As an important and independent predictive factor of morbidity and mortality, right ventricular (RV) function should be carefully assessed in patients with chronic obstructive lung disease, lung fibrosis, liver cirrhosis, or obesity. RV assessment requires a complete study of the ‘RV-pulmonary circulation unit’ with estimation of RV preload, RV intrinsic contractility, and RV afterload. Therefore, estimation of pulmonary arterial pressure, pulmonary vascular resistance, and left ventricular systolic and diastolic function should be included in this evaluation, in addition to conventional RV systolic function assessment. Three-dimensional echocardiography has emerged as an interesting tool in RV assessment and exercise echocardiography may be interesting in the risk stratification of patients.

This chapter shows that the Main Lemma implies the main theorem. It proves Theorem (10.1) by inductively applying the Main Lemma in order to construct a sequence of solutions of the Euler-Reynolds system. At each stage of the induction, an energy function is chosen along with a parameter whose choice determines the growth of the frequency parameter and the decay of the energy level. A base case lemma is then established, after which the proof of the Main Theorem (10.1) is presented so that the Main Lemma implies the Main Theorem. The Main Lemma is employed to approximately prescribe the energy increment of the correction. The solution obtained at the end of the process is nontrivial.

African American entrepreneurship has been an integral part of the American economy since the 1600s. On the eve of the Civil War, the collective wealth of free blacks was approximately $50 million. In 2006, African Americans earned a whopping $744 billion, a figure that exceeds the gross domestic product of all but 15 nations of the 192 independent countries in the world. As W. Sherman Rogers so ably demonstrates, African Americans have achieved these economic gains under difficult circumstances. Slavery, segregation, and legally limited access to property, education, and other opportunities have taken a heavy toll, even to this day. Besides providing a penetrating glimpse into the world of black entrepreneurship both past and present, this book urges African Americans to gain financial independence as entrepreneurs. This chapter reviews normal blood function and disorders of haemopoiesis. Blood consists of cells of three main types, suspended in plasma. The cellular component comprises about 40%–50% of the total volume and consists of red cells (erythrocytes), white cells (leucocytes), and platelets. Blood cells are formed from progenitor cells in the bone marrow by haemopoiesis, a process of proliferation and differentiation. Failure of haemopoiesis usually results from damage to proliferating marrow cells by cytotoxic drugs or radiation, haemopoietic malignancy, or a combination of the two.

This chapter describes the different modalities for assessment of systolic function by transthoracic echocardiography. Firstly, the basic principles of physiology and the determinants of left ventricular (LV) performance are considered, followed by a systematic appraisal of the methodologies for global LV systolic function assessment. Starting with M-mode echocardiography, passing through the traditional two-dimensional echocardiography evaluation to three-dimensional echocardiography approaches, main strengths and limitations are described. Power Doppler usefulness, regarding stroke volume calculations and dP/dt measurement are summarily explained, taking into consideration the usual pitfalls found in daily practice. There is a section dedicated to regional systolic function evaluation, with the recommendations for standardized LV division and differential characteristics of wall motion abnormalities. Additionally, more recent approaches with tissue Doppler imaging and strain analyses for global and regional LV function assessment are described. Finally, a section is dedicated to right ventricle systolic function which describes all modalities of evaluation.

The central thesis of the book is that in order to evaluate monetary policy, one should have a clear idea about the characteristics and functions of money as it evolved and in its current form. That is to say that without an understanding about how money evolved as a social institution, what it is today, and what is possible to know about monetary phenomena, it is not possible to develop a meaningful ethics for money; or, to put it differently, to find what kind of institutional arrangements may be deemed good money for the kind of society we are in. Many systemic diseases, however, may affect the cardiac valves and not infrequently, echocardiography may be the first clue to a systemic illness. In our times, changing features of the workplace environment provide opportunities to focus on the latter two goals, and to liberate education from supposed economic constraints. By doing so, we can improve the lives of individuals, and build more solid foundations for democracy. Philip Kitcher’s humanistic vision of educational reform is not, however, divorced from the realities of contemporary life nor doomed by any conflict with sound economics. After an accessible discussion of central philosophical questions, he examines the content of the curriculum, identifies the social changes required if a fully adequate education is to be provided to all, and considers how the proposals can be reconciled with financial stability. How children obtain an understanding of mental states in others—“mindreading” or “theory of mind” (ToM)—during their cognitive development is a major concern in developmental psychology. There is also much debate about and empirical research on the developmental relationship between ToM and the set of processes that monitor and control thoughts and actions, i.e., executive functioning (EF). Until recently, little was known about the cross-cultural variation of both concepts. This chapter presents empirical findings on these concepts and takes a metacognitive perspective to clarify their relationship. A series of cross-cultural studies have been undertaken to specify the relationship between EF and ToM by verifying assumptions about the quality of conflict inhibition necessary for the development of ToM’s key aspect, false-belief understanding. The main argument is that an experience-based view of the metacognitive mechanisms involved might give a more parsimonious explanation of their relationship and their cultural variations.

This chapter discusses financing devices that, although performing an equivalent function to security interests, are not generally considered ‘security’ under English law. These devices fall into two main categories: those involving the retention and those involving the transfer of title. Such financing devices are often called ‘quasi-security’ interests, to acknowledge that their economic function, in ‘securing’ the performance of obligations, is the same as that of true ‘security’. The chapter also considers the characteristics that they normally display, dividing up the discussion by the nature of the collateral involved: goods, investment products, and receivables.

The placenta is a complex and changing organ necessary for normal fetal growth and development and for maintenance of a healthy pregnancy. It has three major functions: a protective function of the fetus, an endocrine function, and a metabolic function. The main functional unit of the placenta is the chorionic villous, responsible for the majority of the fetal–maternal exchanges. Migration of trophoblastic cells induces a remodelling of the uterine arteries, with vasodilatated and compliant vessels, unresponsive to maternal vasomotor control. Therefore, any significant change in maternal blood pressure, in particular in the context of general or regional anaesthesia, can directly impact on uteroplacental perfusion. Most anaesthetic drugs cross the placental barrier, but without significant consequences on the fetal well-being.

Higher-function words such as complementizers, negation, functional prepositions, definiteness particles, comparative markers, and so forth, occurring to the left of their lexical category, are argued to be base-generated as complex left branches, rather than spelling out the main functional sequence. This is generalized to all (base-generated) pre-asymmetries and post-asymmetries and derived from the structure of the lexical entries of the function words, dispensing with idiosyncratic notational devices equivalent to [+ suffix] or [+ needs-to-move]. These complex left branches require a merge-XP operation, and the place of this operation in the algorithm of spellout-driven movement is discussed.

Sexual stimuli (tactile, visual, olfactory, and imaginative) are processed and integrated in the central nervous system which then activates certain autonomic and somatic pathways within the peripheral nervous system. This coordinated activation of the central and peripheral nervous systems leads to penile erection which is actually a result of relaxation of vascular and cavernosal smooth muscle in the penis. In the flaccid (detumescent) penis, the smooth muscle tone is heightened. Penile erection (tumescence) requires a decrease in the smooth muscle tone. The tone of the penile smooth muscle therefore is the main determinant of erectile function. In this chapter, the current information on the control of erectile function by this central-peripheral-smooth muscle axis will be reviewed.
