Full textBackground: The National Board of Health and Welfare estimates that 110 000 patients or 8% of all patients in the somatic hospital care are affected by health care errors. A health care error is an avoidable damage and patient safety are defined to protect patient against damage. An operation theatre is a complex environment with advanced technical equipment. Interdisciplinary groups of health professionals work together in teams to deliver safe care for the patients. The operating theatre nurse shall work to give the patient safe perioperative nursing. The core of their work is to ensure patient safety during surgery, managing risks and preventing harm. However, to analyze and illuminate the responsibility of the operating theatre nurses work to secure patient safety during perioperative nursing seems to be scarcely investigated.Aim: The aim of this study was to illuminate the operating theatre nurses perceptions of patient safety in perioperative nursing in a Swedish context.Method: Qualitative semi structured interviews were conducted with 14 operating theatre nurses in a hospital in the South of Sweden. The data was analyzed using content analysis.Results: These categories were identified in the results; clinical competence, communication, care environment and skills development with eleven subcategories. These categories reflected how the operating theatre nurses perceived patient safety related to their role and responsibility in perioperative nursing.Conclusion: The result indicates that these operating theatre nurse perceive that the main responsibility in perioperative nursing is hygiene and to preserve sterility. Furthermore, work to prevent health care error and to give the patient safe care. There are circumstances in the organization of the operation theatre, in the team and lack of respect for the operating theatre nurses competence that complicates that work.

Full textKirurgiska processer består av ett antal komplext inbördes relaterade faktorer som inkluderar individuella färdigheter, teamarbete och miljön i operationssalen. Individuella färdigheter kan delas till tekniska och icke-tekniska färdigheter. Denna åtskillnad mellan tekniska och icke-tekniska färdigheter är ganska ny i omvårdnadslitteraturen. Det har genomförts studier där man har bedömt icke-tekniska färdigheter som utförs i operationssalen men studier för att bedöma operationssjuksköterskans tekniska färdigheter har saknats fram till nyligen. Syftet med denna studie är att utvärdera den kliniska tillämpligheten av ett nyutvecklat observationsverktyg i olika kontext och genom att observera kunna beskriva likheter eller skillnader mellan tekniska färdigheter. Observationerna i den aktuella studien är strukturerade med ett nyutvecklat observationsverktyg som är utvecklad för att bedöma en operationssjuksköterskas tekniska färdigheter. De två författarna till denna studie genomförde datainsamlingen samtidigt på olika operationsavdelningar. Den aktuella studiens resultat visar att de tekniska färdigheterna som krävs av en operationssjuksköterska är betydligt olika i två olika länder. Därför är det svårt att använda ett observationsverktyg utan att anpassa det enligt de lokala rutinerna.

Full textIntroduction: Theatre nurses in perioperative care often meet children who are to undergo surgery. Requirements for production and routines govern that the first meeting takes place with the anesthesia nurse and thus the theatre nurse meeting with the child becomes more limited. This can lead to difficulties in creating a care relationship with the child and in planning the care based on the child's needs. Aim: To describe theatre nurses experiences of nursing directed to children in relation to perioperative care. Method: A qualitative descriptive study was conducted based on a convenience sample and 13 active theatre nurses were interviewed. The interview material was analyzed with a qualitative content analysis according to Graneheim & Lundman (2004). Results: The analysis of the interview material resulted in three categories: The importance of adapting care based on production requirements, the importance of communication and the importance of experience and competence. The categories described the theatre nurse´s different experiences of their care for children in relation to routines adapted to children's needs, collaborations and experiences in perioperative care. Conclusion: The study showed that theatre nurses felt that nursing was governed by production, time and resources, which could lead to the child's best interests and safety being jeopardized. They used creative solutions, compassion, competence, cooperation and parental support. They experienced a need for education and improvement work.

Full textIn according to organ transplantation, a tissue and organ procurement will be completed where organs will be removed from a donor. Prior to this intervention the patient will be taking care of in an intensive care unit, where there is excellent access to research regarding how the intensive care staff would experience their participation and caring of a potential donor. This though is in contrast to the lack of research into how the perioperative staff experiences their participation in the removal of organs from a donor. The aim of the study was to describe the perioperative nurses' experience when participating in the tissue and organ procurement of a deceased person prior to organ transplantation. The study was carried out as a qualitative, descriptive study and content analysis. Three nurse anesthetists and three theatre nurses at a west Swedish hospital were interviewed. The results showed that interventions of this type follow the routine for an operation with a pre- intra- and postoperative phase. Three categories emerged in the study: Preparation before the operation; Teamwork; and Processing of the experience. The conclusion was that the preparation for the procedure ought to be improved. Debriefing after the operation functions well, but could be improved. By discussing more openly about ethically difficult situations, such as tissue and organ procurement and the stress they induce, the perioperative nurses are hereby given opportunities to create their own coping strategies.

