 In recent years, several studies have demonstrated the involvement of the intestinal microbiota in immune-mediated diseases such as diabetes, ulcerative colitis, and multiple sclerosis. There are few data on the follicular microbiome and its role in the pathogenesis of scalp diseases. Some studies show influence of dysbiosis on these diseases, and manipulation of the microbiome may represent a possible therapeu- tic option. This article reviews current knowledge regarding the impact of dysbiosis on dermatological diseases of the scalp, such as seborrheic dermatitis, psoriasis, alopecia areata, androgenetic alopecia, lichen planus pilaris, frontal fibrosing alopecia and decalvant folliculitis. A broader understanding of this may suggest additional treatments beyond conventional therapies.

Glucocorticoids are a class of steroid hormones, vital for mammalian life. They have a plethora of biological effects, mainly supporting metabolic, cognitive, and immunological functions. The pharmacological use of glucocorticoids makes them one of the most frequently prescribed drugs across all continents, and in all types of forms. Nevertheless, a number of serious adverse effects accompany the prolonged treatment with high doses of glucocorticoids. Research developments over the last 20 years have gradually reshaped the way we think about glucocorticoid-based therapeu¬tics. Aside their circadian rhythm and their delayed regulatory influence over an extensive number of sensitive genes, glucocorticoids also possess an underlying, ultradian rhythm, and also exert rapid, non-genomic effects. The notion that chronicity of glucocorticoid stimulation may differentially modulate the type of biological effects of the hormone brings various chronopharmacological concepts on the table of modern glucocorticoid-based therapeutics.

Abstract:
Colorectal cancer (CRC) is the third most common cancer and is associated with a poor prognosis. The mutation profile and related involved pathways of CRC have been, in broad terms, analyzed. The main current therapeutic approaches have been comprehensively reviewed here, and future possible therapeu-tic options and related technologies have been perspectively presented. The complex scenario represented by the multiple-level resistance mechanism in the epidermal growth factor receptor (EGFR) pathway, including mutations in KRAS, NRAS, and BRAF V600E, is discussed. Examples of engineered therapeutic approaches from the literature along with a drug combination tested in clinical trials are discussed. The encouraging results observed with the latter combination (the BEACON clinical trial), totally free from chemotherapy, prompted the authors to imagine a future possible nanotechnology-assisted therapeutic approach for bypassing multiple-level resistance mechanisms, hopefully allowing, in principle, a complete biological cancer remission.

Neem&nbsp;<em>Azadirachta indica</em>&nbsp;is a useful traditional medicinal plant growing in Nigeria, India, and America. The phytochemicals and the biopesticidal components present were ascertained. The results showed that saponins, steroids and terpenes were mostly present, while tannins and glycosides were moderately present, and alkaloids, flavonoids, phenols and oxalic acid were least present. The presence of these phytochemical could account for the therapeutic uses of neem.

Quinapril HCl is a novel, nonsulf hydryl angiotensin-converting en zyme (ACE) inhibitor. The Study Group evaluated its efficacy (20, 40, 80 mg daily with forced dose titration determination at trough blood pres sure) once daily versus twice daily versus placebo, as well as its tolera bility and safety, in 270 patients with mild to moderate essential hyperten sion (WHO Stages I and II, sitting diastolic blood pressure [DPB] ≥95 mm Hg), for twelve weeks. Reduc tions in DBP of up to 13 mm Hg were obtained, and in full dosage more than 65% of patients achieved a re duction in DBP of 10 mm Hg or more from baseline or reduced their DBP to 90 mm Hg or less. Quinapril was well tolerated, and reported adverse effects were scarcely more frequent than in the placebo group. Once daily doses of quinapril were as safe and effective as twice-daily doses. Quina pril is likely to exhibit good therapeu tic utility in the management of essential hypertension.

