Background: Habit cough is one of the causes of chronic cough in children. Many cases do not reach the psychiatrists due to lack of specific clinical features and diagnostic tests. We present a case of 13 year old boy with habit cough who was successfully treated by suggestion therapy and lignocaine based mouth gargle as a distracter. This case highlights that suggestion therapy can be an important treatment option to this fairly common condition.

Hasat çizelgeleme de bu karar verme ve optimizasyon problemlerinden biridir. Bu çalışmada, yılda ortalama üç kez hasat edilen çay bitkisi için bir hasat optimizasyonu gerçekleştirilmiştir. Çiftçilikle mevsimlik olarak ilgilenen insanların genellikle birincil meslekleri farklıdır. Hasat günlerinde çiftçiler bu birincil mesleklerini yerine getirememektedir. Bu nedenle, hasat çizelgesinin oluşturulması için çiftçilerin uygun gün tercihlerinin de dikkate alınması sürdürülebilir tarım adına önem taşımaktadır. Bu çalışmada fabrika ve alım yeri kapasitelerinin yanı sıra çiftçilerin uygun/uygun olmayan gün tercihlerini optimize etmek için hedef programlama modelleri geliştirilmiştir. Gerçekleştirilen vaka çalışması sahası şu özelliklere sahiptir: 12 alım yeri, 988 çiftçi ve 3392 dekar çay tarlası. Önerilen modelin performansının test edilmesi için çiftçi uygun/uygun olmayan günlerinin rassal olarak belirlendiği veri setleri üretilmiştir. Bu şekilde birbirinden farklı üretilen bin ayrı veri seti ile duyarlılık analizi yapılmıştır. Yapılan analizler önerilen modeller ile oluşturulan çizelgelerin çay hasat sürecindeki sürdürülebilirliği ve verimliliği artırdığını göstermiştir.

Abstract:
Aim of study. Considering the high spreading of tinnitus in population, the purpose of our research was to study the possibility and expediency of using hypnosuggestive therapy in treatment of tinnitus. Material and methods. The study included 55 people with tinnitus complaints. We divided three groups of patients depending on the severity of the manifestations, audiometry results, Tinnitus Handicap Inventory (THI) score according to this scale of severity, results of GAD-7 test and the PHQ-9 (Patient Health Questionnaire -9). Results. In the first group of patients, according to the tinnitus severity scale the score was up to 19 points and was considered as insignificant and mild. The score of the GAD-7 questionnaire was less than 8 points, PHQ-9 - up to 5 points. In the second group the severity of tinnitus was moderate and severe and counted more than 38 points. The GAD-7 questionnaire score was less than 8 points, PHQ-9 - up to 5 points. In the third group, 12 people complained of chronic tinnitus, sleep disturbances, nervousness, and reduced work capacity. The score of tinnitus severity in patients of this group was more than 58 points and was assessed as severe and catastrophic. According to the results of the GAD-7 and PHQ-9 questionnaires, all 12 people were diagnosed with anxiety disorders and depression, two parients even had suicidal thoughts. In the third group of patients with +++ hypnotability, after ten sessions of individual hypno-suggestive therapy, it was possible to reduce the severity of tinnitus to the third moderate degree in 7 people. 4 patients reported about complete regress of tinnitus. Conclusions. Tinnitus is a common problem and can negatively affect work, family and social life. It requires the individual approach and involvement of specialists of various profiles in the diagnostic process. The diagnostic algorithm for patients with tinnitus should include questionnaires to estimate the psychosomatic profile. The use of hypno-suggestive therapy can significantly reduce the severity of tinnitus and improve the quality of lives of patients.

Abstract:
We present the case of a patient with a first single episode of a dual drug-induced aseptic mening (DIAM) due to amoxicillin and ibuprofen and a short review of updated literature. A 76-year-old man was admitted to our hospital with slowness and confusion following a dental and gingival inflammation treated with oral amoxicillin 500 mg bid and ibuprofen 600 mg tid for 1 week. His mental state and higher functions abruptly worsened after therapy increase leading to hospitalization. Both the drugs were stopped and the patient improved rapidly within 2–3 days and was released asymptomatic after a week. On the basis of this temporal relationship with a comprehensive negative neuroimaging and laboratory testing for viral, bacterial, and mycobacterial micro-organisms, a DIAM by amoxicillin and ibuprofen was diagnosed. We support the hypothesis that this dual therapy was causative because of the progressive onset of central nervous system symptoms starting at a low amoxicillin dose with a high ibuprofen intake and that this sort of chemical meningoencephalitis was mostly due to the pharmacokinetic of amoxicillin after its dose increase. To our knowledge, this is the first documented publication of a severe first episode of DIAM with predominant higher function involvement caused by these two drugs commonly used together, amoxicillin and ibuprofen.

