<p>C-uppsatsen behandlar ämnet Animal Assisted Therapy (AAT). Författarens Syfte är att ge en översikt av Arbetsformen AAT samt teoretisk och vetenskaplig bakgrund. Övergripande frågeställning är: Kan husdjur/hundar användas som medhjälpare i terapeutiskt arbete? Underliggande frågeställningar är: Vilka för- och nackdelar kan finnas med att använda husdjur/hundar som medhjälpare i terapeutiskt arbete?, Finns det vetenskaplig grund för att använda husdjur/hundar som medhjälpare i terapeutiskt arbete?, Vilka teorier ligger till grund för att använda husdjur/hundar som medhjälpare i terapeutiskt arbete?</p><p>Metoden är en kvalitativ, explorativ och kombinatorisk översikt av material om AAT, bandet mellan djur och människa samt vetenskapliga resultat om detta.</p><p>Teoretiskt anknyter uppsatsen till etologi. Författaren tar upp Darwins teori om det naturliga urvalet tillsammans med Lorenz teorier om instinkter och prägling, Bowlbys bindningsteori och Ainsworths teori om modern som barnets trygga bas.</p><p>Uppsatsen behandlar hundens kvaliteter och fördelar som therapet (medhjälpare i terapi), nackdelar och fördelar med AAT som behandlingsform samt metoder för testning och träning av therapets.</p><p>Mina slutsatser pekar på att AAT är ett bra alternativ och/eller komplement till andra terapeutiska behandlingsformer. De visar också att arbetsformen är i behov av fler vetenskapliga undersökningar och en mer tydlig teoretisk grund.</p>

Abstract:
Spinal muscular atrophy (SMA) is a genetic disease which is characterized by muscle weakness and atrophy. The disease arises from mutations in the survival motor neuron 1 (SMN1) gene causing degeneration of spinal cord motor neurons. No effective treatment is currently available for SMA however it may be possible to treat the disease using gene therapy. The aim of this project is to develop potential therapies for SMA by investigating different viral and non-viral gene therapy vectors and assessing the effect of potential disease modifying genes. The data collected are described under four chapters as follows: 1: The aim here was to develop a novel approach based on polymer nanoparticles (polymersomes) for gene delivery. Encapsulation of DNA by polymersomes was optimised and polymersomes were used to restore SMN levels into a fibroblast cell line isolated from a child with severe SMA. 2: The ability of adeno-associated virus serotype 5 (AAV5) vectors expressing GFP to transduce the central nervous system (CNS) following intravenous injection was tested in neonatal wild-type mice. Overall transduction efficiency of AAV5-GFP in the brain was low and very few lumbar spinal cord neurons were found to be transduced, suggesting that AAV5 is not an appropriate vector to treat diseases such as SMA. 3: AAV6 was used to overexpress hnRNP R in an in vivo model of SMA. hnRNP R is a candidate disease modifying gene for SMA due to its interaction with SMN and β-actin. However this strategy had only a very marginal effect on the phenotype and life-span of this SMA mouse model. 4: Finally AAV9 was used to silence phosphatase and tensin homolog (PTEN) in an in vivo model of SMA. PTEN is a negative regulator of growth which acts on the PI3K/Akt pathway. AAV9-mediated PTEN silencing resulted in a significant increase in the lifespan of a SMA mouse model coupled with an improved phenotype. In conclusion this work highlights two major findings: i) polymersomes can be used to deliver SMN plasmid DNA to restore SMN mRNA and protein levels in an in vitro model of SMA; ii) AAV9-mediated silencing of PTEN can improve the phenotype and increase lifespan of a SMA mouse model.

The overall objective of this research project was to explore the combination of a traditional treatment for postnatal depression (PND), such as cognitive behavioral therapy (CBT), with the use of virtual reality (VR) technology. This project set out to respond to five objectives 1) to study which treatments were the most effective and frequent for the treatment of PND, 2) to identify which components of CBT were used most frequently for the treatment of PND, 3) to identify whether VR had ever been used for the treatment of PND, and whether VR could be combined with traditional therapy and implemented in a clinical setting, 4) to study the feasibility and acceptance of this CBT plus VR intervention protocol 5) to explore the preliminary efficacy of the combination of CBT with VR for the treatment of PND.<br>El objetivo general de este proyecto de investigación era explorar la combinación de un tratamiento tradicional para la depresión postnatal (DPN), como la terapia cognitivo-conductual (TCC), con el uso de la tecnología de realidad virtual (RV). Abstract:
Mental disorders are a prevalent health issue that has an extensive amount of variability in the quantity of disorders, affected populations, and treatment. There are a multitude of approaches or therapeutic methods used by the mental health community. Although each therapy is valuable in unique ways, each therapy may not be suitable to treat every disorder or client. Traditional approaches, such as talk therapy, are effective for certain individuals, whereas some clients may not respond well to any of the traditional talk therapies. When traditional therapies are not working, a referral can be made for a therapy that might suit the client better. Equine-Assisted Psychotherapy (EAP) is an experiential psychotherapy that has done well not only in assisting typical clients, but also with clients that have had trouble in traditional modes of therapy. This is a collaboration of current works and research in EAP, along with the knowledge base of a current licensed practitioner of EAP. The purpose is to lay out the fundamentals of EAP, which offers a different modality than traditional talk therapies. Syfte: I denna studie undersöktes bildterapeuters berättelser om hur de arbetar med affektmedvetenhet i bildterapi. Metod: En kvalitativ studiedesign användes genom semistrukturerade intervjuer med fyra bildterapeuter. För att utvinna data valdes tematisk analys. Bearbetning av intervjuerna utgick ifrån fyra dimensioner av affektmedvetenhet enligt Monsen, Eilertsen, Melgård och Ødegård (1996): Uppmärksamma, Tolerera, Icke-verbalt uttryck samt Verbalt uttryck av känslor. Resultat: Analysen av intervjuerna resulterade i nio teman/bildterapeutiska arbetssätt för ökad affektmedvetenhet hos klienten: Lägga märke till känslor, Synliggöra omedvetna känslor, Synliggöra mer medvetna känslor, Växla mellan tanke och känsla, Reglera överväldigande känslor, Omstrukturera, Kroppsfokus, Öppna bilden och Låna bilden. Slutsats: Genom om att belysa bildterapimetoder med fokus på affektmedvetenhet tydliggörs viktiga tekniker och aspekter, såväl icke-verbala som verbala. Resultatet kan generera idéer till interventionsforskning kring bildterapeutiskt arbete med affekter.<br>Aims: In this study, art therapists' stories of how they work with affect awareness in art therapy were examined. Methods: A qualitative study design was used with semi-structured interviews with four art therapists. The thematic analysis was based on four dimensions of affect awareness: Affect awareness, Affect tolerance, Nonverbal and Conceptual expression of affect (Monsen, Eilertsen, Melgård &amp; Ødegård 1996). Results: Analysis of the interviews resulted in nine themes/art therapy methods for increased affect awareness: Paying attention to affect, Highlighting unconsiousnes affects, Highlighting more aware affects, Switching between thought and affect, Regulate overwhelming affects, Restructure, Body focus, Opening the image and Borrowing the image. Conclusion: By highlighting art therapy methods focusing on affect awareness, important techniques and aspects, both non-verbal and verbal, are clarified. The result can generate ideas for intervention research on art therapy with affects.

