Full textThe low power lasers (TLBP) is a therapeutic modality widely studied in scientific field, its application in clinical medicine still generates many conflicts since literature reports proliferation in cancer cells. The objective of this study was to evaluate the effects of TLBP on cell growth using as model the line B16F10 in state of homeostasis and redox state and investigate the chemotactic behavior of B16F10 lineage through transwell migration assay in response to TLBP in different energy densities. For this purpose five experimental groups were assembled using a laser emission at λ = 660 nm: control group (G0) where no irradiation was performed; G30 (30J/cm2), G60 (60J/cm2), G90 (90J/cm2); G120 (120J/cm2); G150 (150J/cm2) with the respective doses used. All experiments were performed in triplicate and the results were statistically analyzed. Under the experimental conditions of this study, our results show that TLBP did not induced changes in cellular metabolism that influence proliferation at 48 h and 72 h, independent nutritional status. It was possible to observe changes in behavior pattern chemotactic of cell line B16F10 with TLBP at red emission.

Full textParacoccidioides brasiliensis (Pb) is the etiologic agent of Paracoccidioidomycosis (PCM), an endemic systemic mycosis most prevalent in Latin America and Brazil. Through the study of experimental model, it was possible to make a comparison and classify different strains of inbred mice infected with Pb as resistant or susceptible and the understanding the mechanisms of immune response involved in the disease. Once infected, the host immune system has several mechanisms both the innate to adaptive immunity to contain the infection. Phagocytes, such as macrophages and polymorphonuclear neutrophils (PMN) play a central role in the immune response against the fungus. Besides being the link between innate and acquired immunity, the PMN are effective in host defense against Pb because are restricted to the site of injury caused by fungus. In this perspective, therapy using Low Level Light Therapy (LLLT) with infrared laser in vivo emerges as a method to stimulate the activity stimulated by immune cells such as PMN, without changing its function, to be described as a method of modulating immune cells. LLLt was applied ate two points of paw hind mouse, a power of 50 mW and a wavelength of 780nm, mice infected with Pb18 or inoculated with Zymosan by method air pouch. On irradiated and non-irradiated animals, we assessed the effect of LLLT in vivo production of total protein production capacity of reactive oxygen species (ERO) and freshly isolated PMN fungicidal capacity of the site of infection. The results showed that the active LLLT PMNs, for groups of irradiated mice regardless of the stimulus for inflammation, there was an increase in the production of total proteins, the ability fungicide and ROS production compared with the control group. Although the laser has decreased the total number of PMNs collected from mice infected with Pb18, the viability of the cells in all experimental groups remained highly viable. The results showed that the active LLLT PMNs, resulting in a greater capacity fungicide, suggesting that the laser can be used as adjunctive therapy in the treatment of PCM.

Full textOral mucositis is one of the most common and debilitating adverse effects in patients undergoing bone marrow transplantation (BMT). The low-power laser (LBP) has been established as an important tool in the prevention of mucositis, during conditioning with high-dose chemotherapy in BMT. However, protocols that support such intervention vary and risk factors for mucositis in patients with different types of neoplasms and conditioning protocols are still unknown. This study aimed to assess the prevalence of mucositis, its relationship with clinical parameters and predictive factors in patients undergoing BMT and receiving prevention with LBP. A retrospective analysis was performed concerning 374 patients who were submitted consecutively to BMT at the AC Camargo Cancer Center, from January 2006 to January 2013. All patients received prophylaxis for oral mucositis with LBP using only protocol from the first day of conditioning until D + 2 (2 days after the infusion of the bone marrow). Patients continued to receive the LBP in the case of oral mucositis grade >= 2 until the complete remission of the lesions. Clinical data, such as febrile neutropenia, mouth pain, parenteral nutrition usage and the use of morphine were collected daily through the medical records of patients from the first day of admission until discharge. The clinical variables such as age, weight and renal function data were collected on the day of admission for conditioning. Of the 374 patients selected for this study, 37 (9%) were excluded, totaling 337 patients. Of these, 43 (12.76%) had no mucositis, 166 (49.25%) expressed mucositis grade 1, 84 (24.93%) grade 2, 32 (9.50%) grade 3 and 12 (3.56 %) grade 4. Patients with mucositis grade <= 2, showed an average of 1.4 days of pain in the mouth compared with 9.2 days in patients with mucositis >= 3 (p <0.0001). Regarding the length of hospital stay in patients with mucositis grade <= 2, the average was 27.16 days compared to 36.07 in with mucositis grade >= 3 (p <0.0001). The logistic regression model showed that each increase of one unit of creatinine drives the chance of mucositis grade >= 3 4.3 times higher (OR = 4.37; 95% CI: 1.68 to 11.32 ; p = 0.0024). In addition, patients undergoing allogeneic transplantation showed about 5.97 times more likely to have grade >= 3 mucositis compared with patients undergoing autologous transplantation (OR = 5.97; 95% CI: 3.02 to 11.99 p <0.001). The study found that the incidence and intensity of severe oral mucositis was low in patients undergoing BMT, probably due to prophylaxis with LBP. In addition, the greater the severity of oral mucositis, the longer the pain in mouth, parenteral feeding use, morphine and time of hospitalization. Allogeneic transplantation and increase in serum creatinine level were considered risk factors for the occurrence of severe oral mucositis. Further studies are needed to define specific protocols for LBP in these patients at higher risk for developing severe mucositis.

