Full textToday's information society, is offering new ways to interact, to establish relationships and maintain contact between people.Technology and therapy is a way to maintain a therapeutic relationship with the help of Skype as an information technology tools. The purpose of the study is to examine psychotherapist’s experiences of the therapeutic relationship in psychodynamic psychotherapy via videocommunication. Research questions focuses on the psychotherapists' use of psychodynamic theories, the description of the therapeutic relationship and how the therapeutic alliance is being built in videocommunications. The study was a qualitative research approach with a hermeneutic method. Six therapists who pursued therapy via Skype were interviewed. The results show that therapists felt that there was a distance in the relationship which could be used positively to investigate deeper into the therapy. The therapies became wordier as transferense and countertrancferences could not be identified as easily as in a physical meeting. The therapeutic alliance felt more fragile on Skype. Discussions from the study shows in order to pursue a psychodynamic psychotherapy on Skype the therapists needed to adapt the theory to the technology. The distance in the relationship was used as a means of therapy to be developed into insight therapy but could also seem to strengthen the defenses.

Full textBackground: The therapeutic alliance is the relationship that arises when the professional and the patient meets during treatment. Previous research, which often is from the professional’s perspective, has shown that the therapeutic alliance promotes a positive outcome for the treatment. This is something that both professionals and patients consider to be a positive basis for the recovery-oriented practice. That is why we have decided to have the patients perspective in this study. The form of self-harm we are writing about in this study is the non-suicidal self-injury. Self-harming is a common thing to do when to avoid experiencing feelings of emptiness, to punish yourself or to deal with tough feelings. The theories that has been used to analyze the collected data is object relation theory and attachment theory. We chose to use these two theories because both of them describe previous relationships that might affect present and future relationships and that might also affect the recovery of every individual. Purpose: The purpose of this study is to explore the importance of therapeutic alliance, when treating young females between 20-29 years with non-suicidal self-injury and other underlying issues. We wanted to find out if the patients with experience of treatment for self-harm considers that the therapeutic alliance had a beneficial effect during treatment. Method: This study is a qualitative study with semi structured interviews, done both in real life and online. We choose to interview four previous patients that have own experience from mental illness and self-harm, that is because we consider their angle of the therapeutic alliance is important to make visible, because their voice rarely has been heard in previous research. Results: The results of this study have been shown in four different themes. These themes are; ”problems related to self-harming”, ”desirable characteristics with professionals”, non-desirable characteristics with professionals” and ”helpful relationships outside of professional care”. The common thing for all of these themes are that the therapeutic alliance is an important basis for the patients recovery, and how the professional chooses to meet and help the patient is of great importance to create a good relationship. A positive outcome for a treatment has its basis in the relationship between the professional and the patient, and the patient needs to trust and feel safe in that relationship. The basis is that the patient feels safe every single time that the patient meets the professional. Conclusion: What we found in this study was that the therapeutic alliance has a big difference and impact when it comes to the patient's experience of the treatment. And that also affects the outcome of the treatment. The people that we interviewed preferred professionals that had characteristics such as being safe, calm, caring and when necessary dared to break some rules. These days the requirement to meet every individuals needs is big and the therapeutic alliance is needed so each patient can feel seen, heard and get the attention it needs - in each individual meeting. Full textIntroduction: Different therapy methods provide different help. ISTDP is distinguished by being more intensive and, as in all methods, also here alliance is important. Therapists are the one who are working with the alliance but how they experienced it, what similarities and differences there are in the different methods they work with, has rarely been investigated. The goal with the study is to illustrate psychodynamic psychotherapists view on alliance in both ISTDP and classic psychodynamic psychotherapy. Questions: How do clinically effective psychodynamic psychotherapists see the therapeutic alliance in both classic psychotherapy and ISTDP? What are the differences and similarities in how they create and maintain it? Method: Six licensed psychodynamic psychotherapists who also are educated in ISTDP have been interviewed. The research is based on grounded theory. Result: Differences that are found are the need for contract and frame and how the method is applied. A gender difference is in the therapist's view of the possibility of engagement, in ascribing signification to the interventions that bring about alliance and in the way alliance, attachment and emotional contact has been used. Similarities are regarding the importance of therapist's engagement, competence and patient's problems. Discussion: The result was discussed on base of the therapist's view upon alliance. Therapist's engagement and patient's problems as well as how the method has an impact on creating the alliance has been discussed. Therapist's experience of being able to create and maintain the alliance has been discussed from a gender perspective and legibility.

