Abstract:
IntroductionThe peptic ulcer disease (PUD) is a paradigm of the mind body interaction. This model has been a source of much research until the rise of an infectious etiologic model and improvement in pharmacotherapy. To the best of our knowledge, there is no epidemiological data investigating the co-occurrence of PUD with personality disorders based on DSM-IV criteria.ObjectiveTo assess the association between peptic ulcer and a wide range of personality disorders in a large sample representative of the general population in the United States.MethodsData were drawn from the National Epidemiologic Survey on Alcohol and Related Conditions, on the basis of a face-to-face interview of more than 43,000 adults. Univariate and multivariate logistic regression were used to examine the relationship between self-reported “stomach ulcer” and personality disorders.ResultsAll seven personality disorders assessed in the National Epidemiologic Survey on Alcohol and Related Conditions (i.e., avoidant, dependent, obsessive-compulsive, paranoid, schizoid, histrionic, and antisocial personality disorders) were associated with stomach ulcer, with odds ratio ranging from 2.26 (obsessive compulsive personality disorder) to 5.54 (dependent personality disorder). Participants with ulcer were five times more likely to have more than three personality disorders than participants without ulcer. The relationship between ulcer and personality disorders was only slightly attenuated after adjusting for sociodemographic conditions, physical and psychiatric disorders, and addictions.ConclusionsSelf-reported peptic ulcer is associated with increased rates of personality disorders, beyond the influence of psychiatric disorders or addictions.Psychosom Med. 2010 Aug 17. Abstract:
Introduction. Watermelon stomach (WS) or gastric antral vascular ectasia (GAVE) is a rare cause of upper gastrointestinal bleeding described in a variety of autoimmune disorders. Association of watermelon stomach with Sj?gren's syndrome is extremely rare. Case report. We presented a 67-year old female with primary Sj?gren's syndrome (SS) who had developed a persistent severe iron-deficiency anemia. An upper gastric endoscopy revealed the presence of gastric antral vascular ectasia (GAVE) as a cause of occult gastrointestinal bleeding. The treatment with argon-plasma coagulation was postponed as the conservative therapy with iron substitution and proton pump inhibitor led to improvement of anemia and hemoglobin levels normalization. Conclusion. This is the first report of WS in a patient with primary SS without the presence of coexisting autoimmune disorder. Recognition of this rare, but clinically important, cause of gastrointestinal bleeding may decrease comorbidity in patients with autoimmune disorders including primary Sj?gren's syndrome.

Abstract:
Blood serum fructosamine level were measured on an empty stomach in 97 normal subjects and subjects with various disorders of carbohydrate metabolism (the so-called prediabetes and changed glucose tolerance) in order to elucidate the significance of this factor as a marker of such disorders. Fructosamine concentrations were for the first time measured in children whose parents suffered from insulin-dependent or noninsulin-dependent diabetes mellitus. The content of fructosamine on an empty stomach was found to depend on the degree of carbohydrate metabolism disorders, increasing with the progress of these disorders. The findings permit a conclusion on the possibility of using fructosamine as a marker for the diagnosis of prediabetes in subjects with its levels of 2.5 to 2.99 mmol/1 and of disordered glucose tolerance in those with its levels over 3 mmol/1 and glucose level on an empty stomach no more than 6 mmol/l.

Abstract:
Morbid obesity is a distinct disorder which leads to metabolic disorders and to the development of many severe chronic diseases, therefore, the effective treatment of the disorder is an urgent problem of modern medicine. Currently, morbid obesity and the corresponding disorders are a growing problem associated with a significant risk of morbidity and mortality. The study was aimed to assess the morphological and functional changes in the stomach wall after gastroplication performed by the new method worked out in the Clinic of the Department of Surgical Diseases and New Technologies of the Bashkir State Medical University. Gastroplication was performed in 15 male rabbits of the Gray Giant breed aged 12–16 months weighting 3050–5380 g. The animals were withdrawn from the experiment 3, 6 and 12 months after surgery, followed by histological examination of the stomach wall plicated section. Changes in the mucous membrane and the muscle layer of the stomach wall after gastroplication were adaptive and associated with no severe morphological impairment. That made it possible to use the method in clinical practice in 15 patients with morbid obesity, 9 women and 6 men aged 42 ± 2.1. In most patients (13 people, 86.7%), a steady decrease in the body weight was achieved during the first 2–12 months. Long-term treatment results had been tracked during two years. It has been confirmed, that the new laparoscopic gastroplication technique does not lead to pathological changes in the stomach wall plicated section, therefore, the simple and cost-effective method may be used in clinical practice for treatment of patients with morbid obesity.

