Abstract:
Doctoral supervision involves an intensive, interpersonal one-to-one relationship between the supervisor and the student. Supervisors have a responsibility to guide students when choosing their research topics and throughout the research process until completion of their research projects. The purpose of this study is to explore the practices and experiences of faculty members supervising doctoral nursing students in two selected universities in South Africa. This qualitative and explorative study involves all faculty members supervising doctorate nursing students at four South African Universities in Limpopo Province. A purposive sampling was used to select 15 participants who met the inclusion criteria. Data collection was through a telephonic in-depth unstructured interview. Probing was used to elicit more information from participants. Data were analysed through Tesch’s open coding method. Findings reveal three themes as practices and experiences of supervision, namely: research supervisory role, knowledge of models of supervision, and guiding principles towards doctorate supervision. There is a need for orientation of research supervisors and doctoral students before they commence with their supervisory role. Policies and procedures for doctorate supervision need to be communicated to all supervisors. There should be continuous support for both supervisors and students during the process of supervision.

Abstract:
Abstract The need for long-term care workers (LTCW) will grow significantly as the American population ages. Understanding the factors that impact job satisfaction of this workforce has important implications for policy and practice. Previous research has demonstrated the effect of supervisor support on the job satisfaction of these workers; however, much less is known about how this effect differs among different race/ethnicity or immigration groups. This study examined how supervisor support mediates the associations between race/ethnicity, immigration status, and job satisfaction among nursing assistants (NAs). Data of 2,763 NAs were extracted from the National Nursing Assistant Survey (2004). Race/ethnicity groups included White (54%), African American (30%), Asian (2%), Hispanic (10%), and others (4%). Immigration status included U.S.-born citizens (87%), naturalized (7%) and resident/alien (6%). Bivariate analyses showed that Asian NAs perceived higher levels of supervisory support than other races, whereas U.S.-born NAs reported lower levels of supervisory support than naturalized and residents/aliens. Findings from multivariate analyses indicated that non-Hispanic Asians and Resident/Alien workers reported significantly higher levels of job satisfaction than their counterparts, and the associations were fully mediated by NAs’ perceived supervisor support. These findings support prior research that supervisor support is important to improving job satisfaction and contribute to the literature that Asians/Residents/Aliens long-term care workers may be more sensitive to supervisory support and may be more grateful if they received support from supervisors. Managers should be aware of these racial differences and by being supportive they may improve NAs job satisfaction and reduce turnover rates.

Background: Supervision is an essential element in primary healthcare. It is not only the number that matters but the quality and effectiveness of supervision as well. Ineffective supervision is a cause of poor performance that can lead to sub-optimal health outcomes.Objective: To assess the effectiveness of supervision in preventive healthcare institutions under the administrative purview of the Regional Directorate of Health Services (RDHS), Kalutara.Method: A descriptive cross-sectional study was designed and carried out in all preventive sector healthcare institutions under the administrative purview of the RDHS, Kalutara. The entire population of the supervisors (Regional Director and Deputy Regional Director of Health Services, Consultant Community Physician, Medical Officer-Maternal & Child Health, Regional Epidemiologist, District Dental Surgeon, Supervisory Public Health Nursing Officers, Supervisory Public Health Inspector, Medical Officers of Health, Additional Medical Officers of Health, Supervisory Public Health Inspectors, Public Health Nursing Sisters, and Supervisory Public Health Midwives) and supervisees (Public Health Inspectors and School Dental Therapists) in the RDHS division except the Public Health Midwives (PHMs) was included in the study as their actual numbers are low. The selection of PHMs, the major category of supervisees, was subjected to random sampling. Self-administered questionnaires, focus group discussions, and observatory visits were used to collect the data. Data were analyzed both quantitatively and qualitatively. The effectiveness of supervision was assessed utilizing Manchester Clinical Supervision Scale (MCSS-26). Knowledge, attitudes, and practices of supervisors were assessed through a self-administered questionnaire and to some extent through observations.Results: The overall response rate was 67.8%. The knowledge related to supportive supervision among the supervisors was moderate. The level of knowledge was not significantly different among the different categories of supervisors. The level of attitudes towards supervision was also moderate. However, it was significantly different among the supervisory categories. As per the scale devised through the questionnaire, the supervisory practice was moderate as well, but it was not significantly different among the different categories of supervisors. The mean score of MCSS-26 was 65.67 indicating that overall supervision was ineffective by its threshold limit of 73. Apart from the administrative function of supervision, the supportive and educational functions were largely ineffective.Conclusion and recommendations: The effectiveness of supervision was suboptimal in preventive care health institutions of the RDHS division Kalutara. Appropriate measures that focused on enhancing supportive and educational functions of supervision are therefore required to improve the effectiveness of supervision among different categories of supervisors in the division. Designing and implementing more focused training and development activities to improve knowledge, attitude, and practices in supportive supervision; facilitating, encouraging, and empowering the supervisors to implement their post-supervisory recommendations; and utilizing the MCSS-26 as a tool of measuring supervisory effectiveness for the purpose of monitoring and evaluating the supervisory activities, are therefore recommended to improve the effectiveness of supervision in the division.

