Abstract:
During practice, a 25-year-old Division II men’s basketball player reported lightheadedness, blurry vision, and that his heart was “racing”. After his heart rate was recorded above 240 bpm on two occasions, the athlete was referred for further evaluation. An electrophysiologist diagnosed the patient with supraventricular tachycardia (SVT). The condition was corrected using a catheter radiofrequency ablation and the athlete returned to play 5 weeks after his first presentation of signs and symptoms. The referral, diagnostic, treatment, and return-to-play processes demonstrated in this case represent the successful use of interprofessional practice to manage an athlete with SVT.

Abstract Superficial vein thrombosis (SVT) is regarded a self-limiting disorder, although the authors of recent studies showed that ultrasonographically diagnosed SVT is a precursor for venous thrombosis. We aimed to determine whether the same holds true for clinically diagnosed SVT and to what extent it is associated with thrombophilia in a population-based case-control study (ie, Multiple Environmental and Genetic Assessment of risk factors for venous thrombosis). We found that a history of clinical SVT was associated with a 6.3-fold (95% confidence interval [CI] 5.0-8.0) increased risk of deep-vein thrombosis and a 3.9-fold (95% CI 3.0-5.1) increased risk of pulmonary embolism. Blood group non-O and factor V Leiden showed a small increase in SVT risk in controls, with odds ratios of 1.3 (95% CI 0.9-2.0) and 1.5 (95% CI 0.7-3.3), respectively. In conclusion, clinically diagnosed SVT was a risk factor for venous thrombosis. Given that thrombophilia was only weakly associated with SVT, it is likely that other factors (varicosis, obesity, stasis) also play a role in its etiology.

SummarySuperficial venous thrombotic (SVT) events are a feature of thrombophilic abnormalities, particularly those involving the protein C pathway. We have determined the incidence of SVT associated with pregnancy and the early postpartum period in a retrospective study involving 72 000 deliveries. Fourty-nine cases occurring in 47 individuals were recorded, with an overall incidence of 0.68/1000 deliveries (95% CI 0.48-0.88). None had a previous history of deep vein thrombosis or pulmonary embolism. Most events occurred in the early post-partum period (0.54/1000 deliveries). Twenty-four/fourty-seven were screened for established thrombophilic abnormalities, with only 1 abnormality detected (FVLeiden heterozygote). Thrombophilia may play a minor role in the aetiology of SVT associated with pregnancy, although a larger study is required to confirm this.