Abstract:
The aim of this study was to describe the role of diagnostic imaging and endoscopy to define the diagnose of the upper gastrointestinal tract disorder of the Schnauzer. The information from the owner stated that the dog has been vomiting for a year, sometimes containing blood. There might also be a possibility that it had eaten a corpus alienum. This case study covered physical examination, hematology and blood chemical analysis, diagnostic imaging radiography and ultrasonography as well as endoscopy performed. Physical examination showed weight loss and anorexia. Hematology and blood chemical analysis showed an increase in hemoglobin, hematocrit, lymphocyte, ureum, creatinine, alanine aminotransferase (ALT), and alkaline phosphatase (ALP) values. Abdominal radiograph showed no abnormality in the abdominal organs. Abdominal ultrasonography showed a hiperechoic elongated mass attached on the stomach mucosal surface which forms an acoustic shadowing at the ventral. Endoscopy showed pathological lesions that is inconsistency of stomach mucosa surface, foamy fluid in the stomach, ulcers and erosion of the stomach mucosal surface. Based on the diagnostic imaging and endoscopy performed, the animal was clearly diagnosed with chronic gastritis accompanied by stomach ulcer and erosion.

Abstract:
BackgroundChildren with somatic complaints are at increased risk for emotional disorders during childhood. Whether this elevated risk extends into young adulthood – and to which specific disorders – has rarely been tested with long-term prospective-longitudinal community samples. Here we test whether frequent and recurring stomach aches, headaches, and muscle aches during childhood predict emotional disorders in adulthood after accounting for childhood psychiatric and physical health status and psychosocial adversity.MethodThe Great Smoky Mountains Study is a community representative sample with 1420 participants. Children/adolescents were assessed 4–7 times between ages 9–16 years. They were assessed again up to three times between ages 19–26 years. Childhood somatic complaints were coded when subjects or their parents reported frequent and recurrent headaches, stomach aches, or muscular/joint aches at some point when children were aged 9–16 years. Psychiatric disorders were assessed with the Child and Adolescent Psychiatric Assessment and the Young Adult Psychiatric Assessment.ResultsFrequent and recurrent somatic complaints in childhood predicted adulthood emotional disorders. After controlling for potential confounders, predictions from childhood somatic complaints were specific to later depression and generalized anxiety disorder. Long-term predictions did not differ by sex. Somatic complaints that persisted across developmental periods were associated with the highest risk for young adult emotional distress disorders.ConclusionsChildren from the community with frequent and recurrent physical distress are at substantially increased risk for emotional distress disorders during young adulthood. Preventions and interventions for somatic complaints could help alleviate this risk.

Abstract:
Objective: to study the relationship of the main mediators of atopy and clinical and morphological characteristics lesions of the stomach with its secretory-motor disorders in children with atopic dermatitis. We study 134 patients aged 7 to 14 years with atopic dermatitis and 30 healthy children as control group. All children passed general clinical and laboratory instrumental investigations. In addition, children with manifestations of atopic dermatitis and secretory-motor disorders of the stomach Fibroesophagogastroduodenoscopy, topographic intragastric pH-metry with aspiration biopsy of the gastric mucosa were performed, followed by morphological investigations. In sick children, an increase in the levels of total IgE, histamine and serotonin was detected in the blood, combined with a tendency to decrease acid-forming function, endoscopic and morphological changes in the gastric mucosa. It is assumed that concomitant atopic dermatitis secretory-motor disorders of the stomach are manifestations of gastrointestinal syndrome, which can be both secondary and primary in relation to skin manifestations and in both situations contribute the formation of endoallergens and maintain a vicious cycle of the pathological process.

