IntroductionThe peptic ulcer disease (PUD) is a paradigm of the mind body interaction. This model has been a source of much research until the rise of an infectious etiologic model and improvement in pharmacotherapy. To the best of our knowledge, there is no epidemiological data investigating the co-occurrence of PUD with personality disorders based on DSM-IV criteria.ObjectiveTo assess the association between peptic ulcer and a wide range of personality disorders in a large sample representative of the general population in the United States.MethodsData were drawn from the National Epidemiologic Survey on Alcohol and Related Conditions, on the basis of a face-to-face interview of more than 43,000 adults. Univariate and multivariate logistic regression were used to examine the relationship between self-reported “stomach ulcer” and personality disorders.ResultsAll seven personality disorders assessed in the National Epidemiologic Survey on Alcohol and Related Conditions (i.e., avoidant, dependent, obsessive-compulsive, paranoid, schizoid, histrionic, and antisocial personality disorders) were associated with stomach ulcer, with odds ratio ranging from 2.26 (obsessive compulsive personality disorder) to 5.54 (dependent personality disorder). Participants with ulcer were five times more likely to have more than three personality disorders than participants without ulcer. The relationship between ulcer and personality disorders was only slightly attenuated after adjusting for sociodemographic conditions, physical and psychiatric disorders, and addictions.ConclusionsSelf-reported peptic ulcer is associated with increased rates of personality disorders, beyond the influence of psychiatric disorders or addictions.Psychosom Med. 2010 Aug 17. CoordenaÃÃo de AperfeÃoamento de Pessoal de NÃvel Superior
O Helicobacter pylori à uma bactÃria Gram-negativa que coloniza mais da metade da populaÃÃo mundial; a infecÃÃo por esta bactÃria està associada a diversas afecÃÃes gÃstricas, entre elas: gastrites, Ãlceras pÃpticas e cÃncer gÃstrico. O grau de lesÃes e patogenicidade à muito dependente da diversidade gÃnica da bactÃria. O objetivo do presente estudo foi avaliar os fatores de virulÃncia tnpA, tnpB e cagM do H. pylori em pacientes com gastrite, Ãlcera pÃptica e cÃncer gÃstrico. A genotipagem das cepas de H. pylori oriundas de biÃpsias gÃstricas foi realizada pela tÃcnica de reaÃÃo em cadeia de polimerase. Foram analisados 147 pacientes, os quais 50 eram portadores de gastrite, 51 de Ãlceras pÃptica e 46 de cÃncer gÃstrico; os quais 72 eram do gÃnero masculino e 75 do feminino com mÃdia de idade e desvio padrÃo de 54,18  14,24 anos. A populaÃÃo estudada foi dividida em duas faixas etÃrias, abaixo e acima de 45 anos; a qual 72,8% estavam acima de 45 anos de idade. A frequÃncia dos genÃtipos estudados foi: 70 (47,6%) cepas tnpA; 03 (2,0%) tnpB; 19 (12,9%) cagM. O gene tnpA foi o mais presente no gÃnero masculino nas duas faixas etÃrias estudadas; no entanto, sem associaÃÃo estatÃstica do gene com tais variÃveis. O gene tnpA apresentou uma associaÃÃo negativa com o cÃncer gÃstrico; enquanto apresentou associaÃÃo significativa com a Ãlcera duodenal (p = 0,002). O gene tnpB foi o de menor prevalÃncia e nÃo obteve nenhuma associaÃÃo significante. O gene cagM foi o segundo mais prevalente e apresentou associaÃÃo significativa com Ãlcera duodenal (p = 0,024). Concluiu-se que os genes tnpA, cagM estÃo correlacionados com o risco maior de desenvolver Ãlceras pÃpticas; sugerindo que tais genes sÃo bons candidatos a serem marcadores genÃticos do H. pylori para a Ãlcera pÃptica nestes pacientes de Fortaleza.
