Full textIntroduction: At menopause the ovarian synthesis of estrogen and progesterone decreases, leading to a variety of symptoms and physiological changes that according to its intensity may influence women’s quality of life. Among available therapies, hormone replacement therapy (HRT) is the most widely used to relieve its symptoms and consequences. The decision to start HRT is complex: benefits must be weighed against risks and their impact on quality of life. Objectives: To analyze the perceptions and prescribing patterns of “general and family medicine” (GFM) and “gynecology/obstetrics” (G/O) specialists. Materials and Methods: A cross-sectional observational study was conducted. A selfadministered questionnaire was distributed to “gynecology/obstetrics” and “general and family medicine” specialists attending the “XI Portuguese Congress of Gynecology” and the “XIV National Congress of General and Family Medicine”, respectively. Results: 326 physicians participated in the study – 18,1% G/O specialists and 81,9% GFM specialists. 43,7% specialists are recent graduates (2005-2008). 97,3% GFM specialists and 98,3% G/O specialists admitted that menopause severely affects the quality of life of women. 71,2% GFM specialists and 57,6% G/O specialists felt that HRT should be used only in women with symptoms although they also consider that can be used to prevent others diseases, mainly osteoporosis. 71% GFM specialists and 98,3% G/O specialists prescribe HRT in their daily clinical practice, the former prefer to use the oral form and the second the transdermal form. 100% GFM specialists and 93,3% G/O specialists consider that the quality of life of post-menopausal women improved with HRT. Discussion and conclusion: The study participants demonstrated having appropriate knowledge about symptoms, physiological changes and the impact on quality of life of postmenopausal women as well as of HRT recommendations and risks-benefits. Nevertheless we have identified some discrepancies between the specialties, particularly on what concerns HRT prescription. This may be explained by the limited availability of protocols and standards/guidelines that allow experts to standardize their knowledge, intervention strategies and prescribing patterns of HRT.

Resumo: Devido à baixa sensibilidade do exame parasitológico de fezes (EPF) para o diagnóstico laboratorial da giardíase, o método imunoenzimático (ELISA Enzyme linked immunosorbent assay) para pesquisa de coproantígenos de Giardia tem sido utilizado, mostrando alta sensibilidade e especificidade. Este trabalho foi desenvolvido com o objetivo de se avaliar a eficácia do ELISA ProSpecT Giardia (Alexon, Inc. BioBrás) anti-GSA65 (antígeno fecal específico de Giardia - peso molecular 65.000 Da) como instrumento de controle de cura da giardíase e o seu limiar de positividade. A contagem de cistos por grama de fezes foi feita utilizando a câmara de Neubauer em amostras positivas para G.lamblia de 100 pacientes. Os resultados mostraram eliminação de 3,6 x 106 cistos por grama de fezes em média. Em gráfico esses resultados desenharam uma curva assimétrica positiva, sugerindo a ocorrência de falso-negativos. Avaliou-se também a eficácia do ELISA anti-GSA65 e o método de Faust e Cols (MFc) em detectar mínimas concentrações de cistos de G. lamblia em amostras fecais. Fezes positivas foram diluídas em fezes negativas de modo a se obter amostras com 100, 1.000 e 10.000 cistos por grama de fezes. O ELISA anti-GSA65 mostrou uma positividade significativamente maior que o MFc. Amostras contendo 10.000 cistos por grama de fezes apresentaram 84,7% de positividade pelo ELISA anti-GSA65 e 34,7% pelo MFc. A eficácia do ELISA anti-GSA65, como instrumento de controle de cura após o tratamento pelo metronidazol em 91 pacientes com giardíase, foi avaliada e mostrou que o ELISA anti-GSA65 pode ser usado para monitorar a cura de pacientes utilizando apenas uma amostra fecal, coletada entre o 4º e 7º dias após o tratamento. Abstract: Due to the low sensitivity of the fecal examination (EPF) in detecting cysts of Giardia