Abstract:
ObjectiveTo compare real-world clinical and economic outcomes among atrial fibrillation (AF) patients undergoing cardiac ablation with the contact force-sensing porous tip THERMOCOOL SMARTTOUCH SF (STSF) catheter versus the Arctic Front Advance Cryoballoon (AFA-CB) catheter.DesignRetrospective, observational cohort study.SettingPremier Healthcare Database (PHD), between 1 September 2016 and 30 June 2018.ParticipantsPatients with AF (≥18 years) were included if they had an index ablation procedure performed using the STSF catheter or AFA-CB catheter at a US hospital that consistently provided inpatient and outpatient data to PHD in the 12-month preindex period. Using 1:1 propensity score matching, patient groups were matched on study covariates.Primary and secondary outcome measuresCost, length of stay (LOS), readmissions, direct current cardioversion (DCCV) and reablation outcomes were compared between matched cohorts of STSF and AFA-CB patients.ResultsA total of 3015 patients with AF met the study criteria, of which 1720 had ablation using the STSF catheter and 1295 had ablation using the AFA-CB catheter. In the propensity-matched sample, patients receiving ablation with the STSF catheter had ~17% lower total costs (US$23 096 vs US$27 682, p≤0.0001) and ~27% lower supply costs (US$10 208 vs US$13 816, p≤0.0001) versus patients receiving ablation with the AFA-CB catheter. A significantly lower likelihood of 4-month to 6-month cardiovascular-related readmission (OR 0.460, 95% CI 0.220 to 0.959) was associated with the STSF catheter versus the AFA-CB catheter. No significant differences in LOS, room and board cost, 4-month to 6-month all-cause and AF-related readmissions, DCCV and reablation were observed among technologies. Sensitivity analysis restricting patient sample by provider ablation volume demonstrated similar results.ConclusionLower index ablation total and supply costs were observed among patients with AF undergoing cardiac catheter ablation using the STSF catheter versus the AFA-CB catheter.

Abstract Aims Pulmonary vein isolation (PVI) using ablation index (AI) incorporates stability, contact force (CF), time, and power. The CLOSE protocol combines AI and ≤6 mm interlesion distance. Safety concerns are raised about surround flow ablation catheters (STSF). To compare safety and effectiveness of an atrial fibrillation (AF) ablation strategy using AI vs. CLOSE protocol using STSF. Methods and results First cluster was treated using AI and second cluster using CLOSE. Procedural data, safety, and recurrence of any atrial tachycardia (AT) or AF >30 s were collected prospectively. All Classes 1c and III anti-arrhythmic drugs (AAD) were stopped after the blanking period. In total, all 215 consecutive patients [AI: 121 (paroxysmal: n = 97), CLOSE: n = 94 (paroxysmal: n = 74)] were included. Pulmonary vein isolation was reached in all in similar procedure duration (CLOSE: 107 ± 25 vs. AI: 102 ± 24 min; P = 0.1) and similar radiofrequency time (CLOSE: 36 ± 11 vs. AI: 37 ± 8 min; P = 0.4) but first pass isolation was higher in CLOSE vs. AI [left veins: 90% vs. 80%; P < 0.05 and right veins: 84% vs. 73%; P < 0.05]. Twelve-month off-AAD freedom of AF/AT was higher in CLOSE vs. AI [79% (paroxysmal: 85%) vs. 64% (paroxysmal: 68%); P < 0.05]. Only four patients (2%) without recurrence were on AAD during follow-up. Major complications were similar (CLOSE: 2.1% vs. AI: 2.5%; P = 0.87). Conclusion The CLOSE protocol is more effective than a PVI approach solely using AI, especially in paroxysmal AF. In this off-AAD study, 79% of patients were free from AF/AT during 12-month follow-up. The STSF catheter appears to be safe using conventional CLOSE targets.

Full-duplex (FD) with simultaneous wireless information and power transfer (SWIPT) in wireless ad hoc networks has received increased attention as a technology for improving spectrum and energy efficiency. This paper studies the outage performance for a SWIPT-based decode-and-forward (DF) FD relaying network consisting of a single-antenna source S, a two-antenna relay R, and a multi-antenna destination D. Specifically, we propose four protocols, namely static time-switching factor with selection combining (STSF-SC), static time-switching factor with maximal ratio combining (STSF-MRC), optimal dynamic time-switching factor with selection combining (ODTSF-SC), and optimal dynamic time-switching factor with maximal ratio combining (ODTSF-MRC) to fully investigate the outage performance of the proposed system. In particular, the optimal time-switching factor from the ODTSF-SC and ODTSF-MRC methods is designed to maximize the total received data at the destination. In this context, we derive exact closed-formed expressions for all schemes in terms of the outage probability (OP). Finally, the Monte Carlo simulations are conducted to corroborate the theoretical analysis’s correctness and the proposed schemes’ effectiveness.

Abstract The receptor tyrosine kinases FLT3 and KIT are highly expressed on the surface of leukemic blasts in most patients with acute myeloid leukemia. Although about one third of patients display activating mutations in FLT3 (and more rarely in KIT), the majority of patients have no mutations in FLT3 or KIT. Previously, we demonstrated that Cbl functions as the E3 ligase for both FLT3 and KIT, and that ligase-inactivating mutations of Cbl stabilize FLT3 and KIT on the cell surface by preventing endocytosis and degradation (Sargin et al, Blood 2007). Furthermore, we demonstrated that expression of E3-ligase deficient Cbl mutants led to the development of a myeloproliferative disease in a murine bone marrow transplantation model (Bandi et al, Blood 2009). However, Cbl mutations are rarely found in AML. Here, we investigated the role of the Cbl regulators suppressors of T-cell signaling 1 and 2(STS1 and STS2) in stabilizing wild-type FLT3 and KIT on the cell surface of hematopoietic stem and progenitor cells (HSPCs). STS1 is ubiquitously expressed, while STS2 expression is restricted to the hematopoietic tissue. STS1 and STS2 constitutively bind to Cbl, while their binding to FLT3 and KIT is dependent on ligand-activation by FL and SCF, respectively. Interestingly, STS1 (but not STS2) functions as a tyrosine phosphatase for both ligand-activated FLT3 and KIT. This required the PGM domain of STS1, as PGM point mutant of STS1 did not dephosphorylate FLT3 or KIT. In line with this, knockdown of STS1 using stably expressing shRNA constructs showed a significant hyperphosphorylation of FLT3 and KIT. By using STS1/STS2 single and double knockout mice, we analyzed the effects of STS1 and STS2 on hematopoietic stem and progenitor cells in vivo. We found that deficiency of STS1 causes an increase of both absolute number and frequency of LSK (lineage marker-, KIT+, Sca1+) cells, which contain HSPCs. This phenotype was even more pronounced in STS1 and STS2 double knockout (dKO) mice, and is mainly attributable to the short term hematopoietic stem cell (ST-HSC) and multipotent progenitor (MPP) cell population, as defined by both standard and SLAM markers. Colony assays using primary bone marrow cells revealed a significantly higher colony forming ability in STS1-KO and dKO cells compared to wild type (wt) cells, particularly after serial replating. A careful analysis of the cells derived from methylcellulose culture revealed an increased proportion of immature (Mac1- CD48+ CD16/32-) cells in STS1-KO and dKO cells. Competitive repopulation assays showed an advantage for dKO cells when compared to wt, suggesting that the LT-HSC compartment is also affected. Even more pronounced were the differences in CFU-S assays (colony forming units spleen), displaying significantly more colonies of dKO compared to wt donor cells, functionally demonstrating a significantly increased ST-HSC / MPP population in dKO donors. A detailed analysis of the downstream signaling events demonstrated that loss of STS1 specifically causes an activated PI3-Kinase / AKT pathway. In summary, our data demonstrates that STS1 functions as a phosphatase of FLT3 and KIT and, using genetic mouse models, indicates a critical role in the maintenance and proliferation of long-term and short-term hematopoietic stem cells. To fully utilize the information from the sensors, this paper proposes a new sensor-fusion technique where the data sets for the previous moments are properly transformed and fused into the current data sets to enable an accurate measurement. Exploration of an unknown environment is an important task for the new generation of mobile service robots. The mobile robots may navigate by means of a number of monitoring systems such as the sonar-sensing system or the visual-sensing system. Note that in the conventional fusion schemes, the measurement is dependent on the current data sets only. Therefore, more of sensors are required to measure a certain physical parameter or to improve the accuracy of the measurement. However, in this approach, instead of adding more sensors to the system, the temporal sequence of the data sets are stored and utilized for the accurate measurement. The theoretical basis is illustrated by examples and the effectiveness is proved through the simulations and experiments. The newly proposed, STSF (Space and Time Sensor Fusion) scheme is applied to the navigation of a mobile robot in an unstructured environment, as well as in structured environment, and the experimental results show the performance of the system.