Abstract:
This study aimed to examine the comprehensive impact of five aspects of the nursing work environment as well as supervisory support, nurse characteristics, and burnout on intent to stay (ITS) among Korean hospital nurses. A cross-sectional questionnaire was distributed in seven general hospitals from May to July of 2019. Data were collected from a sample of 631 Korean nurses. The hypothesized model was evaluated using the STATA program for path models. Findings demonstrated that burnout played a mediating role on the relationships between the nursing work environment, supervisory support, nurse characteristics, and ITS. Burnout was the most influential predictor of ITS (β = −0.36, p < 0.001). Nurse participation in hospital affairs (β = 0.10, p = 0.044) and collegial nurse–physician relationships (β = 0.08, p = 0.038) had a direct effect on ITS. Supervisory support had a significant direct effect on ITS (β = 0.19, p < 0.001). Therefore, to increase nurses’ ITS, it is necessary to improve their participation in hospital affairs and collegial relationships, as well as strengthen support from supervisors and reduce burnout.

Background: Clinical education requires competent supervisor to guide student. Supervisory competency should be trained and closely monitored to ensure quality learning process taking place. The aim of this study is to evaluate the effect of micro-skill supervisory model to improve the supervision quality among nursing student. Method: The research used quantitative approach with quasi experimental design. Subject consisted 91 students of the third year Nursing students at Magelang Health Polytechnic. Students were divided into two groups: intervention group (44 students) and control group (47 students) using purposive sampling. Research questionnaire consisted of 21 items which were derived from 5 micro skill domains was used. Intervention was supervision using micro-skill model during 2 weeks in a rotation of clinical practice.Results: There was no difference of quality clinical supervision was used in the control group while in experiment group the pre and post test score was significantly different. The experiment group had higher quality of supervision. The study showed that only 9 items showed significant improvement of supervision and 12 items did not show any improvement.Conclusion: Clinical teaching using micro skill model is more effective in improving the quality of nursing student clinical supervision compare to the conventional method.

Abstract:
This study aims to look at the factors that influence student anxiety in facing clinical nursing practice. This research is a quantitative study using a cross-sectional study design approach. The results showed that the majority of respondents (69.14%) were female, 56 people, and most of the 39 respondents (48.1%) of all respondents had moderate anxiety. In addition, 51 respondents (63%) had good knowledge, up to 52 respondents (64.2%) had good skills, 61 respondents (75.3%) had significant supervisory influence, and 46 respondents (56.8% ) had an excellent environmental effect. In conclusion, there is an important relationship between knowledge, skills, supervisors, and the environment with students' anxiety in facing clinical nursing practice. Background: The shortage of registered nurses is a worldwide concern; with many countries struggling to maintain adequate numbers of nurses. Many factors that contribute to nursing shortage have been studied, but the influence of work–family conflict has received little attention. Work–family conflict has been linked to a number of work-related consequences, including turnover intention among employees. The aim of this study was to evaluate the influence of supervisory support behaviours on the relationship among work–family conflict, Stress, and turnover intention. Sample: 113 nurses. Result: A significant positive correlation was found between work–family conflict and turnover intention. A negative correlation was found between supervisory support behaviours and turnover intention. Both work–family conflict and stress were associated with turnover intention; however, these associations were buffered (weaken), when nurses perceived higher supervisory support behaviours. Conclusion Supervisory support behaviours could be the tool to help married, female nurses play their professional role without compromising their family responsibilities. Nurses’ turnover is a complex issue that may require multiple prevention strategies; however, enhancing family supportive supervisor behaviors could be a key resource for maintaining a positive workplace environment and reducing turnover intention.

A recent ruling by a circuit court—in NLRB v. North Shore University Hospital—prohibited a professional association from serving as a collective bargaining agent of a unit of registered nurses because the court found that statutory supervisors participated in the governance of the association. In this article, the authors analyze the case law concerning supervisory participation in professional associations; discuss nursing representation cases prior to North Shore; describe the responses of nurses' professional associations to North Shore in three states; and discuss the implications of North Shore and related decisions for the representation rights of professional employees. They also propose an analytical model for the NLRB to follow in determining whether supervisory participation in a professional association so endangers the collective bargaining process as to warrant disqualification of that association as a statutory representative.