lamblia, the imunoenzimatic assay (ELISA enzyme linked immunosorbent assay) to detect G.lamblia coproantigens have been used and have showed high sensibility and specificity. With the ain of evaluating the efficacy of ELISA ProSpecT Giardia (Alexon, Inc. BioBrás) anti-GSA65 (Giardia stool specific antigens - molecular weigh 65.000 Da) as monitoring of the cure of giardiasis and its threshold of positivity it was developed this research. The cysts count per grama of faeces was calculated using Neubauer camera in positive fecal samples for Giardia in 100 patients. It was found a concentration of 3.6 X 106 cyst per grama of faeces as average. In a graph the data draw an assymmetrical positive curve, suggesting occurrence of false negative results. It was evaluated the efficacy of ELISA anti-GSA65, and fecal examination by Faust and colls. procedure (MFc), in detecting minimum concentrations of Giardia cysts in fecal samples. To obtain samples with 100, 1.000 and 10.000 cysts per grama, positive samples were diluted in negative samples. ELISA showed a positivity significative higher than MFc. Samples with 10.000 cystis per grama showed 84,7% of positivity by ELISA anti GSA65 and 34,7% by MFc. The efficacy of ELISA anti-GSA65 as cure monitoring after treatment with metronidazole in 91 patients with giardiasis, was evalued and showed that ELISA anti-GSA65 can be used to monitor the cure of patients, using only single sample of each patients, collected between the 4th and 7th days after treatment, whereas with MFc it would be necessary the exam of two or tree samples. It was also evaluated the efficacy of ELISA-anti-GSA65 and MFc in fecal samples of 96 patients. Full textThe objective of this study was to develop a liquid protein hydrolysate from Mechanically Deboned Chicken Meat (MDCM) by enzymatic hydrolysis and to evaluate the addition of different concentrations (10, 20 and 30%) in a reduced-fat mortadella-type sausage. First, it was evaluated the addition of BHT antioxidant (0.01%) and curing salts (0.25%) in the production of protein hydrolysate at different hydrolysis times (5, 15, 30 and 60 minutes) in order to reduce lipid oxidation and brownish color formation. There were evaluated the degree of hydrolysis, pH, proximate composition, lipid oxidation and instrumental color. It was observed that the addition of BHT antioxidant and curing salts did not affect the degree of hydrolysis at 60 minutes. The pH was similar to the MDCM and the proximate composition was not affected by the hydrolysis process or by the addition of BHT antioxidant and curing salts. However, there was a synergistic action between the BHT antioxidant and the curing salts in lipid oxidation prevention of the hydrolysates during the whole period of hydrolysis. Also, there was observed a reduction in the formation of a brownish color mainly by the use of curing salts, evidenced by high levels of values of a* (redness). Subsequently, there was evaluated the addition of different concentrations protein hydrolysate containing BHT antioxidant and curing salts in a reduced-fat mortadella-type sausage. Thus, four treatments were made containing 0, 10, 20 and 30% of MDCM protein hydrolysate. There were evaluated the proximate composition, pH, lipid oxidation, colorimetric parameters, texture profile and microbiological and sensory characteristics during 60 days of storage at 4 ºC. The proximate composition, pH and microbiological characteristics were considered normal for this kind of product. The values of lipid oxidation of products containing MDCM protein hydrolysate increased up to 30th day of storage with subsequent decrease until the end of the storage period (60 days). The products containing MDCM protein hydrolysate had low values of L* (lightness) and a* (redness), higher lipid oxidation and soft textured, evidenced both by the panelists and by instrumental texture profile. The addition of up to 10% protein hydrolysate proved to be viable in the production of reduced-fat mortadella-type sausage, showing quality characteristics closer to the control treatment.
