Aims of the study were to identify the pathophysiological and anatomo-functional factors implicated in the genesis of the motor episodes and possible structural alterations and/or metabolic disorders ,in NFLE patients using advanced techniques of Magnetic Resonance (MR). We have collected a series of patients with NFLE attending the Epilepsy and Sleep Centres of the Department of Neurological Sciences, Bologna University. For each NFLE patient, one control subject of the same sex and age (±5 years) was recruited. All subjects were studied using proton spectroscopy (1H-MRS), diffusion tensor imaging and high resolution 3D-imaging for morphometric analyses. In particular, 1H-MRS was performed in two regions of interest: the thalami and the anterior cingulate gyrus. Nineteen patients (7M), mean age 34 years (range 19-50) and 14 controls (6 M), mean age 30 years (range 19-40), were included in the final analysis. At the level of the anterior cingulate the ratio of the concentration of N-Acetyl-Aspartate compared to Creatine (NAA/Cr) was significantly reduced in patients compared with controls (p=0,021). A multiple regression analysis was performed, showing that the NAA/Cr ratio in the anterior cingulate in patients correlated with the seizures frequency (p=0,048), being lower in patients with very frequent seizures. As NAA is a marker of integrity, density and neuronal function, we speculate that metabolic changes in specific structures, such as the anterior cingulate, may underlie the pathogenesis of NFLE. Our results open up new possibilities for the use of methodologies of biomedical signal/image processing to characterize the areas involved in the genesis of NFLE, which are currently largely unknown and to further clarify the etiology of this type of epilepsy.

Full textPRESUPPOSTI DELLO STUDIO L’eziopatogenesi delle malformazioni anorettali è sconoscosiuta e multifattoriale. La classificazione delle MAR dal punto di vista genetico non è semplice per il fatto che tali malformazioni si presentano sotto differenti forme influenzate probabilmente, da fattori diversi come il genere e le anomalie associate. Sono descritte in letteratura sindromi genetiche e cromosomiche associate a tali malformazioni e per tale motivo risulta importante conoscere il fenotipo con le quali si manifestano ed eseguire un analisi citogenetica classica nei pazienti con MAR al fine di per poterle individuare anomalie cromosomiche che potrebbero essere associate ad una certa ereditarietà. Il normale sviluppo dell’intestino posteriore (hindgut) dipende dal corretto sviluppo della cloaca e della membrana cloacale, nonché dalle corrette interazioni epitelio-mesenchimali indispensabili per una morfogenesi corretta. Sonic Hedgehog (Shh) è un eccellente candidato come mediatore nelle interazione tra endoderma e mesoderma , multipli studi su modelli animali hanno evidenziato come la ridotta o mancata presenza di Shh o dei mediatori a valle del suo segnale, siano in grado di causare difetti diversi nello sviluppo della regione anorettale. SCOPO La prima parte dello studio è per la maggior parte retrospettivo, ed ha lo scopo di classificare dal punto di vista genetico e clinico, i pazienti affetti da MAR seguiti presso il reparto di chirurgia pediatrica fino all’anno 2011 (periodo 1991-2011), suddividendoli in forme sindromiche e non, in casi sporadici o familiari. Un ulteriore parte dello studio ha lo scopo di identificare anomalie cromosomiali associate alle MAR. La seconda parte ha lo scopo di effettuare una caratterizzazione morfologica e immunoistochimica del tessuto prelevato dal sito della malformazione per verificare quali alterazione parietali siano presenti, e valutare l’espressione di Shh nel contesto del tessuto normale e patologico. MATERIALI E METODI La prima parte del lavoro retrospettivo è stato ottenuto rivalutando la storia clinica di tutti i pazienti seguiti presso il reparto di chirurgia Pediatrica dall’anno 1991 fino al 2011. Per il conseguimento del secondo obiettivo è stata effettuata l’analisi citogenetica classica e molecolare (mediante FISH: Fuorescence in situ Hybridization o con array-CGH: array based-comparative genomic Hybridization), del sangue periferico dei pazienti seguiti dall’anno 2000, con eventuale estensione ai genitori o altri familiari. Per lo studio morfologico il materiale proveniente dalla sede della malformazione, è stato valutato al microscopio ottico e sottoposto alla colorazione classica Ematossilina-Eosina, con lo scopo di valutare: il tipo di rivestimento, presenza di erosione e/o di ulcerazione, edema della lamina propria, l’architettura strutturale della tonaca muscolare, la presenza della trama connettivale e della sua disposizione. Per lo studio immunoistochimico sono stati utilizzati di alcuni marcatori dei processi di maturazione e differenziazione muscolare (Miogenina), dei marcatori di staminalità (CD34, CD133,) e delle possibili vie patogenetiche coinvolte (Shh). RISULTATI Sono stati individuati nel periodo 1991-2011, 143 pazienti affetti da MAR suddivisi in forme isolate 56, forme associate 82, con 6 forme sindromiche , 4 associazioni cliniche; 5 casi non sono classificabili. Nel maschio sono 50 le forme associate e 32 le forme isolate; nella femmina sono 32 le forme associate e 24 le forme isolate. Le forme sindromiche riscontrate sono: 2 sindromi cromosomiche, 4 sindromi genetiche. Non è stata