Abstract:
Overweight and obesity affect over half a billion people globally and represent major public health concerns because excess weight gain is a key risk factor for non-communicable diseases. This chapter presents an overview of global trends in obesity, considering both developed and developing country contexts. It describes how this pandemic is rooted in the “nutrition transition” taking place around the world as a result of a globalized agri-food system that is changing the quantity, types, and desirability of foods available for consumption. In some countries, this is contributing to a “double burden of malnutrition,” as the problem of undernutrition now coexists alongside an increasing prevalence of over-nutrition. An overview of key policy responses and policy instruments that governments can utilize to address obesity is provided, recognizing that a holistic food systems response is needed to address the global challenge of obesity. The remainder of the chapter focuses on the food security and obesity challenges facing the Caribbean Community (CARICOM) and profiles a “farm to fork” school feeding project in the island nation of St. Kitts-Nevis that was designed to reduce obesity and improve food security among children. Implementation and key results of this integrated project are presented, including the core components of agricultural production, procurement of local produce, and children’s consumption of nutritious foods. The chapter concludes by identifying lessons for supporting “farm to school”-type projects as a possible food systems response to addressing obesity and food insecurity.

Obesity is widely acknowledged as a risk factor for both the incidence and progression of osteoarthritis. Loss of at least 10% of body weight is recognized as a cornerstone in the management of obese patients with osteoarthritis, and can lead to significant improvement in symptoms, pain relief, physical function, and health-related quality of life. However, questions still remain surrounding optimal management and whether structural disease progression can be arrested. Given the significant health, social, and economic burden of osteoarthritis, especially in obese patients, it is imperative to advance our knowledge of osteoarthritis and obesity, and apply this to improve care and outcomes. This chapter overviews what is known about osteoarthritis, obesity, and weight loss and discusses current key challenges in management and maintenance of weight loss for overweight and obese individuals with osteoarthritis.

Modern lifestyles put a great burden on the human musculoskeletal system. Since 1980, the worldwide prevalence of obesity has tripled in many European countries. Obesity is known to affect both weight-bearing and non-weight-bearing joints due to a combination of mechanical overload and systemic inflammation. On the other hand, both to combat the obesity pandemic and to increase or maintain the quality of life, physical activity and sports are encouraged next to a healthy diet. Although both have a positive influence on cardiovascular risk factors, physical activity and especially sporting activities do lead to increased loading of the active joints and increased risk for joint injuries, which might lead to osteoarthritis development. This chapter provides an overview of the current knowledge on lifestyle risk factors for the development and progression of osteoarthritis as published in recent systematic reviews, complemented with several narrative reviews.

This chapter provides additional explanation about the sociopolitical landscape of non-communicable diseases (NCDs), and focuses on global responses to change the sociopolitical landscape following the 2011 Political Declaration of the High-Level Meeting of the United Nations General Assembly on the Prevention and Control of NCDs. It helps readers to understand agreed global targets related to NCDs set out in the World Health Organization’s global action plan for the prevention and control of NCDs 2013–20 and by the United Nation’s sustainable development goals. It explores why the NCD agenda should be linked with the undernutrition programmes and maternal and child health programmes. The chapter also provides case studies to elaborate the double burden of malnutrition, i.e. coexistence of undernutrition and obesity in the same population.

Primary liver cancer is the sixth most frequently occurring cancer in the world and the second most common in terms of cancer deaths. The global burden of liver cancer is borne principally by countries in East Asia and Africa, where 80% of liver cancer arises. Incidence rates of liver cancer, however, have begun to decline in Asia, while rates are increasing in low-rate areas such as Europe and North America. The dominant histology of liver cancer in almost all countries is hepatocellular carcinoma (HCC). The major risk factors for HCC—chronic infection with either hepatitis B virus (HBV) or hepatitis C virus (HCV), aflatoxin B1 (AFB1) contamination of foodstuffs, excessive alcohol consumption, and diabetes/obesity/fatty liver disease—all result in chronic inflammation in the liver. HBV infection is preventable by immunization, and HCV infection is largely preventable by public health measures and now is curable with new antiviral therapies.

While less apparent than outright hunger or obesity, the lack of essential vitamins and minerals in people’s diets is one of the leading contributors to the global burden of disease. Current interventions, such as supplementation or fortification, are being implemented with varying success, but—while important—overall progress in the fight against micronutrient malnutrition has been limited. Biofortification, the breeding of crops for higher contents of vitamins and minerals, is a new approach to complement existing interventions. This chapter gives an overview of the problem of micronutrient malnutrition and how it is measured; it briefly discusses current micronutrient interventions, and then presents the reasoning behind biofortification before it examines the feasibility of biofortifying crops and summarizes studies on their potential impact and economic justification. After listing current biofortification programs, the chapter looks into the political controversy surrounding genetic engineering in agriculture and how it relates to biofortification; it then concludes with an assessment of the current status of biofortification and its potential.

Prostate cancer is the most common solid tumor and the second leading cause of cancer-related mortality in American men. Worldwide, prostate cancer ranks second and fifth as a cause of cancer and cancer deaths, respectively. Despite the international burden of disease due to prostate cancer, its etiology is unclear in most cases. Established risk factors include age, race/ancestry, and family history of the disease. Prostate cancer has a strong heritable component, and genome-wide association studies have identified over 110 common risk-associated genetic variants. Family-based sequencing studies have also found rare mutations (e.g., HOXB13) that contribute to prostate cancer susceptibility. Numerous environmental and lifestyle factors (e.g., obesity, diet) have been examined in relation to prostate cancer incidence, but few modifiable exposures have been consistently associated with risk. Some of the variability in results may be related to etiological heterogeneity, with different causes underlying the development of distinct disease subgroups.