O objetivo deste trabalho foi desenvolver um hidrolisado proteico líquido de carne mecanicamente separada (CMS) de frango, através de hidrólise enzimática e avaliar a adição de diferentes concentrações (10, 20 e 30%) em mortadela com teor de gordura reduzido. Primeiramente, foi avaliada a adição de antioxidante BHT (0,01%) e sal de cura (0,25%) na produção do hidrolisado proteico em diferentes tempos de hidrólise (5, 15, 30 e 60 minutos) com o objetivo de reduzir a oxidação lipídica e a formação da coloração amarronzada. Avaliou-se o grau de hidrólise, pH, composição centesimal, oxidação lipídica e cor instrumental. Foi possível observar que a adição de antioxidante BHT e sal de cura não afetou o grau de hidrólise ao final de 60 minutos. O pH foi similar ao encontrado na CMS original e a composição centesimal não foi afetada pelo processo de hidrólise nem pela adição de antioxidante BHT e sal de cura. Entretanto, houve uma ação sinergística entre o antioxidante BHT e o sal de cura na prevenção da oxidação lipídica dos hidrolisados durante todo o período de hidrólise. Também, foi observada uma redução na formação da coloração amarronzada pelo uso principalmente do sal de cura, evidenciada pelos maiores teores de vermelho (a*). Posteriormente, foi avaliada a adição do hidrolisado proteico com antioxidante BHT e sais de cura, em mortadela com teor de gordura reduzido, contendo 0, 10, 20 e 30% de hidrolisado proteico de CMS. Foram avaliadas a composição centesimal, pH, oxidação lipídica, parâmetros de cor e as características microbiológicas, sensoriais e perfil de textura durante 60 dias em armazenamento a 4 ºC. A composição centesimal, pH e as características microbiológicas foram consideradas normais para o produto em questão. Os valores de oxidação lipídica dos produtos contendo hidrolisado proteico de CMS aumentaram até o 30º dia de armazenamento com posterior decréscimo até o final do período de armazenamento (60 dias). Os produtos contendo hidrolisado proteico de CMS apresentaram valores de luminosidade (L*) e valores de a* menores e textura mais amolecida, evidenciada tanto pelos provadores quanto pelo perfil de textura instrumental. A adição de até 10% do hidrolisado proteico se mostrou viável na fabricação de mortadela com teor de gordura reduzido, pois apresentou características de qualidade mais próximas ao tratamento controle.

Full textIn the present work, the sugarcane bagasse was pretreated with a chemithermomechanically (CTM) process using three concentrations alkaline-sulfite solution (g/100 g of bagasse): 2,5% Na2SO3 e 1,25% NaOH (QTM2,5%); 5% Na2SO3 e 2,5% NaOH (QTM5%), 10% Na2SO3 e 5% NaOH (QTM2,5%), and disk refining with the aim of increasing cell wall accessibility for subsequent extraction of the hemicellulose fraction. Besides, bagasse was delignified with acidic sodium chlorite for 2 hours (D2) and 4 hours (D4), to produce a model with low lignin content. The original composition of sugarcane bagasse was 38.3% of cellulose, 28.5% of hemicellulose and 22.0% of lignin (g/100 g of original bagasse). After pretreatment, the lignin removal were 18%; 31% e 48% for the bagasse CTM2,5%, CTM5% e CTM10%, respectively. A part of the hemicellulose was removed at levels of 22% for the bagasse CTM2,5%, CTM5% and 17% for QTM10%. Acid chlorite pretreatment removed lignin at 55% and 75% levels for D2 and D4, respectively; and 10% of hemicellulose in D4 bagasse. Hemicelluloses from all pre-treated bagasses were extracted with 3 different commercial endoxylanases preparations using 20UI/g of bagasse, at pH 8.0 (phosphate buffer 50 mM), 50°C and solid:liquid ratio of 1:20. After 24h of reaction, Luminase xylanase showed the best xylan extraction yields of: 4%, 7% and 32% for QTM2,5%, QTM5% and QTM10%, respectively; and of: 34% and 55% for D2 and D4, respectively. The xylan extraction correlated linearly with lignin (R2=0.97) and hidroxicinamic acid (R2=0.88 for ferulic acid; R2=0.80 for p-coumaric acid) removal, and with the increase of accessible surface area of the cellulose (R2=0.95). The production of monomeric sugars in the extracted xylan was not observed. Increasing enzimatic loading by 20 times (100 UI) for the bagasse CTM10% and 12 times (60 UI/) for the D4 bagasse, the xylan extraction was increased 29% and 21%, respectively, suggesting that besides lignin and hydroxycinnamic acids, there are other impediments to enzymatic extraction. The QTM10% bagasse was extracted sequentially twice with 100UI of Luminase per gram of bagasse, and then saccharified with commercial cellulases. The results showed that the xylanes most substituted with arabinose are extracted first and that the extraction of the xylan favors the saccharification of the residual cellulose as compared to a simply pretreated material. However, to access all the bagasse xylan, it is necessary to hydrolyze part of the cellulose, because the QTM10% pretreatment was not enough to completely disintegrate the high cellulose-hemicellulose interaction. Pre-incubation of the QTM10% bagasse for 8h with endoglucanase, followed by xylanase reaction for 24h, increased the xylan extraction by 17.6%. Extracted xylans were recovered by precipitation with ethanol and characterized. Chemical and nuclear magnetic resonance analysis showed that extracted xylan corresponds to a glucurono-arabinoxylan with residues of p-coumaric acid and low amounts of lignin and proteins, derived from the extraction process.

