Objectives: Although testicular cancer (TC) is the most common tumor in young men in Western countries, there is no official cancer detection/screening program for young men in Germany. The most important TC detection tool is self-examination of the testis. Hypothetically medical students may have a diagnosis lead time and detection superiority. This study was designed to analyze whether medical students have a possible knowledge advantage over students of other faculties concerning TC and to compare male and female cancer screening demeanor and mentality. Methods: Male and female students of various faculties at the Goethe University Frankfurt/Main, Germany were invited to participate in this internet-based anonymous questionnaire with questions about TC awareness/knowledge, testicular (self) examination, and cancer screening behavior. Results: In total 1,049 students (329 medical and 716 non-medical students) completed the questionnaire. In general, medical students had a significantly higher TC knowledge, especially in the more advanced stages of their medical studies (year 3-6). About 50% of medical students had knowledge of TC whereas only 21.3% of non-medical students knew about the disease (p < 0.01). In addition, medical students conducted scrotal examinations more frequently (34.7%) than non-medical students (18.8%). Conclusion: The knowledge about TC is low among students. In general, medical students are more aware of TC and perform more frequent testicular examinations compared to non-medical students. Female TC knowledge rises in the clinical part of studies to the same level as their male counterparts, with the result of more testicular partner examinations.

Abstract:
Based on physical examination alone it is often difficult to accurately make a clinical diagnosis and decide whether a palpable scrotal pathology arises from the testis itself or from extratesticular elements within the scrotum. Clinical examination of the testis is especially difficult due to pain, tenderness or when obscured by a large hydrocele. The aim of the study was thus undertaken to evaluate the role of ultrasonography (USG) in patients presenting with scrotal pain. 50 patients who presented to the surgical department with scrotal pain were subjected to USG. The commonest cause for scrotal pain was epididymitis with or without orchitis (40 %), followed in descending order by cystic lesions of the epididymis (18 %), varicocele (12 %), trauma (8 %), torsion (4 %) and tumour (2 %). No abnormality was seen on USG in 7 patients ( 14 %). This study concludes that USG is a sensitive and specific examination in patients with scrotal pain. Besides differentiating testicular from extratesticular origin it can also differentiate solid from cystic masses. Colour flow imaging is especially helpful in varicocele and testicular torsion. Torsion of the testis remains the most urgent and important entity dependent on USG for diagnosis. Abstract:
Two to three weeks old 3 male indigenous Korean calves weighing 35–40 kg were referred to the Animal Medical Centre,ChonbukNationalUniversitywith signs of urine scalding on the ventral perineum. Physical examination in all 3 cases revealed aplasia of the penis along with an undescended testis, bifid scrotum and ventrally incomplete sheath. In case 1, the urethra opened cranially to the bifid scrotum, whereas in case 2 and 3, the urethra opened at the ventral perineum. The diagnosis was made as hypospadias, along with penile aplasia and unilateral cryptorchidism. With an exploratory laparotomy, the abdominal testis was removed. The scrotal testis was also castrated and scrotal ablation was performed. In case 1, prescrotal urethrostomy and in case 2 and 3, perineal urethrostomy was performed. Two weeks after surgical correction, the caves appeared to be urinating normally.

Abstract:
The present study was conducted to quantify in vitro gonadotropin-releasing hormone (GnRH) release parameters in the male marmoset. We established primary cultures of marmoset hypothalamic tissues for ∼2 days (marmosets) to assess GnRH release profiles in vitro in hypothalamic explants from testis-intact and gonadectomized males. Pulsatile GnRH release profiles were readily demonstrated from in vitro hypothalamic explants isolated from adult male marmoset monkeys. Gonadectomy of male marmosets resulted in elevated mean GnRH and pulse amplitude from hypothalamic explants on the 1st day of culture ( day 0). GnRH pulse amplitude increased by day 2 in ∼67% of hypothalamic explants from testis-intact marmosets, suggesting release from an endogenous regulator of GnRH. We also measured GnRH release profiles in vitro in hypothalamic explants from testis-intact and gonadectomized rats. Male rats showed no changes in any concentration or frequency release parameters for GnRH following gonadectomy or during successive days in culture. The present study represents a unique examination of GnRH release from male marmoset monkey hypothalamic tissue and compares release dynamics directly with those obtained from male rat, suggesting a species difference in feedback regulation of GnRH release.

