Abstract:
Abstract FOXN1 deficiency is a primary immunodeficiency characterized by athymia, alopecia totalis, and nail dystrophy. Two infants with FOXN1 deficiency were transplanted with cultured postnatal thymus tissue. Subject 1 presented with disseminated Bacillus Calmette-Guérin infection and oligoclonal T cells with no naive markers. Subject 2 had respiratory failure, human herpes virus 6 infection, cytopenias, and no circulating T cells. The subjects were given thymus transplants at 14 and 9 months of life, respectively. Subject 1 received immunosuppression before and for 10 months after transplantation. With follow up of 4.9 and 2.9 years, subjects 1 and 2 are well without infectious complications. The pretransplantation mycobacterial disease in subject 1 and cytopenias in subject 2 resolved. Subject 2 developed autoimmune thyroid disease 1.6 years after transplantation. Both subjects developed functional immunity. Subjects 1 and 2 have 1053/mm3 and 1232/mm3 CD3+ cells, 647/mm3 and 868/mm3 CD4+ T cells, 213/mm3 and 425/mm3 naive CD4+ T cells, and 10 200 and 5700 T-cell receptor rearrangement excision circles per 100 000 CD3+ cells, respectively. They have normal CD4 T-cell receptor β variable repertoires. Both subjects developed antigen-specific proliferative responses and have discon-tinued immunoglobulin replacement. In summary, thymus transplantation led to T-cell reconstitution and function in these FOXN1 deficient infants.

Abstract Background: DOCK8 deficiency- a combined immunodeficiency characterized by recurrent sinopulmonary infections, severe cutaneous DNA viral infections, eczema, food and drug allergies, virally driven malignancies and vasculopathy- results from recessive mutations in Dedicator of Cytokinesis 8 (DOCK8). We have previously reported our experience with allogeneic hematopoietic stem cell transplant (HSCT) for DOCK8 deficiency using matched donors. We now report our recent experience with haploidentical transplantation for this disease. Methods: Six patients with DOCK8 deficiency (median age 19.5 yrs, range 6-25 yrs) received a related donor, haploidentical HSCT. Co-morbitidies, in addition to active infections, included: 1 subject with prior history of Burkitt's lymphoma, cardiomyopathy and renal artery stenosis; 2 subjects with prior histories of stroke and critical basilar artery stenosis; 1 subject with a prior history of Hodgkin lymphoma and bleomycin pulmonary toxicity; and 1 subject with end-stage liver disease from Cryptosporidium who underwent living donor liver transplant from the haploidentical HSCT donor 2 months prior to HSCT. All donors underwent bone marrow harvest for collection of stem cells. Recipients of haploidentical related donor HSCT were conditioned with cyclophosphamide 14.5 mg/kg on days -6 and -5, fludarabine 30 mg/m2/day on days -6 to -2, busulfan 3.2 mg/kg/day on days -4 and -3 (pharmacokinetically targeted to attain an AUC of 4000), and 200 cGy TBI on day -1. Graft-versus host disease (GVHD) prophylaxis consisted of post-transplant cyclophosphamide 50 mg/kg/day on days + 3 and +4 followed by tacrolimus from day +5 to day +180, and mycophenolate mofetil from day +5 to day +35. Results: The median follow-up was 7 mo (range, 1-13 mo). Five patients engrafted with a mean time of 15.8 days (range, 13 to 18 days). One patient who had received a liver transplant 2 months before HSCT had primary graft failure and received a CD34+ selected stem cell boost. All 5 patients who engrafted had > 90% donor chimerism by day 30. All five of these patients have had reversal of the disease phenotype, which correlated with lymphocyte reconstitution. No subject developed steroid-refractory GVHD-2 subjects had no GVHD, 1 had grade 1 skin GVHD only and was treated with topical steroids; 2 developed grade 2 GVHD with skin/GI GVHD and were steroid responsive. Conclusions: There was minimal regimen-related toxicity, and no incidence of steroid-refractory GVHD despite complete donor chimerism. With genetic testing for DOCK8 deficiency becoming more widely available, we anticipate that earlier diagnosis will enable patients to be transplanted earlier in their clinical course, before significant organ damage or the development of viral-driven malignancies, and that the outcome will continue to improve. Despite extensive co-morbidities in this patient population, haploidentical HSCT was a feasible transplant strategy in patients without matched donors and resulted in reconstitution of the deficient lymphocyte compartments leading to complete reversal of the infection susceptibility phenotype in most patients. SummaryTo clarify the molecular basis of the deficiency of glycoprotein IV (GPIV) of the platelet surface, we analyzed GPIV cDNA synthesized from platelet RNA of five unrelated Japanese subjects whose platelets did not express GPIV.We confirmed the presence of normal-sized GPIV mRNA in platelets from subjects with GPIV deficiency. The sequence of platelet GPIV cDNA from GPIV deficient subject showed three differences when compared with the published sequence; 1) a replacement of a 478CCT codon for proline-90 by TCT for serine, 2) a four-base insertion in the 3′-noncoding region, and 3) a substitution of A for 79C in the 5′-noncoding region. The replacement of Pro90 by Ser predominates in subjects with GPIV deficiency; that is, four out of five platelets with GPIV deficiency contained GPIV mRNA encoding GPIVSer-90, while all platelets from 17 GPIV positive subjects had GPIV mRNA encoding GPIVPro-90. The sequence of platelet GPIV cDNA which did not encode GPIVSer-90 from a subject with GPIV deficiency revealed no abnormality in the coding region. The four-base insertion in the 3′-noncoding region and the substitution of A for 79C in the 5′-noncoding region seems to be unrelated to the expression of GPIV.The substitution of Ser for Pro90 might alter the GPIV structure or impair GPIV biosynthesis, resulting in a lack of detectable GPIV. This hypothesis remains to be tested.

