Salah satu tokoh yang paling konsen dalam menciptakan kerukunan umat beragama di Indonesia adalah Tarmizi Taher. Melihat situasi keberagaman masyarakat di Indonesia, Tarmizi Taher berpendapat bahwa kerukunan antar umat beragama dapat diwujudkan jika masing-masing agama mengakui prinsip umum sebagai landasan bersama dalam merespon situasi keanekaragaman.

Abstract:
Abstract Introduction Iron (Fe) overload is directly related to the morbidity & ultimate survival. NTDT is different. Overloading occurs from ineffective erythropoiesis, dietary absorption & rescue transfusions (Taher-Cappellini, 2009, p 634-40), the result - organ dysfunction related morbidity and mortality. Other than Serum ferritin (SFr) (Origa-Galanello, 2007, p583-8) MRI has been used for organ specific Fe content estimation (Taher-Musallam, 2010, p 288-90). We will try to establish Fe overload exists in subjects of HbE β NTDT & assess the best method for Fe overload in patients & it's relation to increasing age. This is the 1st attempt to study Fe overload in HbE β NTDT patients of India. Study proper Diagnosed by HPLC and confirmed by Sanger sequencing, 105 NTDT HbE β thalassaemia patients of either sex, -ve for hepatitis B, C & HIV, with age ranging of 4 - 55 years, receiving <1 unit to a maximum of 15 transfusions till date, were included. SFr was tested by CLIA and LIC by MRI from Resonance Health, Ferriscan (St Pierre-Clark, 2005, p 855-61). Age, SFr, LIC and complications were collected & their interrelationship analyzed statistically. Results & Discussion Out of the 105 subjects, 51(48.5%) were males, mean age ± SD was 19±9.5 yrs. (range 4 - 54); 41.8% were splenectomized; 31% were transfusion naive & none had received prior chelation. Mean SFr was 1211.9 ± 2048 ng/ml, mean LIC was 11.76 ± 12.41 mg / g DLT. The correlation between age & LIC - 0.159; age & SFr - 0.173 and LIC & SFr - 0.62. Study shows despite the NTDT status, HbE β thalassaemia patients develop Fe overload as evidenced by SFr level > 200-450 ng/ml & LIC by MRI s >3 mg/g of DLT in 63.8%. In this study, mean LIC was 11.76 ±12.41 mg/g DLT, significantly lower than base line of 13.8± 7.6 mg/g DLT, in THALSA (Taher-Porter, 2014, p 521-26) and 14.5±8.8 mg/g DLT (Taher-Porter, 2012, p 970-7). In 61% LIC was >5 mg/g DLT, but SFr <800 ng/ml in 44.6%; in 7.5% LIC <5 mg/g DLT, but SFr level was > 800 ng/ml. So SFr, may lie on both sides of LIC, and is not to be interpreted in isolation. SFr levels may underestimate Fe load in NTDT (Taher-Hershko, 2009, p 634-40), this study shows that it may be higher. In thalassaemia intermedia (TI) patients, advancing age is associated with > SFr and serious morbidities (Taher-Musallam, 2010, p 486-9), but not universally, this study showed that, 83% recorded increased LIC with increaseing age in subjects >15 yrs., 79% age >10 yrs. yearly increase in LIC, & total body Fe was 0.144 mg/g DLT/yr. or 0.57 - 2.49 mg/kg/yr. lower than 1.0 - 3.5 g/yr. by other authors (Taher-Vipraksit, 2013, p 409-15),could be due to Fe deficient diet & black tea consumption. As reported others, +ve correlation between SFr & LIC was observed in NTDT (Taher-Porter, 2012 p 970-77; Taher-Rassi, 2008, p 1584 - 86), high +ve correlation between SFr & LIC (0.81), when SFr level was < 300 ng/ml & LIC of <3 mg/g DLT, in age group ≤10 years in both sexes, also true in >30 yrs. group, +ve correlation of 0.849, but no linear correlation was obtained. In 8 diabetic patients , all had LIC > 6 mg/g DLT but 37.5% had SFr >2500 ng/ml, value of SFr above which complication occurs, so LIC is better predictor of complications. This was comparable to the study among 168 patients of β TI, LIC was ≥ 6mg/g DLT, suffered from endocrine/bone disease & 1mg increase in LIC was independently associated with significantly increased risk of vascular, endocrine & osteogenic morbidities (Musallam-Cappellini, 2011, p 1605-12), this is true for patients with LIC > 6mg/g DLT, as in our study. Conclusion This study concludes that even in the absence of transfusion, Fe overload occurs in NTDT. SFr, an invasive procedure, is a bad predictor of Fe overload, especially in NTDT, it may under / over estimate Fe overload. LIC estimation by MRI is a better predictor of total body Fe than SFr. It bears correlation if SFr is below 300 ng/ml & that of liver Fe is <3mg/g DLT. Iron overload increases with age but the rate can be controlled with Fe deficient diet & regular tea intake. Complications also occur in NTDT from Fe overload & is better predictable by LIC estimation than by SFr levels. Disclosures No relevant conflicts of interest to declare.

