Peirce’s essay “On a New List of Categories,” presented in 1867, apparently anticipates his later work in phenomenology. Some scholars have regarded it as the keystone of Peirce’s philosophy, but others view it merely as a stepping stone to his mature work. In that early essay, Aristotle’s analysis of the proposition and Peirce’s discovery of three specific functions of the copula result in his identification of quality, relation, and representation as the three categories. He would have to abandon this account as his development of the Existential Graphs and study of other grammars led him to reject the Aristotelian theory of the proposition and to regard the copula as logically inessential.

This chapter explores three central questions. The first section describes the four core varieties of capitals: cultural, social, economic, and symbolic. It highlights some peculiarites of Bourdieu’s approach: the focus on symbolic capital, a definition of social capital different from those of North American. A second section questions the limits of economic metaphors. Bourdieu borrows from the lexicon of economics (capitals, conversion rates). But he devotes attention to the sociopolitical dimension of the struggles for defining the conversion rates between capitals and warns that “rational” actions are one historical dimension of a complex space of “reasonable” actions. Finally, the chapter discusses the question of the number of capitals. Should one add to Bourdieu’s list something like a bodily or erotic capital? If each field values a specific capital, should researchers produce an endless list of specific capitals, or are these specific capitals always combinations of the four basic ones?

Much of the court-martial docket involves cases concerned with the same kinds of criminality seen in the civilian courts. ‘Conduct unbecoming and all that’ explains that the docket also includes very different offenses that at times raise thorny questions. Many military codes list, alongside specific offenses, a “general article” that prohibits conduct prejudicial to good order and service discipline and conduct of a nature that brings discredit on the service. This clause covers a multitude of sins and provides ample fodder for litigation. Two other offenses, unique to the military, and not applicable to enlisted personnel, are “conduct unbecoming an officer and a gentleman” and speaking contemptuously of the president and other high officials.

There is considerable overlap of musculoskeletal manifestations of both rheumatological and endocrine disorders. Apart for the well-described autoimmune basis governing the aetiopathogenesis of clinical states pertaining to specific conditions affecting both systems, there is more recent evidence on the overlapping biology and genetics of these organ systems. Not uncommonly, endocrine manifestations can herald the initial presentation of rheumatological illnesses and the converse holds true for endocrinopathies. This review discusses the common endocrine presentations associated with rheumatic illnesses in relation to newer information gleaned from population studies, genetic studies, and clinical presentations. If a pregnant woman has not had pre-conception counselling, it is important to ensure as soon as possible in early pregnancy that she understands her nutrient requirements, and what comprises a balanced diet and healthy body composition. The advice should include weight gain goals in pregnancy, a list of unsafe foods, healthy exercise behaviours, and the effects of alcohol and drugs. Energy intake needs in early pregnancy do not increase from pre-pregnancy levels; women should focus on eating well for pregnancy, and not eating more. Specific attention should be paid to intakes of folate, vitamin A, vitamin B12, vitamin D, iron, and iodine. Glucose status should be monitored in mid-pregnancy. Energy requirements increase in mid- to late pregnancy, but attention should also be paid to the macronutrient balance. If the diet is low in oily fish as a source of polyunsaturated fatty acids, supplementation should be considered.

It also provides an overview of global cancer patterns and trends. Epidemiological observations indicate that cancer development and progression is due to an interaction of environmental exposures with genetic factors. This underscores the importance of using complementary epidemiological measurements to obtain a cohesive and comprehensive panorama of cancer burden. Manifold measurements that capture the number of deaths, incidence/mortality rates, and time trends with respect to variations between countries, regions, and risk factors must be considered. Efforts to quantify the impact of cancer are limited primarily by the fact that only a small proportion of the global population is covered by cancer registries. Collectively, neoplasms are the second largest cause of death worldwide and deaths from site-specific cancers ascended the causes of death list in both low- and high-income countries.

This chapter describes common issues along the transplantation journey from assessment to common conditions that are diagnosed post transplantation. Assessment for transplant suitability against several objective criteria is covered as well as the importance of optimizing techniques prior to this. Recent advances mean that some patients can now be bridged to transplant using extracorporeal membrane oxygenation (ECMO) when previously they would have been removed from the transplant list. Drawbacks to ECMO are discussed. Ex-vivo lung perfusion (EVLP) of a donor organ is covered. Follow-up is considered, especially in the early phase whilst being stabilized on their new medications as well as monitoring for the development of lung rejection (acute and chronic). These conditions often present when patients are being seen away from the transplant centre. CF patients have the best outcomes of the groups after lung transplantation, emphasising that lung transplantation should be considered in this specific group of patients.