Abstract:
Objective. A Chinese herbal prescription, Yiqi Jiedu (YQJD) decoction, is used for injuries caused by radiotherapy. This study aimed to investigate the dynamic protective effects of YQJD decoction in mice on radiation damage at different time points of the testis. Methods. The 200 male Balb/c mice were randomly divided into 5 groups (blank, model, Anduolin, low-dose YQJD, and high-dose YQJD groups). Blank and model groups were all given stomach deionized water. Anduolin group and high and low dose of Chinese herb groups were treated with Chinese material medical decoction compound for 2 weeks before being irradiated. Except for the blank group, other groups were given whole-body one-time irradiation of 5.5 Gy 60Co gamma rays. Histopathology examination of testes was conducted on the 1st, 7th, 21st, and 35th day after irradiation, respectively. Indexes of sperm motility rate, sperm count, serum levels of testosterone (T), estradiol (E2), follicle-stimulating hormone (FSH), and luteinizing hormone (LH) were also measured correspondingly. Result. Compared to 1 day and 7 days after irradiation, most of the indexes, including testis index, sperm motility, and count, were decreased on the 21st and 35th day in all irradiated groups. However, compared with the model group, the indexes to assess sperm quality and the morphological structures of testis in low-dose and high-dose YQJD groups were significantly recovered on the 21st and 35th days. Additionally, reproductive hormones, including testosterone and estradiol, were recovered more in low-dose and high-dose YQJD groups on the 21st and 35th days. Conclusion. The YQJD decoction is able to accelerate the recovery of sperm indexes, testosterone, and estradiol levels of sex hormones in mice after irradiation and demonstrated good protective effects on the testis.

Abstract:
Malignant mesothelioma of the tunica vaginalis testis is an extremely rare tumor representing 0.3% to 5% of all malignant mesotheliomas. Gross examination of testicular mesotheliomas typically reveals tumor nodules studding the thickened tunica vaginalis and, in some cases, infiltrating the testicular parenchyma, leading to diagnostic challenges. Microscopically, the tumor is characterized by epithelioid cells arising from the tunica vaginalis with papillary, tubulopapillary, or solid architectural patterns. The papillae are usually lined by a single layer of cells with relatively bland cytologic features. An epithelial cell phenotype admixed with a sarcomatoid pattern has also been described in a few cases. Immunohistochemically, the tumor is usually positive for calretinin, Wilms tumor-1, epithelial membrane antigen, D2-40, thrombomodulin, cytokeratin 7, and cytokeratin 5/6. Electron microscopic studies reveal epithelial cells joined by tight junctions, forming lumina, and displaying long microvilli with length to width ratios often greater than 10. The most important differential diagnostic considerations include florid mesothelial hyperplasia, adenomatoid tumor, carcinoma of the rete testis, and serous papillary tumors. In addition, the various types of testicular germ cell tumors should be considered, including seminomas, embryonal carcinomas, and intratubular germ cell tumors, particularly in tumors with testicular parenchymal involvement. Pleomorphic sarcomas should also be considered, particularly when dealing with the biphasic variant. The prognosis for this entity is grave, with a median survival of 23 months. Aggressive therapy with radical orchiectomy remains the mainstay of treatment.

A highly specific antiserum was raised against purified bovine endothelin ETB receptor and used to determine the tissue distribution of the receptor subtype ETB and to localize the receptor immunohistochemically in the kidney, adrenal gland, lung, cerebellum, and pituitary gland whose functions are known to be under strong influence of endothelin. The antiserum raised in a rabbit specifically recognized the receptor band on Western blot analysis of membrane proteins. Furthermore, it immunoprecipitated only ETB, establishing its ETB specificity. By determination of the percentage of the total number of the endothelin receptors that is immunoprecipitable with the antiserum, the amounts of the ETB relative to those of the ET receptors were found to vary from tissue to tissue: lung (70%), cerebellum (55%), pituitary gland (50%), kidney (25%), adrenal gland (10%), and testis (< 2%). This means that, in the lung, ET is the major form, whereas in the testis, ETA is predominant, comprising >95% of the receptors. Immunohistochemical examination of tissue sections revealed endothelium localization of the ETB endothelin receptor.