Currently, chronic obstructive pulmonary disease (COPD) has an impact on morbidity and mortality throughout the world with high prevalence and a high economic and social cost. It is characterized by slow and progressive airflow limitation, triggered by the inhalation of harmful particles present in cigarette smoke. In this context, several experimental models have been proposed, aiming at the discovery of new therapeutic options. In this regard, low-power laser therapy (LBP) is a relatively new and effective therapy for very low cost, no side effects and possible use in the treatment of chronic pulmonary diseases. In the present study we study some parameters in animals with COPD undergoing therapy diode laser (660 nm), 30 mW, 60 s per point (3 points per application) for 15 days prior to euthanasia. The protocol used for COPD induction consisted of nebulized female C57BL / 6 with cigarette smoke for 75 days (2 times / day). On the day 76, the animals were anesthetized, tracheotomy, canulated and bronchoalveolar lavage (BAL) was collected. Structural and functional parameters were studied, the total and differential counts of cells recovered from BAL fluid, cytokine levels (IL-1 , IL-6, IL-10, IL-17, TNF- and CXCL1/KC) as well as the accumulation of mucus and collagen. It also performed lung histology to quantify the number of lymphocytes in the airway, and bronchial remodeling. Moreover, we will evaluate the expression of the purinergic receptors P2Y2 and P2X7 in the homogenate of lung tissue by Western Blotting technique. The results showed that LLLT significantly reduced the number of total cells in the bronchoalveolar (BAL), the levels of cytokines IL-6, IL-1β, IL-17, TNF-α and CXCL1/KC, mucus production and collagen, cell death, alveolar enlargement, as well as the expression purinergic P2X7 and increased cytokine anti-inflammatory (IL–10). Therefore,we suggested that the laser 808nm is effective to reduce lung inflammation in an experimental model of chronic obstructive pulmonary disease (COPD).