SummaryIn order to assess the clinical implications of hereditary F XII deficietrcy, all available members of Swiss families with F XII deficiency were investigated. Based on the F XII: C values and the family pedigree, the 74 subjects, aged 8-–82 years, were classified as homozygotes/double heterozygotes for F XII deficiency (n = 18), as obligatory (n = 20) or possibly (n = 25) heterozygotes, respectively, and as norrnals (n = 11). None of the 18 subjects with F XII: C <0.01 U/ml and only one possibly heterozygous woman had an abnormal bleeding tendency, confirming the notion that Hageman trait generally does not result in a hemorrhagic diathesis. Two of the 18 subjects with severe F XII deficiency had suffered from venous thromboembolic disease at age <40 years. One heterozygous woman had a leg ulcer probably due to venous thrombosis. Thus, whereas homozygous F XII deficiency may be associated with an increased risk for venous thromboembolic disease, partial F XII deficiency is not, by itself, a strong risk factor for thrombosis.Where as 17 of the 18 subjects with F xII : C <0.01 U/ml had no detectable F XIL Ag, one cross reacting material-positive F XII deficient subject (F XII:Ag = 0.11 U/ml) was identified. The dysfunctional F XII, present in this subject's plasma and tentatively called F XII Bern, is the fourth abnormal F XII molecule identified so far.

Abstract The subject of the deficiency judgments has been poorly examined due to a lack of relevant data and the complexity of the issue. Some comprehensive studies have explored whether allowing deficiency judgments decreases the likelihood of strategic defaults in the U. S. mortgage market. Little, however, has been done to determine whether there is any direct correlation between legal standing allowing recourse and loan pricing. Hence, additional work regarding this subject is needed. This study seeks to fill this gap by exploring the impact of allowing deficiency judgments on mortgage pricing policy in various U. S. states. Seven distinctive mortgage types in two groups of states were compared. We conclude that there is no statistically significant difference between recourse and non-recourse states in terms of mortgage pricing, regardless of mortgage type.

Abstract Iron has been recognized as a potent hematinic since the inception of hematology as a clinical discipline. Even ancient civilizations believed in the beneficial effects of medicinal iron. Nevertheless, the precise functional liabilities of iron lack remain the subject of continuing debate. The consequences of iron deficiency, particularly from a socioeconomic standpoint, are especially important in light of its high global prevalence. A recent review of the literature indicates that approximately 30% of the estimated world population of nearly 4.5 billion are anemic, and at least half of these, 500 to 600 million people, are believed to have iron deficiency anemia.

As theoretical basis, we use the Discourse Analysis Theory at the articulation of the contribution from Michel Pêcheux (1997c, 2006, 2007) and Michel Foucault (2006b, 2007a, 2010a, 2010b, 2010d). We emphasized the body as discursive production that goes in the event order and allows the construction of willingness of truth in different statements, which are affected by knowledge/power relations. We understand that discursive practices permit to discuss the emergence of objects that are in constant transformation, and concerned to the production of meaning effects at the discourse historic network. According to that, the inclusion discourse comes in the order of sayings, over the spread of different materialness that circulate in the social context, conferring to the body with disability a place at the everyday discursive practices. To instigate the emergence of that discourse, we realized a research emphasizing the power relations and the history of the body with disability. Therefore, we do the analysis of the linguistic-imagetic materialness, which presents the construction of meanings linked to disciplinary techniques and to the biopower effects, that take in evidence the power upon life, promoting displacement of the disabled person. Through the thematic line of labour market and education, the analyses permit to recognize the construction of ways of subjectivity, connected to the idea of happy and productive body, promoting, then, a pedagogy of smile that builds the subject with deficiency. Thereby, the discourse of social inclusion is considered as a possibility of marking the difference that has being governed by different instances of power. The disciplinary techniques and the biopower procedures work to the discourse legitimation through insistent strategies of governamentality that help to understand who is the subject with deficiency constructed at the statements from the Sentidos magazine. Furthermore, there is a handbook effect that goes through the statement, promoting a recipe of happiness about the subjectivity ways which teach how to live better and happier.<br>Esta Tese objetiva analisar o discurso da inclusão social do sujeito com deficiência que circula na revista Sentidos. Como respaldo teórico, usamos a Análise do Discurso, na articulação entre as contribuições de Michel Pêcheux e Michel Foucault, com destaque para o corpo como produção discursiva que entra na ordem dos acontecimentos e possibilita construir vontades de verdade em diferentes enunciados, afetado por relações de poder/saber. Entendemos que as práticas discursivas oportunizam discutir a emergência de objetos em constante transformação, relacionados à produção dos efeitos de sentidos na rede histórica dos discursos. Nesse foco, o discurso da inclusão entra na ordem das dizibilidades, através da proliferação de diferentes materialidades que circulam socialmente, conferindo ao corpo com deficiência um espaço nas discursividades do cotidiano. Para apontar a emergência desse discurso, realizamos um percurso genealógico, enfatizando as relações de poder e a história do corpo com deficiência. Assim, realizamos a análise da materialidade linguística, a qual apresenta a construção dos sentidos ligados às técnicas disciplinares e aos efeitos do biopoder, que enfatizam o poder sobre a vida, promovendo movências sobre o ser deficiente. Por meio do trajeto temático mercado de trabalho e educação, as análises possibilitam perceber a construção dos modos de subjetivação, ligado a ideia de corpo feliz e produtivo, promovendo, assim, uma pedagogia do sorriso que edifica o sujeito com deficiência. Desse modo, o discurso da inclusão social é visto como uma possibilidade de marcação da diferença sendo governada por diferentes instâncias de poder. As técnicas disciplinares e os procedimentos de biopoder agem na legitimação do discurso por meio de insistentes estratégias de governamentalidade que ajudam a entender quem é o sujeito com deficiência construído nos enunciados da revista Sentidos. Além disso, há o efeito manual que perpassa os enunciados, promovendo a receita da felicidade sobre os modos de subjetivação que dão instrução de como viver melhor e mais feliz.