Full textThe aim of the study was to investigate psychotherapists perceptions about and experiences of identifying ruptures in the therapeutic alliance in cognitive behavioural therapy (CBT). The data collection consisted of narrative interviews. The qualitative method, a phenomenological hermeneutical method for researching lived experience (Lindseth & Norberg, 2004), was used in the analysis and interpretation of the texts. The result showed that the therapeutic skills awareness and metacommunication was used to identify, prevent and resolve ruptures in the alliance. The conclusion is that psychotherapists stories carries a hidden meaning of any therapeutic interventions in CBT at the same time constitutes relational actions, and a language for the same is desirable. Meaningfulness and professional pride can also be adopted, by shifting attention from failures, into courage to take responsibility for the therapeutic process and the relationship.

Full textThe aim of this study was to describe the function of a working alliance in the social work of family therapists in order to make contribute to the knowledge discourse of the importance of working alliance in social work. The questions were what of importance the work alliance is in the task of family therapists and what the important factors are in how to create, sustain and end a working alliance in a good way within the social work with families. Interviews were pursued with six family therapists. The respondents described the function of the working alliance from their perspective. The result was analysed with social construction theory, system theory, attachment theory and a multidimensional perspective. The central result of the study was that the therapists experienced that the working alliance was of great importance in their work which is in agreement with psychotherapy research which emphasise the importance of a working alliance in order to reach a good result of the therapy.

Full textThe main purpose of this report was to bring clarity to the definition about the concept of the therapeutic relationship, as well as providing a description of what the therapist needs to consider, when it comes to creating the “good therapeutic relationship”.This has been done on the basis of qualitative in-depth interviews with a total of seven therapists, who in their descriptions, together came to form an understanding of these two different aspects of the concept. The collected material from these interviews was then thematized, in the basis of a hermeneutic interpretation method, in after witch these results were interpreted from a social constructivist, and a learning-theory perspective.The main results show that a definition of the therapeutic relationship was difficult to distinguish from the descriptions of how therapists work to create a good therapeutic relationship, as well as the personal characteristics of “the good therapist”.Keywords: Common factors, Four stages of competence, Social constructivism, The therapeutic alliance, The therapeutic relation, Therapy

Full textDuring the pandemic COVID-19 the health care has suddenly been forced to adjust their work and start using video conference to keep the continuity, which also impacts psychotherapy. Despite extensive evidence of the efficiency of video conference psychotherapy there are still myths among clinicians that the therapeutic alliance can be endangered by the digital format, which can complicate the implementations. The aim of this study is to examine the psychotherapists' experiences of psychotherapy via video conference, with particular focus on the therapeutic alliance (TA). Through semi-structured interviews we examined the psychotherapists conceptions, adaptations and the experiences of this readjustment. Data were analyzed based on qualitative content analysis; Communication, The solid relation and the CBT-interventions. The main results indicated that the clinicians needed to adapt the treatment contextually, communicatively and in terms of content to frame TA, and that TA can be developed via video conference. This coincides with previous international research. Areas of development is the new phenomenon self-exposure and transformation/change of equalization of power. The conversion has positively exceeded the expectations and has partly changed the respondents attitudes to therapy via video conference. Within the limitations of this study, implications and further directions for research are described.

Full textDen här studien undersökte hur fyra missbruksbehandlare som haft ett eget missbruk konstruerar sin yrkesidentitet utifrån föreställningar om kompetens, makt, normalitet och avvikelse. Studien utgick från kvalitativa intervjuer med fyra sådana behandlare kring deras yrke i relation till de egna missbruks-erfarenheterna. Intervjuerna analyserades utifrån en symboliskt interaktionistisk och socialkonstruktivistisk referensram med hjälp av några vanliga begrepp inom diskursanalys. Resultatet visade att alla fyra upplevt att de behövde gå i en sorts karantän från behandlings-världen innan de kunde fullborda sin identitetsresa från klient till behandlare. En viktig aspekt av denna karantän var att erövra en professionell legitimitet och äkthet som behandlare. De fyra behandlarna tycktes konstruera sin yrkesidentitet i ett maktspänningsfält mellan å ena sidan upplevda skillnader mot klienter och å andra sidan upplevda likheter med dem. I detta spänningsfält använde de flera olika strategier för att både befästa och göra motstånd mot den egna behandlarmakten.

