This study seeks to explore narratives of care-related violations for patients with life-threatening illness receiving palliative care. Narratives told in dialogues with the researcher were processed phenomenologically hermeneutically. Four structures of meanings are described: focal points in recalling the experiences, experienced consequences of being violated, relationships causing violation, and personal struggling. The phenomenon of care-related violations means a complex experience of suffering as being abandoned, confronted with hopelessness, and further wounded. This experience may be directed toward readiness to share, introspectiveness, willingness to comprehend the incomprehensible, the riskiness of facing others, and attentiveness to acts of caring. It can be experienced in various relationships to professionals, family members and friends, to the mass media, and to welfare systems. Care-related violating episodes reveal the vulnerability of the person who is already suffering and makes him or her still more wounded, when actually comfort is expected. To receive affirmation in the state of fragility with increased suffering provoked by care-related violations can contribute to a transformation from human degradation into dignity, finding meaning, or reaching reconciliation in suffering.

Abstract Introduction: Suffering is a universal multifaceted phenomenon hard to define and often connected to pain. Suffering is not a homogeneous concept. Instead, suffering is a diffused term that includes innumerable ways of dealing with depression, pain, loss, and adversity. Suffering is described as a response or behavior recognized by introspection or observation of the behavior of the person and his/her environment. Objective: The purpose of this article is to describe the role of the advanced nurse practitioners when dealing with the construct of suffering and to present a review of literature related to the human suffering experience. Methods: The CINAHL Complete, PubMed, One Search, MEDLINE, PsychInfo, ProQuest Dissertation and Theses Global databases were search using the keywords suffering, experiences, meanings and perceptions of suffering. Search limits included peer-reviewed articles published in the English language from 1980 to 2016. Results: A total of nine qualitative and five quantitative studies were reviewed. Three major themes were identified: the dimensions of suffering (physical, psychological, social, existential and ethical), enduring suffering and measuring human suffering, and perceiving another’s suffering by using reliable and valid instruments.

Examining the contestation of interpretations around this work, I argue that the proliferation of exegetical material on Sophocles’s Antigone is related to a noncomprehension of the human motives behind her transgressive action. Did she ever love, and is there any suffering in her piety? If she didn’t love (her brother), could she have suffered? I read the play alongside Kamila Shamsie’s postcolonial rewriting of it in Home Fire to elaborate on the relationship between personal loss and collective (and communal) suffering, particularly as it is focalized in the novel by the figure of a young woman who is both a bereaved twin and a vengeful fury.

Abstract. Although suffering is a central issue in pain, there is only little research on this topic. The aim of this study was to assess suffering in an experimental context using various stimulation methods and durations, and to examine which psychological or psychophysiological measures covary with pain-related suffering. Twenty-one healthy volunteers participated in two experiments in which we used tonic thermal and phasic electric stimuli with short and long stimulus durations. The participants rated pain intensity, unpleasantness, and pain-related suffering on separate visual analog scales (VAS) and completed the Pictorial Representation of Illness and Self Measure (PRISM), originally developed to assess suffering in chronic illness. We measured heart rate, skin conductance responses (SCRs), and the electromyogram (EMG) of the musculus corrugator supercilii. For both heat and electric pain, we obtained high ratings on the suffering scale confirming that suffering can be evoked in experimental pain conditions. Whereas pain intensity and unpleasantness were highly correlated, both scales were less highly related to suffering, indicating that suffering is distinct from pain intensity and unpleasantness. Higher suffering ratings were associated with more pronounced fear of pain and increased private self-consciousness. Pain-related suffering was also related to high resting heart rate, increased SCR, and decreased EMG during painful stimulation. These results offer an approach to the assessment of suffering in an experimental setting using thermal and electric pain stimulation and shed light on its psychological and psychophysiological correlates.