Abstract:
Testicular cancer, although relatively rare, is the most common malignancy in men at the age of 15 to 35 years. Germ cell tumor (GCT) is the most common testicular tumor (90%-95%) and peak age of incidence is between 20 years to 50 years. Choriocarcinoma is very rare in male as a pure testicular tumor (<1%) but may be seen as a component of mixed GCT. These tumors characteristically secrete hCG (human chorionic gonadotrophin) into the serum, which use as an important serum tumor marker for these tumors. A 30 years old male presented to us with the complaints of hard feeling of the left testis for 6 months which was initiated with mild pain but the size of the testis was unchanged. On examination there was an indurated area (2cm X 2cm) at lower part of the left testis and epididymis felt separately. He had no history crypto-orchidism, orchitis and scrotal trauma. Ultrasonogram showed a mixed echoic mass lesion (13 X 10mm) at infero-medial aspect left testis with an epididymal cyst. On laboratory investigation his ?-hCG and AFP level were 306 MIU/L(high) & 6.3 IU/L respectively. Contrast enhanced CT scan of the whole abdomen revealed no abnormality except enlarged lymph node (> 1 CM) in right lower lung. Fine needle aspiration cytology (FNAC) showed features of chronic orchitis. We explored his left testis because of high ?-hCG through inguinal approach and suspected testicular tissue was sent for frozen section biopsy which also revealed inconclusive findings. On the basis of his high ?-hCG, we performed left radical orchidectomy. Histopathology of the left testis showed features of Choriocarcinoma, epididymis and spermatic cord were normal. According to TNM classification, AJCC (American Joint Committee on Cancer) staging and international germ cell consensus prognostic classification it was T3N0MISI, stage IIIa and intermediate prognosis group respectively. Postorchidectomy ?-hCG dropped to 7.3 MIU/ML. Now he is on systemic chemotherapy BEP(bleomycin, etoposide, cisplatin). Choriocarcinoma, though is a rare malignancy, it may affect young men in the prime of life and is the most aggressive histologic variant of germ cell tumor. But it has a good prognosis if diagnosed early and treated accurately. Serum ?-hCG level plays most important role in diagnosis, in monitoring therapy and follow-up of patients with choriocarcinoma. Abstract:
This investigation examined the self-efficacy beliefs of medical professionals towards medically-related tasks. Although several instruments existed that measured different aspects of career self-efficacy, none of them was relevant nor appropriate to people who have already chosen their occupational field of choice. The typical scales utilized in assessing career self efficacy are generalized across occupations and not relevant to someone who was already employed in a profession. The purpose of the present study was to develop a new scale, Medical Tasks Self-Efficacy (MTSE), to measure the self-efficacy beliefs of a specific employed population toward their occupational tasks. The analysis of the MTSE was conducted using respondents (N=307) of medical professionals, specifically physicians and nurses. The reliability and validity of the scale was analyzed through four phases of scale development.Phase one consisted of item development which included the generation of an item pool from special occupational books and interviews with medical professionals (N=8). Additional experts (N=3) in scale construction were also consulted during this phase. Thirty-two items were developed originally and then, through consultation, reduced to 27 items. Phase two involved a pilot study with respondents from various medical settings in the midwest (N=34). The pilot study helped to provide preliminary reliability information and to modify the necessary demographic information. One more item was dropped from the scale during this phase and thus the revised MT SE consisted of 26 items.Phase three consisted of a major investigation (N=209) to test the initial factor structure of the MTSE and to examine the internal consistency. Using a principal components extraction, the MTSE yielded a two factor solution which seemed to best fit the data both in terms of statistical configuration and theoretical soundness. Factor One constituted tasks involving “medical content competencies" and held an alpha coefficient of .93. Factor Two constituted tasks involving "interpersonal process competencies" and held an alpha coefficient of .87.The fourth and final phase (N=64) was conducted to test the convergent and discriminant validity of the scale and to examine the test-retest reliability. Convergent and discriminant validity were tested using the Task Specific Occupational Self-Efficacy Scale (TSOSS) and the Beck Depression Inventory (BDI). The MTSE was hypothesized to demonstrate convergent validity with the TSOSS. Both factors of the MTSE were significantly correlated with the overall TSOSS, and additionally correlated with the four factors of the TSOSS, with one exception. Only Factor Two ("interpersonal process competencies") of the MTSE did not correlate significantly with factor four of the TSOSS. The BDI was used to test discriminant validity. The BDI did not correlate significantly with either of the factors of the MT SE. Thus, the MTSE demonstrated both convergent and discriminant validity with the instruments utilized in this study. The two-week test-retest correlation was .80, additionally demonstrating a stable reliability estimate in the MT SE. Neuropsychologists are increasingly asked to determine whether a patient may be malingering symptoms of a mild closed head injury. This issue is particularly salient within the context of civil litigation and the potential of significant financial awards. Patients' performances on neuropsychological tests have historically been assumed to accurately reflect their abilities and deficits. Optimal motivation and performance cannot be automatically assumed within the context of litigation. Moreover, comorbid anxiety and depression are frequently present in head injury patients and adversely affect the patient's performance.The frequent comorbidity of psychiatric and medical symptoms complicates interpretation of a patient's neuropsychological evaluation whether or not the patient is involved in litigation. This comorbidity may result in an inaccurate diagnosis, thus delaying treatment potentially causing greater harm to the patient.The present study was conducted to expand previous research that discriminated between simulated malingered and neurological patient responses to a neuropsychological self-report inventory. Additionally, the study investigated the, utility of the Neuropsychological Symptom Inventory in discriminating between simulated medical, psychiatric and malingered patient responses. Results indicated that the NSI was able to discriminate malingered responses from medical and psychiatric patient responses. However, applying a lie scale derived from previous research with the NSI did not allow discrimination between the malingered group and the psychiatric patients. Use of a factor solution derived from earlier research may offer not only greater prediction in detection of malingerers, but also evaluation of symptom profiles of medical and psychiatric patients. The NSI may provide an efficient screen for exaggerated symptoms as well as an indication of the level of general neuropsychological functioning of the patient when included in a neuropsychological evaluation.<br>Department of Educational Psychology