Full textBackground: Today is a shortage of operating theater nurse in Sweden, which has resulted in several operational departments are unable to engage fully operational business. Experienced operating theater nurse will retire in the next few years while the interest to train as a operating theater nurse has been neglected. The lack of competent operating theater nurse may worsen if no measures implemented to change the situation, which can have serious consequences, such as the number of operations must be further reduced or canceled, which would contribute to the suffering of the patient and an increased workload on existing staff. Aim: The aim of the study was to describe nurses perceptions of operating theater nurse profession. Method: A qualitative study with phenomenographically was performed. A strategic choice was conducted, which resulted in nine individual interviews nurses at a hospital in southern Sweden. Results: The analysis resulted in four categories: An interesting profession, A demanding profession, A technical profession and An invisible profession. Each of the categories consisted of two categories. Conclusion: Nurses have inadequate knowledge and understanding of operating theater nurse profession. Increased clinical experience and information about the operating theater nurse profession could create an increased interest in or facilitate the election of a future operating theater nurse education.

Full textThis study is a theoretical explication of an idea of a theatre called Theatre of the Contact Zone. Its main feature is the collaboration between the playwright and the director in the transformation of dramatic texts into performance texts. Within the pragmatics of this theatre the playwright's initial task is to provide a working script. It is only a foundation, the basis, without which the director has nothing to begin coordinating the collaborative process of theatre making. The writing of the script continues through out rehearsals. The final script is compiled after the production incorporating changes made in the course of its transformation into a performance, as well as insights gained through watching the production. The first experiment was through a play called An African Syzygy which I wrote and was directed by Sanjin Muftic (a fellow postgraduate student whose orientation in theatre studies is directing).

Full textBackground: In an operating theatre teamwork is important, partly to get the job done, but also to ensure patient safety. An important aspect of teamwork is that perioperative nurses feel that the other members of the team understand their work assignments. Aim: The aim of this study was to highlight perioperative nurses' experiences of teamwork based on an understanding of their work assignments. Method: A qualitative inductive interview study was conducted with 16 perioperative nurses from three small to medium-sized hospitals in southern Sweden. The data were analyzed based on a qualitative latent content analysis. Results: The study shows that there are several aspects that affect teamwork and nurses feel that although understanding of their work assignments from other team members usually are good, there are possibilities for improvement. The overall theme independence requires participation emerged from the result, as well as the categories profession, collaboration, work environment and patient safety with related subcategories. Conclusion: To give perioperative personnel an insight into what might facilitate teamwork can be beneficial. Further research on the topic is required, but it is also important that schools and workplaces practice teamwork to increase knowledge and understanding between different professions.

Full textThe theatre nurse works in a technologically advanced environment and works actively to prevent risks and health-related injuries by planning and documenting the patient's care based on a nursing process. Missing or deficient documentation of tacit knowledge that can adversely affect patient safety as performed actions are not traceable in the patient's journal and invisible to the theatre nurses’ expertise in nursing. Aim: The aim was to describe the theatre nurses’ experiences of documentation of perioperative nursing from patient safety. Method: Qualitative method with inductive approach and fifteen semi-structured interviews were conducted. The material was interpreted based on conventional content analysis. Result: The material resulted in three categories: Technical conditions, A personal and shared responsibility, and the Meaningfulness of the documentation. The categories describe the operations nurse’s experiences regarding documentation with different emotions and improvement suggestions are raised and reported. Conclusion: The study shows that the theatre nurses’ documentation can be reinforced in the form of verbal reporting to the post-anaesthesia care unit to strengthen and maintain patient safety, and to make visible the nurse competence for nursing where tacit knowledge emerges. Much of the routines around the documentation are related to the culture in the workplace, which the theatre nurses were aware of and wanted to change. It also turned out that the documentation was something that the operating nurse herself could influence by prioritizing and creating good conditions for and collaboration with the assistant on ward.

Full textBackground: It requires a good and deliberated introduction to the newly graduated theater nurses to begin to work in the new workplace. Aim: The aim of this pilot study was to describe newly graduated theatre nurses' experiences of the introduction to the new professional role. Method: Four theatre nurses with 9 months to 21 months of experience in the theatre nursing profession were interviewed. The interviews were analyzed by qualitative content analysis. Results: Planning and structure where it turned out that there was a plan for introduction of newly graduated theatre nurses, but it differed depending on the operating theatre. Personal responsibility where it was shown that as time passed it was the nurses´ own responsibility to find out what was necessary to learn. Colleagues' expectations where the newly graduated theatre nurses experienced that after a time into the introduction they felt stressed to meet the unspoken expectations from both surgeons and theatre nursing colleagues to know and understand things they had not been shown. Collegially support where it was shown that the newly graduated theatre nurses felt acceptance from colleagues when they had lots of questions and concerns in the new professional role. The results of the pilot study can be helpful in surgical wards in the development of procedures to make the introduction of new graduate theatre nurses to be as optimal as possible.