Abstract:
Mood disorders are more prevalent in individuals with chronic physical illness compared to individuals with no such illness. These disorders amplify the disability associated with the physical condition and adversely affect its course, thus contributing to occupational impairment, disruption in interpersonal and family relationships, poor health and suicide. This study used data collected in the Canadian Community Health Survey, cycle 3.1 (2005) to examine factors associated with comorbid mood disorders and to assess their association with the quality of life of individuals living in Ontario. Results indicate that individuals with chronic fatigue syndrome, fibromyalgia, bowel disorder or stomach or intestinal ulcers had the highest rates of mood disorders. The odds of having a comorbid mood disorder were higher among women, the single, those living in poverty, the Canadian born and those between 30 and 69 years of age. The presence of comorbid mood disorders was significantly associated with short-term disability, requiring help with instrumental daily activities and suicidal ideation. Health care providers are urged to proactively screen chronically ill patients for mood disorders, particularly among the subgroups found to have elevated risk for these disorders.

Abstract:
Purpose: To study late mental and behavioral disorders (ICD-10) in individuals who were accidentally exposed in the Southern Urals. Material and methods: The research subjects are the persons who were exposed to radiation as a result of dumping of radioactive waste into the Techa River (1949–1956), who were born before 1953 and lived in the Techa River basin between 1950 and 1960. The study group consists of 425 people, women predominate (72.5 %), people with secondary special education make up 44.5 %, and pensioners amount to 89.4 %. Mean age at examination was 67.3 ± 5.3 years. Information on radiation doses included data on the absorbed dose to the stomach, which was considered as an analog of brain exposure (50 ± 3 mGy). The paper uses clinical-anamnestic, clinical-psychopathological, clinical and instrumental methods. Results: It has been demonstrated that in the long-term period, accidentally exposed individuals have organic nonpsychotic mental disorders (72.2 %), with the predominance of organic asthenic (29.6 %) and organic cognitive (36.5 %) disorders. Among the studied individuals, only 20.9 % showed no signs of mental illness. Higher absorbed doses to the stomach (70 ± 8 mGy, p = 0.01) was determined in patients with organic cognitive disorder. Concomitant somatic pathology is present in patients with organic cognitive disorder — hypertension (90.3 %, p = 0.005), ischemic heart disease (56.7, p = 0.004), chronic brain ischemia (43.2 %, p = 0.04). Patients with organic asthenic disorder may have hypertension (94.4 %, p = 0.0002), ischemic heart disease (51.6 %, p = 0.04), chronic brain ischemia (35.7 %, p = 0.02). In the group of patients without mental pathology, the most common were hypertension (76.4 %), diseases of the musculoskeletal system (61.7 %, p = 0.03), and ischemic heart disease (37.1 %). Multiple correlation analysis showed a weak statistically significant association of organic mental disorders with chronic brain ischemia, hypertension, ischemic heart disease and the age of patients. Conclusion: The performed study complements the current understanding of long-term mental disorders in exposed individuals, and requires further research to study the pathogenesis of cerebrovascular and cardiovascular diseases in individuals exposed to radiation.

Abstract:
Annotation currently, in domestic cats (felis silvestris catus), degenerative processes of the intestine and stomach are widespread. The leading place among pathologies is occupied by conditions caused by the regular consumption of lactose. Veterinary gastroenterology as an independent area is a relatively young science, which is being improved from year to year. The increase in cases of visits to veterinary clinics of owners of cats with pathological conditions of the intestines and stomach causes increased professional interest among veterinarians of general practice, as well as the felinological community. The article presents the results of studies proving the important role of food antigens in the early formation of degenerative processes of the intestine and stomach, leading to a significant deterioration in the standard of living of the animal with a subsequent fatal outcome.

Abstract:
Gastroparesis is a condition that affects the normal motility of muscle in the stomach and is commonly called as paralysis of stomach. Electrogastrography is the technique for measuring electrical activity of the stomach. The Electrogastrography is also used to measure the gastric mobility and various stomach disorders like tachy-gastria, brady-gastria, dyspepsia, peptic ulcer, gastric tumor etc. Electrogastrogram is the graphical representation of the Electrogastrography. The Electrogastrography procedure is recommended by the physician to diagnose the dis orders in the stomach. Electrogastrography procedure is taken generally under two conditions: Fasting condition and post prandial condition. This procedure is followed strictly to measure the gastric activity and gastric emptying test. The food is propelled into the stomach and the pacemaker of the stomach fails to contract the muscles of stomach that leads to gastroparesis condition. The diabetic patients often get into the gastroparesis condition. This remains as a major factor; stomach motility is slowed down. In this paper, data acquisition system for acquiring multichannel electrogastrogram is developed and the gastric signal is acquired and analysed using MATLAB. Through the spectral analysis of gastric slow-wave, the gastroparesis condition is diagnosed. The MEGG-Multichannel Channel Electrogastrogram signal is acquired in two states: Fasting state and Postprandial state.