Introduction: Chronic diseases can be prevented and improved by regular physical activity and reduced sedentary time. An adequate leg strength and leg function in treatment and prevention of these disease is something to strive for. To the Lifestyle Reception at Karlskoga Hospital are patients with mild to moderate mental illness referred where a new method Self-measurement tests of leg strength is used, containing 30 seconds chair-stand test, squat, heel rise and maximal step-up test, aiming to simple training and indicating an objective measure of leg strength. Objective: To describe the group of patients referred to the Lifestyle Reception and investigate change of leg strength/leg function using Self-measurement tests, self-rated health and self-rated physical function following 12 weeks of intervention. Methods: A clinical intervention study of 29 patients referred to the Lifestyle Reception at Karlskoga Hospital between Nov 2017 and March 2018. They participated in an intervention consisting of group training twice a week for 12 weeks. A sign-in and a sign-out interview was carried out where data (tests and a questionnaire) of the study was collected. Result: 29 patients (24 women / 5 men) participated. The mean age was 39 years and the mean BMI was 31 (corresponding to obesity). All patients were on sick leave to some extent. The theoretical individual maximal step-up height (% -tMSH) improved from 65% to 75% (p &lt;0.001). The patients could on average do 5.5 more 30s-chair-stand-test (p &lt;0.001) and the proportion who managed to do squats without support increased from 57% to 90%. Significant improvements were seen for the self-rated health (p &lt;0.001) and also for the HAD scale (anxiety p = 0.012; depression p = 0.04). However, the improvement of the self-rated physical function was not significant. Conclusion: Significant improvements in all four self-tests of leg strength / leg function could be measured after the intervention as well as a significant improvement of self-rated health. Self-measurement test of leg strength is considered to be a simple and useful test method in clinical practice.

Abstract:
Each year hundreds of thousands of women in Europe need to catheterize themselves instead of urinating the usual way, because of diseases or injuries. However, self-catheterization is hard for a lot of women due to the location of the urethra, and they need some type of aid to help them. The design company MMID in the Netherlands developed a concept with a mirror and a light, which is attached to a toilet seat. Though, it was noticed that it was too hard to handle for the users, hence the authors were asked to improve the user friendliness with the product. Since the intuition aspect was the largest problem they decided to focus on intuition. MMID’s original prototypes were tested by patients and continence nurses to get relevant information that only real users can give, and by mixed women without troubles, to get an insight in the intuition aspect. The nurses and the patients tested the prototypes and were observed, whereupon they took part in a semi-structured interview and a questionnaire, respectively. During the intuition tests the women’s interactions with the prototypes were filmed, and they answered a short questionnaire. The films were then analyzed. The tests showed that the product was very hard to understand and that it gave a feeling of being unstable. This gave the project the focus to make the product easy to open up, without having to assemble it before each use. All important aspects were gathered and several ideation sessions were performed. The results were put together into three concepts and 3D printed prototypes. These models were tested by women interacting with them to see their intuitive actions, which were also filmed. The women also participated in semi-structured interviews. Based on these results a concept choice was made where the simplest solution with one pivoting point was chosen. The concept was further developed and functional prototypes and non-functional prototypes were made. The non-functional models were tested for only intuition among random women, the same way as before. The functional models were tested by patients and nurses at four hospitals in the Netherlands, also these tests were similar to the ones in the first round. The overall results were positive and the product had been significantly improved; a lot thanks to the fact that it did not need to be assembled before it could be used. The biggest negative aspect was that a signifier for opening the product was missing.<br>Varje år behöver hundratusentals kvinnor i Europa katetrisera sig själva istället för att urinera som vanligt, på grund av sjukdomar eller skador. Självkatetrisering är dock krångligt för många kvinnor på grund av placeringen av urinrörets öppning. Designföretaget MMID i Nederländerna tog fram ett koncept med en spegel och en lampa som fästs på toalettsitsen för att hjälpa dessa kvinnor. Det upptäcktes dock att detta koncept var svårförstått och således ombads författarna att förbättra användarvänligheten hos produkten. Då intuitionsaspekten hos produkten var det största problemet bestämde de sig för att fokusera på intuitionen. MMIDs ursprungliga prototyper testades av patienter och urologisköterskor för att få relevant information som bara riktiga användare kan ge, samt av blandade kvinnor utan åkommor för att få inblick i intuitionsaspekten. Sköterskorna och patienterna testade prototyperna och observerades, varpå de deltog i semistrukturerade intervjuer respektive fyllde i ett frågeformulär. Under intuitionstesterna filmades kvinnornas interaktion med prototyperna, varpå de svarade på ett kort frågeformulär. Filmerna analyserades sedan. Testerna visade bland annat att produkten var väldigt svårförstådd samt gav en känsla av att vara ostadig. Detta gav projektet fokuset att skapa en produkt som kan öppnas upp på ett enkelt sätt utan att behöva monteras ihop innan varje användning. Alla viktiga aspekter sammanställdes och flertalet idégenereringar genomfördes. Resultaten från dessa sattes ihop till tre koncept, vilka överfördes till 3D-printade modeller. Modellerna testades genom att kvinnor fick interagera med dem varpå deras intuitiva handlingar observerades. Detta spelades in på video och kvinnorna ombads även att svara på frågor i en kort semistrukturerad intervju. Baserat på dessa resultat gjordes ett konceptval där den enklaste lösningen, med en rotationspunkt, valdes. Efter detta utvecklades konceptet vidare och funktionella samt icke-funktionella prototyper tillverkades. Dessa prototyper testades i en tredje testomgång, där de icke-funktionella prototyperna testades för enbart intuition av godtyckliga kvinnor, på samma sätt som tidigare. De funktionella prototyperna testades av patienter och sköterskor på fyra sjukhus i Nederländerna, likt första testomgången. De övergripande resultaten var positiva och produkten var signifikant förbättrad, mycket tack vare att den inte behövde sättas ihop innan den kunde användas. Den största negativa aspekten var att det saknades en indikator som underlättade öppnandet av produkten.