Bakgrund: Vårdskador är vanligt förekommande i både svensk och utländsk sjukvård. Överlag bidrar vårdskadorna med flera dygns förlängda vårdtider och upptar ett stort antal vårdplatser. Detta upptar resurser och kostar samhället många miljarder kronor varje år. Hälso- och sjukvårdspersonal ansvarar för patientsäkerheten i vården, vilket i Sverige är reglerat i lagar och föreskrifter. Dessa finns till för att skydda patienter mot vårdskador och innebär en skyldighet för hälso- och sjukvården att ständigt jobba för att utveckla kvalitet och säkerhet i vården. Syfte: Syftet med litteraturöversikten var att beskriva patienters upplevelser av att drabbas av vårdskada. Metod: En litteraturöversikt enligt Friberg har genomförts. Sökning efter resultatartiklar till översikten gjordes i CINAHL Complete och PubMed. Elva originalartiklar hittades, vilka utgör underlag för resultatet i översikten. Resultat: Med stöd i Katie Erikssons vårdteori identifierades tre huvudkategorier av upplevelser, vilka var vårdlidande, sjukdomslidande och livslidande. Varje kategori hade tillhörande underkategorier där patienternas upplevelser beskrivs. Diskussion: Huvudfynd i resultatet har diskuterats utifrån bakgrund, annan forskning och Katie Erikssons vårdteori. Kommunikation och information från vårdpersonalen efter en vårdskada upplevs av patienter bristfällig. I de fall då kommunikation och information upplevs tillfredställande minskar också vårdlidandet. Patienters upplevelser visar att vårdpersonal inte följer sitt ansvar och befintliga lagar, vilket tycks utgöra ett problem som hämmar kvalitet och säkerhet i vården. I kontrast till de negativa upplevelserna förekommer också positiva upplevelser vilka relateras till god vård innan och efter vårdskadan.
Background: Adverse events are common in Swedish and foreign healthcare. These events overall contribute to several days of prolonged hospital stay and occupies a large number of hospital beds. It occupies resources and costs the society billions of Swedish crowns (SEK) every year. Healthcare professionals are responsible for the patients’ safety in the healthcare, which in Sweden is regulated by laws and regulations. These are designed to protect patients against adverse events and injuries in the healthcare and entail obligations for healthcare professionals to constantly work to develop the quality and safety for the patients in the healthcare.  Aim: The aim of this literature review was to describe patients´ experiences of being affected by an adverse event. Method: A literature review has been performed according to Friberg’s method. The databases CINAHL Complete and PubMed were used for searching articles for this literature review. Eleven original articles were found, which are used to form the results of this review. Results: With support in Katie Eriksson´s theory of caring, three main categories of patients experiences were found, these categories were suffering related to healthcare, suffering from illness, suffering of life. Each category had related subcategories where patients' experiences are described. Discussion: Main findings has been discussed on the basis of the background, other research and Katie Eriksson’s theory of caring. A lack of communication and information from the healthcare professionals after an adverse event was experienced. In cases when the communication and information perceived satisfactorily the suffer were reduced. Patients’ experiences show that healthcare professionals do not follow their responsibilities and the existing laws, which inhibits the quality and safety of the healthcare. In contrast to the negative experiences there were also positive experiences, which are related to good care before and after the adverse event.

Abstract:
Humanism is a worldview emphasizing interpersonal warmth, emotional expression, imagination, and humanity’s inherent goodness. Normativism is a worldview emphasizing discipline, emotional restraint, empiricism, and humanity’s inherent badness. We reasoned that worldviews could influence how people construe suffering. Prior research suggests there are two prominent suffering construals in contemporary U.S. culture. A Redemptive construal posits that suffering’s purpose is to lead to greater awareness and self-development, while a Biomedical construal sees biological frameworks as the standard for understanding suffering. In two studies, we examined how these worldviews and construals shape how people interpret suffering arising from depression. In Study 1, we predicted correlations between Humanism and a Redemptive construal, and Normativism and a Biomedical construal of depression. Results confirmed the hypothesis. In Study 2, we hypothesized that priming participants with Humanism or Normativism would increase endorsement of Redemptive and Biomedical construals, respectively. We discovered that exposure to Humanism (compared to Normativism) induced more support for a Redemptive construal, and less for a Biomedical construal, of depression. However, Normativism exposure did not result in an increased preference for a Biomedical, compared to a Redemptive, construal. We propose that the worldviews people encounter and endorse can shape their interpretations of depression.

