Abstract:
Darbo tikslas – įvertinti KMUK padalinių vadovų nuomonę apie telemediciną bei jos taikymo galimybes Lietuvoje. Tyrimo metodika: KMUK vadovaujamą darbą dirbančių gydytojų (klinikų, skyrių, sektorių vadovų) anoniminė anketinė apklausa vyko 2008 metų sausio –vasario mėnesiais. Anketos buvo išdalintos visiems 128 respondentams. Statistinėje analizėje naudota 101 teisingai užpildyta anketa. Atsako dažnis 78,9 proc.. Rezultatai. Geriausiai su telemedicinos taikymo galimybėmis susipažinę klinikų vadovai. Respondentų nuomone, radiologija, oftalmologija ir kardiologija – sritys, kuriose telemedicina galėtų būti plėtojama labiausiai. 23,8 proc. respondentų telemedicinos plėtros galimybes KMUK įvertintos palankiai. Galimybės plėtoti telemediciną Lietuvos mastu buvo vertinamos atsargiau: dauguma (39,6 proc.) apklaustųjų manė, jog kol kas tam nėra pakankamų galimybių. Svarbiausi veiksniai, skatinantys sėkmingą telemedicinos diegimą Lietuvoje: gydytojų specialistų noras ir pastangos dirbti novatoriškai, telemedicinos metodų ir priemonių kūrimas bei taikymas praktikoje, teisinės bazės, susijusios su šia sritimi, tobulinimas. Aim of the aim. To evaluate the opinion of the heads of the departments of Kaunas University Hospital on telemedicine and the possibilities of its application in Lithuania. Methods. Anonymous survey of the heads of the departments of Kaunas University Hospital was carried out in January and February, 2008. 128 heads of the department were distributed. 101 questionnaires were filled in correctly and were used for a statistical data analysis. The frequency response 78.9%. Results. The heads of different clinics of Kaunas University Hospital knew the possibilities of using telemedicine. The primary fields of medicine where the potential use of telemedicine could be developed include radiology, ophthalmology and cardiology. 23.8% of respondents thought that there were enough options to develop telemedicine in Kaunas University Hospital, whereas the options to develop it in Lithuania were assessed moderately. Many respondents (39.6 %) thought that there was not enough potential to do that. The most important factors in developing telemedicine in Lithuania were: wishes and efforts of doctors to work in a different way, the establishment and use of telemedicine’s methods and devices in practice and the improvement of telemedicine law. The idea is that both patients and doctors will use this prototype for exchanging useful information and thus reduce some of the multiple inconvenient that this disease has.

The solution proposed in this project can be generalized to cover other aspects of the communication between the patient and the doctor.

Abstract:
This project investigates whether customer arrivals to digital healthcare providers can be appropriately modeled by non-homogeneous Poisson processes (NHPPs). Data from a large Swedish telemedicine provider was examined, with the goal of confirming that the data exhibited the two required properties of Poisson processes: customer interarrival times being (i) exponentially distributed, and (ii) independently and identically distributed. The arrival process was modeled as a piecewise constant arrival function, with several interval lengths considered. Two statistical tests were performed, the Kolmogorov-Smirnov and Brock-Dechert-Scheinkman tests, to confirm whether the two properties held. The experiments showed that a majority of intervals exhibited both properties, suggesting that NHPPs are a useful approach for modeling telemedicine customer arrival data.
Detta arbete undersöker kundankomster till digitala vårdgivare, och huruvida dessa kan modelleras med en inhomogen Poissonprocess (NHPP). Data från en stor svensk digital vårdgivare har undersökts, med målet att bekräfta om datan uppvisar de två egenskaperna som krävs av en Poissonprocess: att tiden mellan ankomster är (i) exponentialfördelade, och (ii) oberoende och likafördelade. Markovprocessen som beskriver ankomsterna har beskrivits som en styckvis konstant funktion. Två statistiska test har genomförts, Kolmogorov-Smirnov och Brock-Dechert-Scheinkman. Enligt resultaten uppvisade en majoritet av datan de två egenskaperna, vilket antyder att inhomogena Poissonprocesser är en användbar modell för ankomster till digitala vårdgivare.

Den svenska sjukvården står idag för ett antal utmaningar om den skall fortsatt kunna erbjuda god service som vårdgivare och vara attraktiv som arbetsgivare. Bland annat diskuteras frågor som förvärrad platsbrist, läkarbrist samt avstånd till specialistsjukvård. På Karolinska Universitetssjukhuset i Solna ligger Radiumhemmet och där behandlas bland annat lungcancerpatienter. Där diskuteras huruvida telemedicin kan vara en möjlig väg för att lösa ovanstående problem. Denna uppsats utreder på vilket sätt telemedicin kan användas på Radiumhemmet för lungcancerpatienter. För att kunna utreda en telemedicinsk lösnings möjligheter på Radiumhemmet genomfördes intervjuer och fokusgrupper med personalen. Externa experter från sjukvården och industri intervjuades och ett besök gjordes på barnsjukhuset Childrens Healthcare of Atlanta, USA, där man har kommit långt i användningen av telemedicin. Det finns delar av verksamheten på Radiumhemmet där telemedicin skulle kunna bidra till att skapa möjligheter till en tätare kontakt mellan patient och vårdpersonal. Detta i syfte att lugna oroliga patienter och hjälpa till med lättare symptombedömningar och på så sätt minska väntetiderna. Genom de undersökningar som utfördes upptäcktes dock även svagheter i en telemedicinsk lösning avsedd för lungcancerpatienter. Patientgruppen ofta är äldre med liten erfarenhet av datorer och sjukdomen är allvarlig. Det krävs personlig kontakt och fysiska undersökningar, men i vilken utsträckning är individuellt. Telemedicin kanske inte kan ses som en absolut lösning för de problem som råder i sjukvården idag när det kommer till lungcancerpatienter, men väl som ett komplement. Om lungcancerpatienter är den optimala målgruppen är ifrågasättbart men att telemedicin kan underlätta i den svenska sjukvården står klart.
Nowadays the health care system in Sweden is faced with several challenges like shortage of space, physicians and long distances to specialized health care. A possible solution for this being discussed at the lung cancer department of Karolinska University 2 Hospital is the use of telemedicine. If implemented it would be part of the followup treatment. The objective of our research is to find out if this technology can help improve the health care. In order to investigate the opportunity for a telemedicine solution, we collected qualitative data from multiple different sources. This included two doctors specialized in lung cancer, and a focus group with nurses from Radiumhemmet. We also conducted interviews with relevant individuals outside the hospital including Nirav Desai who is the Founder and CEO of Hands On Telehealth; furthermore, we visited the Children’s Healthcare of Atlanta based in Atlanta, Georgia where telemedicine is used on a daily basis. Thanks to the carried out research, we have discovered that telemedicine could be used in certain scenarios and contribute towards a more frequent contact between the patient and the medical professionals. Thus, this new technique could help nurses execute lighter symptoms assessment remotely and reduce waiting times. We also discovered some inconveniences in a telemedicine solution designed for lung cancer patients. We personally do not think they are the best target group for such a solution since the patients are mostly the elderly with little computer experience. Also the disease is severe and requires physical examinations where the telemedicine existing today would not improve the care giving. To all intents and purposes, telemedicine might not be the only and ultimate solution for the problems identified within healthcare for lung cancer patients at Radiumhemmet, but it can work well as a supplement. 3

A design of computer systems, that effectively supports the user, is a major goal within human-computer interaction. To achieve this, we must understand and master several tasks. These tasks concern firstly what to develop and secondly how to develop the system. The design and implementation of effective and efficient user interfaces is a prerequisite for the successful introduction of computer support in the medical domain. We base our work on a fundamental understanding of cognitive aspects of human-computer interaction, as well as on detailed analysis of the specific needs and requirements of the end users, i.e., the medical professionals. This thesis presents several approaches for development of systems for computer-supported work in health care. The solutions described concern vital problem areas: (1) the focus on the work tasks to be performed, (2) the cost of software and the way competition works in a networked world. Solutions to these problems can lead to more usable systems from a user's perspective but may also change the nature of computer applications.