Bifidobacteria represent the largest group of human intes nal bacteria. They have an important place in human health and represent the dominant group of micro ora. For this reason, they are key members of the human gut microbiota, particularly during the first one to two years following birth. The aim of this study was to test the sensibility of Bifidobacterium isolated from some strains at different concentrations of salinity and high concentration of bile as a criterion to select probiotic strains. Following the behavior of strains of Bifidobacteria isolated from different ecosystems was considered in order to develop the therapeu c aliment food . Samples from newborn infants aged 3 months, bee intestine, fermented commercial milk and yoghurt of bifidus containing Bifidobacteria was used and the isolated strains belonged to -breve and -longum species. The results of resisitance of our species of Bifidobacteria demonstrated a total inibition of croissance at 5.5% of NaCl but in 5% media of NaCl we observed a moderate croissance at di erent me of kinetics croissance .

Innovative immunotherapy in the form of Immune Checkpoint Inhibitors (ICIs) has significantly improved outcomes for patients with numerous metastatic cancers. A double-edged sword, we are increasingly learning about the immune-related adverse events (irAEs) associated with use of these novel therapeu- tic agents.Here we present the case of a 53 years old woman with background of malignant melanoma of the leg and metastatic lesions to the lung and brain. She commenced treatment with dual immunotherapy with Nivolumab/Ipilimumab with subsequent resolution of both metastatic lesions.Two years following the completion of immunotherapy, she developed left sided arm and leg weakness, hemianopia, left sided Epilepsia Partialis Continua (EPC) and cognitive decline. MRI showed diffusion res- triction along the active cortex during the period of EPC that settled as AEDs suppressed clinical seizure activity. However, while there was no tumour recurrence with interval oncology MRI, there was progressive asymmetrical atrophy of the right cerebral hemisphere with associated white matter signal abnormality in keeping with Rasmussen’s encephalitis scan changes.We propose that this represents a late delayed reaction to immunotherapy, beyond the recognised encephalitis that was the likely cause of EPC. Increasing use of ICIs will likely reveal more cases.

Chronic atrophic gastritis (CAG) is a precancerous atrophic gastritis of the stomach, which generates an urge to develop novel therapeu-tic schedules. This study aimed to investigate the effect of experimental folic acid administration on N-methyl-N’-nitro-N-nitrosoguanidine (MNNG)-induced CAG through the PI3K/Akt pathway in rats. The rats were divided into a Model Group, a Folic Acid Group and a Blank Group. Rats in the Model Group were induced by MNNG and given 10 mL/kg/d distilled water by gavage, while rats in the Folic Acid Group were induced by MNNG and given 5 mg/kg/d folic acid suspension by gavage. As a control, rats in the Blank Group were given the same amount of distilled water as MNNG and 10 mL/kg/d distilled water by gavage. The levels of gastrin (GAS) and motilin (MTL) in serum were measured by enzyme-linked immunosorbent assay (ELISA), and the mRNA and protein ex-pressions were detected by quantitative polymerase chain reaction (q-PCR) and Western blot. According to hematoxylin and eosin (H&amp;E) pathological analysis, there were inflammatory factors infiltration and derangement of mucosal epi-thelial cells in the model group, while the gastric tissue injury in the folic acid group was improved. Folic acid could decrease the content of GAS, increase the content of MTL in the serum of the rats, and regulate the expression of PI3K and AKT signal pathways. Folic acid can have a therapeutic effect on CAG by reducing the concentration of GAS in serum and increasing the concentration of MLT in serum. Our study would lay a theoretical foundation for using folic acid to investigate new therapies for CAG in humans.

