Abstract Drug-resistant bacterial infections constitute a major threat to global public health. Several key bacteria that are becoming increasingly resistant are among those that are ubiquitously carried by human beings and usually cause no symptoms (i.e. individuals are asymptomatic carriers) until a precipitating event leads to symptomatic infection (and thus disease). Carriers of drug-resistant bacteria can also transmit resistant pathogens to others, thus putting the latter at risk of infections that may be difficult or impossible to treat with currently available antibiotics. Accumulating evidence suggests that such transmission occurs not only in hospital settings but also in the general community, although much more data are needed to assess the extent of this problem. Asymptomatic carriage of drug-resistant bacteria raises important ethical questions regarding the appropriate public health response, including the degree to which it would be justified to impose burdens and costs on asymptomatic carriers (and others) in order to prevent transmission. In this paper, we (i) summarize current evidence regarding the carriage of key drug-resistant bacteria, noting important knowledge gaps and (ii) explore the implications of existing public health ethics frameworks for decision- and policy-making regarding asymptomatic carriers. Inter alia, we argue that the relative burdens imposed by public health measures on healthy carriers (as opposed to sick individuals) warrant careful consideration and should be proportionate to the expected public health benefits in terms of risks averted. We conclude that more surveillance and research regarding community transmission (and the effectiveness of available interventions) will be needed in order to clarify relevant risks and design proportionate policies, although extensive community surveillance itself would also require careful ethical consideration.

These control measures may in many ways affect the lives and well-being of carriers of MDRO, resulting in complex ethical dilemmas that often remain largely implicit in practice. In this chapter, we propose to conceptualize the impact of MDRO control measures on the well-being of individual carriers in terms of capabilities and functionings. A capabilitarian framework for the ethical treatment of MDRO carriers commits us to conceptualize the harm done to carriers in terms of the impact that MDRO control measures have on what they are able to do or be. Adopting and adapting Nussbaum’s list of ten central human capabilities, we present a taxonomy of capabilities and functionings that are normatively relevant for the design and evaluation of MDRO control measures.

AbstractThe aim of this work was to design and optimize compositions of three-component composites based on polyetheretherketone (PEEK) with enhanced tribological and mechanical properties. Initially, two-component PEEK-based composites loaded with molybdenum disulfide (MoS2) and polytetrafluoroethylene (PTFE) were investigated. It was shown that an increase in dry friction mode tribological characteristics in metal-polymer and ceramic-polymer tribological contacts was attained by loading with lubricant fluoroplastic particles. In addition, molybdenum disulfide homogenized permolecular structure and improved matrix strength properties. After that, a methodology for identifying composition of multicomponent PEEK-based composites having prescribed properties which based on a limited amount of experimental data was proposed and implemented. It was shown that wear rate of the “PEEK + 10% PTFE + 0.5% MoS2” composite decreased by 39 times when tested on the metal counterpart, and 15 times on the ceramic one compared with neat PEEK. However, in absolute terms, wear rate of the three-component composite on the metal counterpart was 1.5 times higher than on the ceramic one. A three-fold increase in wear resistance during friction on both the metal and ceramic counterparts was achieved for the “PEEK + 10% PTFE + 0.5% MoS2” three-component composite compared with the “PEEK + 10% PTFE”. Simultaneous loading with two types of fillers slightly deteriorated the polymer composite structure compared with neat PEEK. However, wear rate was many times reduced due to facilitation of transfer film formation. For this reason, there was no microabrasive wear on both metal and ceramic counterpart surfaces.

The chapter demonstrates the intersections among tourism and diaspora studies and issues of consumption, mobility, culture, sexuality, and sexual labor. Many critics of tourism are referenced in order to trace the legacy of slavery and colonialism found in the tourist industry, which emerges in economic, socio-political, cultural, and sexual terms. The sexual-cultural politics of tourism are introduced as well the sites of resistance and the methodology and feminist postcolonial framework of “resisting paradise.”

Three of the forms and assumptions of moral practice are that moral judgements are truth-apt, sometimes true, and that they express moral beliefs. Vindicating these assumptions seems inconsistent with expressivism as traditionally conceived. However, minimalist accounts of truth-aptness, truth, and belief may help the expressivist. Minimalism says that the correct account of a notion is revealed by all and only those platitudes surrounding it. Practical expressivists accept that moral sentences satisfy truth-aptness, and they also accept that moral sentences are truth-apt. This helps expressivism secure truth-aptness, but also encourages the thought that there is nothing distinctive in the expressivist position. But creeping minimalism can be resisted since there is a robust sense of belief that resists minimalism. It is in this robust sense that expressivists will deny (and descriptivists accept) that the meaning of moral judgements is to be explained in terms of their expressing moral beliefs.

Comprehensive neurophysiology, structural neuroimaging, and neuropsychological and psychiatric assessment are regarded as essential elements. Invasive electroencephalography (EEG) monitoring in terms of subdural electrodes, depth electrodes, foramen ovale electrodes, and more advanced technique using stereoelectroencephalography (SEEG) are strong armamentarium for epilepsy surgeon. Epilepsy surgery in terms of resection, disconnection, or neuro-modulation should be recommended after a multi-disciplinary agreement.
