Since 1950, detailed records of all climbing accidents in Grand Teton National Park were maintained by rangers (and others) involved in rescue. In this paper, we present: 1) an overall summary of backcountry accidents, their locations, and causes since 1950; 2) several summaries of the accidents by decades, locations and activity levels; 3) a fatality synopsis; and 4) an in-depth overview of the current accident trend from 1994 through 1996, which includes a male and female accident profile, cause and location summary, cost analysis, and other important information. From 1950 through 1996, 609 significant backcountry accidents have occurred, resulting in an average of about 13 incidents per year. The two categories involving the greatest number of accidents include Fall on Rocks (195) and Fall on Snow (155). Most accidents occurred during the 1970's, and are currently on the decline. Accidents caused by Rockfall or Icefall were most prevalent in the 1960's, indicating that perhaps many of the popular routes were "cleaned" during this time period. From 1950 through the 1970's, the accident rate, when scaled for activity level during the decades, increased. Since the 1970's, the rate has decreased to a low in the 1990's (0.22%). The overall scaled accident rate for the period from 1950 through 1996 was 0.31%. The vast majority of climbing accidents occur in the Central and Northern climbing areas. In the Central Climbing area, the Grand Teton has the greatest number of incidents (121), followed by Disappointment Peak (39), Middle Teton (38), and Teewinot Mountain (32). In the Northern Climbing area, Symmetry Spire exhibits the greatest number of accidents (39). When examined by decades after being scaled for decade activity, a decline in the number of accident incidents for the 1990's is displayed in nearly all locations. One exception is Albright Peak in the Southern Climbing area, which shows a steady increase in incidents over the decades. Although the Grand Teton has experienced the greatest number of climbing accidents, the relative number of incidents is low when scaled for climbing activity. Interestingly, Storm Point and Symmetry Spire in the Northern Climbing area actually exhibit the greatest number of incidents when backcountry activity is taken into account. Cascade Canyon receives the greatest number of non-climbing incidents, both in actual numbers and when scaled for backcountry activity. Most fatalities in the Tetons result from unroped falls from rock that occur while ascending and descending routes. The current accident trend (1994-1996) indicates that falls on snow are the most prevalent cause of accidents, followed by falls on rock. The Grand Teton and Middle Teton experience the greatest number of climbing accidents, but when scaled for climbing activity, Storm Point and Mt. Owen experience the greatest number of accidents. Cascade Canyon has the greatest number of non­climbing accidents, but when scaled for backcountry activity, Garnet Canyon demonstrates the greatest number of accidents. Most women who experience accidents in the Tetons suffer from falls on snow, have an average age of 29.7 years, and almost always fall during the descent. Most men who experience accidents in the Tetons suffer from falls on snow and have an average age of 35.7 years. Fifty-seven percent of men who experience accidents on climbing trips fall during the descent. Two accident situations involving only men were getting stuck and falling during rappel. Most accidents are currently reported by people from other parties; however, 25% of the reports come from backcountty rangers in the field. The use of cell phones to report accidents has grown dramatically. Recently, there has been an average of 119 incidents per year involving some form of rescue response, at an average annual cost of $73,215.21.

Abstract:
The DENND1A locus is associated with polycystic ovary syndrome (PCOS), a disorder characterized by androgen excess. Theca cells from ovaries of PCOS women have elevated levels of a DENND1A splice variant (DENND1A.V2). Forced expression of this variant in normal theca cells increases androgen biosynthesis and CYP17A1 expression, whereas knockdown of the transcript in PCOS theca cells reduced androgen production and CYP17A1 mRNA. We attempted to create a murine model of PCOS by expressing hDENND1A.V2 using standard transgenic approaches. There is no DENND1A.V2 protein equivalent in mice, and the murine Dennd1a gene is essential for viability since Dennd1a knockout mice are embryonically lethal, suggesting that Dennd1a is developmentally critical. Three different hDENND1A.V2 transgenic mice lines were created using CMV, Lhcgr, and TetOn promoters. The hDENND1A.V2 mice expressed hDENND1A.V2 transcripts. While hDENND1A.V2 protein was not detectable by Western blot analyses, appropriate hDENND1A.V2 immunohistochemical staining was observed. Corresponding Cyp17a1 mRNA levels were elevated in ovaries and adrenals of CMV transgenic mice, as were plasma steroid production by theca interstitial cells isolated from transgenic ovaries. Even though the impact of robust hDENND1A.V2 expression could not be characterized, our findings are consistent with the notion that elevated hDENND1A.V2 has a role in the hyperandrogenemia of PCOS.