Sammanfattning
I Sverige utreds årligen uppskattningsvis 100 000 barn av socialtjänstens individ- och famil-jeomsorg och antalet strukturerade öppenvårdprogram har i det närmaste fördubblats sedan 1999. Många av dessa insatser har kommit att ersätta den traditionella institutionsvården. I och med ökningen av att antalet unga som numera hamnar i strukturerade öppenvårdsprogram behövs mer kunskap om såväl innehåll som resultat. Denna studies syfte är att undersöka och belysa en öppenvårdsverksamhets, i en mellansvensk kommun, innehåll d.v.s. vilka föränd-ringar som familjerna kan beskriva har skett samt om positiva förändringar kvarstår efter be-handling. Studien är av kvalitativ karaktär och resultaten bygger på intervjuer med familjer som deltagit i den aktuella behandlingen. Studiens tolkningsram belyser förutsättningar vilka ligger till grund för framgångsrik behandling. Viktiga inslag såsom terapeutisk allians och generella behandlingsprinciper vilka tar upp interventioner riktade mot avgörande risk- och skyddsfaktorer belyses. Resultatet visar att öppenvårdsverksamhetens behandlingsfaktorer intensitet och tillgänglighet har varit verksamma för att uppnå förändringar. Flertalet positiva förändringar har bestått efter behandlingens avslut. Resultatet antyder på att verksamheten skulle vinna på att bättre informera om behandlingens innehåll innan familjer tar ställning till att delta i behandling eller ej. Resultatet visar att arbetsplaner och målbeskrivningar skulle kunna göras tydligare för att bättre nå behandlingsresultat. Studiens resultat indikerar också betydelsen av att interventioner inom skolsituationen har betydelse för positiva förändringar. Ytterligare visar resultatet att flertalet familjer beskriver en mycket god terapeutisk allians och dess betydelse för uppnådd förändring.

Abstract
In Sweden approximately 100,000 cases involving children are investigated annually by the social services office for individual and family services. The number of registered cases in the open care service programs has almost doubled since 1999, replacing traditional institutional care. As a result of the increase in the number of children, more knowledge is needed con-cerning the underlying issues and results from the open care service program. The purpose of this study is to investigate and clarify the open care service program of a medium-sized Swe-dish county, to explain changes that have take place as described by the families themselves and which positive changes remain after treatment. The study’s analysis sheds light on which premises should exist for positive treatment. Important elements such as therapeutic care al-liance and general treatment programs and which interventions directed towards conclusive risk and protective factors are addressed. Results show that factors such as intensity and avail-ability of open care service programs treatment are instrumental in reaching changes and that the majority of changes have persisted after concluded treatment. Results suggest that open care service programs could benefit by better informing about treatment’s before families decide on participating in treatment or not. Also results indicate that action plans and descrip-tion of objectives could be made more explicit to better attain therapeutic results. Results also show that the majority of families describe a favorable therapeutic alliance and their signific-ance to viable change. Results also corroborate the importance of intervention in the school environment on attaining positive effects.

Full textThe purpose of the study was to investigate therapists' experience of therapeutic alliance in therapy through digital format. A qualitative method has been applied and data collection has been done through semi-structured interviews. The study involved seven participants, five of whom were women and two were men. The study's collected material has been analyzed based on Braun and Clarke's (2006) Thematic analysis. The Thematic Analysis generated three themes: The first theme: The uncertainty in what happens, and what one does not see or hear, which describes the participants' experience of obstacles in communication through the digital meeting. Second theme: Limited room for maneuver, describes the participants' feeling that the digital format reduces their toolbox as a therapist. The last theme: Lack of physical proximity, describes how the participants lack the physical proximity in the digital format but experience the digital format as a good complement to the physical space, however, no adequate replacement.

