communism by 1980. I consider how this famously forward-looking program gave rise to an institutionalized retrospectivism, as Soviet policymakers turned to the past to mobilize popular support for socialist construction. I examine how this process played out in the Russian northwest, where Soviet citizens were encouraged to turn inward, to examine their local history and traditions, and to reread these through a socialist lens.

Aim. To determine the features and main problems of statin therapy, as well as assess the possibility of achieving the target level of lipid pattern in patients with high and very high cardiovascular risk (CVR) in real clinical practice.Material and methods. The design of the “PRIORITET” observational program is an open observational study. Patients with high and very high CVR were divided into 3 groups in accordance with the initial data: (1) not taking statins, (2) taking statins, but not reaching the target low-density lipoprotein cholesterol (LDL-C) level, (3) taking statins with the achievement of the target LDL-C level, which is justified in replacing the statin inside the class — adverse effects (AE), high price, etc. Within 12 weeks 3 visits of patients to hospitals were carried out: baseline visit (B0), visit 1 month after the study initiation (B1) and visit 3 months after the study initiation (B3). The choice of atorvastatin or rosuvastatin was assessed by the doctors.Results. Groups 1, 2 and 3 included 112, 170 and 16 people, respectively. At B0, 145 (48,7%) patients were prescribed atorvastatin, and 153 (51,3%) — rosuvastatin. Three people dropped out of the study to B3, 295 patients completed the program. Lipid pattern of 285 patients were analyzed: 121 (41%) people (101 with very high CVR and 20 with high CVR) achieved the target LDL-C level, the remaining 164 (59%) patients (CVR — 156 and 8, respectively) — no. The most pronounced dynamics of LDL=C level was revealed in group 1, the differences between group 1 and groups 2 and 3 are highly statistically significant (p<0,0001). There were no differences in the frequency of reaching the target LDL-C level between patients taking atorvastatin or rosuvastatin. The target level of LDL-C (p=0,003) in the treatment of rosuvastatin in patients with high CVR was reached significantly more often than in patients with very high CVR. Also 3 non-serious AEs were reported. On average, in 9% of cases, reaching the target level of LDL-С during visits B1 and B3 was wrong interpreted by the attending physicians.Conclusion. The main problems of statin therapy in real clinical practice are the wrong interpretation of reaching the target level of LDL-C, inertness of doctors in titrating of statins doses and achieving the target level of lipid pattern. It may be the cause of reduced efficiency and deterioration of lipid-lowering therapy results in patients with high and very high CVR. The results of the “PRIORITET” study demonstrated the possibility of improving the practice of statins use and its accordance with clinical guidelines.Skibitsky V. Abstract:
On 1 December 2016, British Columbia’s (BC) provincial drug insurance program changed which medications in certain classes would benefit under the insurance program in an attempt to reduce expenditure. As part of the modernization, HMG-CoA reductase inhibitors (Statins), Angiotensin converting enzyme inhibitors (ACEI), angiotensin receptor blockers (ARB), and dihydropyridine calcium channel blockers (CCB) were affected. Prescribers and pharmacists had six months to discuss the changes with patients, and change medications if deemed necessary. Purpose: To quantify the changes made to prescriptions and to adjust to the Modernized Reference Drug Program. Methods: A retrospective chart review was conducted at two clinics in Prince George, BC. Charts for patients that were prescribed any drugs in the affected classes were reviewed to determine if, and when, they had been changed, and by which health care professional. In December 2016, a clinical pharmacist, integrated within the study clinics, informed prescribers of the changes, and made patient-specific clinical notes within the charts. The notes described the changes and recommended alternative agents and appropriate dosing in order to assist the prescriber to have a conversation with the patient regarding the switch. Results: Out of 429 unique patients, 233 patients were prescribed a Statin, 229 patients an ACEI, 110 an ARB and, 83 a CCB. Sixty-five drug changes were indicated to reflect the modernization, and with guidance from a clinical pharmacist, nurse practitioners (NPs), and family physicians (FPs), 65% of these identified drugs were switched to reflect the modernization. Community pharmacists made no drug changes in the study sample, despite the prescriptive authority and compensation available to do so. Province-wide, approximately 21% to 33% of affected drugs were switched during the same time-frame. Direct collaboration between a clinical pharmacist, working alongside NPs and FPs, was more successful in optimizing these medications when compared to standard practice, or community pharmacists alone.