Abstract:
This thesis offers a review of the historical development of telemedicine services in remote health care. It addresses the professional concerns in practising medicine in isolated conditions, and the advances in telecommunications technology since the telephone was invented. It also examines the application of telemedicine in remote environments across the world, such as in indigenous communities, remote industrial work sites and at scientific bases in Antarctica. At its most exotic, a review is offered of the health care for space crews. The literature review highlights a number of concerns about the state of the art knowledge on remote health care services. These concerns are the minimal training requirements of individuals who act as health care practitioners in the remote environment, the additional training requirement upon the advising medical practitioner, and the design of a system for the collection of clinical information from the patient. In response to the above a two year study was conducted. Quantitative and qualitative observation of remote health care consultations was undertaken. The environments studied were simulation cases occurring in the UK and Antarctica, and real cases presenting on oil installations in the North Sea. The study results answer the original concerns about the training levels, data collection and communications components of a remote health care service. In addition, they offer valuable input towards the design of a telemedicine model for remote health care. The telemedicine model is presented as a framework upon which future developments in the field of telemedicine may be approached.

Objetivo: Pernambuco atualmente oferece poucas unidades especializadas em estomatologia. O objetivo do presente trabalho foi desenvolver e verificar a funcionalidade de um sistema de telediagnóstico de doenças orais no estado de Pernambuco. Métodos: Quinze computadores e a mesma quantidade de câmeras digitais foram distribuídos em centros secundários de saúde onde Cirurgiões-Dentistas foram encorajados a visitarem um website e enviarem imagens dos seus pacientes que apresentassem lesões orais para avaliação à distância por dois profissionais com experiência em estomatologia. Um formulário de feedback com opiniões dos dentistas e pacientes também foi enviado. Resultados: Entre 325 pacientes, 313 (96,31%) acharam o procedimento de tirarem fotos de suas bocas bastante confortável, 265 (81,5%) ficaram satisfeitos em ver o interior de suas bocas e 288 (88,62%) acharam o procedimento útil no entendimento dos seus problemas clínicos. Foi estabelecida uma concordância entre os avaliadores e um correto diagnóstico final em 195 (60%) casos. Em 98 (30,16%) casos somente um avaliador acertou a hipótese diagnóstica sugerida à distância. Em 26 (8%) dos casos nenhum avaliador acertou o diagnóstico. 94,44% dos cirurgiões-dentistas consideraram que o auxílio do centro de referência contribuiu para os seus conhecimentos em relação à desordem/doença e acreditaram que o processo de telediagnóstico evita um encaminhamento desnecessário do paciente Conclusão: o telediagnóstico de doenças orais provou ser um modo viável de se evitar algum encaminhamento desnecessário de pacientes.
Objective: Pernambuco, located in Northern Brazil currently offers few oral medicine specialized units. Based upon this, the aim of the present study was to develop and verify the functionality of a telediagnosis system of oral diseases in Pernambuco state. Methods: fifteen computers and the same amount of digital cameras were distributed to secondary health care centers. Dentists were then encouraged to visit a website and send pictures of their patients who presented oral lesions. A feedback form with dentists and patients opinions was also sent. Results: Among 325 patients, 313 (94.3%) found the procedure of recording images of their mouth very comfortable, were happy to view the inside of their mouths 265 (81,5%), and found the procedure generally useful in understanding their clinical problem (288 out of 325 individuals (88%). In 195 (60%) of the instances an agreement between examiners and a correct final diagnosis was established; only one consultant made a correct diagnosis in 104 of the cases (32%); and neither consultants established a correct diagnosis in only 26 cases (8%). 94,44% of dentists considered that the help of a specialized center contributed to the improvement of their knowledge concerning oral diseases and believed that the telediagnosis system avoided unnecessary referrals. Conclusion: telediagnosis of oral diseases proved a feasible way of avoiding some unnecessary referrals.

To analyze the effectiveness of a teledermatology system, the systematization of digital photographs from cutaneous lesions on technical quality, morphologic and clinical aspects was performed. The study showed that unsatisfactory technical quality, palpatory morphology and less typical lesions should be supplemented by clinical description of the lesions and patient information to increase diagnostic accuracy. The dermatologic Internet-based teleconsultation system has diagnostic performance comparable to face-to-face consultation, and all the hypotheses, not only first one, should be considered at virtual diagnostic evaluation

The present study aims to establish the level of diagnostic reliability provided by the transmission of a videolaryngoscopy by videoconference (VC), evaluating the quality of the transmitted images and its impact on diagnoses. Two DVDs with 60 laryngoscopy (30 fiberscopies and 30 telescopies) images, corresponding to 30 original and 30 transmitted by VC, were evaluated by 6 laryngologists who answered a questionnaire on the quality of the selected images (scores from 0 to 3) and on the diagnoses of the related disease. A third DVD with 26 pairs of images, an original and a VC, or two original, was also analyzed by the examiners who had to select which image presented the best quality, which corresponded to the VC image, and the possible impact of such differences on the quality of diagnoses. A high level of agreement (K = 0.911) was found among the examiners in relation to the identification of the VC (96,15% of accuracy) and to the best quality of the original image (K = 0,850). Although a partial agreement has been found among the examiners (K = 0,371), diagnoses was not jeopardized by the quality of the VC image in 75,64% of the cases. Original videotelescopy images (83,33% very good) showed the best quality and videofiberscopy VC the worst (36,66% very good and good). A high degree of correctness was found for the diagnoses by VC images (86,67% by fiberscopy and by telescopy). Consequently, the use of a VC to transmit videolaryngoscopy images has proved to be efficient for the remote diagnoses of larynx diseases

“Telemedicine” denotes medicine at a distance using telecommunications and information technologies. The aim of the thesis is to reveal determinants and outcomes of telemedicine diffusion—that is, the spread of telemedicine in time and space. Telemedicine is examined both at the national level and in a regional case study. At the national level, quantitative methodology is utilized to analyze the emergence of health care facilities using telemedicine to receive medical assistance. The regional case study examines the diffusion and outcomes of telemedicine networks in Northern Sweden based on non-quantitative data sources. The first telemedicine applications in Sweden emerged between hospitals in southern Sweden in the 1970s. It was in the 1990s, however, that the practice of telemedicine took off on a larger scale. By the year 2000, the number of remote sites was quite evenly distributed across the country and between specialist and primary care facilities. In northern Sweden, telemedicine was first implemented in the mid-1980s, and is now carried out in all counties in the region. The telemedicine networks in northern Sweden primarily concern teleradiology, general telemedicine, and telepathology. Commonly, telemedicine is carried out within counties in hierarchical hub and spoke-type networks connecting specialist and primary care facilities. The study shows that existing telemedicine facilitates its further diffusion at the regional level, but shows a lack of the obvious neighborhood effect that could be expected in light of many previous diffusion studies and geographical diffusion theory. The health care system in Sweden is characterized by a high degree of regional autonomy. Contacts between medical staff, and thus dissemination of information and opinions concerning telemedicine, are therefore likely to be more prevalent within than between the regional health care organizations. The health care organizations are also decision-making bodies with different telemedicine policies. In addition, already implemented telemedicine programs tend to expand within the health care organization in which they originated. Although no traditional neighborhood effect to speak of can be noticed in Swedish telemedicine diffusion, distance matters in the diffusion process in a quite different respect. The study shows that health care facilities located far from more specialized care are especially likely to adopt telemedicine. Another local factor, facility size, is also positively correlated with telemedicine diffusion. This correlation between facility size and telemedicine adoption, as well as the circumstance that the diffusion process started with, and has progressed the furthest in, specialist care, suggest that the diffusion of telemedicine exhibits hierarchical characteristics. The development of telemedicine technology significantly influences the rate of telemedicine diffusion. In a simulation of the diffusion process, it is shown that—given that computers and information technology continue to develop according to the present trend—there will be a significant increase in the number of primary care facilities functioning as remote telemedicine sites during the first decade of the 21st century.

Abstract:
This thesis employs data-mining techniques and modern information and communication technology to develop methods which may improve efficiency of prevention oriented telemedical programs. In particular this thesis uses the ITAREPS program as a case study and demonstrates that an extension of the program based on the proposed methods may significantly improve the program's efficiency. ITAREPS itself is a state of the art telemedical program operating since 2006. It has been deployed in 8 different countries around the world, and solely in the Czech republic it helped prevent schizophrenic relapse in over 400 participating patients. Outcomes of this thesis are widely applicable not just to schizophrenic patients but also to other psychotic or non-psychotic diseases which follow a relapsing path and satisfy certain preconditions defined in this thesis. Two main areas of improvement are proposed. First, this thesis studies various temporal data-mining methods to improve relapse prediction efficiency based on diagnostic data history. Second, latest telecommunication technologies are used in order to improve quality of the gathered diagnostic data directly at the source.