Denna studie har granskat nedanstående frågeställningar, där punkt 2 varit i huvudfokus. 1. I vilken omfattning anser gymnasielärare att förkunskaperna i bråkräkning brister hos nyblivna gymnasieelever och vilka konsekvenser anser lärarna att sådana brister får när det gäller elevernas möjligheter att tillgodogöra sig gymnasiets matematikundervisning? 2. Vilka strategier använder gymnasielärare för att hantera bristande förkunskaper i bråkräkning och på vilka grunder vilar dessa strategier? Bakgrunden till studien var en uppfattning att förkunskapsbristerna i bråkräkning hos nyblivna gymnasieelever är utbredda och att gymnasielärare på något sätt behöver finna sätt att förhålla sig till dessa brister. Tidigare forskning indikerade stora brister i förkunskaper. Det teoretiska ramverket utgick bland annat från den didaktiska ämnesanalysen, framförallt såsom presenterad av Löwing (2016). Datainsamling gjordes genom semistrukturerade intervjuer med fem lärare på två skolor. Materialet bearbetades och analyserades med hjälp av tematisk analys. Lärarna i studien gav sammantaget en bild av att det finns ett icke oansenligt problem med bristande förkunskaper i bråkräkning, i synnerhet om man beaktar de många områden inom gymnasiematematiken där bråkräkning behövs för att adekvat tillgodogöra sig innehållet. En grund för att hantera bristande förkunskaper i bråkräkning, var att synliggöra elevernas förkunskapsnivå, på individuell basis såväl som för klassen. Ett huvudinstrument för synliggörande var de inledande diagnosprov som de undersökta skolorna använder sig av. De strategier som används för att möta förkunskapsbristerna kunde sorteras i två teman, Repetition och Eleven. Den dominerande repetitionen görs i inledningen av gymnasiet. Det som dock särskilt utmärkte de lärare i undersökningen som hade väl utvecklade ämnesdidaktiska strategier var dels att de hade en motsvarande utvecklad analys av problembilden, dels att de aktivt arbetade med konceptuell förståelse och integrering av procedurkunskap, snarare än att fokusera på renodlad procedur repetition. Det gick att dra paralleller mellan lärarnas ämnesdidaktiska strategier och den didaktiska ämnesanalysen i teoriramen, även om lärarna i denna studie inte arbetade på samma systematiska och detaljerade nivå. Under temat Eleven arbetades aktivt med att utveckla elevernas inställning, självförtroende och motivation när det gäller matematik, även här grundat i en aktiv problemanalys. De som läser natur- och teknikprogram, det vill säga matematikkurser på c-nivå, har typiskt inte denna utmaning tillnärmelsevis i samma utsträckning, som de som läser till exempel hantverksprogram eller estetprogram.<br>This study has researched two issues, where the second has been the primary focus. 1. To what extent do upper secondary school teachers view new students’ prior knowledge of fractions as deficient and what consequences do the teachers consider that such deficiencies have regarding students' abilities to assimilate the ensuing mathematics teachings? 2. What strategies do upper secondary school teachers use to manage deficiencies in prior knowledge in fractions and on what grounds do these strategies rest? The background to the study was a perception that the deficiencies in prior knowledge in fractions of upper secondary school students is substantial and that teachers somehow need to find ways to manage these deficiencies. Previous research indicated major deficiencies in prior knowledge. The theoretical framework was based primarily on the Educational Subject Matter Analysis, as presented by Löwing (2016). Data collection was done through semi-structuredinterviews with five teachers at two schools. The material was processed and analyzed using thematic analysis. Overall, the teachers in the study gave the impression that there is a substantial problem with prior knowledge deficiency in fractions, especially considering the many areas in upper secondary mathematics where fractions are needed to adequately assimilate the teachings. One basis for dealing with prior knowledge deficiencies, was to assess the students' level of prior knowledge, on an individual basis as well as for the class. A main assessment instrument was the initial diagnostic tests used by the surveyed schools. The strategies used to meet the prior knowledge deficiencies was sorted into two themes, Repetition and The Student. The dominant repetition is done in the beginning of the school year. However, what particularly distinguished the teachers in the survey who used well-developed didactic strategies was that they had a correspondingly developed problem analysis, and also that they actively worked with conceptual understanding, rather than pure procedure repetition. Parallels could be drawn between the teachers' didactic strategies and the Educational Subject Matter Analysis, although the teachers in the study did not apply it systematically. The Student theme primarily involved active work to develop the students' attitude, self-confidencea and motivation with regards to mathematics. Those who study science and technology programs, i.e. advanced level mathematics courses, typically do not have this challenge to the same extent as those who study for example craft aesthetics programs.