A comparison has been made of two methods for electron-density map improvement by the introduction of atomicity, namely the iterative skeletonization procedure of the CCP4 program DM [Cowtan & Main (1993). Acta Cryst. D49, 148–157] and the pseudo-atom introduction followed by the refinement protocol in the program suite DEMON/ANGEL [Vellieux, Hunt, Roy & Read (1995). J. Appl. Cryst. 28, 347–351]. Tests carried out using the 3.0 Å resolution electron density resulting from iterative 12-fold non-crystallographic symmetry averaging and solvent flattening for the Pseudomonas aeruginosa ornithine transcarbamoylase [Villeret, Tricot, Stalon & Dideberg (1995). Proc. Natl Acad. Sci. USA, 92, 10762–10766] indicate that pseudo-atom introduction followed by refinement performs much better than iterative skeletonization: with the former method, a phase improvement of 15.3° is obtained with respect to the initial density modification phases. With iterative skeletonization a phase degradation of 0.4° is obtained. Consequently, the electron-density maps obtained using pseudo-atom phases or pseudo-atom phases combined with density-modification phases are much easier to interpret. These tests also show that for ornithine transcarbamoylase, where 12-fold non-crystallographic symmetry is present in the P1 crystals, G-function coupling leads to the simultaneous decrease of the conventional R factor and of the free R factor, a phenomenon which is not observed when non-crystallographic symmetry is absent from the crystal. The method is far less effective in such a case, and the results obtained suggest that the map sorting followed by refinement stage should be by-passed to obtain interpretable electron-density distributions.

Background and aim: This study was to evaluate atorvastatin in the management of hypercholesterolemia of cardiovascular risk patients as well as the implication of National Cholesterol Education Program (NCEP) Adult Treatment Panel (ATP) III guidelines in Bangladesh.Methods: A total 1685 patients aged 28 years or older who had evidence of hypercholesterolemia with or without Coronary Heart Diseases (CHD) and cardiovascular risk factors were assigned to receive atorvastatin for 2 months. The change of fasting LDL at baseline visit and after 2 months of the statin treatment was measured.Results: One thousand six hundred (95%) patients out of 1685 who were assigned to receive atorvastatin with TLC were found significant mean reduction (26.1%?) in their LDL levels from baseline visit. Triglycerides and total cholesterols were also reduced (16.6% ? and 21.6% ? respectively) wheras high-density lipoprotein was increased (16.5% ?) significantly. LDL of 23% patients was decreased more than 30 mg/dl after 2 months of atorvastatin treatment. Fifty nine percent of the patients reached the treatment goal of reducing LDL > 20 mg/dl.Conclusion: Atorvastatin would achieve a good effective control in the management of hypercholesterolemic patients with or without CHD and risk factors following the guidelines of US NCEP ATP III. Abstract:
Background: Leptospirosis is a bacterial, zoonotic disease associated with environmental factors and human behavior related to occupation. This research aimed to investigate the association between human behavior and leptospirosis. Case mapping and topographical mapping were done for a comprehensive visualization.Methods: A matched case-control study design was conducted in Gunungkidul, Indonesia, from December 2017 to January 2018. Cases were selected from those reported as suspicious of leptospirosis by the Gunungkidul District Health Office during 2017 and controls were matched according to sex and age. Chi-squared, Fisher exact test, and Odds ratios were employed to find out the association between exposure and outcome for a significance level of 0.05. Quantum GIS-Web Map-Stamen terrain was used to overlay case and landscape.Results: Bivariate analysis showed that four exposure variables that enhanced the risk for leptospirosis though not significantly associated were history of injuries, habit of taking a bath or wash the clothes in the river, not using personal protection during work and presence of an animal fence surrounding the house. Most of the leptospirosis cases (>70%) resided in a hilly area. This finding assists in developing prevention strategies concerning leptospirosis infection.Conclusions: Human behaviour is vital in leptospirosis prevention. Accordingly, this study can broaden the understanding horizon, particularly for a decision-maker where and how to implement the Leptospirosis prevention program. Prevention should address the current situation in the field and based on population and local wisdom to result in the successful implementation.