Abstract:
Abstract BACKGROUND: Myeloproliferative neoplasms (MPN), including polycythemia vera (PV), essential thrombocythemia (ET), and primary meylofibrosis (PMF), are frequently associated with splanchnic vein thromboses (SVT). Risk factors for SVT in MPN patients differ from risk factors for all-cause thrombosis. This is likely due to differing disease mechanisms at play, and suggest a separate disease phenotype for MPN/SVT patients. While several studies have characterized the features of MPN patients with SVT, a direct comparison of MPN/SVT versus all MPN patients has been lacking. METHODS: We performed a retrospective, cross-sectional analysis of patients at Barnes-Jewish Hospital from 2000-2014 with MPN and SVT. Patients were identified using ICD-9 codes in the electronic medical record. 52 available patients with both MPN and SVT were included. Randomly selected 134 patients with MPNs only were used as controls. Clinical and laboratory variables were compared between the two groups. Quantitative JAK2 V617F allele burdens were available in 20 patients. As continuous variables were not normal in distribution, a Mann-Whitney U test was performed. Non-continuous variables were compared with an N-1 Chi-squared test to accommodate rare events. All p-values were corrected for multiple testing. RESULTS: MPN/SVT patients were significantly younger at time of MPN diagnosis (median age 47 vs 57 years, p=0.003). MPN/SVT patients were more likely to have splenomegaly (83% vs 30%, p=0.003), deep vein thrombosis (37% vs 15%, p=0.003), and concomitant thrombophilia (17% vs 2%, p=0.003). MPN/SVT patients had a higher proportion of females (63% vs 54%), but this finding did not reach significance. However, PV/SVT patients had a significantly higher proportion of females compared to PV alone (67% vs 37%, p=0.02). There were no significant differences in JAK2 mutation status, race, smoking status, presence of stroke or coronary artery disease risk factors. MPN/SVT patients had significantly lower hemoglobin (13.1 vs 14.6, p=0.024), hematocrit (39.3 vs 43.5, p=0.027), and platelet count (513 vs 698, p=0.003) at time of MPN diagnosis. When analysis was restricted to PV, only hemoglobin (14.6 vs 17.24, p=0.007) and hematocrit (44.3 vs 50.68, p=0.012) were significantly lower in SVT patients. No significant differences in cell counts were detected in ET and PMF patients. MPN/SVT patients had significantly lower JAK2 mutant allele burdens, with no MPN/SVT patient having an allele burden greater than 10% (p=0.019) (Figure 1). In contrast, mutant allele burdens for MPN patients ranged from 0.1 to 99.7%, with median allele burden being 36.3%. DISCUSSION: This is the first study to directly compare clinical and laboratory features of MPN patients with and without SVT. Our results confirm that MPN/SVT patients are younger, and within PV are more likely to be female. We also demonstrate that MPN/SVT patients have lower cell counts and lower JAK2 mutant allele burdens, findings not previously shown in the literature. MPN/SVT patients are more likely to have splenomegaly, concurrent thrombophilia, and additional DVT. These results indicate that MPN/SVT patients exhibit a disease phenotype distinct from MPN patients without SVTs. While the nature of this study is retrospective and causality cannot be definitively established, the findings of younger age, lower laboratory values, and lower JAK2 allele burden are consistent with the hypothesis that MPN/SVT patients present early in disease. It is possible that in MPN/SVT patients, other environmental and host factors (such as concurrent thrombophilia), in combination with early MPN disease, result in the first manifestation of SVT. These findings have important implications, as investigating the natural course of MPN/SVT patients would allow insight into MPN disease pathogenesis. These findings also suggest that SVTs in MPN patients are not solely mediated by elevated cell counts. In addition, while the presence of the JAK2 V617F mutation likely does affect thrombotic risk, the finding of lower allele burdens in MPN/SVT patients suggests that additional interactions mediate SVT development. These interactions are likely multifactorial and include both environmental and genetic factors. Of particular interest would be the presence of yet unidentified driver mutations present in MPN/SVT patients. Abstract Background There is a paucity of data on the incidence of risk factors for splanchnic vein thrombosis in current published literature. The present study is an attempt to determine the risk factors, diagnostic methods employed, treatment modalities, and outcomes in patients with splanchnic vein thrombosis in a single institution over a two-year period. Methods Retrospective chart review of patients, 18-90 years old, who were diagnosed with splanchnic vein thrombosis (SVT) at a single institution from January 1, 2010 to November 10, 2012. They were grouped as those with Budd-Chiari syndrome (BCS) and those with portal vein thrombosis (PVT), including those combined with splenic vein thrombosis (SPVT) and those with mesenteric vein thrombosis (MVT). Results Among the 246 patients studied, 21 had BCS and 225 had PVT. Associated risk factors in the order of frequency were liver disease being present in 48% of BCS, 69% of PVT, 45% of PVT+SPVT, and 52% of PVT+MVT. Next was regional cancer, being present in 24%in BCS and 47% of PVT. Third commonest was pancreatitis being present in 14% of BCS, 9% of PVT, 18% of PVT+SPVT, and 6% of PVT+MVT. Hereditary thrombophilias were found in 10% of the BCS group and 4% of PVT; however, it constituted 18% of the PVT+SPVT group, and 12% of the PVT+MVT group. 10% of patients in both the BCS and PVT groups had a liver transplant during their lifetime. The most common presenting symptom was abdominal pain occurring in 57% patients with BCS and 50% patients with PVT. The majority had laboratory findings of liver dysfunction at presentation with 86% in BCS group and 78% in PVT group. JAK2 V617F mutation, when tested, was present in 14% of those with BCS, 20% of the PVT group, 29% of those with PVT+SPVT and 22% of those with PVT+MVT. Diagnosis of SVT was most commonly made by computerized tomography (CT) with contrast (57% for BCS, 56% for PVT). Approximately 60% of BCS patients and 30% of PVT patients received either short-term or long-term anticoagulation; 20% of both groups received transjugular intrahepatic portal system (TIPS) catheterization. Recurrence of symptoms requiring a second hospitalization occurred in 24% of those with BCS and 15% of patients with PVT (36% of the PVT+SPVT and 27% of the PVT+MVT). In those patients with a greater comorbidity profile, including hypertension, diabetes, and malignancy, PVT is more likely than BCS to occur. Regional presence of inflammation or cancer, specifically underlying liver disease, hepatocellular carcinoma, pancreatic cancer, pancreatitis, as well as regional surgical procedures appear to play major role in splanchnic vein thrombosis, while hereditary thrombophilias and the JAK2 V617F mutation make up an important but small component of splanchnic vein thrombosis. Contrast-enhanced CT was the most commonly successful radiologic technique for diagnosis, though magnetic resonance imaging (MRI) provides a more accurate alternative. Anticoagulation was largely limited to patients with the most severe cases of SVT, and symptomatic recurrence was also more likely in these populations. Conclusions The present findings of risk factors associated with SVT are at variance with those in the current published literature, with higher incidence of regional cancer and lower incidence of JAK2 V617F mutation. There are, however, limitations to this study, including the fact that this is a retrospective analysis with data from a single institution. Verification of these findings has to been made in a prospective multi-institutional study involving a larger number of patients and a longer period of observation. The authors report a case of cardiac arrest in a patient receiving intravenous (IV) metoclopramide and review the pertinent literature. A 62-year-old morbidly obese female admitted for a gastric sleeve procedure, developed cardiac arrest within one minute of receiving metoclopramide 10 mg via slow intravenous (IV) injection. Bradycardia at 4 beats/min immediately appeared, progressing rapidly to asystole. Chest compressions restored vital function. Electrocardiogram (ECG) revealed ST depression indicative of myocardial injury. Following intubation, the patient was transferred to the intensive care unit. Various cardiac dysrrhythmias including supraventricular tachycardia (SVT) associated with hypertension and atrial fibrillation occurred. Following IV esmolol and metoprolol, the patient reverted to normal sinus rhythm. Repeat ECGs revealed ST depression resolution without pre-admission changes. Metoclopramide is a non-specific dopamine receptor antagonist. Seven cases of cardiac arrest and one of sinus arrest with metoclopramide were found in the literature. The metoclopramide prescribing information does not list precautions or adverse drug reactions (ADRs) related to cardiac arrest. The reaction is not dose related but may relate to the IV administration route. Coronary artery disease was the sole risk factor identified. According to Naranjo, the association was possible. Other reports of cardiac arrest, severe bradycardia, and SVT were reviewed. In one case, five separate IV doses of 10 mg metoclopramide were immediately followed by asystole repeatedly. The mechanism(s) underlying metoclopramide’s cardiac arrest-inducing effects is unknown. Structural similarities to procainamide may play a role. In view of eight previous cases of cardiac arrest from metoclopramide having been reported, further elucidation of this ADR and patient monitoring is needed. Our report should alert clinicians to monitor patients and remain diligent in surveillance and reporting of bradydysrrhythmias and cardiac arrest in patients receiving metoclopramide.