Pathomorphological research of dogs intes-tine allowed us to describe in more detail all encountered cases of pathological manifesta-tion and effect of broad tapeworm on differ-ent tissue structures and their frag-ments.Enteropathies are polymorphous and are introduced with different etiological fac-tors. Intestine helminthiases have heavy share of ethiofactors.Microscopic signs of intestinal lesion of dogs during diphylloboth-riasis were characterized with obvious changes in mucosa and submucosa and their damage pathological changes in a smooth muscle layer were rarely noted.Serous coat and all its structural fragments complied with the norm and didn't have pathological. Extensive zones of damage from mehanical injury were not noted,there were areas of gut wall changes with damage and intestinal villi destruction and epithelial membrane destruc-tion possibly caused by morphological and physiological structural features of the para-site. Characteristic pathological signs were symptomatic desquamation of tessellated epithelium.Extensive damage of serous coat tracedin all the microslides prepared for his-tologic study.All these changes can be a pos-sible reason of development of reduction of nutrient intake.Such kind of research as pathological process encourages more de-tailed identifying of clinical status during diphyllobothriasis and its not characteristic or specific manifestations and will be the additional criteria of high-impact therapeu-tics or its evidence.

Autoimmune hemolytic anemia (AIHA) has become recognised as a complex and heterogeneous disease characterised by a wide range of immunopatholog- ical and biochemical mechanisms. Several treatment options exist in the field of AIHA however; there is as yet little or no evidence of therapeutic strategies targeted on molecular aspects of the disease. An up-to-date comprehensive sys- tematic review covering immunopathology for emerging areas of experimental therapy of AIHA will help direct future research and optimize clinical deci- sions, by focusing on issues needing improvement. This structured systematic review will focus on the immunopathological molecular mechanisms and the most recent emerging therapeutic strategies for AIHA or emerging therapeu- tic strategies targeted at the molecular mechanisms of the disease. The aim of the review is to collect, critically evaluate, and systematically summarise all available evidence on the immunopathology and experimental therapy of AIHA. For the immunopathology of AIHA, the main issues of interest are: the initiation and development of the disease, the immunological and biological mechanisms of red blood cell destruction, diagnostic markers predicting the response to therapy, the relationship with other autoimmune disorders and aspects of the genetics of AIHA. Experimental therapy will include newly ap- plied or planned treatment protocols and compounds, or medications present- ing disease-modifying and non-conventional mechanisms of action. This review will be oriented toward researchers and clinicians engaged in the management and research of AIHA, and results will be analysed and presented in light of their potential to translate findings into practice. Eligibility for inclusion criteria, salience assessment, and data extraction analysis aims to critically synthesize available data, identify research gaps and assess the quality of ev- idence. Substantial advances in the comprehension of the immunopathology of the disorder or the emergence of new advanced science verified drugs and other compounds in therapy will be explored. Summarised results will point toward research fields most urgently needing further investigation and provide a comprehensive synthesis of evidence helpful for the optimization of clinical decisions and the initiation of innovative therapies.

Abstract:
Tooth extraction is a common oral surgical procedure in dental practice. Indications for this procedure include extensive caries, trauma, inflammatory processes, cystic and tumorous formations, as well as orthodontic and pros- thetic needs of the patient. The development of implant technology and aesthetic dentistry has necessitated the preservation or creation of a biological base after tooth ex- traction. Alveolar preservation has been recommended as an effective, economical, and simple method to maintain the contours of the bone base. For this purpose, a variety of materials and techniques are used, including autografts, allografts, xenografts, and alloplastic materials, which offer osteogenic, osteoinductive, and osteoconductive abilities. An improved method of obtaining platelet-rich fibrin (PRF) was presented by Dr. Choukroun in 2001, offering a simple and more effective alternative compared to previous tech- niques. In this case report, a 42-year-old patient underwent multiple tooth extractions with alveolar preservation using PRF. After the application of the PRF protocol, the clinical width of the alveolus was evaluated, and clinical parame- ters were monitored for 3 months. The results indicate that the use of PRF for alveolar preservation provides an effec- tive and safe method for promoting wound healing and bone regeneration after tooth extraction. Furthermore, the study’s conclusions confirm that PRF is a valuable therapeu- tic material for alveolar preservation after tooth extraction and suggest that this method may yield optimal clinical outcomes for patients.

