In recent years, several studies have demonstrated the involvement of the intestinal microbiota in immune-mediated diseases such as diabetes, ulcerative colitis, and multiple sclerosis. There are few data on the follicular microbiome and its role in the pathogenesis of scalp diseases. Some studies show influence of dysbiosis on these diseases, and manipulation of the microbiome may represent a possible therapeu- tic option. This article reviews current knowledge regarding the impact of dysbiosis on dermatological diseases of the scalp, such as seborrheic dermatitis, psoriasis, alopecia areata, androgenetic alopecia, lichen planus pilaris, frontal fibrosing alopecia and decalvant folliculitis. A broader understanding of this may suggest additional treatments beyond conventional therapies.

Abstract:
Glucocorticoids are a class of steroid hormones, vital for mammalian life. They have a plethora of biological effects, mainly supporting metabolic, cognitive, and immunological functions. The pharmacological use of glucocorticoids makes them one of the most frequently prescribed drugs across all continents, and in all types of forms. Nevertheless, a number of serious adverse effects accompany the prolonged treatment with high doses of glucocorticoids. Research developments over the last 20 years have gradually reshaped the way we think about glucocorticoid-based therapeu¬tics. Aside their circadian rhythm and their delayed regulatory influence over an extensive number of sensitive genes, glucocorticoids also possess an underlying, ultradian rhythm, and also exert rapid, non-genomic effects. The notion that chronicity of glucocorticoid stimulation may differentially modulate the type of biological effects of the hormone brings various chronopharmacological concepts on the table of modern glucocorticoid-based therapeutics.

Abstract:
Colorectal cancer (CRC) is the third most common cancer and is associated with a poor prognosis. The mutation profile and related involved pathways of CRC have been, in broad terms, analyzed. The main current therapeutic approaches have been comprehensively reviewed here, and future possible therapeu-tic options and related technologies have been perspectively presented. The complex scenario represented by the multiple-level resistance mechanism in the epidermal growth factor receptor (EGFR) pathway, including mutations in KRAS, NRAS, and BRAF V600E, is discussed. Examples of engineered therapeutic approaches from the literature along with a drug combination tested in clinical trials are discussed. The encouraging results observed with the latter combination (the BEACON clinical trial), totally free from chemotherapy, prompted the authors to imagine a future possible nanotechnology-assisted therapeutic approach for bypassing multiple-level resistance mechanisms, hopefully allowing, in principle, a complete biological cancer remission.

Little, Daniel. "Development of therapies for spinal muscular atrophy using gene therapy and nanotechnology." Thesis, University of Sheffield, 2013. http://etheses.whiterose.ac.uk/4350/.

Full text
Abstract:
Spinal muscular atrophy (SMA) is a genetic disease which is characterized by muscle weakness and atrophy. The disease arises from mutations in the survival motor neuron 1 (SMN1) gene causing degeneration of spinal cord motor neurons. No effective treatment is currently available for SMA however it may be possible to treat the disease using gene therapy. The aim of this project is to develop potential therapies for SMA by investigating different viral and non-viral gene therapy vectors and assessing the effect of potential disease modifying genes. The data collected are described under four chapters as follows: 1: The aim here was to develop a novel approach based on polymer nanoparticles (polymersomes) for gene delivery. Encapsulation of DNA by polymersomes was optimised and polymersomes were used to restore SMN levels into a fibroblast cell line isolated from a child with severe SMA. 2: The ability of adeno-associated virus serotype 5 (AAV5) vectors expressing GFP to transduce the central nervous system (CNS) following intravenous injection was tested in neonatal wild-type mice. Overall transduction efficiency of AAV5-GFP in the brain was low and very few lumbar spinal cord neurons were found to be transduced, suggesting that AAV5 is not an appropriate vector to treat diseases such as SMA. 3: AAV6 was used to overexpress hnRNP R in an in vivo model of SMA. hnRNP R is a candidate disease modifying gene for SMA due to its interaction with SMN and β-actin. However this strategy had only a very marginal effect on the phenotype and life-span of this SMA mouse model. 4: Finally AAV9 was used to silence phosphatase and tensin homolog (PTEN) in an in vivo model of SMA. PTEN is a negative regulator of growth which acts on the PI3K/Akt pathway. AAV9-mediated PTEN silencing resulted in a significant increase in the lifespan of a SMA mouse model coupled with an improved phenotype. In conclusion this work highlights two major findings: i) polymersomes can be used to deliver SMN plasmid DNA to restore SMN mRNA and protein levels in an in vitro model of SMA; ii) AAV9-mediated silencing of PTEN can improve the phenotype and increase lifespan of a SMA mouse model.