Abstract:
This study aims to explore agenda empowerment of people in Tarmizi Taher point of view. Tarmizi Taher in his book harmony of religious life and study of religions, that religious harmony can be formed by supporting harmonious religious communities, it is necessary for clerics, da’i (preachers), priests, priests and other religious leaders to instill to people about the inevitability of religious pluralism in social life. That the diversity of religions is a fact that cannot be denied. So the consequence is that every religious community has an obligation to recognize and respect other religions, without the need to elevate or demean a religion. The result shows that in Tarmizi Taher's view, community empowerment can be done by providing skills education conducted by the government so that people can compete with the advancement of science and technology.

This article is based on an attempt to introduce and study a new-found manuscript containing four Hadith books: "Tohfa-al-seddigh, Fasl-al-khetab, Ons-al-lohfan, Matlubon-kolle-taleb." These four Hadith books include Hadiths of the first four caliphs, interpreted by Rashid-al-din Vatvat; distinguished writer of sixth century in Kharazmshahian court. The remarks of two hadith books (Fasl-al-khetab and Matlubon–kolle-taleb) are also changed into persion poem. With a thematic point of view, some scribes have collected these four Hadith books into one book and named it: " Remarks of the four Chosen fellows" or " Hundred remarks of the four prophets fellows". This manuscript is written in Naskh in the same style by Taher.

Maghamate Lozumiyeh by Abu Taher Serghesti is one of the most important literary sources of Andalusia literature and time element is one of the most striking elements that is revealed in these Maghamat. Therefore, this element is significant due to an important role in narrated works in revealing the meaning. That is why it has attracted many critics and researchers’ attention. Hence, the basic issue in narrated context, is the time problem. Gérard Genette’s research is one of the most important scientific projects in this field that distinguished in three dimensions between time of the story and the anecdote and they are: order, continuity and sequence. The author relying on the analytical method and with regard to Genette and his followers’ scheme has analyzed time element in these Maghamat. From the results of this article we can conclude that in most of Maghamate Lozumiyeh, order links have got schematic balance in a way that the incidents are compatible with reality temporally. As Maghamat’s incidents are not repeated because of framework’s limitations and the article text size and the narrator has replaced some other expressions instead. However, comparing to continual links, Maghamat are full of two techniques of Liberating and scenery.

Abstract The aim of this paper is to present a feasibility study of a grid connected photovoltaic (PV) and biomass Integrated renewable energy (IRE) system providing electricity to rural areas in the Beni Suef governorate, Egypt. The system load of the village is analyzed through the environmental and economic aspects. The model has been designed to provide an optimal system configuration based on daily data for energy availability and demands. A case study area, Monshaet Taher village (29° 1′ 17.0718”N, 30° 52′ 17.04”E) is identified for economic feasibility in this paper. HOMER optimization model plan imputed from total daily load demand, 2,340 kWh/day for current energy consuming of 223 households with Annual Average Insolation Incident on a Horizontal Surface of 5.79 (kWh/m2/day) and average biomass supplying 25 tons / day. It is found that a grid connected PV-biomass IRE system is an effective way of emissions reduction and it does not increase the investment of the energy system.

