Between the Middle Ages and the end of the eighteenth century, taxation in Europe was very different from that of modern society. It was characterised by the predominance of indirect taxes over direct, with the latter being substantially irregular. The studies published in these proceedings of Study Week promoted by the Fondazione Datini examine the comparative evolution of the European fiscal systems and fiscal policies and the effects of taxation on the economy. The wealth of specific and original contributions, as well as the attempts at structural organisation, reveal the distinct vivacity of European and western historiography, offering an overall and articulated picture of the European tax systems in the pre-industrial age.

This chapter explores the basic ethical principle of autonomy and the related principles of veracity, fidelity, and avoidance of killing. The pharmacist’s role in recognizing and respecting the individual patient’s moral interests is discussed. The chapter addresses the psychological and moral meanings of the principle of autonomy and delineates the elements of a substantially autonomous decision. Circumstances are discussed when it would be morally justifiable to override the actions of a substantially autonomous person. Cases highlight the issues that arise in determining whether a person is substantially autonomous for health care purposes, those posed by external constraints on autonomy, and those that arise in consideration of overriding patient autonomy.

Consideration of costs and budgets plays an increasingly important role in decisions on access to innovative technologies. When clinicians want to influence such decisions, it is essential to understand the information on the burden of the disease and the evidence on cost-effectiveness of technologies. This chapter provides guidance to understanding the key methodological principles of economic evaluations, and describes available evidence on these issues in axial spondyloarthritis (axSpA). In the prebiologics era, the cost-of-illness for society of ankylosing spondylitis was slightly lower than for rheumatoid arthritis, and substantially lower than chronic low back pain. Cost of sick leave and work disability accounted for up to 75% of total cost-of-illness. Treatment with biologics increased cost-of-illness substantially, but the important gain in quality-adjusted life years resulted in acceptable cost-effectiveness in patients with active disease. There remains a gap in knowledge about the cost-effectiveness of diagnosing and treating axSpA earlier.

Management of Alport syndrome has in the past been expectant and supportive. Modern hearing aids have substantially improved the function of affected individuals. However, animal data and more recently observational data from Alport registries strongly suggest a protective effect of angiotensin-converting enzyme inhibitors. There is a suggestion that early commencement of treatment may slow progression substantially. These should now be recommended for all with proteinuria, and possibly even before then for those known to harbour mutations certain to cause end-stage renal failure. A very small minority develop the difficult post-transplant complication of Alport anti-glomerular basement membrane disease. This can rarely be treated successfully and leaves some patients on long-term dialysis. However, overall, patients with Alport syndrome have better than average survival and other outcomes than other patients with end-stage renal failure. Most are successfully transplanted. The question of risk to heterozygous carriers from donating kidneys to their affected relatives arises frequently. The risks may be felt acceptable in some circumstances. Additional therapies are under investigation.

Chapter 53 concerns express conditions. The term express condition refers to an explicit contractual provision under which either: (1) A party to a contract does not come under a duty to perform unless and until a designated state of affairs occurs or fails to occur, or (2) If a designated state of affairs occurs or fails to occur a party’s duty to perform a contract is suspended or terminated. The normal sanction for breach of a promise is damages. In contrast, the normal sanction for nonfulfillment of an express condition is suspension or termination of the contract by the party in whose favor the condition runs. If a promise is substantially performed, then even though the promise is not perfectly performed the promisor normally can sue for expectation damages, with an offset for damages resulting from the breach. In contrast, if an express condition is not perfectly fulfilled then even though the condition is substantially fulfilled the traditional rule is that the party in whose favor the condition runs has the right to terminate the contract.

Abstract:
The article gives an overview on the concept of individualism in cognitive science. Individualism maintains that optimal human reasoning is substantially asocial, and therefore implies that sociality does not facilitate, and may impede, reasoning. The cognitive science of morality very frequently proceeds with individualist assumptions. The individualist may allow that normal development requires sociality, but deny that optimal reasoning in mature individuals requires it. The optimal cognitive functioning is both developed and sustained through sociality. The optimal exercise of rationality is a socially embedded process. It means that sociality is not just a precondition of rationality, but that even among those with normal cognitive functioning, the optimal exercise of rationality typically occurs as part of a social process. The sociality has a significant role in substantial cognitive achievement, such as scientific and technological discovery. A large body of research indicates that motivation plays a crucial role in reasoning. The optimal human reasoning is substantially asocial, and sociality is necessary for the development of optimal reasoning. The sociality is necessary for the sustenance of optimal reasoning, and for the transmission of information. One important feature of group interactions is that they are likely to induce emotional responses. Many familiar emotions such as anger, guilt, and sympathy are characteristically triggered by cues in social interaction.

Chapter 5 provides new insights into exactly how the vicious wartime struggle between pro- and antislavery partisans in Missouri drew impetus from antagonistic northern and southern evangelical worldviews infused by religious belief and sentiment. Conflict between northern and southern evangelicals over the question of slavery contributed substantially to the pro- and antislavery guerrilla depredations that disrupted congregations and church activities through the bloody conflict. This chapter also demonstrates that northern and southern evangelicals invoked Divine Providence variously to justify their respective causes and to settle the daunting existential question of which worldview was true.