TNF-α, a pro-inflammatory cytokine, is implicated in the immune response in chronic obstructive pulmonary disease (COPD) secondary to Alpha-One Antitrypsin Deficiency (A1ATD). This thesis firstly describes studies in monocytes from A1ATD-related COPD subjects, examining the effect of the rs361525 TNF-A single nucleotide polymorphism, previously associated with 100-fold greater TNF-α concentration in the sputum of affected patients. Secondly, the autocrine effects of TNF-α on monocytes from healthy subjects are considered, in particular the differential roles of its two receptors, TNF-α receptor 1 (TNFR1) and 2 (TNFR2), an important topic given recent interest in selective TNFR1 blockade in TNF-α associated diseases. Unexpectedly, TNF-α mRNA expression and secreted protein was not greater in A1ATD-related COPD subjects with the rs361525 polymorphism when compared to matched wild-type subjects. Reasons may include the cell type and stimulus used or inadequate power. In monocytes from healthy subjects, autocrine binding of TNF-α increased production of pro-and anti-inflammatory cytokines. Trends were observed for TNFR1 blockade to reduce both types of cytokine, for IL-10 to be reduced by TNFR2 blockade and for TNFR1 expression at the monocyte surface to be up-regulated by TNF-α-TNFR2 binding. Further studies are required to fully characterise the relative roles of TNFR1 and TNFR2 in monocytes.

Este estudo interroga os efeitos de sentido no discurso sobre o sujeito deficiente produzido pela legislação educacional brasileira e enunciado nas Constituições e nas Leis de diretrizes da educação. O dispositivo teórico se filia à Análise de Discurso. A Análise de Discurso trabalha com o lingüístico e com o ideológico no processo de produção do sentido e constituição do sujeito. Visa compreender o funcionamento do discurso, a produção de sentidos entre locutores, bem como interrogar os sentidos estabilizados e sua relação com a língua. O sujeito da Análise de Discurso é o resultante de um processo de assujeitamento pela língua e pela história, e não o sujeito corpóreo. A análise do funcionamento da língua, enquanto materialidade dos discursos legislativos, possibilita compreender os efeitos de sentido constituídos sobre o sujeito deficiente. Ao dispositivo teórico da Análise de Discurso agrego noções de construção multicultural da igualdade e da diferença e de hibridismo cultural e entre-lugar. O corpus discursivo retido para análise revela que o discurso sobre o sujeito deficiente produzido pela legislação é da ordem da organização do social e produz um efeito de sentido que limita o funcionamento do discurso da ordem do simbólico. Para o discurso da legislação, a incompletude e a falta são significadas como falha, imperfeição e não como possibilidade, como desejo. A produção, ampliação e renovação da legislação sobre o sujeito deficiente parecem constituir uma forma de vigilância, uma estratégia de afastamento, um mecanismo de proteção da sociedade frente à insegurança do imaginário hostil da deficiência. O excesso da legislação preenche o espaço da indeterminação necessária para emergir a subjetividade, porque o processo de tornar-se sujeito implica a idéia de um possível, de uma falta. As diferentes formas de designar o sujeito deficiente, na perspectiva discursiva, vão além de uma retórica ingênua, pois as palavras constroem uma memória que remete a sentidos que as marcou em outros momentos da relação língua-ideologia. Essas designações podem revelar que, para além dos fatores biológicos, são os processos ideológicos, sociais, econômicos que constituem e atualizam os sentidos e o sujeito deficiente.<br>This study inquires the effects of senses in the speech about deficient subject produced by the Brazilian educational laws e enunciated in the Constitutions and in the directive Laws of the education. The theoretical device joins the Speech Analysis. The Speech Analysis works with the linguistics and with the ideological in the production process of sense and constitution of the subject. It seeks to understand the function of the speech, the production of senses between speakers, as well as to question the stabilized senses and its relation with the language. The Speech Analysis subject is a result of a process of becoming the subject through the language and through the history, and not the corporeal subject. The analysis of the language functioning, while being materiality of the legislative speeches, allows the comprehension of the sense effects constituted over the deficient subject. To the theoretical device of the Speech Analysis I add notions of multi-cultural construction of the equality and of the difference and of the cultural hybridism and between-place. The discursive corpus retained to be analyzed reveals that the speech about de deficient subject made by the legislation is from the order of the organization of the social and it products an effect of sense that limits the function of the speech from the order of the symbolical. To the legislation speech, the unfinished and the fault are meant to be flaws, imperfections and not as possibility, as desire. The production, amplification and renovation of the legislation about the deficient subject seem to constitute a mode of vigilance, a strategy to repel, a society’s mechanism of protection against the insecurity of the hostile imaginary of the deficiency. The legislation excess fills up the space of the indetermination necessary to emerge the subjectivity, because the process of becoming subject implicates an idea of a possible, of a lack. The different ways of appointing the deficient subject, in the discursive perspective, go on beyond a naive rhetoric, because the words build a memory that addresses to senses that have marked it in other moments of the relation ideology-language. These designations may reveal that, to beyond the biological factors, are the biological, social and economical process that constitute and update the senses and the deficient subject.