Salah satu sistem kripto yang masih digunakan sampai saat iniadalah sistem kripto ElGamal yang diperkenalkan oleh Taher ElGamal . Padasistem kripto ElGamal klasik dan sistem kripto modifikasi ElGamal yangdikontruksikan oleh Marc Joye, masing-masing sistem kripto ini menggunakankonsep bilangan bulat (integer). Fakta ini memotivasi suatu ide untukmenggantikan konsep bilangan bulat (integer) menjadi suatu matriks yangberukuran n n  yang dinamakan dengana General Linear Group . Keunggulandari sistem kripto yang diusulkan adalah terdapat ruang plaintext yang lebihbesar dari sistem sebelumnya, sehingga ciphertext menjadi lebih acak dankeamanan pengiriman data menjadi lebih aman.Penelitian ini merupakan jenis penelitian studi literatur. Sedangkan tujuan daripenelitian ini adalah untuk memodifikasi sistem kripto ElGamal hasilkonstruksi Marc Joye menggunakan konsep General Linear Group sehinggadihasilkan suatu modifikasi sistem kripto ElGamal usulan yang lebih aman darisistem kripto ElGamal hasil konstruksi Marc Joye. Untuk itu, pada makalah inidiusulkan suatu modifikasi yang menggabungkan keunggulan dari sistem kriptoElGamal yang dikontruksikan oleh Marc Joye dan prinsip general linear grup.Hasil menunjukkan bahwa modifikasi sistem kripto ElGamal yang dihasilkanharus menggunakan general linear khusus yaitu matriks-matriks sirkulan yanginvertible atas modulo p.

Background:Multiple autoimmune syndrome (MAS) is a rare entity, defined by the association of three autoimmune diseases in the same patient. MAS can be classified into three groups.Objectives:The objective of this work was to describe the autoimmune diseases profile in MAS in An Internal Medicine Department.Methods:We report a retrospective analysis including 14 cases of MAS seen in The Internal Medicine Department at Taher Sfar Hospital, Mahdia, TUNISIA over a period of 10 years.Results:We followed 14 patients with MAS. They were 14 women. The mean age of patients was 52 years. SAM was type 3 in 12 patients (85%), type 2 in one patient (7.1%) and one patient satisfied both type 2 and type 3 MAS criteria (7.1%). No cases of MAS type 1 had been reported. We found 13 patients with 3 associated autoimmune diseases and one patient with 4 associated autoimmune diseases. The autoimmune diseases were: Sjögren’s syndrome in 14 patients (100%), Hashimoto’s thyroiditis in 11 cases (78%), systemic lupus erythematosus in 7 cases (50%), Addison’s disease in 4 cases (28, 5%), scleroderma, thymoma, vitiligo, Biermer and primary biliary cholangitis each in one case (7.1%).Conclusion:Multiple autoimmune syndrome remains a rare but probably under-diagnosed entity. Thus, in patients with autoimmune disease, initial investigation and follow-up of clinical signs and biological stigmas of other autoimmune diseases should be cautious.Disclosure of Interests:None declared

Abstract Aim The purpose of this study was to compare bone healing and coronal bone remodeling following both immediate and delayed placement of titanium dental implants in extraction sockets. Methods and Materials The study included 49 patients (28 women, 21 men) in need of implant treatment in the anterior teeth region of either the maxilla or mandible. Twenty-three subjects received 37 immediate (Im) implants and 26 subjects received 37 delayed (De) implants. The implants were placed immediately in the alveoli following the extraction in the Im group and the implants in the De group were placed approximately six months following the extractions. The width and depth of the marginal bone defects mesial and distal to the implants were evaluated radiographically using computer software designed to measure distances in digitized radiographs. The mean reduction of bone defect over time was 48% (from 3.4 to 1.3 mm) in the Im group which was statistically significant (P <0.05) when tested using a sample t-test, while the mean reduction was 17% (from 2.1 to 1.9 mm) in the De group. Conclusion The potential exists for bone healing and remodeling in fresh extraction socket defects associated with immediately placed implants. This article is a critical assessment of the historical backdrop to contemporary theorizations of the objectives of Sharia (Maqasid), starting from Imam Taher Ben Achour. The criterion of this assessment is what the article accepts as essential for the science of the Principles of Fiqh (Usul al Fiqh) in order to apply to contemporary challenges, i.e. the ability to go beyond the formal and fragmented approaches in dealing with the sacred texts and the context. The paper focuses upon the most important achievements in contemporary theorizing of Maqasid from this perspective, while also identifying the most important deficiencies for contemporary reform. The main finding of the research is that contemporary Maqasid-related discourse, despite the abundant literature, has not been able to shift the discipline of Maqasid from theorization of the mechanisms and methodological underpinnings to a necessary new stage – namely, the application of Maqasid theory in various fields of human activity.