<p>Objective: The aim of this study was to evaluate a new underfill-function in a POCT-instrument from HemoCue AB (Ängelholm, Sweden). The instrument is in use today among diabetes patients for self-monitoring blood glucose (SMBG). The new function is supposed to guarantee that measuring only will be performed on a sufficient sample volume to assure that the correct glucose value is received.</p><p>Methods and results: Blood samples (whole blood) from 12 patients were analysed with the instrument. Measuring were performed using different volumes in the cuvette. Full cuvette, 3µL, 2µL, 1µL and a measuring on an empty cuvette. The instrument performed measurements on all volumes added to the cuvette except for the empty cuvette. The less sample volume that was used the lower glucose values were reported by the instrument.</p><p>Conclusions: The new under fill-function did not work satisfactory. If such function would be more reliable it would be beneficial for the patient controlling hers/his bloodglucose provided that the testing procedure is being correctly done. This is very important because the results are often used to treat the patient.</p>

Since 2002, the Canadian state has mandatorily tested applicants for permanent residence for HIV (Human immune deficiency virus). The policy and practices associated with this screening have never been critically scrutinized. Authoritative claims about what happens in the conduct of the immigration medical examination are at odds with the experience of immigrant applicants living with HIV. This is the analytic entry point into this inquiry that is organized within the theoretical and methodological frame offered by institutional ethnography and political activist ethnography. Analysis is connected to broader research literatures and the historical record. The goal of this study is to produce detailed, contextualized understandings of the social and ruling relations that organize the lives of immigrants to Canada living with HIV. These are generated from the material conditions of their lives. An assumption about how organization happens is the social and reflexive production of knowledge in people’s day-to-day lives through which connections between local and extra-local settings are empirically investigable. I investigate the organization of the Canadian immigration process. How is this institutional complex ordered and governed? How is immigration mandatory HIV testing organized, and with what consequences to HIV-positive applicants to Canada? This is a text-mediated organization where all the sites are connected by people’s work and the texts they circulate. The positive result of an immigration HIV test catalyzes the state’s collection of medical data about an applicant. These are entered into state decision-making about the person’s in/admissibility to Canada. I focus on a key component of the immigration process, which is medical examination and HIV testing with this, along with the HIV test counselling practices that happen (or not) there. The reported absence of the latter form of care causes problems and contradictions for people. This investigation adopts the standpoint of these persons to investigate their problems associated with HIV testing. The main empirically supported argument I make is that the Canadian state’s ideological work related to the HIV policy and mandatory screening ushers in a set of institutional practices that are highly problematic for immigrants with HIV. This argument relies on data collected in interviews, focus groups, observations, and analysis of texts organized under Canada’s Immigration and Refugee Protection Act (S.C., 2001, c. 27) and textually mediated, discursively organized concepts that shape people’s practice. Canadian immigration medical policy makers should make use of these findings, as should civil society activists acting on behalf of immigrants to Canada living with HIV. I make nine specific recommendations for future action on HIV and immigration in Canada.

The Study of Medical Testing for Cataract Surgery was a randomized clinical trial in patients scheduled to undergo elective cataract surgery to determine whether routine medical testing before cataract surgery reduced the rate of complications during the perioperative period. The study showed that routine preoperative medical testing before cataract surgery does not reduce the risk of intraoperative or postoperative adverse medical events when compared to selective or no testing, nor does it reduce the rate of ocular surgical complications (such as posterior capsule rupture) or influence visual acuity outcome. The authors recommended that preoperative clinical assessment by a physician at least 7 days before surgery is a more logical and efficient approach. Tests should be ordered only when indicated by the history or findings on physical examination.