The present study aimed to evaluate the effect of replacing fishmeal for hydrolysate fish protein (HFP) and biofloc flour (BF) in diet of marine shrimp Litopenaeus vannamei postlarvae (2mg). The present study aimed to evaluate the effect of replacing fishmeal for hydrolyzed fish protein (HFP) and biofloc flour (BF) in Litopenaeus vannamei postlarvae. The HFP used in the diets formulation were produced from fish industry waste and the biofloc used for BF production was obtained in cultivation tanks of a commercial shrimp farm. Feed for this phase was designed to be isoenergetic and isoproteic. This experiment consisted of six treatments with three replicates each, corresponding to the control treatment T0, using fish meal as the main protein source. The other treatments (T10, T20, T30, T40) were a gradual replacement of fishmeal by HFP and BF in the proportions of 10, 20, 30 and 40% respectively. In the experiment a commercial feed was used as external control. It was a 42 days assay. Shrimp survival was above 99% in all treatments. After statistical analysis to zootechnical parameters (final weight, weight gain, specific growth rate) shrimps fed with formulated diets had better results than the ones fed with commercial feed. Tested parameters had ideal points indicated by regression in levels ranging between 15 and 16% of fishmeal replacement by HFP and associated BF. In the present study we inferred that BF and HFP are potential ingredients to replace fish meal in diets for L. vannamei postlarvae.
A presente dissertação teve como objetivo avaliar o efeito da substituição da farinha de peixe por hidrolisado proteico de peixe (HPP) e farinha de biofloco (FB) na alimentação de pós-larvas (2mg) do marinho camarão Litopenaeus vannamei. O HPP utilizado na elaboração das rações foi produzido a partir de resíduos de uma indústria de pescado. O biofloco para a produção da farinha foi obtido em tanques de cultivo de uma carcinicultura comercial. As rações para esta fase foram elaboradas para serem isoproteicas e isoenergéticas. Este experimento consistiu de seis tratamentos com três repetições cada, correspondendo ao tratamento controle o T0, utilizando a farinha de peixe como principal fonte proteica, e os demais tratamentos com substituição gradual da farinha de peixe por HPP e FB nas proporções de 10, 20, 30 e 40% correspondendo aos tratamentos T10, T20, T30 e T40 respectivamente. No experimento foi utilizado um tratamento com ração comercial que serviu como controle externo. Este ensaio teve duração de 42 dias. A sobrevivência dos camarões cultivados ficou acima de 99% em todos os tratamentos. As respostas estatísticas para os parâmetros de desempenho zootécnico (peso final, ganho de peso, taxa de crescimento específico) indicaram melhores resultados para os camarões alimentados com as rações formuladas quando comparados com os camarões alimentados com ração comercial. Os parâmetros testados tiveram seus pontos ideais indicados pela regressão, em níveis que variaram entre 15 e 16% de substituição da farinha de peixe por HPP e FB associados. Os resultados encontrados neste experimento indicaram o potencial dos ingredientes testados na substituição da farinha de peixe em rações para L. vannamei.