Abstract:
Despite wide agreement in the systemic field that therapists should take a non-blaming stance, historically there has been little exploration of how this stance is achieved in practice. The difficulty in knowing how to put ‘non-blaming’ into practice is further heightened by competing models of intervention with families affected by psychosis. This study contributes to a body of literature that is concerned with how complex issues of morality are achieved dialogically by considering how family therapists manage the tension of intervening to promote change whilst maintaining a multi-partial, non-blaming stance. Two therapies carried out with families affected by psychosis are analysed using the methods of Conversation Analysis (CA) and Membership Categorization Analysis (MCA). In both therapies the sequences examined are drawn from the second session of therapy where explicit blaming events occur. By examining blaming events chronologically through the course of a session the study shows how the rules about the way blame is talked about are achieved interactionally. The analysis demonstrates that systemic theory’s emphasis on the importance of being non-blaming is grounded in a sophisticated understanding of the threat blame poses to co-operation and agreement. In both therapies, the delicacy and ambiguity with which blame is treated serves to enable the conversation to continue without withdrawal. However the cost of ambiguity is a possible misunderstanding of the intent of the speaker. The resulting misalignment, where it continues over several turns and sequences, leads to explicit blame becoming relevant as a solution to a redundant pattern of interaction. The findings indicate that the management of blame requires both the exploration of blame and its interruption when emotions and conflict run high. The former enables understanding and movement towards therapeutic goals while the latter is necessary to promote therapeutic and family alliances. An unintended consequence of the injunction to be non- blaming might be the premature closing down of topics, militating against problem resolution. The study concludes that CA and MCA offer a wealth of knowledge about mundane conversational practices that can be applied fruitfully to systemic therapy process research, teaching and supervision.

Abstract:
The first chapter consists of a systematic literature review and meta-analysis, which explores the efficacy of using mindfulness-based interventions (MBIs) to reduce stress among both professional and parental caregivers of individuals with intellectual and/or developmental disabilities (IDD). A systematic review returned 2,346 papers, of which, 14 studies met the inclusion criteria (six including professional caregivers, and eight including parental caregivers). Consistent with previous reviews, the meta-analyses identified a small to moderate effect size of MBIs in reducing stress among parental caregivers. No effect was found for professional caregivers; study findings were mixed, although did highlight a possible doseresponse relationship. Further research is necessary in order to develop the evidence base, however preliminary findings show promise for the use of MBIs for caregivers within ID services. The second chapter presents findings from an empirical study, which adopted a qualitative approach to explore service users' experiences of adapted Dialectical Behaviour Therapy (DBT) in community ID services. The principles of interpretative phenomenological analysis (IPA) guided the analysis of semi-structured interviews, conducted with six participants. Four super-ordinate themes emerged from the data; representing the concurrent challenging and rewarding nature of participants' therapeutic journeys. Implications for clinical practice and recommendations for future research are discussed. The third chapter considers theoretical and clinical implications that arose from the first two papers and highlights a significant paucity of research generally within the field of ID. The paper concludes with personal reflections of the research process.

Computer-based psychological interventions have emerged as an efficacious and more accessible alternative to traditional face-to-face mental health services. In recent years, these programs have also begun to feature heavily in the literature and commentary on stepped-cared and sustainable approaches to mental health services globally (Hosie, Vogl, Hoddinott, Carden, & Comeau, 2014). Australia is among those countries leading the way in the research of computer-based therapies, yet even here, these innovative services remain underutilised in the community (Lawrence et al., 2015). We need to think critically around why this is and how we can improve uptake, to improve access to interventions for youth. Understanding consumer attitudes towards these programs is the first step. Literature in this area is developing, particularly among allied health professionals and adult populations, yet children and youth have gained little attention. Syftet med studien var att beskriva arbetsterapeuters resonemang kring preventiva och hälsofrämjande insatser för äldre och deras erfarenheter av hur dessa insatser uppmärksammas och kommuniceras inom och mellan olika vårdnivåer. För att besvara syftet har fokusgruppmetod använts för datainsamling. Fem olika fokusgrupper har genomförts med fyra arbetsterapeuter i respektive grupp som representerade olika vårdnivåer. Data har analyserats enligt analysprocess för fokusgrupper vilken resulterade i två centrala teman. Det första temat Ge arbetsterapiinsatser i rätt tid innehöll underkategorierna Vikten av att etablera kontakt tidigt och Att vara en del i en process över tid. Det andra temat Betydelsen av kollegial och teambaserad samverkan innehöll underkategorierna Vikten av ett fungerande team, Att handleda och kommunicera insatser till andra professioner och Olika kommunikationsvägars betydelse. Resultatet visar att tidig kontakt med de äldre där en personcentrerad relation etableras är en förutsättning för preventiva och hälsofrämjande arbetsterapiinsatser. En god kommunikation och etablerade relationer inom multiprofessionella team inom och mellan vårdnivåerna skapar förutsättningar för fånga upp den äldre tidigt samt för att preventiva och hälsofrämjande arbetsterapiinsatser skall erbjudas i rätt tid. Studies resultat visar att det finns behov av ytterligare forskning som kan generera kunskapsutveckling kring preventiva och hälsofrämjande arbetsterapiinsatser för äldre som är samhällsbaserade.<br>The purpose of the study was to describe occupational therapists´reasoning about preventive and health-promoting interventions for elderly and their experiences of how these interventions are noticed and communicated within and between different levels of care. To answer the purpose a focus group design has been used for data collection. Five different focus groups were conducted with four occupational therapists´ representing different levels of care. Data have been analyzed accordning to the analysis process for focus groups, which resulted in two central themes. The first theme Giving occupational theraphist interventions in the right time contained the subcategories The importance of establishing contact early and Being part of a process over time. The second theme The importance om collegial and team-based collaboration contained the subcategories The importance of a functioning team, Tutoring ang communicating interventions to other professions and The importance of different communication channels.  The results shows that early contact with the elderly where a person-centered relationship is established is a prerequisite for preventive and health-promoting occupational therapy interventions. Good communication and established relationships witin multi-professional teams within and between the care-levels create conditions for engaging the elderly early and for preventive and health-promoting occupational therapy initiatives to be offered at the right time. The results of the present study indicates that there is a need for further research that can generate knowledge development about preventive and health-promoting occupational therapy interventions for the elderly who are community-based.