Abstract:
This qualitative research focused on the linguistic variations of the matrilineal society of Tetun ethnic background in Belu, East Nusa Tenggara. The purpose of this study is to describe differences in the use of prosodic features, lexicons, and imperative sentences between men and women. The informants were determined by using a snowball sampling technique. Data were collected by means of nonparticipatory method through recording technique and interviews. The result of this study indicates that there are differences in the linguistic variations between men and women. In terms of prosodic features, women tend to lengthen a vowel sound at the end of a word. In the lexicon level, the word he’e ‘yes’ is used by men and women in formal and informal situation whereas hi’i ‘yes’ is used only by women in informal situation. In addition, there is a difference in the use of address terms. Women use mane and bo’o for boys while men use bete and bu’i for girls in addressing their children. In the syntactic level, men use more varied types of imperative sentences. The data show that although women occupy a superordinate position in the society, they tend to use more polite imperative utterances.

KTH Agro Sumber Rejeki is one of the forest farmer groups located in the Register 40 production forest area in South Lampung Regency. This community service in the context of diversifying non-timber forest products (NTFPs), namely citronella oil. The purpose of this community service activity is to provide knowledge, technical production skills and quality requirements for citronella massage oil and telon oil, through empowering women from KTH Agro Sumber Rejeki. The method used in this PKM activity consisting of several stages, first technical guidance for making telon citronella oil and citronella sequence oil with 3 variants: natural, lemon and rose as well as counseling on herbal medicine quality requirements The effectiveness of PKM activities is evaluated by carrying out a pretest and postest. The average postest score was 83.53, an increase of 49.41 points from the pretest score with a total score of 100. The evaluation results showed that there was an increase in knowledge and technical skills in making telon citronella oil and citronella massage oil and also understanding the quality requirements of herbal medicines in women of KTH Agro Sumber Rejeki. . Abstract:
Napan village is located in the Indonesia-Timor Leste border region whose people speak in Dawan and Indonesian. The problem in this studyare the accommodation of Dawan speakers, what is the direction of their accommodation between languages, and how do speakers’ who accommodate at the syntactic level based on gender and age groupThis study aims to identify the level of syntax used in language accommodation and its frequency based on the characteristics of respondents in Napan Village, the Indonesia-Timor Leste border region, East Nusa Tenggara. The method of the study is the survey method with 72 respondents of Dawan speakers which are divided into 36 men and 36 women as the samples. The results show that speakers of Dawan accommodate the language because of the security, familiarity, and growing trust within the speech participants. Speakers of Dawan are very positive about their language, positive about Indonesian language and quite positive about the languages of the neighboring country, namely Tetun Portu or Tetun Dili. Speakers of the Dawan language accommodate the Tetun Portu or Tetun Dili language at the lexical level, phrases, sentences, and expressions. Based on gender, female speakers of Dawan accommodate the Tetun Portu or Tetun Dili language (9.7%) more than male speakers (8.3%). Meanwhile, male speakers of Dawan (41.7%) are less accommodative to Tetun Portu or Tetun Dili than female speakers (40.3%). Based on age, speakers of Dawan who accommodate the Tetun Portu or Tetun Dili languages dominantly come from the ages of 26--50 years old (8.3%) compared to ages less than 25 years old (5.5%) and more than 51 years old (4, 2%). In addition, speakers of Dawan who do not accommodate the Tetun Portu or Tetun Dili languages are speakers older than 51 years old (29.2%) more dominant than those aged less than 25 years (27.8%) and those aged between 26-50 years (25%). ABSTRAKDesa Napan terletak di wilayah perbatasan Indonesia-Timor Leste yang masyarakatnya bertutur dalam bahasa Dawan dan bahasa Indonesia. Permasalahan dalam penelitian ini ialah mengapa penutur bahasa Dawan berakomodasi, bagaimana arah akomodasi antarbahasa, dan bagaimana penutur yang berakomodasi pada tataran sintaksis berdasarkan jenis kelamin dan kelompok usia? Penelitian ini bertujuan untuk mengidentifikasi tataran sintaksis yang digunakan dalam akomodasi bahasa dan frekuensinya berdasarkan karakteristik responden di Desa Napan, wilayah perbatasan Indonesia-Timor Leste, Nusa Tenggara Timur. Dalam penelitian ini digunakan metode survei. Sampel dalam penelitian ini adalah 72 responden penutur bahasa Dawan, yang terdiri atas 36 pria dan 36 wanita. Hasil penelitian menunjukkan bahwa penutur bahasa Dawan di Desa Napan berakomodasi karena kenyamanan, keakraban, dan untuk menumbuhkan rasa percaya mitra tutur. Penutur bahasa Dawan sangat positif terhadap daerahnya (bahasa Dawan), positif terhadap bahasa Indonesia, dan cukup positif terhadap bahasa negara tetangga, yaitu bahasa Tetun Portu atau Tetun Dili. Penutur bahasa Dawan berakomodasi terhadap bahasa Tetun Portu atau Tetun Dili pada tataran leksikal, frasa, kalimat, dan ungkapan. Berdasarkan jenis kelamin, penutur perempuan bahasa Dawan yang berakomodasi terhadap bahasa Tetun Portu atau Tetun Dili (9,7%) lebih dominan daripada laki-laki (8,3%). Sementara itu, penutur laki-laki bahasa Dawan yang tidak berakomodasi terhadap bahasa Tetun Portu atau Tetun Dili (41,7%) lebih dominan daripada perempuan (40,3%). Berdasarkan kelompok usia, penutur bahasa Dawan yang berakomodasi terhadap bahasa Tetun Portu atau Tetun Dili tampak dominan yang berusia 26—50 tahun (8,3%) dibandingkan dengan yang berusia <25 tahun (5,5%) dan > 51 tahun (4,2%). Di samping itu, penutur bahasa Dawan yang tidak berakomodasi bahasa adalah penutur yang berusia >51 tahun (29,2%) lebih dominan daripada yang berusia <25 tahun (27,8%) dan yang berusia 26—50 tahun (25%).