O presente estudo consiste em descrever e analisar a evolução da Telemedicina no Brasil, abrangendo as experiências efetivamente implantadas até dezembro de 2000. Várias instituições no país têm adotado programas de telemedicina, tais como hospitais, laboratórios de análises clínicas, clínicas especializadas em realização de eletrocardiogramas à distância, universidades, o Exército e o próprio Governo. Cada uma das experiências tem diferentes objetivos e não há nenhuma integração entre elas. As aplicações estudadas variam desde aquelas que requerem uma estrutura tecnológica simples como a realização de eletrocardiogramas à distância, até outras mais sofisticadas tecnologicamente como a realização de teleconferências com transmissão de imagens clínicas para segunda opinião médica e monitoração remota de leitos. Para a realização do estudo foram feitas entrevistas com os responsáveis pelos programas e um levantamento de dados em publicações especializadas. As conclusões apontam para a falta de coordenação entre os modelos adotados, para a resistência cultural relativa à informática médica e à telemedicina e para o papel tímido do Governo no fomento da telemedicina.
The present study consists in describing and analyzing the evolution of Telemedicine in Brazil, comprehending the experiences established until December 2000. Several institutions in the country use telemedicine programs, as hospitals, pathology analysis clinics, remote electrocardiograms clinics, universities, the Brazilian Army and the federal Government. Each experience has its own goals and there is no integration among them. The telemedicine programs comprehend a wide range of applications: while some require a plain technological structure as remote electrocardiograms, others demand more sophisticated resources as teleconferences with clinical images transmission and remote bedside monitoring. To accomplish this study it was necessary to conduct interviews with the leaders of the projects and also data mining in specialized publications. The conclusions indicate that there is lack of coordination among the experiences, that there is a cultural resistence both to medical informatics and telemedicine, and the faint Goverment role in stimulating telemedicine.

Introdução: O envelhecimento da população é um fenômeno que tem ocorrido mundialmente, inclusive no Brasil, caracterizado por alterações físicas, cognitivas e sensoriais. Nesse contexto é possível verificar alterações sensoriais importantes como a deficiência auditiva, onde o uso do Aparelho de Amplificação Sonora individual (AASI) tem como proposta minimizar os efeitos da deficiência auditiva propiciando o maior aproveitamento da audição residual. No entanto, a dificuldade de manipulação do AASI é um dos fatores que fazem com que o indivíduo deficiente auditivo protele ou abandone o uso. Iniciativas utilizando Teleducação tem tido êxito na orientação de indivíduos na área da saúde, incluindo a Fonoaudiologia. Portanto, é necessário o desenvolvimento de projetos que auxiliem o deficiente auditivo idoso no processo de adaptação do AASI. Objetivo: avaliar o DVD Conhecendo e Aprendendo sobre o meu Aparelho Auditivo (Vol. 1) no processo de adaptação do AASI. Metodologia: A metodologia foi dividida em 2 etapas de desenvolvimento. Na primeira etapa o projeto foi dividido em duas fases, senda uma de avaliação e outra a reestruturação do material didatico. Na primeira fase o DVD foi, primeiramente, avaliado por 22 fonoaudiólogos e por 25 idosos usuários de AASI. Na segunda fase, as sugestões feitas pelos avaliadores foram analisadas para a reestruturação do referido DVD volume 1. A segunda etapa consistiu na verificação da eficácia da nova versão do DVD junto aos indivíduos deficientes auditivos idosos novos usuários de AASI, com um grupo controle, composto por 14 indivíduos, que receberam apenas as orientações fonoaudiológicas e o grupo experimental, composto por 13 indivíduos, que além das orientações fonoaudiológicas, também recebeu o material didático em DVD volume 2. A eficácia da versão do DVD foi mensurada por meio da utilização do questionário PHAST, o qual foi aplicado nos grupos E e F. Resultados: Na primeira etapa os avaliadores sugeriram atualização dos modelos de AASI retroauricular, inserção do tempo de ativação da pilha e menor velocidade de fala e legendas. A reestruturação do material didático em DVD foi realizada por meio do financiamento da FAPESP, que possibilitou a contratação de uma produtora para realizar o trabalho. Houve reestruturação do roteiro, das filmagens e das edições do vídeo. Quanto à segunda etapa, não houve diferença estatisticamente significante entre a facilidade/dificuldades de manuseio encontradas nos grupos controle e experimental. A avaliação feita pelo Grupo experimental sobre o quanto o DVD volume 2 ajudou na compreensão dos conteúdos sobre o AASI, a maioria da amostra relatou que esse ajudou muito em todos os pontos avaliados. A menor classificação foi satisfatória para os aspectos abordados no DVD, não havendo diferença estatisticamente significante quando comparado ao grupo de antigos usuários de AASI. Discussão: O material didático foi avaliado, sendo sua reestruturação respaldada em estudos voltados para elaboração de materiais didáticos à distância. Todas as dificuldades encontradas pelos grupos controle e experimental corroboraram com a literatura. As avaliações feitas pelo grupo experimental mostrou o elevado grau de satisfação com o DVD Volume 2. Conclusões: O DVD Conhecendo e Aprendendo sobre o meu Aparelho Auditivo volume 2 foi avaliado, se mostrando eficaz no auxílio de indivíduos idosos novos usuários de AASI retroauricular
Introduction: The high rate of aging is a phenomenon that has occurred worldwide, including Brazil and the process leads to physical, cognitive and sensory. One change is the sensory hearing loss, which may cause the wearer to isolation. The hearing aids (HA) are used to minimize the effects of hearing loss producing good results. However, the difficulty in handling the hearing aid is one of the factors that make a person deaf procrastinate or abandon the use. Teleducation initiatives have been successful using the instruction of individuals in health, including Speech Pathology and Audiology area. Therefore, it is necessary to develop projects that assist the hearing impaired elderly in the process of fitting of hearing aids. Objective: To evaluate the DVD \"Knowing and Understanding about My Hearing Aid\" in the process of fitting of hearing aids. Methodology: The methodology was divided into two stages of development. In the first phase of the first stage, the mentioned educational materials on DVD was first evaluated by 22 audiologists and 25 elderly hearing aid users. In the second phase of the first stage, the suggestions made by the evaluators were analyzed for the restructuring of this DVD Volume 1. The second step was to verify the effectiveness of the new version of the DVD along with the elderly hearing impaired individuals new hearing aid users, with a control group of 14 individuals who received only hearing aid orientation and the experimental group, composed of 13 individuals who apart from hearing aid orientation, also received educational materials on DVD Volume 2. The effectiveness of the DVD version was measured by using the PHAST questionnaire, which was applied in groups E and F. Results: In the first stage, the evaluators suggested updating of models of BTE hearing aids, insertion time of cell activation and slower rate of speech and subtitles. The restructuring of the teaching material on DVD was accomplished through funding from FAPESP, which enabled the hiring of a producer to do the job. There was a restructuring of the script, filming and edits the video. The second stage, there was no statistically significant difference between the ease / difficulty of handling found in the experimental and control groups. The assessment made by the experimental group over how much the Volume 2 DVD helped in understanding the content on their devices, most of the sample reported that this \"very helpful\" in all points evaluated. The lowest rating was \"satisfactory\" to the points raised in the DVD, with no statistically significant difference when compared to former users of hearing aids. Discussion: The teaching material was evaluated, and backed its restructuring in studies aimed at preparing distance learning materials. All the difficulties encountered by the control and experimental groups corroborated the literature. The assessments made by the experimental group showed the high degree of satisfaction with the DVD Volume 2. Conclusions: The DVD \"Knowing and Understanding My Hearing Aid\" Volume 2 was evaluated, proving effective in helping the elderly new users of BTE hearing aids.