Abstract:
Der implantierbare Kardioverter-Defibrillator gehört zu der Behandlung von herzinsuffizienten Patienten. In dieser Arbeit wird eine Risikostratifizierung anhand der vorliegenden Grunderkrankung analysiert. Untersucht wurden 330 konsekutive Patienten mit ischämischer oder nicht-ischämischer dilatativer Kardiomyopathie, die zwischen 2009 und 2011 einen ICD erhalten haben. Im Rahmen des Follow-up von 19 ± 9 Monaten erlitten 29 % der Patienten adäquate ICD-Therapien. Als signifikante Risikofaktoren für adäquate Therapien waren die sekundärprophylaktische Implantation bei ICM-Patienten und die Präsenz von Vorhofflimmern bei DCM-Patienten zu beobachten. Die leitliniengerechte CRT-Implantation zeigte eine prophylaktische Wirkung bei ICM-Patienten.

In fact, in the case of infectious diseases such as AIDS and tuberculosis, combination therapies are now the standard of care. With the emergence of drug-device combinations, genetic testing, and individualized medicine, this trend towards combination therapies is likely to continue to grow. In this context the pricing of combination therapies is a critical component that needs to be understood by medical practitioners, payors and policy makers. There are three factors to consider in the pricing of combination therapies: the characteristics and structure of the market in which the combined product is sold, the absence or presence of market exclusivity, and the prices of the components of the combined product, when sold individually. When one or more of the components of the combined product has market exclusivity, additional factors such as exclusionary bundling, tying, and double marginalization may come into play.<br>(cont.) In this thesis I discuss combination therapies, describe the factors that can affect the pricing of combination therapies, and then attempt to identify the relationships among component pricing, market forces, market exclusivity and the pricing of combination therapies. To illustrate these relationships empirically, I will analyze data from a sample of unified combined drugs, a subset of combination therapies. 50 Patienten wurden mit einem herausnehmbaren unterkiefervorverlagernden Schienensystem im Zeitraum der Jahre 2000 bis 2002 behandelt. Die schlafmedizinisch-diagnostischen Untersuchungen vor Therapiebeginn erfolgten bei 25 Patienten ambulant und bei den anderen 25 Patienten stationär. Es wurden schlafmedizinischen Parameter zur Diagnosestellung herangezogen. Hälftig bestanden bei diesen Patienten die schlafmedizinische Diagnosen einer Rhonchopathie (reine Schnarcher, ohne internistische Besonderheiten) und einer leicht bis mittelgradigen obstruktiven Schlafapnoe. Bei 23 Patienten mit einer Rhonchopathie wurde das Funktionsprinzip der Unterkiefervorverlagerung durch den Wilcoxon-Test bei der Veränderung der schlafmedizinischen Parameter Entsättigungsindex, Schnarchindex, Anzahl der Entsättigungen und niederste Entsättigung bestätigt. Bei zwei (8%) der 25 Patienten veränderten sich die Parameter nicht positiv, so dass die Schienentherapie abgebrochen wurde. Bei 20 Patienten mit leicht- bis mittelgradiger Schlafapnoe bestätigte der Wilcoxon-Test die Signifikanz der Veränderungen bei den polysomnographischen Werten AHI , REM und Schlafeffizienz. Die Signifkanzschranke wurde bei dem somnologischem Wert Tiefschlaf nicht erreicht. Obgleich bei 13 Patienten die Schienentherapie aus unterschiedlichen Gründen innerhalb der zwei Untersuchungsjahre abgebrochen werden musste, wurde die Behandlung bei den 37 verbliebenen Patienten (74%) als erfolgreich und zufriedenstellend eingeschätzt.