Abstract Thrombosis is the most frequent complication in essential thrombocythemia (ET) that impairs quality of life and contributes to mortality and morbidity of patients. The molecular basis of thrombosis in ET remains unclear. Polymorphisms (SNPs) in coagulation factors (FII-G20210A, FV-Leiden) and the V617F mutation of JAK2 gene (JAK2-V617F) have previously been shown to associate with thrombosis in ET. Since thrombosis occurs frequently in JAK2-V617F negative ET and FII-G20210A and FV-Leiden polymorphisms occur only in up to 5% of patients, we examined the hypothesis that other genetic factors contribute to the thrombotic risk in ET. In addition to FII-G20210A and FV-Leiden we studied thirteen candidate SNPs of genes involved in coagulation (F7, F13A1, VWF, THSD1, ATIII), platelet aggregation (P2Y12, GPIIIA, GPIIB, FCGR2A) and endothelial function (NOS3, ADRA2B) previously implicated in thrombosis in non-MPD patients. We studied 94 patients with the diagnosis of ET retrospectively observed for clinical variables such as venous and arterial thrombosis, stroke, myocardial infarction, deep vein thrombosis (DVT), bleeding, angina pectoris, superficial venous thrombosis (SVT), and pulmonary embolism. SNP genotyping was performed using allele-specific PCR and sequencing. We examined the association of individual SNPs with the presence or absence of clinical variables (adjusted for age, sex, and BMI). In our ET cohort, JAK2-V617F showed significant correlation with the presence of DVT (OR 10.08; 95%CI 1.15–88.49; P=0.01). Both FII-G20210A and FV-Leiden were rare in this cohort (2% and 4%, respectively) and thus did not show any significant associations. Homozygosity for the 298Asp variant of NOS3 gene (encoding the endothelial nitric oxide synthase) showed significant correlation with stroke (OR 46.75, 95%CI 1.38–147, P=0.014). Similarly, homozygosity for the 131Arg variant of Fc receptor gamma 2A (FCGR2A) involved in platelet GPVI signaling correlated with the presence of SVT (OR 9.46, 95%CI 1.48–60.82, P=0.014). Our results indicate that genetic factors other than JAK2-V617F, FII-G20210A and FV-Leiden contribute to thrombotic risk in ET. Furthermore, the nitric oxide and the platelet GPVI signaling pathways are likely to play an important role in the pathogenesis of thrombosis in ET.

