Abstract:

Die suprakondyläre Humerusfraktur stellt eine der häufigsten Frakturen im Kindesalter dar. Hierbei richtet sich die Therapie einerseits nach dem ossären Dislokationsgrad entsprechend der Gartland-Klassifikation. Andererseits ist die initiale Erfassung von begleitenden neurovaskulären Läsionen aufgrund der Beeinträchtigung des Langzeitverlaufes essenziell, jedoch in der Klinik nicht immer einfach. Die Kenntnis der Besonderheiten von suprakondylären Humerusfrakturen im Kindesalter stellt somit eine essenzielle Voraussetzung zum Erreichen eines funktionell und kosmetisch zufriedenstellenden Langzeitergebnisses dar.

Abstract:

ZusammenfassungFrakturen des distalen Humerus sind die häufigsten Verletzungen am kindlichen Oberarm. Im überwiegenden Teil handelt es sich hierbei um eine suprakondyläre Humerusfraktur. Prinzipiell muss man diese rein metaphysären Frakturen und die ebenfalls extraartikulär liegenden Frakturen im epikondylären Bereich von den intraartikulären Frakturen im kondylären Bereich bez. der Behandlungsstrategie und der Prognose unterscheiden. Die Wachstumspotenz und damit das Korrekturpotenzial sind am distalen Humerus selbst bei sehr jungen Kindern gering, dies muss daher bei der Behandlungsstrategie beachtet werden. Undislozierte und stabile Frakturen können immer konservativ behandelt werden. Ziel der operativen Therapie bei den dislozierten und instabilen Frakturen ist die anatomische Reposition und wenn möglich bewegungsstabile Frakturfixation, wobei Osteosynthesen mittels K-Drähten einer zusätzlichen Ruhigstellung bedürfen. Bei Beachtung der grundlegenden therapeutischen Prinzipien kommt es in aller Regel zu exzellenten funktionellen Ergebnissen. Durch übersehene Fehlstellungen oder unzureichend reponierte Frakturen können selten durch Pseudarthrosenbildung, häufiger durch Wachstumsstörungen vor allem kosmetisch störende Fehlstellungen (Cubitus valgus oder varus), aber auch Bewegungseinschränkungen, Schmerzen und Nervenirritationen resultieren, die das Outcome dieser Verletzungen deutlich verschlechtern.