Abstract:
Based on the developed expert case record for evaluating quality and practices of medical care the most significant and common defects in medical service are identified in the paper. They include incomplete anamnesis on admission (48%), insufficient physical examination (25%), lack of staged diagnosis (16%), ab-sence of correction in the dynamics of therapeutic actions (11%), partial description of patient’s condition in journals and examination data (32%). However implementation of full and timely laboratory and instrumental examinations (93%), as well as timeliness of diagnosing in 85% of cases are revealed. The program of therapeu-tic measures was chosen rationally (79%). Effectiveness and high results of medical service were achieved in 78% of cases. Advices for the rehabilitation of a patient in accordance with diagnosis and assistance rendered were given professionally and in full volume (93%). The use of a formalized approach and a mathematic model, conversion of the function of quantitative assessment of quality from 0 to 1 to the usual five-point system of evaluation of expert activity results make it possible to level difficulties in perception of ranking and simplify understanding of the assessment criteria. Detection of defects in time, reduction of the number of errors in the process of quality control, intensification of diagnostic and therapeutic measures, as well as of diagnosing should serve as a basis for optimizing medical and organizational technologies to render medical care to patients.

Abstract:
The aim: To determine the effectiveness of application of special therapeutic physical exercises to improve the indicators of body weight, anthropometric parameters and quality of life of obese women in the conditions of quarantine restrictions. Materials and methods: We examined 10 women aged 37 ± 5 years with obesity of various degrees, which was determined by the body mass index (BMI, kg/m2). All women for two months were involved in specially designed therapeutic exercises in the remote mode. Evaluation of the effectiveness of therapeu¬tic exercises use was performed using the survey method to determine the quality of life of obese women according to a short version of the questionnaire “The world health organization quality of life (WHOQOL-BREF)”; anthropometric measurements of body parts sizes; bioimpedance analysis of body weight composition; statistical methods of data processing. Results: The effect of therapeutic gymnastics according to the proposed programme on the component composition of body weight of obese women was proved, which indicates a decrease in total body weight, reduction of body fat, increase in total body water and muscle component of body weight. It is shown that under the influence of corrective physical exercises the proportions of the body of women changed, as evidenced by the dynamics of the circumferences of all measured parts of the body of obese women. The improvement of women’s quality of life in all indicators was identified. Conclusions: The use of special complexes of physical exercises, aimed at correction of the body weight of obese women,showed its significant effectiveness and led to the expected result.

Abstract:
Our studies are devoted to the search for new drugs with antioxidant, antihypoxant, immunomodulatory effects in order to in-crease the productivity of poultry. Due to the fact that beta-glucan has not previously been studied on poultry, before determining the effective therapeutic range of its doses, it is necessary to study the effect of a conditionally therapeutic dose of the drug on the body. A conditionally therapeu-tic dose - hereinafter - means the average therapeutic dose of a drug offered by the manufacturers of the respective drugs, calcu-lated for humans (mammals).The main pur-pose is to study the effect of beta-glucan on meat productivity, morphological, biochemi-cal indices of blood. In the experiment we used clinically healthy young hens of meat breeds of three months of age of the “Cobb 500” cross. The broilers of the first group (n = 9) were not prescribed the drug, they served as controls; the second group (n = 9) were given beta-glucan at 10 mg/kg body weight, corresponding to 125 g/t of food, for 14 days. The use of beta-glucan was accom-panied by an increase in the average daily body weight, gain by 4,9%, slaughter yield by 1,5%, a significant increase in blood erythrocyte content by 32,6% on day 7, monocyte level by half, a decrease in total bilirubin by 21,0%, and a decrease in blood malonic dialdehyde by 10,8% on day 14. Cumulatively all data collected indicates positive effect of beta-glucan on meat productivity, morphological composition of blood broilers and metabolism in organism and prospects of its use in poultry industry.