In this era of high technology and expensive testing, there is nothing more important than a good history and physical examination in the process of medical care. A focused cardiac history allows the clinician to establish a differential diagnosis. The physical examination is utilized not only as an overall assessment of the cardiovascular system but more specifically to pursue those diagnoses in the historical differential that can be confirmed or refuted by the physical findings. Critical physical examination findings are rarely uncovered unless the clinician is specifically probing for given physical examination characteristics and knows what would be expected in the face of disease. Only by applying skilled history and physical examination techniques can the clinician determine the rank order of the differential diagnosis and order those tests most likely to answer the question with the least inconvenience, pain, and cost to the patient.

This chapter presents two practice tests with a mix of question types (e.g. multiple choice or ranking), content (e.g. domain- tested), and styles (e.g. patient, colleague, or personal). Each includes 30 questions and broadly reflects the type of questions likely to be asked in the SJT. To make the most of this test, you should complete it in one sitting within an hour before checking your answers. When checking your answers to ranking questions, remember that credit is still given for ‘near misses’ and so there is no need to hit the ‘correct’ sequence every time. The practice test answers are not accompanied by detailed explanations. For this reason, it would be preferable to complete all the questions in Section 2 before attempting the test. To replicate the SJT as closely as possible, you should ideally complete these questions within an hour under formal examination conditions. Once you have attempted all the questions, turn to to check your answers. It is difficult to interpret your final score, as your rank will depend entirely on how well your colleagues (and every other medical student in the country) fare. If you are organized, you could arrange a study group to work through this book and/ or complete the practice test. Marking your answers as a group will give some indication as to your performance relative to others. It will also provide an opportunity to discuss the various options (including disagreement with our answers) and so gain a deeper understanding of the issues tested by the SJT.

This book has a combined focus on two neurodegenerative conditions: dementia with Lewy bodies and Parkinson’s disease with dementia. While patients with either disorder experience quite variable problems, these two disorders have striking similarities when viewed in the aggregate. Thus, the symptoms of these two conditions are much the same, and so are the treatment strategies. Before addressing treatment, it is crucial to define the relevant terms, broaden our understanding, and discuss how these diagnoses are made. We will start with some basics. These disorders typically start in middle age and later, where selected brain circuits deteriorate for unknown reasons. Common neurodegenerative conditions include Parkinson’s disease, Alzheimer’s disease, and amyotrophic lateral sclerosis (ALS; Lou Gehrig’s disease). Such conditions involve limited regions of the brain or spinal cord, slowly progressing and leading to disability. Each is clinically identified by the specific neurologic deficits unique to that condition. Why each affects certain brain regions, sparing others, is a crucial but unanswered question. Although much has been learned about degenerative syndromes, we do not know the causes of any of them. Dementia implies a loss of intellectual abilities sufficient to compromise activities of daily living. Most often the term dementia is used in the context of neurodegenerative disorders. Mild thinking and memory problems that do not substantially interfere with daily routines fall into the category of mild cognitive impairment (MCI; see below). Doctors diagnosing dementia rely on the history from the patient and family, plus cognitive tests. Short tests assessing memory, attention, and calculation, among other things, can be done in the doctor’s office. Such tests include the so-called Mini-Mental State Examination and the Short Test of Mental Status. More refined and informative tests, termed psychometric testing, are done under the auspices of psychologists; these typically require 2 to 4 hours. Clinicians addressing dementia must also look for treatable causes before concluding that the problem is a neurodegenerative dementia. This assessment typically includes a brain scan, blood tests, and a review of the patient’s medical history and medication list, which may indicate the need for additional testing.

The problem of determining the physical fitness of 1st year students and identifying their risk of developing cardiovascular disease in the first stage of physical education before the students’ medical examination is considered. During the analysis of literature sources, test tasks were selected in order to investigate and assess the level of physical fitness of students according to the methodology proposed by the Jyvaskyla University (Finland) and the risk of cardiovascular disease according to A. Z. Zapesochny’s methodology. 670 first-year students participated in the study. The assignment to evaluate fitness levels consisted of recording the walking time at a maximal pace over a 2 km distance and determining the post-exercise heart rate. The evaluation of the results was performed against the fitness level index. When testing according to Zapesochny’s methodology, the following indicators characterising risk factors for cardiovascular disease were considered: body weight and length, gender, blood pressure, hereditary factors, bad habits and physical activity. All the results, which were evaluated in scores, were combined to calculate the cardiovascular disease development risk level. An analysis of the results showed that 32.24% of first year students had a high level of physical fitness and only 8.21% had a low level. In 48.07% of the tested persons, the risk of cardiovascular diseases was absent. The explicit and maximum risk observed in 2.98% of the tested persons. This data helped in the first stage of physical education to select an appropriate physical activity for the students without risk of harm to their health prior to undergoing medical check-up.

