Abstract:
Acute gastroenteritis with persistent vomiting, high degree fever and blood streaking stools often suggests bacterial aetiology in children. Authors report a 13-year-old boy presenting with acute watery diarrhoea with persistent vomiting, fever of 103°F, abdominal cramps and blood streaking stools who failed to show any response to parenteral third-generation cephalosporin for 72 hours. The stool examination revealed numerous cystic and amoeboid forms of Blastocystis hominis. Metronidazole was started and the boy promptly responded within 24 hours. There was no recurrence of symptoms then onwards. The case highlights the crucial stool examination in case of acute diarrhoeal disease for rare aetiology.

Abstract:
Microscopic examination of a wet mount of the stool has been the standard practice for the laboratory diagnosis of intestinal parasitic infections. Here we describe a novel method of stool microscopy of 80 stool samples, 31 (38.75%) were positive by the new thick stool smear wet mount method, whereas the corresponding figure for the conventional method using lacto-phenol cotton blue was 16 (20%). The difference was found to be statistically significant ( P<0.04 by McNemar's test). The thick stool smear wet mount procedure promises to be superior to the direct wet mount method in the detection of the intestinal parasites.

A retrospective analysis of the results of 2,704 ova and parasite (O & P) examinations performed on stool specimens collected from 1,374 patients between October 1996 and September 1997 was performed to evaluate the utility of performing O & P examinations on multiple, independently collected stool specimens in a high-prevalence setting. A total of 995 specimens (36.8%) examined during the study contained parasites; 546 (20.2%) contained pathogenic organisms. The positivity rate (54.5%) for the patients from whom three specimens were examined was significantly higher than for the patients from whom either two specimens (33.3%) or a single specimen (19.8%) was submitted for examination. For the group of patients from whom at least 3 specimens were submitted for O & P examination, 373 independent opportunities for diagnosing infection with intestinal parasites could be analyzed. The first stool specimen collected proved to be adequate in only 75.9% (283 of 373) of evaluated cases; however, examination of two specimens increased the sensitivity of O & P detection to 92% (343 of 373). The third specimen collected provided additional information on only 30 of 373 occasions (8%). These data indicate that in populations with a high prevalence of intestinal parasitic infections, two independently collected stool specimens should be subjected to O & P examination to ensure adequate diagnostic sensitivity.

Abstract:
We undertook a study to evaluate Streck tissue fixative (STF) as a substitute for formalin and polyvinyl alcohol (PVA) in fecal preservation. A comparison of formalin, PVA, (mercuric chloride based), and STF was done by aliquoting fecal samples into each fixative. Stool specimens were collected in Haiti, and parasites includedCyclospora cayetanensis, Giardia intestinalis,Entamoeba coli, Iodamoeba butschlii,Endolimax nana, Ascaris lumbricoides,Trichuris trichiura, Strongyloides stercoralis, and Necator americanus. Preserved stools were examined at various predetermined times (1 week, 1 month, and 3 months) to establish the quality of the initial preservation as well as the suitability of the fixative for long-term storage. At each time point, stool samples in fixatives were examined microscopically as follows: (i) in wet mounts (with bright-field and epifluorescence microscopy), (ii) in modified acid-fast-, trichrome-, and safranin-stained smears, and (iii) with two commercial test kits. At the time points examined, morphologic features remained comparable for samples fixed with 10% formalin and STF. For comparisons of STF- and 10% formalin-fixed samples, specific findings showed that Cyclospora oocysts retained full fluorescence, modified acid-fast- and safranin-stained smears of Cryptosporidium and Cyclosporaoocysts were equal in staining quality, and results were comparable in the immunofluorescence assay and enzyme immunoassay commercial kits. Stool fixed in STF and stained with trichrome showed less-than-acceptable staining quality compared with stool fixed in PVA. STF provides an excellent substitute for formalin as a fixative in routine examination of stool samples for parasites. However, modifications to the trichrome staining procedures will be necessary to improve the staining quality for protozoal cysts fixed in STF to a level comparable to that with PVA.

O método de diagnóstico coproparasitológico de concentração em formolacetato de etila foi empregado para a quantificação de ovos de helmintos. O método quantitativo proposto foi padronizado utilizando-se o Sistema Comercial Coprotest® e amostras fecais contendo diferentes cargas de ovos de Ascaris lumbricoides. O \"Coprotest quantitativo\" foi aplicado em estudo populacional numa área de baixa endemicidade para parasitas diagnosticáveis através das fezes, e os resultados analisados comparativamente ao método de Kato-Katz. A metodologia proposta foi ainda comparada a outros métodos quantitativos, utilizando-se amostras fecais, preparadas em laboratório, com cargas decrescentes de ovos de A. lumbricoides, Trichuris trichiura e Schistosoma mansoni. Discutem-se as vantagens de se empregar um método capaz de detectar o maior número de espécies, tanto de helmintos quanto de protozoários, . e que permita, concomitantemente, estimar a intensidade das infecções por geohelmintos e S. mansoni nas populações. O \"Coprotest quantitativo\" mostrou ser de aplicação viável em trabalhos de campo, fornecendo resultados comparáveis aos outros métodos quantitativos já descritos na literatura.