In the Penguin Classics edition of Jane Austen’s Emma, the editor of the volume, Fiona Stafford, writes: “From its first appearance, late in December 1815, Emma has been criticized for its lack of action. . . .” These are harsh words for a novel, suggesting deficiency in story. Is Emma then a failed novel? Yet the novel is published in a series called Penguin Classics. In other words, Emma belongs to the Western literary canon. That hardly spells “failure.” It appears that Emma presents a kind of informal paradox or dilemma for the philosophy of art. Emma, although it is agreed on all hands to belong to the canon, far from telling a whopping good story, is agreed on all hands to be sadly deficient in the storytelling department. It is this dilemma or paradox that is the subject of this chapter.

This chapter explores the overlap and potential synergies of integrative medicine and preventive medicine in the context of levels of prevention, acknowledging the relative deficiency of research on the effectiveness of practice-based integrative care. The goal of integrative medicine should be to make the widest array of appropriate options available to patients, ultimately blurring the boundaries between conventional care and CAM. Both disciplines should be subject to rigorous scientific inquiry so that interventions that work are systematically distinguished from those that do not. The case is made that responsible use of science and responsiveness to the needs of patients that persist when the data from randomized controlled trials have been exhausted can be reconciled. Integrative medicine is a framework for this reconciliation and, practiced judiciously, offers the promise of better patient outcomes.

One obvious shortcoming of the lex Julia (the subject of chapter 4) was that it applied only to senators and not to the equestrian members of a governor’s cohort, who might be equally guilty of extortion in the provinces. This chapter explores Pompey’s unsuccessful attempt in 55 to rectify that deficiency by extending the law to equestrian officials. The attempt was unpopular, but had the vigorous support of a few senators who should be identified as Cato and friends. In fact, Cato seems to have revived the attempt by back-door means when, as praetor in 54, he facilitated the trial of the equestrian financier Gaius Rabirius Postumus on grounds that had more to do with extortion proper—and larger concerns about Roman dealings with Egypt—than with the recovery provision under which Rabirius was charged. The chapter includes a detailed discussion dating the trials of Gabinius and Rabirius to 54.

AbstractThis paper focuses on finger force magnitude analysis during stiffness discrimination task. In the frame of their Study and research work MS students from the Université Grenoble Alpes specially designed an experimental bench allowing to simulate a pseudo-haptic spring. Then, a series of stiffness discrimination tests between reals springs and a pseudo-haptic spring were performed. Finger pressing forces and students’ (subjects’) perception of spring stiffness were recorded and analyzed. The analysis of psychometric curves indicates that subjects underestimate the simulated stiffness of the pseudo-haptic spring. The results also indicate that the peak of finger force applied on pseudo-haptic spring increases as the simulated stiffness increases. Moreover, it was found that the relationships between the logarithm of stiffness and the finger force were linear for the real springs and the pseudo-haptic spring. Pseudo-haptics effect being provided by specially designed isometric force feedback device, the results of this study may be useful for computer-based rehabilitation tasks designed for motor disorder patients with muscle deficiency associated with limited joint movement range or for injured athletes in the process of rehabilitation.

In the US, increasing passenger speeds to improve trip time usually involves increasing speeds through curves. Increasing speeds through curves will increase the lateral force exerted on track during curving, thus requiring more intensive track maintenance to maintain safety. These issues and other performance requirements including ride quality and vehicle stability, can be addressed through careful truck design. Existing high-speed rail equipment, and in particular their bogies, are better suited to track conditions in Europe or Japan, in which premium tracks with little curvature are dedicated for high-speed service. The Federal Railroad Administration has been conducting parametric simulation studies that examine the performance of rail vehicles at high speeds (greater than 90 mph) and at high cant deficiency (greater than 5 inches). The purpose of these analyses is to evaluate the performance of representative vehicle designs subject to different combinations of track geometry variations, such as short warp and alinement.