identificata nessuna forma familiare Le osservazioni più interessanti dello studio istologico riguardano la tonaca muscolare valutata sia nella sua componente circolare interna che longitudinale esterna: nella maggior parte dei difetti si osserva un importante disarrangiamento delle fibre muscolari con rilevante dissociazione da parte della componente fibrosa. L’espressione di CD133 è stata valutata nei diversi livelli della parete intestinale; vi è un’intensa espressione di tale marker ma in misura minore rispetto il CD34 che presenta un’intensa positività in tutti gli strati, soprattutto nella lamina propria direttamente al di sotto dell’epitelio. La miogenina è costantemente negativa. Per quanto riguarda Shh i livelli di intensità del segnale è notevolmente più basso nella tonaca muscolare. CONCLUSIONI I risultati di questo lavoro indicano, in accordo con la letteratura, che il 60% dei pazienti con MAR ha un difetto associato. La fistola rettovescicale nel 80% dei casi si associa ad altre malformazioni seguita da: cloaca (78%), fistola rettouretrale (71%), fistola rettovestibolare (65%), fistola perineale (49%). Dalla ridotta espressione di Shh a livello del tessuto mesodermico del difetto si può ipottizare che o, ad un alterazione che colpisce direttamente Sonic o i suoi mediatori a valle, porti all’assenza di un ottimale gradiente parietale necessario per un corretto modellamento degli strati parietali intestinali, e quindi alle alterazioni in parte osservate in questo studio preliminare. Questa down-regulation può fornire una supporto molecolare all’ipotesi che l’incompleta formazione e divisione della cloaca determina un’ampia varietà di malformazioni dell’intestino posteriore. Per confermare tale ipotesi sono comunque necessari ulteriori studi sull’espressione dei mediatori a valle del segnale Shh quali per esempio Gli2, Bmp-4, Hox geni e le proteine Wnt.

Full textIn mammals, two functionally different type of adipose tissues are contained in a multi-depot organ: the adipose organ. It consists of several subcutaneous and visceral depots. Some areas of these depots are brown and correspond to brown adipose tissue, wich is specialized in energy expenditure, while many are white and correspond to white adipose tissue, the primary site of triglyceride storage. Cold acclimatisation induces an increase in the brown component without affecting the overall number of adipocytes; so far this form of plasticity was associated to obesity and diabetes resistance in experimental models. In this work we performed an anatomical study of adult C57BL/6J mice, which on HFD develop severe obesity, hyperglycemia and isulin resistance. The aim of this work was to check the percentage of white and brown adipocytes of all fat depots that make up the adipose organ of this strain at 28 ° C (control group) and 6 ° C. Was also monitored the thermogenic status of ML adipocytes through the expression of UCP1 protein and carried out a systematic study on the number of tyrosine hydroxylase parenchymal (noradrenergic)-positive nerve fibers (no. of fibers per 100 adipocytes). The cellular composition of all depots was mixed, with both cellular populations represented, there are not completely pure. This shows the existence of an adipose organ also in C57BL/6J. In the control animals the prevalent cell type were withe adipocytes (77% of all adipocytes), conversely in the cold-acclimated mice there was a prevalence of brown adipocytes (60% of all adipocytes); of these the 79% were immunoreactive for UCP1 antibody. After cold acclimation the total number of adipocytes in the adipose organ was unchanged, the number of brown adipocytes increased significantly (+37% p = 0.011) and the number of white adipocytes decreased by approximately the same amount (-41% p= 0.05). No evidence of apoptosis of white adipocytes was detected. The most prominent differences in cell composition (adipocyte plasticity) were found in the anterior subcutaneous (ASC) and in the abdomino- pelvic depots (AP). In a study conducted in 2005 on strain considered resistant to dietary induced obesity (SV129), was observed that the adipose organ contained a greater amount of ML in both conditions (28 ° C and 6 ° C) and the most changed depots were posterior subcutaneous (PSC) and the mesenteric (MES). In the control group the thermogenic protein UCP1 was expressed only in 34% of ML adipocyes, but their number increase significantly after cold exposure (79% p=0,02). The proportion of brown adipocytes UCP1+ is positively correlated with noradrenergic fiber density especially in the cold-acclimated mice, suggesting a crucial role of nervous system on the changes of the adipose organ after cold exposure. In many depots of C57BL (both 28°C and 6°C),especially after cold acclimation were observed Paucilocular cells (the “transdifferentiation marker”). In In this strain, like in SV129 the adipose organ shrank in volume; this reduction corresponded to the volume reduction of each brown and white adipocyte. Overall, these modifications of the adipose organ suggest a process of transdifferentiation of white into brown adipocytes in cold-acclimated mice confirming what was observed in the SV129 mice. In conclusion we can affirm that the phenotypic plasticity of the whole adipose organ is independent of genetic background, that could affect a greater change in a depot rather than in another .