Helicobacter pylori is a Gram-negative bacterium that colonizes more than half the world's population; Infection by this bacterium is associated with various gastric diseases, including: gastritis, peptic ulcers and gastric cancer. The degree of injury and pathogenicity is very dependent on the genetic diversity of the bacterium. The aim of this study was to evaluate the virulence factors tnpA, tnpB and H. pylori cagM in patients with gastritis, peptic ulcer and gastric cancer. Genotyping of H. pylori strains arising from gastric biopsies was performed by reaction using polymerase chain. 147 patients were analyzed, of which 50 had gastritis, 51 peptic ulcers and 46 gastric cancer; which 72 were male and 75 female, mean age and standard deviation of 54.18 Â 14.24 years. The study population was divided into two age groups below and above 45 years; which 72.8% were above 45 years of age. The frequency of genotypes was: 70 (47.6%) tnpA strains; 03 (2.0%) tnpB; 19 (12.9%) cagM. The tnpA gene was more prevalent in males in both age groups; however, no statistical association of the gene with such variables. The tnpA gene showed a negative association with gastric cancer; while significantly associated with duodenal ulcer (p = 0.002). The tnpB gene had the lowest prevalence and got no significant association. The cagM gene was the most prevalent second and showed a significant association with duodenal ulcer (p = 0.024). It was concluded that the tnpA genes, cagM are correlated with increased risk of developing peptic ulcers; suggesting that these genes are good candidates for genetic markers of H. pylori in peptic ulcer disease in these patients Fortaleza.

INTRODUÇÃO: O câncer vem apresentando um impacto cada vez maior nas populações em todo o mundo. Apesar de recente queda global na sua incidência, o câncer gástrico ainda é o quinto tipo mais comum. Sua patogênese é multifatorial, envolvendo a interação de fatores genéticos, ambientais e infecciosos. OBJETIVO: Avaliar a associação de tabagismo, consumo de álcool e nível de escolaridade com o desenvolvimento de câncer gástrico. MÉTODOS: Trata-se de um estudo caso-controle de base hospitalar em que foram incluídos pacientes com diagnóstico de adenocarcinoma de estômago confirmado por exame histopatológico sem tratamento prévio para a neoplasia. Posteriormente, os casos foram divididos em subtipos de acordo com a histologia (intestinal e difuso) e localização da lesão (proximal, distal e outras). Os indivíduos do grupo controle foram selecionados entre pacientes admitidos no mesmo hospital, sem história ou suspeita de câncer de estômago, emparelhados por frequência aos casos segundo sexo e idade. Tabagismo foi classificado em maços-ano e consumo de álcool em gramas-ano. RESULTADOS: Foram analisados 240 casos e 499 controles recrutados no período de junho de 2001 a dezembro de 2007. Não frequentaram a escola ou apresentavam ensino fundamental incompleto 94 indivíduos (39,2%) no grupo dos casos e 187 (37,5%) no grupo de controles. Ensino universitário foi atingido por 12 indivíduos (5%) no grupo de casos e por 45 indivíduos (9%) do grupo de controles. Não houve associação de nível de escolaridade com risco de desenvolvimento de câncer de estômago. Tabagismo esteve associado ao risco de câncer gástrico com odds ratio (OR) de 2,25 (IC95%: 1,53-3,31) para ex-tabagistas e de 2,67 (IC95%: 1,72-4,13) para tabagistas atuais. Com relação à localização e tipo histológico, tabagismo foi associado com todos os subtipos de tumores gástricos analisados, com destaque para os tumores proximais que apresentaram OR de 5,38 (IC95%: 2,15-13,45) para consumo superior a 38 maços-ano. Consumo de álcool também esteve associado a risco de desenvolvimento de câncer gástrico em todos os subtipos analisados. Entretanto, esta associação apresentou características distintas do tabagismo. Ex-consumidores de álcool apresentaram risco mais elevado (OR=3,81; IC95%: 2,45-5,91) que consumidores atuais (OR=2,06; IC95%: 1,31-3,26). A análise da interação mostrou que o efeito conjunto de tabagismo e consumo de álcool encontrado foi maior que o esperado, evidenciando interação positiva [?=1,51 (IC 95%: 1,05 - 1,96)]. CONCLUSÕES: Tabagismo e consumo de álcool apresentaram associação com o risco de desenvolvimento de câncer gástrico, com destaque para tabagistas atuais e maior consumo de maços-ano. O consumo associado do tabaco e do álcool aumenta esse risco
