Abstract:
There is wide-spread recognition that people who struggle with mental illness often face negative reactions from others or systems with which they interact, and often internalize these reactions. This volume explores the many faces of the stigma of mental illness through a series of carefully chosen foci. The book begins with a series of chapters that explore models of stigma, and how stigma can present itself in different contexts such as health care, the workplace and postsecondary settings. The measurement of stigma is highlighted as an area of recent development, that is critical for evidence-based programming. A third part of the volume presents examples of innovative interventions that reduce the stigma of mental illness. Overall, the book provides a theoretically current, conceptually rich, and yet practical approach to the issues it covers, and includes a number of practical suggestions and tools for the reader from a series of global leaders.

Abstract:
This chapter presents a critical summary of stigma-change strategies employed in the mental health field as a means to inform the anti-stigma efforts of advocates in the broader health arena. This is done by drawing on the comprehensive research and conceptual work targeting the stigma of mental illness while also weaving in the emerging research on stigma-change related to physical illness. The chapter first provides a brief overview of the targets of stigma change—public stigma, self-stigma, and label avoidance—and specific agendas corresponding with the types of stigma that guide anti-stigma programs—service engagement, rights achievement, and self-worth. It next presents a discussion of the various approaches to stigma change organized by these targets and agendas. The chapter concludes with future directions and goals for advocates and researchers to effectively challenge the stigma of physical illnesses.

Abstract:
People with mental illnesses experience physical illness and premature mortality at much higher rates compared to people without such illnesses. This chapter proposes that the stigma of mental illness comprises an important set of causes of this physical health disparity. It draws on classical and modified labeling theory from sociology for insights and propositions as to why mental illness stigma might affect physical health. The chapter proposes that the stigma of mental illness might affect not only the future experience of mental illness but also a broad range of physical illnesses, thereby contributing to the substantial physical health disparity that people with mental illnesses experience. The chapter develops a conceptual model that places at its center stigma processes including structural, interpersonal, social psychological, and internalized processes. Stigma processes at these levels induce stress and reduce resources, which in turn compromise physical health to produce large physical health disparities.

Chapter 7 discusses the differences between misconceptions that respond to information, and prejudices that are deep-seated and resistant to change. It is important to maintain a clear distinction between programs that are designed to reduce disability by improving mental health literacy and help-seeking, from those designed to decrease stigma and social rejection. There is a danger that increased understanding of the neurobiological basis for mental illnesses engenders stigma by sharpening divisions between what is considered “normal” and “abnormal” and by consolidating stereotypes of immutability, uncontrollability, and dangerousness.
