Abstract:
This chapter considers the challenges for clinicians involved in genetic counseling for Huntington’s disease, referring to the literature and best-practice guidelines. The issues involved in genetic testing and counseling around reproductive options are discussed. Ways of tailoring predictive test counseling to take better account of individuals’ experiences and knowledge are explored, and more complex predictive test counseling scenarios, such as requests from minors and individuals at 25% risk, are also covered. The process of assisting families near the time of diagnosis is explored, including ways of helping individuals in apparent denial of symptoms and facilitating communication among family members. Finally, this chapter reflects on lessons learned from our collective experiences and considers areas in which more work is needed.

Abstract:
In this chapter, we discuss prospects for a future computational neuropsychology. Computerized approaches to assessment, the ability to implement life-like scenarios in a controlled virtual environment, and teleneuropsychology offer promise for expanding available approaches to cognitive remediation and self-monitoring. Computational models are also available increasingly for integrating neuroimaging into the assessment process. Neuropsychologists can use neuroimaging to develop new frameworks for neuropsychological testing that are rooted in the current evidence base on large-scale brain system interactions. This will allow for traditional assessment of discrete areas of neurocognitive functioning to be brought in line with recent findings that highly nuanced relations exist among brain networks. Furthermore, the new findings from systems neuroscience may allow for the development of neuropsychological assessments with greater accuracy and increased targeted testing. Neuroinfomatic approaches offer computational neuropsychology an approach to knowledge sharing via well-defined neuropsychological ontologies and collaborative knowledgebases.

Abstract:
Among the advanced cancer population, existential concerns are major issues that promote significant distress. For patients who are facing death, meaning and the preservation of meaning are not only clinically and existentially important but also central concepts to a therapeutic intervention. Based on Viktor Frankl’s logotherapy and the principles of existential psychology and philosophy, “meaning-centered psychotherapy” was developed to help patients with advanced cancer sustain or enhance a sense of meaning, peace, and purpose in their lives. This chapter provides an overview of work developing and testing individual meaning-centered psychotherapy (IMCP). It provides an overview of the session content in the IMCP intervention. It also presents findings from clinical trials, which support the efficacy of IMCP as an intervention to increase a sense of meaning, spiritual well-being, and hope while decreasing end-of-life despair. Furthermore, it presents difficult scenarios that may arise when delivering IMCP for clinicians interested in this work.

This book describes the most influential studies that have shaped today’s clinical practice of urology. Its scope spans the spectrum of clinical urology, including genitourinary oncology (prostate, renal, bladder, and testis cancer), stone disease, benign prostatic hyperplasia, erectile dysfunction, and female urology. Most included studies are landmark randomized controlled trials related to questions of therapy, but the selection also includes prospective observational cohort studies as well as some case series that have resulted in paradigm-shifting changes in patient management. Each individual study is succinctly presented using a standardized format that focuses on the most important aspects relating to its design and main findings. This is followed by a summary of relevant, closely related studies as well as a brief critique of each study’s limitations. The evidence-based format of this book is further underscored by the frequent reference to relevant clinical practice guidelines. Each chapter closes with a clinical scenario that presents a management question for which an expert clinician proposes an answer based on the current best evidence.