Abstract:
Apoptosis, necroptosis, and autophagy are cellular mechanisms by which cells are programmed to die under various physiological and devel-opmental stimuli. A multitude of protein mediators of programmed cell death have been identified, and apoptosis, necroptosis, and autophagy signals have been found to utilize common pathways that elucidate the proteins involved. This narrative review focuses on caspase-dependent and caspase-independent programmed cell death systems. Including studies of caspase-dependent pro-grammed cell death, extrinsic pathway apoptotic mechanisms, phosphatidyl-serine (PS), FAS (APO-1/CD95), tumor necrosis factor (TNF) receptor type 1 (TNF-R1) and TNF-related apoptosis-inducing ligand (TRAIL), and intrinsic or mitochondrial pathway such as cytochrome C, the Bcl-2 family of proteins and Smac/Diablo. The Bcl-2 family has apoptotic mediators Bcl-2-associated X protein (Bax) and Bcl-2 homologous antagonist/killer (Bak), Bcl-2-interacting protein BIM (Bim), Bcl-2 agonist of cell death (Bad), Bid, Bcl-2 adenovirus E1B 19kDa-interacting protein 1 NIP3 (Bnip3), BMF, HRK, Noxa and PUMA and an-tiapoptotic proteins such as Bcl-2 itself, Mcl-1, Bcl-w, A1, and Bcl-XL. Moreover, caspase-independent programmed cell death pathways include the mitochon-drial pathway with the protein mediators apoptosis inducing factor (AIF) and endonuclease G, and the pathways necroptosis, and autophagy. Understanding programmed cell death from those reported in this review could shed substantial light on the processes of biological homeostasis. In addition, identifying specific proteins involved in these processes is mandatory to identify molecular biomarkers and therapeutic targets. Furthermore, it could provide the ability to modulate the programmed cell death response and could lead to new therapeu-tic interventions in a disease.

Industrial injuries in pig farms of closed type are a very common pathology. Howev-er, nowadays, injury prevention and treat-ment of sick animals in the conditions of industrial pig breeding complex do not bring good results and require improvements. Ac-cording to the literature, there is no infor-mation about the results of the use of oint-ments and immuno- stimulators in the treat-ment of pigs with bitten wounds of the ears, tails and other parts of the body. In this re-gard, we have set a goal to develop therapeu-tic measures for injuries of piglets in a pig breeding complex of a closed type. We ana-lyzed the prevalence of industrial injuries in pigs. During the clinical examination, we studied the specific structures of injuries. The main cause of pigs injuries in industrial farming is cannibalism. In 28 days, 60% of pigs in the second experimental group, showed complete cicatrization of the wound, and 40% of animals showed this process regenerated on 85-95%, meanwhile 2 days they also had complete scarring of the wound. In 28 days, 50% of pigs of the third experimental group had a complete cicatriza-tion of the wound, and for 50% were ob-served scarring of the wound by 75-85%, and only after 4 days they demonstrated the complete scaring of the defect. For the treatment of bitten wounds as a result of developing cannibalism, it is recommended, to use local treatment of wounds with chlor-hexidine solution and argosulfan ointment daily 2 times a day and also to use the im-munostimulator “Ferrovir” in a dose of 1.0 ml/m2 per week. Such scheme of treatment of bitten wounds gives the best therapeutic effect.

Background and Aims: Wet cupping, which is known as Hijamah, is one of the most commonly practiced therapeu¬tic procedures in Saudi Arabia. Hijamah centers have been really popularized, reaching up to 119 licensed centers in the country. Despite its prevalence in the region, there is still insufficient research on such procedure. The aim of this study is to assess patients’ knowledge, beliefs, attitudes toward Hijamah, and to determine its impact on health-related quality of life. Methods: This is an observational, cross-sectional study. A total of 551 Saudi participants, both genders, above the age of 18 years, who were practicing Hijamah were included in this study. The data were collected through an online validated self-administered questionnaire from July 2020 to March 2021. The questionnaire requested information about age, gender, educational level, as well as the participants’ knowledge, attitude, and perceptions of cupping therapy. (SPSS Inc., Chicago, IL, USA) was used for statistical computations. Chi-square test was used to assess the associations. P-value of less than 0.05 was considered significant. Results: Out of 551 participants, 43.4% believe that Hijamah is an effective alternative therapy, most of them being older in age. 42.8% of diabetic participants noticed an improvement in their condition. The majority stated that Hijamah was used therapeutically, mainly for fatigue and headache. Almost all study participants (92.4%) would recommend Hijamah to others. Majority of participants (65.9%) reported they would inform their physicians before undergoing Hijamah. Lastly, (71.69 %) of participants believe their physicians did not trust in Hijamah therapy. Conclusions: This study shows that Hijamah is highly accepted in our society. Hijamah might have a positive impact on health-related quality of life, and might be used to treat or prevent chronic illnesses. Further future research is needed to understand the biological effects of Hijamah and to formulate an evidence-based knowledge.

