"Background: Chemotherapy still represents the backbone of systemic treatment for many solid tumors. Alopecia remains one of its most distressing side effects. Scalp cooling is an effective strategy to reduce the risk of alopecia induced by chemotherapy, therefore this study aimed to assess the efficiency of DigniCap scalp-cooling system in our center. Material&Methods: 113 patients diagnosed with solid tumors, who underwent chemotherapy using the DigniCap scalp-cooling system in our center between January 2018 - December 2020, were included in the study. Alopecia was evaluated by the physician according to Dean’s scale at baseline and after the last cycle of chemotherapy. The primary endpoint was to evaluate scalp-cooling efficiency defined by a Dean's scale score of 0–2 (hair loss ≤50%). Results: 78 (72.3%) out of the 113 patients who used scalp cooling presented hair loss of 50% or less, the majority were females (91.2%) and had a diagnosis of breast cancer (49.6%) or gynecological cancer (27.4%). The mean number of chemotherapy cycles using DigniCap was 4.82 cycles. Hair loss greater than 50% of the scalp surface was observed in 27.7% of patients, the highest grade of alopecia being observed for anthracycline-based chemotherapy. No serious adverse events were reported. Conclusion: The results of this study support the evidence existing so far in the literature. Among patients who underwent chemotherapy for various cancer types, the DigniCap System significantly prevented hair loss and we consider that it should be integrated in the supportive care of patients."

Objectives Term limits might be appropriate for leadership positions in academic public health. This study assessed the appointment processes and terms for deans, directors, and chairs of schools and programs of public health and their views on term limits. Methods A 10-question survey was developed for the Association of Schools and Programs of Public Health and provided electronically to 127 deans and program directors in November and December 2019, of whom 58 (46%) responded. Results Of 54 respondents to the question on term limits, 45 deans and directors of schools and programs of public health served with no terms or limits and 9 served with terms of 3-5 years with no limits on the number of terms. Respondents largely agreed with most arguments for or against term limits. Of 51 respondents, most indicated completely or moderately valid support for 2 arguments for term limits: diversity (n = 40) and succession planning (n = 40). Of 51 respondents, most indicated completely or moderately valid support for 3 arguments against term limits: stable and continuous leadership (n = 40), time for leadership development (n = 37), and loss of institutional memory (n = 35). Twenty-seven of 53 responding deans and directors viewed the most appropriate terms and limits as being more restrictive than their current terms; the other 26 viewed the most appropriate terms as being the same as their current terms. No respondents preferred less restrictive limits than their current terms. Conclusion Although term limits for deans, directors, and chairs are rare in schools and programs of public health, many deans and directors view term limits as appropriate. Schools and programs may reconsider their current policies for term limits.

Abstract:
Introduction Telemedicine is a remote medical practice using information communication technology (ICT), and has been increasing in France since 2009. With all new forms of medical practice, education and training (ET) is required for quality and safety. To date, implementation of telemedicine ET has not been assessed in France. The objective of this study was to describe the implementation of telemedicine ET and evaluate the knowledge, attitudes and practices (KAP) of deans and associate deans from all medical schools in France. Methods A cross-sectional non-mandatory, descriptive online survey with a self-administered questionnaire was performed from 15 November to 6 December, 2017. Respondents were accessed through the ‘ Conférence des doyens des Facultés de médecine’. Results There were 48 respondents with a 47.4% response rate among deans. Telemedicine ET was limited in France; 10.4% in 1st year medicine (PACES); 4% in the final 3 years of medical school (D.F.A.S.M.) and 18.8% in medical residency. Emergency medicine, dermatology, radiology, neurology and geriatrics were specialties with implemented telemedicine training during residency. Of all respondents, 90% expressed a need to increase telemedicine ET, among which 75% accepted external support. A highly positive attitude towards telemedicine practice was reflected by 60.4% of respondents, and 56.2% practiced telemedicine at least once. Discussion This study was the first to assess national telemedicine ET implementation in France. Telemedicine was integrated into initial medical education; however, telemedicine ET remains limited despite the positive attitudes of deans and associate deans. Further research would need to be conducted on telemedicine ET implementation and KAP of medical students and residents.