Full textBackground: Preoperative radiotherapy with or without chemotherapy has been demonstrated of value in reducing local recurrence rates and improving overall survival in rectal cancer. Radiation-induced changes in the tumor are well described, whereas less attention has been given to the non-neoplastic mucosa. Our aim is to provide a detailed analysis of morphological, immunohistochemical and genetic features present in non-neoplastic mucosa. Pathologists need to be familiar with aforementioned morphological features, when evaluating rectal cancer specimens of patients preoperatively treated with radiotherapy, especially with short-course regimen, in order to avoid misdiagnosis. Methods and Results: We compared 2 groups of 95 rectal cancer patients treated preoperatively with either short-course (25 Gy administered in 5 consecutive days, followed by surgery a few days after; 45 patients) or long-course radiotherapy (45-50 Gy in 4-6 weeks, followed by surgery 4 weeks later; 50 patients). Depending on the type of protocol, different histopathological features, in terms of inflammation, glandular abnormalities and endocrine differentiation were seen in the non-neoplastic mucosa within the irradiated volume. Of note, features mimicking dysplasia, such as crypt distortion, nuclear and cytoplasmic atypia of glandular epithelium, were identified only in the short-course group. DNA mutation analysis, using a panel of 56 genes frequently mutated in colorectal cancer, and p53 immunostaining were performed on both neoplastic and radiation-damaged non-neoplastic mucosa in a subset of short-course cases. Somatic mutations were identified only in neoplastic mucosa, supporting the concept that tissues with radiation-induced “dysplastic-like” features are not genetically transformed. Conclusions: Pathologists should be aware of the characteristic morphological changes induced by radiation. The presence of features simulating dysplasia in the group treated with short-course radiotherapy may lead to serious diagnostic mistakes, if erroneously interpreted. NGS analysis further validated the morphological concept that radiation-induced abnormalities do not represent pre-neoplastic lesions.

Full textIn recent years there has been a wider spread of so-called new pets, among them is the rabbit. This led to an increase in the request for veterinary examination and the need to implement specialist knowledge. This leads to the requirement of knowing the specific anatomical, physiological and pathological characteristics of the species, significantly different from the commonly treated domestic animals. It is in this context that the research object of this thesis is framed, in which the aim was to investigate - from a morphological and morphometric point of view - the rabbit gastrointestinal system, using two techniques which are only apparently distant from each other: the microscopic anatomy and the “in-vivo” and “ex-vivo” ultrasound. The microscopic investigation allowed for the accurate evaluation of the normal wall characteristics of each section taken into consideration, measuring the thicknesses of the four tunics (serosa, muscularis, submucosa and mucosa) and obtaining the absolute and relative percentages values. This data served as reference values to evaluate the correctness of the same observations and measurements obtained with the ultrasound examination and to facilitate their interpretation. The study showed that the ultrasound exam proved to be reliable for the measurement of the total and partial thickness of the submucosa and the muscularis, while for the remaining tunics, serosa and mucosa, the value was not consistent with the anatomical data. This, presumably, is related to the limited resolution of the probe and to the phenomena of attenuation, diffusion and axial resolution. Overall, it is hoped that both the anatomical and high-resolution ultrasonography research carried out in the present study on the gastrointestinal apparatus of healthy subjects, represent the basis for a better understanding of the anatomopathology findings and of the ultrasound images in the presence of gastrointestinal lesions, with consequent important therapeutic and prognostic implications.