Abstract:
Aim/Purpose: The purpose of this paper is to illuminate the learning that happens in assuming a supervisee’s role during the postgraduate study. Background: The facilitators and barriers students encountered while pursuing postgraduate studies, strategies to achieve success in postgraduate studies, and how to decrease attrition rates of students, have been sufficiently explored in literature. However, there is little written about the personal and professional impact on students when they are being supervised to complete their postgraduate studies. Methodology: Autoethnographic method of deep reflection was used to examine the learning that transpired from the supervisee’s perspective. Two lecturers (a Senior Lecturer in Nursing and an Aboriginal Tutor) focused on their postgraduate journeys as supervisees, respectively, with over 30 years of study experience between them, in Australia and abroad. Contribution: Future postgraduate students, researchers, would-be supervisors and experienced supervisors could learn from the reflections of the authors’ postgraduate experiences. Findings: Four themes surfaced, and these were Eureka moments, Critical friend(s), Supervisory relationship, and Transformative learning. The authors highlighted the significance of a supervisory relationship which is key to negotiating the journey with the supervisor. Essential for these students also were insights on finding the path as well as the destination and the transformative aspects that happened as a necessary part of the journey. Conclusion. The postgraduate journey has taught them many lessons, the most profound of which was the change in perspective and attitude in the process of being and becoming. Personal and professional transformative learning did occur. At its deepest level, the authors’ reflections resulted in self-actualization and a rediscovery of their more authentic selves. Recommendations for Practitioners: This article highlights the importance of the supervisory relationship that must be negotiated to ensure the success of the candidate. Reflections of the transformation are recommended to support the students further. Recommendation for Researchers: Quality supervision can make a significant influence on the progress of students. Further research on the supervisory relationship is recommended. Impact on Society: The support in terms of supervision to ensure postgraduate students’ success is essential. Postgraduate students contribute to the human, social, professional, intellectual, and economic capital of universities and nations globally. Future Research: Further reflections of the transformative learning will advance the understanding of the personal and professional changes that occur with postgraduate supervision.

Det saknas studier hur handledarna upplever handledning på avancerad nivå inom psykiatrisk omvårdnad. Rollen handledarna har är komplex, de ska både fungera som en mentor och som den som bedömer studenternas prestationer samt ge god vård åt patienten. Syftet med denna studie är att belysa hur handledning upplevs av den handledande sjuksköterskan under Verksamhetsförlagd utbildning (VFU) för specialistsjuksköterskor i psykiatri. Att få kunskapen studien genererar skapar förutsättningar för organisationen och universitetet att anpassa utbildning och återkoppling som gynnar handledarna och studenternas utveckling samt höjer vårdkvaliteten för patienterna. En explorativ kvalitativ ansats enligt fokusgruppsintervjuer har ansetts lämplig för studien då målet är att fånga vidden och komplexiteten i handledning. När deltagarna dras med i diskussionen skapas ett reflekterade klimat som på ett omfattande sätt belyser individens upplevelser, tankar och känslor. Studien kommer att utföras inom psykiatriska kliniken på Länssjukhuset Ryhov efter tillstånd från etisk kommitté.

Abstract:
The purpose of this dissertation is to research aspects of the historical development of Nursing Education and Nursing Management at the Carinus Nursing College from 1949 to 1987; to determine and explain how the College has adapted and coped with historical change and to determine whether proposals for the future can be made. Research has been done by applying the general systems theory and by using the standard methods of historical analysis. Data has been collected by means of oral history, literature search and documentation. The variables isolated are the College as a system; the government or influential super systems; resources such as financial, personnel and students and material inputs; and throughput or processing the work in the output, which leads to the professional nurse. The models used are Bucheles' organizational system, Sharma's flow chart pattern, Mintzberg's parts of organizational systems, and power flows and as shown in Emery, Feibleman and Friends relations and rules of interaction in systems thinking. Parsons' "imperatives of maintenance of a system" as well as Alvin Toffler's "second and third wave phenomena as responses to change" were two further models that were used. By using Robert Buchele's model, the work is divided into four parts: - i) the College as a system ii) the super systems iii) the resources iv) the throughput or processing. A further design that emerged was that two eras could be distinguished, within which three historical phases: - Early, Middle and Late are developed.