Secondary data gathering by unraveling various literature, and related institution. Although the reduction in mangrove forest area is still in small quantities, to maintain the sustainability and ecological function it needs to management strategies. Results of analysis for management strategies found that is conservation to preserve the mangrove ecosystem area (34%), managing the mangrove ecosystem as ecotourism (32%), conservation of mangrove land as aquaculture area (21%) and make local regulations for the management of mangrove ecosystems (13%).

Introduction: Fungal rhinosinusitis (FRS) remains a rare disease. The noninvasive forms are hard to diagnose. The management protocols remain controversial. We aim to describe the clinical, radiological and pathological features of noninvasive FRS and present our management protocol and follow-up results. Patients and methods: This descriptive study was conducted in the ear-nose-throat department of the university hospital, Taher Sfar in Mahdia, Tunisia. All patients who responded to the definition of noninvasive FRS (fungal balls and allergic fungal sinusitis) were included. The study was conducted over a three year period (May 2017 – April 2021). Results: Eleven patients were included in this study: four cases of fungal balls and seven cases of allergic fungal sinusitis. Patients presented with symptoms of chronic recurrent rhinosinusitis with no response to conventional treatments. Computed tomodensitometry scan showed opacification of the paranasal sinuses in all patients. Other signs were heterogeneous opacities, local calcifications and thinning of the bony walls of the sinuses. Histopathological findings were inflammatory polyps in all cases of allergic FRS with the presence of fungal hyphae in 42.8% of the cases. All patients underwent surgery after a median delay of 12 [6–24] months of the symptom’s onset. The used procedures were endoscopic middle meatal antrostomy for all patients, ethmoidectomy (81.8%) and sphenoidotomy (36.4%). None received systemic antifungals or corticosteroids with a favorable outcome in all cases. Conclusion: Symptoms of noninvasive FRS are nonspecific. The scan images contribute to the diagnosis, but the perioperative findings and the histopathological results remain crucial. The management is mainly surgical.

Methodology Sixty specimens were prepared; 15 specimens of each material (each group n = 5, control after 15 days, at-home and in-office). All specimens were stored in distilled water at room temperature for 24 hrs before testing. A universal testing machine (Micromet 2100 series micro hardness testers) was used for testing Vicker's surface hardness for the three groups for every tested material. All results were statistically analyzed with one way analysis of variance (ANOVA), Post hoc Tukey HSD tests (P<0.05), and percentage changes for Tukey. Results All the tested materials showed an increase in Vicker's surface hardness between base line (24 hrs) and the control group after 15 days storage in distilled water except DY which showed a decrease in surface hardness. All tested materials showed a decrease in surface hardness from control group after 15 days and both at-home and in-office bleaching agents except DY which showed increased values. Significance At-home as well as in-office bleaching agents have a softening effect on some tooth colored restorative material, and the patient must be aware before using them. Citation Taher NM. Abstract Aim This in vitro study aims to measure the temperature changes of resin luting cements cemented to human dentin when using different light curing systems for photo-activitation. Methods and Materials The three different types of light-curing units (LCUs) used for photoactivation were quartz-tungsten halogen (QTH), light emitting diode (LED), and plasma arc (PAC). Two types of dual cure resin cements were used [Variolink II™ (VL) and Choice™ (CH)]. Feltik Z250™ composite resin material was used to prepare composite discs. Thirty human dentin specimens were prepared for each resin luting cement (ten for each light source). A total of 60 specimens were fabricated. Resin cement was applied on a dentin bridge and covered with the prepared composite disc where specimens were fabricated. Temperature change was recorded with a digital thermometer. Results: The lowest temperature was recorded