Activated protein C functions as an anticoagulant by enzymatically degrading factors Va and Villa in the clotting cascade. Protein C may be converted to its enzymatically active form bythrombin. The rate at which thrombin cleavage of the zymogen occurs is greatly enhanced when thrombin is bound to an endothelial cell receptor protein, thrombomodulin. Activated proteinC has a relatively long half-life in vivo and the formation of activated protein C in response to low level thrombin infusion suggests that the protein C system may provide a feedback mechanism to limit blood clotting. Clinical support for such a physiologic role for activated protein C includes an increased incidence of thrombophlebitis and pulmonary emboli in heterozygous deficient individuals, and severe, often fatal, cutaneous thrombosis in homozygous deficient newborns. A third thrombotic condition associated with protein C deficiency is coumarin induced skin (tissue) necrosis. This localized skin necrosis occurs shortly after the initiation of coumarin therapy and is hypothesized to bedue to the rapid disappearance of protein C activity in the plasma beforean adequate intensity of anticoagulation is achieved. Recent estimates of heterozygous protein C deficiency range as high as 1 in 300 individuals in the general population. Since coumarin compounds are in routine clinical use throughout the world and skin necrosis remains a relatively rare clinical finding, this suggests that factors other than protein C deficiency alone may be involved in the pathogenesis of the skin necrosis.The anticoagulant properties of activated protein C are greatly enhanced by another vitamin K-dependent plasma protein, protein S. Protein S functions by increasing the affinity of activated protein C for cell surfaces.Protein S is found in two forms in plasma: free and in complex with C4b-binding protein, "an inhibitor of the complement system. Free protein S is functionally active and the complexed protein S is not active. Individuals congenitally deficient in protein S ae subject to recurrent thromboembolicevents. At least two classes of protin S deficiency occur.Some patienshavedecreased levels of protein S antigen and reduced protein S functional activity. A second group of deficient individuals have normal levels of protein S antigen but most or all their protein S is complexed to C4b-binding protein and they have little or no functional protein S activity. Such a protein S distribution could result from abnormal forms of protein S or C4b-binding protein or some other abnormal plasma or cellular component. Patients with functionally inactive forms of protein S have yet to be identified. Identification of protein S deficient individuals is complicated by thepossible effect of sex hormones on plasma protein S levels. Total protein S antigen is reduced during pregnancyand during oral contraceptive administration. This finding is of practicalclinical importance since the decrease in protein S which occurs during pregnancy may be an added risk factor for congenitally protein S deficient women and may explain why some proteinS deficient women experience their first episode of thrombosis during pregnancy.In addition to having anticoagulant properties, activated protein C enhances fibrinolysis, at least in part,by inhibiting the inhibitor of tissueplasminogen activator. This profibrinolytic effect is enhanced by protein S and cell surfaces. This protection of plasminogen activator activity suggests that the combination of tissue plasminogen activator and activated protein C may be useful in the treatment of coronary artery thrombi. Tissueplasminogen activator would promote clot lysis while activated protein C protected the plasminogen activatorfrom inhibition and also prevented further clot deposition. There is no evidence at present that fibrinolytic activity is reduced in protein C deficient individuals. The possible clinical relevance of this aspect of protein Cfunction in the predisposition of protein C deficient individuals to thrombosis remains to be defined.

Abstract:
Congenital protein C (PC) deficiency is associated with thrombophilia. Heterozygotes with about half-normal plasma PC levels may present with venous thromboembolic events usually beginning during adolescence or young adulthood. A 26-year-old swiss woman had experienced an iliofemoral deep-vein thrombosis without obvious etiology six years.ago. In June, 1986 very low levels of PC antigen (25 %) and activity (27 %) were found when she was six-month pregnant. Three other family members (62, 24 and 19-year-old) had PC levels around 50 % but were symptomfree. Because of the post-thrombotic~syndrome and the pregnancy, ambulatory heparin therapy was immediately started in the patient (10-16'000 IU twice daily, s.c.) in order to maintain a plasma heparin level between 0.2 and 0.5 U/ml six hs after the morning injection. Delivery was induced at full-term whilst heparin was stopped for a few hours. Four weeks after delivery anticoagulation was discontinued and, so far, the patient remained symptomfree. The newborn showed no perinatal problem and the PC antigen level assayed in the umbilical venous blood was 22 % (normal range in the literature 18-46 %). Antithrombin III and protein S levels as well as fibtinolytic potential were within normal values in all family members.Conclusions. 1) Only one out of four heterozygote PC deficient family members had presented with venous thromboembolism. 2) The symptomatic subject had the lowest PC level in the family (activity and antigen around 25 %). 3) This woman experienced an event-free pregnancy and delivery although heparin was started only in the sixth month of pregnancy. 4) Thus, penetrance of the thrombotic trait may be quite variable (and low) amongst PC deficient heterozygotes, an observation which raises the question of the indication of long-term anticoagulation in these individuals. 5) Pregnancy did not affect the PC antigen and activity levels in our patient.