Changes in social interactions in contemporary society limit the transmission of the traditional play as a significant segment of cultural intangible heritage. It is assumed that the introduction of the traditional play in the institutional Early Childhood Education and Care (ECEC) would contribute to its preservation.The opinions of teachers and parents of children of early and preschool age on the importance and purposefulness of the application of traditional play in the educational process were examined.The importance of traditional play was assessed more significantly by participants of older than younger age, and those highly educated than those less educated.The place where the participants grew up was related to the assessment of the limitations of the traditional play. Assessing outdoor play without adult supervision positively correlates with assessing children’s preoccupation with the virtual world. Conducting action research on the possibility of implementing traditional play, along with longitudinal research on children’s development, could possibly indicate the importance of traditional play.

Abstract:
Passion plays in Europe exhibit a great many similarities as far as general content and presentation are concerned. I do not really think it is relevant for the non-Dutch reader, therefore, to delve too deeply into the specifics of content and presentation with regard to the passion plays in Tegelen in Limburg, the southernmost province of the Netherlands. However, a great deal of the external interference surrounding the passion plays in Tegelen, has also been experienced by the organisers of numerous passion plays in Europe. Therefore, rather than discuss their content, it is far more interesting to investigate these passion plays through their development from 1931 when they began. Through this we can gain insight into the manner in which a social and cultural phenomenon such as a passion play can play a role in the maintenance and even the imposition of a particular world view. For the quality of the text and performance of passion plays are less important than the manner in which a community, usually a small one, driven by a communal ideal, expresses religious feelings which tens of thousands of spectators share. In such a performance there is communal activity with a ritual character. Performers are frequently anonymous and spectators and critics set aside any criticism of individual acting or the text because a passion play entails more than merely a large-scale theatrical performance by enthusiastic amateurs.

This paper describes an IPTV set-top box video player based on chip BCM7466, simplifies the design and improves performance, realizes interaction between browser and streaming media server and plays the video stream with specified formats. It introduces the design scheme of STB software, a detailed description of the design and implementation process of each module, service control, current client, stream processing and control core. By testing, this player is stable.

In this paper the authors wish to examine kinesthetic forms of learning involving the body and the physical realm. The authors look at two particular techniques; using Legos to build metaphorical models and living the physical experience of metaphors in the shape of labyrinth-walking and its attendant activities. The authors begin by discussing their experiences using LEGO building bricks as a reflective tool. While LEGO lends itself particularly effectively to metaphorical modeling (not least through its status as a globally known iconic toy and connection to childhood) the process can take place using any set of objects that are used to represent something other than their real nature. This will be apparent to anyone who has sat at in a restaurant and used the salt and pepper cellars to describe a relationship, car maneuver, choice between two options, altercation or offside rule in Soccer. Buttons, sticks, candles, pots, peas, matches, or any other assortment of items which the user finds sufficiently rich to embody their ideas and convey their intentions work just as well. The point is that the user assigns specific meanings to the materials to illustrate some sort of process or relationship.

Video game interfaces featuring multiple distinct icons that enable a player to quickly select specific actions from a larger set of possible actions have the potential to inform the development of interfaces that enable a single operator to control multiple unmanned vehicles (UVs). The goal of this research was to examine the design of a video game inspired interface for delegating actions (called “plays”) to highly autonomous UVs. Specifically, the impact of color coding (by Play Type, by Vehicle Type, and No Color) and icon row assignment (by Play Type, by Vehicle Type, and Random) for a delegation play calling interface was evaluated in terms of participants’ performance in identifying and manually selecting a commanded play icon in an interface depicting a large set of UV plays. Both the objective performance data and subjective ratings indicated that icon row assignment impacted icon selection, whereas color coding did not. Mean icon selection time and subjective ratings were more favorable when the icons were assigned to rows in the Play Calling interface by vehicle type. Suggestions are made for follow-on research.