As the nation’s healthcare system moves through the 21st century, unprecedented changes are occurring on both a local and global stage. Healthcare organizations are faced with creating and implementing leadership strategies to enhance the overall patient experience. When coupled with the need to ensure increased nurse job satisfaction, improved cost-effectiveness and healthy organizational cultures with fiscally sound budgets, improvement work has led efforts to ensure effective leadership techniques are used across an agency. This task may be viewed as commonplace for some organizations, while others may perceive this process as a complete paradigm shift from historical practices related to leadership style, behaviors and performance. A successful transition during this time of unprecedented change may depend on an organization’s ability to accept and implement the tenets of transformational leadership. Empirical research illustrates that transformational leadership empowers staff, increases job satisfaction and facilitates cost-effectiveness while constructing an environment conducive to the development of a supportive organizational culture. In an effort to ascertain the current state of leadership at a southeastern Veterans Affair Medical Center, this research study explored the self-reporting leadership practices of all supervisory staff employed at the facility. As a means of measurement, the Leadership Practices Inventory (LPI) was coupled by demographic questionnaire developed by the researcher. Both measurement tools were used to collect the data.

Resumo: O ambiente hospitalar pode ocasionar condições inadequadas de trabalho, expondo os trabalhadores os riscos de ordem biológica, física, química, ergonômica, mecânica, psicológica e social. O objetivo deste estudo foi avaliar a associação do absenteísmo doença com sexo, estado civil, idade, escolaridade, local, turno de trabalho, tempo de trabalho na instituição, função e vínculo empregatício. Estudo descritivo exploratório, com abordagem quantitativa, desenvolvido em um Hospital Universitário público do interior paulista, de alta complexidade. O banco de dados foi obtido do setor de recursos humanos de um de Janeiro de 2011 a 31 de dezembro de 2011. O absenteísmo doença de curto prazo foi considerado aquele até 15 dias consecutivos de atestado médico. Dos 994 profissionais, 645 apresentaram pelo menos um dia de afastamento por licença médica. Quanto os fatores associados com o absenteísmo doença foram: grupo etário de 30-39 anos, categoria profissional técnicos e auxiliares de enfermagem, lotados no CC/CCA/CME, e quanto ao tempo de trabalho na Instituição o grupo de cinco a nove anos foi estatisticamente significante. O absenteísmo doença tem fatores complexos e multifatoriais que precisam ser analisados sob a perspectiva dos processos de trabalho, cultura institucional, saúde e satisfação do trabalhador de enfermagem
Abstract: The hospital environment can lead to poor working conditions, exposing workers to risk of biological, physical, chemical, ergonomic, mechanical, psychological and social. The aims was evaluate the association of the disease with absenteeism sex, marital status, age, education, location, shift work, working time in the institution, role and employment. It's exploratory descriptive study with a quantitative approach, developed in a public university hospital in São Paulo State, high complexity. The database was obtained from the human resources department of 1st January, 2011 to 31th December, 2011. The short-term sickness absenteeism was defined as up to 15 consecutive days of medical certificate. Of 994 professionals, 645 had at least one day of absence due to sick leave. O enfermeiro com cargo de chefia, ao liderar uma equipe e conduzir um serviço de saúde, assume importante papel na prevenção de exposição ocupacional envolvendo material biológico potencialmente contaminado, pois lidera a equipe de profissionais que mais se acidenta dentro do ambiente hospitalar, por estarem constantemente em contato com sangue e fluidos corpóreos. Este estudo do tipo descritivo, foi realizado em um hospital universitário de grande porte de uma cidade do interior de São Paulo e teve como objetivo avaliar o conhecimento e as crenças dos enfermeiros com cargos de chefia acerca da exposição ocupacional a material biológico. A coleta de dados foi realizada no período de maio a agosto de 2006 e os dados foram analisados qualitativamente, por meio do método de Análise de Conteúdo de Bardin (1977), e quantitativamente, através da estatística descritiva, tendo como base o Modelo de Crenças em Saúde (MCS) de Rosenstock (1974a,b) que possibilitou compreender o conhecimento dos enfermeiros com cargo de chefia sobre o risco biológico e principais condutas diante de uma situação de acidente, evidenciando-se assim as falhas na formação e as lacunas no processo de aperfeiçoamento e atualizações desses profissionais. Através do MCS identificou-se as barreiras enfrentadas por enfermeiros com cargo de chefia, para adesão dos trabalhadores de enfermagem às medidas de prevenção à exposição ocupacional envolvendo sangue e fluídos corporais, relacionadas à infra-estrutura, aos recursos humanos e de materiais da instuição. A resistência ao uso do equipamento de proteção individual, o descarte incorreto de materiais contaminados e o número inadequado de profissionais foram apontados como os principais motivos para a ocorrência de acidentes na instituição. Os enfermeiros consideraram-se mais susceptíveis ao vírus da imunodeficência adquirida do que aos vírus das hepatites C e B e o conhecimento das doenças que podem ser transmitidas através de fluídos corporais foi bastante limitado, evidenciando-se a necessidade de trabalhar com educação permanente junto a eles para que possam orientar sobre as condutas e os riscos de exposição ocupacional envolvendo material biológico nos respectivos setores de trabalho. Esses profissionais acreditam que os benefícios da prevenção de acidentes são: prevenção de doenças futuras ou doenças que podem trazer complicações graves, e com risco de morte, no entanto, muitos relataram que a prevenção de acidentes é necessária para não perderem dias de trabalho decorrentes de afastamento e conseqüentemente redução salarial. A educação permanente como estratégia de comunicação em serviço poderá auxiliar programas que visam a prevenção de acidentes ocupacionais com material biológico envolvendo a equipe de trabalho. Além disso, os enfermeiros que ocupam cargos de chefia devem manter uma postura de valorização do clima de segurança no ambiente laboral.
By leading a team and conducting a health service, head nurses assume an important role in the prevention of occupational exposure to potentially contaminated biological material, as they lead professional teams that are the most frequent accident victims in the hospital environment, due to they constant contact with blood and body fluids. This descriptive study was carried out at a large university hospital in the interior of São Paulo and aimed to assess head nurses? knowledge and beliefs about occupational exposure to biological material. Data were collected between May and August 2006 and subject to qualitative analysis, using Bardin?s Content Analysis method (1977), and quantitative analysis through descriptive statistics, based on Rosenstock?s (1974a, b) Health Belief Model (HBM), which allowed us to understand head nurses? knowledge on biological risk and the main conducts towards an accident situation. This evidenced defects in these professionals? training and gaps in their improvement and recycling process. Through the HBM, we identified the barriers head nurses face with a view to nursing workers? adherence to measures aimed at preventing occupational exposure to blood and body fluids. These barriers were related to the institution?s infrastructure, human and material resources. Resistance to the using of individual protection equipment, incorrect discarding of contaminated materials and inadequate number of staff were indicated as the main motives for accident occurrence at the institution. Nurses consider themselves more susceptible to the acquired immunodeficiency virus than to hepatitis C and B virus, and knowledge about body fluid-borne diseases was quite limited, evidencing the need for permanent education, so that these nurses can provide adequate orientations about behaviors and occupational exposure risks involving biological material in their respective work sectors. These professionals believe that the benefits of accident prevention are: prevention of future diseases or disease that can cause severe complications and death risk, although many of them reported that accident prevention is needed to avoid losing work days deriving from leave of absence and, consequently, to avoid a salary reduction. Permanent education as an in-service communication strategy can help programs to prevent occupational accidents with biological material involving the work team. Moreover, head nurses need to preserve an attitude that values the safety climate in the work environment.

