Abstract:
Because of many features of handicap, such as the number of actors and interactions, the complexity of the field, handicap must be considered from a system point of view, and dealt with as any system. That makes it possible to build a comprehensive response to the question of the inclusion and full citizenship of people with disabilities, better than some fragmentary solutions often proposed. The paper emphasizes relationships inside the disability field and external relationships with the environment, namely valid people in everyday life and workfield; the consideration of those external relationships leads to the compensation of handicap for a better accessibility to buildings, transports, care, communication, and soon; unexpected spin-offs of that compensation on the whole of population can be observed.

Since taking the decision to pursue a career in mental handicap, I have been increasingly aware of the debate surrounding the role of the consultant psychiatrist in this field. Nowhere else in medicine does there seem to be such uncertainty about the continued need for an already established specialty. I believe that one of the major reasons for the continued difficulty in attracting trainees into mental handicap is precisely this uncertainty about its future, which is in no way ameliorated by the College's view that a full time specialist appointment in the psychiatry of mental handicap is not superior to a joint appointment either with adult or child psychiatry.1

Increasing concern has been expressed in the psychiatric literature and general press about the use of neuroleptic medication for the treatment of behaviour disturbance in the mentally handicapped (Buck & Sprague, 1989). The issue was highlighted in a television documentary (Public Eye, BBC2, 1 May 1992) which reported a number of cases of tardive dyskinesia in mentally handicapped people who had been treated with neuroleptics. The programme, although taking a characteristically slanted view, will have made an impression on relatives, patients and the general public. It was also intimated that British families may be preparing to test the legal grounds for prescribing these drugs to the mentally handicapped, particularly where there is no formal psychiatric diagnosis.

Objective. To describe the results of a vestibular rehabilitation (VR) program in the timed up and go (TUG), gait speed (GS), and dizziness handicap inventory (DHI) scores for elderly vestibular patients in a developing country.Methods. Descriptive study with retrospective data collected from the clinical records of vestibular patients. The following information was recorded: sex, age, type of vestibular disorder, DHI score, and performance in TUG and GS, before and after participation in a VR program taking place from January 1 to August 30, 2017. The VR program consisted of 10 twice weekly sessions in the clinic and daily exercises at the patient’s home. We used Student’st-test for paired and Wilcoxon’s test according to the data distribution. The level of significance was 5%.Results. Data from 57 patients (49 females; 78 ± 5.8 years old) were used. There were statistically significant differences in TUG (12.52 versus 11.56), GS (0.81 versus 0.90 m/s), DHI total handicap (46 versus 24), physical (14 versus 8), emotional (14 versus 6), and functional (18 versus 12) domains.Conclusion. The functional outcome measures reported, including TUG, gait speed, and DHI, reflect statistically significant improvements in elderly patients after vestibular rehabilitation; the DHI improvements are clinically relevant.

SummaryWe investigated research activities on schizophrenia in 17 non-English speaking (13 European: Austria, Belgium, Denmark, Finland, France, Germany, Greece, Italy, Luxembourg, Netherlands, Portugal, Spain, and Sweden and 4 Asian: China, Japan, South Korea, and Taiwan) countries via a MEDLINE database survey. Despite the handicap of language, a reassuringly large number of English-written papers in research into schizophrenia have been reported; 892 papers from these countries appeared over the period 1990–1994. Among the countries investigated, Scandinavian countries in particular were found to have produced more communicable papers after taking into account the number of psychiatrists.

An investigation in a large mental handicap hospital revealed that 24% of the in-patients were receiving antipsychotic drugs. Chlorpromazine and thioridazine prescriptions accounted for 62% of the total while 10% of patients received depot preparations. Fifty-five per cent of the patients receiving these drugs had no established psychiatric diagnosis; most of these could be categorised as having a behaviour disorder. Patients aged 30–50 received higher doses, and female patients received a significantly higher mean dosage than male ones. In the patients receiving neuroleptic drugs who had also been taking them four years previously, there was a significant reduction in the dosage of the drugs received and the extent of polypharmacy of these agents. A mandatory requirement to review all prescriptions annually, implemented in 1984, may be a reason for this.

