In 1903, the Vatican’s secretary of state, Cardinal Mariano Rampolla was heir apparent to the papacy. But he became unexpectedly out of a job due to a veto by the Austro-Hungarian emperor. Rampolla retreated to a life of scholarship to edit a manuscript he had discovered in a monastic library decades earlier, Gerontius’s Life of Melania the Younger. Rampolla’s publication spawned a series of early twentieth-century reactions to Melania’s biography. This work sometimes appeared in scholarly journals. Much appeared in popular press articles such as one that compared Melania’s fasts to those of early twentieth-century suffragettes’ or a series of newspaper headlines referring to “the richest woman in the history of the world!” The hagiography also inspired early twentieth-century imitations, such as Sainte Mélanie, a pious and expanded version of Melania’s Life. This chapter examines an earlier rediscovery of this “indomitable little saint” who was seen as “Richer than Rockefeller.”

This chapter proposes a novel method, based on the multivariate parametric statistical tests of hypotheses, which classifies the normal skin lesion images and the various stages of the melanoma images. The melanoma images are categorized into two classes, such as initial stage and advanced stage, based on the degree of aggressiveness of the cancer. The region of interest is identified and segmented from the input skin melanoma image. The features, such as HSV color, shape, and texture, are extracted from the region of interest. The features are treated as a feature space, which is assumed to be a multivariate normal random field. The proposed statistical tests are employed to identify and classify the melanoma images. The proposed method yields an average correct classification up to 91.55% for the normal skin lesion versus the initial and the advanced stages of the melanoma images, up to 91.39% for initial stage melanoma versus the normal skin lesion and the advanced stages melanoma, and up to 92.27% for the advanced stage melanoma versus the normal skin lesion and the initial stage melanoma. The proposed method yields better results.

Melanoma is a severe form of skin cancer characterized by the rapid multiplication of pigment-producing cells. A problem on analysis of these images is interesting because of the existence of artifacts that produces noise such as hair, veins, water residue, illuminations, and light reflections. An important step in the diagnosis of melanoma is the removal and reduction of these artifacts that can inhibit the examination to accurately segment the skin lesion from the surrounding skin area. A simple method for artifacts removal for extracting skin lesion is implemented based on image enhancement and morphological operators. This is used for training together with some augmentation techniques on images for melanoma detection. The experimental results show that artifact removal and lesion segmentation in skin lesion images performed a true detection rate of 95.37% for melanoma skin lesion segmentation, and as high as 92.5% accuracy for melanoma detection using both GoogLeNet and Resnet50.

Normal ethnic skin colouration is due to melanin in the skin. However, in this short chapter abnormal blue, grey, or black changes in skin colour, both localized or generalized, are discussed and illustrated by a few clinical images. The Mongolian blue spot that occurs as a physiological change in the skin of the majority of Asian, Black, and Chinese infants is due to melanin. It is uncommon in Caucasian skin types and not always recognized. A good illustration of the relatively common blue naevus is provided and much rarer examples discussed such as naevus of Ota, naevus of Ito, and alkaptonuria. In addition to melanin, other chromophores deposited in the skin may appear blue or black, such as haemoglobin or rarely drug deposits. The popularity of tattoos and associated problems are also mentioned.

A variety of tumours, both benign and malignant, are found in skin. Benign skin lesions, such as seborrhoeic keratoses and skin tags, are often just a cosmetic nuisance, but some benign skin lesions can be a component of diseases with serious medical consequences (e.g. neurofibromatosis or LEOPARD syndrome). Skin cancer is the most common human cancer and its incidence continues to increase. It most commonly affects older, fair-skinned individuals who have had either acute intermittent exposure to ultraviolet light or chronic ultraviolet light exposure. Organ transplant recipients have a 200-fold increased risk of squamous cell carcinoma. About 2% of patients who develop skin cancer have a genetic predisposition, for example, Gorlin’s syndrome in basal cell carcinoma and familial melanoma syndromes in malignant melanoma. Mutations in the PTCH gene cause Gorlin’s syndrome, and loss of heterozygosity at that locus is also present in most sporadic basal cell carcinoma.