Apesar dos avanços tecnológicos voltados para o uso das Tecnologias da Informação e Comunicação em Saúde, vários aspectos normativos ainda estão sendo discutidos em âmbito nacional para o estabelecimento de uma legislação específica para a área de telemedicina. As normativas mais utilizadas como referência para definição do escopo de atuação profissional em telemedicina são: a RESOLUÇÃO CFM nº 1.643/2002, que define e disciplina a prestação de serviços através da Telemedicina e a RESOLUÇÃO CFM nº 2.107/2014, que define e normatiza a Telerradiologia e revoga a RESOLUÇÃO CFM nº 1890/09. Essa dissertação apresenta um estudo retrospectivo, quantitativo, descritivo, baseado em levantamento bibliográfico que tem como proposta verificar a conformidade das formas de uso da telemedicina e da telessaúde, nos contextos nacional e internacional, em relação ao modelo proposto para essa atividade pelo CFM, representado pelas resoluções 1.643/2002 e 2.107/2014. O levantamento bibliográfico foi feito no período de 2009 a 2015, nas bases de dados Scielo, PubMed e IEEE, com base nas palavras chaves videoconferencing, webconferencing, VTC, meeting, telemedicine e telehealth e suas combinações. Foram encontrados, incialmente, 2352 artigos que foram reduzidos para 572 após a leitura de seus resumos e, novamente reduzidos para 115, após a leitura parcial dos artigos. Os 115 artigos selecionados foram lidos na íntegra. Os resultados obtidos mostraram que o uso da telemedicina pode ampliar consideravelmente a área de atendimento especializado dos grandes centros médicos, o que deve ser incentivado, principalmente em países com menores recursos financeiros, ou com poucos especialistas, ou extremamente extensos, como no caso do Brasil
Despite technological advances aiming the best use of Information and Communication Technology in Healthcare, several normative aspects are still being discussed in national scope, in order to establish a specific legislation for telemedicine area. There are some specifically normative aspects for professional orientation in telemedicine as follows: CFM no. 1.643/2002 that define disciplines and services provided by telemedicine, and CFM no. 2.107/2014 that define and guide the teleradiology services. This work was a retrospective, quantitative and descriptive study based on bibliographic surveying, in order to verify the conformity of the use of telemedicine and telehealth in both national and international contexts, according to the CFM proposed model for this activity, represented by CFM resolutions no. 1.643/2002 and no. 2.107/2014. The bibliographic surveying between 2009 and 2015 used Scielo, PubMed, and IEEE databases, and the key words: videoconferencing, webconferencing, VTC, meeting, telemedicine, and telehealth plus their combinations. We found 2352 articles, reduced to 572 after reading their abstracts. One hundred and fifteen potentially relevant articles were selected after partial reading of the articles. After that, the 115 selected articles were fully read. The results showed that the use of telemedicine could considerably increase the area of specialized attending crew on important medical facilities, what should be encouraged mainly in countries with few or no financial resources, or the small number of specialists, or extensive areas such as Brazil

INTRODUÇÃO: A hanseníase persiste como grave problema de saúde pública no Brasil, e não foi atingida até o momento a meta de eliminação determinada pela Organização Mundial da Saúde. As Equipes de Saúde da Família (ESFs) constituem uma ampla rede de profissionais que atuam na atenção primária. A educação a distância em hanseníase destas equipes representa uma estratégia atraente. Ao permitir a identificação precoce de casos, seguida de tratamento adequado, favorece a cura e reduz incapacidades e/ou seqüelas. MÉTODOS: Foi planejado o desenvolvimento de um Curso de Extensão Universitária, utilizando tecnologias interativas voltadas para a educação a distância dos profissionais da saúde. Contou-se com planejamento pedagógico específico. Durante este processo, previu-se que os centros de referências em hanseníase pudessem atuar junto a estes profissionais que trabalham em pontos isolados e com alta prevalência da doença. Foi planejada a elaboração do ambiente de teleducação para oferecimento do curso, incluindo a criação de um simulador de casos voltado para o reconhecimento da doença, além de outras informações julgadas essenciais por dermatologistas. Elaborou-se uma metodologia para avaliar a adequação do seu conteúdo. Baseado no questionário validado WEBMAC (Web Site Motivational Analysis Checklist), um instrumento padronizado foi desenvolvido para avaliar a satisfação deste curso a distância. RESULTADOS: Foi realizado um curso de hanseníase a distância, voltado para as Equipes de Saúde da Família (ESF) nas cidades de Manaus e Parintins, contando com fundamentação pedagógica baseada em conceitos construtivistas e da andragogia. As ferramentas do curso viabilizaram a interação entre os professores e os participantes. O Simulador de Casos (SC) demonstrou diferenças na capacidade de reconhecimento de casos em três grupos distintos, formados por: dermatologistas com experiência específica em hanseníase, dermatologistas sem experiência específica e alunos do quinto ano de medicina. Os dermatologistas com experiência específica obtiveram um melhor desempenho no SC. O seu conteúdo mostrou-se adequado na avaliação dos três grupos. Os participantes apresentaram retenção dos conceitos abordados no SC após um ano (avaliação tardia). Foi obtido elevado grau de satisfação com o curso por meio de instrumento padronizado. CONCLUSÃO: A estratégia educacional mostrou-se factível, e os resultados alcançados favorecem o desenvolvimento futuro de iniciativa em larga escala destinada à educação a distância em Hanseníase das ESFs e a abordagem de outros temas em saúde.
INTRODUCTION: Leprosy remains as a serious public health problem in Brazil, and it was not yet achieved the goal of eliminating leprosy determined by the World Health Organization. Family Health Teams (FHTs) form a wide network of professionals who work in primary care. Distance education in leprosy of these teams represents an attractive strategy. Since it could allow early case identification, followed by appropriate treatment, it maximizes cure and reduces disability and/or sequels. METHODS: It was planned to develop an Extension University Course, using interactive technologies directed to distance education of health professionals. It had specific educational plan. During this process, it was decided that leprosy reference centers could act with these professionals who work in isolated areas and with a high prevalence of the disease. It included the creation of a case simulator toward disease recognition, and extra information considered essential by dermatologists. A methodology to evaluate adequacy of its contents was created. Based on WEBMAC (Web Site Motivational Analysis Checklist) validated questionnaire, a standardized instrument was developed to assess the satisfaction of this course. RESULTS: It was realized a leprosy distance course, directed to FHTs from the cities of Manaus and Parintins, that included pedagogical fundamental based on constructivist and andragogic concepts. Course tools enabled interaction between teachers and participants. Case Simulator (CS) disclosed differences in the ability of recognition of cases into three distinct groups, formed by: dermatologists with specific experience in leprosy, dermatologists without specific experience and fifth year students of medicine. The dermatologists with specific experience obtained a better performance in the CS. Its content assessed by all three groups seemed to be appropriated. Participants showed retention of the concepts cited in the CS after a year (late evaluation). The course obtained high degree of satisfaction and it was demonstrated by standardized instrument. CONCLUSION: The course proved to be feasible, and results favor the development of future large-scale initiative focused on distance education in leprosy for FHTs and the approach to others health subjects.

