Abstract:
The aims of this research are finding out the correlation between product and consumer purchasing decisions, price and consumer purchasing decisions, promotion and consumer purchasing decisions, place and consumer purchasing decisions. This research has already done form May until July 2020. The samples were 50 respondents taken by nonprobability sampling technique. Data were collected by questionnaires. Data analysis were descriptive analysis technique and Spearman’s Correlation analysis using SPSS 23 software. The results of this research shows the product is strongly related with the correlation coeffecient value positive, which means it has a directional correlation and product has a significant correlation with the consumer purchasing decision at Malabar Mountain Cafe in Bogor. Abstract:
This in vitro study assessed the efficacy of functionalized graphene oxide (f-GO) nanocomposites on the decalcification of dentin, because dental caries of the root surface is becoming one of the new problems in aged society. Hydroxyapatite plates (HAP) and dentin slices were coated with f-GO nanocomposites by comparing them to silver diamine fluoride as a positive control, then treated with decalcification solutions such as ethylenediaminetetraacetic acid and citrate at 37°C for 24 h. Scanning electron microscopy (SEM) revealed significant protection of the surface morphology of HAP and dentin. On the other hand, a cariogenic Streptococcus mutans growth was inhibited by f-GO nanocomposites. In addition, cytotoxicity of them to epithelial cells was much less than that of povidone-iodine, which is commonly used for oral disinfectant. We synthesized 5 different f-GO nanocomposites such as GO–silver (Ag), GO-Ag–calcium fluoride (CaF2), GO-CaF2, GO-zinc, and GO–tricalcium phosphate (Ca3(PO4)2). They were standardized by evaluating under SEM, transmission electron microscopy (TEM), X-ray diffraction (XRD), X-ray photoelectron spectroscopy (XPS), thermogravimetry analysis (TGA), and Raman spectra after being synthesized in an aseptic technique. The abilities of GO-Ag, GO-Ag-CaF2, and GO-CaF2 nanocomposites were most preventive for decalcification. In addition, GO-Ag and GO-Ag-CaF2 almost completely inhibited S. mutans growth. However, they did not exhibit cytotoxicity to epithelial cells except at the highest concentration (0.1 w/v%) of GO-Ag and GO-Ag-CaF2. Furthermore, these f-GO nanocomposites exhibited less or no discoloration of dentin, although commonly used silver diamine fluoride causes discoloration of dentin to black. Thus, these f-GO nanocomposites are useful to protect dental caries on the tooth root that becomes a social problem in aged society.

Objective: Critical care echocardiography has become an integral tool in the assessment and management of critically ill patients. Critical care transesophageal echocardiography (TEE) offers diagnostic reliability, superior image quality, and an expanded diagnostic scope to transthoracic echocardiography. Despite its favorable attributes, TEE use in North American intensive care units (ICUs) remains relatively undescribed. In this article, we seek to characterize the feasibility, indications, and clinical impact of a critical care TEE program. Design: Retrospective, observational study. Setting: Tertiary care, academic critical care program consisting of 2 hospitals in Ontario, Canada. Participants: Consecutive critical care TEE examinations on ICU patients performed between December 2012 and December 2016 Interventions: None. Measurements and Main Results: Consecutive critical care TEE studies on ICU patients from December 1, 2012, to December 31, 2016, were reviewed. The TEEs performed on cardiac surgery patients and those without reports were excluded. Examination details, including indications, complications, examination complexity (number of views, Doppler techniques), and clinical recommendations were aggregated and analyzed. Two hundred seventy-four TEE studies were performed by 38 operators. Common indications for TEE studies were hemodynamic instability (45.2%), assessment for infective endocarditis (22.2%), and cardiac arrest (20.1%). A change in patient management was proposed following 79.5% of TEE studies. Thirty-eight percent of TEE studies were performed during evening hours or on weekends. There were no mechanical complications. Conclusions: Our observational data support intensivist-performed TEE as being safe and therapeutically influential across a broad range of indications. Our program’s demonstrated feasibility and impact may act as a model for TEE adoption in other North American ICUs.