This 75-year-old woman had phacomorphic angle closure, dense nuclear sclerosis, deep set eye, miotic pupil and tight corneal wound during phacoemulsification. Phacoemulsification wound burn was noted at the end of surgery. Tenon was harvested from the inferior conjunctiva, placed over the gape and anchored by two radial corneoscleral 10–0 nylon. Ten days later, anterior optical coherence tomography showed good wound apposition and sutures were removed with visual recovery to 20/25 (6/7.5) without astigmatism.

Abstract:
Group B streptococcus (GBS) is a commensal bacterium of the human gastrointestinal and genital tracts. It is a leading cause of neonatal sepsis and meningitis, and has also been recognized as an important pathogen in pregnant women and the elderly. We investigated mechanisms of macrolide and tetracycline resistance in GBS colonizing women in Egypt. A total of 100 isolates were screened using standard antibiotic susceptibility tests. A multiplex PCR assay was used to detect macrolide- and tetracycline-resistance determinants. All isolates were uniformly susceptible to penicillin G, ampicillin, cefotaxime, vancomycin and levofloxacin. The resistance rates to erythromycin, clindamycin, azithromycin, tetracycline and chloramphenicol were 17, 14, 16, 98 and 1 %, respectively. Among the erythromycin-resistant isolates, 82.4 % had constitutive macrolide–lincosamide–streptogramin B (cMLSB) resistance, 5.9 % had inducible MLSB (iMLSB) resistance and 11.8 % had M phenotype resistance. Among the cMLSB phenotypes, 64.3 % of isolates harboured the ermB gene and 35.7 % of isolates harboured none of the investigated macrolide-resistance genes. The single strain expressing the iMLSB phenotype possessed the ermA gene. Of the two strains with the M phenotype, only one possessed the mefA/E gene. Conversely, seven macrolide-sensitive strains (MIC <0.03 µg ml−1) were ermB positive and one macrolide-sensitive strain (MIC <0.03 µg ml−1) harboured mefA/E. Tetracycline resistance was predominantly due to tetM, which was detected alone (83.7 %) or in association with tetL (12.2 %), tetK (1 %) or tetO (1 %). One strain carried tetM associated with both tetL and tetK, and another strain carried tetO alone. The tetO strains were positive for the mefA/E gene, and the tetL and tetK carrier strains harboured the ermB gene. Susceptible strains harbouring but not expressing an antibiotic-resistance gene should be regarded as potentially resistant. These results emphasize the need to monitor the epidemiology of GBS antibiotic resistance in Egypt.

ABSTRACTBackground: Leucorrhea is a problem that is often experienced by women. Female adolescents have inadequate knowledge about handling and preventing leucorrhea. This inadequate knowledge has an impact on the perception that leucorrhea will not affect their health. But in reality, for the last 3 (three) years from 2014 to 2016, there was an increase in complaints according to the screening results by Puskesmas Temon 2 (Community Health Center). Because of this reason, interventions are needed regarding the knowledge about leucorrhea prevention applying the Health Belief Model (HBM) theory, including perceptions of seriousness, vulnerability, benefits, obstacles, and impulse to act that can be measured by a perceptual score.Objectives: This study aimed to determine the effect of reproductive health counseling on the perceptual score of seriousness, vulnerability, benefits, obstacles, and impulse to act involving female students’ perceptions in leucorrhea prevention behavior of SMAN 1 Temon (Senior High School) before and after health counseling. The score was measured by the HBM.Methods: The research was a quasi-experimental study with a time series design. The population of this research was the female students of class X and XI SMAN 1 Temon. The sample consisted of 55 female students who had fulfilled the inclusion and exclusion criteria by using simple random sampling technique. Data were analyzed using eepeated ANOVA test with significance level (α) of 0.05.Results: The results of this study indicated that there were significant differences in perception scores (p = 0.000) before counseling and after counseling in perceptions of seriousness in prevention, vulnerability to leucorrhea, benefits in prevention, barriers to preventive action, encouragement to act and total HBM score. Conclusions: Reproductive health counseling about leucorrhea based on the Health Belief Model theory is effective to improve female adolescent students' perception in taking action to prevent leucorrhea, especially in perception scores of seriousness, vulnerability, benefits, obstacles, and impulse to act in leucorrhea prevention behavior of female students in SMAN 1 Temon.