Power line noise introduces distortions to recorded electrocardiogram (ECG) signals. These distortions compromise the integrity and negatively affect the interpretation of the ECG signals. Despite the fact that the amplifiers used in biomedical signal processing have high common mode rejection ratio (CMRR), ECG recordings are still often corrupted with residual Power Line Interference (PLI) noise. Further improvement in the hardware solutions do not have significant achievements in PLI noise suppression but rather introduce other adverse effects. Software approach is necessary to refine ECG data. Evaluation of PLI noise suppression in ECG signal in the wavelet domain is presented. The performance of the Hard Threshold Shrinkage Function (HTSF), the Soft Threshold Shrinkage Function (STSF), the Hyperbola Threshold Shrinkage Function (HYTSF), the Garrote Threshold Shrinkage Function (GTSF), and the Modified Garrote Threshold Shrinkage Function (MGTSF) for the suppression of PLI noise are evaluated and compared with the aid of an algorithm. The optimum tuning constant for the Modified Garrote Threshold Shrinkage Function (MGTSF) is found to be 1.18 for PLI noise. GTSF is found to have best performance closely followed by MGTSF in term of filtering Gain. HTSF recorded the lowest Gain. Filtering against PLI noise in the wavelet domain preserves the key features of the signal such as the QRS complex.

Identification marker of bacterial leaf blight (BLB) gene resistance on introducted and dihaploid rice germplasm. Bacterial leaf blight (BLB), caused by bacterial pathogen Xanthomonas oryzae pv. oryzae (Xoo), is one of the most devastating diseases in rice. The use of BLB-resistant rice varieties is one of the most efficient ways to protect rice from this disease. BLB-resistant varieties can be produced through the breeding program by using the diverse rice germplasm. The objective of this research was to identify BLB resistance gene on 37 introducted and dihaploid rice lines derived from wide genetic background double crossing from local rice : IR54/Parekaligolara and Bio110/Markuti, by using the molekular markers. As control plant used 23 differential varieties (monogenic lines/IRBB) and TN1 (susceptible). All plants tested were inoculated by 3 selected dominant BLB races (Race III, IV dan VIII). The selected of 19 polymorphism molecular markers used to identify the BLB resistance genes on rice lines tested. The result of this research showed that 4 rice lines were resistance to all BLB races tested. Three molecular markers were specific associated with resistant gene to Race III (Xa7-STS40, Xa1-STS14 and Xa4-STS50); Race IV (Xa1-STS5, Xa4-STS50 and Xa26-STS1), and Race VIII (Xa21-STS6, Xa7-RM20590 and Xa7-STS40). These markers could be utilized for the selection process the developmen of BLB resistance rice lines breeding program.

Abstract:
Introduction. Scientific and technical support (STS) of survey (STSS), design (STSD), and construction (STSC) in accordance with regulatory documents and standards must be executed at facilities belonging to the KS-3 Class and characterizing by an enhanced level of responsibility. However, currently, there is no clear understanding of the necessary and sufficient volume of work accomplished in the course of the STS as well as of the skill level of companies carrying out the work.
 Materials and methods. Existing regulatory documents regarding the STS were analysed. A system approach was suggested for forming the initial data array. The approach is based on the decomposition of the erection process of industrial and civil buildings. Specific conditions for the scientific and technical support of survey, design, and construction were considered.
 Results. Existing regulatory documents regarding the STS of survey, design, and construction does not permit the unambiguous formation of the necessary and sufficient support program. In connection with this, the main steps to be accomplished for the creation of an STS system were formulated. Such an approach will allow all the participants of the construction process to understand the STS requirements and volume unambiguously, avoiding various subjective opinions. The main guidelines of the STS accomplishment were identified. Such types of construction activity as new material or equipment creation, on which recommendations are given for the STS accomplishment, are presented.
 Conclusions. A roadmap of six top-priority steps is suggested. Its realization in the closest time would allow accomplishing the facility STS on terms unified for all the STS participants.

Abstract:
A calcineurin (type 2B)-like protein phosphatase gene designated ppb1+ was isolated from the fission yeast Schizosaccharomyces pombe. The predicted amino acid sequence was 57% identical to rat PP2B alpha. ppb1 null mutant could form colonies at 33 degrees C but the size of the colonies was small at 22 degrees C. Cytokinesis was greatly delayed at 22 degrees C, and a large number of multi-septate cells were produced. The cell polarity control was impaired, causing branched cells. ppb1 null was virtually sterile. These phenotypes were rescued by a plasmid carrying the ppb1+ gene. Multi-septate cells were also produced in wild type at 22 degrees C by cyclosporin A, an inhibitor of calcineurin. This drug effect was enhanced in stst1 null mutant, which was hypersensitive to various drugs and cations. ppb1 null was not affected by cyclosporin A, consistent with the hypothesis that ppb1 is its target. Double-mutant analysis indicated that ppb1 had a function related to that of two other phosphatases, type 1-like dis2 and 2A-like ppa2.ppb1 null-sts1 null showed the severe multi-septate phenotype in the absence of cyclosporin A. ppb1+ and sts1+ gene functions are related. The double mutant ppb1-sts5 was lethal, indicating that the ppb1+ gene shared an essential function with the sts5+ gene. Overexpression of ppb1+ caused anomalies in cell and nuclear shape, microtubule arrays and spindle pole body positioning in interphase cells. Thus the ppb1+ gene appears to be involved in cytokinesis, mating, transport, nuclear and spindle pole body positioning, and cell shape.