Abstract:

The Black community has historically been beset by much higher school suspension rates when compared with other racial groups. The U.S. Department of Education reports that black K-12 students are 3.8 times more likely to receive school suspensions as white students. Researchers have determined that suspensions also enhance dropout rates for students. Across all racial demographics, dropping out of school promotes negative social behaviors and below average successes in a youth’s lifetime, which is more evident in the Black community. Black suspended youth, if not assisted, have the potential to become high-school dropouts which may result in becoming juvenile delinquents, social nonconformists, and later underachieving adults. This doctoral project consists of assessing two existing programs for black youth through IN-AGAPĒ Family Life and Educational Center organization: (1) Stay Up While You Are Out, which supports youth who are suspended for a short-term or a long-term from the Newport News Public School system, and (2) One More Chance, which supports the youth that is recommended to the program by the Newport News Juvenile Court system but can return to school after a period of court-imposed probation.

Abstract:
The purpose of this study was to determine if there were differences between 5th to 8th grade youth who stayed enrolled and those youth who dropped out of the Pima County Extension 4-H Youth Development Program. Youth who dropped out were surveyed with the National 4-H Impact Assessment Survey. The survey examined eight critical elements of positive youth development and asked the additional question why they dropped out. The research questions sought to find if there was a difference between 5 th to 8th grade youth who stayed enrolled and those who dropped out of the Pima County 4-H Youth Development Program. What perceptual differences were there between the genders and between the groups, stayed enrolled and dropped out? Were there differences amongst the grade levels of this study? The theoretical foundation for this research study centered on Erikson's psychosocial stage theory and crises, Vygotsky's theory of the zone of proximal development, and Gilligan's theory on gender development. It was found that there were differences between grade level, gender, and group (those who stayed enrolled versus those who dropped out). Amongst female respondents, significant factors were "Feelings about 4-H", "Learning in 4-H", "Helping Others", "Planning and Decision-Making in 4-H", and "Belonging in 4-H". For male respondents, there were no conclusive patterns for the factors between the two groups. Significant factors for grade level were: "Adults in 4-H", "Learning in 4-H", "Helping Others", and "Planning and Decision Making in 4-H". The significant factors for grade level were "Belonging in 4-H" nor "Feelings About 4-H".

This book that you have in hand was built from the master's research entitled "Environmental Education at school as a tool for the resignification of the identity of the place" concluded in the year 2019 in the Graduate Program in National Network for the Teaching of Environmental Sciences (PROFCIAMB), authored by Cliciane de Souza Meduna under the guidance of Prof. Dr. Helena Midori Kashiwagi. The research was carried out at the Escola Municipal do Campo Cipriano Librano Ramos, in Colônia Pereira, a rural area of the municipality of Paranaguá, Paraná coast. This memory game was created from photos recorded by students of basic education participating in the research. "Rediscovering the Pereira Colony" is composed of images that have meaning and are part of the historical context of the community. We understand that recognizing oneself within one's physical space goes beyond working on geography, sense of location, map interpretation, data collection and everything else that is involved in this process. It is to plant the seed that "it is possible to be "someone" in life even staying in the field" (original title of my Project to join PROFCIAMB), is to understand that many struggles are to come, that we will have many challenges to overcome, difficulties to be overcome, among them our location, but that it will not be an obstacle to continue studying and be who we want.

Shnookal illustrates how and why Cold War scare tactics were so effective in setting the airlift in motion, focusing on their context: the rapid and profound social changes unleashed by the 1959 Revolution, including the mobilization of 100,000 Cuban teenagers in the 1961 national literacy campaign. Other reforms made by the revolutionary government affected women, education, religious schools, and relations within the family and between the races. Shnookal exposes how, in its effort to undermine support for the revolution, the U.S. government manipulated the aspirations and insecurities of more affluent Cubans. She traces the parallel stories of the young “Pedro Pans” separated from their families—in some cases indefinitely—in what is often regarded in Cuba as a mass “kidnapping” and the children who stayed and joined the literacy brigades. These divergent journeys reveal many underlying issues in the historically fraught relationship between the U.S. and Cuba and much about the profound social revolution that took place on the island after 1959.