when VL and CH were photo-activated with the PAC unit. The PAC unit produced significantly lower recorded temperatures than the LED and QTH units. No significant difference appeared between QTH and LED units in terms of recorded temperature. Results The lowest temperature was recorded when VL and CH were photo-activated with the PAC unit. The PAC unit produced significantly lower recorded temperatures than the LED and QTH units. No significant difference appeared between QTH and LED units in terms of recorded temperature. Conclusion The PAC unit produced significantly lower temperature changes compared to QTH and LED curing units. The risk for temperature rise should be taken into consideration during photo-polymerization of adhesive resins with LED or QTH in deep cavities when dentin thickness is 0.5 mm. La presente investigación tuvo como objetivo evaluar la transferencia de calor de gases de combustión mediante la simulación computacional al insertar un turbulador helicoidal al interior de un tubo de gases, variando sus parámetros de diseño para redistribuir los gases y conseguir un incremento en la turbulencia de gases con cierto grado de control de su régimen; esto se ha logrado variando el ángulo de ataque y el número de alabes helicoidales. Para predecir la trasferencia de calor se utilizó la simulación computarizada, los autores Zhang et al. 2009, Taher et al. 2012, y Jian et al. 2015 utilizaron el software COMSOL Multiphysics versión de prueba como modelo Multifísico No-Isotermal Flow turbulento k- 𝜀 y el modelo de conductividad turbulenta Kays-Crawford. Se evaluó los parámetros de transferencia de calor, el coeficiente de transferencia de calor que aumentan conforme disminuye el ángulo de ataque con rango de [20º-80º] para los siguientes parámetros, con ángulo óptimo de 65º, caída de presión 131 𝑃𝑎, energía cinética de turbulencia 2.11 𝑚2/𝑠2 y vorticidad 1.05𝑥104 1/𝑠, coeficiente de transferencia de calor equivalente 18.956 W/𝑚𝐾 para el caso en estudio. Se concluyó que los valores analizados de transferencia de calor y régimen de flujo de gases tienen parámetros óptimos a partir de los cuales se reducen vibraciones y la caída de presión no deseada (Goyder, 2002). Los resultados obtenidos indicaron que el ángulo de ataque óptimo es de 65º, consiguiéndose un aumento en la transferencia de calor de hasta 15.1 %, el cual produjo un efecto, incrementando la velocidad en un 40 %.

Abstract Abstract 5111 Background Clinically mild forms of thalassemia exist that, unlike β-thalassemia major, require no or only infrequent transfusions (eg. β-thalassemia intermedia, HbH disease). However, due to increased gastrointestinal iron absorption secondary to ineffective erythropoiesis these patients may still develop iron overload. For example, thalassemia intermedia patients (n=74) within a cross-sectional study had a mean serum ferritin (SF) of 1023 ng/mL (range 15–4140) and a mean liver iron concentration (LIC) of 9 mg Fe/g dw (range 0.5–32.1) at baseline despite most being transfusion-naïve (n=20) or rarely transfused (n=45), and only nine receiving regular transfusions (2–4 times/yr) (Taher et al. ITIFPaP: 13th International TIF Conference for Thalassaemia Patients & Parents, October 8–11 2008, Singapore, poster number MON04). Non-transfusional iron overload leads to the same serious clinical sequelae as transfusional iron overload, including liver, cardiac and endocrine dysfunctions. As patients with non-transfusional iron overload are not candidates for phlebotomy due to their underlying anemia, chelation therapy is the only available option for decreasing their iron burden. However, there is currently limited data available on the use of chelation in this population. The once-daily oral iron chelator deferasirox (Exjade®) is currently approved for the treatment of iron overload in patients with transfusion-dependent anemia. This prospective, randomized, double-blind, placebo-controlled Phase II ‘THALASSA’ study will evaluate the efficacy and safety of deferasirox in patients with non-transfusion-dependent thalassemia. Methods Non-transfusion-dependent thalassemia patients aged ≥10 yrs will be randomized 2:1:2:1 to starting doses of deferasirox/placebo 5 mg/kg/day/ deferasirox/placebo 10 mg/kg/day over a planned 12-month treatment period. Doses can be doubled after 6 months should patients require a higher