Melaleuca deanei F.Muell. is listed under state and national protection legislation occurring as small, disjunct populations restricted to the Central Coast botanical district of New South Wales. This paper reports on the population structure, reproductive output and phenology of large and small field populations of M. deanei, compared with three common congeneric species in the area, namely, Melaleuca nodosa (Sol. ex Gaertn.) Sm., Melaleuca thymifolia Sm. and Melaleuca styphelioides Sm. Both M. deanei and M. styphelioides had few seedlings per population at the sites studied. Results indicated that seedling recruitment is rare and would appear to require specific conditions. In addition, M. deanei had a low incidence of flowering within the small populations, significantly fewer fruiting plants per population and significantly lower numbers of viable seeds per square metre, most likely compounding its limited recruitment. Flowering, when it occurred in M. deanei, was from mid to late October through to late November–early December with increased flowering in response to fire and along road edges. Further study is needed to determine reproductive success at the individual-plant level, the breeding system and recruitment requirements of these species.

The article reviews the last (December 4, 2020) meeting of the Forum of principals of the humanities universities and deans of the humanities faculties of member states of the Commonwealth of Independent States, which coincided with the thirtieth anniversary of the Commonwealth. The main source is the shorthand report of the Forum online meeting. By pointing out that the Forum was attended by representatives of seven member coun- tries of the Commonwealth of Independent States (Uzbekistan, Kazakhstan, Tajikistan, Kyrgyzstan, Belarus, Moldova and Armenia), the author reports that the plenary panels of the Forum discussed new forms of cooperation in the pandemic, the issues of using digital technology in the humanities education and the development of distance learning methods. Stating that the coronavi- rus pandemic that broke out on the cusp of the years 2019 and 2020 was the greatest challenge to the world education and science, forum participants could not but discuss the mental state of students, teachers and researchers who have continued to perform their duties under the force majeure conditions. The participants of the Forum made a point of the ever-increasing role of modern media, which act nowadays as specific tool for building a common scientific and educational space in the post-Soviet region.

Objective and rationale: Comparison of self-perceived genetic-genomic knowledge of nurse educators and graduate degree nursing students enrolled at a large diverse urban university in the US. Comfort level in performing genetic-genomic related tasks and its perceived relevance to nursing also explored. Practicing clinicians are expected to have Genetics and Genomics (G-G) knowledge to provide care to a client and their family with a genetic condition and faculty expected to be able to educate these practitioners.Methods: Two groups of participants asked to complete identical survey instrument, Genetics/Genomics Literacy Assessment (GGLA). Data was collected from September 2014-December 2015. Deans/Directors from university’s nursing programs (N = 17) sent introductory email with survey link and asked to forward to their faculty. APRN/DNP students at one of the university’s graduate programs sent email with survey link.Results: Fifty-three nurse educators and thirty-six graduate degree nursing students completed survey. Comparison of categorical data revealed nurse educators perceived G-G knowledge correlated to graduate degree students. Majority of participants demonstrated significantly lower correct percentages (< 55%) correct] to survey questions. Majority of participants (> 75%) attitude agree it is important for nurses to know this content and be able to teach this material. However, majority (> 75%) were not comfortable with teaching or explaining this material.Conclusions: Nurse educators need to be knowledgeable in order to educate their students who are expected to practice at advanced efficiency. A prepared nursing workforce is crucial for the translation of G-G integration into personalized precision healthcare.

e12075 Background: Hair loss (alopecia) is recognized as one of the most common and distressing side effects of chemotherapy. The scalp cooler first used in 1970s against the Chemotherapy CIA (chemotherapy induced alopecia). This study aimed at assessing the effectiveness of scalp cooling (PAXMAN Cooler ORBIS II SCALP COOLER) in reducing chemotherapy-induced alopecia. Methods: This prospective study was conducted at the Mount-Lebanon Hospital in the one day chemotherapy between 01 January 2017 and 31 December 2018. 109 women diagnosed with breast cancer with stage I or II undergoing neoadjuvant, and adjuvant chemotherapy were enrolled. Patients with scalp cooling 54.1% (n=59), control 45.9 % (n=50). Scalp cooling done by using the scalp cooling device. The primary end point was to assess the hair loss using the dean’s scale after multiple cycles of chemotherapy. The clinical assessment and alopecia evaluation were done after each chemotherapy cycle. The secondary endpoint was the side effect of the scalp cooler such as headache. Results: The Mean age was 55.73 ± 11.9 years. 82.56 %received Anthracycline-based chemotherapy regimen with Docetaxel, and 14.69 % Anthracycline-based chemotherapy regimen with Paclitaxel and 2.75 % took only anthracycline with cyclophosphamide. The effectiveness of hair preservation was successfully reported as 62.7% grade 0, 22.01 % grade 1 and 15.29% grade 2. There were highly significant differences between patients underwent scalp cooling and patients without scalp cooling with different regimens of chemotherapy (P value <0.0001). The major side effect, the headache where was reported in 25.7% from the total (P value <0.0001). Conclusions: Hair preservation in stage I , II breast cancer undergoing multiple regimens of chemotherapy was significantly more effective in the scalp cooling group.