Full textIntroduction: Patients may suffer from medical injuries associated with surgery positioning. There is the theatre nurse together with the operating team who is responsible for the patients positioning. The aim of the study was to describe theatre nurses perceptions of nursing interventions conjunction with the positioning of the patient in leg bearer. Method: The study was conducted using qualitative methodology and data collection method was individual interviews. Data processing was carried out with qualitative content analysis. Results: The analysis resulted in two categories with the associated six subcategories. The first category shows that health interventions performed by the theatre nurses is adopted for the patients’ physical conditions and needs. The second category shows that theatre nurses take responsibility for and control the positioning that it is carried with the best possible relief of pressure and tension are avoided. It proves that guidelines serves an important function to prevent the time in leg bearer from being exceeded. It is important that the operating team working together to find solutions. Conclusion: This study shows that for a successful positioning in the leg bearer need to take account of patient's body conditions and needs. A pre- and intraoperative dialogue provides knowledge about the patients’ needs prior to positioning. The study shows that theatre nurses take responsibility for and make sure that positioning is carried out properly and monitor the time in restricted following the guidelines. They control the positioning before draping is done and has a readiness to act throughout the surgery to protect the patient. When limitations to lie in the leg bearer the position are tried out together with the patient before spinal anesthesia is given to prevent damage. The study further shows that the patient's participation is important for achieving the best possible relief in connection with the positioning and that the whole operating team collaborates and contributes with new flexible solutions to problems.

Full textBackground: There is a shortage specialized nurses both national and international which leads to increasing waiting time for surgery and a suffering for the patient. The lack of operating theater nurses makes it important to examine how the profession is seen and if it needs to be illuminated to make it more interesting. Purpose: The purpose was to examine nurses' perceptions of the operation theatre nursing profession. Method: A descriptive cross-sectional study with a quantitative approach containing qualitative elements based on web survey with open and closed response options. Result: Nurses consider the knowledge of the operating nursing profession to be satisfactorily. The profession is considered as important, physical demanding and unknown for nurses outside the operating ward. Conclusion: The operating theater nurse is considered as important for the patient safety and is an important part of the surgical team. Despite that nurses consider themselves having knowledge of the profession the operating theater nurse is considered a hidden profession and there is a need to further make the profession visible.

Full textBackground: Trauma care is included in Swedish healthcare services. A patient in need of trauma care is treated by the severity of injuries. According to national guidelines trauma is divided into two different levels, level-1 being the most severe that often require lifesaving efforts. Operating theater nurses is one of the team members that is part of the operative care. Professional competence is required along with stress resistance in the pre- and intraoperative care. The aim: The aim of the study was to illustrate experiences of operating theater nurse´s care of patients with trauma level-1 with focus on qualifications. Method: A qualitative method was used where the data material was analyzed with qualitative content analysis. The data is based on interviews with ten operating theater nurses from a University hospital and a regional hospital. Results: One main theme with three underlying categories emerged. Operating theater nurses participation in provided care and sense of safety is an overall theme that contains three categories which are personal and professional resources, learning resources and organizational resources, each with associated subcategories. Conclusion: Participation in the trauma care, sense of safety and a variety of learning sources was evident as qualifications for care. Structure and leadership was further conditions that emerged as important parts for collaboration that enhances the possibility for the operation theater nurse to perform trauma care level-1.

Full textIntroduction: We are living in a multicultural society, which puts great importance on the theatre nurses´ care for patient´s security during surgery. Aim: To investigate the theatre nurses' experiences of language- and cultural issues in the perioperative care. Method: Qualitative method with inductive approach was applied and data was collected through individual interviews with ten surgical nurses, eight women and two men, at three hospitals in the Midwest part of Sweden. A qualitative analysis of the content was used to process the interviews. Results: The analysis resulted in three categories and eight subcategories. The three categories were: Theatre nurse in multicultural health care, Communication and Communication Aids. The result showed that patient's wishes were considered if they did not affect the safety of the patient. Operations in the intimate parts of the body felt harder than others. Theatre nurses felt restricted when they could not communicate with the patient. The patient own personal experience of information prior to the surgery wasn´t satisfactory. It was considered safer to use a professional interpreter than a family member as an interpreter. There were requests from the theatre nurses for communication aids. Conclusion: The study revealed that language- and cultural issues affect the theatre nurse perioperative work. They experienced difficulties in both collecting and disseminating information that had significance for perioperative patient care and patient safety.

Full textBackground: The transfer of information within perioperative care, is a continuous process. When a patient, together with the relevant patient-information is transferred to a new carer and care unit, there is considered to be an element of risk involved. Continuity, quality, efficiency and patient safety will be achieved when the operating theatre nurse initiates and implements an adequate transfer of information, in connection with the patient’s surgical procedure. Aim: To illustrate how perioperative nurses experience the pre and postoperative transfer of information. Method: A systematic literature review in accordance with Bettany-Saltikov and McSherry (2016). The result is based on an analysis of twelve qualitative, scientific articles. Results: The study results demonstrate that there are several deficiencies during the transfer of perioperative information, and that it is perceived as complex. The results are presented in three main categories; Scattered and inadequate information, Prerequisites for information transfer and The human influence on information transfer. Conclusion: Based on the experiences of the perioperative nurses, there are many factors which influence the pre and postoperative transfer of information. As members of the surgical team do not always participate in the postoperative patient handover, there is a risk that surgical information may be lost. There can be negative consequences for the patient when the transfer of information is inadequate.