It is about a descriptive and qualitative research which scenario was the Santa Maria University Hospital (HUSM), Rio Grande do Sul. The participants were nurse managers and the data were collected in April and May 2012, using semi-structured interviews.The analysis had been started concomitant with the data collect, with theuse of Grounded Theory (GT), theoretical support of Symbolic Interactionism and others leadership studies.Were followed the ethical principles of Resolution No. 196/96 of the National Health Council, which regulates research involving humans.That way, the audio data´swere transcripts and coded according to the order: open coding, axial code and selective coding.The results originated the thematic categories: realizing a nurse manager, looking for meanings to the process of leading and being leader, experiencing the service reality, confronting management challenges, establishing interpersonal relationships and seeking improvements to the service.The interconnection of these, through the analysis with the paradigmatic model has made emerge a theoretical matrix, composed by the central phenomenon following:experiencing nursing management in a hospital context: meanings to leadership. Finally, it was possible identified that the nurse manager realizes the importance of their actions at the managerial dimension and seeks to achieve their objectivesthrough attitudes that approaches a participative leadership model.Thus, should be paid attention with regard the need to awaken a look that valuates interpersonal relationships and the development of the human potential inhealthcare organizations.
Mais do que constatar a importância da presença de líderes ativos no contexto da enfermagem hospitalar, buscou-se resgatar os significados dos enfermeiros que vivenciam a posição de gerentes em enfermagem e que possuem suas ações permeadas de possibilidades relativas ao desenvolvimento de liderança. Em vista disso, o objeto da pesquisa realizada foi a liderança de enfermeiros-gerentes que atuam em um Hospital Universitário do interior do Rio Grande do Sul. Para isso, elaborou-se a questão norteadora: como o enfermeiro-gerente exerce a liderança no contexto hospitalar? Foram traçados como objetivos: analisar a liderança de enfermeiros-gerentes de um Hospital Universitário, descrever as vivências relacionadas à liderança no cotidiano gerencial dos enfermeiros e elaborar uma matriz teórica representativa da experiência dos enfermeiros-gerentes em relação à liderança. Trata-se de uma pesquisa descritiva e qualitativa cujo cenário foi o Hospital Universitário de Santa Maria (HUSM), Rio Grande do Sul. Os sujeitos foram enfermeiros-gerentes, e os dados foram coletados em abril e maio de 2012, por meio de entrevistas semiestruturadas. A análise foi iniciada concomitante à coleta, com a utilização da Teoria Fundamentada nos Dados (TFD) e o respaldo teórico do Interacionismo Simbólico e estudos sobre liderança. Foram seguidos os princípios éticos da Resolução nº 196/96 do Conselho Nacional de Saúde que regulamenta a pesquisa envolvendo seres humanos. Dessa forma, os dados em áudio foram transcritos e codificados de acordo com a ordem: codificação aberta, codificação axial e codificação seletiva. Os resultados obtidos originaram as categorias temáticas: percebendo-se enfermeiro- gerente, buscando significados para o processo de liderar e ser líder, vivenciando a realidade do serviço, enfrentando desafios gerenciais, estabelecendo relações interpessoais e buscando melhorias para o serviço. A interconexão dessas, por meio da análise junto ao Modelo Paradigmático, fez emergir uma matriz teórica, composta pelo seguinte fenômeno central: vivenciando a gerência de enfermagem no contexto hospitalar: significações para a liderança. Por fim, foi possível identificar que o enfermeiro- gerente percebe a importância de suas ações na dimensão gerencial e busca atingir seus objetivos por meio de atitudes que se aproximam de um modelo de liderança participativa. Assim, deve-se atentar no que se refere à necessidade de despertar um olhar voltado para a valorização das relações interpessoais e ao desenvolvimento do potencial humano nas organizações de saúde.