The discussion of impersonal narration is based on Wayne Booth's theories about the confusion of distance resulting from impersonal narration; the discussion of free indirect discourse is drawn from a number of definitions. Utilizes a number of specific references to the texts and to criticisms of the texts to demonstrate the absence of norms and the effect that the ambiguity and deception may have on readers.

ENGLISH ABSTRACT: Background - The workload of the forensic pathologist and Forensic Pathology Services staff is increased by the referral of potentially unnecessary natural cases to the Forensic Pathology Services. The primary aims of the medico-legal autopsy are limited to establishing a cause of death in presumed unnatural cases, and to exclude criminality or negligence. Objective – To determine the final outcomes of forensic post-mortem examinations in “sudden and unexpected” adult deaths over a 5 year period. Methods - An observational, retrospective, descriptive study was conducted. ”Sudden and unexpected” adult deaths referred to Tygerberg Forensic Pathology Services between 1 January 2001 and 31 December 2005 were reviewed. Data was collected from the autopsy reports, contemporaneous notes and hospital records. Findings – A total of 816 adult cases of sudden and unexpected death were referred to Tygerberg Forensic Pathology Services over the 5 year period studied. Complete autopsies had been performed in 74% (601/816) of cases. The presumed manner of death was natural in 79 % of cases, and an increase in the number of natural cases autopsied per year was noted over the 5-year study period. Diseases of the cardiovascular, respiratory and central nervous systems were responsible for the majority of natural deaths. Infectious diseases were responsible for most deaths in the youngest age group studied (18-29 years). Acute alcohol poisoning was responsible for the deaths of 35 (6%) cases, with an average blood alcohol concentration of 0.38g/100mL in these cases. Eight deaths were drug-/substance related. Waiting times for blood alcohol and toxicology results increased over the 5-year study period. No cause of death was found in 10.6% of cases. Conclusions -The questionnaire and interviewing structure could possibly be improved in order to obtain better pre-autopsy information and to reduce the number of “unnecessary” medicolegal autopsies, thereby reducing the burden of cost on the Forensic Pathology Services.

Background Tuberculosis (TB) remains a major cause of disease and death worldwide. In 2008, South Africa ranked third in the world in terms of the total numbers of new TB cases. Little is known about the profile of TB deaths at individual hospital level. Hence, the aim of the study was to describe the profile of TB deaths in Charles Hurwitz TB Hospital for the period January to December 2007. Methods A descriptive study was done, based on retrospective record review of all patients who died between January and December 2007 at Charles Hurwitz TB Hospital, irrespective of the date of admission. The data was analysed using Microsoft Excel. Findings The mean age at death was 41 years (standard deviation =10.9 years). Less than half of deceased individuals were employed (43.4%), more than one third had a history of smoking (42%) and the majority had a history of alcohol consumption (60.5%). Almost three quarters of the patients (75.3%) were being treated for the first time. The majority (85.1%) of deceased patients tested for HIV were HIV positive, but only 23.3% of those referred for treatment were actually on ART, indicating missed opportunities in treatment and care at the hospital. Conclusion There is need for ongoing vigilance and training to ensure that TB hospitals and individual health care providers comply with the national quality of care and TB management standards, and that missed opportunities are eliminated to reduce avoidable TB deaths.