Full textDepression is a debilitating disorder that affects a large part of the adult population every year. Yet there is still a lack of access to effective care for people in need. Cognitive Behaviour therapy (CBT) is an evidence-based method for treating depression that together with the increased availability of Internet services provides an opportunity to increase access to effective treatment. Internet-based interventions can be effective in the treatment of depression, but there is a lack of knowledge concerning which formats of delivery that can be used and if therapeutic alliance is of equal importance when providing treatment over the Internet. The overall aim of this thesis was to examine the effects of different treatment formats (email therapy, guided self-help, and blended treatment) in internet-based CBT for depression and to further examine the role of alliance in these treatment modalities. Findings from this thesis show that email therapy and internetbased treatment programs were effective methods for treating depression. Alliance ratings were high, showing that a positive therapeutic alliance can be achieved in internet-based treatments. Patient-rated alliance could not predict outcome in any of the different treatment formats. However, therapist-rated alliance predicted change in depression during blended treatment. This thesis includes the first randomized controlled study on CBTbased email therapy, and the first internet-based behavioral activation program with ACT-components, for adult depression.

Full textBackground: Patients with persistent spinal pain often experience a loss of function, without a clear association with tissue damage. Loss of function may partly be healthcare caused, after patients being provided counterproductive advice by their caregiver. Purpose: To investigate the differences in attitudes among first term and last term physical therapy students at Uppsala University, in regards to persistent spinal pain and loss of function and to investigate if student’s own experience of persistent pain influences these attitudes. Method: The students answered the Health Care Providers Pain and Impairment Relations Scale (HC-PAIRS) Questionnaire. HC-PAIRS measures to what extent health care providers link loss of function to persistent spinal pain. Results: There was a significant difference between the semesters. The students in semester one having a higher overall score compared to the students in semester six (55,5 compared to 46). There were significant differences regarding “functional expectations”, “social expectations” and “need for cure”, but not in “projected cognition”. The students’ own experiences of persistent pain did not affect their scoring. Conclusion: Students in semester six had a more positive outlook on the functional ability among patients with persistent spinal pain than students in semester one. However, there was a vast spread of results within both semesters.

Full textThe purpose of this pilot study was to explore clients undertaking equine assisted treatment for mental illness (n = 13) view of the horses attachment and working alliance facilitating abilities. Further, the study aimed to explore to which extent the respondents viewed themselves as being authoritative in relation to the horse. For comparison a group of recreational equestrians (n = 23) were used. The respondents in the client group were recruited from the association Organisationen för Hästunderstödda Insatser (OHI) and the recreational equestrians were recruited from two riding schools. The participants undertook a survey with questions regarding attachment facilitating qualities in their relationship with the horse, and which relationship facilitating characteristics they experienced in the horse. The results indicated that the respondents in both groups over all viewed the horses’ characteristics and their relationship with the horse as positive. The results indicated that the client group to a higher extent regarded the horse as having characteristics and relational qualities that could be viewed as alliance and attachment facilitating. The recreational equestrians reported an authoritative approach toward the horse to a larger degree than the client group. Although there were limitations regarding the size of the sample, and coherence in the results, the results indicated that there are grounds for future research of the subject and research question at hand, with the use of a larger population and a revised survey, as the pilot study did find significant differences between the groups.

Frågeställningar: Hur påverkas familjebehandlarna i sitt arbete med att skapa en terapeutisk allians när familjen är under/eller hot om LVU? Finns speciellt viktiga relationella inslag i familjebehandlingen, som bidrar till förändring då det föreligger LVU eller hot om LVU? Metod: Kvalitativ intervju-undersökning och tematisk analys. Resultat: Resultatet av studien visar att familjebehandlarna för att nå en terapeutisk allians. upplever det som av stor vikt att de visar sig vara en ”vanlig människa”, att de får ges möjligheten att samgå med familjen samt att de lyfter styrkor snarare än svagheter. Resultatet pekar på att familjebehandlarna som tillfrågats upplever det som försvårande när det kommer till skapande av allians, vid de tillfällen då samarbetet med socialtjänsten är komplicerat. De upplever det lättare att skapa allians då familjen själva efterfrågar behandling, än när insatsen är tvingande. De menar att förändring är möjlig, om än svårare, då tvång föreligger. Diskussion: Studiens resultat diskuteras, delvis ur ett intersubjektivt och systemiskt perspektiv.

