Gout typically presents as recurrent flares of acute self-limiting arthritis. The acute gout flare is characterized by severe joint inflammation. In the presence of prolonged untreated hyperuricaemia, some people with gout may develop gouty tophi, which cause retracted joint movement, ulceration, and joint damage. The differential diagnosis for gout includes septic arthritis (which may co-exist with gout), joint injury, calcium pyrophosphate deposition, basic calcium phosphate arthritis or tendinitis, reactive arthritis, and psoriatic arthritis.

Abstract:
Chronic shoulder pain has multiple etiologies, including tendon tears (rotator cuff or biceps tendon), subacromial impingement (leading to subacromial bursitis or tendinitis), osteoarthritis, and shoulder joint instability. The condition is often associated with limited mobility; therefore, most interventions aim at preserving range of motion. Among the various treatment modalities, physical therapy is an effective first intervention. Corticosteroid injections do not necessarily help all chronic shoulder pain but may allow effective physical therapy for functional improvement. Multimodal analgesics and modulating agents (anti-inflammatories, antiepileptics, etc.) can also be used as part of a conservative regimen. Surgery is recommended when conservative therapy fails.

Abstract:
Basic calcium phosphate (BCP) crystals deposit within periarticular and articular sites, leading to a variety of clinical presentations. The most well-recognized clinical syndromes associated with BCP crystal deposition are calcific tendinitis and BCP crystal-associated destructive arthritis such as Milwaukee shoulder syndrome. There is emerging evidence that BCP crystals also play a role in osteoarthritis pathogenesis. Within the joint, tissue responses to BCP crystals include induction of pro-catabolic and inflammatory pathways. Clinical management of BCP crystal-associated arthropathies is limited by the lack of reliable and feasible methods to detect crystals, and the current unavailability of systemic therapies that promote BCP crystal dissolution.

Abstract:
This chapter discusses sonographically guided elbow, tendon, and joint procedures used in the management of patients presenting with tendon and joint disorders of the elbow. Two sonographically guided corticosteroid injection techniques have been used for common extensor tendinosis, a superficial technique and a deep technique. Sonographically guided percutaneous longitudinal tenotomy has also been described for chronic flexor-pronator tendinosis. Distal biceps tendinopathy is a broad term that includes both inflammatory (tendinitis) and chronic degenerative (tendinosis) conditions. As the proximal portion of the distal biceps tendon can be easily palpated anteriorly, an anterior injection approach will typically provide uncomplicated access to the tendon and/or adjacent bursal fluid. Common elbow conditions are amenable to intra-articular elbow aspiration and injections (typically with corticosteroid).