This dissertation explores the problem and process of the isolated intellectual’s accommodation in four of American author Saul Bellow’s novels: Herzog, Humboldt’s Gift,The Dean’s December,and Ravelstein. The protagonists of these novels, most of whom are, like Bellow, of European-Jewish background,feel alienated from the mainstream, mid-to-late-20th-century U.S. society, and try in various ways to more fully enter into it. Yet “accommodation,” as it is interpreted here, really means a going-halfway, a process of mediation through which the character maintains his own individual, subjective integrity as a “thinker” while also communicating and interacting more with others (including the “masses,” the non-intellectuals), that is, engaging more actively in the “mass culture.”The novels’protagonists, although they manage to “fit into” the larger society to varying degrees,share a keen awareness of the need for the internal and subjective—as opposed to the merely external, the commercial, financial, mechanical, technological—and for a sense of humanity, a “law of the heart,” a compassionate attitude toward a suffering (and indeed increasingly alienated) mankind. However, this mode of the individual’s accommodation with his society is also closed to another key duality, that of mind (or soul) and body (or material world). They thus seek an accommodation or mediation between such opposite ideas or values as self and other, individual and social, internal and external, thinking and feeling, past and present, local and universal. These novels seem to be structured, then, around a dialectical dynamic, a pulling- both-ways, just as the characters are inevitably driven by inner conflicts, often perplexed and confused, moving in both directions at once. But this is a very human complexity, and Bellow describes it with great energy and passion.

Two deaths occur in American football on November 25. Henry MacCracken, chancellor of New York University, where one of the fatal injuries occurred, invites the nineteen colleges NYU has played since 1895 to a conference on December 8, aiming to abolish or at least reform American football. Camp persuades the rules committee to meet December 9, the day after the MacCracken conference, and develops a plan for Yale’s position during that meeting. While Camp is out of town, Alice communicates the plan to Yale’s President Hadley, who summarizes it in a memo and apparently approves. Roosevelt meets with Harvard’s coach and appears supportive of unspecified actions being taken at Harvard (possibly, as later events suggest, proposals being developed by Harvard’s special committee).

A novel coronavirus, Severe Acute Respiratory Syndrome Coronavirus -2 (SARS-CoV-2), first appeared in the city of Wuhan in Central China in December 2019. Initial cases appeared to be centred on a so-called wet market, but the outbreak spread rapidly. The World Health Organisation (WHO) declared a Pandemic Health Emergency of International Concern on 30 January, 2020. At the time of writing, there have been over 11 million cases globally and more than 500,000 deaths.

Following the Battle of Prairie Grove in December 1862--chronicled in the previous chapter--the remainder of the conflict sees little action by large armies of either side. Instead, the official military activity concentrates on raids, counterinsurgency efforts, and punitive attacks. Much of the chapter deals with life on the home front in the Ozarks no-man’s-land and the guerrilla fighters who came to represent the region’s war over the final two years.

Olivares believed that after four years of assisting the refugees the time had come to announce public sanctuary. La Placita would be a safe space for refugees against immigration officials who regarded them as “illegal aliens.” On December 12, 1985-the feast day of Our Lady of Guadalupe-and in a formal Mass, Fr. Olivares declared La Placita a public sanctuary. This chapter also deals with the tense relationship of Fr. Olivares with Archbishop Roger Mahony over sanctuary.

This paper is questioning which kind of urban development is adequate for Algiers far from oil dependency and basing on what was achieved since the approval of its Masterplan in December 2016 and regarding to the potentialities and opportunities already set in the territory. The very principle of development in the territorial model already outlined a solution oriented towards the reorganization of traffic according to a principle of macro-mesh transport network structuring a new poly-centrality. At the nodes of this network, the place of these centralities, exchange hubs have been programmed. Thus, all the conditions are met so that in a second step, in terms of details, the city of Algiers can opt for a Transit-Oriented Development TOD that makes Algiers an attractive and competitive territory. By an analysis of the contents of the policy orientations of the PDAU (masterplan) of Algiers1 , a participative observation of the actions carried out by the city of Algiers, one will identify the goals already achieved for the development strategy plan. One will also show that the TOD solution is the easiest if not the only one and that all the legal and strategic conditions are met to facilitate the transition to detailed development plans.