In 2018, KPAI noted that 84% of students in Indonesia had experienced violence at school. The research conducted by Jannah (2018) on early adolescents at Surabaya A Junior High School revealed that 76% of students had hit, 66% had fought, 46% had damaged items when angry, 90% had been hostile, 84% were angry and indicated adolescents tend to be prone to aggressive behavior. Adolescents who are male have a tendency to behave aggressively higher when compared to women (Santrock, 2007). The development of impulsive aggression involves emotional processes (Teten et al, 2008). Emotional states during adolescence are characterized by unstable and turbulent emotions, so that the mood can change quickly. Teenagers are said to have reached emotional maturity if they do not "blow up" their emotions in front of other people but wait for a more appropriate time and place to express their emotions in ways that are more acceptable. Adolescents whose emotions mature provide stable emotional reactions, do not change from one emotion or mood to another. The difficulty of reacting to emotions is called alexithymia. According to Sadock & Virginia (2010), alexithymia is a person's inability to describe or have difficulty describing or being aware of his emotions or mood. Goleman (2017) states that emotional blindness will cause several disadvantages including aggressive behavior. Research shows that high alexithymia levels are associated with high aggressive behavior and impulsive behavior (Teten et al, 2008). Salminen et al. (1998) stated that men were more likely to be not expressive in conveying emotions so that the possibility of alexithymia occurring in men was more than women. Based on the theoretical study above, there is an association between alexithymia and aggressive behavior in male adolescents.

Este trabalho é uma análise do conto “O papel de parede amarelo”, de Charlotte Perkins Gilman. Sua leitura contemporânea, sob ótica feminista, possibilita revisitar esse clássico a partir de questões sociais contemporâneas. Dessa forma, o objetivo é estudar a obra a partir de seus diferentes aspectos e significados ligados ao gênero e à leitura de autoras mulheres. Para isso, foram utilizados tanto o conto em si quanto a obra teórica “Women and economics” (Gilman, 1998) e “Um teto todo seu” (Woolf, 1990). Esses textos possibilitam pensar a ficção a partir de seu aspecto de crítica social, mas também realizar uma crítica da forma como a ficção produzida por mulheres é comumente recebida. O trabalho trata-se de uma tentativa de releitura de um clássico da literatura a partir de um viés feminista que busca não menosprezar a obra de sua autora. Para tal, aborda a forma como a noção de racionalidade é perpassada pelos papéis sociais de gênero e instrumento para a opressão.

Abstract:
Purpose. To report the first case of cystoid macular edema (CME) induced by nanoparticle albumin-bound- (nab-) paclitaxel treated with sub-Tenon injections of triamcinolone acetonide (STTA) with detailed long-term follow-up.Case. A 39-year-old Japanese woman with breast cancer presents with decreased vision in both eyes while receiving nab-paclitaxel. Two STTA treatments were administered for persistent CME in her right eye. Central retinal thickness (CRT) of the treated eye decreased after the first STTA, but there was no change after the second STTA. CRT of the other eye and bilateral visual acuity (VA) showed no change after each treatment. However, this patient experienced gradual recovery of visual function after nab-paclitaxel treatment was completed, 3 months after the second STTA. Improvements in VA and CRT did not overlap in time. Moreover, there was a big improvement time lag in VA between the eyes.Conclusion. Cessation of nab-paclitaxel could lead to resolution of CME more than STTA, although STTA had some effect. Since nab-paclitaxel has been recently approved for treating more types of malignancies, the number of the patients with this CME is expected to increase in the near future. Patients and physicians should understand this side effect and prepare for other treatment options.

Muslim women is required to wear hijab as a religion obligation which symbolizes women’s faith to the creator (Quran 24:21). Many types of hijab are available in market, with ready-to-wear (RTW) hijab being the most popular choice of hijab in Malaysia. Limited knowledge on fabric characteristics among consumers has created dissatisfaction in many aspects of hijab performance associated to appearance and comfort. This paper presented the study of hijab appearance performances. Ten commercial fabrics (Cotton, Tetron Cotton, silk, rayon, Crepe back satin, Valentino dull satin, chiffon crepe, French voile, and Koshibo) which are commonly used in hijab making, were investigated. Stiffness, drape, wrinkle aspect, pilling, and snagging of fabric, which are associated with hijab appearance, were examined. A fabric ranking index was then developed to further support the analysis. In order to validate the result, a wear test was conducted where the tested fabrics were develop into hijabs and wear assessment was conducted. Feedback from the respondents were obtained and compared with the fabric ranking index. The results of this study concluded that Koshibo, Valentino Dull Satin, Chiffon crepe and French Voile exhibited the highest performance ranking among all the fabric tested. The study provides the reference to hijab manufacturers and end users in selecting the most suitable fabric for hijab in terms of the respective appearance properties.