Abstract:
Diphyllobothriosis is a chronic intestinal helminthiasis in which all parts of the gastrointestinal tract are involved in the pathological process. Experimental diphyllobotriose observed violation of digestion of food substances in the small intestine, in clinical studies - disorders of absorption. The works of several researchers found that acid formation and motility of the stomach, the absorptive function of the small intestine are not restored immediately after deworming. This requires a period longer than the two-month observation accepted in polyclinics. An important task is to determine the optimal timing of the follow-up.

Abstract:
This study explored physical and psychiatric comorbidities of mood disorders using association rule mining. There were 7709 subjects who were patients (≥19 years old) diagnosed with mood disorders and included in the data collected by the Korean National Hospital Discharge In-depth Injury Survey (KNHDS) between 2006 and 2018. Physical comorbidities (46.17%) were higher than that of psychiatric comorbidities (27.28%). The frequent comorbidities of mood disorders (F30–F39) were hypertensive diseases (I10–I15), neurotic, stress-related and somatoform disorders (F40–F48), diabetes mellitus (E10–E14), and diseases of esophagus, stomach, and duodenum (K20–K31). The bidirectional association path of mood disorders (F30–F39) with hypertensive diseases (I10–I15) and diabetes mellitus (E10–E14) were the strongest. Depressive episodes (F32) and recurrent depressive disorders (F33) revealed strong bidirectional association paths with other degenerative diseases of the nervous system (G30-G32) and organic, including symptomatic and mental disorders (F00–F09). Bipolar affective disorders (F31) revealed strong bidirectional association paths with diabetes mellitus (E10–E14) and hypertensive diseases (I10–I15). It was found that different physical and psychiatric disorders are comorbid according to the sub-classification of mood disorders. Understanding the comorbidity patterns of major comorbidities for each mood disorder can assist mental health providers in treating and managing patients with mood disorders.

Abstract:
Proximal gastric resection (PGR) is still one of the most difficult and dangerous surgical interventions, and it is the only method of curative treatment of diseases of the cardiac part of the stomach in many cases. 5 types of gastrointestinal reconstruction are mainly performed after PRG: submerged rectal esophagogastroanastomosis, antireflux interposition of the jejunal segment, interposition of the jejunal segment of the jejunal pocket, interposition of the jejunal segment as a double tract and reconstruction with the formation of a gastric tube. Modern literature includes many works devoted to the development of methods of physiological reconstruction after subtotal and total removal of the stomach; however, none of the existing methods is recognized as optimal. Among the main indicators characterizing the effectiveness of PGR are the frequency and severity of the development of a large group of functional disorders, characterized as «diseases of the operated stomach», or «post-gastro-resection disorders», which significantly worsen the quality of life (QOL) and reduce the working capacity, which causes disability of the operated patients Assessment of QoL after surgery for diseases of the cardiac stomach is a fundamentally important component of monitoring the effectiveness of treatment and, therefore, allows one to determine the indications for certain types of interventions. The available world scientific literature does not provide convincing data on the assessment of QOL in patients after proximal resection for cardiac part of the stomach diseases, since there are no clear criteria for its assessment depending on the chosen methods and methods of reconstruction of gastrointestinal tract (GIT). In this regard, there is a need for further study of proximal resection with reconstruction of the gastrointestinal tract in surgical diseases of the cardiac part of the stomach on the basis of analysis of immediate and long-term results, as well as the quality of life of patients who underwent these surgical interventions.