Atualmente a Doença Pulmonar Obstrutiva Crônica (DPOC)apresenta impacto em morbidade e mortalidade em todo o mundo,com alta prevalência e um elevado custo econômico e social. Ela é caracterizada por limitação lenta e progressiva do fluxo aéreo, desencadeado pela inalação de partículas nocivas presente na fumaça do cigarro. Neste contexto, vários modelos experimentais têm sido propostos, objetivando a descoberta de novas opções terapêuticas. Neste sentido, a terapia com Laser de Baixa Potência (LBP) é uma terapia relativamente nova e eficaz, de baixíssimo custo, sem efeitos colaterais e de possível utilização no tratamento das doenças crônicas pulmonares. No presente estudo estudamos alguns parâmetros em animais com DPOC submetidos à terapia com laser de diodo (808nm), 30 mW, 60s por ponto (3 pontos por aplicação) por 15 dias antes da eutanásia. O protocolo utilizado para a indução da DPOC consistiu em nebulizar camundongos fêmeas C57BL/6 com fumaça de cigarro por 75 dias (2 vezes/dia). No dia 76, os animais foram anestesiados, traqueostomizados, canulados e o lavado broncoalveolar (LBA) coletado. Foram estudados parâmetros funcionais e estruturais, como contagem total e diferencial de células recuperadas do LBA, os níveis de citocinas (IL-1, IL-6, IL-10, IL-17, TNF- e CXCL1/KC) bem como o acúmulo de muco e colágeno. Foi realizada também a histologia nos pulmões para a quantificação do número de linfócitos nas vias aéreas, assim como o remodelamento brônquico. Além disso, avaliamos a expressão dos receptores purinérgicos P2X7 e P2Y2 no homogenato do tecido pulmonar por meio da técnica de Western Blotting.Os nossos resultados mostraram que a terapia com laser de baixa potência em um modelo experimental de doença pulmonar obstrutiva crônica (DPOC), diminuiu o número de células no LBA, a produção de citocinas pró-inflamatórias (IL-1β, TNF-α, IL-6, IL-17), quimiocina (CXCL1/KC), produção de muco e colágeno, morte celular, alargamento alveolar, bem como a diminuição da expressão do receptor purinérgico P2X7 e também aumento da citocina anti-inflamatória (IL-10). Portanto, sugerimos que o laser 808nm é eficaz para reduzir a inflamação pulmonar em modelo experimental de doença pulmonar obstrutiva crônica (DPOC).

Resumo: A Disfunção temporomandibular (DTM) é uma expressão coletiva que engloba vários sinais e sintomas clínicos relacionados aos músculos da mastigação, articulações temporomandibulares (ATMs) e estruturas associadas. As principais queixas relatadas são ruídos e dor na região da ATM, com maior prevalência em adultos jovens, especialmente mulheres. A inflamação intra-articular é considerada uma das mais importantes fontes de dor nesta disfunção. O processo inflamatório pode se localizar na membrana sinovial, cápsula articular ou tecidos retrodiscais, e na presença de alterações degenerativas articulares recebe o nome de artrite. Assim, o laser de baixa potência (LBP) e o hialuronato de sódio de alto peso molecular (HSAPM) tem sido utilizados para o tratamento de distúrbios inflamatórios da ATM e, resultados clínicos promissores têm sido descritos. Estudos experimentais, sob a óptica da biologia celular e molecular, fazem-se necessários para melhor compreensão de seus respectivos efeitos biológicos e elaboração de protocolos clínicos seguros. Desta forma, o objetivo do presente estudo foi caracterizar a ação morfológica e bioquímica da terapia com LBP e HSAPM sobre a artrite induzida na ATM. Utilizou-se 48 ratos machos, Wistar, divididos nos grupos: GA, animais com artrite induzida por meio de injeção intra-articular de adjuvante completo de Freund (CFA); GL, animais com artrite e tratados com LBP; GH, animais com artrite e tratados com HSAPM e; GHL, animais com artrite e tratados com LBP e HSAPM. Todos os experimentos ocorreram na ATM esquerda. As ATMs direitas no GA foram utilizadas como controle (GC). Foram realizadas análises morfológicas por meio de cortes corados em hematoxilina-eosina (HE), azul de toluidina e Picrosirius. Nos discos articulares, também foram realizadas análises histomorfométricas e birrefringência de fibras colágenas (microscopia de polarização). As análises bioquímicas dos tecidos da ATM foram realizadas por meio de dosagens de glicosaminoglicanos sulfatados (GAGs) e zimografia para avaliação das metaloproteinases (MMPs) 2 e 9. Os dados obtidos foram analisados por meio dos testes ANOVA one-way com pós-teste de Tukey e, Kruskal-Wallis seguido do pós-teste de Dunn. O GA exibiu acentuadas alterações morfológicas nos componentes da ATM tais como: elevada espessura do disco articular; hiperplasia da membrana sinovial e formação de pannus; reabsorção óssea da fossa mandibular; aplainamento do côndilo mandibular e presença de intenso infiltrado de células inflamatórias mononucleares no tecido subsinovial. A microscopia de polarização mostrou menor birrefringência das fibras colágenas, indicando alto grau de desorganização das fibras colágenas. Os GL, GH e GHL apresentaram menor espessura do disco articular e maior birrefringência das fibras colágenas em comparação ao GA (p<0,05). Na análise bioquímica, ficou demonstrado que a concentração de GAGs e atividades de MMP-2 e MMP-9 foram estatisticamente maiores no GA em comparação ao GC (p<0,05). Os GL, GH e GHL exibiram menores atividades das isoformas ativas da MMP-2 e MMP-9 em comparação ao GA (p<0,05). Com base nos resultados, concluiu-se que o LBP e HSAPM demonstraram efeitos anti-inflamatórios e protetores sobre as estruturas articulares. A terapia com uso simultâneo do LBP e HSAPM não foi superior aos respectivos tratamentos isolados