Although the Dowers are stationed on the other side of Mwanza, Peppy and Aileen often met for lunch in Mwanza, and Peppy sought out Aileen’s marriage experience to understand her own marriage problems.

The study was carried out to explore the experiences of frontline library staff and users during the COVID-19 pandemic. With the unprecedented nature of the pandemic, institutions, including universities, have found means of staying afloat through the use of technology to offer services virtually. Makerere University rolled out an emergency program for online, distance, and e-learning, and the library has been supporting learning and research, since March 2020. The chapter sought to find out the experiences of library staff at the forefront and users in order to understand how online information services can be enhanced and retooled to support eLearning. Structured telephone interviews were conducted, and findings show virtual services that have been rendered to staff, students, and researchers as well as challenges. Challenges included equipment, poor infrastructure, and data costs. The university has signed up with major service providers to alleviate some of the handicaps to learners' virtual experiences.

In this article, the author presents Robotics-based learning interventions and the experiences at 56th Junior High School of Athens within the RoboESL Erasmus project; as well as a teaching approach using Educational Robotics. The RoboESL project aims to exploiting the potential of robotics for developing extra-curricular constructivist learning activities in schools that will help children at risk of failure or Early School Leaving (ESL) practice and develop their creativity skills, raise self-esteem, motivate their interest in schooling, and finally encourage them towards staying at school. During the implementation, students worked in a constructionist learning environment and were engaged in team activities. The author runs the project for two consecutive school years using EV3 Lego Mindstorms and participated in dissemination events organizing workshops where the students participated in the program taught elementary school pupils.

Social justice education is a viable alternative to reach out to all participants with equity towards construction of democracy. In this chapter, the authors share about their co-teaching experience in a required Multicultural Education course for pre-service teachers where a social justice framework guided their work. They expand on the course objectives and their social justice aims. They describe how their praxis was conducive to building community in the classroom and being with each other. They expose and explore, however, a misalignment between theory and praxis surrounding social justice education when preservice teachers transition from teacher preparation programs to their own classrooms as practicing teachers. They describe through the concrete experience of one of the authors how practicing teachers are faced with different particular variables that may hinder the full realization of a social justice approach to education.

Heat recovery steam generators (HRSGs) downstream of gas turbines are often used in combination with process steam applications. A HRSG trip is more severe in applications with highly important process steam than in applications producing only electricity. HRSGs in important process steam applications can be equipped with a supplementary and fresh air firing system having the capacity of replacing at least the total waste heat coming from the gas turbine. A fresh air firing system provides the capability of keeping the HRSG in operation without the gas turbine running. If the HRSG is required to stay in operation even after a gas turbine trip, a change over from waste heat firing to fresh air firing has to follow immediately. The gas turbine speed rundown after a trip occurs very rapidly, so the change over procedure has to be carried out within a few seconds to avoid a HRSG shut down. Enormous gradients of heat input and steam mass flow extracted from the HRSG occur in such cases. This will cause the HRSG parameters to deviate considerably from the steady state, thus conventional HRSG calculations cannot be used in such cases. The thermal inertia of the HRSG needs to be considered, which requires the use of special software programs. A commercial boiler software program with a dynamic calculation module for unsteady calculations was utilized and a comparison with data gathered from the operation of the HRSG was performed. The boiler performance parameters during change over procedures were investigated in detail using extensive measurements at an Austrian steel mill (VOESTALPINE). The parameters of this investigation were compared with calculation results gained from a commercial software program for validation purposes. This comparison will enable predictions to be made for future projects with sufficient accuracy, which will allow the risk to be reduced when offering guarantees in this regard.