Full textIntroduction: Psychodynamic couple therapy is about the interaction in between three people. Meetings which aim to change the interaction and find new understanding of each other’s life experiences. The purpose of this study was to analyse psychotherapists’ understanding and experience of changes in couple therapy. Topics of discussion: Which therapeutic factors are relevant for couple therapy? Which therapeutic factors affect the interaction of the couple? What benefits or difficulties is a therapist posed to? Method: Five therapists were interviewed with respect to both qualitative and thematic methods. Conclusion: The intersubjective interaction between therapist and couples has relevance for implementing changes in psychodynamic couple therapy. Personal attitude and receptive ability to perceive communicative alliance created through a therapeutic environment in which acceptance of inequalities and difference of opinions was developed. Discussion: The therapeutic framework, and the informants’ simultaneous ability to establish and monitor the alliance, were factors of particular significance for the three parallel ongoing relationships of the couple therapy. The informants’ simultaneous ability to perceive and investigate communicative alliance was a factor that positively affected the therapeutic relationship. The therapists’ application of such interactions increased the couple's engagement in the therapy. By being personal and making use of primarily soft emotions, the couple became more open and were able to reach deeper emotional experiences. These intersubjective meetings led to a changed relationship with more secure emotional ties and an increased psychological presence.

Full textSyftet med studien har varit att undersöka hur hälso- och sjukvårdskuratorer inom akutsjukvården upplever den terapeutiska relationen i deras stödjande arbete på distans. För att få veta mer har vi intervjuat sju sjukhuskuratorer inom akutsjukvården och frågat dem om möjligheter och utmaningar med att arbeta på distans kopplat till den terapeutiska relationen. Vi ville veta om det gick att uppnå samma nivå av terapeutisk relation när det stödjande arbetet på sjukhusen styrdes om till att i större utsträckning än tidigare ges på distans. Materialet, vår empiri, har analyserats utifrån en hermeneutisk tolkningstradition och resultatet har analyserats med hjälp av Asplunds teori om social responsivitet. Studiens resultat visar på två olika svar. Det ena resultatet visar att kuratorer upplever att det är relationsstärkande när patienten själv får välja var och hur man vill ha sitt samtal med kuratorn vid samtal på distans. Det andra resultatet visar att det finns något i kommunikationen, och därmed också i den terapeutiska relationen som går förlorat när samtalet inte sker fysiskt och kuratorn och patienten inte kan ta del av varandra med både verbalt språk och kroppsspråk. Kuratorerna har berättat om svårigheter att bekräfta patienten när man inte ser minspel och kroppsspråk eller när ljudet fungerar dåligt och man kan behöva be patienten upprepa något mitt uppe i en känslosam berättelse. Kuratorerna har också poängterat att de sannolikt kunnat fånga upp fler patienter än vad de annars hade gjort genom att kunna erbjuda samtal på distans och därigenom uppnått en terapeutisk relation, även om kontakten skedde på distans. Människors olika behov har blivit tydligt genom studien och genomsyrar våra resultat som visar på kuratorernas upplevelse av att det har varit relationsstärkande att kunna erbjuda olika samtalsmöjligheter.

Full textBackground: Anorexia nervosa classifies as an eating disorder that involves a intentional weight loss that is caused by changed body image. In the caring of anorexia nervosa, it is essential that the nurse pay focus to the individual behind the eating disorder, as well as being supporting. The nurse plays a major role in the nursing care relationship with the patient. Purpose: The aim of this study was to describe the nurse and patient with anorexia nervosa perspective on a therapeutic alliance. Method: General literature review. Data were analysed on qualitative steps and quality-tested based on five analysis steps. After quality review, nine articles used for the result. Result: The results that emerged from the study were that a positive therapeutic alliance between the nurse and patient can be decisive for recovery. The categories that emerged in the result were: nursing care, support and motivation, the nurse’s participation and negative care relationship. Conclusion: The therapeutic alliance was essential for the patient’s wellbeing and feeling of safety during treatment. If the nurse work from a therapeutic alliance and support the patient during treatment, the patient gains motivation to undergo treatment.