Signal transduction from the T cell receptor (TCR) is crucial for T cell–mediated immune responses and, when deregulated, also contributes to the development of autoimmunity. How TCR signaling is regulated is incompletely understood. In this study, we demonstrate a ubiquitin-dependent mechanism in which the deubiquitinase Otud7b has a crucial role in facilitating TCR signaling. Upon TCR ligation, Otud7b is rapidly recruited to the tyrosine kinase Zap70, a central mediator of TCR-proximal signaling. Otud7b deficiency attenuates the activation of Zap70 and its downstream pathways and impairs T cell activation and differentiation, rendering mice refractory to T cell–mediated autoimmune and inflammatory responses. Otud7b facilitated Zap70 activation by deubiquitinating Zap70, thus preventing the association of Zap70 with the negative-regulatory phosphatases Sts1 and Sts2. These findings establish Otud7b as a positive regulator of TCR-proximal signaling and T cell activation, highlighting the importance of deubiquitination in regulating Zap70 function.

Staf sales mengirimkan catatan pesanan kepada staf admin menggunakan aplikasi jejaring sosial yaitu LINE. Staf admin mencatat kembali pesanan untuk staf gudang agar menyiapkan barang sesuai pesanan. Analisis susulan sistem PO merupakan analisis model / cara sistem lama, dimana dapat dijadikan sebagai solusi alternatif kekurangan sistem lama. Analisis usulan sistem PO ini berfokus pada beberapa aktor, yaitu staf admin, staf sales, dan staf gudang. Aktor staf admin memiliki hak untuk mengelola data pegawai, mengelola data pelanggan, mencetak laporan, dan mengelolah data barang. Aktor staf sales memiliki hak untuk menampilkan katalog, melihat daftar data barang, dan mengelola PO. Aktor staf gudang memiliki hak untuk melihat laporan PO. Object oriented analysis (OOA) merupakan metodologi yang digunakan untuk melakukan analisis, dan menggunakan unified modeling language (UML) untuk sebagai analisis proses bisnis dan sistem. Hasil dari analisis sistem PO adalah bentuk rekomendasi analisis dan rancangan tampilan antarmuka untuk rancang bangun aplikasi ke tahap selanjutnya. Analisis rancangan arsitektur sistem dan kebutuhan perangkat dengan penerapan tren teknologi yang akan memberi kemudahan dan dukungan kepada staf admin, staf sales, dan staf gudang yang akan menggunakan sistem PO di UD. The Indonesian government formed the National Team for the Acceleration of Poverty Reduction (TNP2K) to eradicate poverty. TNP2K requires identification of priority areas or poverty hotspots so that the program can be targeted. Scan statistic is one of the most widely used methods to identify poverty hotspots. Cylindrical STSS uses cylindrical scanning windows while most geographical areas are not circular. Flexible STSS is able to detect poverty hotspots in a flexible form. This study aims to identify poverty hotspots using Cylindrical and Flexible STSS then compare the results of both and then determine the best STSS method. Cylindrical STSS tends to have wider hotspots than Flexible STSS. There are a number of districts that are not eligible to be included as poverty Flexible STSS is able to produce better poverty hotspots by not including these districts Poverty hotspots produced by Flexible STSS have higher LLR values. The more suitable STSS method has optimal K values and high suitability with TNP2K priority areas. Cylindrical STSS has an optimal K value when K = 8 and 9. Flexible STSS has a constant LLR value. Flexible STSS has a higher LLR value than Cylindrical STSS at each K value. Flexible STSS with K = 9 has optimal K and high suitability with TNP2K priority areas so that it is the more suitable STSS method to identify poverty hotspots in Java.

Penelitian ini menggunakan metode dekriptif kualitatif, dengan memakai teknik pengumpulan data wawancara, observasi dan dokumentasi. Penelitian ini dilakukan di Kabupaten Tanah datar, yang mana disana terdapat dua orang Staf Ahli yaitu Staf Ahli bidang Pemerintahan, Hukum, Politik, Kemasyarakatan dan Sumberdaya Manusia dan Staf Ahli bidang Ekonomi Pembangunan dan Keuangan. Sampel yang digunakan adalah purposive sampling, yaitu sampel yang diambil berdasarkan pertimbangan peneliti sendiri. Hasil penelitian mengatakan bahwa Staf Ahli secara umum telah mampu melaksanakan tugasnya dengan baik, belum mempunyai target-target tertentu dan program-program yang dijalankan. Staf Ahli juga sudah mampu melaksanakan tugas sesuai tujuan, namun belum mempunyai batasan waktu dalam melaksanakan tugasnya, Staf Ahli sudah bisa melaksanakan tugasnya dengan baik, bertanggungjawab dengan baik. berdasarkan faktor sumberdaya manusianya Staf Ahli sudah mempunyai kualitas yang bagus, dan professional dalam menjalankan tugasnya. Staf Ahli merupakan orang yang sudah berpengalaman dibidangnya. sarana dan prasarana yang diterima oleh staf Ahli memang masih jauh dari kata memadai. Untuk budaya organisasi juga belum terlalu baik penghargaan kepala daerah kepada Staf Ahli yang masih sangat kurang. Dari hasil tersebut dapat disimpulkan bahwa kinerja Staf Ahli Kabupaten Tanah Datar belum optimal. Karena berbagai hambatan, kekurangan yang terjadi. Baik dari faktorintern Staf Ahli itu sendiri maupun dari faktor ekstrern nya. Masih banyak hal yang perlu diperbaiki untuk mencapai kata optimal dalam hal kinerja Staf Ahli ini.

BACKGROUND: Different studies have implicated the exposure to systemic conditions in the aetiology of cardiovascular diseases like chronic inflammation including chronic periodontitis.
 AIM: The present study has been conducted to examine whether biomarker sTNF-R was elevated in apical periodontitis as sTNF-R is a systemic marker of inflammation and has been identified as risk factors for cardiovascular diseases.
 MATERIAL AND METHODS: sTNF-R levels were measured in 52 patients with apical periodontitis (M:F::25:27), aged 20-45 years and in 20 control patients without periodontitis (M:F::10:10, aged 20-48 years). Measurement of sTNF-R1 and sTNF-R2 was carried out in duplicate with standardised, commercially available enzyme immunoassays (R&D Systems Europe, Abingdon, UK).
 RESULTS: The mean sTNF-R1 and sTNF-R2 levels in periodontitis were 820 (240) pg/ml (413 – 1620 pg/ml) and 1309 (403) pg/ml (540 – 2430 pg/ml), while in normal sTNF-R1 and sTNF – R2 levels were 740 (340) pg/ml (407-1240 pg/ml) and 1283 (414) pg (480 – 2340 pg/ml) respectively. Results indicated a positive high relationship between cardiovascular markers such as sTNF-R1 and sTNF – R2 and apical periodontitis.
 CONCLUSION: Elevated levels of sTNF-R1 and sTNF – R2 in apical periodontitis patients indicate an increased independent risk of coronary heart disease.