Abstract:
Purpose To describe the outcomes of a novel technique for pterygium removal: tailored corneo-conjunctival autograft, which offers the advantages of better postoperative cosmetic outcomes and recurrence prevention. Methods This retrospective interventional noncomparative study included 420 eyes in 370 patients (ages 25-60 years) (127 women, 243 men). We divided the patients into 2 groups. The first group included 348 primary pterygium; the second group included 72 recurrent pterygium. All underwent the same type of corneo-conjunctival autograft surgery from April 2009 to March 2014. The average follow-up period was 12 months. The main outcome measures were to assess pain after surgery, conjunctival healing, cosmetic appearance, and pterygium recurrence at the end of follow-up. Results In both groups, the graft donor site healed promptly without excessive scarring. No patient complained of pain during first week after surgery, except for mild discomfort. No major complications were observed such as infection or graft necrosis. In the first group, 2 patients had graft retraction towards the nasal side and were successfully managed conservatively. One patient had Tenon granuloma, which was removed surgically. Recurrence during the follow-up period was seen in one patient (0.2%). In the second group, no recurrent pterygium cases were observed. Conclusions The proposed type of corneo-conjunctival autograft offers the advantage of better postoperative cosmetic outcomes. This new technique is effective for surgical management of both primary and recurrent pterygium and preventing its recurrences.

As obras cinematográficas são consideradas eficientes para avaliar a sociedade e os ambientes de trabalho, pois refletem a maneira como a sociedade, de modo geral, pensa, age e se comporta sobre determinados assuntos. Diante do papel do cinema de reproduzir e produzir elementos sociais e individuais que envolvem a vida privada e profissional de mulheres que ambicionam ter sucesso nas suas carreiras, o presente artigo possui como objetivo analisar como os filmes retratam os desafios que as mulheres enfrentam para avançar em suas carreiras através das lentes das metáforas da desigualdade de gênero. Os filmes foram acessados por meio do site IMDb (Internet Movie Database), usando os descritores: women, business, career e manager. Considerados filmes produzidos de 1981 a 2019, obteve-se 205 peças cinematográficas.Aplicados os critérios de inclusão e exclusão, restaram 39 filmes para análise. Desse modo, a maneira como as mulheres de negócios são representadas pelo cinema é discutida por meio das metáforas: Teto de Vidro; Penhasco de Vidro; Fenômeno Abelha Rainha; Parede Materna; Segundo Turno; Labirinto; e Tokenismo. A construção das personagens, segundo as obras visitadas nesse artigo, evidenciou singularidades na abordagem na construção das protagonistas. Todos os filmes analisados nessa pesquisa apresentaram em alguma cena, diálogo ou modo como a personagem principal é descrita dentro da narrativa pelo menos uma das metáforas abordadas.

Every household always hopes to have enough food available. While fertile agricultural land is decreasing, food needs increase with increasing population. The use of the yard is expected to help improve food availability for the community.In realizing a climate village tourism village in Pandowoharjo Village, a series of activity programs are designed to be carried out continuously for three years. In Year I of the program, community education activities focused on harvesting and using rainwater for vegetable cultivation in the yard. The design of rainwater harvesting facilities is carried out in the form of IPAH, infiltration wells and dead-end channels (Rorak) in six padukuhan which have been designated as the center of tourism village development namely: Brayut, Temon, Pajangan, Dukuh, Karangtanjung and Plalangan.This method of mentoring the PPDM scheme in Pandowoharjo was carried out in three stages, namely program socialization, training and implementation. The mentoring process is carried out with two approaches, namely theoretical training and skills improvement with practice in the field.The results of the activities in Year I of the PPDM program have benefited partners directly, including: a) Increasing number of people who understand, and have skills in responding to climate change; b) Number of facilities for IPAH, infiltration and rorak wells increased by 15 units; c) The number of people who harvest and utilize rainwater increases by 15KK; d) The number of members of the farmer group of women who use the yard for the cultivation of vegetables with vertikulture and hydroponic systems increases by 20 people.Some achievements that are non-physical such as: network of rainwater harvesters, improvement of knowledge and skills of community members, and pilot cooperation partnerships that integrate several productive economic business groups with BUMDes.

