World over life was going at its normal pace when an outbreak occurred in Hubei province of China in the later part of the year 2019. This outbreak was soon found to be caused by a virus named coronavirus (COVID-19). Rapidly the virus spread globally leading to a pandemic. The mortality rate was increasing day by day and helplessly everyone was wondering what actually could be done to prevent the spread. Lessons from the past epidemic made it possible to think that maintaining social distancing and adequate hygiene might help to combat the ailment. In India, majorly affected were the people from poor strata and the businessmen who were earning their daily bread by selling things of daily need. The health sector too witnessed an alarming ratio of patients suffering from COVID-19. The second wave, which soon followd the first wave, caused much more havoc. Overall, the COVID-19 pandemic, exposed and challanged the health security system of every country. As the danger of pandemic still prevails, steps to curtail the spread of disease and future management strategies should be formulated from the lessons learnt through the COVID-19 phase.

Chronic pain is a debilitating condition that affects millions of people world-wide, leading to physical incapacitation and financial strain. Common methods for treatment include physical therapy, oral medications, injections, surgery, and neuromodulation. Injectates with steroids and local anesthetics can be a temporizing measure with intolerable side effects. The use of autologous biologic injectates (e.g., platelet rich plasma, bone marrow aspirate concentrate, tissue grafts, and stem cells) is growing in therapeutic potential and enthusiasm, giving hope to a subset of patients that have either failed conventional therapy or are not candidates for traditional steroid injections. In this chapter, we will describe different cases in which regenerative medicine can help in painful conditions as well as neuro-degenerative conditions. Regenerative medicine can be the new frontier in providing long lasting relief through changes in cell-signaling cascades, however further trials are needed to validate their use.

Although natural gut microbiota containsEscherichia coli as a commensal, this bacterium, along with other members of the Enterobacteriaceae family, are usually known for their pathogenic potential. Interestingly, E. coli colonizes first and remains all through life, and in fact, some strains possess beneficial properties such as antibacterial colicin secretion. Among the beneficial strains, E. coli Nissle, isolated in 1917, has been the most extensively explored strain. Adaptability to survive under diverse conditions coupled with facile genetic manipulations enabled the design of E. coli strains with properties to deliver antioxidant, anti-inflammatory, and antitumor molecules. Moreover, genetically modified E. coli strains secreting enzymes for converting sucrose and fructose into insulin and mannitol, respectively, were very effective in preventing the onset of metabolic disease by acting as synbiotics. Thus, E. coli is emerging as a very potent probiotic platform for developing strains with the potential of controlling many metabolic and multifactorial diseases, including cancer.

This report summarizes the innovative approach leading to the discovery of the precision probiotic H. alvei HA4597® and its benefits on body weight and metabolic parameters. H. alvei HA4597® has been identified after the striking findings on the effects of the bacterial metabolite ClpB (Caseinolytic peptidase B) on appetite regulation, through a screening of ClpB-producing strains. Its efficacy in humans has been validated by a multicentric, double-blind, randomized placebo-controlled trial including 236 overweight adults. The successful results on body weight loss of the clinical study support the use of H. alvei HA4597® in the global management of excess weight.

This chapter suggested new methods for monitoring the dynamic processes of rolling stock/rail interaction. This study develops a new technical solution for measuring the wheel/rail interaction forces on a significant part of the sleeper. The theoretical part of this study, using FEM, confirm the ability of piecewise continuous recording of vertical and lateral forces from the wheel/rail interaction by measuring the stresses in two sections of the rail. Also, the optimum location of strain gauges and the effective length of the measuring zone have been determined. The experimental part of this study has been carried out on the stands and the railway track to confirm the effectiveness of the method to determine the vertical and lateral wheel/rail interaction forces, increase the reliable statistical data, improve the measurement accuracy, reducing the time and cost compared with current testing methods. The developed method is recommended to determine the wheel/rail interaction forces and identify defects on the wheels when diagnosing rolling stock on operational and travel regimes.

Attached mass bioreactors have extensively been adopted in the last decades when specific needs have suggested this choice. Benefits and advantages of this multi-faceted technology in wastewater treatment processing are well known, along with the kinetic and mass transfer aspects regarding their operation, essentially belonging to the mass transfer with chemical reaction theory applied to enzymatic catalysis, referred to as Languimur-Hinshelwood kinetics, notably Monod/Michaelis Menten equations. On the other hand, a consolidated literature has dealt with many aspects of the development of strain colonies forming a biofilm. However, a few works have been devoted to the systematic analysis of its physiology, within the framework of the wastewater management of complex substrates and high-loads effluents. This article presents the experimental findings of a research activity covering the junction area between microbiology and bioreactor engineering, against a multifaceted set of operating parameters directly affecting health and stability of the attached biomass. In this respect, important results have been obtained, providing guidance on the attached mass reactor start-up, steady- state operation, impact of xenobiotic substrates, role of nutrients, filaments and foam formation, as well as qualitative aspects of the post-treatment effluent.

Doxorubicin is an anthracycline antibiotic extracted from the bacterium Streptomyces peucetius. Its cytotoxic effect produced by intercalating with DNA causing breakdown of DNA strand which causes cancer cell apoptosis. Despite being an effective anticancer agent it causes several crucial side effects like carditoxicity, neuropathy, hepatotoxicity, nephrotoxicity, alopecia, typhlitis, myelosuppression, neutropenia, anaemia, thrombocytopenia, nausea, and diarrhoea were caused mainly due to the inability to distinguish between cancer cells and normal cells. This chapter mainly focuses on doxorubicin’s side effects, current understanding of the molecular mechanisms, and management and preventive strategies of doxorubicin’s cardiotoxicity during the treatment of various type of cancer.

Mechanical thrombectomy, catheter-based clot retrieval, is an effective way to eliminate emboli, restore cardiopulmonary function, and prevent ischemic injury. Surgical care is an essential component of health care. This basic universal right is not available to everyone. Indeed, countries with low economic resources suffer from a lack of access to surgical care and the most developed countries will have to reduce the cost of health care to ensure the sustainability of provided care quality. New communication technologies have invaded the field of health and have led to the development of a new concept of mobile health. The purpose of this paper is to answer the following question: Can these new tools, and in particular the Smartphone, remedy, even partially, the lack of health care in poor countries and reduce the cost of health care in rich countries? New communication tools, led by the Smartphone, have the capacity to capture, store, retrieve and transmit data to provide instant and personalized information to individuals. This information could be a key element in health systems and can contribute to monitoring health status and improving patient safety and care quality. Mobile telephony via applications and connected objects can facilitate the pre-, intra- and post-operative management of patients. These mobile systems also facilitate the collection and transmission of data. This will allow better analysis of this data and will greatly pave the way to the introduction of artificial intelligence in medicine and surgery. The Smartphone can be used as an important tool for both, diagnosis care and surgical training. Surgeons must adapt their equipment to local resources while respecting safety standards. Covid-19 has put health systems around the world under severe strain. Decision-makers are being forced to make adjustments. The long-vaunted digital health is becoming a reality and a necessity. Healthcare authorities and strategy specialists face challenges in terms of disease prevention and therapy, as well as in terms of health economics and management.