Abstract: The research focuses on the possibility of transferring theoretical ideas of Le Corbusier into educational programs of the very young children – between three and six. The worldwide development of civilization changed the natural environment of the human. For the average European citizen a city is more natural place for living than a forest. Simultaneously, in these days many inhabitants present an extremely conformist approach to life and to the surrounding space. The participation of members of the society in the shaping of public spaces is possible only through the involvement and practice, but the democratic responsibility does not appears out of nowhere. It must be fostered and nurtured as early as in childhood. According to developmental psychology, children in the age of 3-6 are very susceptible to the acquisition of new skills and learn it in an intuitive way. The proper education program using Le Corbusier’s lectures and theory could help them to understand the space better. The seeming simplicity of above rules is an advantage in this case – thereby it can be explained to even such an audience as small children. On the other hand, some kind of abstract and hidden difficulty included in this theory becomes an opportunity to create a very absorbing and stimulating workshops that follow the needs of younger and older children. Le Corbusier’s legacy includes not only physical issues and can be used in many different ways. As Pablo Picasso once stated: every child is an artist, the problem is staying an artist when you grow up. For those reasons, incorporating such an innovative strategy for kids’ education reveals a great potential. Resumen: Los estudios realizados enfocan en comprobar las posibilidades de usar las ideas teóricas de Le Corbusier en los programas educativos para niños de tres a seis años. El desarrollo de la civilización ha cambiado el entorno natural del ser humano - una ciudad para el ciudadano promedio de Europa es un lugar más natural para vivir que el bosque. Al mismo tiempo, hay que reconocer que muchas personas muestran el enfoque muy conformista a la vida, tanto al espacio circundante. Esta situación se podría mejorar mediante la participación consciente de los miembros de la sociedad en la creación del espacio público, su compromiso y la práctica. Pero la responsabilidad democrática no aparece sola, se debe estimularla y nutrir ya en la infancia. De acuerdo con la psicología del desarrollo, los niños de 3 a 6 años son muy susceptibles a la adquisición de nuevas habilidades y aprenden de una manera intuitiva. Programa educativo que utiliza los cursos y la teoría de Le Corbusier puede ayudarles a entender mejor el medio ambiente. La aparente sencillez de los principios proclamados por él en este caso es una ventaja. La abstracción y escondidas dificultades incluidas en ellos,nos permiten a crear actividades estimulantes que correspondan a las necesidades de los niños pequeños, tanto y mayores. It is known that operational costs of health care organizations constitute an important part of health expenditure in the world. Careful examination of all kinds affecting costs is inevitable in order to provide more effective and efficient health care services. For this reason, in the study, impact of factors affecting hospitalization period on costs is mentioned. The purpose of the study is to measure the effects of these factors on costs by identifying the factors prolonging the duration of patients’ hospital stay. It is regarded that the factors extending the duration of patients’ hospital stay increase the costs. In the scope of the study, 405 patients who are discharged from Cerrahpaşa Faculty of Medicine Hospital Pediatric Nephrology Service in 2012 are discussed. Variables of the study, the average hospitalization days, number of patients, the maximum number of patients, bed occupancy rate, bed turnover and bed units are used in the rev range. The data of the patients included in our study has been gathered with the help of retrospective study area, retrospective method, scanning archives, and financial epicrisis in the system of hospital automation. The obtained data have been studied on standard deviation, maximum and minimum value by making use of the excel programme. As a result of studies based on these reviews, it has been found out that the factors extending the period of patients’ hospital stay increase the costs.

A study was carried out to determine the probability of avoiding severe weather conditions generated by historical hurricanes to improve the management of dynamically-positioned (DP) drilling vessels operating in the Gulf of Mexico during hurricane season. Rather than staying on location during an approaching hurricane, DP drilling vessels secure well operations, pull riser, and evade the hurricane. The study considers several factors affecting the measure of success including inherent errors in forecasting hurricane strength, track and speed of advance; types and speeds of vessels; vessel sensitivity to wind and wave conditions; evasion time and direction of evasion; location of the drilling site relative to the hurricane track; and escape route water depth limitations. The results provide a statistical basis for the required evasion time and would be part of the Hurricane Evacuation Plan that stakeholders agree upon to achieve acceptable risk. The simulations could also be used in a training program for captains and considered in the tactical decision-making process regarding when to leave the site and in which direction to go.