Full textEn el sentido de contribuir al desarrollo de la investigación en el área del proceso y el resultado terapéutico al nivel nacional, este estudio observó características del terapeuta y del cliente, así como varios aspectos del proceso y el resultado terapéutico: i) el estilo de terapeuta y el estilo de coping, grado de resistencia y motivación del cliente para la terapia; al nivel del proceso, ii) la alianza terapéutica y, al nivel del resultado terapéutico, iii) evaluó el bienestar general y la condición de sintomatología en el cliente. La muestra es compuesta por 39 díadas de terapeutas y clientes evaluadas cuanto a estas dimensiones en cuatro momentos distintos: en la 1ª, la 3ª, la 5ª y la 8ª sesiones. Los hallazgos se presentan a partir de tres estudios. El primer estudio procuró delinear la ruta estadística de los distintos instrumentos utilizados ("Determinar el confort estadístico"). El segundo estudio observó de forma más detallada "El encuentro inicial de las díadas" y procuró conocer factores iniciales que pudieran explicar las desistencias precoces de los clientes. Por último, ha sido posible elaborar un estudio de caso sobre un único terapeuta que reunía a 19 clientes, estudio ese que se intituló "El terapeuta campeón", que permitió conocer más a fondo el estilo y los efectos terapéuticos de un único terapeuta. Los resultados sugieren consideraciones interesantes sobre el estilo del terapeuta y el estilo de los clientes. Ayudar a los terapeutas en formación a conocer su estilo y a entender sus compatibilidades y tasas de mayor éxito con determinados estilos de clientes, podrá ayudarles a mejorar su desempeño con un mayor número de clientes. Conocer, Flexibilizar y Ajustar (C.F.A.) su estilo a cada cliente deberá considerarse un principio orientador de la psicoterapia. Del conocimiento de este grupo de clientes, se pudo también inferir que el género femenino y un perfil exteriorizador de cliente parecen adherir más al proceso terapéutico que el género masculino y el perfil interiorizador. Identificar los niveles de auto-determinación para la terapia en el cliente podrá aumentar el éxito terapéutico y la alianza terapéutica demostró ser un buen indicador de la evolución del proceso terapéutico. En el ámbito de los resultados terapéuticos, se constata que ante una muestra clínica sin patología grave, tan solo se puede utilizar el CORE-OM, que reveló indicadores del resultado terapéutico semejantes a los del SCL90R. Se concluye que la evaluación del resultado terapéutico debe implementarse como práctica regular en psicoterapia y que las 3ª y 5ª sesiones parecen ser puntos de inflexión para los clientes, y podrán considerarse como momentos terapéuticos de redireccionamiento de la propia intervención.
In order to increase the development of the process and the therapeutic result at a national level, this study observed several characteristics of the therapist and the client, as well as some sources of the process and the therapeutic result: i) the style of therapist and the style of client, degree of resistance and motivation of the client for therapy; at the level of the therapeutic process observed the ii) therapeutic alliance and, at the level of the therapeutic result, iii) the assessment of the general well-being and the condition of sintomatology of the client. The sample is composed by 39 dyads of therapists and clients evaluated in these dimensions at four moments of the process, on the 1st, 3rd, 5th and on the 8th session. The results are presented through three studies. The first study delineates the statistical way of the different instruments used ("Determine the statistical comfort"). The second study observed "the initial meeting of dyads" and looked for initial factors that could explain the earlier drop outs of the clients. Finally, it was possible to elaborate a case study of only one therapist who congregated 19 clients, entitled "the champion therapist", which allowed to know better the style and the therapeutic effects of a single therapist. The results suggest interesting