This book has been written to reflect modern ideas about what constitutes good mental health nursing care, and you will see that values such as partnership working within the framework of a therapeutic relationship have been deliberately stressed. However, the role of the mental health nurse has always required some involvement in what is essentially custodial care. It is necessary for us as mental health nurses to try to balance the demands of these two seemingly paradoxical elements of the role of a mental health nurse. This chapter is intended to introduce you to some aspects of mental health law. We have partly based this upon the law as it currently applies in England and Wales but you will notice that we have also tried to include some material from an international perspective. The history of the profession of mental health nursing is inextricably bound up with the story of the rise and fall of the asylum and with institutionalized models of care. It was only following the Macmillan commission, which was set up to investigate allegations of abuse at Prestwich Hospital in 1924, that the term ‘psychiatric nurse’ (which later evolved to mental health nurse, Department of Health 1994) became a commonly used description (Coppock and Hopton 2000). Prior to this time, people working in institutions for the mentally disordered were more oft en referred to as ‘attendants’ or ‘keepers’ (Nolan 1998), and as these names imply, their roles were mostly custodial or supervisory in nature. Mental health nursing has moved away from this limited model of providing ‘care’ but is still unusual amongst other health care professions in that its members continue to be involved in compulsory detention (even though the main responsibility for this rests with the medical profession; Rogers and Pilgrim 2001). In England and Wales the 1983 Mental Health Act and its 2007 update is currently the legislation directing compulsory treatment of people with mental disorder. In common with legislation in most countries, this Mental Health Act aims to achieve a balance between the rights of the individual mental health patient to be treated and protected and the perceived need to protect others.

Abstract Up to this point we have been discussing ethical issues that involve identifiable individuals; ethical inquiry, however, may lead us beyond specific individuals to social structures. For what reasons, if any, do ethical considerations require us to identify faults in social structures and then attempt to remedy them? Last weekend, staff nurse Andrea Moore, who works in ICU, felt she was not able to give patients the kind of care, including frequent enough observations, that she should provide because the unit was short-steffed. After she told the charge nurse that she had too much to do, the charge nurse called the supervisor, whose answer was to “make do.” Andrea knew then that she had to handle the situation as best she could and leave low-priority work undone. One of her patients was on a respirator and required numerous treatments, including /Vs. Another man had an aortic aneurysm and two women had had major surgery. When the physician ordered a variety of treatments and observations, including some scheduled for every fifteen minutes, the charge nurse again called the supervisor for more help and again was told to make do with the staff on duty.