Introduction: Early tangential excision (TE) and split-thickness skin graft (STSG) have increased the outcome in burn patients treated at specialized burn centers. This study was conducted to compare the length of stay (LOS) in burn patients undergoing early TE & STSG and delayed TE & STSG. Method: This is a retrospective cross-sectional study including 42 patients with varied burn degrees, and TBSA admitted to Jakarta Islamic Hospital Cempaka Putih (JIHCP) Burn Unit. Patients were assigned to two study groups, the early TE & STSG group including 32 patients and the delayed TE & STSG group including 10 patients. All data were collected from the medical record and compared between two study groups. Result: The mean of LOS in a group with early TE & STSG was shorter (9.81±6.41 days) than LOS in the delayed TE & STSG group (15.80±5.67 days). The data of LOS between these groups were compared using an independent T-test. The LOS in the early TE & STSG group was significantly shorter than the delayed TE & STSG group (p=0.012). Conclusion: In patients with burn injuries, early TE & STSG is associated with a shorter length of stay than the delayed TE & STSG. Our study indicates that early excision within five days after burn injury is optimal to reduce the length of stay in burn patients.

A rad23Δ rpn10Δ double mutant accumulates multi-Ub proteins, is deficient in proteolysis, and displays sensitivity to drugs that generate damaged proteins. Overexpression of Sts1 restored normal growth in rad23Δ rpn10Δ but did not overcome the DNA repair defect of rad23Δ. To understand the nature of Sts1 suppression, we characterized sts1-2, a temperature-sensitive mutant. We determined that sts1-2 was sensitive to translation inhibitors, accumulated high levels of multi-Ub proteins, and caused stabilization of proteolytic substrates. Additionally, ubiquitinated proteins that were detected in proteasomes were inefficiently cleared in sts1-2. Despite these proteolytic defects, overall proteasome activity was increased in sts1-2. We propose that Sts1 is a new regulatory factor in the ubiquitin/proteasome pathway that controls the turnover of proteasome substrates.

Abstract The tumor necrosis factor (TNF) plays a key role in inflammatory processes, as this cytokine is one of the earliest to be produced in such a condition, and triggers the following cytokine cascade. In addition, the TNF and their receptor system are believed to play a key role in the growth, differentiation, and/or apoptosis of the malignant cells. As for TNF receptors, the two types, the 55 kDa (p55, TNFR; TNF-R1) and the 75 kDa (p75, TNFR; TNF-R2) are simultaneously expressed on many cells at different levels. The extracellular domains of these two receptors are released from the cell membrane by cleavage of TNF-Rs as soluble TNF-Rs (sTNF-R1, sTNF-R2). Reportedly the serum TNF-Rs level rise in patients with some malignancies. The aim of the present study was to assess the prognostic significance of serum sTNF-R in aggressive non-Hodgkin’s lymphoma (NHL). Consecutive 110 previously untreated patients with aggressive NHL (diffuse large B-cell lymphoma; 94, peripheral T-cell lymphoma; 16) prospectively participated in this study between 1997 and 2002. The patients were treated with 6–8 cycles of CHOP or THP-COP regimens. To evaluate serum levels of sTNF-Rs (p55; TNF-R1, p75; TNF-R2), venous blood samples were drawn from patients immediately before the initiation of treatment. Serum sTNF-R1 and sTNF-R2 were determined using a sandwich enzyme-linked immunosorbent assay (ELISA). In healthy control subjects, the median of serum sTNF-R1 and sTNF-R2 levels were 1.2 ng/ml (range 0.3–2.9) and 4.17 ng/ml (range 1.91–8.51), respectively. High serum sTNF-R level was associated with some poor prognostic factors and low complete remission (CR) rate. Patients with high sTNF-R1(4 ng/ml and over) and sTNF-R2 (15 ng/ml and over) at onset had significantly lower survival rates (5-year: 19%, 19%) than those with low sTNF-R1 (under 4 ng/ml) and sTNF-R2 (under 15 ng/ml) (62%, 69%), respectively (p<0.0005, p<0.0001). Multivariate analysis employing sTNF-R2 and some conventional prognostic factors demonstrated that sTNF-R2 and performance status for overall survival (OS) and sTNF-R2, sIL-2R, and LDH for event free survival (EFS) were significantly poor prognostic factors. As for TNFa, a serum TNFa level is not related with sTNF-R1 or sTNF-R2 level in aggressive NHL. In addition, serum TNFa level is not associated with OS and EFS. In conclusion, serum sTNF-R2 might be a significant prognostic factor for aggressive NHL and a useful tool for selecting the appropriate therapeutic strategy in the treatment of aggressive NHL. The most reliable prognostic factor and the best combination of some prognostic factors for aggressive NHL should be clarified in order to assist in selecting appropriate treatment.

TNF is important in rheumatoid arthritis (RA) and spondyloarthritis (SpA) pathogenesis. Recently reports showedsTNF levels were significantly lower in SpA than RA synovial fluid (SF). Therefore, the differential regulation of TNFin both diseases were investigated focusing on the balance of sTNF-tmTNF and both TNF receptors expression. Synovialfluid from 22 RA and 25 SpA were analyzed by ELISA. sTNF was confirmed significantly higher in RA than SpA.Total TNF of synovial tissues assessed by qPCR showed similar levels, suggesting a higher tmTNF/sTNF ratio in SpA.The levels of sTNF-R1 and sTNF-R2 were significantly lower in SpA than RA SF. Interestingly, sTNF-R2/sTNF-R1increased in SpA. qPCR on synovial tissue showed similar mRNA levels for TNF-R2 but decrease of TNF-R1 in SpA.IL-6R levels did not showed differences and TIMP-3 tended to be higher in RA than in SpA SF. In conclusion, weobserved a shift from sTNF/TNF-R1 in RA to tmTNF/TNF-R2 in SpA.