Introdução: A Telessaúde caracteriza-se pelo uso da tecnologia de informação e comunicação na área da saúde. Na Fonoaudiologia, o uso desta ferramenta por meio da Teleducação e Teleassistência se caracteriza como uma importante estratégia que leva a informação para alunos, profissionais, pacientes e comunidade em geral. Esse fator, aliado à grande extensão geográfica do Brasil e a distribuição heterogênea dos profissionais da saúde, vem ao encontro da necessidade de atendimento especializado para pacientes e descentralização do conhecimento para profissionais. Neste contexto, o uso da Teleducação emerge como uma proposta para minimizar as dificuldades encontradas pelo paciente, no processo de adaptação do aparelho de amplificação sonora individual (AASI), na qual muitos materiais educacionais são produzidos com o intuito de orientar e facilitar a adaptação do indivíduo deficiente auditivo, à sua nova realidade. Objetivo: Este projeto tem por objetivo elaborar um programa de capacitação por meio da Teleducação Interativa, sobre o aparelho de amplificação sonora individual, e analisar a eficácia do mesmo, quanto ao aprendizado e motivação dos profissionais. Resultados: A primeira etapa compreendeu a elaboração do programa de capacitação, na qual foi desenvolvido um cybertutor sobre o tema AASI e as ferramentas fórum de discussão e web conferência, com o intuito de complementar o aprendizado. Na segunda etapa, referente à análise do programa, verificou-se que não houve significância estatística no aspecto relacionado ao aprendizado do conteúdo, na avaliação pré e pós teste. Em relação à motivação dos profissionais, o estudo obteve resultado positivo, sendo avaliado como curso impressionante. Discussão: O programa de capacitação foi desenvolvido, respaldado em estudos voltados para elaboração de materiais didáticos à distância. No que se referem aos resultados da avaliação, todas as dificuldades encontradas neste aspecto corroboraram com a literatura. Conclusão: A elaboração do Programa de Capacitação por meio da Teleducação Interativa sobre o tema aparelho de amplificação sonora individual (AASI) foi realizada satisfatoriamente, alcançando um índice de motivação em relação à participação altamente positivo, porém os resultados da comparação da avaliação da situação problema pré e pós teste não foram estatisticamente significantes.
Introduction: Telehealth is characterized by the use of information technology and communication in healthcare. In audiology, the use of this tool through Telecare and Tele-education is characterized as an important strategy that leads the information for students, professionals, patients and the general community. This factor, combined with the large geographical extent of Brazil and the heterogeneous distribution of health professionals, meets the need of specialized care for patients and decentralization of knowledge for professionals. In this context, the use of Tele-education has emerged as a proposal to minimize the difficulties encountered by the patient in the process of adaptation of a hearing aids (HA), in which many educational materials are produced in order to guide and facilitate the adaptation HA to their new reality. Objective: This project aims to develop a training program through the Interactive Tele-education on the individual sound amplification devices, and analyze the effectiveness of the same as learning and motivation. Results: The first stage of the preparation of the training program, which was developed on the theme Cybertutor hearing aids and tools discussion forum and web conferencing in order to supplement learning. In the second stage, regarding the analysis of the program, it was found that there was no statistical significance in the learning aspect of the content, in the pre and post test. Regarding the motivation of staff, the study was positive, which was assessed as \"impressive progress\". Discussion: The training program was developed, backed up by studies aimed at developing distance learning materials. In that refer to results of the evaluation, all difficulties found in this aspect corroborated with the literature. Conclusion: The development of the Training Program through the Interactive Tele-education on the subject of HA was performed satisfactorily, reaching a level of motivation in relation to participation highly positive, but the comparison results of the evaluation of the problem situation pre and post-test were not statistically significant.

Information and communication  technologies  (ICTs)  are  having a great  capability to face some challenges in both developed as well as developing countries which includes providing access,  cost  effective  and  high  quality  health  care  services.  Telemedicine  uses  ICT  to overcome  geographical   barriers,  and  increase  access  to  health  care  services.  This  is particularly beneficial for rural and underserved communities in India. This paper focuses on the rising profile of e-health on the international policy agenda and related developments  in the  National  Health Service. Few developments  in public health today create energy and opportunity present in the word "e-health". As a developing country, India is implementing the technology to deliver health information and services regarding health widely across the country. These solution  though promising also present new challenges particularly in acceptable standards, choice of technologies, overcoming traditional jurisdictional boundaries, investment, privacy and confidentiality.   In this study, examine current state of telemedicine in a developing country, India. Telemedicine has brought several benefits to the people of India, especially those living in rural and remote areas. Discussed the Indian telemedicine implementation case of Apollo Hospital and Chakrajmal village, consolidate lessons learned from cases, and culminate with potential researchable critical success factors that account for the growth and modest successes  of telemedicine  in India. This  research will  discuss  how e-health  provides the opportunity for patients to maintain longer independence and for the providers to monitor    a condition more closely. It also describes barriers and challenges for current status of e­- health. Also, an attempt is made for presenting future e-health status in Indian scenario. Further,some  recommendations  are provided for  improvement.

Durante as últimas décadas, as organizações de saúde têm vindo a adotar continuadamente as tecnologias de informação para melhorar o funcionamento dos seus serviços. Recentemente, em parte devido à crise financeira, algumas reformas no sector de saúde incentivaram o aparecimento de novas soluções de telemedicina para otimizar a utilização de recursos humanos e de equipamentos. Algumas tecnologias como a computação em nuvem, a computação móvel e os sistemas Web, têm sido importantes para o sucesso destas novas aplicações de telemedicina. As funcionalidades emergentes de computação distribuída facilitam a ligação de comunidades médicas, promovem serviços de telemedicina e a colaboração em tempo real. Também são evidentes algumas vantagens que os dispositivos móveis podem introduzir, tais como facilitar o trabalho remoto a qualquer hora e em qualquer lugar. Por outro lado, muitas funcionalidades que se tornaram comuns nas redes sociais, tais como a partilha de dados, a troca de mensagens, os fóruns de discussão e a videoconferência, têm o potencial para promover a colaboração no sector da saúde. Esta tese teve como objetivo principal investigar soluções computacionais mais ágeis que permitam promover a partilha de dados clínicos e facilitar a criação de fluxos de trabalho colaborativos em radiologia. Através da exploração das atuais tecnologias Web e de computação móvel, concebemos uma solução ubíqua para a visualização de imagens médicas e desenvolvemos um sistema colaborativo para a área de radiologia, baseado na tecnologia da computação em nuvem. Neste percurso, foram investigadas metodologias de mineração de texto, de representação semântica e de recuperação de informação baseada no conteúdo da imagem. Para garantir a privacidade dos pacientes e agilizar o processo de partilha de dados em ambientes colaborativos, propomos ainda uma metodologia que usa aprendizagem automática para anonimizar as imagens médicas
During the last decades, healthcare organizations have been increasingly relying on information technologies to improve their services. At the same time, the optimization of resources, both professionals and equipment, have promoted the emergence of telemedicine solutions. Some technologies including cloud computing, mobile computing, web systems and distributed computing can be used to facilitate the creation of medical communities, and the promotion of telemedicine services and real-time collaboration. On the other hand, many features that have become commonplace in social networks, such as data sharing, message exchange, discussion forums, and a videoconference, have also the potential to foster collaboration in the health sector. The main objective of this research work was to investigate computational solutions that allow us to promote the sharing of clinical data and to facilitate the creation of collaborative workflows in radiology. By exploring computing and mobile computing technologies, we have designed a solution for medical imaging visualization, and developed a collaborative system for radiology, based on cloud computing technology. To extract more information from data, we investigated several methodologies such as text mining, semantic representation, content-based information retrieval. Finally, to ensure patient privacy and to streamline the data sharing in collaborative environments, we propose a machine learning methodology to anonymize medical images.

Abstract:
According to a recent medical article concerning the Navajo nation, the death rates that result from illnesses such as diabetes and tuberculosis are much higher among Native Americans in comparison to non-Native individual s (190% higher and 7S001o higher respectively). This paper looks at how the broadening institution of telemedicine can be applied to the Navajo nation in a constructive and helpful manner. The paper begins by looking at both the traditional beliefs of Navajo medicine as well as the overall pivotal considerations of telemedicine such as the advantages and barriers presented. The paper brings these two parameters into conjunction by proposing ideas of telemedicine implementation on the Navajo reservations and by examining uses of telemedicine that are currently occurring on these same reservations. Overall, the paper concluded that telemedicine should be used as the prevalent fixture on the reservations while also taking into account the Navajo traditional medical practices and preferences.

The concept of the project taken from Telemedicine. It is a rapidly developing application of clinical medicine where medical information is transferred through interactive audiovisual media for the purpose of consulting, and sometimes remote medical procedures or examinations. Telemedicine may be as simple as two health professionals discussing a case over the telephone, or as complex as using satellite technology and videoconferencing equipment to conduct a real-time consultation between medical specialists in two different countries. Telemedicine generally refers to the use of communications and information technologies for the delivery of clinical care. Care at a distance (also called in absentia care), is an old practice which was often conducted via post. There has been a long and successful history of in absentia health care which, thanks to modern communication technology, has evolved into what we know as modern telemedicine. In its early manifestations, African villagers used smoke signals to warn people to stay away from the village in case of serious disease. In the early 1900s, people living in remote areas in Australia used two-way radios, powered by a dynamo driven by a set of bicycle pedals, to communicate with the Royal Flying Doctor Service of Australia
Advances in sensor technology, personal mobile devices, and wireless broadband communications are enabling the development of an integrated personal mobile health monitoring system that can provide patients with a useful tool to assess their own health and manage their personal health information anytime and anywhere. Personal mobile devices, such as PDAs and mobile phones, are becoming more powerful integrated information management tools and play a major role in many people's lives. Here I focus on designing a Mobile health-monitoring system for people who stay alone at home or suffering from Heart Disease. This system presents a complete unified and mobile platform based connectivity solution for unobtrusive health monitoring. Developing a hardware which will sense heart rate and temperature of a patient, using Bluetooth modem all information lively transmitted to smart phone, from smart phone all information transmitted to server using GPRS. At server the received data compared with the standard threshold minimum and maximum value. The normal range of heart rate is 60 to 135 and the temperature of the patient is said to be normal above 95^F and below 104^F. If at all the rate increases above 145 or decreases below 55,it may be fatal and if it crossed this threshold limit then SMS will be sent to the relative of patient and Doctors along with measured values. The build-in GPS further provides the position information of the monitored person. The remote server not only collects physiological measurements but also tracks the position of the monitored person in real time. For transmitting data from Smartphone to the server using GPRS, here we need to create a website on data will be continuously transmitted from Smartphone to the website and from website data will be downloaded continuously on the server. Thus the system helps in tracking down the patient without getting the patient into any sort of communication. Undue mishaps can be avoided within the golden hours after a patient is struck with a heart attack.

