The neurological manifestations of systemic lupus erythematosus are protean: headache, affective disorders, cognitive dysfunction, seizures, strokes, psychosis, acute confusional states, myelopathies, chorea, mimics of demyelinating disease, meningitis, polyneuropathy, mononeuropathy or mononeuritis multiplex, cranial neuropathies, autonomic dysfunction, Guillain-Barre syndrome, or myasthenia gravis make an incomplete list. Each neurological manifestation needs to be analyzed separately to understand pathogenesis, possible relation to primary lupus-related inflammation and vasculopathy, and optimal treatment.

Abstract:
Systemic lupus erythematosus is a chronic multisystem autoimmune inflammatory disorder of unknown etiology. Numerous abnormalities within the innate and adaptive immune system have been described. The hallmark of this disease is B-cell hyperactivity resulting in autoantibody production, abnormal T-cell function, impaired clearance of immune complexes (resulting in their deposition in tissues), complement activation, and defective cellular apoptosis. However, these abnormalities of the immune system are not uniform across patients or within the same patient at different stages of the disease, resulting in heterogeneity in its presentation and progress.

Systemic lupus erythematosus (SLE) is a systemic, autoimmune disease that can affect any part of the body, causing the immune system to attack the body’s cells and tissue, and resulting in inflammation and tissue damage. It is characterized by the presence of autoreactive B and T cells and the production of a broad, heterogeneous group of autoantibodies (autoAb); the absence of a unique presentation makes its diagnosis difficult, even for qualified clinicians. Systemic Lupus Erythematosus focuses on providing a practical approach to the assessment and management of patients with this complex, multisystem, autoimmune disease, in order to improve the diagnosis and treatment of the disease and its complications. It provides detail on the history and epidemiology of SLE alongside comprehensive sections on clinical features, treatment, and special situations. As well as detailing the challenging management issues of SLE, this title provides an overview of the numerous investigations specific to the condition, assessment of disease activity, symptomatic treatment, patient education, and biologic therapies. A specific section on juvenile-onset systemic lupus erythematosus also provides the practising clinician with the knowledge needed to manage this distinct and aggressive stage of SLE.

The role of early atherosclerosis and cardiovascular disease as a cause of death in patients with SLE is increasingly recognized and will present further challenges in the future.

Abstract:
Juvenile-onset systemic lupus erythematosus (JSLE) is similar to adult-onset SLE, but there are distinct differences in clinical features, serology, and management requirements. It is more aggressive than adult-onset SLE with frequent renal and haematological manifestations and higher mortality rates. The cause of JSLE is unknown but appears to be multifactorial with genetic, immunological, hormonal, and environmental influences. Macrophage activation syndrome is a potentially life-threatening complication, and may mimic the underlying disease or be confused with sepsis. Transferring care from paediatric to adult care can be a difficult milestone and should be tailored to the individual patient. Management requires a multisystemic, holistic approach with recognition of psychosocial factors that occur during normal childhood and adolescence. International collaboration and further research is needed to optimize care for these patients.