In the late nineteenth century, German-speaking physicians and psychiatrists intensely debated the benefits and risks of treatment by hypnotic suggestion. While practitioners of the method sought to provide convincing evidence for its therapeutic efficacy in many medical conditions, especially nervous disorders, critics pointed to dangerous side effects, including the triggering of hysterical attacks or deterioration of nervous symptoms. Other critics claimed that patients merely simulated hypnotic phenomena in order to appease their therapist. A widespread concern was the potential for abuses of hypnosis, either by giving criminal suggestions or in the form of sexual assaults on hypnotized patients. Official inquiries by the Prussian Minister for Religious, Educational and Medical Affairs in 1902 and 1906 indicated that relatively few doctors practised hypnotherapy, whereas the method was increasingly used by lay healers. Although the Ministry found no evidence for serious harm caused by hypnotic treatments, whether performed by doctors or by lay healers, many German doctors seem to have regarded hypnotic suggestion therapy as a problematic method and abstained from using it.

Background: A habitual cough, persisting after the cause is gone, was described in a 1694 medical book. Successful treatment of this disorder known as habit cough was reported in 1966 by the “art of suggestion”. The purpose of this article is to provide the current basis for diagnosis and treatment of the Habit Cough Syndrome. Method: The epidemiology and clinical course of habit cough were reviewed; original data were obtained from three sources. Results: Unique clinical presentation was the basis for diagnosis of habit cough. Diagnosis was made 140 times with increasing frequency over 20 years at the University of Iowa clinic and 55 times over 6 years at a London clinic. Suggestion therapy provided more frequent cessation of cough than just reassurance. A Mayo Clinic archive of chronic involuntary cough found 16 of 60 still coughing 5.9 years after initial evaluation. Ninety-one parents of children with habit cough and 20 adults reported cessation of coughing from viewing a publicly available video of successful suggestion therapy. Conclusions: Habit cough is recognizable from the clinical presentation. It is effectively treated in most children by suggestion therapy in clinics, by remote video conferencing, and by proxy from viewing a video of effective suggestion therapy.

The relationship between kidneys and anticoagulation is complex, especially after introduction of the direct oral anticoagulants (DOAC). It is recently growing evidence of an anticoagulant-related nephropathy (ARN), a form of acute kidney injury caused by excessive anticoagulation. The pathogenesis of kidney damage in this setting is multifactorial, and nowadays, there is no established treatment. We describe a case of ARN, admitted to our Nephrology Unit with a strong suspicion of ANCA-associated vasculitis due to gross haematuria and haemoptysis; the patient was being given dabigatran. Renal biopsy excluded ANCA-associated vasculitis and diagnosed a red blood cell cast nephropathy superimposed to an underlying IgA nephropathy. Several mechanisms are possibly responsible for kidney injury in ARN: tubular obstruction, cytotoxicity of heme-containing molecules and free iron, and activation of proinflammatory/profibrotic cytokines. Therefore, the patient was given a multilevel strategy of treatment. A combination of reversal of coagulopathy (i.e., withdrawal of dabigatran and infusion of its specific antidote) along with administration of fluids, sodium bicarbonate, steroids, and mannitol resulted in conservative management of AKI and fast recovery of renal function. This observation could suggest a prospective study aiming to find the best therapy of ARN.

Although controversy surrounds the relative authenticity of discontinuous versus continuous memories of childhood sexual abuse (CSA), little is known about whether such memories differ in their likelihood of corroborative evidence. Individuals reporting CSA memories were interviewed, and two independent raters attempted to find corroborative information for the allegations. Continuous CSA memories and discontinuous memories that were unexpectedly recalled outside therapy were more likely to be corroborated than anticipated discontinuous memories recovered in therapy. Evidence that suggestion during therapy possibly mediates these differences comes from the additional finding that individuals who recalled the memories outside therapy were markedly more surprised at the existence of their memories than were individuals who initially recalled the memories in therapy. These results indicate that discontinuous CSA memories spontaneously retrieved outside of therapy may be accurate, while implicating expectations arising from suggestions during therapy in producing false CSA memories.

Abstract:
This article explores the concept of minor or general psychotherapy championed by physicians seeking to popularise psychotherapy in the post-Stalin Soviet Union. Understood as a set of skills and principles meant to guide behaviour towards and around patients, this form of psychotherapy was portrayed as indispensable for physicians of all specialities as well as for all personnel of medical institutions. This article shows how, as a result of Soviet teaching on the power of suggestion to influence human organisms, every interaction with patients was conceptualised as a form of psychotherapy, leading to the embrace of placebo as a legitimate form of therapy, and to the blurring of the boundary between therapy and other activities in the clinic. The principles of minor psychotherapy reveal a concept of psychotherapy that is much wider, and rooted in different priorities, than the dominant understanding of this type of treatment found in Western Europe and North America. This article addresses the ethical principles implicit in the Soviet perspective, demonstrating that despite fighting against the uncaring and dismissive attitude of other physicians, Soviet psychotherapists remained rooted in the paternalistic tradition. Finally, it traces the efforts to establish minor psychotherapy as standard practice in medical institutions, which, like many other plans and ambitions of Soviet psychotherapists, were constrained by a lack of resources in the healthcare system.