Regulatorische T-Zellen (Tregs, CD4+CD25hi-Tregs) haben u. a. die Aufgabe, die Immunantwort sowie die Zytokinfreisetzung zu steuern, um die Immuntoleranz gegenüber körpereigenen Antigenen aufrecht zu erhalten. Es wurde beschrieben, dass depressive Patienten eine erniedrigte Konzentration von Tregs aufweisen. Da es Hinweise darauf gibt, dass Zytokine wie Interleukin (IL)-1IL-6 und Interferon (IFN)-eine Rolle in der Pathophysiologie der Depression spielen, und dass sich die Konzentrationen dieser Zytokine während antidepressiver Therapie ändern, untersuchten wir Veränderungen der Produktion von IL-1IL-6 und IFN- und Veränderungen der Konzentration von CD4+CD25hi-Tregs während antidepressiver Therapie. Wir gewannen dazu das Blut von 16 Patienten mit depressiver Störung in der ersten und sechsten Woche nach stationärer Aufnahme, indem wir die Plasmakonzentrationen von IL-1bestimmten. Ferner wurde die Produktion von IL-1, IL-6 und IFN-in einem Vollblut-Assay unter immunologischer Stimulation mit Lipopolysaccharid (LPS) oder Newcastle Disease Virus (NDV) in-vitro gemessen. Die Lymphozyten wurden differenziert und CD4+CD25hi-Tregs mittels Durchflusszytometrie bestimmt. Der psychopathologische Status wurde mit der Hamilton-Depressionsskala (HAMD-21) erfasst. Der HAMD-21-Score, die IL-1-Plasmakonzentrationen sowie die LPS-induzierte IL1-- und IL-6-Produktion waren nach sechs Wochen antidepressiver Behandlung signifikant gegenüber der Baseline erniedrigt. Dagegen stieg der Anteil der CD4+CD25hi-Tregs unter den Lymphozyten von 2,74% ± 0,88 (Mittelwert ± Standardabweichung) auf 3,54% ± 1,21 signifikant (p = 0,007) an. Es fand sich keine signifikante Änderung der NDV-induzierten IFN--Produktion. Der Anstieg der CD4+CD25hi-Tregs während antidepressiver Therapie könnte mit dem Abfall der Zytokinproduktion und der psychopathologischen Verbesserung der Patienten in einem kausalen Zusammenhang stehen.

Le syndrome cutané radio-induit (SCR) est une lésion cutanée spécifique secondaire à une irradiation localisée de forte dose, qui se caractérise par des douleurs importantes, des vagues inflammatoires extensives, un défaut de revascularisation et de cicatrisation. De nouvelles stratégies thérapeutiques peuvent être envisagées pour la prise en charge de cette pathologie avec l'émergence récente des cellules souches mésenchymateuses. Ces cellules multipotentes ont des propriétés de transdifférenciation et de sécrétion de facteurs favorisant la régénération tissulaire. L'association de la chirurgie d'exérèse, des greffes de peau autologue et des injections de cellules souches autologues isolées à partir de la moelle osseuse (Bone Marrow Mesenchymal Stem Cells ou BM-MSC) a confirmé ces hypothèses mais leur utilisation reste limitée à des cas de patients isolés.L'objectif de ce travail était d'optimiser la prise en charge du SCR par thérapie cellulaire pour l'étendre aux cas d'accidents de masse ou d'irradiation hétérogène associée.Les cellules souches dérivées du tissu adipeux (ASC pour Adipose tissue derived Stem Cells), plus abondantes que les BM-MSC et présentant des propriétés pro-angiogéniques, anti-apoptotiques et immunomodulatrices équivalentes voire supérieures, ont été choisies pour développer cet outil de thérapie cellulaire. La faisabilité de différentes stratégies thérapeutiques a été évaluée dans ce travail : la constitution de banques cellulaires autologues et allogéniques, une potentialisation des effets des ASC par thérapie génique transitoire et l'utilisation du sécrétome des ASC.La constitution de banques autologues ou allogéniques de cellules souches permet de pallier le délai d'obtention de greffons de taille suffisante en termes de quantité de cellules souches. Cette stratégie évaluée sur un modèle in vivo de miniporc a démontré la pertinence des banques d'ASC autologues mais a également mis en évidence les difficultés liées aux injections d'ASC allogéniques en particulier de type immunologique.La stratégie d'optimisation des ASC par manipulation ex vivo a pour but de potentialiser leurs effets par la sécrétion d'une protéine ad hoc impliquée dans les processus de réparation tissulaire et ainsi, de diminuer la quantité de cellules injectées et donc le temps de culture. La protéine sélectionnée dans ce travail est la protéine Sonic Hedgehog (Shh) décrite comme présentant des propriétés pro-angiogéniques et anti-apoptotiques. L'évaluation d'un protocole de thérapie génique transitoire utilisant des ASC transfectées par une construction plasmidique codant pour Shh (ASC-Shh) a montré la faisabilité de cette stratégie sur un modèle in vivo de miniporc.Les effets pro-angiogéniques de même que les effets du sécrétome des ASC et ASC-Shh sur des fibroblastes irradiés ont confirmé le rôle des facteurs paracrines des ASC dans la survie cellulaire et mis en évidence la potentialisation de leurs effets pro-angiogéniques par Shh. Ainsi l'utilisation du sécrétome des ASC ou ASC-Shh apparaît également comme une stratégie pertinente permettant de s'affranchir de la compatibilité donneur-receveur lors des greffes de cellules souches.Ces différentes techniques basées sur les cellules souches dérivées du tissu adipocytaire représentent donc des stratégies prometteuses pour la prise en charge du syndrome cutané radio-induit<br>Cutaneous radiation syndrome (CRS) is a specific skin lesion caused by high dose local irradiation. It is characterized by severe pain, extensive inflammatory waves, incomplete revascularization and wound healing. For the last few years, mesenchymal stem cells (MSC) have become a promising cell source for cell therapy. MSC are multipotent cells which can transdifferentiate and secrete many paracrine factors involved in tissue regeneration, and they may be considered as a new therapeutic strategy for the management of this pathology. This was confirmed by the results of works associating surgical resection, autologous skin graft and autologous bone marrow mesenchymal stem cells (BM-MSC) injections, although the use of the latter remains limited to isolated cases of patients.The aim of this work was to optimize the management of CRS using stem cell therapy in order to extend it to mass casualties or associated heterogeneous irradiation.Adipose tissue derived stem cells (ASC) were chosen to develop cell therapy as they are more abundant than BM-MSC and exhibit higher pro-angiogenic, anti-apoptotic and immunomodulation properties.The feasibility of different therapeutic strategies has been evaluated in this work: the establishment of autologous and allogeneic cell banks, potentiation of ASC effects by transient gene therapy and the use of ASC secretome.The constitution of autologous or allogeneic stem cell banks overcomes the cell expansion delay required to obtain stem cells of clinical quantity. This strategy was evaluated in an in vivo minipig model of CRS and the relevance of autologous ASC banks was shown but this study highlighted difficulties associated with allogeneic injections especially immunological ones.Here is also reported an ASC optimization by ex vivo manipulation that aims to decrease the amount of injected cells and culture time through the secretion of an ad hoc protein involved in tissue repair processes and thus to, decrease the amount of injected cells and therefore the culture time. The protein chosen was Sonic Hedgehog (Shh), known to be involved in pro-angiogenic and anti-apoptotic processes. The evaluation of a transient gene therapy protocol using ASC transfected with a plasmid construct encoding Shh (ASC-Shh) confirmed the feasibility of this strategy in the same minipig model of CRS.The pro-angiogenic effects as well as other effects of ASC and ASC-Shh secretome on irradiated fibroblasts have confirmed the role of ASC paracrine factors in cell survival and demonstrated potentiation of their pro-angiogenic effects by Shh. Thus the use of ASC or ASC-Shh secretome also appears as a relevant strategy to overcome the donor-recipient compatibility during stem cell transplants.These techniques based on adipose tissue derived stem cells represent promising strategies for the management of cutaneous radiation syndrome