This paper deals with the recovery operation of the bottom station MABEL (Multidisciplinary Antarctic Benthic Laboratory) with a mass of 1,7 tons in air that has been deployed in December 2005 from the German research vessel Polarstern, by means of a release transponder at a water depth of 1850 m, close to the shelf ice edge near the German polar research station Neumayer. The project is run under the umbrella of the Italian Antarctic programme by the project leader INGV (Istituto Nazionale di Geofisica i Vulcanlogia) /1/. During the cruise ANT XXV-2 of the Polarstern (organized by the German AWI - Alfred-Wegner-Institut) the German partners TFH Berlin and TU Berlin have been participating with their module MODUS (Mobile Docker for Underwater Sciences) to recover the station from the deep sea. The special circumstances in the Antarctic sea — the ice coverage of the deployment area and the tight time schedule for the operation — make such an operation quite delicate. This paper describes the special technology used both for the station and the recovery module. The operation itself will be discussed, showing the data of operation using a combined tracking of GPS-data and the underwater positioning system Posidonia of the Polarstern. The special circumstance of the operation was the inadequate data of the position achieved during the deployment, so that a safe search strategy had to be found. The mission ended successfully on December 16th, 2008 with the recovery of the MABEL station. Simulations for the system behaviour will be shown.

Abstract:
The USS Frank Cable (AS-40) was conducting steam testing of boiler safety valves in December 2006. While conducting this test, two generating tubes failed above the water drum causing rapid expansion of the boiler water. Subsequently, the boiler casing failure above the economizer resulted in three deaths and two critical injuries. This failure was directly caused by poor fabrication of a reinforcing plate to the corner seam weld of the exhaust stack transition piece above the economizer of No. 1A Boiler. Structural analysis showed that if the boiler had been constructed per design, the loss of boiler water flashing to steam, and subsequent pressure increase in the boiler casing, would have contained the rapidly expanding steam and would not resulted in a failure of the boiler casing. The analysis of the as-built structure was unable to resist the pressure increase from the tube failure.

Abstract:
The fast spread of coronavirus (COVID-19) had negative impacts on financial markets worldwide. It created uncertainty and a lack of confidence, causing unprecedented levels of risk, causing sharp losses to investors in a very short period. In view of these events, this essay aims to investigate the relationship between Covid-19 (confirmed cases and deaths), and the stock market indexes of Greece (ATG), France (CAC40), Germany (DAX 30), United Kingdom (FTSE 100), Italy (FTSE MID), Spain (IBEX 35), Ireland (ISEQ), and Portugal (PSI 20), from December 31st, 2019 to July 23rd, 2020. In order to achieve such an analysis, we want to validate if: the increase in cases and deaths resulting from Covid-19 have any connection with the financial markets under analysis? If so, do these connections cause shocks in European financial markets? The results suggest structure breaks, mostly, in March 2020. Covid-19 data (confirmed cases) integrate with the Covid-19 data series (deaths), with the Spanish market (IBEX 35), Greece (ATG), and Italy (FTSE MID). However, the Covid-19 data series (deaths), is synchronized with the Covid-19 data (confirmed cases), with the markets of Germany (DAX 30), France (CAC 40), Ireland (ISEQ), Italy ( FTSE MID), United Kingdom (FTSE 100) and Portugal (PSI 20), just does not synchronize with the Greek market (ATG). We can easily see that the Covid-19 data series (deaths) has a major impact on Europe's financial markets. The results of the VAR Granger Causality / Block Exogeneity Wald Tests model suggest 2 bidirectional causal relationships between confirmed cases and deaths from the Covid-19 virus. However, there were no shocks between Covid-19 data (confirmed cases and deaths) and the financial markets under analysis. As a final discussion, we consider that investors should avoid investments in the stock exchange, at least while this pandemic lasts, and rebalance their portfolios in hedging and/or sovereign debt assets, to mitigate risk and improve the efficiency of their portfolios.

The article deals with the issues of professional psychological selection in the internal Affairs bodies as an integral part of the system of measures to prevent professional deformation in service collectives. The implementation of the measures organized on the basis of monitoring of data received by psychologists of internal Affairs bodies is aimed at maintaining and improving the efficiency of the police. The purpose of this area of work of psychologists is to equip police service teams with reliable employees who meet the requirements of the modern legal state. The presented materials consider the practice of implementing by police psychologists the decree Of the Government of the Russian Federation dated December 06, 2012 No. 1259 «on approval of the Rules of professional psychological selection for service in the internal Affairs bodies of the Russian Federation». The materials are consistent with the results of research conducted by Russian and foreign researchers. The conclusions are based on a comparative analysis of the results of the work of the Commission on psychological selection of the regional Department of the Ministry of internal Affairs. Describes some of the individual characteristics of the persons who were refused admission to the service. Individual factors that hinder the process of effective adaptation in service teams are listed, as well as the role of early maladaptive schemes in the process of adaptation. Individual and environmental factors of influence on the formation and development of deforming influence on employees in the course of performance of official activities are presented, based on the analysis of incidents that occurred in collectives in the period 2018–2020. The article deals with the actual difficulties of practical implementation of psychological selection for service as the basis for early prevention of professional deformation in the service collectives of internal Affairs bodies.