Helminthic zoonosis constitutes part of the public and collective health problems due to the ever-closer approximation between humans and animals. There is a shortage of information about the contamination, prevention and care forms to be taken to avoid these conditions which makes difficult to reduce it on the population. Therefore, it is essential to research about the life cycle of parasites and its human-animal relation. Among the transmission forms, soil stands out, since parasitized animals defecate on the soil and disseminate eggs, what makes possible the infection of others. Including the places that the animals usually visit, there are parks and groves of the cities. Therefore, the aim of this research was to analyze on the collected dogs and cat’s stool samples on these places in Goiânia, Goiás, the existence of helminths that can cause diseases in humans, as well as to evaluate the perception of the people who visit these places about the zoonosis and the hygiene care of their pets. It was also aimed to present a literature review about the helminths found during the research, that are transmitted by cats and dog’s feces in urban areas. Nine parks/groves were visited, one per city region, to the collection of the feces and parasitological tool analysis and a quiz with 30 alternative and discursive questions was applied to 164 interviewees which affirmed to have a shortage knowledge about zoonosis. Out of 116 analyzed samples, the coproparasitological test presented the presence of Ancylostoma spp. in 11.21% and Toxocara spp. in 2.5% of them. Regarding the questionnaire, most of people were dog owners (68.9%) and 82.84% of them reportedly gave anthelmintics to their animals. When asked about zoonosis, only 4.88% of people knew its meaning. Despite most people do not have any knowledge about zoonosis, the results may be associated, due to the better care of people with their pets, but this still was not enough to avoid the transmission of these helminths. Actions and measures should be taken, such as creating a practice of monitoring a potential zoonotic parasite in public places by public health agencies, for the purpose of aiming at the prevention against the parasitosis and few spending with treatment.
As zoonoses helmínticas constituem parte dos problemas em saúde pública e coletiva devido a aproximação cada vez mais estreita entre humanos e animais. Há escassez de informações acerca das formas de contaminação, prevenção e cuidados a serem tomados para evitar essas afecções, o que dificulta sua redução na população. Dessa forma, é fundamental que sejam realizados estudos sobre os ciclos de vida de parasitos e sua relação homem-animal. Dentre as formas de transmissão desses, o solo se destaca, uma vez que animais parasitados defecam no solo e disseminam ovos, o que possibilita a infecção alheia. Entre os locais em que estes costumam frequentar em comum se encontram os parques e os bosques dos municípios. Logo, o objetivo deste trabalho foi analisar nas amostras fecais de cães e gatos colhidas nesses locais de Goiânia, Goiás, a existência de helmintos que possam causar doenças em humanos, bem como, avaliar a percepção das pessoas que visitam estes locais acerca das zoonoses e cuidados de higiene com seus animais de estimação. Também visou-se apresentar uma revisão de literatura a respeito das helmintoses encontradas durante a pesquisa, que são veiculadas por meio das fezes de cães e gatos em ambiente urbano. Foram visitados nove parques/bosques municipais, um por região da cidade, para colheita das fezes e análises coproparasitológicas e aplicação de um questionário a 164 entrevistados, que confirmaram o conhecimento escasso sobre zoonose. Nos exames coproparasitológicos detectaram-se 11,21% de ovos de Ancylostoma spp. e 2,5% de Toxocara spp. em 116 amostras fecais. Quanto ao inquérito, a maioria das pessoas possuíam cão em casa (68,9%) e 82,84% destas afirmaram administrar anti-helmínticos. Sobre as zoonoses, apenas 4,88% das pessoas sabiam seu significado. Os resultados podem estar associados entre si, devido ao melhor cuidado das pessoas com seus animais de estimação, porém ainda não é o suficiente para evitar a transmissão dessas helmintoses. Ações e medidas profiláticas devem ser tomadas, como criar uma prática habitual de monitoramento de parasitos com potencial zoonótico em locais públicos por meio de órgãos públicos da saúde, com a finalidade de visar a prevenção contra essas parasitoses e menos gastos com tratamentos.