PURPOSE AND DESIGN: To review briefly the growing body of published data about circadian variation in cytotoxic drug metabolism and tissue sensitivity to chemotherapeutic agents. The suggestion that toxicity may be reduced and anticancer efficacy improved by administering antineoplastic agents at carefully selected times of the day was assessed. RESULTS AND CONCLUSION: The medical literature describing molecular, cellular, and organismic time-keeping mechanisms, as well as circadian rhythms, in cytokinetic, pharmacokinetic, and pharmacodynamic parameters relevant to cancer chemotherapy, which support the predictable rhythmic relationship between dose and effect that occurs during each day, were reviewed. Advantages for optimal circadian scheduling have been demonstrated for diminishing side effects and increasing maximal safe dose-intensity of drugs of diverse class. The use of the predictable circadian relationship of dose and response provides another increment of progress in the treatment of cancer patients.

Abstract:
The objective of this study was to analyze the dependence of the neutron dose from the geometry of the second band of the maze using dosimetric measurements of neutrons and Monte Carlo simulations, and based on those results to design a novel radiotherapy room layout. Measurements of the neutron dose at a two-band maze therapy room were performed for a 15 MeV photon beam only. Monte Carlo simulations were performed using the GEANT4 toolkit. In order to obtain the geometry dependence, we were changing the second band angle while we kept the length, height, and width the same as in reality. Results show that the highest calculated dose was obtained for the 60? angle of the second maze. It is 17 % higher than for standard 0? angle. For 30? it was 30 % smaller and for 90? was 10% smaller. Although the lowest dose was obtained for 30? band angle with calculations, it is not very practical for clinical use. Clinically the most interesting would be the 90? angle which is practically a short three-band maze, which could be promising from the perspective of neutron radiation protection since it could offer a compact constructional solution, and better optimization of the available space.

Abstract:
Postoperative discitis (POD) accounts for 20% to 30% of all cases of pyogenic spondylodiscitis, while POD may be mis-or-under-diagnosed, due to the vague related symptomatology and the non-specific imaging findings. Most studies report infection rate of less than 1%, which increases with the addition of non-instrumented fusion to 2.4% to 6.2%. It remains controversial whether POD is caused by an aseptic or infectious process. Positive cultures are presented only in 42–73% of patients with Staphylococcus species being the most common invading organisms, while Staphylococcus aureus is isolated in almost 50% of cases. The onset of POD symptoms usually occurs at 2–4 weeks after an apparently uneventful operation. Back pain and muscle spasms are usually refractory to bed rest and analgesics. Magnetic Resonance Imaging (MRI) is the most sensitive and specific imaging diagnostic technique. Antimicrobial therapy depends on the results of tissue cultures, and along with bracing represents the mainstay of management. Surgical intervention is necessary in patients failing conservative treatment. For the majority of cases, extensive surgical debridement, antibiotic therapy, and orthosis immobilization are effective in eliminating the infection. According to this, we recommend an Algorithmic approach for the management of POD. Postoperative infections after spinal surgery pose a certain clinical challenge, and in most cases can be treated conservatively. Nevertheless, disability may be persistent, and surgery could be necessary. The purpose of this concise review is to describe the manifestation of post-discectomy infection, its pathogenesis and particularly a rational approach for its management.

Progress in elucidating neuroimmune connections has created new opportunities for improving the treatment of Parkinson’s disease (PD). In recent years, mind-body therapies have been shown to have positive effects on the immune and nervous systems, but interactions at the molecular level have not been tested. Thus, the main aim of the present study was to investigate the effects of therapy based on visualization and suggestion on the concentrations of IL-6, CRP, DA, BDNF, CoQ10, and TAC in patients with advanced PD. Eight patients with PD and 8 elderly healthy men (control group) were enrolled in the study. The therapy lasted 19 days and consisted of three parts: individual sessions with the therapist, mixed (therapist and audio-file), and self-training (audio-file). Blood samples were taken before training and at the end of each part. The expected changes in the investigated markers was observed during therapy: the serum concentration of IL-6 and CRP decreased, whereas DA and BDNF increased, however, this change was observed only after the first part of intervention when the therapy was conducted by the therapist. In the subsequent stages, the levels returned to the baseline. Noteworthy, after the therapy, we observed a significant increase in the motor and intellectual skills of the PD patients. No such changes were observed in the control group. Mind-body therapy based on visualization and suggestions aimed at changing the concentrations of signaling molecules, which are crucial in the development or treatment of PD, may be an effective element in supporting pharmacological therapy.

Abstract:
This study aims to see an increase in the degree of Self-esteem to all wives who became victim of domestic violence in Bandung after being given Cognitive Behavior Therapy. The measuring equipment used is Self-esteem questionnaire based on Self-esteem Theory of Murk as a dependent variable that consists of 32 items. Validity Test used to measured Self-esteem was done by using Expert Validity method. The data obtained in this study will be analyzed qualitatively. The results showed there is an increase in Self-esteem scores on domestic violence victims after the therapy. The theoretical suggestion for further research is to add more varied ways of assigning tasks to the client, therefore made the therapy sessions more interesting and the clients more motivated to be actively involved in the completion to the assigned tasks. Suggestions for Psychologists who will handle cases of domestic violence, could utilize Cognitive Behavior Therapy to increase Self-esteem on domestic violence victims.