568 Background: Axillary lymph node metastasis is still the important prognostic factor in the management of breast cancer (BC). Where we have moved towards axillary conservation in clinically node negative (cN0), the debate on what after 1-2 sentinel lymph nodes positive (SLN+ve) still continues. The ideal situation would be wherein we can accurately predict which patient has a risk of additional non SLN+ve. Several nomograms have been developed to predict the risk of NonSLN+ve. But in view of the differences in tumor size and nodal burden between our patients and the western data, we conducted a study to validate some of these nomograms in our cohort of early BC with positive LN on Low axillary sampling (LAS). Methods: Clinico-pathological data was collected for operable BC (OBC) with cN0 who underwent upfront SLNB or AS. This was entered into the various nomograms and the probability of the Non SLN+ve was calculated. Nomograms with AUC of greater than 0.7 were pre-defined as considerable discrimination. Results: From 2013 to 2018, 2350 women with cN0 OBC underwent LAS. Of which, 670 (28.5%) had a positive node on LAS. Median pT size was 3 cm with 327 (48%), LVI +ve 152 (77%) ENI +ve, 525 (78.4%) Hormone receptor +ve and 485 (72.4%) grade 3 tumors. Of 670, 239 (35.7%) had a NonSLN+ve on completion axillary dissection. The AUC values for nomograms included,ie. MSKCC, MDAnderson, Tenon, Cambridge, Shanghai, Mayo clinic and Turkish were 0.769, 0.77, 0.55, 0.74, 0.65, 0.529, 0.563 respectively. Only three nomograms, MDA, MSKCC and Cambridge had an AUC of more than 0.7. However, they were associated with poor sensitivity and specificity and high FNR (Table) making them clinically unreliable for this cohort. Conclusions: All 7 nomograms were not validated in our study. The larger T size and higher nodal burden of our cohort may be responsible for the same. We thus need to develop an Indian nomogram to predict the risk of non SLN+ve for our patients. [Table: see text]

The relevance of the work is associated with the high prevalence and socio-economic significance of neurotic disorders associated with stress. Standards of medical care for patients with neurotic disorders associated with stress do not provide for the use of any physiotherapy or hardware effects. Drug therapy is mainly used, but its use is not always effective and is limited by the risk of developing allergies, drug addiction, toxic manifestations, side effects, etc. Therefore, more and more attention has been paid to non-drug treatment methods recently. Non-invasive neuromodulation is a therapeutic exogenous effect on various parts of the nervous system using an electromagnetic physical factor in the conditions of drug therapy. Purpose. Study the effectiveness of noninvasive neuromodulation in patients with asthenic neurosis. Material and methods. A total of 79 patients with the diagnosis of F48.0 Neurasthenia were studied (59 women and 20 men). The median age was 44 4.5 years. The main group 49 patients, 30 patients made up the control group. The course of non-invasive neuromodulation was performed against the background of medical treatment in accordance with the MES with the use of a pulse generator "TETOS" with feedback in the form of electroencephalography (registration certificate no.FRS2011/11198). Results. Immediately after the end of the course of treatment, 80% of patients manage to achieve a significant improvement in their subjective state. As a result of the proposed method, not only significant, but also persistent improvement in the condition of patients is achieved. Conclusions. The basis of this method is the interaction of two methods of treatment: physiotherapy and medication. As a result of the application of the proposed method, the main cause of the disease (psychotraumatic factor, life circumstances, etc.) is gradually identified, and the patient himself becomes aware of it, with subsequent correction of the condition. As a result, not only significant, but also persistent improvement in the condition of patients is achieved. According to the results of observations of the authors of the application, the disease does not resume for 1.52 years.

Category: Ankle Introduction/Purpose: Repeated microtrauma is thought to play a major role in the occurrence of osteochondral lesions of the talus (OLTs), but much remains unknown. Two-dimensional assessments of the relationship between ankle bone morphology and OLTs are occasionally seen. The purpose of this study was to evaluate the bone morphology of the ankle in OLT 3-dimensionally using three-dimensional computed tomography (3DCT), and to investigate the factors related to the occurrence of OLTs. Methods: The subjects were 19 patients (19 ankles) who underwent surgery for medial OLTs (OLT group). They included 13 men and 6 women. A healthy group without ankle disease served as a control group with the same number of 19 ankles. Three-dimensional ankle joint models were made based on DICOM data obtained with CT images. In the 3D model, the medial malleolus articular surface and the tibial plafond surface, the medial surface of the trochlea of the talus, and the lateral surface of the trochlea of the talus were defined. The tibial axis-medial malleolus (TMM) angle, the medial malleolus surface area (MMA), the medial malleolus volume (MMV), and the anterior opening angle of the talus were measured 3-dimensionally and compared in the OLT and control groups. Results: The mean TMM angle was significantly larger in the OLT group (34.2 ± 4.4°) than in the control group (29.2 ± 4.8°; p = 0.002). The mean MMA was significantly smaller in the OLT group (219.8 ± 42.4 mm2) than in the control group (280.5 ± 38.2 mm2; p < 0.001). The mean MMV was significantly smaller in the OLT group (2119.9 ± 562.5 mm3) than in the control group (2646.4 ± 631.4 mm3; p = 0.01). The mean anterior opening angle of the talus was significantly larger in the OLT group (15.4 ± 3.9°) than in the control group (10.2 ± 3.6°; p < 0.001). Conclusion: It was shown with 3DCT measurements that, in medial OLT patients, the medial malleolus opens distally, the MMA and MMV are small, and the talus anterior opening angle was significantly larger than in controls. This study suggests the possibility that the 3D bone morphology of both the mortise and tenon of the ankle joint are closely related to the occurrence of OLTs.