considerations concerning the style of therapist and the style of the client. Moreover, to help the therapist in early stages of formation to recognize his style, to perceive his compatibility and levels of higher success with certain style of clients can improve his performance with a number of clients. Identify, be Flexible and Adjust (I.F.A.) each style to each client will have to be a guiding principle of psychotherapy. Of the knowledge of this group of clients, it equally results to know that the feminine gender and externalised profile of clients seemed to remain more in the therapeutic process than the masculine and the internalized profile. Also, identify the levels of self-determination for therapy in the client could increase the therapeutic success and a strong therapeutic alliance is a good predictor of a high-quality outcome in therapy. In the scope of the therapeutic results, evidence that emerges is that in face of a clinical sample without severe pathology, the CORE-OM could be used as a single assessment instrument, since that it disclosed similar pointers of the therapeutic result as the SCL90R. It is concluded that the evaluation of the therapeutic result must be implemented as regular practice in the psychotherapy and that the 3rd and 5th sessions seemed to be moments of turn over for the clients, and should be considered therapeutic moments of redefinition for the therapist.
No sentido de contribuir para o desenvolvimento da investigação na área do processo e do resultado terapêutico a nível nacional, este estudo observou características do terapeuta e do cliente, bem como várias vertentes do processo e do resultado terapêutico: i) o estilo de terapeuta e o estilo de coping, grau de resistência e motivação do cliente para a terapia; a nível do processo, ii) a aliança terapêutica e, a nível do resultado terapêutico, iii) avaliou o bem-estar geral e a condição de sintomatologia no cliente. A amostra é composta por 39 díades de terapeutas e clientes avaliadas nestas dimensões em quatro momentos de avaliação, na 1ª, na 3ª, na 5ª e na 8ª sessão. Os resultados são apresentados a partir de três estudos. O primeiro estudo procurou delinear o caminho estatístico dos diferentes instrumentos utilizados ("Determinar o conforto estatístico"). O segundo estudo observou de forma mais minuciosa "O encontro inicial das díades" e procurou conhecer factores iniciais que pudessem explicar as desistências precoces dos clientes. Por último, foi possível elaborar um estudo de caso, sobre um único terapeuta que reunia 19 clientes, estudo este intitulado de "O terapeuta campeão", que permitiu conhecer mais a fundo o estilo e os efeitos terapêuticos de um único terapeuta. Os resultados sugerem considerações interessantes acerca do estilo de terapeuta e do estilo de clientes. Ajudar os terapeutas em formação a conhecerem o seu estilo, a perceberem as suas compatibilidades e taxas de maior sucesso com determinados estilos de clientes, poderá melhorar a sua performance com um maior número de clientes. Conhecer, Flexibilizar e Ajustar (C.F.A.) o seu estilo a cada cliente deverá ser um princípio orientador da psicoterapia. Do conhecimento deste grupo de clientes, resulta igualmente saber que o género feminino e um perfil externalizador de cliente parecem aderir mais ao processo terapêutico do que o masculino e o internalizado. Identificar os níveis de auto-determinação para a terapia no cliente poderá aumentar o sucesso terapêutico e a aliança terapêutica revelou ser um bom indicador da evolução do processo terapêutico. No âmbito dos resultados terapêuticos, constata-se que perante uma amostra clínica sem patologia severa, poder-se-á apenas utilizar o CORE-OM, que revelou indicadores do resultado terapêutico semelhantes aos do SCL90R. Conclui-se que a avaliação do resultado terapêutico deve ser implementada como uma prática regular na psicoterapia e que a 3ª e a 5ª sessão parecem ser momentos de viragem para os clientes, poderão ser considerados momentos terapêuticos de redireccionamento da própria intervenção.