Abstract:
During the past several years, we have initiated studies to determine the role of plasma factors and platelets, and the properties of the blood vessel, which influence the activation of the coagulation mechanism on the subendothelium. Studies were performed by exposing everted segments of de-endothelialized rabbit aorta, mounted in a perfusion chamber, to non-anticoagulated human blood for 5 to 10 minutes under a range of flow conditions, and measuring fibrin and platelet deposition on the subendothelium, and fibrinopepstide A (FPA) levels in post-chamber blood. In normal subjects, platelet deposition increased progressively with increasing shear rates (50-2600 sec-1 ), whereas fibrin deposition and FPA levels decreased sharply at shear rates greater than 650 sec-1 . To examine the role of plasma coagulation factors, we utilized a shear rate of 650 sec-1 to study patients with severe deficiencies of factors XII, XI, IX or VIII. In contrast to the partial thromboplastin time (PTT), which was most strikingly abnormal in patients with factor XII or XI deficiency, fibrin deposition and FPA levels were greater in patients deficient in factor XII or XI than in those with factor VIII or IX deficiency. In addition, we observed smaller platelet thrombi in hemophilia (but not afibrinogenemia), suggesting that thrombin influenced the formation of platelet thrombi under these shear conditions. The findings suggested that tissue factor-Vila activation of factor IX could be important in mediating fibrin deposition on subendothelium and might explain why patients with factor XII deficiency (and some with factor XI deficiency) do not bleed. Initial studies to demonstrate tissue factor activity in subendothelium were inconclusive. More recently, utilizing shorter (1.5, 2 and 3 min) perfusion periods, we have observed decreased fibrin deposition and FPA levels in patients with factor VII deficiency and we have obtained further support for the presence of tissue factor in subendothelium in experiments utilizing a monoclonal antibody to tissue factor. Our studies suggest that activation of factor IX by tissue factor-Vila could account for the results obtained in patients with plasma coagulation defects. Direct experimental verification of this hypothesis will require more extensive studies on the kinetics governing the activation of coagulatjon factors on the subendothelium. In subsequent studies, we examined the role of platelets in mediating fibrin deposition. At a shear rate of 650 sec-1 we found (utilizing patients with thrombocytopenia) that platelets were required for fibrin deposition ; little or no fibrin was deposited on the subendothelium when platelet adhesion was less than 4%, corresponding to blood platelet counts less than 5000/ul. Studies performed in patients with functional platelet disorders provided additional information on the specific platelet properties that contribute to fibrin deposition at this shear rate. Decreased fibrin deposition was observed in a patient with Scott Syndrome, a disorder characterized by an impaired capacity of the platelets to catalyze the conversion of factor X to factor Xa (in the presence of factor IXa and VIII) and prothrombin to thrombin (in the presence of factor Va), the latter defect owing to a decreased factor Xa-binding capacity of the platelets. In contrast to the findings in Scott Syndrome, both fibrin deposition and FPA values were completely normal (and possibly increased) in patients with glycoprotein Ilb/IIIa deficiency. In patients with glycoprotein lb deficiency, the major defect was an impaired association of fibrin with platelets, but not subendothelium. The findings in patients with functional platelet disorders indicate that a monolayer of platelets (including those deficient in glycoprotein Ilb/IIIa) is completely active in promoting fibrin deposition on subendothelium. In addition, they suggest that an agent capable of inducing a platelet defect similar to that observed in Scott Syndrome might prevent platelet-fibrin thrombi at shear rates (200-800 sec-1 ) comparable to those in the coronary circulation. Studies performed at a variety of shear rates in both normal subject^ and patients with platelet disorders suggested that, under the conditions used, platelets were essential for fibrin formation at intermediate (650 sec-1 ), but not low (50 sec-1 ) shear rates. Since platelets have been shown to bind activated coagulation proteins (such as factor Xa, Va, and IXa) to their surface, the presence of adherent platelets on the subendothelium could, with increasing shear rates, serve to maintain activated coagulation proteins in the .boundary layer at a concentration that would otherwise be reduced through convective diffusion in their absence. Thus, at low shear rates (50 sec-1 ), the concentration of activated coagulation factors in the boundary layer might be sufficient to support fibrin deposition despite the absence of platelets, whereas at very high shear rates (2,600 sec-1 and above), even the presence of platelets is insufficient to maintain the required concentration. The shear-dependent defect of fibrin formation that we observed in Scott Syndrome is consistent with such a theory. The results of our various studies demonstrate the complex role of blood flow, plasma coagulation factors, specific platelet properties, and the procoagulant properties (tissue factor) of the vessel in mediating subendothelium-induced coagulation and suggest further experiments for studying the mechanisms involved.

Abstract:
A new personal device to assist the upper limb capable to be mounted on a wheelchair is being tested. The robot is equipped with three brushless motors powered by four electronic boards appositely designed and communicating via I2C protocol; one board works as master for the other three, which have simpler tasks. Most of the driving software has been developed with Matlab and mainly translated into C++ for memory space and boards’ efficiency matters. The system’s end-effector is connected to the subject’s forearm and can cooperate to the arm motion in several different ways. In order to avoid the overstress of the natural joints no further connections are made to the upper limb. The working volume of the limb connected to the device allows the execution of the trajectories required for most of daily living activities. It is addressed to post stroke rehabilitation and to the self-treatment of other patients with serious deficiency of arm forces, like individuals affected by muscular dystrophy. Some working modes exploit the mechanical impedance control to gently interfere with the residual natural motion capability of the subjects. The very preliminary tests of the prototype fitted on a power wheelchair are encouraging: it is light, not too noisy and easy to move for the subject. The first working mode, with full arm gravity compensation, and the second working mode, with partial compensation, have been implemented and are currently being tested with patients, as well as the mode in which the subject is helped to repeat a stereotyped exercise for self-physiotherapy. Beside the fact the control system must be calibrated on patient characteristics, for these initial tasks the system reveals to be user-friendly. Other working modes require to interpret the patient intention to move the upper limb. For a natural approach it is sufficient to decode the movement intention of the patient and the final position he wants the hand to reach, whilst the rest of the limb can be automatically positioned by the system. Different approaches to solve the problem have been experimentally tested, including the use of a headgear with a brain interface. To present knowledge the best results have been obtained by monitoring the movement of another body segment such as the head. The device designed for the functional recovery of upper limb can furthermore be used to monitor and easily certify the evolution of the patient conditions.