Abstract: Temporomandibular disorder (TMD) is a collective term that encompasses several clinical signs and symptoms related to mastigation muscles, temporomandibular joint (TMJ) and its associated structures. The main complaints reported are noise and pain at TMJ region showing higher prevalence in young adults, especially women. The intra-articular inflammation is considered one of the main sources of pain in the development of TMD. The inflammatory process may be located in the synovial membrane, articular capsule or retrodiskal tissues, and the presence of degenerative joint changes is called arthritis. Currently, Low-level laser (LLL) and high molecular weight sodium hyaluronate (HMWSH) have been used for treating TMJ disorders and promising clinical results have been reported. Experimental studies, from the perspective of cellular and molecular biology, are needed for better understanding of their respective biological effects allowing the development of secure clinical protocols. Thus, the aim of this study was to characterize the morphological and biochemical action of the therapy with LLL and HMWSH on a TMJ with induced arthritis. 48 male rats were divided into the following groups: GA, animals with arthritis induced by an intra-articular complete Freund's adjuvant (CFA) injection; GL, animals with arthritis treated with LLL; GH, animals with arthritis treated with HMWSH; and GHL, animals with arthritis treated with LLL and HMWSH. All experiments occurred in the left TMJ. The right TMJs in GA were used as controls (GC). Morphological analysis were performed using slices stained with hematoxylin-eosin (HE), toluidine blue and Picrosirius. Histomorphometric analysis and quantification of birefringence collagen fibers (polarization microscopy) were performed on the articular disc. The biochemical analysis of the TMJ tissues were performed by dosing sulfated glycosaminoglycans (GAGs) and zymography to assess the matrix metalloproteinases (MMP) 2 and 9. Data were statistically analyzed using the one-way ANOVA test with the Tukey's post hoc test and Kruskal-Wallis test followed by the Dunn's post hoc test. GA exhibited marked morphological changes in the components of the TMJ such as thickness of the joint disk; hyperplasia of the synovial membrane and pannus formation; bone resorption of mandibular fossa; flattening of the mandibular condyle and the presence of intense inflammatory infiltrate of mononuclear cells in the subsynovial tissue. The polarization microscopy showed lower birefringence of collagen fibers, indicating a high degree of collagen fibers disorganization. The GL, GH and GHL showed narrower articular disc and higher birefringence of collagen fibers compared to GA (p<0,05). In biochemical analysis, it was demonstrated that the concentration of GAGs and MMP-2 and MMP-9 activities were statistically higher in GA compared to GC (p<0,05). The GL, GH and GHL showed lower activity of the active isoform of MMP-2 and MMP-9 in comparison to GA (p<0,05). Based on the results, it is concluded that The LLL and HMWSH demonstrated anti-inflammatory and protective effects on joint structures. Therapy with concurrent use of LLL and HMWSH was not superior to the respective single treatments

Full textPhotobiomodulation therapy (PBMT) has been widely used in order to accelerate the process of tissue repair, especially in some systemic conditions, such as Diabetes Mellitus. However, just a few studies have the objective of analyzing the dosimetric parameters that can be used in order to establish the best laser protocol that can be delivered to the tissue.Therefore, this study aimed to evaluate, through clinical, histological and biomechanical analyzes, the process of tissue repair afterPBMT, considering the dose, frequency and mode of delivery of the energy.For this, 282 Wistar rats were used, divided into 2 groups: diabetic and non-diabetic. For the induction of diabetes, a dose of 60 mg / kg of Streptozotocin, diluted in 0.1 M Sodium Citrate buffer, pH 4, was injected intraperitoneally. After 72 h of the injection, the glycemia of the fasted animals was measured, using a glucometer (Accu-Chek Performa®). Those animals presenting