Abstract:
Jährlich erkranken in Deutschland über 11.000 Patienten an Pankreaskarzinomen. Für die ganz überwiegende Mehrzahl dieser Patienten ist die Diagnosestellung gleichbedeutend mit einem Todesurteil innerhalb von wenigen Wochen, hauptsächlich bedingt durch das meist fortgeschrittene Krankheitsstadium bei Diagnosestellung sowie die relative Therapieresistenz des Tumors. Die vorliegende Arbeit faßt wissenschaftliche Untersuchungen zusammen, die in den zurückliegenden vier Jahren in Berlin zu dieser Thematik durchgeführt wurden. Stadienadaptiert wurden Behandlungskonzepte und klinische Studien entwickelt und durchgeführt, die von der adjuvanten Chemotherapie über ein Radiochemo-therapiekonzept bei lokal fortgeschrittenen Stadien hin zur Entwicklung einer neuen Zytostatika-Kombinationstherapie für die Behandlung metastasierter Pankreastumoren im Rahmen einer multinationalen Phase III Studie geführt haben. Neben diesen Schemata konnte auch eine wirksame Zweitlinientherapie geprüft werden. An Tumormaterial konnte gezeigt werden, daß sich keine Her2/neu-Überexpressionen bei Pankreastumoren nachweisen lassen, die therapeutisch nutzbar wären. Bei ca. 80 % aller Pankreaskarzinome lassen sich Mutationen des K-ras-Onkogens nachweisen. In Kooperation mit einer anderen Arbeitsgruppe wurde ein Verfahren entwickelt, mit dem sich qualitativ und semiquantitiativ schnell und zuverlässig ras-Mutationen nachweisen lassen. Diese klinischen Studienergebnisse geben Grund zur Hoffnung, die Prognose von Patienten mit Pankreaskarzinomen in den nächsten Jahren durch die hier vorgestellten Konzepte zu verbessern. Fortschritte im Verständnis der molekularen Karzinogenese, in Diagnostik und Therapie lassen in naher Zukunft Ergebnisse erwarten, die zumindest denen bei anderen soliden Tumoren nahekommen. Daher ist der vereinzelt noch verbreitete therapeutische Nihilismus bei der Behandlung des Pankreaskarzinoms als nicht länger gerechtfertigt und akzeptabel anzusehen.<br>In Germany, more than 11,000 patients are diagnosed with pancreatic cancer each year. For the vast majority, this means a death verdict within a few weeks, primarily due to the advanced stage of the disease at diagnosis and the relative chemoresistance of the tumor. This thesis summarizes the scientific work regarding pancreatic cancer that has been done within the last four years. Several clinical treatment concepts and studies were developed and conducted that covered the different stages of the disease, including adjuvant therapy, radiochemotherapy for locally advanced disease and a multinational phase III study for the patients with metastatic disease. In addition, an effective second line regimen was developed. Using tumor material, we found no overexpression of Her2/neu which would have been a therapeutically usable target. Mutations of the K-ras oncogene can be found in approximately 80% of patients with pancreas carcinoma. A method for a rapid and reliable qualitative and semiquantitative determination detection of ras mutations was developed in cooperation with another research group. The clinical results give rise to the hope that the prognosis of patients with pancreatic carcinoma can be improved during the next years using the concepts outlined above. Recent improvements in our understanding of the molecular carcinognesis together with advances in diagnostics and therapy give rise to the expectation that clinical results will be achievable that will be at least as good as those for other solid tumors. Therefore, nihilism regarding the treatment of pancreatic cancer that still can be found is no longer justifiable or acceptable.