Objetivos: Avaliar a concordância no diagnóstico das lesões cutâneas pigmentadas por meio do exame dermatoscópico presencial e a distância. Analisar a concordância diagnóstica da teledermatoscopia e do exame dermatoscópico presencial em relação ao resultado anatomopatológico. Verificar a acurácia, a sensibilidade e a especificidade da teledermatoscopia na detecção das lesões pigmentadas malignas. Métodos: Sessenta e quatro lesões cutâneas pigmentadas foram submetidas a exame clínico e dermatoscópico realizado presencialmente por dois examinadores com experiência em dermatoscopia, durante o ano de 2005, no ambulatório de dermatoscopia do Departamento de Dermatologia da Universidade Federal de São Paulo (UNIFESP). Após dois anos, imagens digitais clínicas e dermatoscópicas destes casos foram reexaminadas pelos mesmos especialistas por meio da transmissão de imagens e de dados clínicos via web. Resultados: Observou-se boa concordância diagnóstica entre o exame presencial e a teledermatoscopia com valor de Kappa de 0,698 (IC a 95% - [0,575; 0,821]). Verificou-se também boa concordância entre o exame presencial e a teledermatoscopia com o resultado anatomopatológico (padrão ouro) com valores de Kappa de 0,728 (IC a 95% - [0,608; 0,848]) e 0,656 (IC a 95% - [0,526; 0,785]), respectivamente. Constatou-se 84% de acurácia, 93% de sensibilidade e 78% de especificidade na detecção das lesões pigmentadas malignas no exame dermatoscópico a distância. Dois casos falsos negativos foram observados pela teledermatoscopia. Não houve diferença estatisticamente significante entre a sensibilidade, a especificidade e a acurácia das duas modalidades diagnósticas. Conclusões: O exame dermatoscópico a distância pela transmissão de imagens digitais e de dados clínicos proporcionou acurácia diagnóstica, sensibilidade e especificidade comparáveis ao exame convencional. A teledermatoscopia mostrou ser método eficaz e confiável na detecção das lesões malignas constituindo ferramenta importante na triagem das lesões pigmentadas. A boa concordância diagnóstica entre a avaliação via web e o exame presencial destaca a aplicabilidade da teledermatoscopia como instrumento de assistência especializada em regiões de difícil acesso ou carentes no setor de saúde.
Objectives: To evaluate agreement in diagnosis of pigmented skin lesions through presential or remote dermoscopic examination. To analyze diagnosis agreement of teledermoscopy and presential dermoscopic examination regarding pathological findings. To verify accuracy, sensibility and specificity of teledermoscopy in detection of malignant pigmented lesions. Methods: A total of 64 pigmented skin lesions were submitted to clinical and dermoscopic examinations performed presentially by two examiners experienced in dermoscopy, at the Dermoscopy Outpatient’s Clinic, Department of Dermatology, Universidade Federal de São Paulo (UNIFESP), in 2005. Two years later, the digital clinical and dermoscopic images of these cases were re-examined by the same specialists through transmission of images and clinical data via web. Agreement between presential diagnosis and telediagnosis was assessed by the Kappa (κ) statistic calculation and its respective 95% confidence interval (95% CI). Through this calculation, the agreement between presential presumptive diagnoses and telediagnoses with pathological findings was analyzed. Sensitivity, specificity and accuracy of two diagnostic modalities were obtained using the pathological examination as the gold standard. The respective 95% confidence interval (95% CI) was calculated for each diagnostic measurement. Results: Good diagnostic agreement was observed between presential examination and teledermoscopy, with a Kappa value of 0.698 (95% CI - [0.575; 0.821]). Good agreement was also found between conventional examination and teledermoscopy, and the pathological findings (gold standard) had Kappa values of 0.728 (95% CI - [0.608; 0.848]) and 0.656 (95% CI - [0.526; 0.785]), respectively. Accuracy of 84%, sensitivity of 93% and specificity of 78% were observed in detecting malignant pigmented lesions by remote dermoscopic examination. Two false negative cases were observed through teledermoscopy. There was no statistically significant difference in sensitivity, specificity and accuracy of both diagnostic methods. Conclusions: Remote dermoscopic examination by means of digital image transmission and of clinical data provided diagnostic accuracy, sensitivity and specificity comparable to conventional examination. Teledermoscopy proved to be an efficient and reliable method to detect malignant lesions, representing an important tool to screen pigmented lesions. The good diagnostic agreement between assessment via web and conventional examination highlights the applicability of teledermoscopy as a specialized assistance tool in regions that are difficult to reach or lack health resources.

Abstract:
Telemedicine is the combination of information technology (IT) and health care and is an area that has seen increase during the last few decades due to breakthroughs in IT. Today there are still parts of the health care system that can be counterproductive with regards to the time and commitment necessary. One such part is medical consultations for private individuals where many of the simpler ones are still carried out in-person. Presented in this thesis is the implementation of a prototype telemedicine system that could be used as a substitute for in-person doctor-patient consultation. Necessary 3rd party components and their implementations into one system are described on a general use level. The result is a web application that supports login and payment via trusted sources, booking of meetings and carrying them out with video and audio. The prototype is a continuation of a telemedicine web application idea that was put on ice by a company that already provides solutions for health controls and blood analysis.
Telemedicin är kombinationen av informationsteknik (IT) och hälsovård och är ett område som har sett en ökning de senaste årtiondena på grund av genombrott inom IT. Idag finns det fortfarande delar av hälsovården som kan vara kontraproduktiva med avseende på den tid och det åtagandet som krävs. En sådan del är medicinska konsultationer för privatpersoner där många av de simplare mötena fortfarande sker i person. Presenterat i denna avhandling är implementationen av ett distribuerat telemedicinsystem som kan användas som ett substitut för de konsultationer mellan doktor och patient som sker i person. Nödvändiga tredjeparts komponenter och deras implementationer in ett system är beskrivna på en allmän användningsnivå. Resultatet är en webbapplikation som stödjer login och betalning via en pålitligt källa, bokning av möten och utförandet av dem med bild och ljud. Den presenterade prototypen är en fortsättning på en telemedicinsk webbapplikationsidé som lagts på is av ett företag som redan erbjuder lösningar för hälsokontroll och blodanalys.

Tavarua is a platform designed to support mobile telemedicine systems over wireless wide area networks, WWANs. It utilizes network striping and other complementary techniques to send uni-directional near real time video and audio data streams from a imobile node to a stationary location. The key technical challenge is transmitting high-bandwidth, loss-sensitive data over multiple low-bandwidth, lossy channels. We overcome these challenges using dynamic adjustment of the encoding parameters and a novel video encoding technique (grid encoding) that minimizes the impact of packet losses. Using five WWAN interfaces, our system reliably and consistently transmits audio and diagnostic quality video, with median PSNR values that range from 33.716dB to 36.670dB, with near real-time latencies. We present a study of the characteristic behavior of WWANs, and a description of our system architecture based in part on the lessons gleaned from that study. Through a set of experiments where we transmit video and audio data from a moving vehicle we evaluate the system, focusing on consistency, reliability, and the quality of the audio and video streams. These experiments demonstrate that we can transmit high quality video and audio in varying conditions and even in the presence of hardware failures.