The staggering cost of energy has become a major challenge for industrial facilities in their constant effort to reduce product cost. For some manufacturing industries in the United States, the energy cost has even exceeded labor cost of their products. Staying viable in a world with a large appetite for energy resources depends on making energy efficient practices a part of day-to-day operation. Engineering programs need to prepare prospective engineers to become critical thinkers in the area of energy usage. This paper outlines the process of industrial energy audit projects conducted in senior level thermal-fluid design classes at the University of Wisconsin–Platteville (UWP) and University of Arkansas at Little Rock (UALR). In the frame of project work, each student team visited a local industrial facility, found out facility’s energy consumption data, identified and evaluated opportunities for energy saving. Total of eight projects were carried out for diverse manufacturing industries such as iron melting foundry, engine industry, aircraft and vehicle industries in Northeast Wisconsin and Central Arkansas. The energy audit reports included energy assessments of various systems such as fans, air compressors and compressed air lines, lighting, and motors. The energy audit reports also evaluated redesign of a large cooling water pipe network and design of a Rankine cycle to utilize the large amount of heat wasted during the iron melting process.

ABSTRACT Background: Hospitals are demanded to be more efficient and effective in providing medical services to avoid losses in the era of National Health Insurance. The increase in cases of dengue hemorrhagic fever (DHF) is a burden on the cost of medical services in Indonesia. This study aimed to examine the determinants of hospital lost under Indonesia Case-Based Groups (INA-CBGs) reimbursement for patients with dengue hemorrhagic fever (DHF). Subjects and Method: A cross-sectional study was carried out at dr. Soeroto hospital, Ngawi, East Java, from September to October 2019. A sample of 200 in-patients was selected by simple random sampling. The dependent variable was tariff difference between INA-CBGs and hospital cost. The independent variables were class of treatment, length of stay, blood transfusion, co-morbidity, and complication. The data were obtained from DHF in-patients’ medical record. The data were analyzed by path analysis model run on Stata 13. Results: Tariff difference was directly and positively affected by length of stay (b= 2.77; 95% CI= 1.78 to 3.75; p<0.001), blood transfusion (b= 2.95; 95% CI= 0.36 to 5.54; p= 0.025), patients age (b= 0.09; 95% CI= -1.52 to 1.71; p= 0.907), co-morbidity (b= 0.58; 95% CI= -1.07 to 2.22; p= 0.491), and complication (b= 0.34; 95% CI= -1.69 to 2.36; p= 0.743). Tariff difference was directly and negatively affected by type of treatment (b= -0.81; 95% CI= -2.02 to 0.40; p= 0.191). Tariff difference was indirectly affected by co-morbidity, complication, class of treatment, and patient age. Conclusion: Tariff difference is directly and positively affected by length of stay, blood transfusion, patient age, co-morbidity, and complication. Tariff difference is directly and negatively affected by type of treatment. Tariff difference is indirectly affected by co-morbidity, complication, class of treatment, and patient age. ABSTRACT Background: The government’s efforts to improve public health level are by providing excellent health service facilities, including promotion, preventive, curative, and rehabilitative. The outcome of quality health service can be measured by patient perception and satisfaction. This study aimed to examine factors affecting the perceived quality of service and patient satisfaction on inpatient care of Nabire Hospital, Papua, Indonesia. Subjects and Method: A cross-sectional study was carried out at Nabire regional hospital, Papua, Indonesia, from March to Mey 2020. A sample of 207 inpatients was selected by stratified random sampling. The dependent variable was patient satisfaction. The independent variables were age, income, and length of stay type class health insurance, working, and patient perception toward doctor, nurse, and inpatients facilities. The data were collected by questionnaire and analyzed by a multiple logistic regression. Results: Inpatients satisfaction decreased with age ≥50 years (OR= 0.72; 95% CI= 0.24 to 2.65; p= 0.720), income ≥Papua minimum wage (OR= 0.77; 95% CI= 0.22 to 2.73; p= 0.685), and length of stay ≥7 days (OR= 0.13; 95% CI= 0.03 to 0.53; p= 0.004). Inpatients satisfaction increased with class 2 and 3 (OR= 1.15; 95% CI= 0.43 to 3.07; p= 0.773), non national health insurance (OR= 1.21; 95% CI= 0.46 to 3.23; p= 0.700), working (OR= 2.13; 95% CI= 0.58 to 7.85; p= 0.258), good patient perception toward doctor (OR= 3.03; 95% CI= 1.15 to 7.99; p<0.001), good persepsi patient perception toward nurse (OR= 4.04; 95% CI= 1.15 to 14.17; p<0.001), and patient perception toward inpatients facilities (OR= 26.8; 95% CI= 11.0 to 65.32; p<0.001). Conclusion: Inpatients satisfaction decreases with age ≥50 years, income ≥Papua minimum wage, and length of stay ≥7 days. Abstract:
Steady natural convection heat transfer from vertical array of equally-spaced isothermal horizontal cylinders has been investigated experimentally and numerically. Experimental study was carried out at different ambient temperatures in a conditioned room which can be maintained at a stable required value and inside a sufficiently designed test cabin. The ambient and cylinders’ surface temperatures varied 20°C to 30°C and 30°C to 60°C respectively. The experimental apparatus was designed to adjust different operating parameters such as number of cylinders, cylinders’ surface temperatures, distance between the cylinders and environmental temperature. Each cylinder surface temperature can be accurately adjusted to the desired temperature by means of specially designed measurement and control system. Copper test cylinders have length of 1 m and outer diameter of 4.8 mm. The uncertainty analysis method proposed by Kline and McClintock was used and explained elaborately. Detailed information and algorithm of numerical method are given to ease the understanding of the numerical part of the study. The problem was solved numerically by means of a CFD program in 2D. Average Nusselt numbers are given based on the experimental data for single and each two horizontal cylinders. Heat transfer rates obtained from experimental and numerical studies for upper and lower cylinders were compared with each other. The deviation of experimental and numerical heat transfer rates are in a good agreement and stay in the range ± 20%. It is seen that heat transfer from the lower cylinder is close to the single cylinder case. However, higher temperature of the passing air reduces the heat transfer from the upper cylinder for S/D = 2.