It is increasingly argued that social and economic inequities poorly affect overall health. One of the means through which these inequities are translated to the body is via negative emotions, which carry known psychological and physiological responses. This paper examines migration-related psychosocial stressors impacting first-generation Mexican immigrants in southern Arizona, and reports on the primary emotional experiences immigrants associate with these stressors. Data were drawn from a qualitative, ethnographic study conducted over the course of 14 months during 2013–2014 with first-generation Mexican immigrants (N = 40) residing in Tucson Arizona and service providers working directly in the immigrant community (N = 32). Results indicate that the primary structural vulnerabilities that cause emotional hardship among immigrants are pre-migration stressors and adversity, dangerous border crossings, detention and deportation, undocumented citizenship status, family separation, and extreme poverty. Many of these factors have intensified over the past decade due to increased border security and state level anti-immigrant legislation in Arizona. Immigrants connected these hardships to the emotions of trauma (50%), fear (65%), depression (75%), loneliness (75%), sadness (80%), and stress (85%), and most respondents reported suffering from three or more of these emotions. Given the heavy emotional toll of migration and the direct impact that regional legislation and border security had on well-being, this paper argues that emotion be considered an important mechanism for health declines in the immigrant community. In order to stem the frequency and intensity of emotional stress in the Mexican immigrant community in Tucson, it is imperative to support organizations and policies that promote community building and support networks and also expand access to and availability of mental health services for immigrants regardless of documentation status.

This study investigates the role that the notions of trauma and culture play in relation to the alleviation of distress within an war-affected population. It analyses how local, cultural conceptions of distress and those held by psychological service providers relate to one another, and how they contribute to improving the well-being of the displaced. Fieldwork was conducted with urban and rural displaced populations in the south-eastern province of Huila in Angola. Ethnographic, psychometric and participatory methods were used to examine issues of health, illness and distress amongst the displaced. Local idioms of distress in the form of pensamentos, mutima, madness and high and low blood pressure are common ways of expressing suffering related to war. The different explanatory models held about these illnesses and the various resources available in the popular, folk and professionals sectors of the health care system were explored. The religious and spiritual domains were found to be influential in the treatment of distress-related illnesses. The psychological services available in the war-displaced communities were examined in terms of their common theoretical and practical elements. These were then analysed in relation to the conceptualisations held by local populations, and points of similarity and difference were noted. Specifically, the conceptualisation of suffering as trauma and the cultural misunderstandings that arise as a result of this, and the representation of the displaced as traumatised and therefore dependent and passive people, are discussed. A particular subgroup in the community, the adolescents, was identified and participatory methods were employed to investigate the strategies and resources this group uses for coping with war-related distress. The youths predominantly make use of distraction, conselho, religious and cultural resources. The application of a PTSD scale, the EARAT, suggests that 71% of the adolescents had symptoms of trauma consistent with a diagnosis of PTSD. It is argued that for the vast majority such a conceptualisation does not reflect the adolescents' abilities to function on social, vocational, educational and physical levels. The implications of these findings for research and practice in the field of psychosocial work are discussed.

Adult day care rehabilitation centers represent options for the elderly population that long-term care professionals have long been seeking. Chapter One represents the statement of the problem, historical overview of adult day care/rehabilitation centers in general and in Georgia, conceptual framework and theoretical grounding and research method. Chapter Two discusses the problem in contextual review, services, access, quality in adult day care and rehabililitation, the federal government and adult day care programs. Further discussions focus on centers and their relationship of variables from the instrument in the participation of persons suffering from Alzheimer's diseasae and related disorders in Georgia. This chapter seeks also to utilize secondary data on previous studies relating to adult day care/rehabilitation centers to supplement the study. Primary emphasis was to determine the extent to which those adult day care/rehabilitation centers studied provide for Alzheimer's disease and related disorders patients through their services. Chapter three will cover presentation of data and discussion in the area of analyzing and examining the centers having Alzheimer's disease and related disorders patients. Presentation of data and interpretation of findings, sampling frame characteristics, key variables and utility and limitation of the study will all be included in this chapter. Included also, will be a comparison of participation of the Alzheimer's disease and related disorders patients between adult day care and rehabilitation centers, a breakdown of adult day care, adult day rehabilitation and combination centers, and the relationship between adult day care and selected variables from the instrument. Finally, in Chapter Four, a summary of findings, conclusions and study recommendations if need be as to how the adult day care and rehabilitation centers can or should be operated in order to accommodate a greater number of Alzheimer's disease and related disorders cases in Georgia will be addressed.