Pain is managed by using drugs such as analgesics, sedatives, antispasmodics. Long-term use may be ineffective and it could have bad side effects on the kidneys and liver. The culture of Indonesian people who like to use eucalyptus oil and telon oil to relieve dizziness, nausea and pain. The widespread use of essential oil aromatherapy which underlies this research.Objective : To determined the effect of aromatherapy peppermint essential oil by warm compress methods can reduce menstruation pain in Fakultas Kedokteran Universitas Hang Tuah Surabaya students.Methodology : This study uses an experimental research design. This study was carried out using a pretest-post-test control group design. Two groups consist of the intervention group (n = 21) were given peppermint essential oil by warm compresses five days before menstruation and the control group (n = 21) without any intervention. Primary data taken from filling out VAS and VMS pain scale questionnaires on the first day of menstruation after intervention.Results : Based on the results of the Mann-Whitney U non-parametric statistical test between the two groups measured using Visual Analogue Scale (VAS), a significance value of p = 0.001 (p <0.05) was obtained , H0 was rejected and H1 was accepted, a significant difference in peppermint aromatherapy on menstrual scale of pain in Fakultas Kedokteran Universitas Hang Tuah Surabaya students. The statistical test between the two groups was measured using a VMS (Visual Multidimensional Scoring System). Significant value obtained p = 0.001 then p <a, meaning that significant difference in the effect of peppermint aromatherapy on menstrual scale of pain in Fakultas Kedokteran Universitas Hang Tuah Surabaya students.Conclusion : The use of peppermint aromatherapy by warm compresses affected the reduction of the visual analogue score (VAS) and VMS (Visual Multidimensional Scoring System) on menstrual scale of pain in Fakultas Kedokteran Universitas Hang Tuah Surabaya students.Keywords : warm compress, peppermint, dysmenorrhea.

Background:Clinical manifestations are scarce in AA amyloidosis (AAA) and, contrary to other types of amyloidosis, involvement of the peripheral nervous system was rarely reported in AAA. However, the usual absence of hypertension despite chronic renal failure and the digestive involvement may be secondary to dysautonomia, but the autonomic nervous system has rarely been studied in AAA (1). Measure of the electrochemical skin conductance (ESC) is a simple and reproducible method to evaluate the function of eccrine sweat glands, which are innervated by small non-myelinated C fibers, and patients with AL and hereditary transthyretin amyloidoses show decreased ESC values (2,3).Objectives:To evaluate ESC values by Sudoscan in patients with AAA.Methods:Patients diagnosed as having AAA based on positive immunohistochemistry with an anti-serum amyloid A antibody followed at the national reference center for AAA in Tenon Hospital between July, 2017 and September, 2020, were routinely assessed for ESC with FDA approved Sudoscan (Impeto Medical, Paris, France). An ESC value above 60 microSiemens (µS) or 70 µS were considered normal for hands or feet, respectively. Categorical variables are reported as percentages and continuous variables are expressed as means±standard deviation. Correlations between age, body mass index (BMI), hemoglobin levels, C-reactive protein levels, estimated glomerular filtration rate using the Chronic Kidney Disease Epidemiology Collaboration equation (defined as 0 for dialysis patients) and ESC values were calculated using the nonparametric Spearman test. GraphPad Prism Version 7 software (GraphPad Software, San Diego, California, USA) was used for statistical analyses. A p-value <0.05 was considered as statistically significant.Results:Overall, 32 patients (16 women) were included, with a mean age of 57.4±13.6 years and a mean BMI of 25.2±6.8 kg/m2. Six (19%) had diabetes mellitus, and 5 (16%) had a kidney transplantation. The main causes of AAA were: monogenic autoinflammatory diseases (n=11, 34%, including 9 patients with familial Mediterranean fever), chronic and/or recurrent infections (n=5, 16%), obesity (n=3, 9%) and undefined (n=3, 9%). The mean hands’ ESC values was normal at 65.5±21.1 µS, although 8 (25%) patients had ESC values below 60 µS, including 2 diabetic patients. In contrast, the mean feet’s ESC values was abnormal at 62.7±23.7 µS, including half of the patients with ESC values below 70 µS (2 diabetic patients). Eight patients had abnormal ESC values only for feet, and 1 had abnormal values only for hands. Apart from a significant correlation between feet and hands’ ESC values (p<0.0001), only the estimated glomerular filtration rate was significantly associated with hands’ ESC values (p<0.01).Conclusion:To our knowledge, this is the first study to assess ESC in AAA. Feet’s ESC values were moderately impaired in half of the patients with AAA. Therefore, this study reinforces the previously reported alterations in the autonomic nervous system in patients with AAA that should probably be searched for in these patients. Abstract Background Streptococcus agalactiae (Group B Streptococcus, GBS) serotypes, sequence types, and antimicrobial resistance profile vary across different geographic locations affecting disease patterns in newborns. These differences are important considerations for vaccine development efforts and data from large countries in Africa is limited. The aim of this study was to determine serotypes and genotypes of GBS isolates from pregnant women and their newborns in Ethiopia. Methods A hospital based cross-sectional study was conducted at three hospitals in Ethiopia from June 2014 to September 2015. Out of 225 GBS isolates, 121 GBS were recovered, confirmed and characterized at CDC’s Streptococcus Laboratory using conventional microbiology methods and whole genome sequencing. Results Of the 121 isolates, 87 were from rectovaginal samples of pregnant women, 32 from different body parts of their newborns and 2 from blood of newborns with suspected sepsis. There were 25 mother-infant pairs and 24 pairs had concordant strains. The most prevalent serotypes among mothers and/or their babies were II, Ia and V (41.5, 20.6, 19.5 and 40.6%, 25 and 15.6%, respectively). Multilocus sequence typing (MLST) on 83 isolates showed ST10 (24; 28.9%) and ST2 (12; 14.5%) as most predominant sequence types. All GBS strains were susceptible to penicillin, cefotaxime and vancomycin, which correlated to the presence of wildtype PBP2x types and the lack of known vancomycin-resistance genes. Tetracycline resistance was high (73; 88%, associated primarily with tetM, but also tetO and tetL). Five isolates (6%) were resistant to erythromycin and clindamycin and 3 isolates were fluoroquinolone-resistant, containing associated mutations in gyrA and parC genes. All isolates were positive for one of four homologous Alpha/Rib family determinants and 1–2 of the three main pilus types. Conclusions Predominant serotypes were II, Ia, and V. A limited number of clonal types were identified with two STs accounting for about half of the isolates. All strains collected in this study were susceptible to beta-lactam antibiotics and vancomycin. Typical of most GBS, these isolates were positive for single alpha-like family protein, serine-rich repeat gene, as well as 1–2 pilus determinants.