ENGLISH ABSTRACT: A telemedicine service is a healthcare service (-medicine) that is delivered over a distance (tele-). The interest in the potential of telemedicine to increase the quality, accessibility, utilization, e ciency and e ectiveness of healthcare services is fuelled by the rapid development of information and communication technology (ICT) and connectivity. Despite this potential, the success rate of telemedicine services disappoints. Many mistakes in the implementation of telemedicine services are repeated over and over again and best practices are not captured and replicated. This study responds to the need for reference models for the assessment and optimization of telemedicine services in a consistent, systematic and systemic way. Maturity models are reference models that describe typical patterns in the development of organizational capabilities and depict a sequence of stages towards the desired state. Many reference models exist that are applicable to telemedicine services, but none of these provide guidance for the optimization of services, like a maturity model does. Many maturity models exist within a health systems context, but none of these can be applied "as is" to telemedicine services. In this study an iterative top-down design approach is followed to develop a Telemedicine Service Maturity Model (TMSMM). This model facilitates the assessment of a telemedicine service on micro, meso, and macrolevel along all the domains that comprise the telemedicine health system. Sets of capability statements are de ned, which follow each other in a cumulative manner, hence providing a maturation path towards the desired maturity state. These sets of capability statements provide yardsticks according to which quantitative values are allocated to an intangible concept, such as maturity. Once an individual service is assessed, further actions towards the optimization of the service can be derived from these yardsticks. The multidimensional design of the TMSMM, as well as the fact that capability statements facilitate the consistent quanti cation of maturity, makes it possible to analyze the aggregated results of cohort of services. To accomplish this, principles of business intelligence and data warehouse design are applied together with online analytic processing (OLAP) procedures. The TMSMM addresses the previously unful lled need for a reference model to assess and optimize telemedicine services in a consistent, systematic and systemic way. This study spans several academic and professional domains and thereby contributes to the scienti c world of telemedicine and ehealth.
AFRIKAANSE OPSOMMING: 'n Telegeneeskunde diens is 'n gesondheidsdiens (-geneeskunde) wat oor 'n afstand gelewer word (tele-). Met die snelle ontwikkeling van inligtings-en kommunikasietegnologie hou telegeneeskunde die potensiaal in om die kwaliteit, toeganklikheid, benutting, doelmatigheid en doeltre endheid van gesondheidsdienste te verhoog. Ten spyte van hierdie potensiaal, stel die aantal onsuksesvolle telegeneeskunde dienste teleur. Heelwat foute in die implementering van telegeneeskundedienste word oor en oor gemaak, terwyl die beste praktyke nie vasgevang en herhaal word nie. Hierdie studie is onderneem in reaksie op die behoefte aan 'n verwysingsmodel vir die assessering en optimering van telegeneeskunde dienste op 'n konsekwente, sistematiese en sistemiese manier. ENGLISH ABSTRACT: Primary health care telemedicine services require the acquisition and transmission of patient data including high quality still and video images via telecommunication networks. The objective of this thesis is to investigate the implementation of a generalpurpose medical camera as an alternative to the complex and costly CCD based cameras generally in use at present. The design is based on FillFactory’s SXGA (1280 ×1024) CMOS image sensor. A low-cost Altera Cyclone FPGA is used for signal interfacing, filtering and colour processing to enhance image quality. A Cypress USB 2.0 interface chip is employed to isochronously transfer video data up to a maximum rate of 23.04 MBytes per second to the PC. A detailed design and video image results are presented and discussed; however the camera will need repackaging and an approval for medical application by medical specialists and concerned bodies before releasing it as full-fledged product.

Patients admitted to the Intensive Care Unit (ICU) are critically ill and often at extremely high risk of death. These patients receive aggressive interventions to prolong their lives. Despite these measures, many patients still succumb to their illness. Although ICU physicians are good at predicting which patients have a high risk of mortality, they are still offering interventions that do not prolong life, but potentially cause more suffering at the end of life. This is because there is a lack of high quality and early communication to discuss prognosis and establish patients' goals of care. This gap in communication is even more profound when patients are transferring from rural hospitals to busy tertiary care centers. This dissertation discusses the utilization of tele-video conferencing to enhance early communication with family members/loved ones of critically ill patients prior to their transfer from a rural hospital to a tertiary care center. It begins with a description of telemedicine and its uses in the ICU to date. Chapter 2 discusses the poor prognoses of patients receiving high intensity interventions such as cardiopulmonary resuscitation (CPR). The extremely dismal outcomes underscore the importance of early, thorough discussions regarding prognosis and goals of care in these patients. The next chapter describes a pilot study utilizing telemedicine to conduct formal unstructured telemedicine conferences with family members prior to transfer. This study demonstrated that palliative care consultations can be provided via telemedicine for critically ill patients and that adequate preparation and technical expertise are essential. Although this study is limited by the nature of the retrospective review, it is evident that more research is needed to further assess its applicability, utility and acceptability. Chapter 4 describes an investigation into the barriers and facilitators of conducting conferences via telemedicine and the perceptions of clinicians regarding the use of telemedicine for this purpose. This chapter identified unique barriers and facilitators to the use of telemedicine that will need to be addressed when designing a telemedicine intervention for conducting family conferences. This thesis describes the importance and process of implementation of telemedicine for the novel purpose of enhancing early communication among physicians and family members of critically ill loved ones. Further studies are needed to refine and investigate patient and family centered clinical outcomes utilizing this intervention.

Abstract:
Virtual Reality (VR) is a highly evolving domain and is used in anincreasing number of areas in today's society. Among the technologiesassociated with VR and especially mobile VR, is hand tracking and handgesture recognition. Telemedicine is one of the elds where VR is startingto thrive, and so the concept of adding the use of hand gestures came to bein order to explore the possibilities that can come from it. This researchis conducted with the development of a prototype application that usessome of the most emerging technologies. Manomotion's hand trackingand hand gesture recognition algorithms, and Photon's servers and developerkit, which makes multi-user applications achievable, allowed theconceptual idea of the prototype to become reality. In order to test itsusability and how potential users perceive it, a user study with 24 participantswas made, 8 of which were either studying or working in themedical eld. Additional expert meetings and observations from the userstudy also contributed to ndings that helped show how hand gesturescan aect a doctor consultation in Telemedicine. Findings showed thatthe participants thought of the proposed system as a less costly and timesaving solution, and that they felt immersed in the VR. The hand gestureswere accepted and understood. The participants did not have dicultieson learning or executing them, and had control of the prototype environment.In addition, the data showed that participants considered it to beusable in the medical eld in the future.

Objective: Evaluate The Ottawa Hospital Viral Hepatitis Program (TOHVHP) telemedicine (TM) program for patient retention, treatment initiation and sustained virologic response (SVR) rates. Methods: Retrospective analysis of TOHVHP cohort data for patients entering HCV care between 2012 and 2016. Logistic regression modeling was used to assess characteristics associated with patient retention, treatment initiation, and achieving SVR. TM outcomes were compared to the standard outpatient clinic and mixed delivery outcomes. Results: Treatment initiation rates were comparable between TM and the outpatient clinic. TM delivered Direct Acting Antiviral treatments achieved high SVR outcomes across all patient populations. Patient retention was lower among TM patients. Conclusion: TOHVHP TM program engaged patients facing barriers to traditional HCV care models. Efforts to improve TM retention are needed.

Bakgrund   I takt med samhällets digitalisering har telemedicin, vård på distans, kommit att spela en allt mer framträdande roll inom hälso-och sjukvården. Inom ramen för det digitala kan patienter numera uppsöka samt erhålla vård från sina egna hem. Den ökade tillgängligheten som telemedicinen för med sig kan fostra en mer aktiv patientroll inom vilken egenvården är av särskild betydelse. Lärdomar kan tas från patienter som använt sig av telemedicinska tjänster och genom att utforska deras upplevelser kan möjligheter och hinder identifieras.  SyfteSyftet var att beskriva patienters upplevelser av egenvård vid användning av telemedicin. Metod Denna studie är utförd som en litteraturöversikt och baserade sig på 16 relevanta vetenskapliga artiklar med en kvalitativ ansats. Artiklarna har med olika sökordskombinationer valts ut från databaserna PubMed och CINAHL. De valda artiklarna kvalitetsgranskades enligt Sophiahemmet högskolas bedömningsinstrument varpå en integrerad innehållsanalys utfördes. Resultatartiklarna delades in i tre kategorier med sex tillhörande underkategorier.     Resultat Resultatet delades in i tre kategorier; egenvårdsbehov, egenvårdskapacitet samt egenvårdsbrist. Den första kategorin handlar om patienters upplevelser av autonomi och tillgänglighet. Den andra kategorin behandlar patienters nya roll samt upplevelser av delaktighet. Den tredje kategorin behandlar aspekterna relation och partnerskap.   Slutsats  Denna studie belyser hur patienter ser både för- och nackdelar med telemedicin. Patienter upplever att telemedicin i mångt och mycket har kapacitet att förstärka deras möjligheter till egenvård och delaktighet men att det finns brister. Telemedicinen är till för alla men förutsätter ett visst krav på tekniskt kunnande samt en vilja till förändring vilket riskerar att utelämna patienter som inte vill eller har möjlighet. Därmed bör lärdom tas från patienternas erfarenheter för att på så sätt använda telemedicinen som ett verktyg för att bidra till en god och jämlik hälso- och sjukvård där alla inkluderas.