Full textThe present dissertation refers to the theoretical background known in the literature as Empirically Supported Therapy Relationships, a research tradition focused on the relationship and its nature, beyond the outcome and process research opposition. The three studies included in this work focused on different portions of treatment: respectively the initial intake session, the first two sessions, and the overall treatment. In the first study different outcomes were compared in order to examine if the effects of interpretations were connected to specific factors of psychotherapeutic process. The second study aimed to assess clinical cases of premature termination, differentiated on the basis of the contract attainment: the purpose was to depict specificities due to the different phase of dropping out. Finally, the third study was a single-case research examining alliance ruptures within an entire clinical process of forty sessions.

Full textA intervenção terapêutica grupal com adolescentes é ainda insuficientemente estudada, verificando-se uma maior atenção às terapias individuais e familiares. Os objetivos do presente estudo são avaliar a aliança terapêutica, e descrever e explorar as intervenções de terapeutas ao longo de onze sessões grupais com adolescentes. A presente investigação é qualitativa exploratória. A amostra é constituída por três terapeutas com uma média de idades de 42 anos. Estes, conduziram 11 sessões grupais nas quais participaram 10 adolescentes com uma média etária de 13 anos. Os terapeutas e os adolescentes preencheram um questionário sociodemográfico e o instrumento Inventário de Aliança Terapêutica-R (IAT-R) no final de cada uma das onze sessões grupais. Estas sessões foram registadas em formato áudio e vídeo por uma câmara fotográfica digital e tiveram como base um programa de intervenção terapêutico em grupo. A aliança terapêutica dos terapeutas e dos jovens foi média, apesar da dos jovens ter sido ligeiramente superior. Therapeutic group intervention with adolescents is still insufficiently studied, once it has been given more attention to individual and family therapies. The goals of the present study are evaluate the therapeutic alliance, and describe and explore therapists’ interventions during eleven sessions with a group of adolescents. The present investigation is qualitative exploratory. The sample is constituted by three therapists with an average age of 42 years old. These therapists led eleven group sessions in which participated ten adolescent with an average age of 13 years old. Therapists and participants answered one socio-demographic questionnaire and the instrument Inventário de Aliança Terapêutica-R (IAT-R) at the end of all sessions. Sessions were recorded with a digital camera and were based on a therapeutic group program. The therapists’ and adolescents’ therapeutic alliance was medium, although youth alliance was slightly higher. Therapists’ most frequent interventions were instigating individual participation of adolescents, listening actively to their communications, exploring their personal contents, and therapists themselves exposing personal ideas and opinions.

Full textRecent studies point out the need to explore the psychotherapeutic process from the clients and the therapists’ perspective. Although the literature has recognized the importance of investigating the mechanisms of change in the psychotherapeutic process, there is a significant gap in research regarding the association between therapists’ self-efficacy and therapeutic alliance. In accordance, the main objective of the present study was to analyze in which way the therapists’ self-efficacy relates to the clients’ perception of the therapeutic alliance. The current study included 128 participants, 32 therapists and 96 clients, in a proportion of three clients to each therapist. Therapist’s self-efficacy was measured by the Counselor Activity Self-Efficacy Scales (CASES-G) and the therapeutic alliance perceived by the clients was assessed by the Working Alliance Inventory (WAI-SR, short revised). Results revealed that therapists perceived themselves as efficient as related to the psychotherapeutic process and that clients reported a strong therapeutic alliance. Correlation analysis indicated that the subscale “session management” of the CASESG was negatively linked to the subscale “tasks” of the WAI-SR. These findings suggest that the therapists who are more confident in their abilities to perform specific technical tasks regarding session management may have more difficulty in investing in the client and in the psychotherapeutic process. This may lead to lower synchronicity between the client and the therapist and be an indicator of loss of the therapeutic alliance quality.

BEd (Spesialisering in Skoolvoorligting)studente het 'n bepaalde behoefte aan 'n gestruktureerde en sistematiese benadering tot die opleiding van terapeutiese onderhoudvoeringsvaardighede uitgespreek. 'n Literatuurstudie oor die fenomeen terapeutiese ondemoud het eerstens aan die lig gebring dat vaardighede onderliggend aan die terapeutiese onderhoud geidentifiseer en operasioneel gedefinieer kan word en tweedens dat hierdie vaardighede sistematies aangeleer kan word. 'n Repertoire van onderhoudvoeringsvaardighede is egter nie 'n waarborg dat terapie 'n positiewe uitkoms sal he nie. In die ontwerp van 'n model vir opleiding in terapeutiese onderhoudvoeringsvaardighede moet aandag aan beide inhoud en opleidingsprosedure geskenk word. In hierdie model word nie net die vaardighede vir terapeutiese onderhoudvoering aangespreek nie, maar ook vaardighede om empatie, warmte, respek, egtheid en onvoorwaardelike positiewe agting aan die klient te kommunikeer. Vir die doeleindes van hierdie studie is die terapeutiese onderhoud in ses fases verdeel. Vaardighede wat in elke fase gebruik word, is gerdentifiseer, gedefinieer en bespreek. 'n Loodsondersoek is ondemeem om die effektiwiteit van die opleidingsmodel te evalueer. Proefpersone is aan die hand van die voorgestelde opleidingsprosedure opgelei. Ten einde die proefpersone se vordering en so ook die effektiwitieit van die model te evalueer, is elke proefpersoon se voor- en naopleiding onderhoude vergelyk. 'n Evalueringsskaal is vir hierdie doel ontwerp. Die resultate van die loodsondersoek het getoon dat die proefpersone se onderhoudvoeringsvaardighede verbeter het. Die hoop word uitgespreek dat hierdie studie die weg sal baan vir vernuwing in die opleiding van voorligters en terapeute in terapeutiese onderhoudvoering.