A systematic review identified limited studies exploring oral formulation-related barriers to medicines administration in children. Owing to the lack of licensed medicines suitable for use in children, manipulation is often required to deliver a specific dose or to facilitate medicines administration. Little is known regarding the prevalence and nature of issues encountered when administering medicines to children in the domiciliary setting. This study adopted a pragmatic approach to determine the nature and prevalence of oral formulation-related barriers to medicines administration in children suffering from a range of chronic conditions. Problems reported by healthcare professionals, patients and their parents/carers were identified to inform future pharmaceutical development and improve children’s medicines. Focus groups (n=4) were conducted with healthcare professionals in the West Midlands and semi-structured face-to-face interviews (n=278) were conducted with parents/carers/young people at University Hospitals Coventry and Warwickshire. Questions explored barriers to medicines administration, refusal and manipulation. In total, 31% of interview respondents reported medicines refusal. Taste was the most commonly reported barrier to medicines administration. Almost one fifth (19%) of medicines administered to children were reported to require manipulation. Findings indicate that age-appropriate medicines are required to provide both suitable dose units and acceptable taste for paediatric patients.

Bakgrund: Alzheimers sjukdom (AS) kallas ofta för den anhöriges sjukdom med anledning av att sjukdomen även påverkar de närstående. Det är därför av vikt att sjuksköterskan arbetar utifrån ett helhetsperspektiv för att på så sätt få en förståelse för vilken inverkan AS har på familj och närstående. Syfte: Syftet med studien var att beskriva upplevelsen av hur livskvalitén påverkas av att vara närstående till en person med AS. Metod: En kvalitativ innehållsanalys, där självbiografiska verk har analyserats Resultat: Resultatet presenteras med tre kategorier ”En ny vardag”, ”Leva med ett lidande” och ”Ett tungt ansvar”. Varje kategori har två underkategorier vardera. . Det liv och den vardag som tidigare var känd för den närstående blev till ett minne. Nya utmaningar i vardagen och ett tungt ansvar väntade som resulterade att livskvalitén blev påverkad. Psykisk och fysisk ohälsa blev ett faktum. Slutsats: Det fanns flera faktorer till att livskvalitén blev försämrad. En faktor var att den närstående fick ta hela ansvaret för att vardagen skulle fungera i hemmet och spelade därför en viktig roll för personen med AS. Med anledning av detta måste sjuksköterskan kunna stödja den närstående så att denne orkar med sin vårdande roll och minimera risken för psykisk ohälsa.
Background: Alzheimer´s disease is often called the relative's illness on the grounds that the disease also affects the relatives. It is therefore important that the nurse working with a holistic approach and obtain an understanding of the impact of Alzheimer´s disease on family and relatives. Purpose: The purpose of this study is to describe the experience of how quality of life is affected by being related to a person with Alzheimer's disease. Method: This is an qualitative content analysis, where autobiographical works have been analyzed. Results: The results are presented in three categories: "A new day", "Living with suffering" and "A heavy responsibility." Each category has two subcategories. The life that was previously known to the relatives became a memory. New challenges in everyday life and a heavy responsibility that resulted in that the quality of life was affected. Mental and physical health became a fact. Conclusion: There were several factors that led to impaired quality of life. One factor was that the relatives had to take full responsibility for everyday life and played an important role for the person with Alzheimer´s disease. For this reason, the nurse must be able to support the relatives so that they can cope with their caring role and minimize the risk of mental illness.

Bakgrund: Självskadebeteende har tidigare uppfattats som ett tecken på allvarlig psykisk sjukdom och det ansågs enbart vara mycket förvirrade människor som skadade sig själva. Studier visar att självskadebeteende oftast är något människor begår i ensamhet och majoriteten väljer heller inte att uppsöka sjukvård. Kunskap om självskadebeteende och attityder till patienter som skadar sig kan bidra till en ökad förståelse hos sjuksköterskor för fenomenet.  Syfte: Syftet med denna litteraturstudie är att beskriva sjuksköterskors attityder till patienter med självskadebeteende. Metod: Uppsatsen är baserad på en litteraturstudie där nio artiklar analyserats med vägledning av Fribergs kriterier för analys av vetenskapliga studier. Resultat: Resultatet presenteras utifrån fyra huvudteman med 16 tillhörande subteman. De fyra huvudtemana är; sjuksköterskors föreställningar, sjuksköterskors känslor, sjuksköterskors värderingar samt sjuksköterskors handlingar. Resultatet visar att sjuksköterskors attityder till patienter med självskadebeteende är mångskiftande. Diskussion: Författarna skapade sex teman utifrån det i resultatet som kunde vara av intresse att lyfta fram. En diskussion kring dessa sex teman fördes sedan mot bakgrundsinformationen och Katie Erikssons teori om lidandet. Med hänsyn till vetskapen om att tankar och känslor inte alltid styr vårt handlande, valde författarna att inte dra slutsatsen att sjuksköterskor endera har positiva eller negativa attityder till patienter med självskadebeteende.