Abstract Background Streptococcus agalacticae (Group B Streptococcus, GBS) is one of the most important causative agents of serious infections among neonates. This study was carried out to identify antibiotic resistance and virulence genes associated with GBS isolated from pregnant women. Methods A total of 43 GBS isolates were obtained from 420 vaginal samples collected from HIV positive and negative women who were 13–35 weeks pregnant attending Antenatal Care at Chitungwiza and Harare Central Hospitals in Zimbabwe. Identification tests of GBS isolates was done using standard bacteriological methods and molecular identification testing. Antibiotic susceptibility testing was done using the modified Kirby-Bauer method and E-test strips. The boiling method was used to extract DNA and Polymerase Chain Reaction (PCR) was used to screen for 13 genes. Data was fed into SPSS 24.0. Results Nine distinct virulence gene profiles were identified and hly-scpB-bca-rib 37.2% (16/43) was common. The virulence genes identified were namely hly 97.8% (42/43), scpB 90.1% (39/43), bca 86.0% (37/43), rib 69.8% (30/43) and bac 11.6% (5/43). High resistance to tetracycline 97.7% (42/43) was reported followed by 72.1% (31/43) cefazolin, 69.8% (30/43) penicillin G, 58.1% (25/43) ampicillin, 55.8% (24/43) clindamycin, 46.5% (20/43) ceftriaxone, 34.9% (15/43) chloramphenicol, and 30.2% (13/43) for both erythromycin and vancomycin using disk diffusion. Antibiotic resistance genes among the resistant and intermediate-resistant isolates showed high frequencies for tetM 97.6% (41/42) and low frequencies for ermB 34.5% (10/29), ermTR 10.3% (3/29), mefA 3.4% (1/29), tetO 2.4% (1/42) and linB 0% (0/35). The atr housekeeping gene yielded 100% (43/43) positive results, whilst the mobile genetic element IS1548 yielded 9.3% (4/43). Conclusion The study showed high prevalence of hly, scpB, bca and rib virulence genes in S. agalactiae strains isolated from pregnant women. Tetracycline resistance was predominantly caused by the tetM gene, whilst macrolide resistance was predominantly due to the presence of erm methylase, with the ermB gene being more prevalent. Multi-drug resistance coupled with the recovery of resistant isolates to antimicrobial agents such as penicillins indicates the importance of GBS surveillance and susceptibility tests. It was also observed that in vitro phenotypic resistance is not always accurately predicted by resistance genotypes.