a glycaemia of 250 mg / dL or higher were considered diabetic animals. After 15 days of theinduction of diabetes, the animals weresubdivided according to the dose, frequency and mode of energy delivery, as follows: control group and diabetic (without irradiation); L1 and L1D (non-diabetic and diabetic irradiated with 1 point of 10J/cm², respectively), L3 and L3D (non-diabetic and diabetic irradiated with 5 points of 10J/cm², respectively). non-diabetic and diabetic irradiated central point of 50J/cm², respectively); L4 and L4D (non-diabetic and diabetic irradiated with a point of 10J/cm2, respectively), L5 and L5D (non-diabetic and diabetic irradiated with 5 points of 10J/cm2, respectively), L6 and L6D (non-diabetic and diabetic irradiated with 1 point of 50J/cm2, respectively). Photon Lase III-DMC®, ? 660nm, power of 40mW was used. There was photographic follow-up of the lesions during the experiment, for later clinical analysis.After 7, 10 and 14 days of tissue excision, the animals were anesthetized, euthanized, and the samples removed for analyzes of re-epithelialization and inflammatory infiltrate;collagen formation, cytokeratin 10 immunoexpression and analysis of the tensile strength analysis. The results suggest that, regardless of the systemic condition of the animal, the dosimetric factors influenced the tissue repair time. Higher frequency of irradiationwith lower and more distributed doses in the lesion resulted in a faster clinical repair, higher expression of cytokeratin 10 and greater tissue resistance to rupture.Furthermore, for diabetic animals, higher frequency of irradiation with lower and more distributed doses in the tissue also resulted in a decrease of inflammatory process, greater production and remodeling of collagen, better process of re-epithelization and maturation of the epithelium, resulting in a fasterwound healing process. Thus, we can conclude that a higher frequency of irradiation and a better distribution of energy in the tissue result in a faster tissue repair process.

Full textThe diagnosis and early treatment of skeletal posterior cross bite or maxillary atresia is extremely important, and the use of disjunction appliances is the treatment of choice for these malocclusions. The use of low-power laser with the purpose of stimulating bone repair and diminishing the painful sensation in this type of treatment is being studied and has shown promising results. The aim of this work is to evaluate the osteoinductive action of low power laser on median palatine suture, and the effects of this radiation in the painful sensation in patients undergoing rapid maxillary expansion (REM). As a secondary objective, the dental arches space, before and after the procedure of REM, will be assessed. A total of 34 individuals participated in the research, being 16 in the Control Group and 18 in the Experimental Group. Orthodontic documentation was requested and they did confirm that the individuals had skeletal maxillary atresia, and the treatment plan consisted in the use of Hyrax disjunction appliance. Patients in the Experimental Group underwent Diode Laser irradiation (980nm, 0.3W) every 5 days during the active phase of treatment, and 8 weekly applications after overcorrection and stabilization of the expander screw (passive phase of treatment) in six points distributed bilaterally to the median palatine suture, for 10 seconds on each point. In patients of the Control Group irradiation was held in the Standby mode of the device to characterize the placebo effect. During treatment, patients in both groups underwent digital occlusal x-ray examinations according to the following timetable: stabilization of screw, 1 month, 2 months, 3 months and 6 months, patterned with positioner and penetrometer. The Image J program was used to compare the x-rays. The influence of laser in painful sensation among the groups was evaluated through the Visual Scale of Pain. After removing the disjunction appliance, new orthodontic documentation was requested for comparison of dental arches space, before and after the procedure, according to Moyerss method. Inter and intra-examiners concordances were evaluated by means of intraclass correlation tests. The generalized mixed model was used, considering the individual as random factor followed by Tukeys post-test, for statistical analysis of bone formation. The mixed generalized model for ordinal repeated data was also used for the analysis of pain. All analyses were carried out in the statistical program SAS 9.3. The results showed that there was no statistical difference between the groups regarding the variables analyzed and that there was space gain in the upper arch. The paper concludes that the low-power laser therapy, in the parameters used, did not influence bone regeneration and the soreness; and that the REM procedure did promote space gain on the upper arch.

