The shoulder is a complex joint with static and dynamic stabilising structures working synchronously. These allow a full range of movement while preserving stability of the joint. Patients may present with pain, stiffness, weakness, deformity or instability. The authors suggest a systematic examination sequence to ensure that important pathology is not overlooked. Adopting this approach allows common pathologies, including tears of the rotator cuff, impingement and tendinopathy, to be easily identified. This shoulder examination sequence may be used by all healthcare professionals and can also act as a revision aid for those undergoing exams in this field, at different levels of training.

Introduction: Delayed diagnosis and treatment might cause gangrene perforation and diffuse peritonitis. This makes radiological exams easy and sensitive and specific. The radiological examination, which includes ultrasound and picture, can be done quickly. In addition to difficult exams like CT scans and barium enemas, these are done. The aim: This study review imaging for appendicitis. Methods: For this systematic review, publications that were published from 2012 to 2023 were taken into account during the search process. This was achieved through the utilization of numerous online reference sources, such as Pubmed and SagePub. The decision was made to exclude review articles, previously published works, and incomplete works. Result: We found seven studies related to the ability of radiological examination in the diagnosis of appendicitis. The research discusses CT scans and ultrasonography. Conclusion: There are several examinations that can be done, for example US, plain radiology and CT scan. An ultrasound examination is an examination that can be carried out quickly and with sufficient accuracy to determine whether a person can be diagnosed as a patient with appendicitis.

AbstractLaws regulating patient care are an essential component of protecting patients and doctors alike. No studies have previously examined what laws exist regarding pelvic examinations in the United States (US). This study systematically reviews and compares regulation and legislation of pelvic examinations in the U.S. and provides a comprehensive resource to educate clinicians, patients, and lawmakers. Each of the fifty States in the U.S. was included. The primary outcome was existence of any pelvic or rectal exam laws. Data was obtained for the type of examination defined within the law, exceptions to the law, to whom the law applied to, the type of consent required, and to whom the consent applied to. Laws were identified from each of the individual state legislative websites. All sections of each law pertaining to pelvic examination were reviewed and organized by state. Descriptive statistics were performed for each of the variables, including frequencies of each amongst the fifty states. State regulation for pelvic examinations varied from no law or regulation to laws pertaining to pelvic, rectal, prostate, and breast examination performed in any context. As of November 22, 2022, there are twenty states (40%) with pelvic examination laws applying to anesthetized or unconscious patients. Thirteen additional states (26%) have proposed pelvic exam laws. Seventeen states (34%) do not have any laws regarding pelvic examinations. Regulation of pelvic examinations has become an increasingly important issue over the past few years in response to growing concerns of patient autonomy and the ethical issues raised by such sensitive examinations. While pelvic examination laws that balance protection for patient autonomy and the needs of caregivers and educators exist in much of the U.S., more work needs to continue in consultation with physicians and health care providers to ensure that all states have reasonable laws protecting the autonomy of patients while also maintaining quality of care.

ABSTRACT Background In-training examinations (ITEs) are intended for low-stakes, formative assessment of residents' knowledge, but are increasingly used for high-stake purposes, such as to predict board examination failures. Objective The aim of this review was to investigate the relationship between performance on ITEs and board examination performance across medical specialties. Methods A search of the literature for studies assessing the strength of the relationship between ITE and board examination performance from January 2000 to March 2019 was completed. Results were categorized based on the type of statistical analysis used to determine the relationship between ITE performance and board examination performance. Results Of 1407 articles initially identified, 89 articles underwent full-text review, and 32 articles were included in this review. There was a moderate-strong relationship between ITE and board examination performance, and ITE scores significantly predict board examination scores for the majority of studies. Performing well on an ITE predicts a passing outcome for the board examination, but there is less evidence that performing poorly on an ITE will result in failing the associated specialty board examination. Conclusions There is a moderate to strong correlation between ITE performance and subsequent performance on board examinations. That the predictive value for passing the board examination is stronger than the predictive value for failing calls into question the “common wisdom” that ITE scores can be used to identify “at risk” residents. The graduate medical education community should continue to exercise caution and restraint in using ITE scores for moderate to high-stakes decisions.