Abstract:
This study investigates the sustainable values of cafes established using idle industrial facilities that are a part of the cultural heritage of South Korea in terms of the characteristics of the architectural space and consumers’ space utilization. Twenty regenerative cafes in five regions were selected, and five of them were analyzed by comparing their characteristics with those of the conventional cafes. Unlike conventional cafes, regenerative cafes have architectural spaces that seem to be non-everyday and elicit a feeling of the passage of time. Users utilized these cafes as spaces for activities and experiences for long periods compared to conventional cafes. Consequently, regenerative cafes were found to contain sustainable values as complex networking spaces, as cultural heritage that can be experienced and as independent tourist destinations. Regenerative cafes have become unique differentiated architectural spaces utilized by several users.

Método. trata-se de uma pesquisa descritiva e de abordagem qualitativa, realizada com sete enfermeiros que atuam na UTI de um hospital público de Mosso­ró-RN. Os dados foram coletados a partir de entrevistas semiestrutu­radas que foram discutidas à luz da análise de conteúdo. A pesquisa só teve início após aprovação pelo comitê de ética institucional mediante protocolo n. 107/10 e CAAE n. 2973.0.000.351-10. Resultados. percebeu-se que a maioria dos Enfermeiros considerava a avaliação neurológica indispensável no cuidado do paciente com TCE. Porém, se limitavam à aplicação da Escala de Coma de Glasgow, assim como havia a falta de padronização nessa assistência, transformando-a em procedimentos tipicamente individuais. Conclusão. os resultados apontados no estudo sugerem a limitação do conhecimento acerca da avaliação neurológica aplicada às vítimas de TCE’s, por parte dos en­fermeiros do setor estudado.

Abstract:
The TEG® 5000 and novel TEG® 6s measure the viscoelasticity of whole blood during in vitro clot formation. The two devices measure similar coagulation variables but utilize distinctly different technologies. This study aimed to determine the correlation and agreement between the thrombelastographic parameters obtained by the two devices during liver transplant surgery. We obtained blood samples at six predefined intervals during the surgery of 10 consecutive patients. Two operators proficient in the use of the TEG® 6s and TEG® 5000 systems performed thrombelastographic measurements on each sample: non-citrated TEG® 5000, citrated TEG® 5000 and citrated TEG® 6s. Agreement and correlation were assessed using Bland Altman plots and Lin’s concordance correlation. There was considerable inter-device variability for the different parameters measured by the TEG® 5000 and TEG® 6s devices. Acceptable agreement was observed when results were within the normal reference ranges. However, with increasing coagulopathy, agreement was poor and results could not be considered interchangeable. Although each of the three tests appeared reliable for qualitative detection of abnormalities of clot formation during liver transplant surgery, we found their quantitative results were not interchangeable.

Introduction: The use of transesophageal echocardiography (TEE) by intensivist physicians (IPs) and emergency physicians (EPs) in critically ill patients is increasing in the intensive care unit, emergency department, and prehospital environments. Coagulopathy and thrombocytopenia are common in critically ill patients. The risk of performing TEE in these patients is unknown. The goal of this study was to assess whether TEE is safe when performed by IPs or EPs in critically ill patients with high bleeding risk (HBR). Methods: All TEEs performed by an IP or EP between January 1, 2016, and July 31, 2019, were reviewed as part of a quality assurance database. A TEE performed on a patient was deemed HBR if the patient met at least one of the following criteria: undergoing therapeutic anticoagulation, had an INR > 2, activated partial thromboplastin time >40 seconds, fibrinogen <150 mg/dL, and/or platelet count <50 000/μL. The medical record was reviewed on each patient to determine whether upper esophageal bleeding, oropharyngeal bleeding, esophageal perforation, or dislodgement of an artificial airway occurred during or after the TEE. Results: A total of 228 examinations were reviewed: 80 in the high-risk group and 148 in the low-risk group (LBR). There were complications potentially attributable to TEE in 8 (4%) of the 228 exams. Total complications were not different between groups: 4 (5%) in the HBR group versus 4 (3%) in the LBR group (odds ratio [OR] = 1.89 [0.34-10.44], P =.368). Upper esophageal bleeding occurred in 5 total examinations (2%), which was not different between groups: 3 (4%) in the HBR group and 2 (1%) in the LBR group (OR = 2.84 [0.31-34.55], P = .238). There were no deaths attributable to TEE in either group. Conclusion: Transesophageal echocardiography can be safely performed by IPs and EPs in critically ill patients at high risk of bleeding with minimal complications.