Full textTemporomandibular disorders (TMD) affects more than 25% of de adult population. Physiotherapy resources as low level laser therapy (LLLT) is use in treatment of many types of pain including pain in TMJ. The serum cortisol level can be altered in cronic pain due the excess hipotalamic-hipofisary-adrenal axis stimulation. Combinations of the hipotalamic-hipofisary-adrenal axis and symphatetic-adrenal secretions would constitute the neuroendocrinology answer to stressfull incentives. The heart rate frequency alterations reflect in simphatetic and parassimphatetic nervous system alterations and it makes possible the autonomic nervous system study starting from the study of the heart rate variability. The aim of this study was evaluate myofascial chronic pain in women with TMD and its relationship with serum cortisol and heart rate variability after low level laser therapy. 15 womens, age 18-fourty years, with myofascial chronic TMD was evaluated from Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD), submited to eletrocardiography (ECG) to evaluate the heart rate variability, and concentration of serum cortisol collect both before and after ten sessions of LLLT. The pain and palpation pain were registered in the visual analogic scale (VAS) and in RDC/TMD respectively. The women with myo heart rate fascial chronic TMD presented a significant decrease of the pain and palpation pain levels after LLLT (p < 0,001). There was a significant increase in serum cortisol levels after LLLT (p < 0,005), and in the oppening mandibular movements with or without pain and passive oppening mandibular movements (p < 0,001); the heart rate variability in the frequency domain presented significant increase the increment in very low frequency (VLF) and low frequency (LF) before treatment (p < 0,007 and p < 0,001) and no significant after treatment (p < 0,06 and p < 0,1). These results suggest that women with myofascial chronic pain showed low levels of serum cortisol and a large increment in the simphatetic nervous system activity in the heart with altered inversely in the pain decrease after LLLT.

Full textThe injuries on the face are more common in today\'s society, involving motor vehicle accidents, falls or iatrogenic consequences after procedures. These injuries can affect the nerves responsible for facial muscles reflecting in physical, emotional and psychosocial changes, for example in the stomatognathic system, in voice, facial expression and aesthetics, feelings and social life of the individual. Current researches are aiming best techniques to the process of peripheral nerve repair and functional rehabilitation of these injuries. Traditional techniques such as end-to-side epineural suture coaptation by fibrin sealants are used for this purpose. The objective of this study was to evaluate the repair of buccal branch of the facial nerve injured by end-toside technique in two different ways: epineural suture and the new heterologous fibrin sealant, associated or not to treatment with low power laser. Thirty-two rats (Rattus norvegicus, Wistar) were used. They were 80 days old and were randomly divided into Control Group (CG, n=8) and Experimental Groups (Experimental Suture Group ESG and Experimental Fibrin Group - EFG; n=12; Experimental Suture Laser Group ESLG and Experimental Fibrin Laser Group - EFLG; n=12). The CG animals did not receive surgery; the ESG was performed on the right side of the face, the section of the buccal branch of the facial nerve, where the proximal stump was sutured to the subcutaneous tissue and the distal stump sutured end-to-side portion of the zygomatic branch of the facial nerve; EFG on the left side of the face, the same procedures were carried ESG, but was used coaptation with fibrin sealant of the distal stump. The groups ESLG and EFLG, and the techniques described, have been treated with the application of Laser Gallium Aluminum Arsenide (GaAlAs), continuous pulse, wave length 830 nm, 6J /cm2 for 24 seconds, three times per week for five weeks, at three points of the local operated from both sides. It also performed a functional analysis of the movement of the vibrissae of all groups in the 5th and 10th post-operativeweeks. The animals were euthanized 10 weeks after surgery and the fibers were morphologically analyzed, using light and electron microscopy, and morphometrically by means measures the area and diameter of the fiber, area and diameter of the axon, thickness and diameter of the myelin sheath. It was possible to observe regeneration in the distal stump of myelinated and unmyelinated fibers. The measured variables, there was a significant difference in the area of the nerve fiber between the EFG and EFLG groups. Functional recovery of the movements of the vibrissae the best results in the groups with fibrin sealant use (EFG and EFLG), and the groups associated with laser therapy were the most nearly CG. Therefore, it follows that the two methods discussed for peripheral nerve repair was effective, allowing for axonal sprouting in the distal stump, and that the laser treatment yielded better results, both morphometric and functional.