Examinations or assessments play a vital role in every student’s life; they determine their future and career paths. The COVID pandemic has left adverse impacts in all areas, including the academic field. The regularized classroom learning and face-to-face real-time examinations were not feasible to avoid widespread infection and ensure safety. During these desperate times, technological advancements stepped in to aid students in continuing their education without any academic breaks. Machine learning is a key to this digital transformation of schools or colleges from real-time to online mode. Online learning and examination during lockdown were made possible by Machine learning methods. In this article, a systematic review of the role of Machine learning in Lockdown Exam Management Systems was conducted by evaluating 135 studies over the last five years. The significance of Machine learning in the entire exam cycle from pre-exam preparation, conduction of examination, and evaluation were studied and discussed. The unsupervised or supervised Machine learning algorithms were identified and categorized in each process. The primary aspects of examinations, such as authentication, scheduling, proctoring, and cheat or fraud detection, are investigated in detail with Machine learning perspectives. The main attributes, such as prediction of at-risk students, adaptive learning, and monitoring of students, are integrated for more understanding of the role of machine learning in exam preparation, followed by its management of the post-examination process. Finally, this review concludes with issues and challenges that machine learning imposes on the examination system, and these issues are discussed with solutions.

Courts in England, Wales and Northern Ireland have identified children and adults with intellectual disabilities (ID) as vulnerable witnesses. The call from the English Court of Appeal is for advocates to adjust questioning during cross-examination according to individual needs. This review systematically examined previous empirical studies with the aim of delineating the particular communication needs of children and adults with ID during cross-examination. Studies utilising experimental methodology similar to examination/cross-examination processes, or which assessed the communication of actual cross-examinations in court were included. A range of communication challenges were highlighted, including: suggestibility to leading questions and negative feedback; acquiescence; accuracy; memory and understanding of court language. In addition, a number of influencing factors were identified, including: age; IQ level; question styles used. This review highlights the need for further research using cross-examination methodology and live practice, that take into consideration the impact on communication of the unique environment and situation of the cross-examination process.

The aim of this study is to conduct a systematic review of the studies on ensuring body privacy during patient examination or intervention. In the systematic review, 1821 studies conducted between 2015 and 2023 within the scope of patient examination and intervention and by entering the keywords 'Patient privacy, Privacy, Private Life, Patient privacy' in the electronic databases of National Academic Network and Information Center, Higher Education Institution Theses, Pubmed, Google Scholar were scanned. A total of 6 studies that met the inclusion criteria were reached and compared in terms of their results. It was stated that the inpatients thought that their privacy was taken care of by the staff of the institution, the patients who participated in the research observed that the care shown to their privacy was better in private hospitals than in public hospitals, the average level of privacy knowledge of the patients was 73.9%, the average level of privacy evaluation was 81.6%, and the inpatients thought that their privacy was taken care of by the staff of the institution. It was found that the use of apron during the intervention to the patients increased the patient's sense of protection of privacy. In ensuring patient privacy; patient confidentiality all healthcare personnel who work in different institutions by showing the same attention to treating patients may contribute to the increase of level of privacy. It may be recommended to develop different aprons to protect their body privacy during the procedures applied to patients

Introduction American Board of Surgery In-Training Examination (ABSITE) performance has become an important factor when monitoring resident progress. Understanding which prospective factors predict performance can help identify residents at risk. Methods A literature search was conducted searching PubMed, EMBASE, and JAMA Network from June 2011 to June 2021, in accordance with the PRISMA guidelines. Searches were performed for the terms “ABSITE” and “American Board of Surgery In-Training Examination.” Prospective factors such as prior examination performance, clinical evaluations, and demographics were evaluated. Results A final 35 studies were included. The prospective factor most consistently found to predict ABSITE performance is performance on prior knowledge-based examinations such as the USMLE step exams. The ACGME Medical Knowledge 1 milestone evaluation also appears to correlate to ABSITE performance, although clinical evaluations, in general, do not. Demographics have no significant correlation to ABSITE performance. Discussion Using performance on prior knowledge-based examinations programs may be able to identify residents at risk for failing ABSITE. It may be possible to initiate early intervention before rather than only remediation after poor performance.