dose, which will be determined after 6 months of treatment. All patients are required to have a baseline LIC of ≥5 mg Fe/g dw, as measured by R2 magnetic resonance imaging, and SF levels of >300 ng/mL. Patients will be excluded if they have: anticipated regular transfusions during the study (sporadic transfusions, such as in cases of infection, are allowed); any transfusion within 6 months prior to study start, chelation within 1 month prior to study start; HbS variants of thalassemia; impaired renal and liver function. Primary efficacy endpoint is absolute change from baseline in LIC at 12 months; secondary efficacy endpoints include change from baseline in LIC after 6 months and in SF after 6 and 12 months, as well as change in hematological and iron metabolism parameters (eg hemoglobin, transferrin saturation). Safety assessments include adverse event and laboratory parameter monitoring. 156 patients are planned for inclusion. Results As of 3 August 2009, 18 sites had been activated. Sites currently activated are in Thailand (n=5), Turkey (n=4), Italy (n=3), Malaysia (n=2), UK (n=2) Lebanon (n=1). Fifty-seven patients have been randomized to either deferasirox or placebo and their demographic data are shown in Table 1. Conclusions Similar to transfusion-dependent thalassemia patients, non- transfusion-dependent thalassemia patients also develop iron overload. This ongoing study will generate prospective efficacy and safety data for the use of deferasirox in non-transfusion-dependent thalassemia patients with iron overload. To prevent long term complications due to iron overload, it is important to assess iron chelation in this patient population as they are not candidates for phlebotomy due to the underlying anemia. Abstract Abstract 4077 Poster Board III-1012 Introduction Patients with thalassemia intermedia (TI) have an increased risk of thrombosis compared with thalassemia major patients. Thromboembolic events are more frequently observed in older TI patients (>20 yrs) who are splenectomized and non-transfused. Brain involvement has not been widely studied; however, one study has documented silent strokes on brain MRI in 37% of patients, indicating that further investigation is warranted. Patients and methods This was a cross-sectional study on 30 TI patients >18 years of age, splenectomized, not receiving regular transfusion therapy, and receiving no antiplatelet or anticoagulant medication. Patients were initially screened to establish absence of neurological signs or symptoms and other stroke-related disease (diabetes, hypertension, carotid stenosis, cardiac disease or prothrombotic mutations). All patients underwent both MRI and 18F-FDG PET scan on the same day. The criterion for abnormality on PET was the decrease in neuronal function (glucose utilization). MRIs were performed using T1- and T2- weighted spin-echo techniques. Abnormality was defined as an area of abnormally increased signal intensity on the T2-weighted pulse sequences. Results The mean age was 31.9 yrs (range: 18–54 yrs) with a male to female ratio of 13:17. Among the group of 30 subjects, 18 (60%) had abnormal MRI findings, 19 (63.3%) had abnormal PET findings, and 26 (86.7%) had either an abnormal MRI or abnormal PET or both. Solitary findings were identified in 7 (26.9%) of patients while multiple lesions were observed in 19 (73.1%) of patients who had abnormality. Bilateral findings were documented in 14 (53.8%) of patients with abnormality. The most common regions of abnormality were the parietal (76.9%) and frontal (69.2%) lobes; followed by the temporal (53.8%) and occipital (15.4%) lobes. Conclusions MRI/PET evaluation has demonstrated that silent brain abnormality is a common finding in this population of adult, splenectomized, non-regularly-transfused TI patients. Background:Knee osteoarthritis and obesity are both major health problems. It is now admitted that the prevalence of knee osteoarthritis gets higher with obesity and that weight loss helps knee function and allows patients to avoid surgery.Objectives:The aim of this study was to study the influence of obesity on knee osteoarthritis features.Methods:A cross-sectional study was conducted in the university hospital Taher Sfar of Tunisia over a period of 6 months. Patients who had knee osteoarthritis confirmed by radiographs were included. Sociodemographic, clinical, radiological