Abstract40 children attending a nursery and school for the handicapped were seen in a total of 55 consultations. Following detailed case taking from parents and school staff, medicines were selected by computer repertorization and verified against the child's constitutional type. Outcome was assessed with reference to preselected target symptoms.18 of the 40 children responded well in all target symptoms, 11 showed improvement in some but not all target symptoms; 7 children did not respond to treatment, and in 4 outcome could not be assessed. Children with organic brain damage, autism and definite syndromes generally responded well; those whose handicap was due to social deprivation generally failed to respond. Major responses were increased drive and improvement in hyperactive and aggressive behaviour, which enabled the children to benefit more from their education. Physical symptoms and problems such as stammering and enuresis also improved.

Finite element computer programs are frequently used to analyze and design embankments and similar earth structures. In most of the available computer programs, lack of a proper constitutive relationship to deal with volume change when an increase in the degree of saturation occurs, namely collapse phenomena, is a major handicap. In this paper, volume change results obtained from isotropic compression tests conducted on unsaturated compacted soil specimens are presented. Dependence of the bulk modulus of the soil on water content is investigated. Next, a hyperbolic formulation for volume change behavior of unsaturated soils taking into account variation of soil water content is presented. This hyperbolic model relates mean applied stress, volume change, and water content and represents a three-dimensional surface, the so-called "state surface". Suitability of the proposed model to predict collapse phenomena is verified by examining the model prediction against available experimental data.Key words: hyperbolic, unsaturated soil, collapse, volume change, suction pressure, bulk modulus.

Abstract This paper explores sort humor ‘black humor’, a key concept in Danish conversational humor. Sort forms part of a larger class of Danish synesthetic humor metaphors that includes other categories such as tør ‘dry’, syg ‘sick’, and fed ‘fat’. Taking an ethnopragmatic perspective on humor discourse, it is argued that such constructs function as a local catalogue for socially recognized laughing practices. The aim of the paper is to provide a semantic explication for sort humor and explore the discursive practices associated with the concept. From a comparative perspective, it is demonstrated that the Danish conceptualization of “blackness” differs from that of l’humour noir, a category of French surrealism, and English black humor with its off-limit topics such as death and handicap. In Danish discourse, sort humor has come to stand for a practice of collaborative jocular non-sense making. It is further argued that the main function of sort humor is to establish or enhance a feeling of “groupy togetherness”.

In the present educational context, active methodologies and new technologies are aspects that should be included when teaching and learning a subject area. For the education to be successful, classroom management must be considered, since problems may arise and handicap this process. In order to promote learning and reduce negative behaviors and increase positive ones, intervention on students of 1st year of ESO (compulsory secondary education) was implemented. The intervention used gamification as the educational approach and ClassDojo as the online tool to track behavior to determine the effectiveness of both elements to achieve the goal. Taking advantage of the game design principles and mechanics in the learning environment to create motivation and interest, in addition to the information and communication technologies (ICT) represented by ClassDojo, the experimental study showed the benefit of this method and app regarding the improvement of desired behaviors as well as the decrease of the disruptive ones. The implementation engaged the students and activated their behavioral development in order to display a better performance.