This chapter examines the home health aides' (HHAs) mantra: “observe, record, and report” (O.R.R.). At the end of each shift, Cooperative Home Care Associates (CHCA) aides filed an automated report by telephone, pressing numerals to respond to recorded questions about the tasks they had done that day. Any anomalies in the patient's physical or mental health had to be reported immediately to the client's nurse supervisor. The aides served as the eyes and ears of a patient's care team, and nowhere was O.R.R. more important as a prevention tool than with skin care. The Obama administration issued a rule allowing consumers to sue nursing homes for negligence rather than submit to binding arbitration, as is frequently specified in nursing home contracts with patients and their families. Seven months into his term, Donald Trump moved to eliminate the rule and allow nursing homes once again to insert a binding arbitration clause in their contracts. The chapter then traces the long history of money troubles between care workers and the people for whom they care. Almost all of CHCA's revenue came from clients whose home health care was paid for by federal Medicaid insurance.

In 1993, Taiwan has transformed into an aged society. Compared with general patients, older patients differ considerably in body functionality, mentally, and mobility-wise. The primary customers of medical institutions are adult patients in Taiwan, and the needs of older patients are often ignored; however, older patients visiting the case hospital in this study accounted for nearly 30% of the total number of outpatients. This indicates that the needs of older patients are a matter requiring the serious attention of medical personnel. This is a common issue for all countries that were entered an aging structure of the population. This study investigated the attention paid by supervisors and medical personnel involved with medical decisions to the quality of medical services received by older outpatients. An analytic hierarchy process (AHP) was employed to interview 10 experts in older medical care providing age-friendly medical care at the case hospital, medical staff including senior physicians, nursing department supervisors, administration department supervisors, and certified managers. The results indicate that the supervisors and medical personnel considered the “communication and services” provided to older patients during their medical treatments to be the most crucial factor in their treatment process, rating it twice as important as the next most important criteria, the care process and physical environment. Medical personnel paid great attention to communicate with and servicing older patients; listening to their needs and concerns was deemed the most critical, followed by being able to empathize with them to solve problems. Asking patients about their problems when they visited the hospital was considered the most crucial aspect of the care process; in the physical environment, it was spatial planning and traffic flow design. The results of this study enable other countries' medical institution managers and relevant competent authorities to gain insight into the attention paid by front-line employees in promoting age-friendly medical care. If supervisors and medical personnel involved with medical decisions share a similar attitude toward promoting medical service quality, government authorities and medical institutions can implement and enhance an age-friendly medical treatment environment for older patients, as well as increase quality of patient-centered medical services.

Aim/Purpose: [This Proceedings paper was revised and published in the journal "Issues in Informing Science and Information Technology,"16, 21-40.] The purpose of this paper is to illuminate the learning that happens in assuming a supervisee’s role during the postgraduate study. Background: The facilitators and barriers students encountered while pursuing postgraduate studies, strategies to achieve success in postgraduate studies, and how to decrease attrition rates of students, have been sufficiently explored in literature. However, there is little written about the personal and professional impact on students when they are being supervised to complete their postgraduate studies. Methodology: Autoethnographic method of deep reflection was used to examine the learning that transpired from the supervisee’s perspective. Two lecturers (a Senior Lecturer in Nursing and an Aboriginal Tutor) focused on their postgraduate journeys as supervisees, respectively, with over 30 years of study experience between them, in Australia and abroad. Contribution: Future postgraduate students, researchers, would-be supervisors and experienced supervisors could learn from the reflections of the authors’ postgraduate experiences. Findings: Four themes surfaced, and these were Eureka moments, Critical friend(s), Supervisory relationship, and Transformative learning. The authors highlighted the significance of a supervisory relationship which is key to negotiating the journey with the supervisor. Essential for these students also were insights on finding the path as well as the destination and the transformative aspects that happened as a necessary part of the journey. Conclusion. The postgraduate journey has taught them many lessons, the most profound of which was the change in perspective and attitude in the process of being and becoming. Personal and professional transformative learning did occur. At its deepest level, the authors’ reflections resulted in self-actualization and a rediscovery of their more authentic selves. Recommendations for Practitioners: This article highlights the importance of the supervisory relationship that must be negotiated to ensure the success of the candidate. Reflections of the transformation are recommended to support the students further. Recommendation for Researchers: Quality supervision can make a significant influence on the progress of students. Further research on the supervisory relationship is recommended. Impact on Society: The support in terms of supervision to ensure postgraduate students’ success is essential. Postgraduate students contribute to the human, social, professional, intellectual, and economic capital of universities and nations globally. Future Research: Further reflections of the transformative learning will advance the understanding of the personal and professional changes that occur with postgraduate supervision.