Abstract:
With the use of the radionuclide gastric-emptying test and a new data processing method, the contraction characteristics of the stomach were analyzed. After ingestion of a radiolabeled test meal, dynamic images of the stomach were acquired and analyzed to determine the frequency, amplitude, and rate of gastric contractions in healthy subjects. The frequency of antral contractions was found to be inversely related with food retention in the stomach; in contrast, the amplitude of the contractions decreased progressively during the course of gastric emptying. The peaks of both antral contraction and filling rate and the time of their occurrence remained constant throughout gastric emptying. The observed patterns of phase distribution and sequential phase changes of the food in the stomach confirmed noninvasively what was already known from invasive technique, i.e., that the proximal stomach does not undergo phasic contractions and that, in the distal stomach, smooth muscle contraction originates in midcorpus and propagates aborally to the pylorus. The scintigraphic test can be used to noninvasively and quantitatively characterize gastric motor function and to delineate the spatial sequence of gastric contractions. This technique can be applied to study the pathophysiology of gastric emptying in various motor disorders.

Abstract:
The different ovine production and breeding systems share the cornerstone of keeping a good body condition to ensure adequate productivity. Several infectious and parasitic disorders have detrimental effects on weight gains and may lead to emaciation. Flock health management procedures are aimed to prevent such conditions. Nutritional management is equally important to guarantee adequate body condition. Persistent bouts of low ruminal pH due to excess concentrate in the diet may lead to subacute ruminal acidosis. Pre-stomach motility disorders may also lead to ill-thrift and emaciation. An adequate mineral supplementation is key to prevent the effects of copper, selenium, and other micronutrients deprivation, which may include, among others, loss of condition. This review elaborates on the clinico-pathologic, diagnostic, and therapeutic aspects of some of these conditions, and highlights the necessity of considering them as contributors to states of wasting in sheep flocks.

Abstract:
The objectives of this study were to identify the prevalence of comorbidities of mental and behavioral disorders and to identify the association rules related to comorbidities as a way to improve patient management efficiently. We extracted comorbidities of 20,690 patients (≥19 years old) whose principal diagnosis was a mental disorder from the Korean National Hospital Discharge In-depth Injury Survey (KNHDS) between 2006 and 2016. Association rules analysis between comorbid diseases using the Apriori algorithm was used. The prevalence of comorbidities in all patients was 61.98%. The frequent comorbidities of mental and behavioral disorders were analyzed in the order of hypertensive diseases (11.06%), mood disorders (8.34%), diabetes mellitus (7.98%), and diseases of esophagus, stomach, and duodenum (7.04%). Nine major association pathways were analyzed. Significant pathways were analyzed as diabetes mellitus and hypertensive diseases (IS scale = 0.386), hypertensive diseases, and cerebrovascular diseases (IS scale = 0.240). The association pathway of diabetes mellitus and hypertensive diseases was common in subgroups of mental and behavioral disorders, excluding mood disorders and disorders of adult personality and behavior. By monitoring related diseases based on major patterns, it can predict comorbid diseases in advance, improve the efficiency of managing patients with mental and behavioral disorders, and furthermore, it can be used to establish related health policies.

Abstract:
Background: Sleep disorders have emerged as potential cancer risk factors. Objective: This review discusses the relationships between sleep, obesity, and breathing disorders with concomitant risks of developing cancer. Results: Sleep disorders result in abnormal expression of clock genes, decreased immunity, and melatonin release disruption. Therefore, these disorders may contribute to cancer development. Moreover, in sleep breathing disorder, which is frequently experienced by obese persons, the sufferer experiences intermittent hypoxia that may stimulate cancer cell proliferation. Discussion: During short- or long- duration sleep, sleep-wake rhythm disruption may occur. Insomnia and obstructive sleep apnea increase cancer risks. In short sleepers, an increased risk of stomach cancer, esophageal squamous cell cancer, and breast cancer was observed. Among long sleepers (>9 hours), the risk of some hematologic malignancies is elevated. Conclusion: Several factors including insomnia, circadian disruption, obesity, and intermittent hypoxia in obstructive sleep apnea are contributing risk factors for increased risk of several types of cancers. However, further studies are needed to determine the more significant of these risk factors and their interactions.

Abstract:
Chronic diseases are diseases with insidious onset, prolonged duration, and intractability and are associated with complex etiology and multiple morbidities, including hypertension, obesity, liver disease, kidney disease, cancer, cardiovascular disease, and chronic lung disease. For patients with chronic diseases, although the cause of the disease is not in the spleen and stomach, medication or further development of the disease will affect the function of the spleen and stomach disorders, leading to worsening of the disease. In this article, ultrasound is used to detect health disorders of the spleen and stomach. Patients should eat a light diet and exercise more. Elderly patients can increase abdominal massage and other health care. It should be emphasized that the prevalence of chronic diseases is not lower in people with high education or high economic income but is closely related to people’s daily living habits; secondly, we should enhance physical exercise and advocate annual physical examination. Finally, we need to develop different measures according to different regions to prevent and treat chronic diseases and give timely feedback to the interventions. New preventive and curative measures are given in time to improve public health.