Full textIntroduction: Preventive and correctly taken nursing measures is part of the operating theatre nurse´s work. The operating theatre nurse´s work with the perioperative process by planning and implementing the nursing measures and evaluating them. Feedback has been shown to improve the care and develop the profession. There are few studies in the literature about the operating theatre nurses experience of feedback during the perioperative process. Aim: The aim of this study was to describe the operating theatre nurse´s experience of feedback and the importance of the feedback for the development of the profession. Method: A qualitative study with semi-structured questions has been analyzed with a qualitative content analysis. 11 operating theatre nurses in the age 26-60 years with the experiences an operating theatre nurse from 7 month to 36 years participated, all were women. Results: The operating theatre nurses experienced that it would be significant with more feedback, both for the development of the profession and to improve the evaluation of taken nursing measures. All feedback was positive. Lack of cooperation between care units affected the possibility of feedback negatively. The lack of feedback led them to seeking feedback of nursing measures by themselves. Conclusion: Through feedback the operating theatre nurses can get answer of their taken nursing measures. The feedback to the operating theatre nurses need to be implemented and develop in the daily work.

Full textBackground: Unexpected events in the operating room affect the theatre nurse in different ways. Tools for reducing and eliminating unexpected events and the negative consequences of these events, have been developed through the WHO's checklist for safe surgery, education and team training. Despite these tools, unexpected events still happen continuously. The theatre nurse shall work for identifying and reducing unexpected events related to surgical procedures, in order to avoid healthcare injuries and reduce patient suffering. This is the foundation of working for safe care for the theatre nurse. Purpose The purpose of this study was to illuminate theatre nurses' experiences and handling of unexpected events in the operating room. Method: The design of the study is qualitative with an inductive approach. Eight theatre nurses were recruited and individual semi-structured interviews were conducted from two operating wards. A qualitative content analysis according to Erlingsson & Brysiewicz (2017) was conducted. Result: Three overarching categories including subcategories describing theatre nurses` experiences and handling of unexpected events emerged after the analysis; To encounter an unexpected event, Valuable conditions for managing unexpected events and Teamwork as a tool. In To encounter an unexpected event, various events emerged that were considered unexpected and emotions associated with these events. In Valuable conditions for managing unexpected events experience, conversation, preparation, focusing and routines were important parts. Collaboration, leadership and communication emerged as important concepts in Teamwork as a tool. Conclusion: Unexpected events are natural parts in the everyday life of the theatre nurse. How these events are experienced and handled depend on a number of different factors, such as previous experience, cooperation and the opportunity for conversation. This study shows that teamwork is important for optimal handling of unexpected events, and thereby not risk patient safety.

Full textBackground: Today’s advanced surgery demands the theatre nurse to be highly skilled and competent. The theatre nurse is working in a constant developing environment. Her specific knowledge of perioperative care makes the theatre nurse a key-player in the operating team. The newly graduated theatre nurse lacks deeper competence in both working experience and professional skills that could help her meet the highly set standards in surgery. Aim: The aim of this study was to examine which challenges newly graduated theatre nurses experience and which coping strategies they use. Method: Six newly graduated theatre nurses working in an operating setting, with six to twelve months of working experience, were interviewed. The narrative data was analyzed with qualitative content analysis. Results: Being a newly graduated theatre nurse with lack of previous working experience created mixed feelings about the profession and the role. The experiences were characterized a satisfaction in learning and developing skills but at the same time experiencing feelings of being overcome by work burden. The challenges they encountered were entering the role of an operation theatre nurse, allowing one self to gain competence and accepting the situation of being a novice. Adjusting to the situation by adopting different strategies became a way of coping with the experienced challenges. Conclusion: Gaining access to the role of becoming a competent theatre nurse can be supported by acknowledging the challenges experienced by newly graduated theatre nurses and by mentoring their progress to prevent negative feelings such as insecurity and stress.

Assisting in traumatic surgery outside the surgical ward and in an unfamiliar environment can be a challenge. Purpose: The purpose of the study is to investigate the experiences of the theater nurse, when participating in emergency surgical procedures outside the operating ward. Method: As a method, qualitative study was used with semi-structured interviews. Thirteen theater nurses in two different hospitals were interviewed. The data analysis was done through one with an inductive qualitative analysis. Results: The study resulted in a theme: a member of a team with complete focus, and three categories; To perform under pressure, aggravating circumstances and priorities in the urgent stage. The categories were divided into eight sub-categories. Conclusion: The theater nurses experienced a high pulse adrenaline rush when participating in emergency surgical procedures outside the operating ward. They also felt that the teamwork worked well and that a good communication was important in emergency surgical procedures in a messy and chaotic situation. The difficulties the theater nurses experienced were related to performing unfamiliar tasks in an unfamiliar work environment.