Abstract:
Mental disorders are a prevalent health issue that has an extensive amount of variability in the quantity of disorders, affected populations, and treatment. There are a multitude of approaches or therapeutic methods used by the mental health community. Although each therapy is valuable in unique ways, each therapy may not be suitable to treat every disorder or client. Traditional approaches, such as talk therapy, are effective for certain individuals, whereas some clients may not respond well to any of the traditional talk therapies. When traditional therapies are not working, a referral can be made for a therapy that might suit the client better. Equine-Assisted Psychotherapy (EAP) is an experiential psychotherapy that has done well not only in assisting typical clients, but also with clients that have had trouble in traditional modes of therapy. This is a collaboration of current works and research in EAP, along with the knowledge base of a current licensed practitioner of EAP. The purpose is to lay out the fundamentals of EAP, which offers a different modality than traditional talk therapies. By crafting a better picture of EAP, perhaps it can kindle further research in its efficacy and guide additional mental health professionals towards this type of therapy.<br>B.S.<br>Bachelors<br>Sciences<br>Psychology

Abstract:
In this study, art therapists' stories of how they work with affect awareness in art therapy were examined. Methods: A qualitative study design was used with semi-structured interviews with four art therapists. The thematic analysis was based on four dimensions of affect awareness: Affect awareness, Affect tolerance, Nonverbal and Conceptual expression of affect (Monsen, Eilertsen, Melgård &amp; Ødegård 1996). Results: Analysis of the interviews resulted in nine themes/art therapy methods for increased affect awareness: Paying attention to affect, Highlighting unconsiousnes affects, Highlighting more aware affects, Switching between thought and affect, Regulate overwhelming affects, Restructure, Body focus, Opening the image and Borrowing the image. Conclusion: By highlighting art therapy methods focusing on affect awareness, important techniques and aspects, both non-verbal and verbal, are clarified. The result can generate ideas for intervention research on art therapy with affects.

Amoss, Sarah. "The negotiation of blame in family therapy with families affected by psychosis." Thesis, University of East London, 2014. http://repository.tavistockandportman.ac.uk/1530/.

Full text
Abstract:
Despite wide agreement in the systemic field that therapists should take a non-blaming stance, historically there has been little exploration of how this stance is achieved in practice. The difficulty in knowing how to put ‘non-blaming’ into practice is further heightened by competing models of intervention with families affected by psychosis. This study contributes to a body of literature that is concerned with how complex issues of morality are achieved dialogically by considering how family therapists manage the tension of intervening to promote change whilst maintaining a multi-partial, non-blaming stance. Two therapies carried out with families affected by psychosis are analysed using the methods of Conversation Analysis (CA) and Membership Categorization Analysis (MCA). In both therapies the sequences examined are drawn from the second session of therapy where explicit blaming events occur. By examining blaming events chronologically through the course of a session the study shows how the rules about the way blame is talked about are achieved interactionally. The analysis demonstrates that systemic theory’s emphasis on the importance of being non-blaming is grounded in a sophisticated understanding of the threat blame poses to co-operation and agreement. In both therapies, the delicacy and ambiguity with which blame is treated serves to enable the conversation to continue without withdrawal. However the cost of ambiguity is a possible misunderstanding of the intent of the speaker. The resulting misalignment, where it continues over several turns and sequences, leads to explicit blame becoming relevant as a solution to a redundant pattern of interaction. The findings indicate that the management of blame requires both the exploration of blame and its interruption when emotions and conflict run high. The former enables understanding and movement towards therapeutic goals while the latter is necessary to promote therapeutic and family alliances. An unintended consequence of the injunction to be non- blaming might be the premature closing down of topics, militating against problem resolution. The study concludes that CA and MCA offer a wealth of knowledge about mundane conversational practices that can be applied fruitfully to systemic therapy process research, teaching and supervision.

Abstract:
AbstractThe conditions for reimbursement for CAR-T cell therapy are not uniform in Europe. Most European countries use a DRG system for billing hospital services, but the details vary. Nonetheless, the similarity is that expensive therapies, such as CAR-T cell therapy, are initially not included in the DRG system. Most countries possess instruments to ensure the financing of such expensive therapies outside the DRG system as separate payments. These reimbursement instruments of DRG systems are used in most countries both for short-term financing for innovative and new therapies and as long-term additional fees within the respective DRG system. Individual countries maintain different regulations, and therefore, hospitals have the responsibility to determine the specific requirements of their country before establishing CAR-T cell therapy.