A música erudita selecionada a partir de um repertório pré-determinado foi investigada neste estudo como terapia complementar no cuidado de mulheres com fibromialgia. Quarenta mulheres foram submetidas a duas sessões musicais, observando-se o intervalo de uma semana entre elas. A dor, bem como os parâmetros fisiológicos indicativos de relaxamento: freqüência cardíaca, freqüência respiratória, eletromiografia dos músculos trapézio e frontal, pressão arterial sistólica e diastólica e temperatura cutânea; complementados pela percepção das pacientes sobre a experiência musical foram avaliados antes e após cada sessão musical para verificação das alterações produzidas em sua decorrência. As pesquisadas apresentaram uma preferência musical que tendeu às músicas orquestrais em relação às de câmara e solo. Dentre os sinais fisiológicos observados, a freqüência respiratória, a eletromiografia, a temperatura cutânea e a pressão arterial sistólica apresentaram alterações estatisticamente significantes nas duas sessões. A pressão arterial diastólica apresentou alterações significativas apenas na segunda sessão. A dor avaliada pela Escala Analógica Visual apresentou redução significativa pós-sessão musical para a maioria das pacientes, ratificada pelo Questionário McGill em todas as dimensões da dor que este instrumento se propõe a analisar (sensitiva-discriminativa, afetiva-motivacional, cognitiva-avaliativa). A percepção da experiência musical relatada pelas pesquisadas evidenciou o potencial desta intervenção de Enfermagem (95% referiram que gostaram muito desta proposta terapêutica), pois além do alívio da dor, diversos aspectos subjetivos emergiram, tais como alterações nos estados de ânimo, facilitação da introspecção, visualização de imagens e sensações estéticas, dentre outros, demonstrando que sua utilização e compreensão merecem ser ampliados.<br>The selected erudite music starting from a predetermined repertory was investigated in this study as alternative therapy in the women care with fibromyalgia. Forty women were submitted for two musical sessions, we observed the interval of one week between the sessions. The pain as well as the physiological parameteres indicative of slackness: heart rate, respiratory rate, trapezius and frontalis muscle, systolic and diastolic blood pressure and cutaneous temperature and all of this parameters were completed by patients perception about musical experience and were appraised before and after each musical session for verification of produce alterations in their results. The enquired women have presented a musical preference that reached to the orchestral songs with reference to the camara music and solo. Among observed physiological signs, the respiratory rate, the electromyographic, the cutaneous temperature and the sistolic blood pressure presented alterations statistically significative on two sessions. The diastolic blood pressure presented significative alterations in the second only. The pain valued by Visual Analogic Scale presented significative reduction after musical session for most of patients, ratified by McGill Pain Questionnaire in all pain extensions that the instrument proposes to analyse (sensitive, affective, and cognitive). The perception of musical experience related by investigated women have evidenced the powerful from this Nursing intevention (95% from those women refered that they have liked very much this therapeutic approach) besides pain relief, many subjective aspects have emerged such as alterations of mood state, facility of introspection, visualization of images and esthetics sensations, among others aspects have demonstrated that their utilization and comprehension to must be amplified.

Abstract:
Im Rahmen der vorliegenden Arbeit wurden die in der Fachliteratur am häufigsten beschriebenen Formen der aviären Frakturtherapie anhand von 147 Wildvogelpatienten mit 237 gebrochenen Knochen im Hinblick auf ihre Einsatzmöglichkeiten, den Stellenwert, sowie auf Vorteile und Risiken der jeweiligen Methoden überprüft und die in der Literatur getroffenen Aussagen den Ergebnissen der eigenen Untersuchungen gegenübergestellt. Weiterhin wurden die publizierten Informationen bezüglich therapiebegleitender Maßnahmen einer kritischen Prüfung unterzogen und um die eigenen Erkenntnisse ergänzt. Rund 55 % der therapierten Wildvögel konnten nach Abschluß der klinischen Versorgung ausgewildert werden, wobei die therapeutische Erfolgsquote von Patienten mit frischen Frakturen und denjenigen, deren Frakturereignis wenige Tage zurücklag nur unwesentlich differierte. Demgegenüber wiesen Tiere mit älteren oder bereits in Heilung befindlichen Knochenbrüchen eine schlechte Prognose auf. Fast 27 % der Vögel verstarben während der Behandlung an den Folgen ihres Traumas oder einer Begleiterkrankung, etwas mehr als 13 % wurden aufgrund eines unbefriedigenden Heilungsverlaufs oder Therapieversagens euthanasiert und in etwa 5 % der Fälle reichte das erzielte Ergebnis zur Wiedereingliederung in die freie Wildbahn nicht aus. Von den untersuchten Therapiemethoden erwies sich lediglich die intramedulläre Versorgung mittels kurzer Polyacrylstäbe nach der „Shuttle-Pin“-Methode sowie die Kombination dieser Implantate mit Knochenzement bei Patienten unter 300 g Körpergewicht als problematisch. Bei einem der auf diese Weise behandelten Patienten wurde zudem eine therapieresistente Osteomyelitis als Folge einer Fremdkörperreaktion auf das eingebrachte Material dokumentiert. Positive Erfahrung konnte in zwei Fällen mit der Verwendung eines die Markhöhle weitestgehend ausfüllenden Polyacrylstabes bei ausgedehnten Splitterfrakturen gesammelt werden – eine bislang nicht beschriebene Variante bei der Versorgung mit Markraumimplantaten. Sowohl die Therapie der geschädigten Knochen mit einem modifizierten Fixateur externe bei geeigneter Frakturcharakteristik, als auch die Markraumnagelung mit Kirschner-Drähten erbrachten zumeist die angestrebten Ergebnisse. Der in der Literatur häufig betonte Vorteil kurzer intramedullärer Implantate, wie auch des Fixateur externe gegenüber einer Markraumnagelung mit Stahlstiften aufgrund einer früheren Beübungsfähigkeit der verletzten Gliedmaße konnte anhand der eigenen Ergebnisse nicht bestätigt werden. Klinische Vorteile infolge einer ungehinderten endostalen Kallusbildung bei Verwendung des Fixateur externe im Vergleich zu intramedullären Implantaten waren nicht zu ermitteln. Die Zeitdauer bis zur Auswilderung der Patienten war in der überwiegenden Zahl der Fälle weniger von der Therapieform, als vielmehr von Frakturausmaß und -lokalisation bestimmt. Die Ergebnisse der konservativen Behandlung waren – eine entsprechende Eignung der Fraktur für das Verfahren vorausgesetzt – bei vergleichsweise geringem Risiko den operativen Methoden zumindest ebenbürtig. Für eine chirurgische Versorgung von Schultergürtelfrakturen ergab sich keine zwingende Indikation; selbst hochgradig dislozierte Coracoidfrakturen konnten mittels konservativer Behandlung funktionell wieder hergestellt werden. Die röntgenologische Kontrolluntersuchung in siebentägigen Intervallen während der ersten 3 Behandlungswochen bewährte sich dabei im Hinblick auf das zeitnahe Befunden von Therapiezwischenfällen, ausbleibenden Therapieerfolgen und insbesondere zur frühzeitigen Diagnose entzündlicher Knochenveränderungen<br>This study examined the methods of avian fracture therapy most commonly described in scientific literature in regard to their usability, their significance/value as well as their advantages and risks. For this purpose 147 wild birds with 237 fractured bones were treated with different methods, depending on the type of fracture. The results were compared to relevant statements found in specialist literature. In addition, published information about supportive treatment was analysed and supplemented with specific additional findings. It was possible to release about 55% of the treated wild birds back into the wild after therapy. The rate of success in therapy showed little difference between patients with fresh fractures and those with fractures that had occurred a few days previously. In contrast to this, animals with older fractures or fractures that were already in an advanced stage of untreated healing process showed a poor prognosis. Almost 27% of the birds died during therapy as a consequence of the initial trauma or accompanying diseases, a little more than 13% had to be euthanized because of an unsatisfactory healing process or failure of fracture repair and in about 5% of the cases the results were not sufficient for rehabilitation into the wild. Among the examined methods of therapy only intramedullary application of short polyacryl rods with the shuttle pin technique and the combination of these implants with bone cement in patients with a body-weight below 300g showed unsatisfactory results. One patient treated by this method developed a therapy-resistant osteomyelitis as a consequence of a foreign-body reaction. Positive results were achieved in two cases with the use of a polyacryl rod filling most of the bone cavity in extended comminuted fractures – a variation of internal fixation not previously described. Treatment with a modified external fixator as well as intramedullary fixation with Kirschner wires resulted in the required outcome in most cases whereas unimpaired endosteal callus formation, often associated with the use of an external fixator, did not result in clinical advantages. Trials did not confirm an earlier return to function by the use of short intramedullary implants or of the external fixator compared to internal fixation with intramedullary pins, which is often emphasised in scientific literature. In most cases duration of hospitlisation and of captivity depended on the extent and site of bone injury rather than on method of therapy. Results of conservative treatment and surgery were similar in terms of low risk – as long as the method of therapy matched the type of fracture. There was no requirement for surgical therapy of fractures of the shoulder girdle. Even severely dislocated fractures of the coracoid returned to normal function through conservative treatment. Weekly radiological examination at intervals of seven days during the first three weeks of therapy proved essential to identify early complications in the healing of the fracture, failure in the healing process, or, in particular, early diagnosis of osteomyelitis

