Abstract:
Family Therapy for Trauma: An Integrative Family and Systems Treatment (I-FAST) Approach offers a stand-alone family therapy treatment approach for trauma, addressing a gap in the trauma treatment literature. The book outlines a flexible yet structured family therapy approach that can integrate intervention procedures from any of the evidence-based manualized trauma treatments into a family treatment framework. The authors show how this flexibility offers great advantages for engaging trauma survivors and their families into treatment, who otherwise would not cooperate with standard trauma treatment approaches. They show how tracking and utilizing client and family frames in the organizing of treatment enhances both family engagement and the healing process in general. We show the role of family interactional patterns in the perpetuation of trauma symptoms and how changing these patterns leads to the resolution of trauma symptoms. The book demonstrates how tracking and enlarging interactional exceptions plays a key role in overcoming problems related to trauma. For clients who are not interested in trauma treatment, the authors show how treatment focusing on whatever issue they are willing to address can simultaneously resolve their trauma symptoms.

Traumatic events, including sexual abuse, experiencing or witnessing violence, and natural disasters, are common among adolescents, and this online therapist guide presents a proven treatment for PTSD that has been adapted for the adolescent population. It applies the principles of Prolonged Exposure (PE) to help adolescents emotionally process their traumatic experiences and follows a four-phase treatment where the patients complete each module at their own rate of progress. It includes modules on motivational interviewing, case management, the rationale for treatment, information-gathering about the trauma, common reactions to trauma, and explains that by systematically confronting situations associated with the trauma, adolescents can overcome avoidance and fear. It covers how memory of the traumatic event can help distinguish the past from the present and promote feelings of mastery, and also includes modules on relapse prevention and treatment termination. It covers the importance of the adolescent's age and developmental level while in therapy, and includes developmentally appropriate materials and guidance on tailoring the treatment to each client's unique situation, including trauma type and family structure.

Respiratory failure can be defined as the inability of the lungs to provide adequate oxygenation or ventilation to sustain life. Respiratory failure can lead to abrupt clinical deterioration and is extremely distressing for patients and families. Advances in technology over the past decade have produced many life-sustaining therapies for patients with respiratory failure. Examples include high-flow oxygen therapy, invasive and noninvasive mechanically assisted breathing ventilation, prostacyclin therapy, and extracorporeal membrane oxygenation (ECMO). The care of these complex patients necessitates policies and procedures to assure quality care in withdrawal. Standardized protocols for withdrawal of life-sustaining respiratory therapies provide structured guidance, reduce variation in practice, and improve family and healthcare provider satisfaction.

Abstract:
There is great variability in how the ketogenic diet (KD) is implemented. Most of the differences are preference based. A consensus statement attempted to unify professional practices. The KD can be safely used in all ages, modified for specific dietary needs, and administered as food, formula, or intravenous alimentation. Effectiveness, compliance, and minimal side effects are achieved using a team-based, family-centered support structure. The KD is commonly initiated in an in-patient setting with a gradual advancement of the fat. Some centers still begin the KD with a brief fast. Out-patient initiation with slower advancement to a full diet can be challenging but safely executed. Time to determine response and duration of treatment in responders varies. Side effects are better understood and best managed proactively by monitoring, but cannot always be prevented and must be weighed with the benefit of continuing the therapy.

The Five-Factor Model (FFM) is arguably the predominant model of general personality structure. There is a considerable body of research supporting its construct validity and practical application. There have been a few books specifically concerning the FFM, but to date there has not yet been a text that brings together in one location all that is known about the FFM. The book begins with an overview chapter on the FFM, followed by in-depth discussions regarding the nature, etiology, importance, and mechanisms of each of the FFM domains. The vast body of research concerning the construct-validity support for the FFM is then provided, including its robustness, factor analytic support, childhood antecedents, cross-language presence, cross-species presence, behavior and molecular genetics, and brain structure and function. The text then provides considerable discussion of the importance and application of the FFM across diverse social concerns, including personality assessment, business and industry, health psychology, marital-family therapy, adult psychopathology, child psychopathology, and clinical utility. There is no comparable text with this much information concerning the validity and utility of the FFM. The text concludes with a final overview chapter.

Abstract:
Catecholaminergic polymorphic ventricular tachycardia (CPVT) is a rare inherited disorder associated with syncope and sudden death manifesting in the young during sympathetic activation. The electrocardiogram is normal and the heart is structurally normal. The diagnosis is usually made with an exercise stress test that shows a typical pattern of onset and offset of adrenergically induced ventricular arrhythmias. Molecular screening of RyR2, the major CPVT gene, is recommended whenever the suspicion of CPVT is high. If a disease-causing mutation is identified, cascade screening allows pre-symptomatic diagnosis among family members. All affected subjects should be treated with beta blockers (nadolol or propranolol). Preliminary data support the association of beta blockers with flecainide. After a cardiac arrest, an implantable cardioverter defibrillator (ICD) should be implanted, but it is accompanied by a disquietingly high incidence of adverse effects. After syncope on beta blocker therapy, left cardiac sympathetic denervation is most effective, preserves quality of life, and does not preclude a subsequent ICD implantation.