Full textBakgrund Världsalliansen för patientsäkerhet, som är en del av WHO, har skapat en checklista för säker kirurgi. Den publicerades för första gången år 2008 och i Sverige har Landstingens Ömsesidiga Försäkringsbolag, LÖF, översatt och bearbetat checklistan för svenskt bruk. Användandet av WHO:s checklista för säker kirurgi har visats minska antalet komplikationer och dödsfall i samband med operationer. Syfte Syftet med studien var att beskriva operationssjuksköterskans medverkan i användandet av WHO:s modifierade checklista för säker kirurgi intraoperativt på två operationsavdelningar på ett sjukhus i Mellansverige. Metod Denna studie har en kvantitativ deskriptiv ansats. En direkt observationsstudie genomfördes och därefter ställdes frågor utifrån en frågeguide. Totalt 16 operationssjuksköterskor deltog i studien. Resultat Ingen operationssjuksköterska medverkade i eller tog upp samtliga punkter i checklistans alla tre faser under en operation. En operationssjuksköterska tog däremot upp alla punkter i avslutningsfasen vid ett tillfälle. Det fanns ingen checklisteansvarig personal som läste upp checklistans alla punkter för operationslaget och som såg till att alla punkterna genomfördes på de båda avdelningarna. Slutsats Bristande följsamhet till checklistorna råder på båda avdelningarna. Avdelningarna är i behov av kvalitetsförbättrande åtgärder för att öka följsamheten till checklistorna för att främja patientsäkerheten. Klinisk betydelse Studien är en beskrivning av hur operationssjuksköterskor arbetar med checklistorna på de två avdelningarna där observationerna utfördes. Den utgör en grund för fortsatt förbättringsarbete på avdelningarna samt ger inspiration till fortsatt forskning.

Full textAim. To illustrate the attitudes of the operating theatre personnel and the risk of intraoperative sharp injuries. Background. There are several ways to manage sharp instruments such as various techniques and safety products to minimize the risk of sharps injuries. There are many studies about working practices to minimize the risk of sharps injuries but only a few that illustrate the importance of attitudes towards techniques and safety products as well as follow up and reporting incidents. Method. The design of the study is an empirical interview study with a qualitative approach. Semi-structured interviews were conducted with seven theatre nurses and four surgeons at two hospitals in Sweden. Data collected October-December 2015. Findings. The findings show that there exists different attitudes of the operating theatre personnel that could affect the risk of sharps injuries. The finding is presented in four themes: To have a safe working environment; To protect oneself, co-worker and patient; To be compliant to guidelines and working practices and To be non-compliant to guidelines and working practices. Conclusions. The risk of sharp injuries can’t be eliminated since there is always a risk of sharp injuries which the operating theatre personnel must pay attention to. There are a lot of knowledge about safety products and techniques for minimizing the risk of sharps injuries but when the attitude of the user come into play the safety product may not be used correctly, if used at all. In this study a indication appears that the attitudes of the operating theatre personnel may be affecting the risk of intraoperative sharp injuries.

The theatre nurse´s competence is required to ensure patient safety of the surgical patient. Aim: The aim was to describe the theatre nurse´s experiences of job satisfaction Method: Semi-structured interviews with 12 theatre nurses at two hospitals and one private clinic in southern Sweden. Content analysis with inductive approach. Results: The theatre nurse's job satisfaction is founded on having a significant role in an interesting and varied profession with development opportunities. Job satisfaction increases through experience and perceived competence as well as good relationships with colleagues and a well-functioning collaboration with them. Staffing problems, high workloads and lack of opportunities to control their working hours pose threats to job satisfaction. The result can be understood and discussed in relation to Antonovskys Sense Of Coherence (SOC). Conclusion: In a resource-constrained organization, the work management needs to be particularly aware of what is important for the theatre nurse's job satisfaction in order to act in a long-term sustainable way in terms of personnel supply and patient safety. For example, mentoring for newly graduated theatre nurses is recommended and that the manager, in the composition of working teams and new recruits, is well acquainted and aware of the work group's dynamics.

However, research is missing regarding the use in practice. Aim: The aim of this study was to observe, with the support of the competence description of the profession, how operating theatre nurses use intraoperative communication with patients in their clinical work, and reasons why communication possibly is not used. Method: The study was performed as a qualitative, ethnographic, observational study. 20 observations of operating theatre nurses at a hospital in northern Sweden were made, and the material was analyzed through content analysis. Findings: The analysis of the material led to three categories: To communicate, To perform person-centered care and To be influenced by the working environment, and eight sub-categories: To provide information, To conduct an intraoperative dialogue, To build a temporary relationship, To identify caring needs, To be committed, To be absent, To have other duties and To be affected by time Conclusion: The findings of this study indicate that intraoperative communication is rarely used in clinical work today, and that operating theatre nurses, for different reasons, usually take a step back and let the other team members take responsibility for the intraoperative nursing.

Theater nurse has to various degrees with a larger role in perioperative caring which include pre-, intra- and postoperatively. Purpose: The purpose of this study was to describe theater nurses perception of the responsibility of the perioperative nursing care. Method: Six interviews were conducted with theater nurses at a hospital in Norrland. The collected data were analyzed using the content analysis. Results: In text analysis identified four categories: Continuous evaluation of their own work, Competence gives greater responsibility, Experiencing that increased contact with patients could improve care and their talents and Theater nurses have their own idea of how her responsibility to follow policies and procedures look like. Theater nurses felt that new procedures were developed when errors are detected. They described a diverse work that was more than just instrumentation, and increased patient contact was believed to contribute to gaining knowledge, both technical and caring manner. What new information and new policies and procedures used were in some degree dependent on the theater nurse as an individual. Discussion: Theater nurses felt that new procedures were developed when errors were detected. The aim: The aim of the study was to describe operating theater nurses’ experiences of working in the operating theater. Method: A qualitative method was used, five semi-structured interviews were made with operating theater nurses that have worked at least one year in the profession. The data was analysed with qualitative content analysis. Result: The analysis resulted in one overarching theme: The work of the operating theater nurse is characterized by continually changing focus between the whole and its parts, and three categories; To work in a team, To find one's place and To focus on the patient. Conclusion: The study showed that team continuity has an impact on how well the communication and teamwork works during a surgical procedure. Teams that often work together will result in a more effective process, a better quality of the outcome and a safer care of the patient.

