Abstract:
Structural Family Therapy was developed by Salvador Minuchin and colleagues during the 1960s as part of the growing interest in systemic ways of conceptualising human distress and relationship dilemmas, and in working therapeutically with those natural systems and relationships, thought to give rise to distress. Structural family therapy is underpinned by a clearly articulated model of family functioning, and has been developed and used most consistently in services for children and families. A growing body of empirical evidence attests to the efficacy of structural family therapy. As an approach it was extensively critiqued during the 1980s by feminist writers and during the 1990s by those interested in the implications of a social constructionist position. Structural family therapy continues to evolve in response to challenges mounted from within and outwith the systemic field, and as part of integrative practice and multi-systemic approaches, with practitioners ever mindful of the need for regular feedback from family members themselves.

“Research has shown that close and supportive family relationships are a key ingredient of successful offender rehabilitation and that intimate partners and minor children may play a critical role in the process” (Datchi & Sexton, 2013, p. 280). Those affected by incarceration are underserved, specifically in the realm of mental health. There is a need for a better understanding of how systemic theories, such as structural family therapy (SFT) can be utilized with the incarcerated population. SFT is an evidence-based systemic model and defines a problem in terms of family structures, boundaries, hierarchies, roles, rules, and patterns of interaction and coalitions. Thus, the current case study examines family therapy utilizing the SFT model with a family, in which a father was incarcerated. The implication of the current case study is to explore the effectiveness of using the SFT model as the primary approach to treating the incarcerated population.

Culture and racial or ethnic background are important variables to consider when conceptualizing families and resilience. Working effectively with Black families requires culturally competent interventions that honor and build upon their strengths and give attention to the intricate dynamics of relationships. This paper offers an examination of the unique stressors and adversity experienced by Black families as well as factors influencing their adaptive functioning. Enriched structural family therapy (ESFT), a versatile, skill-based, systems approach, is introduced as a viable model to promote resilience in Black families. Through ESFT interventions, Black families are able to successfully manage and cope with stressors while improving overall functioning.

A fundamental theoretical premise in Structural Family Therapy (SFT) is that changes in individual members and improvements in intra-familial relations are realized by repairing the family structure. Dysfunctional families are conceptualized in terms of individuals taking on inappropriate roles (e.g., children acting as if they were parents) and the boundaries between parental executive levels and the children/sibling level are unclear, too rigid, or highly permeable. The therapist’s role is to temporarily engage (join) with family members in a way that generates in-session interactions that exemplify the desirable family structure. While the theory supporting these interventions are well developed, there has been little work done on explicating how such tasks may be interactively accomplished in clinical practice. Drawing from the methods of conversation analysis, our aim for this paper is to show how a master therapist in SFT accomplishes some of these transformations during a single therapy session. We focus on the discursive resources through which the therapist is able to readjust the role relationships between a mother and her daughter (i.e., in such a way that the mother can adopt a more agentive position vis-a-vis her children) and how the therapist’s actions indexed core SFT principles of restructuring the family.

There is a gap in existing literature regarding the integration of family therapies and dialectical behavior therapy (DBT) with high-conflict couples and families. A case application is utilized to highlight how the integration of structural family therapy (SFT) and DBT can inform the conceptualization and treatment of clients. The following case study involves a high-conflict, Children’s Services Bureau–referred couple presenting to therapy with reported multiple stressors and problems. For the purposes of this article, therapeutic change occurs within the family unit, by accepting each member as they are, while at the same time working together to change maladaptive familial structures. The aim of this article is to begin closing the gap in research by exploring how structural family therapy and dialectical behavior can be integrated to create a comprehensive theory and approach to treatment.

Recent studies have shown that suicidality among transgender youth are 3.6 times more likely to report suicidal ideation and 3.2 times more likely to attempt suicide compared with cisgender youth. To alleviate the suicide rates among transgender youth, research suggests the need for family-based interventions. This case study explores the integration of structural family therapy and the Satir growth model to increase connection and support within the family, while also creating clear boundaries that are protective and growth-enhancing. This study provides a session by session overview of how the therapist integrated the models while working with the entire family unit.

Abstract:
Italian experience with family therapy of drug addicts is based on research started in 1970. Italian data are comparable with experiences in other countries. Symptoms are considered relevant to the organization of a certain family system. Family therapy is widely used in the public and private health systems. More than 80% of the treatment centers use it, and there is continuous updating of professionals working in the field. Two main strategies are used: structural and counterparadoxical intervention. These therapeutic approaches are explained in light of a typology of drug addicts established from empirical research data, with a discussion of the evaluation of the results depending on the kind of psychopathology. Family therapy with couples is also discussed, and a typology of couples dealing with drug addiction problems is presented.