This paper synthesises the results of three participatory action research (PAR) studies undertaken to improve the integration of evidence-based practice (EBP) education in three undergraduate health courses at one Australian university: Bachelor of Nursing, Bachelor of Occupational Therapy, and Bachelor of Physiotherapy. The PAR process with interested academics uncovered a range of EBP education strengths and weaknesses in the three courses. Common themes were evident, which are likely to be applicable in other similar courses. Identified weaknesses included a lack of explicit teaching about the meaning, principles, steps, and importance of EBP, partly stemming from a lack of shared understanding. A relative lack of emphasis on certain EBP steps was also noted, particularly the first step of ‘asking’ questions. A lack of communication with workplace learning (WPL) supervisors about how to facilitate EBP was also noted, raising concerns about variable EBP-education quality across WPL settings. Opportunities for improvement were identified by academics in each course, across multiple subjects and year levels. In our experience, PAR has been a highly constructive approach to EBP curriculum improvement. We encourage consideration of a PAR approach for addressing similarly complex curriculum challenges.

ABSTRACT Background: Maternal and infant mortality rates remain high in most developing countries including Indonesia. An approach so called as the interprofessional collaboration (IPC) has been considered to have its potential to improve the emergency obstetric and neonatal care. Little is known about the effectiveness of leadership in enhancing IPC. This study aimed to determine the effectiveness of leadership on improving the IPC in the comprehensive emergency obstetric and neonatal services. Subjects and Method: This was a qualitative study using an embedded case study approach. This study was conducted at PKU Muhammadiyah Gamping Hospital, Yogyakarta, as a type C teaching hospital. The data were collected by observation, in-depth interview, and document review. Results: This study found three themes: (1) collaborative leadership; (2) leadership issues; and (3) stakeholder input. The inter-professional collaboration included doctors, consultant doctors, supervisor, shift coordinator, and nurses in charge of nursing care. The interprofessional collaboration had been implemented. The principle of leadership had supported the interprofessional collaboration. The IPC team had understood and applied the principles of leadership that support the IPC. The leadership attributes on demand for the IPC included visionary, participatory, and coaching. The leadership issues included the difference in advice between doctors. The theme for nurses was improving the quality of interprofesional collaboration. Conclusion: The leadership attributes to improve the interprofessional collaboration include visionary, participatory, and coaching for the comprehensive emergency obstetric and neonatal services. INTRODUCTION During the second wave of Covid-19 in January 2021 in Lesotho, MSF carried out an exploratory assessment at hospitals providing care for Covid-19 patients. We observed healthcare teams were understaffed and overworked, with an absence of nurse aides or patient care assistants to provide basic care (helping patients to eat and drink, dress, toilet, changing bed linen). Hence nurses and medical doctors would prioritise skilled tasks, such as medication administration, over more basic care, normally performed by nurse aides. Such basic care is essential to patient experience. quality of care, and dignity. As part of Covid-19 care, training nurse aides on proning or repositioning oxygen masks of hypoxic patients could potentially reduce morbidity and mortality. To date, MSF has never implemented formal training for nurse aides, relying instead on on-the-job training, with significant variations in the delivery of training and what tasks are fulfilled. METHODS A pilot programme was implemented in Lesotho during February and March 2021. 16 nurse aides were trained and supervised by MSF. Further programmes were initiated during the third wave of Covid-19 in Zimbabwe (two hospitals) and South Africa (three hospitals) in 2021. Specific training materials and implementation tools were developed to support deployment of this innovative strategy. As part of programme monitoring, nurse aide and staff surveys covering satisfaction with the programme impact, the experience of staff and patients, and training received were carried out at the end of the interventions. At two sites, nurse aides and their supervisor recorded data for a sample of their daily tasks and the time spent performing each task. ETHICS This innovation project does involve human participants and their data. Permission was granted by the Medical Director of MSF Operational Centre Brussels. RESULTS 100% of medical staff surveyed (nurses, doctors, and nurse aides) from all six hospitals reported satisfaction with this programme for improving the provision of basic patient care during the waves of Covid-19. Qualitative data highlighted the programme helped support basic patient care, to reduce workloads of nurses and doctors for these tasks, and to improve patient dignity. Nurse aides reported overall satisfaction with their training, especially for bedside and practical sessions. A hands-on nursing supervisor was reported as crucial for success. Showing potential for handover, the Ministry of Health continued employing nurse aides at one hospital in South Africa, and a partner non-governmental organisation took over the group trained in Lesotho. CONCLUSION These short programmes supported the surge workload of Covid-19 waves. While the role of nurse aides exists within MSF projects, scope exists to develop formal training packages covering essential patient care. Training can be adapted to extend such roles in the context of other outbreak scenarios, such as cholera or Ebola virus disease, and to support provision of holistic patient care. There is interest in repeating the programme in the southern Africa region, and to share the model as a strategy to support medical human resources. CONFLICTS OF INTEREST None declared.
