Full textDairy production and beef cattle farms form an important agricultural sector. Forage crops used to feed the cattle represent 40% of all the cultivated land in Quebec. Forage quality has an impact on nutritional disorders such as hypomagnesemia and hypocalcemia of lactating cows. The influence of climatic, soil, management and fertilization factors on the yield and quality of hay can be described by linear and mixed models (Mitscherlich); this model allows to account for fixed effects that affect all samples in the same way, and random effects, specific to each sample. The final goal is to develop a forage yield response and hypomagnesemia risk model for environmental and management factors. The model relates the yield, the mineral and textural analysis of the plots where the forage crops were grown, the climatic data collected during the tests, such as the degree-days, the accumulation and diversity of the precipitations, the nitrogen, phosphorus and potassium fertilization, and the forage mixture. The factors that most significantly affect forage yields are nitrogen and phosphate fertilization, an increase in degree-days, increased precipitation, a decrease in clay content, and a decrease in the balance [ Legumes

Full textAlterations of the facial and/or cervical muscles are a few of the symptoms found in patients with tetanus. Speech-language intervention, however, as a field of knowledge that can contribute for the reestablishment of this type of patient, is not mentioned in the consulted literature. The purpose of this dissertation was to characterize the muscle activity and the orofacial functions in patients, hospitalized at an Intensive Care Unit, with the clinical diagnosis of tetanus. Participants of this study were 13 patients hospitalized in the Intensive Care Unit of the Department of Infectious and Parasitic Diseases of the Clinical Hospital of the School of Medicine of the University of São Paulo with the diagnosis of tetanus, in the period of December 2005 and May 2007. The ages of the patients varied between 36 and 85 years, mean age of 53.69 years, being one female and twelve males. Patients were referred to a speech-language assessment during the phase of recovery. For this assessment, information contained in the medical charts, as well as a specific protocol for the assessment of dysphagia were used. This protocol is divided in two sections - examination of the oral-motor functions and clinical swallowing tests. This dissertation was divided in two studies. The first study had as a purpose to characterize the muscle activity and the orofacial functions, and to identify the clinical factors and/or swallowing tests that could be more related to this pathology. According to the results, most of the evaluated aspects were considered primarily altered. The clinical factor GAG Reflex and the elevation of the larynx during the clinical test of water were considered altered in all of the participants who could be assessed. The clinical factors of tongue movement and/or vocal changes were the aspects that presented correlation when comparing both stages of assessment. The second study had as a purpose to characterize, in clinical terms, the muscle activity and the orofacial functions and to determine aspects and/or alterations that could be considered common to this group of patients. The focus of tetanus which prevailed was that resulting from external and traumatic factors, the incubation period had as an average 7 days and the average of progression was of 1 day. All patients were intubated, used tracheostomy, used forms of sedation, used alternative forms of feeding and used mechanical ventilation. The clinical factors of tongue mobility, vocal quality and speech motor functions presented some type of alteration. The results of the study gave evidence to the orofacial myofuntional alterations already pointed in the literature as being related to patients with tetanus, as well as pointing new muscle and functional characterizations, pertinent to the Stomatognathic System, done by professionals with expertise in the field. The study allowed a better and greater possibility of direction in the assessment and treatment of this xiv group of patients, presenting a material that is associated to the symptoms and clinical signs through the process of standardized clinical evaluation.

Full textAccidental tetanus is a millenary disease of high lethal indices, even when proven and effective, low cost prophylactics are available. Several studies have tried to identify the physiopathological process which releases the succession of clinical manifestations of the disease. The objective of these studies is to recognize and make available a new strategy for the treatment of tetanus after exposition to the toxin. The diagnosis of tetanus is eminently clinical. Several factors may occur during the evolution of the disease which complicate its prognosis. The most frequent complications are infections and autonomic dysfunction. Several therapeutic options exist. However, they have a supportive and symptomatic role. This study has as its objective to show the complexity which involves the physiopathological process and treatment of accidental tetanus as well as to emphasize that the only effective way of reducing tetanic mortality is, doubtless, a better vaccine coverage of susceptible individuals. NOTE: Essay with review articles. This is the sum of the first.
O Tétano acidental é uma doença milenar, que mesmo com a disponibilidade de profilaxia de comprovada eficácia e baixo custo, mantém alta taxa demorbiletalidade. Vários estudos vêm tentando identificar em nível biomolecular, o processo fisiopatológico que desencadeia as manifestações clínicas da doença, com o objetivo de conhecer e disponibilizar uma nova estratégia para o tratamento do tétano, após a exposição à toxina e sua fixação nos neurotransmissores. O diagnostico do tétano é eminentemente clínico. Podem ocorrer várias complicações durante a evolução da doença que comprometem o seu prognóstico. As complicações mais freqüentes e de maior gravidade são as infecciosas e a disautonomia simpática. Existem várias opções para o tratamento do tétano, todas porém de caráter suportivo e sintomático. Este estudo tem como objetivos: ressaltar a complexidade do processo fisiopatológico e do tratamento do tétano acidental e enfatizar como única forma efetiva de se reduzir a morbiletalidade do tétano, a cobertura vacinal das populações susceptíveis, constituída basicamente por adultos e idosos. OBS: Dissertação com artigos de revisão. Este é o resumo do primeiro.