Abstract:
The Syrian crisis is the most complex crisis (political, social, humanitarian, security) in the world, since World War II. In regards to the 1951 „Geneva Convention on the Status of Refugees”, states have an obligation to provide refugees with a form of primary protection, which is not intended to replace asylum, but rather to provide an intermediate and immediate protection measure. At European level, for asylum seekers and refugees, there are several programs (developed by international institutions such as "UN for refugees" and "UNHCR") for integration into the host country during the temporary stay and return to the country of origin, when there are safety conditions. If this solution is not viable, they can opt for relocation to a third country. Effective management of the crisis created by the refugees flow requires the adoption of a common political agenda for European states to eliminate inequalities between states and focus on building viable economic strategies. Therefore, it is necessary to develop policies or strategies designed to provide refugees with a sense of security in the host country and their social assimilation, without being forced to give up their cultural identities. This article aims to analyse the strategies of the European states involved in this phenomenon, applied in the "Syrian refugee crisis". At the moment, the biggest part of the effort of counter-terrorism is focused on the criminal justice system. This means focusing almost exclusively on those who already intend to commit a crime and not on prevention. The legal repercussions can further encourage radicalization, which can degenerate later.

Abstract:
During the last twenty years, the Russian adverb davno ‘long ago, for a long time’ was widely discussed in literature. It was recognized that the unique parameter of davno is its inability to be the theme of a sentence. Moreover, if davno functions in the context of aspectual forms relating to the past it can only be the rheme. In the context of the aspectual verbal forms relating to the past but preserving the connection with the moment of speech, davno can be either the rheme proper, or a component of the rheme. A classic example of an aspectual verb form referring to the past is the general factual meaning of the imperfective aspect. At present, the spoken data corpora can shed light on the communicative structure analysis, since the prosodic structure of the sound speech provides a straightforward access to the communicative structure. Novel parameters of davno are as follows. 1) Whereas davno is traditionally recognized as a word of rhematic polarity it can nevertheless function as a component of the theme in the context of attributive clauses and constructions (Davno soglasovannyj visit dolzhen byl sostojatjsja v aprele ‘A visit planned long ago would take place in April’). 2) The general factual meaning of the imperfective aspect, contrary to what was assumed before, is not an absolute prerequisite for davno to function as the rheme. The spoken corpus showed that in the context of negation and in the context of the verbs of speech, the general factual allows for davno to function as a component of the rheme but not the rheme proper (Ja davno tebja ne videl ‘I have not been seeing you for a long time’; My davno govorili, chto nasha zadacha — eto borjba s terrorismom ‘We have been insisting for a long time that our main goal is the struggle against terrorism’). 3) A specific type of questions with the initial davno (as well as with other adverbs with the meaning of a considerable quantity like chasto ‘often’, mnogo ‘much’, and daleko ‘far away’) is singled out. Such questions cannot be unambiguously classified either as yes-no-questions or as wh-questions (I davno vy zdesj stoite? ‘And how long are you staying here?’). A description of unique prosody of such questions is given. 