This book examines the art and ritual of flagellant confraternities in Italy from the fourteenth to the seventeenth centuries. Meeting regularly to beat themselves with whips, members of these confraternities concentrated on the suffering of Christ in the most extreme and committed way, and the images around them provided visual prompts of the Passion and the model suffering body. This study presents new findings related to a variety of artworks including altarpieces, banners, wall paintings, illuminated manuscripts, and paintings for the condemned, many from outside the Florence-Rome-Venice triangle.

Water was a source of wealth which facilitated, fostered or brutally halted economic development in the Ancien Regime. Lack of hygiene meant that water was used less for drinking than other drinks, but as a raw material, source of energy, cooling, rinsing and cleansing agent, water was unequalled. It played a role in public and private relaxation and in health. Water also proved to be an ideal, safe and cheap means of transporting goods and ideas. Urban historians have long pointed to the enormous comparative advantage enjoyed by towns and regions whose favourable maritime or riverine location gave them access to cheap water-borne transport. But water just as often posed a threat to economic development and prosperity, whether due to its absence or its specific composition or level of pollution or to uncontrollable abundance. This duality is still present today in our modern, globalised society. While huge quantities of fresh, potable water are wasted in the West, free or cheap access to fresh and abundant water supplies remains a major challenge for millions of individuals on the planet. Major floods in different parts of the world regularly cause economic damage and endless human suffering. With a Settimana devoted to the management of the water supply, excluding related topics as water consumption, water transport and the use of water in agriculture and industry, the Istituto Datini is seeking to draw attention.

This chapter aims to provide the psychiatrist and ethicist with an understanding of reformational Christian ethics as it pertains to the faith of the believing patient in a psychiatry context. We introduce three reformational principles and apply them cursorily to diagnostic and therapeutic issues in clinical practice, followed by a more detailed application to the topic of suffering owing to mental disorder. Understanding reformational ethics may aid the psychiatrist towards a better relationship with the believing patient and equip him for engagement on issues of guilt, remorse, whether being punished by God, and whether mental disorder results from failure in faith. Clarity on these issues may bring consolation to the believing patient. That applies also to the issue of suffering owing to mental disorder, for which we present a scholastic reformational exegesis of Colossians 1:24, exemplifying the premises and methods for examining issues of reformational Christian faith and mental disorder.

Migraine is one of the most common, disabling neurological disorders in women of reproductive age. It is a disabling condition that can lead to a compromised health-related quality of life. Often `migraineurs' are unable to carry out everyday tasks due to a reduction in functioning and productivity. This burden impacts not only at work, but also on social and family life. Migraine affects not only the physical but also mental and social health. Chapter 5 discusses the available evidence of a correlation between migraine and pre-eclampsia. Pre-eclampsia complicates 3-5% of all pregnancies and remains a leading cause of maternal and perinatal morbidity, and mortality. Although the primary mechanisms of both migraine and pre-eclampsia are not yet well understood, they do share some common pathogenetic aspects. However, studies investigating the clinical association between migraine and pre-eclampsia are sparse. The majority suggest a close association between headaches and hypertensive disorders of pregnancy: gestational hypertension, pre-eclampsia, and eclampsia. There also appears to be a significant increase in the incidence of severe pre-eclampsia in women suffering from migraine. Clinical manifestations of both migraine and pre-eclampsia appear to result from an interaction of the mind and body. A relation with stress has been identified. Further robust research is needed to elucidate the psychosomatic contributions to the pathogenesis of migraine and pre-eclampsia, and the clinical application of their relationship in improving materno-fetal health.

Abstract Veterinary examination of Icelandic competition horses, according to a specially designed protocol 'fit for competition', is performed prior to every entrance to the track during Landsmót, the National Championships of the Icelandic horse. This has provided valuable data for welfare assessment that have been used for improvement of general rules for the benefit of competition horses. It further prevents suffering on an individual level as horses expressing pain during the examination are found not to be fit for competition and are not allowed to start or continue competition during the event. The frequency of bit-related lesions can be regarded as a welfare indicator for Icelandic competition horses, reflecting the pressure placed on the mouth of the horse.