Full textBakgrund: Upprätthållandet av patientens värdighet är en central del i operationssjuksköterskans roll, vilket är extra viktigt då patienten är sövd. Det finns dock få studier inom operationssjukvård som fokuserar på hur den sövda patientens värdighet upprätthålls på operationssalen. Syfte: Syftet med studien var att observera hur den sövda patientens värdighet bevaras på operationssalen och vad i samtal och beteenden som talar för och emot att värdigheten upprätthålls. Metod: En etnografisk observationsstudie med kvalitativ ansats användes. Studien utfördes på en operationsavdelning på ett universitetssjukhus i Sverige. Genom ett strategiskt urval observerades 20 operationer under 40 timmar. Resultat: Resultatet påvisar olikheter i personalens engagemang beroende på operationens omfattning. Vid operationer som krävde mer omfattande förberedelser observerades en större intention hos personalen att göra det bästa för patienten. Vikten av teamarbete observerades och hur personalen tillsammans kan upprätthålla patientens värdighet. Resultatet visade också hur kommunikationen mellan personalen på salen påverkade synen på patienten och vikten av att bevara sin professionalitet för att inte kränka patientens värdighet. Slutsats: Personal observerades bevara patientens värdighet. Det förekom dock situationer, samtal och ageranden som visade på en kränkning av patientens värdighet. Vid en del tillfällen skedde en kränkning, efterföljt av en bevarande åtgärd av en annan personal, vilket visar betydelsen av teamarbete i relation till upprätthållandet av den sövda patientens värdighet. Ju större ingrepp desto fler tecken på engagemang och omsorg om patienten observerades. Detta kan förklaras med att en kränkning av den yttre värdigheten kan ersättas med ageranden och åtgärder som stärker den inre värdigheten.

The occurrence of pressure ulcers may be caused by a combination of several factors and the responsibility of the operating nurses is to minimize the occurrence of pressure ulcers in the pre- and intraoperative nursing care.Aim: The aim was to elucidate the operation nurses’ experiences of the pressure ulcerprevention work in pre- and intraoperative nursing.Method: A qualitative interview study with semi-structured questions analyzed through conventional content analysis. The participants consisted of 12 operating nurses, all women, at three hospitals located in the middle of Sweden.Result: The result showed that the operating nurses obtain information before the operationto be able to perform adequate measures. To carry out pressure ulcer prevention they used various pressure-relieving materials. Through experience, the operating nurses developed knowledge of the pressure ulcer prevention work so they could handle the challenges that occurs in a better way. The collaboration in the operating room around the patient was highlighted as an important part of the pressure ulcer prevention work.Conclusion: The operating nurses, together with the operating team, had some challenges in the pressure ulcer prevention work. To make the surgery as good as possible for the patient the operating nurses used their nursing knowledge. Attention to patients pre- and intraoperative positioning should be kept emphasized as patients still are suffering from complications due to pressure ulcer. Especially patients with multiple risk factors. Therefore, the use of routine documents and follow-up of the patient's pre- and intraoperative positioning are important.

Although there are tools to ensure patient safety, such as the WHO Safe Surgery Checklist, unexpected events can occur. Unexpected events can involve arised errors and complications which can jeopardize the patient's safety. Aim: To investigate the operation theater nurse's experience of unexpected events in the operating room. Method: The study was a qualitative interview study conducted with semi-structured questions. A total of eight operating theater nurse’s were interviewed at two hospitals in southern Sweden. The interviews were analyzed with qualitative content analysis Result: Following the analysis three themes emerged that described how operating theatre nurses’ experienced unexpected events in the operating room; Feeling confident in oneself and others. The experience of preserving and losing control and Emotional aspects of unexpected events. How well-functioning the team is and the degree of professional experience are perceived to be key factors the influence how to handle an unexpected event. Mixed feelings such as inadequacy and stimulation in the profession emerged. Conclusion: Operating theatre nurses generally feel secure when unexpected events occur. The study shows both positive and negative experiences in connection with an unexpected event.

Full textHaving a surgical procedure is associated with risk of complications. WHO has prepared a checklist for safe surgery used by surgical staff to reduce the risk of complications. Patient safety is included as one of the cornerstones in the competence description for anesthesia nurses, and in accordance with this, the anesthesia nurse should work in a patient-safe manner and pay attention to work-related risks. The aim of the study was to examine anesthesia nurses' experiences of using the "Safe Surgery Checklist" in their daily work.A qualitative study was conducted in which seven anesthesia nurses participated. Data was analyzed using qualitative content analysis, which resulted in four categories: Care becomes safer through a comprehensive picture; Anesthesia nurses take great responsibility for the implementation of the checklist due to practical reasons; Making deviations but with the checklist in mind; and Everyone in the team must be involved and take responsibility. Conclusions that can be drawn are that more focus are needed on the education of all professions involved, which could generate increased compliance when using the checklist, increased understanding of each other’s occupations and increase cohesion in the team. Also, there should be more focus on modification of the checklist that can be used in emergency situations and on measures that promote communication and culture in the operating room. Further studies should be done on why deviations are made and how this can be counteracted so that patient safety can be maintained.