Abstract Asef Soltanzadah is one of the most thought-provoking Afghan writers. His work, set exclusively during wartime, may be characterized by both seriousness and playfulness. To borrow Warren Motte’s words, “playing in earnest” is his literary signature. Yet, he occupies a marginal place within the institution of world literature not only because he writes in Persian but also because he is minimally translated and read. In this article, I turn to two of his short stories featuring a game of cards and kite flying, setting them into conversation with theories of play. I argue that by creating a space for play, Soltanzadah brings into visibility and reflects on the process, promise and risk of transforming mere life into life world within the time and space of war, challenging the theoretical framing of play in relation to the real world while questioning the possibility of worlding in the time of war.

Serious gaming is one of the newest developments in the world of learning and is gaining increasing attention within the business environment. Although many practitioners claim that serious gaming has more impact on demonstrated behaviour of trainees when compared to common presentations, little evidence exists. In this paper, the authors present an experiment that builds a foundation for proving that serious gaming is more effective than presentations in ensuring people demonstrate a specific set of behaviours within a training setting. The experiment involved addressing the same content a number of times by either holding a presentation or playing a serious game. Consequently, participants' behaviour was measured within a business simulation environment. Results demonstrate that experiencing failure is an important element of learning and that most learning occurs during a game debriefing when participants reflect on their experiences. The importance of learning elements like goal setting, feedback and challenge is demonstrated as well.

The purpose of this article is to present the person of Al-Saadi Gaddafi, the third son of a dictator who ruled Libya during the years 1969–2011 – Mu’ammar Al-Gaddafi.In the first part of the article, the author writes about the first stage of the life of the dictator’s son, when Al-Saadi Gaddafi, thanks to his father’s acquaintances, enters the world of football, among others through demonstration training sessions at the Italian club Lazio Roma, where it was hoped that over time he would acquire skills to be able to play football in this club. In this part of the article, the author explains why this plan failed.In the next part of the article, the author presents the backstage of Al-Saadi Kaddafi’s tempestuous career first in Libyan football and then in the highest class of the Italian Serie A connected with such clubs as AC Perugia, Udinese Calcio and UC Sampdoria as a footballer and with Juventus Turin as one of the club’s officials and shareholders.In the final part of the article, the author focuses on the fate of Al-Saadi Gaddafi after completion of his football career and presents what happened to him after the outbreak of the so-called Arab Spring as a result of which the Gaddafi family lost their power in Libya.

AbstractThe study analysed the set-to-set variation in performance using match statistics of 146 completed main-draw matches in Australian Open and US Open 2016-2017 men’s singles. Comparisons of technical-tactical and physical performance variables were done between different sets; and the within-match coefficients of variation (CV) of these variables were contrasted between match winning and losing players. All comparisons were realized via standardized (Cohen’s d) mean differences and uncertainty in the true differences was assessed using non-clinical magnitude-based inferences. Results showed that there was possibly to very likely decreases in the serve, net and running related variables (mean difference, ±90%CL: -0.16, ±0.14 to -0.45, ±0.24, small) and an increase in the return and winner related variables (0.17, ±0.24 to 0.24, ±0.14, small) in the last sets when compared to the initial sets, indicating the influence of match fatigue and the player’s choice of match tactics and pacing strategy. Besides, winning players were revealed to have lower CV values in most of performance variables (-0.16, ±0.24 to -0.82, ±0.23, small to moderate) except for the second serve, winner, and physical performance variables (0.25, ±0.26 to 1.6, ±0.25, small to large), indicating that they would sacrifice the consistency to gain more aggressiveness and to dominate the match.

We propose a model to generate a group of artificial lives capable of coping with various environments which is equivalent to a set of requested task, and likely to show that the plays or hobbies are necessary for the group of individuals to maintain the coping capability with various changes of the environment as a whole. This may be an another side of saying that the wide variety of the abilities in the group is necessary, and if the variety in a species decreased, its species will be extinguished. Thus, we show some simulation results, for example, in the world where more variety of abilities are requested in the plays, performance of the whole world becomes stable and improved in spite of being calculated only from job tasks, and can avoid the risk of extinction of the species. This is the good effect of the play.