Full textPompe disease, also known as glycogen storage disease type II or acid maltase deficiency, belongs to a group of lysossomal storage disorders, this being the most severe form of the group. It is an autossomal recessive disease caused by mutations in the gene encoding the lysosomal enzyme acid α-glucosidase, leading to lysosomal glygogen storage. Pompe’s disease has a broad spectrum of clinical manifestations, whose initial symptoms can appear at any age. There was no disease-specific treatment for Pompe disease until the approval, in 2006, of the enzyme replacement therapy with recombinant human acid α-glucosidase. This enzyme replacement therapy is effective in modifying the natural course of the disease and gave a new hope to Pompe patients. The present work intends to review the state of the art of Pompe disease in Portugal, with particular emphasis on the late-onset disease. It is discussed the clinical presentation, pathogenesis, diagnosis methods and current treatment recommendations of this disease. In order to achieve this, a review of the latest published and relevant scientific literature is performed. This paper also includes the presentation and discussion of two patients with lateonset Pompe disease, followed at Hospital Pêro da Covilhã (Centro Hospitalar Cova da Beira).

O diagnóstico clínico da amebíase intestinal continua sendo meramente presuntivo; o diagnóstico de certeza depende sempre de confirmação laboratorial. Com o objetivo de correlacionar os achados clínicos com a pesquisa do coproantígeno GIAP de Entamoeba histolytica, por teste imunoenzimático, foram estudados 105 pacientes de ambos os sexos, com idade entre 13 e 18 anos, provenientes de demanda passiva do Serviço Ambulatorial de Clínica Médica da Polícia Militar. A prevalência de amebíase intestinal encontrada por coproscopia foi de 13,33% (14/105) e por ELISA 24,76% (26/105). Houve diferença estatisticamente significativa na prevalência desta protozoose quando os métodos foram comparados (p<0,05-McNemar). Entre os enteroparasitas detectados por métodos coproscópicos de rotina destacaram-se: Endolimax nana com 61,90% (65/105), Blastocystis hominis 28,57% (30/105), Entamoeba coli 18,10% (19/105) e Giardia amblia em 5,71 (06/105). Entre os helmintos, os mais prevalentes foram o Trichiuris.trichiura com 4,76% (5/105) e de Ascaris lumbricoides em 3,81% (4,105). A prevalência dessas parasitoses na população estudada foi compatível com a casuística regional. No estudo da sintomatologia dos pacientes com teste de ELISA positivo, 73,08% (19/26) relataram um ou mais sintomas sugestivos de amebíase intestinal, observando-se cólicas intestinais em 46,15% (12/26), diarréia sem elementos anormais em 42,31% (11/26), tenesmo em 3,85% (1/26) e constipação intestinal em 11,54% (3/26). A presença desses sintomas quando comparada com os casos clinicamente idênticos, porém com teste de ELISA não apresentaram significância estatística (p<0,05-McNemar). Quanto a diarréia mucossanguinolenta, esta foi referida por 4,76% de apresentação das síndromes clínicas e sua amplitude de diagnósticos diferenciais, aliados às possibilidades de equívocos que os métodos diagnósticos usualmente empregados podem fornecer. Recomenda-se a inclusão do teste de ELISA no diagnóstico da amebíase intestinal como um recurso indispensável na clínica, embora ele não dispense o exame coproparasitológico, por ser capaz de indetificar somente um patógeno, a E. histolyca.