Use of telemedicine as a method of health care delivery, particularly interactive video, has seen a surge in popularity in recent years. However, no standard method has been developed to determine the net benefit to society of these applications. Any such method should recognize that the implementation of telemedicine may affect both access to health care and its quality, as well as the tradeoff between them. This thesis explores the issues surrounding evaluation of telemedicine from an economic benefit-cost analysis standpoint and discusses alternative methods of benefit estimation. Available data are employed to estimate averted cost measures for consumers and isolate determinants of telemedicine use rates for mental health consultations. In addition, data needs for alternative methods of benefit estimation are identified.

Introduction In Senegal, as in many countries, physicians are unevenly distributed. This situation leads to health inequalities, but the solutions adopted to distribute physicians equitably do not give satisfactory results. This situation pushes decision-makers and researchers to look for other solutions such as telemedicine. This technology-based solution is considered to be a good means to improve physician recruitment and retention in underserved areas, thus improving access to healthcare in these areas, but it is still underused in Senegalese health facilities. Objective This study aimed to identify the determinants of the use of telemedicine in Senegal. More specifically, it aimed to identify the individual (micro) and contextual (meso and macro) factors that influence the use of telemedicine in Senegal. Method The study involved mixed methods: a descriptive qualitative study involving individual interviews and a cross-sectional quantitative study involving questionnaires. The first method was used to study the individual (micro) factors such as physicians’ beliefs on the use of telemedicine and physicians’ perception of the impact of telemedicine on their recruitment and retention in underserved areas. It was also used to study the contextual meso (technical, organizational and ethical) and macro (financial, political, legal and socioeconomic) factors that influence the use of telemedicine. The second method was used to study participants’ intention to use telemedicine in their professional activities, another individual (micro) factor. This study included physicians working in public hospitals, those working in district health, and telemedicine projects managers. Qualitative data was collected through individual interviews, transcribed and imported into NVivo 10 where they were coded thematically. Quantitative data was collected by administering questionnaires and analyzed using SPSS 23. Results The results showed that telemedicine could contribute to, but could not guarantee the recruitment and retention of Senegalese physicians in underserved areas. It was found that physicians in Senegal have both positive and negative behavioural, normative and control beliefs that can encourage or discourage them from using telemedicine in their professional practice. The most salient behavioural beliefs were that telemedicine makes experts’ opinions accessible across distances (positive behavioural belief), but could lead to medical errors (negative behavioural belief). The most common normative beliefs were that their patients would most probably approve the use of telemedicine (positive normative belief) while their colleagues working in the public sector might disapprove of it (negative normative belief). The most reported control beliefs were that the use of telemedicine is easy (positive control belief), but could be time-consuming (negative control belief). For physicians in Senegal, the findings also demonstrate that the probability that they will use telemedicine in their professional activities is moderate and influenced by their perceived behavioural control which, in turn, is influenced by their positive and negative control beliefs. Finally, the results demonstrate that many contextual meso (technical, organizational and ethical) and macro (financial, political, legal and socioeconomic) factors influence the use of telemedicine in Senegal. Conclusion The results of this study can inform the choice of telemedicine development strategies. These strategies can help to promote the use of telemedicine in Senegal, which in turn can help to increase physician recruitment and retention in underserved areas, resulting in better access to healthcare and, and as a result improved population health.

Abstract:
Diabetes mellitus is a chronic disease in which the body does not produce sufficient insulin or in which the body has high insulin resistance thus making the regulation of blood glucose metabolism difficult. Currently, diabetes is still incurable. All patients need to well manage their blood glucose levels to reduce the risk of complications. This dissertation is comprised of two major studies. In diabetes type I, the blood glucose can only be managed by multiple daily injection of insulin. However, most patients tend to have difficulty in deciding the right amount of insulin dose. The first study is the development of a mathematical model of blood glucose levels, which leads to the development of a decision support system for diabetes type I using the Markov theory. In some type II and gestational diabetes, blood glucose can be managed by choosing diet properly and by exercising regularly. However, people tend to overestimate their activity levels. The second study describes the design and development of a wearable device based on the triaxial accelerometer that estimates the energy levels of normal daily physical activity with comparable accuracy to the gas analysis. This device development leads to two clinical studies. The first clinical study investigates whether the energy monitor could help people with diabetes in promoting and managing their daily activity and help to improve the glycosylated haemoglobin and body mass index. The second clinical study investigates whether the energy monitor could help pregnant women with gestational diabetes in managing their daily activity, blood glucose levels and body weight gain. This thesis also develops a telemedicine system to automate the data collection during the clinical trial period. The system would securely transmit all diabetes and energy data from the participants' home to a remote server. A key finding of this study was that a higher activity score results in smaller fluctuations in blood glucose levels between measurements in both diabetes and gestational diabetes subjects. This suggests that higher activity levels would make the management of diabetes more effective by reducing the fluctuation in blood glucose levels.

The advent of multimedia computing has lead to an increased demand for digital images and videos (sequence of frames). Telemedicine is a major application of digital image processing and is increasingly deployed over different venues. However, there are still many challenges associated with this technology, especially on high quality real-time video compression. Wavelet transform has been becoming important in image compression applications because of its flexibility and efficiency in representing non-stationary signals. Generally, wavelet based compressions do not result in blocky effects and/or artifacts. However, existing wavelet-based compression systems are computationally intensive for real-time transmission of videos. In this thesis, we propose a novel region of interest (ROI) wavelet-based real-time video compression technique that is especially suited for telemedical applications. We shall demonstrate that with currently available and affordable hardware, the proposed ROI-based compression technique can simultaneously meet the stringent constraints imposed by the special requirements of telemedicine on processing time, bandwidth and high quality. The region of interest (e.g. the area of medical examination/surgery) in the decompressed images will have the highest possible image quality throughout the transmission at the expense of the unimportant outside region. Corresponding to the novel ROI-based wavelet transformation, suitable quantization and coding schemes are explored and incorporated into a complete video compression and transmission system. Factors that influence the ROI system performance with respect to computation time, compression ratio and video quality, are investigated with the aim of providing relevant suggestions on how to achieve desired performances. Software and user requirement factors include video frame size, the size of the ROI, the choice of wavelet filter, the scheme and depth of decomposition, and the quantization and coding schemes. Other performance influencing factors relate to the implementation platform, such as the local machine performance and network bandwidth. Comprehensive testing on the various software influencing factors and the CPU speed win be carried out to demonstrate the viability of the ROI system on currently available technology.

Adverse events and medical errors plague the healthcare system. Hospital acquired infections and teamwork are some of the biggest contributor to these adverse outcomes. In an effort to mitigate these problems, administrators and clinicians alike have developed mechanisms, such as telemedicine. However, little research has been conducted investigating the role of telemedicine on teamwork -- a fundamental component of quality patient care. The purpose of this study is to gain a better understanding of the impact of telemedicine on teamwork behaviors and subsequent teamwork attitudes and cognitions during a common medical task, rounds within the Trauma-Intensive Care Unit. To this end, rounds were conducted with and without telemedicine. During this 60 day period, 16 clinicians completed three surveys and 34 rounds were video recorded. The results of this study suggest that the relationships between teamwork attitudes, behaviors, cognitions, and outcomes are differential impacted under conditions with and without telemedicine. More specifically, telemedicine is associated with an increase in attendance and communication density. Meanwhile, it does not significantly impact teamwork attitudes or cognitions. The primary implications of these findings indicate that telemedicine is not the solution for improving all teamwork elements but yet it is not a complete detriment either.