Discovered on 31st December of 2019, the new Coronavirus has a high transmission capacity and was considered pandemic by the World Health Organization. In only six months is was able to spread all over the world and cause more than 600 thousand deaths. Early diagnosis is essential for governments to take public policies, such as social isolation, commerce control, and contact tracking. In order to make these actions, massive tests are required. On the other hand, diagnosis kits are expensive and not accessible to everyone. Medical imaging, such as thoracic x-ray and Computational Tomography (CT) has been used to visualize the lung and to verify at the first moment the presence of viral pneumonia. However, some countries have few radiologists specializing in chest x-ray analysis. The findings in the image are generally not so easy to see and can easily be confused with traditional pneumonia findings. For this reason, studies in Computer Vision are necessary, both to detect anomalies in imaging and to differentiate the other types of pneumonia. This paper addresses the initial results of a research, which developed an image classification methodology to differentiate x-ray images from sick patients, infected with Coronavirus, and healthy patients. The proposed method, based on the extraction and detection of patterns in texture and color features through a Deep Neural Network, obtained an average accuracy of 95% following a k-fold cross-validation experiment.

Abstract:
New variants of concern (VOCs) now account for 67% of all Ontario SARS-CoV-2 infections. Compared with early variants of SARS-CoV-2, VOCs are associated with a 63% increased risk of hospitalization, a 103% increased risk of intensive care unit (ICU) admission and a 56% increased risk of death due to COVID-19. VOCs are having a substantial impact on Ontario’s healthcare system. On March 28, 2021, the daily number of new SARS-CoV-2 infections in Ontario reached the daily number of cases observed near the height of the second wave, at the start of the province-wide lockdown, on December 26, 2020. The number of people hospitalized with COVID-19 is now 21% higher than at the start of the province-wide lockdown, while ICU occupancy is 28% higher (Figure 1). The percentage of COVID-19 patients in ICUs who are younger than 60 years is about 50% higher now than it was prior to the start of the province-wide lockdown. Because the increased risk of COVID-19 hospitalization, ICU admission and death with VOCs is most pronounced 14 to 28 days after diagnosis, there will be significant delays until the full burden to the health care system becomes apparent.

On 8 June 2020, the Government of Tanzania officially declared the country to be free of COVID-19 and all restrictions have since been lifted. As of 3 December 2020, Tanzania had only 509 confirmed cases of the virus and 21 deaths. Nevertheless, neighbouring countries are still facing the threat of the pandemic, all of which are key trading partners. Their continuing COVID-19 control measures have disrupted regional and domestic agricultural markets and affected local livelihoods and food systems. This study analysed the resulting impacts in those systems in several rice-producing communities in Morogoro Region, south-western Tanzania.