BACKGROUND: Cancer has an increasing impact on populations around the world. Despite a recent overall decline in incidence, gastric cancer stills the fifth most common type. Its pathogenesis is multifactorial involving the interaction of genetic, environmental and infectious factors. OBJECTIVES: To evaluate the association of smoking, alcohol consumption and education level with the development of gastric cancer. METHODS: This is a hospital-based case-control study that included patients with gastric adenocarcinoma confirmed by histopathological examination without prior treatment. Subsequently, patients were divided into subtypes according to histology (intestinal and diffuse) and location of the lesion (proximal, distal and others). Control subjects were selected among patients admitted to the same hospital with no history of gastric câncer, and were frequency-matched to cases for age and sex. Smoking was classified in pack-years and alcohol consumption in grams per year. RESULTS: We analyzed 240 cases and 499 controls recruited from June 2001 to December 2007. Not attended school or had incomplete elementary school 94 subjects (39.2%) in the group of cases and 187 (37.5%) in the control group. University education was achieved by 12 subjects (5%) in the case group and 45 subjects (9%) in the control group. There was no association of education level with increased risk of stomach cancer. Smoking was associated with increased risk of gastric cancer with an odds ratio (OR) of 2.25 (95%CI: 1.53-3.31) for former smokers and 2.67 (95%CI: 1.72-4.13) for current smokers. With respect to location and histological type, smoking was associated with all subtypes of gastric tumors analyzed with emphasis on the proximal tumors that had OR of 5.38 (95%CI: 2.15-13.45) for consumption over 38 packs-years. Alcohol consumption was also associated with increases risk of gastric cancer development in all analyzed subtypes. However, this association showed distinct characteristics of smoking. Former drinkers had higher risk (OR=3.81; 95%CI: 2.45-5.91) than current users (OR=2.06; 95%CI: 1.31-3.26). The analysis of the interaction showed that the combined effect of smoking and alcohol consumption was higher than expected, thus showing up a positive interaction [?= 1.51 (95%CI: 1.05-1.96)]. CONCLUSIONS: Smoking and alcohol consumption were associated with the risk of gastric cancer development, especially for current smokers and higher consumption of pack-years. Association of tobacco and alcohol consumption increases this risk

Abstract:
This study aimed at exploring the impact of early traumatic experiences on bariatric patients with the intent to give "voice" to their experiences. The impact of morbid obesity and the lack of quality of life among those suffering from this form of chronic illness can be devastating. Meaningful support systems and bariatric surgery are therefore considered as forced behavioural interventions to remediate the impact of childhood trauma and subsequent development of morbid obesity among this group of bariatric patients. Through a process of social constructivism and dialogue between the researcher and the five participants, the co-construction according to themes was supported by a qualitative research approach and the case study method. For the analysis of the themes according to the participants' "voices", the thematic content analysis method was used to analyse the data and was finally linked to supportive literature. It is hoped that the results from this study will contribute to the development of a unique assessment and support programme to those who have to endure the burden of morbid obesity associated with early childhood trauma; and that the process prior to and post bariatric surgery will be an important contribution to finding quality of life and giving new meaning to patients after suffering through their bodies and traumatised minds.

Many gastric motility disorders, including gastroparesis, are caused by dysrhythmias occurring in the stomach musculature. Microwave ablation (MWA) offers potential as a minimally invasive endoscopic approach for targeted thermal destruction of the gastric musculature to disrupt irregular electrical rhythm within the stomach wall. An experimental study was conducted in a gel phantom to analyze the transient heating profile of a water-cooled 2.45 GHz MWA antenna enclosed within a PET balloon. Fiber-optic temperature sensors were used to collect temperature data at distances 1.5–7.5 mm from the balloon surface. Ablation profiles were also characterized in ex vivo porcine skeletal muscle. With 20 W applied power and cooling water temperature of 5 °C, temperature measured at 3.5 mm from the balloon surface exceeded the temperature at 1.5 mm from the balloon surface by 3 °C. In ex vivo tissue, for 40 W applied power, tissue within 2 mm of the balloon surface remained unablated. With adequate cooling and power, it may be feasible to thermally spare tissue within 2 mm of the MWA balloon applicator.

Obstructive sleep apnea is a serious sleep disorder that affects an estimated one billion adults worldwide. It causes breathing to repeatedly stop and start during sleep which over years increases the risk of hypertension, heart disease, stroke, Alzheimer's, and cancer. In this demo, we present Yolo4Apnea a deep learning system extending You Only Look Once (Yolo) system to detect sleep apnea events from abdominal breathing patterns in real-time enabling immediate awareness and action.