Abstract:
The treatment landscape for advanced stage melanoma is rapidly evolving due to advancements in our understanding of melanoma biology and the emergence of novel therapies. This necessitates a comprehensive review to guide clinicians in adopting evidence based and patient centric approaches to treat stage IIIB-IIID melanoma. A literature review was conducted to synthesize current information on the most optimal treatment available. Data available from different clinical trials found that neoadjuvant therapy was a more effective treatment compared to adjuvant therapies alone. Furthermore, neoadjuvant therapy with combination therapy was more efficacious in producing a complete pathological response compared to monotherapy. A follow-up study on patients treated with neoadjuvant therapy with PD1 inhibitor pembrolizumab found that patients who produced a major pathologic response (MPR) or complete response (pCR) had higher 5 year Overall Survival rates. The trial highlights the importance of tailoring pharmacological treatment to patients to receive a significant pathological response. In addition, finding robust biomarkers is important in assessing therapies to personalize them to the patient. Longer term follow-up should be emphasized due to observed differences in median time to recurrence.

Adoptive cell therapy, particularly T cell receptor–engineered T (TCR-T) cell therapy, represents a cutting-edge and promising strategy for treating solid tumors [1]. Current methods for developing TCR-T cell therapies yield a limited number of candidate TCRs [2], missing the comprehensive view of the repertoire, which may impede the identification of the most effective TCRs. This limitation highlights the need for new techniques in TCR-T cell therapy development.

Abstract:
The goal of the project is to develop an effective treatment planning computational tool for nanotube-mediated laser therapy that maximizes tumor destruction and minimizes tumor recurrence. Laser therapies can provide a minimally invasive treatment alternative to surgical resection of tumors. However, the effectiveness of these therapies is limited due to nonspecific heating of target tissue and diffusion limited thermal deposition which often leads to healthy tissue injury and extended treatment durations. These therapies can be further compromised due to induction of molecular chaperones called heat shock protein (HSP) in tumor regions where non-lethal temperature elevation occurs causing enhanced tumor cell viability and imparting resistance to chemotherapy and radiation treatments which are generally employed in conjunction with hyperthermia.

Abstract:
Over the last couple of years, the technology of virtual reality had a massive breakthrough, extending its offerings into multiple fields such as education, arts, therapy, entertainment, architecture, sports, film and many others. In this paper, we will focus on the influence of virtual reality in therapy, with emphasis on treating acrophobia with the help of virtual reality. Using virtual reality to treat acrophobia has proved to be very effective, as numerous studies have shown. As medication and standard cognitive behavioural therapy manage to alleviate the symptoms, the most effective therapies require confronting the trauma itself. Hence, virtual reality therapy is becoming more popular, as it is adopted by hospitals and medical centres. Virtual Reality therapy is used to treat patients with different phobias. The patient is exposed to an artificially created environment, the phobia being triggered by both visual and auditory stimuli. During the session, the therapist can monitor the patient’s reactions and adjust the exposure to the environment - exposure therapy, which implies gradually increasing or decreasing the intensity of the exposure, based on the current level of anxiety.

Abstract:
Objectives. To evaluate the effectiveness and comparative effectiveness of opioid, nonopioid pharmacologic, and nonpharmacologic therapy in patients with specific types of acute pain, including effects on pain, function, quality of life, adverse events, and long-term use of opioids. Data sources. Electronic databases (Ovid® MEDLINE®, PsycINFO®, Embase®, the Cochrane Central Register of Controlled Trials, and the Cochrane Database of Systematic Reviews) to August 2020, reference lists, and a Federal Register notice. Review methods. Using predefined criteria and dual review, we selected randomized controlled trials (RCTs) of outpatient therapies for eight acute pain conditions: low back pain, neck pain, other musculoskeletal pain, neuropathic pain, postoperative pain following discharge, dental pain (surgical or nonsurgical), pain due to kidney stones, and pain due to sickle cell disease. Meta-analyses were conducted on pharmacologic therapy for dental pain and kidney stone pain, and likelihood of repeat or rescue medication use and adverse events. The magnitude of effects was classified as small, moderate, or large using previously defined criteria, and strength of evidence was assessed. Results. One hundred eighty-three RCTs on the comparative effectiveness of therapies for acute pain were included. Opioid therapy was probably less effective than nonsteroidal anti-inflammatory drugs (NSAIDs) for surgical dental pain and kidney stones, and might be similarly effective as NSAIDs for low back pain. Opioids and NSAIDs were more effective than acetaminophen for surgical dental pain, but opioids were less effective than acetaminophen for kidney stone pain. For postoperative pain, opioids were associated with increased likelihood of repeat or rescue analgesic use, but effects on pain intensity were inconsistent. Being prescribed an opioid for acute low back pain or postoperative pain was associated with increased likelihood of use of opioids at long-term followup versus not being prescribed, based on observational studies. Heat therapy was probably effective for acute low back pain, spinal manipulation might be effective for acute back pain with radiculopathy, acupressure might be effective for acute musculoskeletal pain, an opioid might be effective for acute neuropathic pain, massage might be effective for some types of postoperative pain, and a cervical collar or exercise might be effective for acute neck pain with radiculopathy. Most studies had methodological limitations. Effect sizes were primarily small to moderate for pain, the most commonly evaluated outcome. Opioids were associated with increased risk of short-term adverse events versus NSAIDs or acetaminophen, including any adverse event, nausea, dizziness, and somnolence. Serious adverse events were uncommon for all interventions, but studies were not designed to assess risk of overdose, opioid use disorder, or long-term harms. Evidence on how benefits or harms varied in subgroups was lacking. Conclusions. Opioid therapy was associated with decreased or similar effectiveness as an NSAID for some acute pain conditions, but with increased risk of short-term adverse events. Evidence on nonpharmacological therapies was limited, but heat therapy, spinal manipulation, massage, acupuncture, acupressure, a cervical collar, and exercise were effective for specific acute pain conditions. Research is needed to determine the comparative effectiveness of therapies for sickle cell pain, acute neuropathic pain, neck pain, and management of postoperative pain following discharge; effects of therapies for acute pain on non-pain outcomes; effects of therapies on long-term outcomes, including long-term opioid use; and how benefits and harms of therapies vary in subgroups.