Despite advances in trauma care, a subset of patients surviving damage control cannot achieve fascial closure and require split-thickness skin grafting (STSG) of their open abdomen. Controversy exists as to whether reconstruction of the gastrointestine (GI) should be staged or performed at the time of abdominal wall reconstruction (AWR). Many surgeons do not believe that operations through the STSG can be completed safely or without loss of graft. This series reviews the outcomes of operations for GI reconstruction performed through the elevated healed STSG. Concurrent series on all patients undergoing abdominal operation through the STSG. The technique involves elevating the STSG, lysing adhesions only as needed, avoid detaching underlying omentum or viscera to avoid devascularization, and then reattaching the elevated STSG to the abdominal wall with simple sutures. From 1995 to 2017, 27 patients underwent 40 distinct procedures during 36 separate abdominal reoperations (89% GI) through the elevated STSG approach at three Level I trauma centers at a mean interval of 11 months from application of the STSG. One STSG was lost (patient closed with skin flaps), one patient had 30 per cent loss of the STSG (regrafted), and one patient had 10 per cent loss of the STSG (allowed to granulate). One patient required a small bowel resection for intraoperative enterotomy during a difficult operative dissection. There were no GI complications, intraabdominal infections, or deaths, and all patients were deemed fit to undergo AWR after three months. Major intraabdominal reoperations can be readily and safely accomplished through the elevated STSG approach with a <4 per cent need for regrafting. This staged approach significantly simplifies and increases the safety of a second stage AWR.

Abstract Abstract 1575 Background: We have previously reported that serum concentration of serum-soluble tumor necrosis factor receptor 2 (sTNF-R2) predicts the clinical outcome of patients with aggressive non-Hodgkin's lymphoma (NHL) treated with CHOP without rituximab (Goto et al. 2005). We extracted the diffuse large B-cell lymphoma (DLBCL) patients from aggressive NHL, used in our previous study, and extended our observation period. We found that serum sTNF-R2 level was a strong independent prognostic factor even for long-term observation in the CHOP era (8-year overall survival (OS); high serum sTNF-R2 group: 17.6%, low sTNF-R2 group: 69.8%, P<0.0001). Based on the result, we analyzed serum sTNF-R2 for patients with DLBCL who had received the CHOP plus rituximab (R-CHOP) regimen. Purpose: We aim to re-assess the prognostic significance of serum sTNF-R2 for DLBCL patients treated with R-CHOP regimen, and to assess sTNF-R2 with subtype of DLBCL, such as germinal center B cell type (GCB type) and non-GCB type which were classified by Hans et al. method (Hans et al. 2004). Methods: Consecutive 154 untreated patients with DLBCL prospectively participated in this study between 2002 and 2008. The patients were treated with R-CHOP. Results: In all patients with DLBCL, the median of serum sTNF-R2 level was 13.72 ng/ml (range 2.66–112.5 ng/ml). Various poor prognostic indicators, such as elderly age, poor performance status, increased LDH, multiple extranodal involvement sites, advanced disease, and existence of B-symptoms, were strongly associated with high serum sTNF-R2 levels. Serum sTNF-R2 levels significantly correlated with the increasing grade of international prognostic index (IPI), revised-IPI (R-IPI) (Sehn et al. 2007), and subtype, respectively (P<0.0001, P<0.0001, P=0.0058). We established the cut-off value for sTNFR2 at 20 ng/ml which was determined by ROC analysis. Patients with high sTNF-R2 (20 ng/ml and over) at onset had significantly lower OS rate (5-year: 29%), than that with low sTNF-R2 (5-year: 83%), respectively (P <0.0001). Multivariate analysis involving R-IPI, subtypes of DLBCL and serum sTNF-R2 revealed that sTNFR-2 significantly correlated with OS and progression free survival (PFS) (OS: P = 0.0001, PFS: P= 0.0085). Additionally, as the OS rate was about 50% even in the poor prognosis group of R-IPI, we tried to divide risk categories of R-IPI into low and high sTNF-R2 groups, resulting actually in five groups: very good risk group, good risk group with high sTNF-R2, good risk group with low sTNF-R2, poor risk group with high TNF-R2, and poor risk group with low sTNF-R2. A strong significant difference was observed. (5-year OS; very good risk group: 100%, good risk group with high sTNF-R2: 20%, good risk group with low sTNF-R2: 85%, poor risk group with high TNF-R2: 28.6%, and poor risk group with low sTNF-R2: 77.8%.) Conclusion: These results suggest that a high serum sTNF-R2 level predicts a poor prognosis in DLBCL and may be a useful biomarker for selecting appropriate treatment. Upfront high dose chemotherapy or alternative therapy of first line chemotherapy instead of R-CHOP might be well indicated for DLBCL patients with high serum sTNF-R2. SUMMARYStreptococcal toxic shock syndrome (STSS) is a severe invasive infection characterized by the sudden onset of shock, multi-organ failure, and high mortality. In Japan, appropriate notification measures based on the Infectious Disease Control law are mandatory for cases of STSS caused byβ-haemolytic streptococcus. STSS is mainly caused by group A streptococcus (GAS). Although an average of 60–70 cases of GAS-induced STSS are reported annually, 143 cases were recorded in 2011. To determine the reason behind this marked increase, we characterized theemmgenotype of 249 GAS isolates from STSS patients in Japan from 2010 to 2012 and performed antimicrobial susceptibility testing. The predominant genotype was found to beemm1, followed byemm89,emm12,emm28,emm3, andemm90. These six genotypes constituted more than 90% of the STSS isolates. The number ofemm1,emm89,emm12, andemm28isolates increased concomitantly with the increase in the total number of STSS cases. In particular, the number ofmefA-positiveemm1isolates has escalated since 2011. Thus, the increase in the incidence of STSS can be attributed to an increase in the number of cases associated with specific genotypes.

Cell morphogenesis is a fundamental phenomenon that involves understanding a number of biological processes including the developmental program, polarity and cell division. Fission yeast sts5 mutant cells are round rather than cylindrical with cortical actin randomly dispersed. Genetic analyses demonstrate that the sts5+ gene is required for maintenance of cell shape during interphase when the cell normally exhibits polarised growth. The sts5 mutant is not defective in cell wall integrity. Deletion of ppe1+, which encodes a type 2A-like protein phosphatase, shows similar phenotypes to the sts5 mutant and these two mutations are synthetically lethal. Multicopy plasmids containing either the protein kinase C-like gene pck1+ or the protein tyrosine phosphatase pyp1+, an inhibitor of an osmosensing Sty1/Spc1 MAP-kinase, are capable of suppressing the sts5 mutation. Consistent with this, we have found that the wis1 mutation, which is defective in a MAP-kinase kinase of the pathway, suppresses the sts5 mutation. The predicted sts5+ gene product exhibits sequence similarity to two yeast proteins, Dis3 and Ssd1 and a nematode protein, F46E8.6, where the former two yeast proteins have been shown to be involved in cell cycle control and cell morphogenesis. The sts5+ gene is not essential for cell viability, but is absolutely required for polarised growth as the gene disruption showed the same phenotypes as those of the original mutants. Overexpression of the sts5+ gene resulted in altered cell morphology and, cortical actin in these overproducing cells was also abnormal, fainter and often dispersed. Anti-Sts5 antibody specifically detected a 130 kDa protein by western blotting. A green fluorescent protein-Sts5 fusion protein localised in the cytoplasm with a discrete punctate pattern, suggesting that the Sts5 protein is a component of a novel structure. These results have indicated that the Sts5 protein is a crucial determinant of polarised growth and that it functionally interacts with the serine/threonine phosphatase, protein kinase C, and an osmosensing MAP-kinase to maintain cell morphology.