In spite of speedy development of technical tools for examination, the importance of macro structural investigation has not decreased; on the contrary, these methods are in the state of their revival. One of evident reasons for it is the fact, that the results of macro structural examinations and the conclusions drawn from them are in the closest relation with the parameters of technology, reflect their changes, and the deviations from the specified values. It is expedient from time to time to check the methods proved earlier suitable for macro structural investigation, possibly to develop and implement new methods. We have to take in account that the methods now widely applied were developed when the technology, composition, type and quantity of pollutants were still basically different from those of the present. We mention as an example that in the fundamental work of George F. Vander Voort published in 1984 the examinations of macrostructure of continuously cast semi products occupied hardly one to two pages, while nowadays they produce about 90% of the annual 1.1 billion tonne steel quantity in continuous casting plants, thus the decisive proportion of continuously cast semi products examination can hardly be questioned. Taking in consideration the above points of view, in the frame of a consortium work we examined the macrostructure of samples taken from bloom slabs cast on vertical continuous casting machine from nine consciously chosen charges, with methods known from the literature or personal information.

Abusive head trauma (AHT) represents a commonly misdiagnosed condition. In fact, there is no pathognomonic sign that allows the diagnosis in children. Therefore, it is such an important medico-legal challenge to evaluate reliable diagnostic tools. The aim of this review is to evaluate the current scientific evidence to assess what the best practice is in order to diagnose AHT. We have focused particularly on evaluating the importance of circumstantial evidence, clinical history, the use of postmortem radiological examinations (such as CT and MRI), and the performance of the autopsy. After autopsy, histological examination of the eye and brain play an important role, with attention paid to correlation with symptoms found in vivo.

It is designed for advocates who do not have significant experience in cross-examination in international arbitration, or otherwise, especially advocates with a background and training in legal systems that do not have a tradition of cross-examination.

It was stated that the inpatients thought that their privacy was taken care of by the staff of the institution, the patients who participated in the research observed that the care shown to their privacy was better in private hospitals than in public hospitals, the average level of privacy knowledge of the patients was 73.9%, the average level of privacy evaluation was 81.6%, and the inpatients thought that their privacy was taken care of by the staff of the institution. It was found that the use of apron during the intervention to the patients increased the patient's sense of protection of privacy. In ensuring patient privacy; patient confidentiality all healthcare personnel who work in different institutions by showing the same attention to treating patients may contribute to the increase of level of privacy. It may be recommended to develop different aprons to protect their body privacy during the procedures applied to patients.

The profile in the frontal section of a solid of revolution forms a perfect circle if all of its points are at an equal distance from the center line. The value of the deviation from circularity is very important and it is measured with a dedicated measuring device. This paper examines the systematic errors in a measuring device in which the measured workpiece is supported and rotated between two live centers, while a dial indicator detects the deviations. Regarding the structure, this paper proposes a model consisting of five systematic error measurements which considers the eccentricity of the live centers and the tilt of the dial indicator axis.

This study aims to reveal the current situation viz. as of year 2024 in experimental educational studies conducted using Google Cardboard as a virtual reality tool. In the study, 36 articles published in journals indexed in Web of Science and Scopus databases and decided to be suitable for the research purpose were analyzed. The data were analyzed by content analysis method. According to the results obtained in the research, while the number of articles increased until 2020, it has been decreasing since this year reaching the year 2024. While the most preferred sample group was the undergraduate level, studies were carried out with the most participants between 11-30. While it is mostly used in STEM education, the number of in-school and out-of-school applications is close to each other. While survey/scale was the most preferred data collection tool, it was concluded that different media types were used in the studies. The advantages of the studies were summarized and analyzed in detail under 3 sub-themes: educational outcomes, affective outcomes and technical advantages. Whereas the most important educational outcome underlined in the articles is increases success (performance), the most important affective outcome is students develop positive attitudes. It is noteworthy that technical advantages are mentioned less frequently. On the other hand, it was concluded that some difficulties were encountered, such as eye strain and headaches.