Key Message Ontario long-term care (LTC) home residents have experienced disproportionately high morbidity and mortality, both from COVID-19 and from the conditions associated with the COVID-19 pandemic. There are several measures that could be effective in preventing COVID-19 outbreaks, hospitalizations, and deaths in Ontario’s LTC homes, if implemented. First, temporary staffing could be minimized by improving staff working conditions. Second, homes could be further decrowded by a continued disallowance of three- and four-resident rooms and additional temporary housing for the most crowded homes. Third, the risk of SARS-CoV-2 infection in staff could be minimized by approaches that reduce the risk of transmission in communities with a high burden of COVID-19. Summary Background The Province of Ontario has 626 licensed LTC homes and 77,257 long-stay beds; 58% of homes are privately owned, 24% are non-profit/charitable, 16% are municipal. LTC homes were strongly affected during Ontario’s first and second waves of the COVID-19 pandemic. Questions What do we know about the first and second waves of COVID-19 in Ontario LTC homes? Which risk factors are associated with COVID-19 outbreaks in Ontario LTC homes and the extent and death rates associated with outbreaks? What has been the impact of the COVID-19 pandemic on the general health and wellbeing of LTC residents? How has the existing Ontario evidence on COVID-19 in LTC settings been used to support public health interventions and policy changes in these settings? What are the further measures that could be effective in preventing COVID-19 outbreaks, hospitalizations, and deaths in Ontario’s LTC homes? Findings As of January 14, 2021, a total of 3,211 Ontario LTC home residents have died of COVID-19, totaling 60.7% of all 5,289 COVID-19 deaths in Ontario to date. There have now been more cumulative LTC home outbreaks during the second wave as compared with the first wave. The infection and death rates among LTC residents have been lower during the second wave, as compared with the first wave, and a greater number of LTC outbreaks have involved only staff infections. The growth rate of SARS-CoV-2 infections among LTC residents was slower during the first two months of the second wave in September and October 2020, as compared with the first wave. However, the growth rate after the two-month mark is comparatively faster during the second wave. The majority of second wave infections and deaths in LTC homes have occurred between December 1, 2020, and January 14, 2021 (most recent date of data extraction prior to publication). This highlights the recent intensification of the COVID-19 pandemic in LTC homes that has mirrored the recent increase in community transmission of SARS-CoV-2 across Ontario. Evidence from Ontario demonstrates that the risk factors for SARS-CoV-2 outbreaks and subsequent deaths in LTC are distinct from the risk factors for outbreaks and deaths in the community (Figure 1). The most important risk factors for whether a LTC home will experience an outbreak is the daily incidence of SARS-CoV-2 infections in the communities surrounding the home and the occurrence of staff infections. The most important risk factors for the magnitude of an outbreak and the number of resulting resident deaths are older design, chain ownership, and crowding. Figure 1. Many Ontario LTC home residents have experienced severe and potentially irreversible physical, cognitive, psychological, and functional declines as a result of precautionary public health interventions imposed on homes, such as limiting access to general visitors and essential caregivers, resident absences, and group activities. There has also been an increase in the prescribing of psychoactive drugs to Ontario LTC residents. The accumulating evidence on COVID-19 in Ontario’s LTC homes has been leveraged in several ways to support public health interventions and policy during the pandemic. Ontario evidence showed that SARS-CoV-2 infections among LTC staff was associated with subsequent COVID-19 deaths among LTC residents, which motivated a public order to restrict LTC staff from working in more than one LTC home in the first wave. Emerging Ontario evidence on risk factors for LTC home outbreaks and deaths has been incorporated into provincial pandemic surveillance tools. Public health directives now attempt to limit crowding in LTC homes by restricting occupancy to two residents per room. The LTC visitor policy was also revised to designate a maximum of two essential caregivers who can visit residents without time limits, including when a home is experiencing an outbreak. Several further measures could be effective in preventing COVID-19 outbreaks, hospitalizations, and deaths in Ontario’s LTC homes. First, temporary staffing could be minimized by improving staff working conditions. Second, the risk of SARS-CoV-2 infection in staff could be minimized by measures that reduce the risk of transmission in communities with a high burden of COVID-19. Third, LTC homes could be further decrowded by a continued disallowance of three- and four-resident rooms and additional temporary housing for the most crowded homes. Other important issues include improved prevention and detection of SARS-CoV-2 infection in LTC staff, enhanced infection prevention and control (IPAC) capacity within the LTC homes, a more balanced and nuanced approach to public health measures and IPAC strategies in LTC homes, strategies to promote vaccine acceptance amongst residents and staff, and further improving data collection on LTC homes, residents, staff, visitors and essential caregivers for the duration of the COVID-19 pandemic. Interpretation Comparisons of the first and second waves of the COVID-19 pandemic in the LTC setting reveal improvement in some but not all epidemiological indicators. Despite this, the second wave is now intensifying within LTC homes and without action we will likely experience a substantial additional loss of life before the widespread administration and time-dependent maximal effectiveness of COVID-19 vaccines. The predictors of outbreaks, the spread of infection, and deaths in Ontario’s LTC homes are well documented and have remained unchanged between the first and the second wave. Some of the evidence on COVID-19 in Ontario’s LTC homes has been effectively leveraged to support public health interventions and policies. Several further measures, if implemented, have the potential to prevent additional LTC home COVID-19 outbreaks and deaths.