Intestinal amoebiasis clinical diagnosis remains presumable and the correct diagnosis always need laboratorial confirmation. A hundred-five patients, of both sexes, age between 13 - 80 years-old, from passive demand of "Serviço Ambulatorial de Clínica Medica da Polícia Militar", were selected for this study in order to correlate the clinical findings and imunoenzimatic E. histolytica GIAP stool antigen search. The prevalence of intestinal amoebiasis was 13,33% (14/105) for single stool examination and 24,76% (26/105) for ELISA. When the two methods were compared, diference statistically significant it was found (p < 0,05 - McNemar). Among detected parasites by single stool examination: Endolimax. nana with 61,90% (65/105), Blastocystis hominis 28,57% (30/105). Entamoeba coli 18,10% (19/105) and Giardia lamblia 5,71% (6/105) were the most prevalent. Trichiuris trichiura with 4.76% (5/105) and A lumbricoides 3,81% (4/105) had the highest prevalence among helminths. The prevalence of such parasitic infections at the studied population was according to local casuistic. In the analysis of the symptomatology of ELISA positive patients, 73,08% (19/26) had reported one or more disease suggestive symptoms, and was observed abdominal pain in 46,15% (12/26) patients, diarrhoea with no abnormal elements in 42,31 % (11/26), tenesmus in 3,85% (1/26) and intestinal constipation 11,54% (3/26). The presence of such symptons when compared with clinical similar cases ELISA negative, were not significative (p > 0,05 - McNermar). In 4,76% patients (5/105), diarrhoea with mucosus and blood was refered. This association with ELISA positive test were observed in 3,84% (1/26), and was statistically significative when compared with ELISA negative ones. The results of this work shows the difficulties in establishing the clinics and laboratory correlation in intestinal amoebiasis. It occurs because of clinical syndroms are not so specifie and the need of different diagnosis, together to the error possibility in routine diagnosis methods. Its is suggested the inclusion of ELISA test in intestinal amoebiasis diagnosis as a necessary clinical resource although, it does not exclude the single stool examination to others parasites, due to be able to identify only the pathogen, Entamoeba histolytica.

Full textThe first part of this thesis deals with the study of the reactivity of electrophilic tellurium compounds, mainly tellurium tetrachloride and aromatic tellurium trichlorides. New aspects of the reactivity of TeCl4 towards alkynes and some acetophenones were disclosed. A mechanistic rationale for each of the processes studied was possible by the determination of the stereochemistry for each product by monocrystal X-ray diffraction analysis. The proposition of the formation of cationic intermediates in the addition reaction of TeCl4 to alkynes was corroborated by the detection and characterization of transient intermediates by ESI-MS/MS experiments. Besides the new aspects of the Tellurium chemistry found, the prepared compounds showed a high and selective activity as inhibitors of cysteine proteases. A pair of enantiomers of a tellurane showed different activities against Human Catepsin B due to a stereochemical dependence in the enzyme/inhibitor interaction. The second part of the present work deals with the development of the ring opening reaction of aziridines by organometallic reagents of copper prepared from vinylic and arylic tellurides. These reactions led to homoallylic and homobenzylic amine derivatives. Finally, the reactivity of 2-alkenyl aziridines was studied towards a series of organometallic reagents of lithium, magnesium, copper and zinc which biased the regio- and stereoselectivities of the ring opening reactions.

Full textHoje em dia sabe-se que a gravidez e o parto são dois momentos de grande stress metabólico para a mulher, pelo que podem contribuir para o agravamento clínico de pacientes com doença de Pompe e doença de Gaucher. Tendo em conta a inexistência de ensaios clínicos aleatorizados que avaliem a segurança do uso de terapêutica de substituição enzimática (TSE) durante a gravidez e a lactação, torna-se relevante analisar a literatura disponível até ao momento, nomeadamente múltiplos relatos de caso de mulheres grávidas sob TSE. Esta revisão aborda o impacto da TSE em mulheres grávidas com doença de Pompe e doença de Gaucher, tendo como objetivos: analisar o perfil de segurança na gravidez; avaliar a sua eficácia na estabilização da doença e, por fim, perceber o impacto da TSE nos diversos outcomes obstétricos e neonatais. Para o efeito, foram analisados 29 artigos científicos utilizando o PubMed.
Nowadays it is known that pregnancy and delivery are two moments of the woman’s life marked by an increase in the cellular’s metabolic stress, which can contribute for the clinical deterioration of Gaucher and Pompe’s disease patients. Taking this into account, so far there are no randomized clinical trials that evaluate the safety of enzyme replacement therapy (ERT) during pregnancy and lactation so, for this paper, it was important to analyze multiple case reports on the matter. This review addresses the impact of ERT on pregnant patients with Gaucher and Pompe’s disease and its ultimate goals are: analyze the safety profile of ERT as the role it plays on the disease’s stabilization and also the impact on obstetric and neonatal outcomes.