Abstract:
Literary historian Terryl L. Givens referenced the visions of Moroni as "exhibit A" of Mormonism for nineteenth century believers. The 1823 visions constituted one of the core tenets of the religion as an underlying premise of The Book of Mormon. The significance of the visions, however, has not translated into many studies on the 1823 visions. This thesis seeks to fill portions of this gap by evaluating the visions within post-Revolutionary evangelical and treasure seeking culture. I contend that the visions drew upon various elements of the culture, but ultimately diverged from the culture. The introduction recounts the vision from the perspectives of Joseph Smith, Jr., Oliver Cowdery, and Lucy Mack Smith. Chapter one provides a historiographical review of the literature and some methodological considerations. Chapter two describes the evangelical and treasure seeking cultures. The examination emphasizes the cultural belief in visions and dreams that contained angels, guides, guardians, or other preternatural beings. Chapter three examines the significance of the dates of the 1823 visions, September 21-22. Three traditions associated significance with the date, witchcraft, astrology, and Christianity. I show that either the date did not match with the holiday of these traditions or that Smith probably did not know of its significance. Many people called the vision a dream, which led Oliver Cowdery to refute that claim. Chapter four analyzes whether the visions constituted dreams or visions, before proceeding to evaluate the imagery of dreams and visions. Smith's visions lacked much of the imagery of other contemporary visionaries. Chapter five evaluates Moroni's message to Smith. I contend that Smith considered the plates a treasure and they fit the cultural pattern of treasure. Moroni, though, directed Smith's attention from the money seeking elements toward religious purposes. Many elements within the vision follow the cultural beliefs concerning visions and dreams, which make the visions appear as a cultural product. Careful evaluation of the details of the visions, shows however, the 1823 visions diverged from many cultural tenets.

According to the demographic ageing of labor force participation in Taiwan, we imported massive foreign laborers to Taiwan for nursing or orther labor force work since October, 1989. However, there was no research to assess the relationships between living habits,basic Information,physical condition of foreign labors with intestinal parasite detection in a single stool examination. Besides, the Enterobius vermicularis, a ubiquitous neglected parasites disease was still unclear in foreign laborers. This was a cross-sectional study. The purposes were of two folds: (1) to understand the relationships between living habits, basic Information, physical condition of foreign labors with the intestinal parasite detection in a single stool examination; (2) to evaluate the effectiveness of single examination to diagnose Enterobiasis by scotch tape perianal swab technique in non-constant time. The survey covered southeast asian laborers from four countries. The data were collected from 868 valid questionnaires and valid stool samples. Descriptive statistics, Chi-square analyses and binary logistic regression analyses were applied to the data.Factors such as sex , diarrhea, eating raw-cooked beef, eating raw-cooked mutton, no washing hands before a meal achieved statistically significant with the intestinal parasite detection in a single stool examination in the Chi-square analyses. Multivariate logistic regression analyses were applied to model the intestinal parasite detection in a single stool examination with sex, eating raw-cooked beef, eating raw-cooked mutton, no washing hands before a meal. However, only the sex and no washing hands before a meal achieved statistically significant. 391 valid perianal scratch swab samples were also collected. The results indicated scotch tape perianal swab technique in non-constant time could increase the detection rate of Enterobius vermicularis.The competent authority should consider scotch tape perianal swab technique as an extra test in foreign laborers' health examinations to increase the sensitivity of intestinal parasites detection by the current method. Physician should pay more attention to the foreign laborers suffering from long-term diarrhea. Occupational health personnel should educate the foreign laborers to get into the habit of washing hands before a meal in their daily practice. The employers should arrange the examination of scotch-tape anal swab technique for caretakers periodically.

Abstract:
For most of the nineteenth century the Brazilian economy grew roughly in line with population expansion. As a result, increases in per capita incomes were modest at best and, even then, were visible mostly in the second half of the century. As the empire came to a close and the republic was ushered in, several obstacles that had previously stood in the way of faster growth were gradually overcome and modern economic growth set in. This chapter discusses the main factors accounting for this story of no/low economic growth, followed by faster per capita income increases from the turn of the nineteenth century onward. Inevitably, in such a complex story as that of economic growth over such a long period, social, political, and demographic factors will be combined with those more strictly “economic” to provide a more comprehensive account of the historical developments under examination.

Abstract:
Four to five per cent of the Western population suffers from chronic diarrhoea (defined as the passage of >3 stools per day, for >4 weeks), with irritable bowel syndrome (IBS) being the commonest cause in 20–40-year-old patients. It is the commonest reason for referral to secondary care gastroenterology clinics. The list of possible causes of chronic diarrhoea is long but, in the absence of rectal bleeding, loss of weight, or abnormal blood tests, it is unlikely to be due to a serious illness. Laboratory investigations should include serum glucose, electrolytes, renal and liver tests, full blood count, thyroid tests, a coeliac antibody test, C-reactive protein (CRP) measurement to check for systemic inflammation, faecal fat and elastase estimation to check pancreatic exocrine function, faecal microscopy, and culture, although this is insensitive for giardiasis. In young patients with typical features of IBS, these laboratory investigations can be abbreviated to include only glucose, electrolytes, the coeliac antibody test, CRP measurement, and thyroid tests. Endoscopic examination of the large and small intestines is generally only required where there is a suspicion of coeliac disease, chronic giardiasis, microscopic colitis, inflammatory bowel disease, or colorectal cancer. A therapeutic trial of metronidazole for giardiasis is justified where this seems a likely diagnosis.