Abstract:
The lack of standardized, efficient, and dependable lucid dream induction methods is one of the major barriers to studying the clinical and non-clinical applications of dream lucidity. Although the interest in therapeutic usage of lucid dreaming strategies in neuroscience of consciousness studies has increased in recent decades, many of the current induction approaches appear to not be replicable reliably. In this study, a new protocol for bringing on lucid dreaming was created, implemented, analyzed, and contrasted with two distinct other approaches. Techniques to induce dream lucidity include reality testing methods, the wake-and-go-to-bed technique, mnemonic initiation, senses-initiation, and a combination of the above. Before beginning their 2 weeks of lucid dream induction new protocol study, a total of 36 participants filled out a pre-test survey, maintained a dream recall diary and sleep log for the duration of the experiment, and completed a final survey. The findings showed that both the mnemonic induction and senses-initiation techniques were mildly effective in bringing about lucid dreaming. The new hybrid protocol, combining the reality check stra- tegy with dream recall amplification, performed significantly better. For lucid dream induction to work optimally, participants needed to become more proficient at remembering their dreams after waking up, so they were requested to maintain a sleep journal for 7 days before the experiment and throughout the study to enhance their ability to dream recall. There was no correlation found between the quality of one's sleep and the frequency of dream lucidity. The results demonstrated that the new induction protocol was more successful than the other two investigated methods. There are many clinical applications for dream lucidity, such as treatment of recurring and debilitating nightmares, narcolepsy, and PTSD. Therapeu- tic potential also includes future usage for anesthesia awareness and locked-in syndrome. As such, further dream lucidity research remains important.

Background. Lower back pain is one of the most common problems in the human body relat-ed to the disturbances of the motor organ’s structure and function. It challenges contemporary physiotherapy which aim is to reduce or, when possible, eliminate pain, restore fitness for eve-ryday life and prevent recurrences. The aim of the study was to compare two physiotherapy programmes in terms of lowering pain intensity, improvement of lumbar and thoracolumbar spine mobility, as well as functional ability in females with lumbar spine discopathy. Material and methods. The study included 100 women aged 45-50 with lumbar spine disco-pathy being treated in outpatient conditions. The patients were divided into two 50-subject groups depending on the programme used. The physiotherapy programme for group I patients included classic massage, kinesiotherapy and interferential currents, and in group II: classic massage, kinesiotherapy and ultrasound. The research tool was the Numeric Pain Rating Scale (NRS), the Roland Morris Disability Questionnaire (RMDQ) and measurements of lumbar and thoracolumbar spine mobility. The Wilcoxon and Mann-Whitney U tests were used for the analysis. Results. After completion of the physiotherapy programme, the women in group I experi-enced significantly lower pain (p=0.037) and higher functional efficiency (p=0,001). The range of improvement, assessed by the difference in the results of study II and III, was signif-icantly higher for the II group of women (p=0,002). One month after completion of therapeu-tic treatment, there were no statistically significant differences between the groups (p=0,169, p=0,067). Conclusions. The use of interferential currents in the physiotherapy of individuals with lum-bar spine discopathy compared to ultrasound therapy allows for greater improvement in reduc-ing pain intensity, lumbar and thoracolumbar spine mobility, as well as higher functional effi-ciency. The effectiveness of ultrasound had increased over time since the end of therapy and only after one month in the case of both physiotherapy programmes, similar effects could be observed.