Shape recovery from memory by porous scaffolds for tissue engineering offers easier insertion and self-retention following placement by minimally invasive surgery. Shape memory testing of porous polymer xerogels focuses on the compression cycle and the special aspects of the cycle and equipment used. This contrasts with normal tensile shape memory (SM) testing. In this work a dynamic mechanical analyzer (DMA) was used on small samples to quickly yield measurement of the SM restoration at various stress levels to emulate the forces exerted on the body by a tissue engineering (TE) scaffold returning to its permanent shape. The DMA testing of a hexamethyl diisocyanate trimer crosslinked castor oil (CO) / polycaprolactone (PCL) blend yielded repeated SM with no creep. The porous CO/PCL showed repeated compressive SM at 50% strain with a SM stress-free recovery ratio of 100%. The peak SM recovery work of 6.4 KJ/m3 was measured at 0.5 MPa stress and 6% to 12% strain. In addition to the potential utility of these materials in a tissue engineering setting, the test methods described here are relevant to a broad range of shape memory applications, from medical devices to morphing airframes to self-deploying structures.

Most of the active research in reducing Parkinsonian tremor involves invasive surgeries or medical treatment. In this paper hand tremors associated with Parkinson’s Disease (PD) are studied and passive vibration control methods are developed and tested. Patients with PD are surveyed regarding difficulties with hand tremor during the act of eating. The result leads to design criteria for an enhanced eating utensil and the establishment of meaningful testing methods for measuring hand tremor. Tremor data collected from several PD patients provides insight into the nature of the motion and allows for the development of test fixture and prototypes. This experimental data is coupled with linear model identification testing for the free response of a “healthy” hand undergoing the same motions. The resulting differential equation model, where the system input is realized as actuation through the biomechanics of the forearm and wrist, is used in the design of an eating utensil for vibration reduction. With self-excitation and the existence of harmonics, the tremor data is also used to develop a nonlinear differential equation model, where the complete neurological/mechanical system is realized with an equivalent mechanical system. This nonlinear model is shown to mimic the tremor data and is used to enhance the development of the vibration absorber. A prototype of the vibration absorber is built, validated on the test fixture, and tremor reduction data is collected again with PD patients.

A new system of dew point temperature hygrometer, based on semiconductor MEMS detector is presented in the paper. Many details of MEMS detector construction are given with details in the report. Basic idea of algorithms of detector control is also discussed. More attention is devoted to subalgorithm for self-adjusted detector threshold operation. Excellent dynamic parameters of the new hygrometer (i.e. 2÷5 dew point detections and temperature measurements per second), proved by the hygrometer tests, will be presented and described in the report. It means that presented hygrometer is 10÷100 times faster than conventional hygrometers. In the end of the paper two medical applications are demonstrated with clinical test results. The first application is for dermatology for TransEpidermal Water Loss (TEWL) factor of human skin. The second is focused on measurement of humidity in human nose cavity and human throat during breathing. In both case fast humidity measurements, with time constant of some 0.5s, have been required.

Abstract:
The aim of this work is to present the state of progress and the main features of the TECO (TEst of COmpetence) project, promoted by the (Italian) National Agency for the Evaluation of Universities and Research Institutes with the aim to assess students’ learning outcomes in the higher education contest. The results of the 2017-2018 TECO survey (which involved voluntarily over 12,500 nursing, physiotherapy and medical radiology students), showed that attending a university programme makes a difference in development of some competencies, especially for the disciplinary sphere. The analysis will focus on the connections between the characteristics of the students at the beginning of their study career and the achieved level of tested competences. The return of the test results to the students and study programmes’ coordinators will be presented and discussed, considering their possible uses in a self-assessment perspective. TECO results and the new indicators that will be originated from this project could prove to be reliable tools for university programmes self-assessment, encouraging the use of evidence-based strategies for higher education improvement.

Abstract:
Based on the bionic artificial joint system, new therapeutic bionic lubricants not only for artificial joints lubrication but also for the treatment of particle-induced osteolysis are developed and investigated. In this study, their tribological characteristics and mechanisms are studied. The tribological experimental results show that the therapeutic lubricants have the best anti-wear performance among the lubricants tested in the experiments, such as hyaluronic acid (HA), bovine serum et al. It is found that there is some membrane self-assembled on the surface of the cup which may give a likely answer to the high lubricating performance of the therapeutic lubricants. The calculated film thickness of therapeutic lubricants is about 0.310 μm. The result indicates it is still in the boundary lubrication state. The preliminary biological experiments show that therapeutic lubricants have good biocompatibility. More tests are needed to clarify the form of the membrane and the medical performance of the therapeutic bionic lubricants towards the undesired reactions induced by the wear debris.