This thesis explores the relationship between architecture and the blind people. For better clarity and understanding of the topic, The work has been divided into five distinctive parts. In the first, introduction part, I approach space sensation of a blind person from the physiological and psychological angle. This analysis is very important for the understanding of imagination, which the blind person has. Another important fact to understand is the way in which the blind person gets the information about the space and what is his ability to process it. Further, I explain the basic principles of the blind person’s movement through space and describe instructions of how to help blind people in the concrete spatial situations. In the next part, I give a short information about the historical progress of the relation between the society and the blind people. An important fact is that, as late as 200 years ago, the blind people were considered as “sub men” and did not have the full freedom privilege. This relation has been modified over time and nowadays, the blind people have the freedom of movement and all the rights to enjoy their lives. To meet the needs of the blind people, many associations have been created that act on their behalf. The associations have different modes of functioning, but their common goal is to help blind people with their social rehabilitation, looking for employment and removing architectural barriers. The core of my work is divided into two parts. In the first one, I focus on the problematic of the blind people and architecture from the functional aspect. The functional aspect is vital for the blind people. It corresponds to their safety in the environment. In this chapter, I treat in detail basic elements for designing buildings, for specifying barriers and principles of design for blind people. Furthermore, I describe the help tools the blind people need in order to move in the space. I evaluate the future of these tools and indicate the rapid technological progression which makes the life of blind people more comfortable. In the next part, I focus on the problematic of the blind people and architecture from the aesthetic aspect. This question has not been much explored yet, but is, nonetheless, very important. In this part of my work, I use more my personal experience and the knowledge coming from the exploration of space. Aesthetical values are subjective for all the people, with or without a handicap. Therefore, in this final chapter, I try to note the facts that can be used in the process of the aesthetical perception and to reflect on them. In the domain of aesthetical research, there also exist tools and institutions that help blind people in their comprehension of architecture. The division of my work emerged from the compilation of diverse facts in the process of writing. This thesis has an informative and practical character. It is intended for people who want to understand and to be informed about how to design architecture for the blind people. It will be directed towards people who build this kind of architecture and need an overview of how to make it. It is also intended for people interested in learning more about architecture.

Along with the rising of Democracy and Law and Human Rights, the protection and preservation of individual right or interest are increasingly gaining more emphasis for physically and mentally handicapped persons. Recently, it has discovered a trend towards to completion for the legislative and revising processes, or the expenditures on related budget year by year. Among which, Article 10 of the Additional Articles of the Constitution of the Republic of China (Taiwan) also clearly stated to guarantee the rights and interests of insurance, medical care, obstacle-free environments, and vocational guidance etc., for physically and mentally handicapped persons with providing the legal basis and legislative foundation. However, physically and mentally handicapped persons have higher inconvenience of seeking medical advice and dependence on medical care than other people. Kinmen County located in offshore islands of Taiwan, and the population of physically and mentally handicapped persons has gradually increased year after year; on the other hand, in Kinmen County, the lack of medical resources and caring manpower certainly will cause an impact on the rights and interests of medical treatment for physically and mentally handicapped persons. As a result, in view of the inconvenient geographic location of Kinmen County, as well as the lack of medical resources that will cause the difficulty in seeking medical advice for physically and mentally handicapped persons, the motivation and objective of this study is attempted to understand the interaction among related members in this network and looked for the current dilemma and solution of improvement through related researches and surveys. Therefore, this study is adopted two research methods: Documentary Analysis and In-depth Interview, to understand the current status and difficulty. At last, several conclusions came out as follows: 1. Medical resources have obviously lacked in Kinmen County. 2. Difficulty encountered during the recruitment and employment of physicians and professionals. 3. Administrative units and non-profit organizations (NPO) have faced a bottleneck in manpower.

Universal newborn hearing screening has meant that babies with significant congenital hearing impairment can be identified soon after birth and management instituted to ameliorate resultant problems, that is, minimize disability and handicap and optimize life chances. Evidence for the value of school entry screening is lacking and there is some evidence it is neither effective nor cost-effective. Further research is needed on this. Otitis media with effusion can cause significant long-lasting effects and may need surgical intervention or the provision of hearing aids. Parents and professionals should be aware of the symptoms that are indicative of possible hearing loss. Parents’ concerns should always be taken seriously.

This chapter describes an objective concept of culture and catalogues, from a pedagogical point of view, large categories of facts that identify cultures. The idealistic perspective of culture is confronted with the perspective of real individuals. When the real subjects become the point of view, the concept of inclusion has to inevitably be taken into consideration. Inclusion, in this context, involves not just individuals with handicaps of whatever kind, but also minority cultures, with a faint voice. It is, therefore, of paramount importance to consider culture from a subjective and inter-subjective perspective.