Heat transfer to the storage fluid is a critical subject in thermal energy storage systems. The storage fluids that are proposed for supercritical thermal storage system are organic fluids that have poor thermal conductivity; therefore, pure conduction will not be an efficient heat transfer mechanism for the system. The current study concerns a supercritical thermal energy storage system consisting of horizontal tubes filled with a supercritical fluid. The results of this study show that the heat transfer to the supercritical fluid is highly dominated by natural convection. The buoyancy-driven flow inside the storage tubes dominates the flow field and enhances the heat transfer dramatically. Depending on the diameter of the storage tube, the buoyancy-driven flow may be laminar or turbulent. The natural convection has a significant effect on reducing the charge time compared to pure conduction. It was concluded that although the thermal conductivity of the organic supercritical fluids are relatively low, the effective laminar or turbulent natural convection compensates for this deficiency and enables the supercritical thermal storage to charge effectively.

Increasing the level of the competition, a worldwide trend in the evolution of electricity markets, has made game theory a notably popular approach to find the market equilibrium. This paper models a retail electricity market with a high penetration of renewable resources. Using game theory, the clearing electricity prices, as well as the optimum behavior of market participants are obtained. In this model, which is inspired by the “Energy Internet” concept, consumers play an active role in managing their load demands. This highly dynamic model allows us to analyze consumers’ reaction to price fluctuations. Spot pricing, which is employed here to model the electricity market, can make consumers react to the high electricity prices. This is particularly important in the demand side management, where consumers should modify their demand through financial incentives. Two types of active players are considered in this electricity market, small electricity suppliers and consumers. Electricity grid, while present in the market, only takes the responsibility to compensate for the deficiency of power from small and mid-size suppliers. The problem is formulated mathematically, subject to a number of local and global constraints to find the Nash equilibrium.

Abstract In this paper, we invite the engineering design research community to examine the current state of the engineering design lexicon. We expose the nature and the pervasiveness of practices that often hinder intelligible discourse within the engineering design literature. In particular, we show how such commonly used terms as criterion and metric are used sometimes as synonyms and sometimes not, often leading to material miscommunications. In our view, the engineering design discipline has reached a point in its evolution where clarity and conciseness of its lexicon should be a priority. Today’s design activity takes place in a truly multidisciplinary environment, which often involves engineers of diverse backgrounds. Written and oral design discourse among design researchers does not rely on a generally accepted and documented lexicon. These situations are symptomatic of a communication infrastructure that is not effectively facilitating the vigorous evolution of the engineering design discipline of recent years. In addition to detailing the outlines of the design lexicon deficiency, we also propose some avenues to a constructive and productive community-wide discussion on this subject. Abstract:
The analysis of theories and models of radicalization existing in psychology and sociology is given. The complexity and transitivity of the world, the emerging methodological trends in psychology, the change of postmodern discourse to metamodernism require new psychological approaches to a research of this phenomenon, which can take into account the role of cultural factors and anthropological turn, as well as space and time as ontological constants of reality. Theoretical: theoretical and methodological analysis of scientific literature, comparison, generalization, interpretation. The paper summarizes a number of empirical studies of the authors related to the problems of extremism. The goal is to consider the evolution of ideas about the radicalization of protest activity and substantiate the high relevance of the subject-being approach to explain the problem of extremism. The authors distinguish six main theories and models that reveal the nature of radicalization: the theory of anomy (R. Merton), the theory of "relative deprivation" (T. Garr), the concept of an authoritarian personality (A. Adorno), a model of social identity in collective activity (M. Van Zomeren ), the model of radicalization (R. Borum), the model of radicalization (F. Mohaddam). The authors note the demand for a metamodern methodological strategy, which makes it possible to record personal transformations and construct new images of a person. The authors come to the conclusion that the substantive differences in approaches lie in the influence quantity of external determinants causing the emergence of radical attitudes. In the development of the theme of extremism, the main ones are the principle of the unity of the personality and its being, the methodological principle of subjectivity and the principle of uncertainty, which reveal additional nuances of the phenomenon that increases cognitive capabilities. The conditions of the subject-being approach are considered and the concept of protest activity is presented, based on the notions of "existential personal identity", "subjective activity", "subjectivity", revealing the reasons for negative transformation of personality, considering extremism as a violation of the developing configuration of identity. The subject-being approach to the personality is recognized as the most efficient theoretical and methodological basis for researching this problem, since it allows us to overcome the deficiency of the content given by the deterministic interpretation of radicalization.