and therapeutic data were collected from medical records and visits. Obesity was defined by a body mass index (BMI) ≥30. Functional impairment was assessed by the Womac index and Lequesne index.Results:The study included 186 patients. There were 31 males and 155 femmes. The mean age was 60±10 years. The percentage of obese patients was 53,8%. The mean age was similar in both groups obese and non obese. There were more women in the obese group compared to the non obese group (p=0.0001), more patients who had diabetes mellitus and dyslipidemia (p=0.002). Non-obese patients had a shorter duration of symptoms with no statistical significance (p=0.151). Obese patients had more involvement of both knees (p<0.0001). Obesity did not have an impact on pain severity. Severity of radiological images (p=0,0001) were more frequent in obese patients. Functional impairment was similar in both groups. However, the percentage of patients having a very important functional impairment with Lequesne index was higher in obese patients (p<0.029). Obese patients also needed more physical therapy sessions (p=0.035).Conclusion:Knee osteoarthritis in obese patients is characterized with the femlae gender predominance, bilateral knee involvement, and a more severe images on radiographs. Abstract Background Chronic iron overload is an important complication of long-term blood transfusions for severe beta-thalassemias, sickle cell disease and other blood disorders. Iron chelation therapy (ICT) is required to bind and excrete excess iron, which would otherwise accumulate and lead to organ damage or failure. Deferasirox is a once-daily, orally administered ICT approved for the treatment of chronic iron overload due to frequent blood transfusions in patients with beta-thalassemia major and other anaemias. A film-coated tablet (FCT) formulation was launched in the UK in 2016 and replaced the dispersible tablet (DT) formulation. In the context of a randomised clinical trial, the FCT formulation showed greater adherence and patient satisfaction, better palatability and fewer tolerability concerns than the DT. Furthermore, treatment compliance by pill count was higher with FCT (92.9%) than with DT (85.3%) (Taher et al, 2017). Little information exists however about compliance, efficacy and tolerability outside of a clinical trial setting. Objectives We wished to assess in a 'real world' situation, the effects of switching the deferasirox formulation from DT to FCT on patient adherence to ICT, iron overload and renal function. Methods Patients receiving ICT with deferasirox who were switched from the DT to FCT formulations were followed over a 12-month period and results audited using hospital dispensing and biochemistry records. The date of the first FCT prescription was defined as baseline. The initial daily dose used for switching from DT to FCT was as per manufacturer's recommendations: 70% of the DT daily dose. The impact on iron overload was assessed by comparing serum ferritin levels at 3, 6, 9 and 12 months post-switch with baseline values. The impact on renal function was assessed by comparing serum creatinine levels at 3, 6, 9 and 12 months post-switch with baseline values as well as the number of serum creatinine increases of 30% or greater above baseline. The changes in serum ferritin and creatinine were subsequently analysed by paired t-test. The Proportion of Days Covered (PDC) was calculated as a measure of patient adherence to ICT in the 12 months before and after switching formulations. Results 74 patients switched from deferasirox DT to FCT with the following diagnoses: beta-thalassemia (n = 45), sickle-cell disease (9), thalassemia-intermedia (6), HbE-thalassemia (5), other transfusion-dependent disorders (9). The median age was 36 (range: 1-78yo), mean baseline serum ferritin was 2767µg/L (range: 412-8742), mean baseline creatinine was 64.5 umol/L (range: 17-140) and the median prescribed daily dose of DT was 1250mg (range: 62.5 - 3500). The mean PDC in the 12 months prior to switching formulations was 0.80 (range: 0.31-1.00). This increased to 0.91 (range: 0.21-1.00) in the 12 months following the switch to FCT. The median prescribed daily dose of FCT was 900mg (range: 90 - 2520) The mean changes in ferritin and creatinine at 3, 6, 9 and 12 months post-switch are shown in the table. 