Cell adhesion is essential for cell cycle progression in most normal cells. Loss of adhesion dependence is a hallmark of cellular transformation. The F-box protein Skp2 (S-phase kinase-associated protein 2) controls G1–S-phase progression and is subject to adhesion-dependent transcriptional regulation, although the mechanisms are poorly understood. We identify two cross-species conserved binding elements for the STAF (selenocysteine tRNA gene transcription-activating factor) in the Skp2 promoter that are essential for Skp2 promoter activity. Endogenous STAF specifically binds these elements in EMSA (electrophoretic mobility-shift assay) and ChIP (chromatin immunoprecipitation) analysis. STAF is sufficient and necessary for Skp2 promoter activity since exogenous STAF activates promoter activity and expression and STAF siRNA (small interfering RNA) inhibits Skp2 promoter activity, mRNA and protein expression and cell proliferation. Furthermore, ectopic Skp2 expression completely reverses the inhibitory effects of STAF silencing on proliferation. Importantly, STAF expression and binding to the Skp2 promoter is adhesion-dependent and associated with adhesion-dependent Skp2 expression in non-transformed cells. Ectopic STAF rescues Skp2 expression in suspension cells. Taken together, these results demonstrate that STAF is essential and sufficient for Skp2 promoter activity and plays a role in the adhesion-dependent expression of Skp2 and ultimately cell proliferation.

 Berdasarkan hasil pembahasan menunjukan bahwa tugas staf HRD telah berjalan sesuai dengan peraturan PT. Jakarta Teknologi Utama yang berlaku. Kendala yang sering ditemukan diantaranya yaitu kurang detailnya instruksi antar karyawan tentang tugas yang diberikan, keterlambatannya sistem PEGA, banyaknya penarikan absensi karyawan dalam satu folder membuat staf kesulitan untuk menemukan data tersebut. Solusi yang dilakukan ialah staf HRD mengkonfirmasi lebih setiap instruksi yang diberikan oleh atasan, masalah berupa keterlambatan sistem PEGA staf langsung menghubungi bagian IT untuk segera melakukan perbaikan, penarikan data absensi yang susah ditemukan staf melakukan pengelompokkan dalam beberapa folder sehingga memudahkan staf untuk menemukannya. Solusi yang dilakukan masih belum efektif karena terkadang masih terganggu dengan pekerjaan urgent yang harus didahulukan.

Few studies have explored the relation between withdrawn behavior (WB) and exercise and screen time. The current study used exploratory factor analysis to examine the factor structure of leisure-time exercise behavior (LTEB) and screentime sedentary behavior (STSB) in a clinical sample of youth. Structural equation modeling was employed to investigate the relations between WB and LTEB and STSB, conditional on gender. WB was assessed using the Child Behavior Checklist, and LTEB and STSB were measured using the Vermont Health Behavior Questionnaire. LTEB and STSB emerged as two separate factors. Gender moderated the structure of STSB only. For boys and girls, WB was inversely related to LTEB but not significantly related to STSB. LTEB and STSB are best represented as distinct, uncorrelated constructs. In addition, withdrawn youth may be at risk for poor health outcomes due to lower rates of LTEB. Mental health clinicians, sports psychologists, and related providers may be uniquely qualified to enhance motivation for sports participation in withdrawn youth.

Abstract We have studied the interference of soluble tumor necrosis factor-alpha receptor p55 (sTNF-R55) on the quantification of tumor necrosis factor-alpha (TNF-alpha) in three immunoassays: the commercially available enzyme amplified-sensitivity immunoassay (EASIA) of Medgenix and the ELISA of Boehringer Mannheim, and an assay based on electrochemiluminescence (ECL) technology. TNF-alpha measurements by EASIA and ELISA were not affected by the presence of sTNF-R55, but results by the ECL method were clearly influenced by the receptor. We then performed a second set of experiments with the ECL system to examine what factors might influence the importance of interference from sTNF-R55 and soluble TNF-alpha receptor p75 (sTNF-R75), which influences TNF-alpha measurements the same as sTNF-R55. We found that extending the incubation time and increasing the concentration of capture antibody decreased the interfering capacity of sTNF-R55 and sTNF-R75. A clear insight into these phenomena is of utmost importance for correct interpretation of cytokine quantification.

Over the past 10 years, U.S. undergraduate participation in study abroad has increased dramatically. The majority of these study abroad experiences have durations of less than eight weeks. Due to the rapid growth in both of these areas, there has been increased interest in assessing student outcomes associated with short-term study abroad (STSA) programs. This paper investigates the impact of an STSA experience on undergraduate students' levels of global awareness over the entire STSA sequence: pre-preparatory course, post-preparatory course, and post-STSA program. We adopt and extend the measures constructed in Chieffo and Griffiths (2004),by implementing a longitudinal study across 23 separate STSA courses. Findings indicate that students’ global awareness does not change significantly after the preparatory course, but that students' levels of global awareness experience statistically significant increases along several dimensions after completing the STSA program. We also find that students who have completed a previous study abroad experience still benefit from an additional STSA course.

The receptors for tumor necrosis factor (TNF) exist in cell-associated as well as soluble forms, both binding specifically to TNF. Since the soluble forms of TNF receptors (sTNF-Rs) can compete with the cell-associated TNF receptors for TNF, it was suggested that they function as inhibitors of TNF activity; at high concentrations, the sTNF-Rs indeed inhibit TNF effects. However, we report here that in the presence of low concentrations of the sTNF-Rs, effects of TNF whose induction depend on prolonged treatment with this cytokine are augmented, reflecting an attenuation by the sTNF-Rs of spontaneous TNF activity decay. Evidence that this stabilization of TNF activity by the sTNF-Rs follows from stabilization of TNF structure within the complexes that TNF forms with the sTNF-Rs is presented here, suggesting that the sTNF-Rs can affect TNF activity not only by interfering with its binding to cells but also by stabilizing its structure and preserving its activity, thus augmenting some of its effects.