Abstract:
Aim. The study of disorders of the motor-evacuation function of the stomach and duodenum (duodenum) in patients with duodenal ulcer and their use in choosing effective therapy.Materials and methods. The study included 107 patients with ulcerous duodenal ulcer with the presence of motor-evacuation disorders of the gastroduodenal zone — group I and 99 patients with ulcerous duodenal ulcer without a violation of motor-evacuation function of the stomach and duodenum — group II. Anamnestic and physical data, results of X-ray and endoscopic examinations were used in verification of the ulcer. Assessment of the motor function of the stomach and duodenum was carried out using the gastroenteromonitor GEM-01 “Gastroskan-GEM”. Depending on the treatment method, patients with ulcerous duodenal ulcer of group I were divided into 2 groups: observation group — 52 patients with ulcerous duodenal ulcer of the duodenal ulcer received 50 mg 3 times a day prokinetic itopride hydrochloride as a part of complex anti-ulcer therapy and a comparison group — 55 patients with ulcerous duodenal ulcer underwent standard antiulcer therapy.Results. In the observation group after treatment, pain disappeared in 90.0% of patients, decreased in 10.0%. In the comparison group, pain completely disappeared in 18.8% of patients, decreased in 76.6%, remained unchanged in 4.7%. Against the background of treatment in patients of the observation group, the frequency and amplitude of the stomach and duodenum corresponded to the normokinetic type of curve. When using itopride hydrochloride in the treatment of patients of the observation group, restoration of the closure function of the pylorus was revealed. After the treatment, the ulcer healed in the observation group in 97.8% of patients (in 88.2% of cases at the stage of a high-quality “white” scar); in the comparison group, ulcer healing occurred in 94.5% of cases, of which in 67.1% — in the stage of poor-quality “red” scar).Conclusion. The use of the prokinetics of itopride hydrochloride as part of complex therapy increases the effectiveness of the treatment of ulcerative duodenal ulcer by restoring the motor-evacuation function of the stomach and duodenum.

Abstract:
Peristalsis, the coordinated contraction—relaxation of the muscles of the stomach is important for normal gastric motility and is impaired in motility disorders. Coordinated electrical depolarizations that originate and propagate within a network of interconnected layers of interstitial cells of Cajal (ICC) and smooth muscle (SM) cells of the stomach wall as a slow-wave, underly peristalsis. Normally, the gastric slow-wave oscillates with a single period and uniform rostrocaudal lag, exhibiting network entrainment. Understanding of the integrative role of neurotransmission and intercellular coupling in the propagation of an entrained gastric slow-wave, important for understanding motility disorders, however, remains incomplete. Using a computational framework constituted of a novel gastric motility network (GMN) model we address the hypothesis that engaging biological oscillators (i.e., ICCs) by constitutive gap junction coupling mechanisms and enteric neural innervation activated signals can confer a robust entrained gastric slow-wave. We demonstrate that while a decreasing enteric neural innervation gradient that modulates the intracellular IP3 concentration in the ICCs can guide the aboral slow-wave propagation essential for peristalsis, engaging ICCs by recruiting the exchange of second messengers (inositol trisphosphate (IP3) and Ca2+) ensures a robust entrained longitudinal slow-wave, even in the presence of biological variability in electrical coupling strengths. Our GMN with the distinct intercellular coupling in conjunction with the intracellular feedback pathways and a rostrocaudal enteric neural innervation gradient allows gastric slow waves to oscillate with a moderate range of frequencies and to propagate with a broad range of velocities, thus preventing decoupling observed in motility disorders. Overall, the findings provide a mechanistic explanation for the emergence of decoupled slow waves associated with motility impairments of the stomach, offer directions for future experiments and theoretical work, and can potentially aid in the design of new interventional pharmacological and neuromodulation device treatments for addressing gastric motility disorders.