In order to explore the value of prenatal systematic ultrasound examination of fetal structural abnormality in the diagnosis of fetal structural abnormality, this paper retrospectively analyzed 200 cases of fetal abnormality referrals from December 2017 to December 2019 in the prenatal systematic ultrasound examination of the designated hospital in this study of pregnant women. By sorting, summarizing and summarizing the actual conditions of the systematic ultrasound examination of each prenatal fetal abnormal section, this study calculated, recorded and analyzed the detection rate, missed diagnosis rate, sensitivity, and specificity of the systematic ultrasound examination of structural abnormality, and then discussed the value of prenatal systematic ultrasound in the diagnosis of fetal structural abnormality. The results showed that, 185 cases were detected out of 200 fetal abnormality with the detection rate of 92.5%; missed diagnosis rate is 7.3%; detection sensitivity is 87.43%; and specificity is 76.58%. Among them, the detection rate of malformations of different diseases from high to low is cardiovascular system (97.54%), nervous system (93.16%), respiratory system (90.62%), facial system (87.54%), and urinary system (85.47%). Therefore, the prenatal systematic ultrasound fetal structural abnormality examination for pregnant women can achieve good diagnostic results, can provide pregnant women with fetal structural abnormality the opportunity to terminate pregnancy early, and effectively reduce the birth of structural abnormal fetuses and the burden of family and society, having important clinical significance and reducing.

Ultrasonography (USG) of the breast generally used for inspection example cyst breast, and the shape of the cyst for breast abnormalities. FNAB is a fine needle biopsy examination and is a preoperative diagnostic tool for breast lesions. Anatomical histopathology is the gold standard for the diagnosis of breast malignancy. Objective: To find out the differences and compare the results of breast tumor examination using ultrasound and FNAB examination Method: This study uses a type of literature review study a Narrative Review design. The method used in the process of using the ultrasound, it can be seen that the gray scale ultrasound examination has a sensitivity of 75-94.4% and a specificity of 18.8-81.8%. Result and Discussion: While PPV 48-95.6%, NPV 42.8-95.7%, and 46.9% accuracy and fine needle examination also has varying accuracy criteria. Its show a sensitivity of 86.7%- 93,4% Specificity 95,7% -100%. Conclusion: Fine Needle Aspiration examination has better specitifity than ultrasound examination.

A systematic approach to the acute traumatized wrist is of importance to achieve early diagnosis, with efficient use of advanced imaging modalities. In a 6-month period 641 patients aged 15 years or over were examined using an algorithm for acute wrist trauma in order to secure early and appropriate treatment. In 293 (46%) cases routine radiographic examination supplied sufficient information to establish a fracture diagnosis. All 33 scaphoid fractures (including five scaphoid non-unions) were demonstrated on the initial X-ray examination. In 56 patients (9%) scintigraphy proved useful for selecting cases in need of further radiographic examination. Among 31 patients with focal activity on bone scintigraphy, CT and/or tomography revealed eight distal radial fractures, one fracture of the ulnar styloid, and eight fractures of carpal bones. Fractures were revealed using tomography and/or “hot spot views” in patients with negative CT examinations suggesting that further examinations should be performed in cases with increased focal activity on bone scintigraphy and a negative CT examination.

Background: Breast reconstruction is a standard procedure in postmastectomy plastic surgery. The necessity of routine histological examinations for mastectomy scars during delayed reconstruction remains a topic of debate. We evaluated the need for histological examination of scars during delayed breast reconstruction. Methods: We conducted a systematic review using PubMed, TDnet, and Cochrane Central in August 2023. Inclusion criteria involved delayed breast reconstruction with histological scar analysis and malignancy reporting. Exclusion criteria encompassed noncancerous breast diseases, prophylactic mastectomies, articles lacking relevant information, case reports, technique descriptions, and reviews. We independently assessed articles. Differences in recurrence rates were determined using a Z-test for proportions. A linear regression model explored the relationship between reconstruction timing and pathological results. The number needed to treat was calculated based on the literature. The Wilcoxon test was used to compare mean reconstruction times and postreconstruction follow-up between groups. Results: Our analysis covered 11 retrospective observational studies published between 2003 and 2018, including 3754 mastectomy scars. The malignancy recurrence rate was 0.19%, consistent with previous reports, with a number needed to treat of 144.93–188.68 patients. The timing of breast reconstruction postmastectomy averaged 19.9 months, without statistically significant association between reconstruction timing and recurrence rates. Postreconstruction follow-up periods ranged from 60 to 87 months. The postreconstruction adverse outcomes ratio was 2.21%. Conclusions: Assessing the necessity of histological examination in breast reconstruction is complex. Based on the literature and this study, we do not recommend routine histological examination of mastectomy scars during delayed reconstruction. A selective approach based on risk factors may be beneficial, warranting further research.