Background Functional exercise capacity assessment is recommended in children with cystic fibrosis (CF). The six-minute walk test (6MWT) is a valid evaluation of exercise capacity but can be technically complex. Inversely, the sit-to-stand test (STST) is a simple method to evaluate exercise capacity, and is validated in healthy children and adults with CF. This study aimed to evaluate STST measurement properties in children and adolescents with CF. Methods In this multicenter study, children with CF (6 to 18 years) performed two iterations of both the STST and the 6MWT in a randomized order. Criterion validity was determined by assessing correlations between STST repetitions and 6MWT distance (6MWD). Intra-rater reliability, test-retest repeatability, mean bias and limits of agreement were also assessed. Relationships with other outcomes (i.e. respiratory and quadriceps muscle strength) and cardio-respiratory responses were analysed for both tests. Results Thirty-six children with CF were included (mean age 12.0 ±3.5 years and FEV1 95.8 ±25.0%). On average, 39.6 ±10.5 repetitions were performed during the STST and mean 6MWD was 596.0 ±102.6 meters. STST number of repetitions was significantly correlated with 6MWD (r = 0.48; p<0.01). Both tests had very good intra-rater reliability (ICCSTST = 0.91 (95%CI 0.76–0.96) and ICC6MWT = 0.94 (95%CI 0.85–0.97)), and a significant test-retest learning effect. The number of STST repetitions was not correlated with quadriceps or respiratory muscle strength test, and the STST induced fewer cardio-respiratory responses than the 6MWT. Conclusions The STST is an easy-to-use functional test with moderate criterion validity when compared to the 6MWT in children with CF, probably because both tests measure different components of functional exercise capacity. The STST is useful when the 6MWT is unfeasible, however further investigations are required to explore the clinical implications of STST results in children with CF. The dorsal bank of the superior temporal sulcus (STSd) bears anatomical relations similar to those of perirhinal cortex, an area critical for visual recognition memory. To examine whether STSd makes a similar contribution to visual recognition memory, performance on visual delayed nonmatching-to-sample (DNMS) was assessed in rhesus monkeys with combined or separate ablations of the perirhinal cortex and STSd as well as in unoperated controls. Consistent with previous findings, ablations of perirhinal cortex produced deficits nearly as severe as that found after rhinal (i.e., entorhinal plus perirhinal) cortex lesions. However, combined lesions of perirhinal cortex and STSd produced a deficit no greater than that produced by perirhinal cortex ablation alone, and lesions of STSd alone were without effect on DNMS. We conclude that STSd is not critically involved in visual recognition memory.

288 Background: Transforming growth factor-beta (TGF-β) is a multifunctional regulatory factor. Here, we measured serum soluble TGF-β (sTGF-β) levels and evaluated its dynamics and prognostic capabilities during chemotherapy in unresectable pancreatic cancer patients. Methods: Sixty patients treated with FOLFIRINOX as the first-line palliative chemotherapy were prospectively enrolled. We prospectively collected blood samples at the time of diagnosis, first response assessment, and disease progression and measured serum sTGF-β using an enzyme-linked immunosorbent assay. Results: The patients’ median overall survival (OS) and progression free survival (PFS) were 10.3 (95% confidence interval [CI], 8.5–12.1) and 6.5 (95% CI, 4.9–8.1) months, respectively. Patients with low sTGF-β at diagnosis ( < 31.2ng/mL) had better OS and PFS than patients with high sTGF-β, respectively (OS, 13.7 vs. 9.2 months; hazard ratio [HR], 2.602; P =0.004; PFS, 9.0 vs. 5.8 months; HR, 2.010; P =0.034). At the time of disease progression, sTGF-β was increased compared with that of diagnosis (mean 26.4 vs. 23.9ng/mL). Especially, in patients with a partial response, sTGF-β was significantly increased at disease progression (mean 25.7 vs. 31.0ng/mL; P =0.049). Conclusions: Pre-treatment sTGF-β levels serve as a prognostic indicator in unresectable pancreatic cancer patients treated with FOLFIRINOX chemotherapy. Likewise, the dynamics of sTGF-β during chemotherapy have prognostic value.

Kajian ini dijalankan bertujuan membina satu sistem pementoran secara berstrategi untuk diguna pakai dan mengenal pasti tahap kesediaan staf akademik di PPD SPACE UTM terhadap sistem ini. Banyak kepentingan dan faedah sistem pementoran kepada staf akademik dan organisasi terutamanya dari segi kenaikan pangkat, pengurusan bakat dan lain-lain. Kajian ini melibatkan 87 responden daripada 94 populasi staf akademik di PPD SPACE UTM di Kampus Kuala Lumpur dan Johor Bahru. Data telah dikumpulkan melalui instrumen borang soal selidik yang telah diedarkan kepada semua staf akademik. Hasil daripada kajian literatur yang telah dilaksanakan, satu sistem pementoran berstrategi telah dibangunkan iaitu objektif kajian pertama. Bagi objektif kedua, analisis kajian mendapati bahawa 97.70% staf akademik bersedia menerima pelaksanaan pementoran berstrategi. Didapati bahawa, faktor kepentingan sistem mentor-menti yang utama kepada staf akademik adalah dapat meningkatkan keberkesanan pengajaran, penyelidikan dan perkembangan profesional iaitu 56.32%. Manakala 28.74% menyatakan pementoran menyokong prestasi kenaikan pangkat di kalangan staf dan 5.75% menyatakan ia meningkatkan minat dalam kerjaya akademik. Dan masing-masing 4.60% berpendapat faedah kepentingan sistem pementoran ini dapat mempromosikan perkembangan kerjaya dan kepuasan serta 4.60% lagi menyatakan ia dapat meningkatkan produktiviti PPD SPACE UTM. Kesimpulannya, terdapat banyak kepentingan yang boleh diperoleh dari sistem pementoran berstrategi ini. Sokongan dan komitmen daripada staf akademik dan pihak pengurusan tertinggi UTMSPACE adalah diperlukan bagi menjayakan program pementoran berstrategi di peringkat PPD SPACE UTM dan seterusnya dikembangkan di peringkat UTMSPACE secara keseluruhannya.

Soft tissue sarcomas (STSs) are rare mesenchymal tumors. With more than 80 histological subtypes of STSs, data regarding novel biomarkers of strong prognostic and therapeutic value are very limited. To date, the most important prognostic factor is the tumor grade, and approximately 50% of patients that are diagnosed with high-grade STSs die of metastatic disease within five years. Systemic chemotherapy represents the mainstay of metastatic STSs treatment for decades but induces response in only 15–35% of the patients, irrespective of the histological subtype. In the era of immunotherapy, deciphering the immune cell signatures within the STSs tumors may discriminate immunotherapy responders from non-responders and different immunotherapeutic approaches could be combined based on the predominant cell subpopulations infiltrating the STS tumors. Furthermore, understanding the immune diversity of the STS tumor microenvironment (TME) in different histological subtypes may provide a rationale for stratifying patients according to the TME immune parameters. In this review, we introduce the most important immune cell types infiltrating the STSs tumors and discuss different immunotherapies, as well as promising clinical trials, that would target these immune cells to enhance the antitumor immune responses and improve the prognosis of metastatic STSs patients.