Empowered women encourage economic development and assist in making stable societies, and everyone benefits from the growth and stability that empowered women add to the economy. According to a study by McKinsey, if the economic participation of women is promoted globally, especially in developing countries, then it might enhance world economic growth by $28 trillion dollars by 2025. In Uzbekistan disparities exist between men and women in education, health, employment and income opportunities, control over assets, personal security, and participation in the political process in the country. The women come across significant barriers which deter her to participate fully and freely in contributing to the economic growth of the country. The stereotypical image of women, where we revere her mainly as a mother, the guardian of family wellbeing, is rooted in the minds of people; it is true, but in today's world, she should not be an inactive bystander, but an active and proactive contributor in the currently going on democratic transformations in the country. There is no denying the fact that Uzbek women have made a considerable progress in the last few years, but they have to struggle against many handicaps and social evils in the male-dominated society. The empowerment of women is one of the solutions to the problems of inequality, subordination, and marginalization. A critical problem and a high priority for the Government of Uzbekistan are to improve women's role and empower them to take part in the economic growth of the world. This chapter speaks about the government initiatives in empowering Uzbek women and bringing gender equality in the society. The data for the chapter was collected from secondary sources. The objectives of this article are to examine and analyse the initiatives taken by Uzbek government and other UN bodies for gender equality and women empowerment. The same strategies can be replicated in other parts of Central Asia where women are still not treated equally and can be empowered to contribute to the socioeconomic growth. The current research may suggest the authorities and civil society organizations to improve and implement inventive methodologies to attain gender equality, advancement, and peace.

Abstract:
Human, as a species, is very variable, and his variability is at the basis of his social organization. This variability is maintained, in part, by the chance effects of gene assortment and the variation in these genes is the result of mutations in the past. If our remote ancestors had not mutated we would not he here; further, since no species is likely to he able to reduce its mutation rate substantially by the sort of selection to which it is exposed, we may regard mutations of recent origin as part of the price of having evolved. We are here: all of us have some imperfections we would wish not to have, and many of us are seriously incommoded by poor sight, hearing or thinking. Others among us suffer from some malformation due to faulty development. A few are formed lacking some essential substance necessary to metabolize a normal diet, to clot the blood, or to darken the back of the eye. We will all die and our deaths will normally be related to some variation in our immu-nological defences, in our ability to maintain our arteries free from occlusion, or in some other physiological aptitude. This massive variation, which is the consequence both of chance in the distribution of alleles and variety in the alleles themselves, imposes severe disabilities and handicaps on a substantial proportion of our population. The prospects of reducing this burden by artificial selection from counsel¬ling or selective feticide will be considered and some numerical estimates made of its efficiency and efficacy. Screening is a procedure by which populations are separated into groups, and is widely used for administrative and other purposes. At birth all babies are sexed and divided into two groups. Later the educable majority is selected from the ineducable minority; later still screening continues for both administrative and medical purposes. Any procedure by which populations are sifted into distinct groups is a form of screening, the word being derived from the coarse filter used to separate earth and stones. In medicine its essential features are that the population to be screen¬ed is not knowingly in need of medical attention and the action is taken on behalf of this population for its essential good. A simple example is provided by cervical smear examination, the necessary rationale for which must be the haimless and reliable detection of precancerous changes which can be prevented from becoming irreversible. Any rational decision on the development of such a service must be based on a balance of good and harm and any question of priorities in relation to other services must be based on costing. The balance of good and harm is a value judgement of some complexity. In the example of cervical smears anxiety and the consequences of the occasional removal of a healthy uterus must be weighed against the benefits of the complete removal of a cancerous one, and such matters cannot be costed in monetary terms. In fact, even such an apparently simple procedure as cervical screening is full of unknowns and many of these unknowns can only be resolved by extensive and properly designed studies. In genetic screening the matter is even more complicated, since the screening is often vicarious; that is, one person is screened in order to make a prediction on what may happen to someone else, usually their children, who may be un¬conceived or unborn. Further, the action of such screening may not be designed to ameliorate disease, but to eliminate a fetus which has a high chance of an affliction, or to prevent a marriage in which there is a mutual predisposition to producing abnormal children. These considerations impose very considerable dif¬ferences, since the relative values placed on marriage, on having children within marriage, and on inducing abortion, vary widely between individuals and between societies.