Pebble Bed HTGR’s like the AVR in Ju¨lich have the advantage of continuous fuelling. However the multiple passes of the fuel pebbles through the core have the disadvantage that the pebble’s movement through the fuelling system and the core produces graphite dust. This dust is transported from the core to other parts of the primary circuit and deposits on components. Although previous experiments performed during AVR operation have given some insight into the dust particle size and activity, there is little information on the behaviour of the dust that was deposited in the system. The decommissioning of the AVR has provided the opportunity to sample and characterise such dust from a number of components and gauge the adhesion strength. From the side of PBMR Pty Ltd this opportunity is considered important to enhance the knowledge about dust characteristics before the PBMR Demonstration Power Plant (DPP) is operational and able to produce specific plant information through sampling and analysis. AVR GmbH has provided a number of pipes and joints for investigation of loose and bound dust. Phase 1 of the analysis was used to determine the best techniques to be used on larger items. No measurable loose dust could be collected. Thereupon rings were cut from a T-section and subdivided into eight segments. The surface of the untreated segments were photographed and documented by optical microscopy, the dose rates were measured and gamma-spectrometry performed. Following this a mechanical or chemical decontamination was carried out to remove and isolate the bound dust. The average isolated dust amount was about 2 mg/cm2. Both decontamination processes indicates a strong bonding of the dust surface layer. In the case of mechanical decontamination about 60% and by chemical decontamination about 95% of the radionuclide inventory could be removed. The contribution of removed metal needs to be investigated in more detail. The median number related particle size measured by optical microscopy was found to be in the range of 0.2 to 0.7 μm whereas the median weight related size is in the range of 0.8 to 1.5 μm. The initial results indicate that this dust sticks very strongly to the pipe surface. Phase 2 will concentrate on longer pieces of piping where hopefully more loose dust can be obtained and analysed. If the same strong bonding is observed the reason for this phenomenon needs to be explained and perhaps tested with non-active dust.

Abstract:
The article clarifies of gender identity stereotypes in modern media. The main gender stereotypes covered in modern mass media are analyzed and refuted. The model of gender relations in the media is reflected mainly in the stereotypical images of men and woman. The features of the use of gender concepts in modern periodicals for women and men were determined. The most frequently used derivatives of these macroconcepts were identified and analyzed in detail. It has been found that publications for women and men are full of various gender concepts that are used in different contexts. Ingeneral, theanalysisofthe concept-maximums and concept-minimum gender and their characteristics is carried out in the context of gender stereotypes that have been forme dand function in the society, system atizing the a ctual presentations. The study of the gender concept is relevant because it reveals new trends and features of modern gender images. Taking into account the special features of gender-labeled periodicals in general and the practical absence of comprehensive scientific studies of the gender concept in particular, there is a need to supplement Ukrainian science with this topic. Gender psychology, which is served by methods of various sciences, primarily sociological, pedagogical, linguistic, psychological, socio-psychological. Let us pay attention to linguistic and psycholinguistic methods in gender studies. Linguistic methods complement intelligence research tasks, associated with speech, word and text. Psycholinguistic methods used in gender psychology (semantic differential, semantic integral, semantic analysis of words and texts), aimed at studying speech messages, specific mechanisms of origin and perception, functions of speech activity in society, studying the relationship between speech messages and gender properties participants in the communication, to analyze the linguistic development in connection with the general development of the individual. Nowhere in gender practice there is the whole arsenal of psychological methods that allow you to explore psychological peculiarities of a person like observation, experiments, questionnaires, interviews, testing, modeling, etc. The methods of psychological self-diagnostics include: the gender aspect of the own socio-psychological portrait, a gender biography as a variant of the biographical method, aimed at the reconstruction of individual social experience. In the process of writing a gender autobiography, a person can understand the characteristics of his gender identity, as well as ways and means of their formation. Socio-psychological methods of studying gender include the study of socially constructed women’s and men’s roles, relationships and identities, sexual characteristics, psychological characteristics, etc. The use of gender indicators and gender approaches as a means of socio-psychological and sociological analysis broadens the subject boundaries of these disciplines and makes them the subject of study within these disciplines. And also, in the article a combination of concrete-historical, structural-typological, system-functional methods is implemented. Descriptive and comparative methods, method of typology, modeling are used. Also used is a method of content analysis for the study of gender content of modern gender-stamped journals. It was he who allowed quantitatively to identify and explore the features of the gender concept in the pages of periodicals for women and men. A combination of historical, structural-typological, system-functional methods is also implemented in the article. Descriptive and comparative methods, method of typology, modeling are used. A method of content analysis for the study of gender content of modern gender-labeled journals is also used. It allowed to identify and explore the features of the gender concept quantitatively in the periodicals for women and men. The conceptual perception and interpretation of the gender concept «woman», which is highlighted in the modern gender-labeled press in Ukraine, requires the elaboration of the polyfunctionality of gender interpretations, the comprehension of the metaphorical perception of this image and its role and purpose in society. A gendered approach to researching the gender content of contemporary periodicals for women and men. Conceptual analysis of contemporary gender-stamped publications within the gender conceptual sphere allows to identify and correlate the meta-gender and gender concepts that appear in society.