Grain refinement is attempted using severe plastic deformation (SPD) through the severe torsion straining process (STSP) which we have developed recently. The STSP is a continuous process for grain refinement without requirement of any die. In this study, an AZ61 Mg alloy was subjected to STSP at a temperature of 573 K with a rotation speed of 10 rpm and a moving speed of 200 mm/min. With this process, an initial grain size of ~20 μm was reduced to ~2~3 μm. Room temperature compression tests revealed that there were no cracks after 15% of compression for the STSP sample whereas fracture occurred for a conventionally extruded sample. For compression tests at 473 K, no cracks occurred in the STSP samples after 80% compression but compression was feasible without cracking only up to 20% for an extruded sample. It is shown that the STSP can be useful for grain refinement and ductility improvement of the AZ61 Mg alloy.

Penelitian ini bertujuan untuk mengetahui pengaruh gaya kepemimpinan transformasional kepala sekolah terhadap etos kerja guru dan staf. Sampel penelitian ini yaitu 56 guru dan staf di Sekolah Menengah Pertama Negeri 12 Bandung. Pengumpulan data dilakukan dengan kuesioner. Data yang terkumpul dianalisis secara kuantitatif menggunakan program Microsoft Excel 2016 dan SPSS 20 for windows. Hasil penelitian menunjukkan bahwa ada hubungan yang signifikan antara gaya kepemimpinan transformasional kepala sekolah dan etos kerja guru beserta staf dengan nilai koefisien sebesar 0,370. Selain itu, gaya kepemimpinan transformasional kepala sekolah berpengaruh sebesar 13,7% terhadap etos kerja guru dan staf. Jika gaya kepemimpinan transformasional kepala sekolah ditingkatkan sebesar 53,483 maka nilai etos kerja guru dan staf akan bertambah sebesar 0,260. Berdasarkan hasil penelitian tersebut, penelitian ini merekomendasikan perlunya pengembangan dan penerapan kepemimpinan transformasional kepala sekolah.

Penelitian ini membahas mengenai penerapan berbagi pengetahuan di Perpustakaan di lingkup Kementrian Kelautan dan Perikanan (KKP) wilayah DKI Jakarta. Tujuan penelitian adalah untuk mengidentifikasi makna pada proses berbagi pengetahuan yang dilakukan oleh staf perpustakaan dan faktor yang mempengaruhinya. Penelitian menggunakan metode kualitatif dengan pendekatan studi kasus. Pengumpulan data dilakukan melalui observasi, wawancara, dan analisis dokumen, selama Februari hingga April 2019. Penentuan informan menggunakan snowball sampling, dan diperoleh 10 informan dari enam perpustakaan di lingkup KKP wilayah DKI Jakarta. Data dianalisis dengan membuat pengelompokan berdasarkan kategori berdasarkan konsep berbagi pengetahuan, yang kemudian diinterpretasi. Temuan menunjukkan bahwa kegiatan berbagi pengetahuan telah diterapkan oleh masing-masing staf perpustakaan sejak lama namun tidak terstruktur. Penerapannya dilakukan melalui sarana formal seperti Sinau SLiMS, Bimbingan Literasi, Rapat, dan Forum, dan sarana informal melalui diskusi, media sosial seperti whatsapp dan email. WAG dimanfaatkan secara lebih maksimal. Faktor yang mendorong staf dalam berbagi dipicu oleh right sizing organisasi yang mengubah struktur organisasi. Hilangnya fungsi pepustakaan mempengaruhi anggaran, jabatan, dan berkurangnya jumlah sumberdaya manusia di dua perpustakaan; berkurangnya kegiatan berbagi pengetahuan antar staf perpustakaan; minimnya imbalan yang berdampak pada turunnya motivasi staf dalam berbagi pengetahuan. Simpulan menunjukkan bahwa penerapan berbagi pengetahuan oleh staf Perpustakaan berjalan kurang efektif. Hal tersebut terlihat dari tidak adanya kebijakan danprogram khusus, sehingga berdampak pada hilangnya rasa keterikatan atau tanggung jawab untuk melakukan kegiatan berbagi pengetahuan. Sarannya adalah organisasi perlu menciptakan kebijakan atau program khusus yang memungkinkan staf membangun keterikatan/rasa tanggung jawab dalam berbagi pengetahuan; menciptakan database atau sistem informasi yang berorientasi pada pemustaka yang mampu menangkap pengetahuan tacit dalam kognisi staf dan mampu mengakomodasi diskusi online; serta menambah jumlah staf perpustakaan.

Penelitian ini dilakukan dengan tujuan untuk melihat dampak penghargaan terhadap kinerja staf pada hotel bintang tiga di Surabaya. Jenis penelitian yang dilakukan pada penelitian ini adalah penelitian kuantitatif deskriptif dan kausal yang bertujuan untuk mengetahui hubungan sebab akibat dari penghargaan dan kinerja staf. Sampel dalam penelitian ini adalah 73 responden yang merupakan staf hotel bintang tiga di Surabaya. Hasil penelitian menunjukkan penghargaan moneter dan penghargaan non moneter yang diberikan pada hotel bintang 3 di Surabaya memiliki dampak yang positif dan signifikan terhadap kinerja staf.

Abstract:
Objective: Recent reports have suggested that the existence of associations between hormonal dysregulation and chronic upregulation of inflammatory markers, which may cause obesity-related disturbances. Thus, we examined whether acylated ghrelin (AcylG) and total ghrelin (TotG) levels could be associated with the following inflammatory markers: C-reactive protein (CRP), tumor necrosis factor α (TNF-α), and soluble TNF receptor 1 (sTNF-R1). Design: Cross-sectional study consisting of 50 overweight and obese postmenopausal women. Methods: AcylG and TotG levels were assessed at 0, 60, 160, 170, and 180 min of the euglycemic/hyperinsulinemic clamp (EHC). We evaluated insulin sensitivity, body composition, and blood lipid profiles as well as fasting concentrations of CRP, TNF-α, and sTNF-R1. Results: In fasting conditions, sTNF-R1 was negatively correlated with AcylG (r = −0.48, P < 0.001) levels. In addition, AcylG/TotG was associated negatively with sTNF-R1 (r = −0.44, P = 0.002) and positively with TNF-α (r = 0.38, P = 0.009) values. During the EHC, TotG (at all time points) and AcylG (at 60 and 160 min) values were significantly decreased from fasting concentrations. AcylG maximal reduction and area under the curve (AUC) values were correlated to sTNF-R1 (r = −0.35, P = 0.02 and r = −0.34, P = 0.02, respectively). Meanwhile, the AcylG/TotG AUC ratio was associated negatively with sTNF-R1 (r = −0.29, P < 0.05) and positively with TNF-α (r = 0.36, P = 0.02). Following adjustments for total adiposity, sTNF-R1 remained correlated with fasting and maximal reduction AcylG values. Similarly, AcylG/TotG ratios remained significantly correlated with sTNF-R1 and TNF-α. Importantly, 23% of the variation in sTNF-R1 was independently predicted by fasting AcylG. Conclusion: These results are the first to suggest that both fasting and EHC-induced AcylG profiles are correlated with fasting values of sTNF-R1, a component of the TNF-α system. Thus, AcylG may act, at least in part, as one mediator of chronic inflammatory activity in human obesity.
