Abstract:
How did men cope with sexual health issues in early modern England? This vivid history investigates how sexual, reproductive, and genitourinary conditions were understood between 1580 and 1740. Drawing on medical sources and personal testimonies, it reveals how men responded to bouts of ill health and their relationships with the medical practitioners tasked with curing them. In doing so, this study restores men’s health to medical histories of reproduction, demonstrating how men’s sexual self-identity was tied to their health. Charting genitourinary conditions across the life cycle, the book

Abstract:
Testicular germ cell tumours (GCTs) represent the most common solid malignancy of young men aged 15–40 years. The disease is rising in incidence. Germ cell tumours are best divided into those with pure seminoma and non-seminoma (NSGCT) histology. While cryptorchidism is clearly established as a risk factor, the pathogenesis of testicular cancer remains unknown. Familial studies and molecular analyses suggest an association to genetic alterations. Most testicular cancer patients present a primary tumour in the testis. Diagnostic examinations include testis palpation and ultrasound, and measurem

Abstract:
Ovarian cancer is the fifth most common cancer in females in the UK. In contrast, testicular cancer is a rare disease: there were 2138 new cases of testicular cancer diagnosed in 2008 in the UK, and only 70 deaths. Ninety per cent of all ovarian cancers are of epithelial origin, although germ cell and sex cord–stromal cell tumours also occur. In contrast, 95% of testicular cancers are germ cell tumours, with stromal cell tumours and lymphomas making up the remaining 5%. This chapter discusses ovarian cancer and testicular cancer, including definitions of the diseases and their etiologies, typi

Abstract:
Tumours of the testis are uncommon and are usually germ cell tumours. They present most often as a scrotal mass in a young man, and are the most frequent malignant tumour in this age group. The incidence has risen over recent decades and is higher in Western Europe. A history of testicular maldescent is a significant risk factor. About 50% are pure seminoma; the remainder non-seminomas may have a single but more usually a mixed histology. Non-seminomas are all called teratoma in the British system. Metastasis readily occurs to paraaortic lymph nodes or the lungs, and some patients present with

Chapter 8c discusses the role of radiotherapy in testicular cancer and how it is becoming less prominent. The mainstay of treatment is radical orchidectomy and, where there is a risk of metastatic disease, combination chemotherapy. Radiotherapy may be indicated in the following situations: 1) Stage I testicular seminoma delivering prophylactic para-aortic lymph nodes irradiation, and 2) Palliative treatment in the management of chemotherapy resistant disease.

Abstract:
Genitourinary cancers examines the malignancies arising in the kidney, ureter, bladder, prostate, testis, and penis. Renal cancer has high propensity for systemic spread, largely mediated by overexpression of vascular endothelial growth factor (VEGF). Treatments include surgery, immunotherapy, and targeted therapy. Wilms tumour, a childhood malignancy of the kidney, warrants specialist paediatric oncology management to provide expertise in its unique pathology, staging, and treatment, often with surgery and chemotherapy. Cancer of the bladder and ureters, another tobacco related cancer, may pr

Abstract:
The assessment of men with fertility problems is described in this chapter. The main causes of male infertility are testicular insufficiency due to congenital and acquired causes, obstructions of the male genital tract, genetic and endocrine abnormalities, urogenital infections, and varicoceles. Lifestyle can also have a negative influence on semen quality: smoking, obesity, drugs, and anabolic steroids influence sperm parameters and may reduce natural conception. Some chronic diseases also have a negative influence on fertility. History taking and physical examination should focus on prevalen