Projeto de Pós-Graduação/Dissertação apresentado à Universidade Fernando Pessoa como parte dos requisitos para obtenção do grau de Mestre em Ciências Farmacêuticas<br>A fibromialgia é uma doença crónica do foro músculo-esquelético com origem ainda desconhecida. Esta doença pode apresentar vários sintomas como dor, fadiga, alterações de comportamento, ansiedade, depressão, alteração do sono, por exemplo. O diagnóstico desta doença nem sempre é fácil porque os sintomas relatados pelos doentes são muitas vezes característicos de outras doenças, podendo levar a um diagnóstico errado. O tratamento da doença é sintomático com objectivo de diminuir todos os sintomas para que os doentes possam ter uma qualidade de vida melhor. São normalmente utilizados fármacos com actividade analgésica fraca ou forte consoante a intensidade da dor, antidepressivos, ansiolíticos, anticonvulsivantes e relaxantes musculares. A terapia farmacológica sozinha muitas vezes não é totalmente eficaz, havendo necessidade de recorrer a terapias alternativas como a acupunctura, prática de exercício físico, terapia usando laser, entre outras. Apesar de existirem alguns estudos sobre a fibromialgia, ainda há um longo caminho a percorrer para se entender completamente esta doença. Fibromyalgia is a chronic musculoskeletal disorders with unknown origin. This disease can have symptoms such as pain, fatigue, behavior changes, anxiety, depression, sleep disturbance, for example. The diagnosis of this disease is not always easy because the symptoms reported by patients are often characteristic of other diseases and may lead to misdiagnosis. Treatment of the disease is symptomatic with the aim of reducing all symptoms so that patients can have a better quality of life. Are commonly used drugs with analgesic activity weak or strong depending on the intensity of pain, antidepressants, anxiolytics, anticonvulsants and muscle relaxants. The pharmacological therapy alone is often not fully effective, no need to resort to alternative therapies such as acupuncture, physical exercise, using laser therapy, among others. Although there are some studies on fibromyalgia, there is still a long way to go to fully understand this disease.

In dieser Masterthese werden Zwischenergebnisse einer dreijährigen Studie vorgestellt, die eine resektive mit einer regenerativen Methode zur Behandlung von Periimplantitis vergleicht. Derzeit liegen Daten über einen sechsmonatigen Beobachtungszeitraum für vier Patienten in der (resektiven) Kontrollgruppe und fünf in der (regenerativen) Testgruppe vor. Patienten beider Gruppen mit einer extensiven Parodontitisvorgeschichte präsentieren sich in der Regel mit größeren Sondierungstiefen. Größere Sondiertiefen zu Beginn der Behandlung zeigen eine Tendenz zu schlechteren Ergebnissen im Verlauf der Beobachtungsphase, wobei der kausale Zusammenhang dazu in einer ähnlichen Pathogenese der Parodontitis und des schlechten Periimplantitisbehandlungserfolges liegen kann, die durch dieselben Risikofaktoren beeinflusst wird (Mundhygiene, genetische Faktoren, Mundflora etc.). Die Wundabdeckung nach der Augmentation scheint eine noch bedeutendere Rolle für die Prognose zu spielen als die Parodontitisvorgeschichte. Wund- und Nahtdehiszenzen sind die häufigsten Komplikationen in der Testgruppe. Bei einem Patienten der Testgruppe entwickelte sich aufgrund einer Wunddehiszenz eine fulminante Infektion und Entzündung, die letztlich zum Verlust des Implantates führte. Sondiertiefen und PES der Testgruppe zeigen drei und sechs Monate nach der Behandlung eine deutliche Verbesserung, wobei der Unterschied in den durchschnittlichen Sondiertiefen zwischen Behandlungsbeginn und drei oder sechs Monate danach z. T. statistisch signifikant war. In der Kontrollgruppe veränderten sich dagegen die Sondiertiefen und der PES nach der Behandlung nicht. Insgesamt sind die Ergebnisse vielversprechend mit deutlichen Verbesserungen des BOP und der Sondiertiefe, jedoch ist der Erfolg der Augmentation von der Operationstechnik abhängig. Für die Beurteilung eines Langzeiterfolges ist es derzeit noch zu früh<br>This master thesis presents intermediate results of a three-year study that compares a resective with a regenerative method for the treatment of peri-implantitis. Currently, data are available over a six-month observation period for four patients in the (resective) control group and five in the (regenerative) test group. Patients in both groups with an extensive history of peri-ondotitis present usually with larger probing depths. Larger probing depths at the beginning of treatment show a tendency to result in worse outcomes during the monitoring phase. The causal relationship of this could be a similar pathogenesis of both peri-odontitis and a unfavorable outcome of the peri-implantitis treatment, which is influenced by the same risk factors (oral hygiene, genetic factors, oral flora, etc.). The wound covering after augmentation seems to play an even greater role for the prognosis compared to a history of peri-ondotitis. Wound and suture dehiscence are the most common complication in the test group. In one patient of the test group, a fulminant infection and inflammation developed as a result of wound dehiscence that ultimately led to the loss of the implant. Probing depths and PES in the test group at three and six months after treatment showed a significant improvement compared to the baseline before the treatment. This difference in the average probing depths between baseline and three or six months thereafter was in part statistically significant. In the control group, however, the probing depths and the PES did not change after treatment. Overall, the results are promising with significant improvements in the BOP and the probing depth, but the success of the augmentation depends on the surgical technique. At this point it is still to early to assess long-term success