The effect of low-dose heparin on postoperative hypercoagulability was assessed using thrombelastography (TEG) in eighteen patients undergoing elective abdominal aortic surgery. Patients received unfractionated heparin 50001U bd SC commencing on the first postoperative day. Native whole blood TEG was performed preoperatively and on day two postoperatively. A heparinase-modified TEG was performed at the same time as the native whole blood TEG on day two. There were no significant changes in the postoperative native whole blood TEG variables (r, K, α, MA) relative to preoperative controls. In contrast, there were significant decreases in r and K, and increases in a in the heparinase-modified TEGs postoperatively (P<0.01). There were significant differences between the postoperative native whole blood and heparinase-modified TEGs for all TEG variables (P<0.01). The results indicate that low-dose heparin reduces postoperative hypercoagulability following abdominal aortic surgery as assessed by thrombelastography.

Abstract:
Background: D-dimer concentration has been used by institutions to identify candidates for intensified anticoagulant treatment for venous thromboembolism prevention and for the mitigation of the microthrombotic complications associated with COVID-19. Thromboelastography (TEG) maximum amplitude (MA) has been validated as a marker of hypercoagulability and MA ≥68 mm has been utilized as a marker of hypercoagulability in other conditions. Methods: The goal of this study was to evaluate the relationship between coagulation, inflammatory, and TEG parameters in patients with COVID-19 on extracorporeal membrane oxygenation (ECMO). We performed a single-center retrospective analysis of consecutive patients that received ECMO for the treatment of COVID-19. TEG, inflammatory, and coagulation markers were compared in patients with and without a thrombotic complication. Correlation tests were performed to identify the coagulation and inflammatory markers that best predict hypercoagulability as defined by an elevated TEG MA. Results: A total of 168 TEGs were available in 24 patients. C-reactive protein and fibrinogen were significantly higher in patients that developed a thrombotic event versus those that did not ( P = 0.04 and P = 0.04 respectively). D-dimer was negatively correlated with TEG MA ( P < 0.01), while fibrinogen was positively correlated ( P < 0.01). A fibrinogen >441 mg/dL was found to have a sensitivity of 91.2% and specificity of 85.7% for the detection of MA ≥68 mm. Conclusions: In critically ill patients with COVID-19 treated with ECMO, D-dimer concentration had an inverse relationship with degree of hypercoagulability as measured by TEG MA. D-dimer elevation may potentially reflect hemostatic perturbation in patients on ECMO or the severity of COVID-19 related sepsis rather than designate patients likely to benefit from anticoagulation. Fibrinogen concentration may represent a more useful marker of hypercoagulability in this population.

We assessed the clinical impact of thrombelastography (TEG®) results (TEG® 5000, Haemonetics Corporation, Braintree, MA, USA) by measuring their ability to cause changes in a theoretical treatment plan and contribute to the understanding of haemostasis. We prospectively included paediatric intensive care unit (PICU) patients who had standard tests of haemostasis and TEG ordered and had an arterial catheter or extracorporeal access port in situ. Blood for standard tests and TEG was taken simultaneously. Independent of patient care, general patient information and results of standard laboratory tests were presented to five clinicians who were asked to document their theoretical treatment plan. Clinicians were then shown TEG results and asked if they caused a change in their plan, if they confirmed initial standard laboratory test results, if they enabled a better understanding of haemostasis and if they provided additional information. Inter-rater agreement between the clinicians was determined. Forty-two TEG results were obtained from 34 patients. Overall, the inclusion of TEG results led to a change in treatment plan in 97 of 207 occasions (47%), confirmed standard laboratory test results in 177 of 204 occasions (87%), enabled a better understanding of haemostasis in 140 of 204 occasions (69%) and provided additional information in 131 of 204 occasions (64%). Variation existed between clinicians, seemingly due to individual differences, with poor inter-rater agreement. We conclude that TEG results led to changes in treatment plans almost half the time, confirmed findings of standard tests and provided a better understanding of haemostasis, but randomised controlled trials are required to determine the role and influence of TEG results on patient outcome.