Significant anthropogenic influence, the emergence of a large number of pollutants of various origins create an adverse effect on the vital activities of organisms inhabiting water bodies. Often, the consequence of such impacts is a violation of the qualitative and quantitative composition of the microbiota - the autochthonous specific water body and the one that forms the microbiocenosis of the organism of aquatic animals, which leads to the development of diseases. Recently, the number of studies indicating the positive effect of the use of probiotics as preventive, therapeu-tic agents and biological factors affecting water quality has been increasing. Probiotic microorganisms not only perform a protective function against pathogens through the production of short-chain fatty acids, bacteriocins, lyso-zyme, hydrogen peroxide, but also synthesize numerous enzymes and metabolites to provide digestive, regulatory, trophic, immunomodulatory and immunostimulatory functions. The range of studied probiotics is represented not only by microorganisms traditional for aquaculture (for example, Bacillus), but also by representatives of the genus Lacto-bacillus, the presence of which is recorded in individual biotopes of fish. We have studied the effect of Lactobacillus сasei probiotic microorganisms introduced into the feed on some individual hematological indicators of Carassius gibelio Bloch and analyzed changes in general behavioral indicators under the influence of modified feed. The effect of probiotics was studied after 15-day acclimatization of fish in aquariums. It has been proven that the introduction of probiotics as part of the feed substrate for 28 days has a positive effect on rapid adaptation to external stress factors, stabilization and weight gain of fish, physiological state of external mucous membranes, behavior and general condition of fish, which may indirectly indicate the role of probiotics in mobilization of body systems in the event of a change in living conditions. An increase in the number of leukocytes, an increase in the level of phagocytic activity of neutrophils, in particular the phagocytic index, was noted, which con-firms the direct effect of probiotics in increasing the body's reactivity.

Современные подходы включают «пассивный таргетинг», который опирается на естественные различия между здоровыми и больными тканями, и «активный таргетинг» с использованием лигандов, которые могут распознавать мишени, экспрессируемые преимущественно в пораженных областях. Клиническое применение этих подходов сталкивается с множеством проблем – в первую очередь, с иммуногенными и токсическими эффектами этих ксенобиотиков, что вынуждает обратить пристальное внимание на эндогенные биологические системы-носители. Данный обзор посвящен стратегиям получения и применения экзосом, как эндогенным компонентам, которые могли бы обеспечивать потенциально наиболее безопасную и эффективную целенаправленную доставку. Экзосомы представляют собой естественные наноразмерные везикулы, включающие природные липидные бислои, содержащие разнообразные адгезивные белки. Current approaches include passive targeting, which relies on naturally occurring differences between healthy and diseased tissues, and active targeting, which utilizes various ligands that can recognize targets expressed preferentially at the diseased site. Clinical translation of these mechanisms faces many challenges including the immunogenic and toxic effects of these non-natural systems. Thus, use of endogenous targeting systems is increasingly gaining momentum. This review is focused on strategies for abjec-tion and implementation of endogenous moieties, which could serve as safe and efficient carriers for targeted drug de-livery, which we consider the most efficient: exosomes application. Exosomes are naturally occurring nanosized vesicles comprising natural lipid bilayers with the abundance of adhesive proteins that readily interact with cellular membranes, which could provide an unprecedented opportunity to avoid entrapment in mononuclear phagocytes (as a part of the host immune system), and at the same time enhance delivery of incorporated drugs to target cells ultimately increasing therapeutic efficacy

Gestufte (Stepped-care) und berufsgruppen-vernetzende (Collaborative-care) Versorgungsstrukturen bilden den strukturellen Rahmen für eine erfolgreiche Umsetzung von LI-CBT. Während sich international großangelegte Forschungs- und Praxisprojekte diesem Thema widmen, liegt die Entwicklung in Deutschland deutlich zurück. Gerade vor dem Hintergrund wachsender Bedarfe und Inanspruchnahmen sowie der Zielsetzung von politischer Seite, neue Versorgungsformen zu entwickeln, stellte das Konzept LI-CBT daher einen vielversprechenden Ansatz dar.