Establishing fresh cow monitoring procedures is considered beneficial for cow health, welfare, and productivity. However, they are time consuming and require the cows to be locked up, which restricts their natural behavior. In this study, different fresh cow monitoring procedures were evaluated. Two experiments were conducted to determine: (1) the duration of various examinations and treatments; (2) the time cows remain locked up in headlocks; and (3) the proportion of examination and treatment times relative to the total headlock time. In advance, standard operating procedures were established. Three veterinarians conducted the examinations and treatments based on changes in milk yield, clinical symptoms, and alarms by an accelerometer system. The headlock time was evaluated for three workflow strategies, which differed in the order of examinations and treatments. To determine the duration, cameras were installed, and the video footage was analyzed. The examinations lasted between 1 and 227 s, and the cows were locked up in headlocks between 0.01 and 1.76 h. The lock-up times differed significantly among the three strategies, as well as the proportion. This study provides information that can be used as a basis for the development of time-efficient strategies, and to minimize the impact on cows’ time budgets.

The number of US exams has nearly doubled in the last ten years. Many researchers point out the probe pressure force influence on image quality and other aspects of examination. This review aims to identify the range of applied probe pressure during US examinations and gather information on probe compression force values during various US examinations (examination types, body regions, etc.). Methods: A systematic review following PRISMA guidelines was conducted using IEEE Xplore, Web of Science, Scopus, and PubMed/MEDLINE. Studies with quantitative data on probe pressure during US by human operators or RUSs (robotic ultrasound systems) were included. Results: From the 26 included studies, force ranges varied up to 34.5 N for abdominal exams. Robotic systems applied slightly higher maximum forces (34.5 N) than human operators (30 N). Most studies reported positive impacts of force monitoring on image quality and diagnostic precision, with no adverse effects on patient comfort. Conclusions: The evidence collectively emphasizes the critical role of applied pressure in US. The nonuniformity of the reviewed studies does not allow for identifying a clearly defined range of probe pressure forces or force monitoring protocols. Integrating RUS and standardized pressure protocols could improve diagnostic consistency and accuracy.

These guidelines were developed by a number of different organizations. The pelvic examination has been acknowledged as an essential component of the well-woman visit ever since ancient times. This session is seen by many women and gynecologic care providers as an opportunity to discuss sexual and reproductive health problems with their patients. Consequently, many women take advantage of this opportunity. A pelvic examination is typically performed on asymptomatic women as a screening tool for gynecologic cancer, infection, and asymptomatic pelvic inflammatory disease; despite evidence to the contrary, some obstetrician&ndash;gynecologists and patients consider it to be important in detecting subclinical disease. a pelvic examination is typically performed on asymptomatic women as a screening tool for gynecologic cancer, infection, and asymptomatic An evaluation of the patient&#39;s external genitalia, an examination of the patient&#39;s internal genitalia with the use of a speculum, rectovaginal examination, and bimanual palpation are all potential components of the pelvic examination. Background: Schistosome egg deposition in pregnant women may affect the placenta of infected mothers and cause placental schistosomiasis (PS). Histopathological examination of placental tissue is an inadequate detection method due to low sensitivity. So far, there has not been any systematic review on PS. Methods: We conducted a systematic literature search on PubMed, EMBASE, and Medline and included all publications that reported microscopically confirmed cases of PS, as well as the relevant secondary literature found in the citations of the primarily included publications. Results: Out of 113 abstracts screened we found a total of 8 publications describing PS with a total of 92 cases describing egg deposition of dead and/or viable eggs and worms of S. haematobium and S. mansoni in placental tissue. One cross-sectional study investigating the prevalence of PS and its association with adverse birth outcomes, found 22% of placentas to be infested using a maceration technique but only &lt;1% using histologic examination. Additionally, no direct link to deleterious pregnancy outcomes could be shown. Conclusions: PS is a highly unattended and underdiagnosed condition in endemic populations, due to a lack of awareness as well as low sensitivity of histopathological examinations. However, PS may play an important role in mediating or reinforcing adverse birth outcomes (ABO) such as fetal growth restriction (FGR) in maternal schistosomiasis, possibly by placental inflammation.