The operating theatre nurse has her own professional responsibility for the patients’ perioperative care, where the preoperative dialogue is an important part. Aim: To describe the operating theatre nurse’s perception and experience of preoperative dialogue with the patient. Method: A qualitative interview study with inductive approach, in which 10 professional operating theatre nurses participated. A manifest content analysis was used. Result: The results are presented in two categories: "A holistic view of man" and "Obstacles to creating a care relationship" with five subcategories: "Getting to know their patient", "Seeing and being responsive to the patient's needs", "Listening and being present in the situation", "Prevailing work routines" and "Personal perception of the importance of preoperative dialogue". Conclusion: Most patient dialogue the operating theatre nurses perform are limited to the operation. The preoperative dialogue was considered as an opportunity to get a holistic view of man, but also as an opportunity to obtain further information. Prevailing work routines created barriers to a care relationship, but it was considered important to listen and attend to the situation in order to incorporate what the patient conveyed. Different personal perceptions regarding the significance of preoperative dialogue existed, but regardless of opinion there was agreement that prevailing work routines did not allow for any longer preoperative dialogues.

Full textBackground: The surgical care is characterized by a hich-tech work environment where the operating theatre nurse has a great responsibility for the patient. The focus is on seeing and protecting the entire patient in order to prevent suffering of the patient as a result of adverse event. By keeping a watchful eye on the environment various risk factors can be prevented. There is a lot of research that concerns the area, but it is very sparingly studied on the basis of the operating theatre nurse experiences and how they consider that different factors pre- and intraoperatively affect patient safety. Purpose: The purpose was to make visible factors that affect patient safety pre- and intraoperatively, based on operating theatre nurse experiences. Method: A systematic literature review was conducted in accordance with Bettany-Saltikov and McSherry (2016). In order to get a deeper interpretation and understanding of the chosen problem area, an inductive and qualitative approach was used. The analysis included twelve scientific articles that were compiled into a result. Result: The result showed that there were many different factors that played a considerable role in patient safety, both pre- and intraoperatively. Four main categories emerged from the result; the importance of the working environment, the importance of the team, planning and preparation and professional-specific skills. Conclusion: Working as an operating theatre nurse meant having full focus on both the patient and the environment in order to prevent risk factors. There were several factors that affect patient safety pre- and intraoperatively, both positive and negative. One factor was that the working environment was characterised by high workload and stress, wich was a major concern. Another factor showed that the cooperation within the operation team was influenced by how we respond to each other. By responding to each other with respect, good cooperation and thus a safer care was promoted.

Instrumentation of sharp instruments must be effective and safe. The management requires knowledge and puts demands on the theatre nurse. Aim To describe the theatre nurses’ experiences of the intraoperative management of sharp instruments. Method Qualitative method with an inductive approach and individual semi-structured interviews. Data was analyzed with manifest and latent content analysis. Results Seven subcategories, three main categories and one covering theme. Theatre nurses were confident in their role and worked with a calm and focused approach with own personal designed work routines to maintain control. Safe management by the surgeon led to feelings of respect towards the theatre nurses and they adapted and reprimanded the surgeon when hazardous management was observed. In case of sharps injury, professional managing with remained asepsis was important. There was some level of fear towards bloodborne pathogen, but at the same time they saw sharp injuries as a common occurrence in their work environment. Conclusion Theatre nurses were confident in the management of sharp instruments which gave them the ability to adapt. Consistent use of personal designed work routines was percieved as safe and contributed to control. Preventative methods against sharps injury were especially used in care of patients with known bloodbourne pathogene as there was a fear of being infected.

Full textBackground:During the past years, there have been investments to make the elective healthcare more effective (Fast-track). The hospital stay after a knee replacement (TKA) is today just a few days. Studies indicate that there is a concern among patients to be discharged too early. There are also studies that shows that the theatre nurse through perioperative care can lay the foundation for an effective rehabilitation of the patient after surgery. This study focuses on the patients experience of a TKA surgery and throughout the care process based on the short hospital stay.Purpose:The purpose with this study was to describe the patient`s experience of undergoing a TKA surgery and the short hospitalization.Method:An interview study was made with ten patients who have undergone a TKA-operation from a strategic selection. The patient`s pain was evaluated with VAS scale.The interviews were recorded, transcribedverbatim and analysed using qualitative content analysis.Outcome:Six subcategories which in turn created three categories emerged; information, treatment and symptom relief. All the patients were very satisfied with the experience of the care process. The treatment and information given from the medical staff was part of what made the patient satisfied. VAS estimation showed that patients scored high pain. Conclusion:The patient ́s experience of undergoing a TKA surgery and the short hospitalization was seen as positive. Despite the positive result further research from a larger number of patients to get a more generalizable result of the patient ́s experiences from TKA and the short hospitalization is advocated. Theatre nurses ́ participation in theperioperative healthcare can advantageously be discussed at the current hospital.