Der dritten Säule der Behandlung, der konservativen Therapie, wird hingegen in Publikationen zu Unrecht deutlich weniger Beachtung geschenkt. Viele dieser Therapien entspringen der Erfahrungsmedizin und sind älter als die medikamentöse und operative Therapie. Vielfach konnte deren Wirksamkeit in Studien nachgewiesen werden. Wie bei der medikamentösen Therapie sprechen die Patienten auch auf die konservative Therapie sehr unterschiedlich an, sodass in der Praxis häufig mehrere Therapien einzeln oder in Kombination angewendet werden, um die für den Patienten wirksamen Therapien zu ermitteln. Jeder Rheumatologe sollte um die Potenz der konservativen Therapie wissen, um die Dosis der Medikamente reduzieren zu können, Operationen hinauszuschieben oder zu vermeiden und die Lebensqualität seiner Patienten zu verbessern.

Die Krankheitsmanifestationen sowie die Verläufe des SLE sind sehr heterogen, eine frühe Diagnosestellung und adäquate Therapie der meist jungen Patient*innen ist für die Langzeitprognose ausgesprochen relevant. Neben der klinischen Einschätzung können die im Jahr 2019 neu publizierten Klassifikationskriterien von EULAR/ACR (EULAR: European League Against Rheumatism; ACR: American College of Rheumatology) bei der Diagnosestellung unterstützend zur Hilfe genommen werden. Die Hauptursache für Morbidität und Mortalität der Patient*innen mit SLE ist die Entwicklung einer Lupusnephritis (LN), sodass hier eine frühe Diagnosestellung für die Prognose der Patient*innen unabdingbar ist. Im Mittelpunkt der Therapie steht das Erreichen einer vollständigen Remission bzw. zumindest das Erreichen einer niedrigen Krankheitsaktivität sowie das Vermeiden aktiver Krankheitsphasen, alles möglichst unter dem Verzicht des Einsatzes von Steroiden. Aufgrund der Heterogenität der Erkrankung sind häufig individualisierte Therapieanpassungen erforderlich. Neuere Erkenntnisse über die Pathomechanismen des SLE und der LN haben zur Entwicklung zielgerichteter Therapien („targeted therapies“) geführt, die zunehmend das Therapiespektrum für Patient*innen mit SLE erweitern.

ZusammenfassungHochwirksame krankheitsmodifizierende Therapien der multiplen Sklerose sind mit einem allgemein höheren Risiko für Infektionserkrankungen verbunden. Darüber hinaus bestehen substanzspezifische Risiken. Unter Anti-CD20-Therapien ist die Reaktivierung einer Hepatitis-B-Infektion möglich. Die Reaktivierung einer Tuberkulose ist vor der Anwendung von Teriflunomid, Cladribin und Alemtuzumab zu berücksichtigen. Zur Risikostratifizierung des Auftretens einer progressiven multifokalen Leukenzephalopathie unter Therapie mit Natalizumab ist der Anti-JCV-Antikörperindex etabliert. Eine Vorbeugung von Varizella-zoster-Virus- (VZV-)Infektionen ist für eine Therapie mit Fingolimod, Cladribin und Alemtuzumab erforderlich. Vor Beginn einer hochwirksamen krankheitsmodifizierenden Therapie sollen ausständige Impfungen verabreicht werden, dabei müssen Lebendimpfungen, wie z. B. gegen VZV, mindestens 4 Wochen vor Therapiebeginn appliziert werden. Unter Therapie können nur Totimpfstoffe verwendet werden, deren Impferfolg reduziert ist. In der aktuellen COVID-19-Pandemie gehören Patient(inn)en mit hochwirksamen krankheitsmodifizierenden Therapien zur Risikogruppe. Die aktuell (Stand Januar 2021) gegen SARS-CoV‑2 zugelassenen Impfstoffe sind unter diesen Therapien anwendbar.