6 out of 74 patients (8%) had a creatinine increase of >30% from baseline whilst receiving the FCT, occurring after an average of 120 days (range: 30-260). All 6 patients were managed by dose adjustment of FCT and creatinine returned to the normal range in 5 out of 6 cases. Conclusions The switch from deferasirox DT to FCT resulted in improved patient adherence to chelation, a reduction in mean serum ferritin and a modest rise in mean serum creatinine. Some patients showed a reversible rise in creatinine from baseline. The median daily dose of FCT prescribed was 72% of the DT formulation, approximately equivalent according to the known bioavailability of the different preparations and suggesting that improvements in serum ferritin were due to the more consistent daily administration of the FCT rather than an increased daily deferasirox dose. We suggest that when the fall in ferritin is abrupt and/or to levels <1000µg/L, serum creatinine should be followed particularly carefully to avoid over-exposure to deferasirox from the FCT. Abstract Aim The aim of this study was to determine in vitro the shear bond strength (MPa) and the type of bond failure when resin-modified glass ionomer cement (RMGIC) was bonded with different tooth-colored restorative materials. Methods and Materials The RMGIC tested was Fuji II LC (FL) and the tooth-colored restorative materials used were composite resin Point-4 (P4), Compomer Dyract AP (DY), and Ormocere Admira (AD). A total number of 60 FL specimens were prepared using Teflon molds. The specimens were divided into six equal groups. Each group of ten specimens was bonded to a tested tooth-colored restorative material as follows: Group I - etched FL bonded to P4; Group II - non-etched FL bonded to P4; Group III - etched FL bonded to DY; Group IV - non-etched FL bonded to DY; Group V - etched FL bonded to AD; and Group VI - non-etched FL bonded to AD. The specimens were stored in distilled water at 37°C for 24 hours. The shear bond strength was measured in a universal testing machine, and the fractured surfaces were examined under a stereomicroscope. Results The results of the shear bond strength indicated the lowest mean value (14.46 MPa) was in Group III, and this was significantly different from the values of other groups (p<0.05). However, Groups V and VI recorded the highest mean values (24.5 MPa and 28.39 MPa) which were significantly different (p<0.05) when compared to other groups. Groups I, II, and IV showed no significant difference with mean values of 20.06, 19.99, and 20.1 MPa which were significantly different from other groups (p<0.05). Conclusion AD showed the highest shear bond strength to RMGIC. All groups demonstrated a cohesive failure in FL except for Group IV where a cohesive failure in DY was recorded. Abstract Thrombotic thrombocytopenic purpura (TTP) in association with sickle cell crisis is rare. We present a case of sickle cell crisis and TTP. This is 48 years old Nigerian male with history of mild sickle cell anemia since childhood presented with sickle cell crisis and mental state changes. On admission labs are hematocrit of 20 and platelet count of 212,000. He was treated for sickle cell crisis but developed acute dysuria and progressively worsening anemia (Hct-13.7) and thrombocytopenia (Plt-9000) with sickle cell and fragmented RBCs on peripheral smear with LDH of 8772. This picture was consistent with TTP. Patient was immediately started on plasma exchange. Patient received a course of plasma exchange as well as hemodialysis and his condition improved, with return of platelet count to normal (232), LDH to baseline (276). Patient was discharged with mild renal insufficiency (serum creatinine-2.3) off dialysis and plasma exchange. Conclusion: TTP is a micro vascular occlusive disorder characterized by systemic or intrarenal aggregation of platelets, thrombocytopenia, and mechanical injury to erythrocytes. It is associated with pentard of signs and symptoms: thrombocytopenia, microangiopathic hemolytic anemia (schistocytes on peripheral blood smear), neurological abnormalities, renal failure and fever. In practice thrombocytopenia, microangiopathic hemolytic anemia and elevated lactate dehydrogenase levels are often sufficient for the diagnosis. Our patient with sickle cell crisis was a diagnostic challenge and it is our belief that TTP evolved during inpatient therapy for painful crisis.