4) In the context of discourse continuity, davno acquires the rising prosody which is in fact uncharacteristic of a word, which is unable be the theme (Xotel eto sdelat’ davno, no teperj sdelaju tochno ‘I wished to do it long ago, but now I will do it for sure’). The rising tone is accounted for by the meaning of continuity, which has the same prosody as the theme. 5) In constructions kogda-to davno ‘once upon a time’, ochenj davno ‘very long ago’, davno-davno ‘very long ago’, davnymdavno ‘very long ago’, dovoljno davno ‘quite long ago’, ne tak davno ‘not so long ago’ davno loses its rhematic polarity. The parameters of davno are exemplified by spoken fragments taken from the Multimodal corpus of the Russian National corpus, and the minor working collection of the Russian speech recordings specifically set up for this investigation. The software program Praat was used in the process of analyzing the sound data.

The Annual Report on Migration and Asylum 2016 provides an overview of trends, policy developments and significant debates in the area of asylum and migration during 2016 in Ireland. Some important developments in 2016 included: The International Protection Act 2015 was commenced throughout 2016. The single application procedure under the Act came into operation from 31 December 2016. The International Protection Office (IPO) replaced the Office of the Refugee Applications Commissioner (ORAC) from 31 December 2016. The first instance appeals body, the International Protection Appeals Tribunal (IPAT), replacing the Refugee Appeals Tribunal (RAT), was established on 31 December 2016. An online appointments system for all registrations at the Registration Office in Dublin was introduced. An electronic Employment Permits Online System (EPOS) was introduced. The Irish Short Stay Visa Waiver Programme was extended for a further five years to October 2021. The Second National Action Plan to Prevent and Combat Human Trafficking was published. 2016 was the first full year of implementation of the Irish Refugee Protection Programme (IRPP). A total of 240 persons were relocated to Ireland from Greece under the relocation strand of the programme and 356 persons were resettled to Ireland. Following an Oireachtas motion, the Government agreed to allocate up to 200 places to unaccompanied minors who had been living in the former migrant camp in Calais and who expressed a wish to come to Ireland. This figure is included in the overall total under the IRPP. Ireland and Jordan were appointed as co-facilitators in February 2016 to conduct preparatory negotiations for the UN high level Summit for Refugees and Migrants. The New York Declaration, of September 2016, sets out plans to start negotiations for a global compact for safe, orderly and regular migration and a global compact for refugees to be adopted in 2018. Key figures for 2016: There were approximately 115,000 non-EEA nationals with permission to remain in Ireland in 2016 compared to 114,000 at the end of 2015. Net inward migration for non-EU nationals is estimated to be 15,700. The number of newly arriving immigrants increased year-on-year to 84,600 at April 2017 from 82,300 at end April 2016. Non-EU nationals represented 34.8 per cent of this total at end April 2017. A total of 104,572 visas, both long stay and short stay, were issued in 2016. Approximately 4,127 persons were refused entry to Ireland at the external borders. Of these, 396 were subsequently admitted to pursue a protection application. 428 persons were returned from Ireland as part of forced return measures, with 187 availing of voluntary return, of which 143 were assisted by the International Organization for Migration Assisted Voluntary Return Programme. There were 532 permissions of leave to remain granted under section 3 of the Immigration Act 1999 during 2016. A total of 2,244 applications for refugee status were received in 2016, a drop of 32 per cent from 2015 (3,276). 641 subsidiary protection cases were processed and 431 new applications for subsidiary protection were submitted. 358 applications for family reunification in respect of recognised refugees were received. A total of 95 alleged trafficking victims were identified, compared with 78 in 2015.