Full textBackground: The theatre nurse uses different prevention measures to protect the patient and the theatre team from any stick and cut injuries. Technique and strategies which could be used are hand-free technique and neutral zone, double gloves and blunt surgical needles. Objective: The point of this study is to illustrate the theatre nurse and the use of methods to prevent stick and cut injuries. Method: The study was conducted by semistructured interviews with four professional theatre nurses. The material was transcribed and analysed with content analyse. The results were presented in themes and categories. Results: One theme and three categories appeared. Beneath theme Intraoperative strategies three categories appeared Order, Technique and Communication. Conclusion: The theatre nurses in this study use an intraoperative strategy with order, diffrent techniques and communication with compliance depending on the current operation.

Full textIntroduction: The operating theatre nurse (OTN) is responsible for the patient’s care during the perioperative process and it should be conducted on the basis of person-centered care (PCC). In the high-tech environment the OTN work as a link between technology and the patient. Only a few studies illustrate the OTN’s perioperative care and the need for a care-oriented work in the operating room is questionable. Aim: The aim of the study was to describe OTNs' experiences of the characteristics of perioperative nursing. Method: The study was a qualitative interview study with an inductive approach. The data was collected through reflexive interviews of ten OTNs’ from two surgical clinics. The interviews were based on a perceived situation of perioperative care. Data analysis was carried out with content analysis. Result: The OTNs' experiences of perioperative care can be summarized in one key category: Patient safe care. Three generic categories emerged: Preserving the patient's dignity, Coordination and control of the patient’s care and Protect the patient's body. The OTNs’ described how they worked according to individual patient needs and had specific operating and technical knowledge which prevented the emergence of health damage to the patient. Conclusion: The perioperative nursing characterized by the OTNs’ through their specific knowledge coordinates and controls the patient's care, protect the patient's body and contributes to the preservation of the patient’s dignity, which contributes to a patient safe care. According to the guidelines, the perioperative care should be person-centered which it partly is but further evidence regarding perioperative PCC would be significant.

Full textO presente relatório de estágio enquadra-se no 1º Curso de Mestrado em Enfermagem Perioperatória, da Escola Superior de Saúde do Instituto Politécnico de Setúbal. Foi redigido segundo a metodologia de projeto, focando-se na integração de conhecimentos e competências adquiridas durante o curso, no domínio da enfermagem perioperatória. Tem como objetivos estabelecidos: (1) reflexão crítica acerca das atividades realizadas em contexto de estágio; (2) desenvolvimento de um projeto acerca das competências não técnicas do enfermeiro instrumentista; (3) reflexão crítica acerca do desenvolvimento de competências de mestre em enfermagem perioperatória. De forma a responder aos objetivos propostos organizou-se a estrutura deste relatório por três capítulos distintos: Capítulo I – onde consta o enquadramento concetual, considerando a teoria do autocuidado de Dorothea Elizabeth Orem; e enquadramento teórico, acerca do conceito de competência em enfermagem perioperatória. Capítulo II – corresponde ao enquadramento metodológico, onde é fundamentado o método seguido para a construção do projeto, exposto o tipo de estudo conduzido, o trabalho de campo desenvolvido, bem como os processos de colheita e tratamento de dados, considerando sempre as questões éticas do processo de investigação. Capítulo III – é realizada a reflexão sobre o estágio realizado, assim como a aquisição do perfil de competências de mestre em enfermagem perioperatória. A realização deste relatório de estágio permitiu a aquisição de conhecimentos no domínio da enfermagem perioperatória. Com os contributos da investigação elaborada, foi possível a compreensão e aplicação destes saberes para a resolução de problemas, em ambiente clinico multidisciplinar, consciente das implicações científicas, éticas, deontológicas e jurídicas.
This internship report is part of the 1st Master in Perioperative Nursing, taken in Escola Superior de Saúde do Instituto Politécnico de Setúbal. It has been drafted according to project methodology, focusing on the integration of knowledge and skills acquired during the course in the field of perioperative nursing. Its stated objetives are: (1) critical analysis of the activities undertaken in the stage context; (2) project development for scrub nurses non-technical skills; (3) critical assessment on the development master competencies in perioperative nursing. In order to meet the proposed objetives, this report is organized by three chapters: Chapter I – with the conceptual framework, considering the self-care theory of Dorothea Elizabeth Orem; and theoretical framework of competence in perioperative nursing. Chapter II - corresponds to the methodological framework, which is based the method followed for the construction of the project, stated the type of study conducted, the field work and the procedures for collection and processing of data, always considering ethical issues in research process. Chapter III - is held to assess the internship stage, as well as the acquisition of master skills profile in perioperative nursing. The completion of this internship report allowed the acquisition of knowledge in the field of perioperative nursing. With the contributions of elaborate investigation, it was possible the understanding and application of this knowledge to solve problems in a multidisciplinary clinical environment, aware of the scientific, ethical, ethical and legal implications.