Was ist Solidarität im Krankenhaus? Im Alltag äußert sie sich darin, dass Pflegekräfte die eigenen Schutzrechte ignorieren, um Patientinnen und Patienten, aber auch ihre Kolleginnen und Kollegen „nicht hängen zu lassen“. Der Beitrag handelt von dem Versuch, aus dieser disziplinierenden Form der Kollegialität eine „Kampf-Solidarität“ zu entwickeln: Teams fordern ultimativ Maßnahmen zur Entlastung und drohen damit, andernfalls freiwillige Leistungen wie das Einspringen außerhalb des Dienstplans zu verweigern. Sie nutzen ihre gestiegene Produktionsmacht, da die Abläufe im Krankenhaus regelhaft darauf basieren, dass sich Pflegekräfte über ihre arbeitsvertraglichen Pflichten hinaus engagieren. Der Beitrag untersucht diese neue Kampfform der Ultimaten anhand von sechs Fallstudien, basierend unter anderem auf Experteninterviews mit Pflegekräften, Gewerkschaftssekretär/innen und betrieblichen Interessenvertreter/innen. Theoretisch knüpft er an Solidaritätskonzeptionen von Bayertz (1998) und den Machtressourcenansatz an. Er kommt zu dem Schluss, dass die Ultimaten dazu führen können, die – wie es ein Krankenpfleger und ver.di-Aktivist ausdrückt – im Klinikalltag „auf den Kopf gestellte“ Solidarität „wieder auf die Füße“ (SWB04, 30-32)1 zu stellen. Es entwickelt sich eine Kampf-Solidarität, die die Durchsetzungsfähigkeit in Arbeitskämpfen, aber auch die Bereitschaft zu widerständigem Alltagsverhalten stärkt.

It has expanded the physician's ability to clearly examine cardiac structures and bloodflow; in many instances it has been the means by which rapid and lifesaving diagnoses have been made. The nurse collaborates with the physician during TEE by ensuring adequate preparation, remaining at the patient's bedside to assist with and monitor the effects of probe insertion and ensuring close observation and postprocedural follow up. The complication rate for TEE is negligible. Possibilities include routine pediatric use, biplane imaging and incorporation with exercise stress testing.

Abstract Purpose To validate our previously designed transcatheter arterial embolization (TAE) technique for bilateral iliac arteries in unstable pelvic fractures, which is designed to also prevent gluteal necrosis and avoid vasopressors. Methods We retrospectively analyzed the data of patients with pelvic fractures who underwent our new TAE procedure to determine the incidence of subsequent gluteal necrosis. We also compared certain variables between patients who underwent TAE before 2005 using a different technique and developed gluteal necrosis and patients who underwent TAE in 2005 and onward using our technique. Gluteal necrosis was confirmed by a radiologist based on imaging findings. Results Seventy patients with pelvic fractures who underwent our TAE technique met the inclusion criteria (bilateral iliac arterial embolization and no embolic agent other than a gelatin sponge). Patients’ median age was 47.5 years, 33 were male, and 92.9% (65/70) had unstable fractures. Sixty-eight patients had severe multiple trauma. No patients developed gluteal necrosis following our TAE procedure and the overall survival rate was 82.9% (58/70). We found no statistically significant difference in procedure time between the previous and new technique, although the new procedure tended to be shorter. Furthermore, overall survival did not significantly differ between the groups. Multiple regression analysis revealed that TAE procedure time and external pelvic fracture fixation were independently related to gluteal necrosis. Conclusions Our non-selective bilateral iliac arterial embolization procedure involves arresting shock quickly, resulting in no post-procedure gluteal necrosis. The procedure involves cutting the gelatin sponge rather than “pumping” and avoids the use of vasopressors.