This book aims to create a new venue for evidence-based practice in psychotherapy—a venue that goes beyond the traditional and unidirectional dissemination of research, whereby clinicians are typically viewed as passive recipients of scientific findings. In contrast, this book is the result of an active, intense, and bidirectional collaboration of psychotherapy researchers and practitioners. Based on an extensive review of literature, it first offers a list of 38 empirically based principles of change that are clustered within five categories: client prognostic, treatment/provider moderating, client process, therapeutic relationship, and therapist interventions. It then illustrates the expertise of six therapists from diverse theoretical orientations who describe how they implement each of these principles with specific cases of depression and anxiety disorders (with or without substance abuse or personality disorder). The book also includes exchanges between researchers and clinicians on several issues regarding the current list of principles of change, such as how similarly and differently they are addressed or used across a variety of treatments, how helpful they can be in clinical routine (and/or under which situations they may not be clinically valid), how they may be combined for particular purposes (such as teaching and training), and how the list can be expanded to guide future research based on clinicians’ observations and reflections. This book is an attempt to advance psychotherapy by having researchers and clinicians share their unique and yet complementary knowledge. It also lays the foundations for further collaborations and partnerships between different stakeholders in mental health services.

A nephrologist should have the generic skills of a consultant general physician, plus theoretical knowledge, and skills in nephrology. Moving on from a purely apprenticeship model, in the last 40 years several countries have listed the desirable knowledge and skills content for a specialist nephrologist in their jurisdiction. Some of these lists are exhaustive. Assessments of competence often include a written test of knowledge, and workplace based assessment or reports on specific skills or overall performance. We list some of the common generic headings used to define these areas of competence, and point to some curricula for specialist nephrologists. We also summarize methods of learning and point to resources which are available internationally, particularly online. The majority of the numerical need for additional nephrologists globally is in regions with fewer physicians, and fewer resources for their education. The ISN has proposed a global curriculum and identified how this might be tailored to regional needs. We might all help with producing support learning internationally, and we point to some good sources below.How to best support the development of nephrologists in these regions without unintentionally attracting them away from places of greatest need is an important question.

In a conservation and management section, the authors identify and evaluate the nature and intensity of factors which threaten river turtle survival--almost all of which involve direct human exploitation or indirect effects of human induced habitat alteration and degradation. They then list and evaluate the various schemes which have been proposed or employed to halt declines and restore populations, and make recommendations for future management plans for specific species and regions. In closing, they state their viewpoint concerning future research directions and priorities, and an evaluation of future prospects for survival of the world's river turtle species.

Using new knowledge derived from a survey among firms from various industries and several case studies, this book seeks to give us better understanding of ‘how’ firms can innovate their business model, ‘what’ kind of levers management should work on, and ‘when’ management should change the business model. It particularly considers one key question: is it better to replicate existing models or develop new ones? Renewal is especially vital in highly competitive environments. Nonetheless, whatever the environment, high levels of both replication and renewal will be key for a firm to succeed. This book looks at four levers that can be used by managers to innovate their business model: management itself, organizational form, technology, and co-creation with external parties are analysed. Furthermore, specific combinations which strengthen business model innovation are analysed. To help firms, the book also explores the different factors that can either enable or inhibit business model innovation. Various disorders result from genetically determined abnormalities of enzymes, the metabolic consequences of which affect the development or functioning of the nervous system. The range of metabolic disturbances is wide, as is the resultant range of clinical syndromes. Although most occur in children, some can present in adult life, and increasing numbers of affected children survive into adult life. In some, specific treatments are possible or are being developed. The last 20 years has seen a considerable expansion in our understanding of the genetic and metabolic basis for many neurological conditions. Particular clinical presentations of neurometabolic disorders include ataxias, movement disorders, childhood epilepsies, or peripheral neuropathy. Detailed coverage of the entire range of inherited metabolic diseases of the nervous system is available in other texts (Brett 1997; Scriver et al. 2001; Menkes et al. 2005).Treatment is possible for some metabolic diseases. For instance, the devastating neurological effects of phenylketonuria have been recognized for many years. Neonatal screening for this disorder and dietary modification in the developed world has removed phenylketonuria from the list of important causes of serious neurological disability in children. This success has led to new challenges in the management of the adult with phenylketonuria and unexpected and devastating effect of the disorder on the unborn child of an untreated Phenylketonuria mother. More recently Biotinidase deficiency has been recognized as an important and easily treatable cause of serious neurological disease usually presenting with early onset drug resistant seizures. This and some other neurometabolic diseases can be identified on neonatal blood screening although a full range of screening is not yet routine in the United Kingdom. More disorders are likely to be picked up at an earlier asymptomatic stage as the sophistication of screening tests increases (Wilcken et al. 2003; Bodamer et al. 2007).Although individual metabolic disorders are rare, collectively such disorders are relatively common. In reality most clinicians will see an individual condition only rarely in a career. Furthermore, patients with certain rare conditions are often concentrated in specialist referral centres, further reducing the exposure of general and paediatric neurologists to these disorders. A recent study into progressive intellectual and neurological deterioration, PIND, gives some information about the relative frequency and distribution of some childhood neurodegenerative diseases in the United Kingdom (Verity et al. 2000; Devereux et al. 2004). Although primarily designed to identify any childhood cases of variant Creutzfeldt- Jakob disease, the study also provided much information about the distribution of neurometabolic disease in children in the United Kingdom. The commonest five causes of progressive intellectual and neurological deterioration over 5 years were Sanfilippo syndrome, 41 cases, adrenoleukodystrophy, 32 cases, late infantile neuronal ceroid lipofuschinosis, 32 cases, mitochondrial cytopathy, 30 cases, and Rett syndrome, 29 cases. Notably, geographical foci of these disorders were also found and correlate with high rate of consanguinity in some local populations.