Abstract:
Prostate Cancer is the most commonly diagnosed cancer in men. The incidence of prostate cancer has risen over the last two decades, although mortality rates have not increased to the same extent. For many men with localised prostate cancer the choice of treatment lies between radical surgery or radiotherapy and active surveillance. Evidence suggests that the survival benefits related to radical treatment are small, and that conservative management may be appropriate for many men. A strategy of focal therapy in prostate cancer aims to treat tumour foci within the prostate, without treating the whole gland, with a resultant reduction in side-effects. Photodynamic therapy (PDT) is the use of a photosensitising drug, which when activated by light can cause cellular damage. Tookad is a new-generation photosensitiser, which is activated in vasculature. Phase I and II clinical trials of Tookad PDT in men with recurrent prostate cancer following radiotherapy have been performed. These studies show that it is possible to ablate prostate tissue with Tookad PDT. This thesis describes a phase I/II clinical trial evaluating the safety and efficacy of Tookad PDT in men with primary prostate cancer. Treatments were performed under general anaesthetic, using 2mg/kg Tookad and interstitial light delivery via cylindrical diffuser fibres. 15 patients were included in part A (single-fibre, light-dose-escalation), 19 patients in part-B (multi-fibre, focal and whole-gland ablation). Follow-up was for 12-months, with assessment of treatment effect made on MRI. Post-treatment imaging demonstrated that prostate tissue can be ablated in a reproducible and controlled manner with Tookad PDT. Although genitourinary side-effects were relatively few, significant cardiovascular complications in this study lead to early termination of the clinical trial and withdrawal of the drug. Results suggest that Tookad PDT could potentially deliver either focal or whole-gland therapy. Further studies with modified photosensitsers are underway.

[137]-146).<br>Machine Therapy is a new practice combining art, design, psychoanalysis, and engineering work in ways that access and reveal the vital, though often unnoticed, relevance of people's interactions and relationships with machines. Machine Therapy will be illustrated through the construction of several systems including re-appropriated domestic devices such as Blendie, wearble apparatuses such as ScreamBody, and body-signal-based companion machines - Umo, Amo, and Omo - that function through visceral interactions including breathing and non-verbal sounds. These systems will be used to explore themes of human-machine relations in terms of visceral, cathartic, and reflexive expressions and communications. This work incorporates elements from my technical research in digital signal processing, machine learning, mechanical engineering, and sensor design. Combining these areas of research and practice, I have been able to help manifest new objects and relationships that are unique in some aspects while maintaining quotidian familiarity in other aspects. These apparatuses enable unusual explorations of what we interact with when we interact with machines. I hypothesize that the answer will turn out to be much more than the machine itself, and will include our sense of self, agency in the interpersonal and political world, and our shared psychological, emotional, cultural, and perceptual approaches to the world. Las fuentes de ingreso para Therapy Kids serán las terapias a domicilio y las consultas del médico terapista., en el primer año, se espera un ingreso de S/5 682 600 sin IGV y la inversión a realizar en dicho año para lograr los objetivos trazados asciende a S/ 285,202.82, de los cuales el 73% provendrá de los accionistas y se buscará un financiamiento por el 27% restante. Abstract:
The purpose of this study was to determine the learning styles of Doctor of Physical Therapy (DPT) students and associate degree Physical Therapist Assistant (PTA) students and identify any association between their learning styles and examine the association between gender and age by learning style. Participants included 337 DPT and PTA students attending CAPTE accredited institutions with doctoral DPT or associate PTA programs in Tennessee and southwest Virginia. The Felder (1996) and Soloman Index of Learning Styles (ILS) was used to determine learning style preferences within 4 learning style dimensions (active-reflective, sensing-intuitive, visual-verbal, and sequentialglobal). Demographics included program of study, gender, age, ethnicity, and highest level of education. Participants were 18-63 years (mean age 25.87, standard deviation 5.62, median age 24); 205 (60.8%) DPT students, 132 (39.2%) PTA students; 205 (60.8%) female, 132 (39.2%) male. Five research questions were evaluated using cross-tabulated tables with frequency counts, percentages, and chi square tests. Statistical significance was established using a .05 alpha. There was a significant difference in the active-reflective learning style among PTA students by age. However, there was no significant difference between the learning styles of DPT and PTA students. Participants were found to be balanced on the active-reflective dimension, sensing on the sensing-intuitive dimension, visual on the visual-verbal dimension, and balanced on the sequential-global dimension. All students displayed preferences were toward the active, sensing, visual, and sequential learning styles. This findings demonstrated that DPT and PTA students have a balanced learning style with a strong preference toward active, sensing, visual, and sequential. Therefore, teaching methods should provide an instructional environment that addresses these learning style preferences. The student’s awareness of his or her learning style will enable the learner to capitalize on strengths and develop areas of weakness. This ability to employ effective learning strategies will equip an individual for the challenges of his or her chosen profession and lifelong learning.