Entre los retos de traducción que plantean los textos ficcionales, cabe destacar la traducción de los particulares ficcionales o irrealia, es decir, aquellas unidades léxicas que designan conceptos ficcionales. Esto se debe, por un lado, a su inexistencia material y a su falta de referencia a objetos del mundo real, lo que dificulta además la labor de documentación y búsqueda terminológica y condiciona la comprensión del texto ficcional. Por otra parte, la traducción de este tipo de unidades exige una habilidad creativa al traductor, que debe crear nuevas palabras para denominar los irrealia empleando los procedimientos de formación de nuevas palabras existentes en la lengua meta. En este sentido, cabe preguntarse si los irrealia pueden considerarse como un tipo particular de neologismo y, en tal caso, si recurren a los mismos procedimientos de creación. Para dar respuesta a estas preguntas, este trabajo tiene como objetivo el análisis de los procedimientos lingüísticos de formación de nuevas palabras utilizadas en las dos traducciones al francés de The Lord of the Rings, de J. R. R. Tolkien (1954-1955), publicadas por Christian Bourgois como Le Seigneur des Anneaux en ambos casos y que corresponden a los traductores Francis Ledoux (1972-1973) y Daniel Lauzon (2014-2016). Finalmente, esto nos permitirá establecer conclusiones sobre la naturaleza lingüística de estos elementos y su relación con los neologismos, así como sobre la relación entre un determinado procedimiento y la estrategia global de traducción.

The study was performed to determine the possible direct effects of low concentrations of prostacyclin that might spill over into the systemic circulation during the administration of inhaled aerosolized prostacyclin. Platelet aggregation in response to adenosine diphosphate and collagen, as well as measurement of the maximum amplitude of the thrombelastograph (TEG), was undertaken in vitro using venous blood exposed to low concentrations of prostacyclin (0, 10, 100 and 500 pg/ml) from eight healthy volunteers. There were statistically significant reductions in parameters of platelet aggregation in response to the agonists adenosine diphosphate (1 μmol/l and 8 μmol/l) and collagen (10 μmol/l) following exposure to as little as 10 pg/ml of prostacyclin. The maximum amplitude of the TEG was unchanged over the entire range of prostacyclin concentrations studied. The results indicate that low concentrations of prostacyclin or prostacyclin metabolite such as may be observed during inhaled aerosolized prostacyclin therapy are likely to be associated with a marked platelet aggregation defect. This defect was not detected by the TEG.

The national targets of reaching carbon peak in 2030 and carbon neutrality in 2060 propose higher requirements for energy conservation and emission reduction of China’s automobile industry. As an important measure for the government, the fuel consumption and new energy vehicle (NEV) credit policy system has a significant impact on the Chinese and even the global vehicle market. Considering the lack of a systematic evaluation model for China’s fuel consumption and NEV credit regulations, this study establishes a hierarchical optimization decision-making model based on technology frontier curves and a multi-dimension database containing extensive data of technologies, products, and enterprises in the Chinese market to simulate and evaluate the technology compliance and policy impact under multiple regulations. The results show that, from the perspective of the technology frontier curve, gasoline technologies still have great cost-effectiveness advantages when the fuel-saving requirement is less than 46%, and the space for plug-in hybrid electric vehicles (PHEVs) and range-extended electric vehicles (REVs) is gradually shrinking due to the cost reduction of battery electric vehicles (BEVs). BEV400 will be better than PHEV70 and REV100 when the fuel-saving requirement is higher than 79%. Diesel vehicles are always not competitive in the passenger car market. In terms of the compliance of corporate average fuel consumption (CAFC) regulation, the start-stop technology will be gradually phased out and mild hybrid electric vehicles will be rapidly introduced due to their high cost-effectiveness in 2025. With the tightening of regulations, the penetration rate of BEVs and PHEVs will be 23.7% and 6.7%, respectively, and mild hybrid electric vehicles will be gradually replaced by strong hybrid electric vehicles in 2030. By 2035, the penetration rate of BEVs and PHEVs will be 43.6% and 6% further. For the CAFC and NEV credit regulation (widely known as the dual credit regulation), the single-vehicle credit poses a greater impact on the penetration of NEVs than corporate credit percentage limitation and is the key factor that should be focused on. The NEV credit limitation in the dual credit regulation could push ‘poor performance’ automakers to produce the required number of NEVs and meet the bottom line. However, in the long term, when compared to the CAFC regulation, the dual credit regulation is more lenient, due to NEVs being able to get double benefits both on NEV credit and CAFC credit, and NEV credit can also unidirectionally compensate CAFC credit under the dual-credit policy context. With the increased penetration and cost reduction of NEVs, the ‘averaging’ effect of dual credit regulation will inhibit the development of energy-saving and new energy vehicles. Therefore, eliminating the connection between NEV credit and CAFC credit or only leaving the CAFC and the fuel consumption limit regulations in the future will be better for the long-term development of the energy-saving and new energy vehicle industry.