AbstractHeini Hakosalo makes use of an extensive collection of written, unpublished tuberculosis-related illness narratives to analyze the experience of tuberculosis and tuberculosis sanatoria “from below” within the context of twentieth-century Finland. Hakosalo argues that by linking their personal illness histories to national history, the authors could give a sense of purpose and meaning to their personal losses and suffering. At the same time, their testimonies stood as a contribution, however modest, to the national knowledge-community. She distinguishes three narrative strands that allowed the authors to assimilate their personal illness with the collective ills of the nation: histories of tuberculosis as stories of progress, stories of war, and stories of belonging.

Abstract:
101 patients suffering from acute myocardial infarction during less than 4 hours were immediately treated with intravenous (i.v.) streptokinase (SK), infused in 1 hour. No concomitant medications like steroids, salicylates or anti-arrhythmic drugs were routinely given.Patients were blindly allocated to one of four dosages of SK (see below). Coronary angiography was performed within 3 hours after SK infusion in 90% of all patients . The infarct-related vessel was open in 59% of 91 patients. The results in the four dosage groups were as follows:Haematomas at the puncture site were common complications in all groups. No strokes occurred, nor life-threatening bleeding complications. Blood transfusion was needed in only one parient, who had an important bleeding and formation of a large haematoma at the puncture site.It is concluded that there is a trend to better results of higher doses of i.v. SK in patients suffering from acute myocardial infarction without an evident rise of the rate of complications. The efficacy and safety of recently developed fibrinolytic drugs and streptokinase should be compared at optimal dosages.

Animation is an exceptionally good medium for educating and raising awareness of the issues that are more difficult to talk about, such as mental illness. The use of animation and graphics in society awareness and education can improve knowledge and communication about issues related to mental illness. Despite various destigmatization activities, people are still reluctant to directly approach individuals suffering from mental illness, partly because they do not have enough knowledge on the subject. Therefore, the aim of our study was to test whether animation can improve the knowledge about appropriate / inappropriate conversations with a person suffering from depression in a Slovenian student population. We focused on education and awareness-based animation (adapted to the target group) to evoke empathy without arousing pity and to indirectly encourage people in need and others to talk about it. The methodology included the surveying of the participants and their knowledge about appropriate and inappropriate conversations with a person suffering from depression (22 phrases, 11 appropriate and 11 inappropriate). 251 participants aged 19 to 30 completed the survey. Participants were asked to rate each phrase for appropriateness on a 5-point Likert scale before and after being exposed to the animation. The results obtained were analysed using the paired samples t-test. The desired change in knowledge was confirmed by comparing the average scores of the survey responses before and after viewing the animation. A statistically significant result was obtained in 18 of 22 sentences. A change in the desired direction was detected in 81% of the statements. Our study had some limitations, but we nevertheless conclude that an awareness-based animation can be a useful tool to increase knowledge and subsequently influence behavioural change in the student population.

There are, however, rare diagnostic methods for the functional gastrointestinal disorders because functional disorders show no evidence of organic and physical causes [1, 2]. Recently our research group identified that the gastrointestinal tract well in the patients with the functional gastrointestinal disorders is more rigid than healthy people. we noticed it with palpating the abdominal regions overlaying the gastrointestinal tract. Therefore we developed a system to detect the rigid organs with ultrasonic technique, which can quantify the characteristic above related to the rigidity of the gastrointestinal tract well.

Acute myocardial infarctions in the vast majority of cases are caused by coronary artery thrombosis at the site of complicated arteriosclerotic plaques. By several trials evidence has been given, that myocard can be preserved, when thrombolytic therapy is started within a short period after thrombotic coronary occlusion. Recently, angioscopic evidence has been given, that the unstable angina pectoris syndrome frequently is assciated with coronary artery thrombosis, too. Thus, thrombolytic therapy should be of comparable benefit for patients suffering from unstable angina pectoris syndrome. Up to now, we have treated four patients suffering from unstable angina pectoris syndrome (two with documented spontaneous reversible ST-segment elevations, two with newly complained recurrent nocturnal episodes of severe angina) with thrombolytic therapy (Pat. 1: 1.5 Mio IE Streptokinase; Pat. 2: 100 mg rt-PA; Pat. 3: 150 mg rt-PA; Pat. 4: 60 mg scu-PA plus 200 000 IE UK). After thrombolytic therapy, all four patients were free of symptoms for at least 60 h. Pat. 3 had recurrance of chest pain with spontaneous reversible ST-segment elevations on the third day after therapy. Pat. 1, 2, and 4 were without clinical symptoms until angiography and secondary intervention (angioplasty (PTCA) /bypass operation (CABG)). Cardiac catheterization was performed within one week after thrombolytic therapy. In all four patients, ischemia related coronary artery was patent at angiography. We conclude, that in unstable angina pectoris syndromes with newly developed nocturnal symptoms and/or spontaneous reversible ST-segment elevations in the ECG can be stabilized by thrombolytic therapy. After thrombolysis, however, recurrance of chest pain may be soon, and PTCA or CABG should be performed as soon as possible.

