Full textHow did cognitive and behavioral therapy, coming from both South Africa and United States, have traveled to France as a therapeutic assemblage? Based on a field study, this research aims to trace the trajectory of cognitive and behavioral therapy and the way they progressively broadcast through the mental health field to be part of the contemporary mental health scene.My inquiry sheds light on the historical and social conditions in which CBT emerged as a therapeutic tool for patients, public health policy actors, historical importers, doctors and psychologists. Dealing with ethnographic observations (individual consultations and groups), interviews (with historical pioneers, both psychiatrists and psychologists from hospital and private practice, patients, public health policy and community-based organizations workers) collecting historical, scientific, institutional archives, the plural approach of the inquiry aims to gather various actors stakeholder’s perspectives in order to enlight tensions brought in heterogeneous worlds of care, where several conceptions of practice, human being’s antagonist ontologies cohabitate. Researchers, experimenters, translators, doctors, psychologists, public health policy professionals all contributed to translate and make cognitive and behavioral therapy intelligible in France. They came to participate, in some various ways, to promote them in the field of French psychotherapy.To this broadcasting collective enterprise, leaded by professionals and culture-brokers, patients soon responded, also becoming the stakeholders who contributed to mediate CBT. Crossing values such as self- reliance movements associated with new emerging patient-driven figure, CBT seems to offer a comfortable grip, easy to handle and to fit-in as a tool for everyday life. CBT stakeholders, driven by a pragmatist impetus in their collective enterprise, seem to be unified by the willing to find ways – whatever works – which can be used as resources for action, paying attention to singularity of objects in action to help patients

Full textStress is a major public health issue, responsible for the development and aggravation ofa large number of somatic (cardiovascular diseases, cancers, infectious diseases, etc.) andpsychiatric (anxiety, depression, etc.) disorders. Stress management through cognitive,behavioral and emotional therapies (CBT) is effective to reduce the negative consequencesof stress and prevent disorders in people at risk, but its access remains limited. Internetand new digital technologies, in particular self-help, e-CBT and heart rate variabilitybiofeedback (HRV biofeedback) have the potential to enrich CBT-based stressmanagement programs and make their access easier. In this context, the objective of thisthesis was to develop and evaluate innovative treatment formats using CBT and newtechnologies.Initially, the Seren@ctif program, the first French e-CBT program dedicated to stressmanagement, was evaluated in a randomized controlled trial on 120 patients respondingto the diagnosis of adjustment disorder with anxiety (ADA) according to the DSM-5criteria and consulting in the outpatient psychiatry service of the Lille University Hospital.The results showed that internet-based CBT guided by a human face-to-face contact timewith a supervised health professional (blended CBT) is as effective as the same face-tofaceCBT program for the treatment of ADA, compared to a control group of patientsbenefiting from usual care by their general practitioner.Secondly, a new HRV biofeedback, directly based on vagal activation, was designed froma new measure of parasympathetic activation developed by the team of ClinicalInvestigation Center of Innovative Technology at Lille. The different stages of elaborationof this new biofeedback are presented, followed by a proof of concept carried out onseveral patients with various anxiety and depressive disorders. This new HRV biofeedbackis a promising approach to non-invasively stimulate the vagus nerve. It could alsosustainably improve vagal activation and physiologically objectify the beneficial effect ofMindful Breathing. It could be complementary to the traditional respiratory sinusarrhythmia biofeedback and would allow to diversify the behavioral techniques associatedwith biofeedback.Researches presented in this thesis contribute to advancing research in the field ofinformation technologies applied to mental and behavioral health. They open upinnovative perspectives on a new way to deliver an internet-based CBT, on the potentialeffectiveness of a new HRV biofeedback or on the interest of a new marker of flexibilityof the autonomic nervous system in the objective evaluation of the effectiveness of thethird wave of CBT, especially Mindfulness

Full textPediatric obsessive-compulsive disorder (OCD) is a frequent but a not usually recognized trouble. The first –line treatment is cognitive-behavioral therapy (CBT). Objective : To demonstrate the feasibility of CBT protocol delivered in an outpatient community-based clinic and to evaluate clinic symptoms at 18 months follow-up. Nine participants (age 6-16 years) received 12 CBT weekly sessions. Assessment is realized at pre and post treatment, and every 6 months during 18 months, including symptoms of TOC (CY BOC), of anxiety(ECAP), of depression ((CDI), global functioning (C GAS) and improvement (CGI). Three WISC IV subtests allow attentional capacity analysis. Results: patients improvements are : 46% at post treatment, and 69% at 18 months follow up. The intra-individual variations show differents evolutionary profiles : quick response and remission, delayed improvement followed by remission or improvement followed by relapse. Only one is non responder. A psychological change in a differential and intra-individual optical, with sample rate (Bootstrap) and profiles comparaison (test de Kolmogorov-Smirnof) is observed for the pre treatment more severe patients. Conclusions : CBT protocol of OCD in children and adolescent is feasible in community-base clinic. Efficiency is showed and continues during 18 months. CBT allows global (CGI, C GAS), symptom (CY BOCS, ECAP, CDI) and cognitive (WISC IV) improvements

Full textChronic exposure to stressful situations, notably atwork, atlers the health of workers and can lead to thedevelopment of low-back pain. A large body of researchhas sought to identify modifiable psychological riskfactors to form the basis of interventions aimed atpreventing the development of prolonged incapacity.However, despite the recommendations of the HauteAutorité de Santé, there is little research intopsychotherapy combined with CBT in France.Numerous international studies on the efficiency of CBTshow modest and irregular effects due to amisunderstanding of psychological factors or imprecisetheoretical foundations.The objective of this doctoral research is two-fold :- Study 1 : This quantitative study was performed with256 patients suffering from work-related chronic lowbackpain. Our results show that psychological riskfactors, particularly cogitive factors (beliefs) aresignificant predictive factors of pain and disability. Also,our results highlighted the role of cognitive andemotional dysregulations in links between risk factors,pain and disability.- Study 2 : Through evaluations of the level of risk andvulnerability factors, this study aims to analyse theefficiency of group CBT treatment for patients sufferingfrom chronic work-related low-back pain (N = 15). Theirresults have compared to those of control group (N =16). Results indicate a significant decrease in levels ofdisability, psychological risk factors and vulnerabilityfactors (rumination, difficulties in emotional regulation,somatosensory amplification), which is specific to CBTgroup.These observed clinical benefits should be extended toa larger number of groups. It would seem to be pertinentto work on a transdiagnostic and processual approachin order to identify underlying psychological factors inthe link between pain and risk factors / emotionalproblems in order to improve treatment efficiency