Abstract:
Background: Suicide is a leading cause of death in young people and an escalating public health crisis. We aimed to assess the effectiveness and harms of available treatments for suicidal thoughts and behaviors in youths at heightened risk for suicide. We also aimed to examine how social determinants of health, racism, disparities, care delivery methods, and patient demographics affect outcomes. Methods: We conducted a systematic review and searched several databases including MEDLINE®, Embase®, Cochrane Central Register of Controlled Trials, Cochrane Database of Systematic Reviews, and others from January 2000 to September 2024. We included randomized clinical trials (RCTs), comparative observational studies, and before-after studies of psychosocial interventions, pharmacological interventions, neurotherapeutics, emerging therapies, and combinations therapies. Eligible patients were youths (aged 5 to 24 years) who had a heightened risk for suicide, including youths who have experienced suicidal ideation, prior attempts, hospital discharge for mental health treatment, or command hallucinations; were identified as high risk on validated questionnaires; or were from other at-risk groups. Pairs of independent reviewers selected and appraised studies. Findings were synthesized narratively. Results: We included 65 studies reporting on 14,534 patients (33 RCTs, 13 comparative observational studies, and 19 before-after studies). Psychosocial interventions identified from the studies comprised psychotherapy interventions (33 studies, Cognitive Behavior Therapy, Dialectical Behavior Therapy, Collaborative Assessment and Management of Suicidality, Dynamic Deconstructive Psychotherapy, Attachment-Based Family Therapy, and Family-Focused Therapy), acute (i.e., 1 to 4 sessions/contacts) psychosocial interventions (19 studies, acute safety planning, family-based crisis management, motivational interviewing crisis interventions, continuity of care following crisis, and brief adjunctive treatments), and school/community-based psychosocial interventions (13 studies, social network interventions, school-based skills interventions, suicide awareness/gatekeeper programs, and community-based, culturally tailored adjunct programs). For most categories of psychotherapies (except DBT), acute interventions, or school/community-based interventions, there was insufficient strength of evidence and uncertainty about suicidal thoughts or attempts. None of the studies evaluated adverse events associated with the interventions. The evidence base on pharmacological treatment for suicidal youths was largely nonexistent at the present time. No eligible study evaluated neurotherapeutics or emerging therapies. Conclusion: The current evidence on available interventions intended for youths at heightened risk of suicide is uncertain. Medication, neurotherapeutics, and emerging therapies remain unstudied in this population. Given that most treatments were adapted from adult protocols that may not fit the developmental and contextual experience of adolescents or younger children, this limited evidence base calls for the development of novel, developmentally and trauma-informed treatments, as well as multilevel interventions to address the rising suicide risk in youths.

Abstract:
Review question / Objective: Is pediatric tuina an effective treatment for recurrent respiratory tract infection in children? Condition being studied: Recurrent respiratory tract infection (RRTI) is a common disease in children, which refers to the recurrence of upper and lower respiratory tract infections within a year, exceeding the prescribed number of times. It is more common in infants under 3 years old. The disease is easy to relapse and lasts for a long time, affecting the normal growth and development of children and physical and mental health, easily causing other diseases, leading to a variety of chronic wasting diseases, and damaging the function of organs and the immune system. Immunotherapy and nutritional therapy are commonly used in Western medicine. At present, the treatment of RRTI in children with traditional Chinese medicine has achieved a certain effect, and the treatment mainly includes internal treatment and external treatment. Tuina therapy is one of the common therapies for the treatment of RRTI in children with traditional Chinese medicine. Because of its advantages, there are many literature reports on tuina treatment of this disease, with a good total effective rate, but whether its therapeutic effect is higher than other therapies has not been determined as a whole. This study used the method of systematic review to collect the published clinical research literature on the treatment of RRTI in children at home and abroad for systematic review, so as to provide a reference for clinical research.