The aim of the paper is to analyse the situation and problems of traditional business in rural areas of Lithuania and Poland and propose development possibilities applying social initiatives and becoming social enterprises. The research object of the paper is traditional and social enterprises in rural areas. The research discloses that in Lithuania and Poland the situation in rural areas has a lot of similarities in terms of development and problems. Social entrepreneurship in both countries is kept as possibility but still problems and specificity exist in the countries.

In order to meet the increasing societal and market demand for a diverse and well-trained Biomedical Engineering (BME) workforce, the University of the District of Columbia (UDC), the nation’s only urban land-grant institution, the District of Columbia’s only public institution of higher education, and a historically black college and university (HBCU), nurtures BME activities focused on exposure, training and cultivation through research and experiential learning. Undergraduate design projects and research-based learning opportunities in BME are key program ingredients. This paper presents the former (i.e., three, BME-related undergraduate senior Capstone Design projects that target devices to aid patient immobility) namely, the design of: 1) an ankle foot orthosis, 2) an upperlimb robotic hand prosthetic, and 3) a chairless chair lower limb exoskeleton. A current focus of the UDC BME program is Rehabilitation Engineering (i.e., interventions and devices aimed at aiding those with mobility impairments). We briefly discuss the necessity for rehabilitation-focused, biomedical-related undergraduate experiences and training for underrepresented minority students at UDC, in particular, undergraduate engineering education through multidisciplinary BME projects that foster hands-on creativity towards innovative designs. In addition to critical design experiences and undergraduate training in BME, devices may have the potential to develop into new commercial technologies and/or research projects that will aid and enhance the quality life of individuals suffering from a wide-range of mobility-related issues.

Several factors contribute to Surinames particular vulnerability to the effects of climate change. It is dependent on fossil fuels, has forests liable to decay, fragile ecosystems, and its low-lying coastal area accounts for 87% of the population and most of the countrys economic activity. Many sectors are at risk of suffering losses and damage caused by gradual changes and extreme events related to climate change. For Suriname to develop sustainably, it should incorporate climate change and its effects into its decision-making process based on scientific- evidence. The State of the Climate Report analyzes Surinames historical climate (1990-2014) and provides climate projections for three time horizons (2020-2044, 2045-2069, 2070-2094) through two emissions scenarios (intermediate/ SSP2-4.5 and severe/ SSP5-8.5). The analysis focuses on changes in sea level, temperature, precipitation, relative humidity, and winds for the seven subnational locations of Paramaribo, Albina, Bigi Pan MUMA, Brokopondo, Kwamalasamutu, Tafelberg Natural Reserve, and Upper Tapanahony. The State of the Climate provides essential inputs for Suriname to develop and update its climate change policies and targets. These policies and targets should serve as enablers for an adequate mainstreaming of climate change adaptation and resilience enhancement into day-to-day government operations.

Abstract:
Suriname is highly vulnerable to the effects of climate change. Among the factors that exacerbate its vulnerability are its dependency on fossil fuels, the degradation of important ecosystems (e.g., mangroves), and the fact that 87% of the population, and most of the countrys economic activity is located within the low-lying coastal area. Many sectors are at risk of suffering losses and damage caused by gradual changes and extreme events related to climate change. For Suriname to develop sustainably, it should incorporate climate change and its effects into its decision-making process based on scientific- evidence. The State of the Climate Report analyzes Surinames historical climate (1990-2014) and provides climate projections for three time horizons (2020-2044, 2045-2069, 2070-2094) through two emissions scenarios (intermediate/ SSP2-4.5 and severe/ SSP5-8.5). The analysis focuses on changes in sea level, temperature, precipitation, relative humidity, and winds for the seven subnational locations of Paramaribo, Albina, Bigi Pan MUMA, Brokopondo, Kwamalasamutu, Tafelberg Natural Reserve, and Upper Tapanahony.
