AbstractBased on evidence and theory, we state that facilitating and supporting people’s meaning-making processes are health promoting. Hence, meaning-in-life is a salutogenic concept.Authors from various disciplines such as nursing, medicine, psychology, philosophy, religion, and arts argue that the human search for meaning is a primary force in life and one of the most fundamental challenges an individual faces. Research demonstrates that meaning is of great importance for mental as well as physical well-being and crucial for health and quality of life. Studies have shown significant correlations between meaning-in-life and physical health measured by lower mortality for all causes of death; meaning is correlated with less cardiovascular disease, less hypertension, better immune function, less depression, and better coping and recovery from illness. Studies have shown that cancer patients who experience a high degree of meaning have a greater ability to tolerate bodily ailments than those who do not find meaning-in-life. Those who, despite pain and fatigue, experience meaning report better quality-of-life than those with low meaning. Hence, if the individual finds meaning despite illness, ailments, and imminent death, well-being, health, and quality-of-life will increase in the current situation. However, when affected by illness and reduced functionality, finding meaning-in-life might prove more difficult. A will to search for meaning is required, as well as health professionals who help patients and their families not only to cope with illness and suffering but also to find meaning amid these experiences. Accordingly, meaning-in-life is considered a vital salutogenic resource and concept.The psychiatrist Viktor Emil Frankl’s theory of “Will to Meaning” forms the basis for modern health science research on meaning; Frankl’s premise was that man has enough to live by, but too little to live for. According to Frankl, logotherapy ventures into the spiritual dimension of human life. The Greek word “logos” means not only meaning but also spirit. However, Frankl highlighted that in a logotherapeutic context, spirituality is not primarily about religiosity—although religiosity can be a part of it—but refers to a specific human dimension that makes us human. Frankl based his theory on three concepts: meaning, freedom to choose and suffering, stating that the latter has no point. People should not look for an inherent meaning in the negative events happening to them, or in their suffering, because the meaning is not there. The meaning is in the attitude people choose while suffering from illness, crises, etc.

Since the 1980’s, questions have been asked the world over about the efficiency and contribution of nursing in the hospital treatment of patients and nursing performance within the framework of nonhospital health care. The cause for these tendencies has many roots. For one, we can determine a push in professionalism through the increasing importance of nursing sciences on whole. The basic focus is on comparability, standardization (Johnson et al., 2005), and securing quality (ICN, 2003). Also, a significant part of nursing systems internationally are publicly financed and legally determined. This has the consequence that the political decision-makers, particularly in context with the financing and planning of nursing structures, have more of an interest in controlling the nursing systems and disposing of useable nursing data.

To help you consider this we have tried to include contributions from people who have a range of backgrounds and perspectives, who all contribute to mental health services, including a student about to qualify, a modern matron, a clinical manager, a director of nursing, a professor, and service users. To set the scene, our first personal contribution is from Julie Cresswell, a third year student nurse at the Birmingham City University and the Royal College of Nursing’s Student Nurse of the Year 2007. We asked Julie a series of questions that we hoped would help you in preparing to become a qualified nurse; these are her answers and thoughts about the future of mental health nursing and her career. In preparation for qualifying, I am now reflecting upon the experience, knowledge base, and skills that I have developed during my training, along with the transferable skills that I already had before I embarked upon this career. As mental health nursing students, we will all have followed a similar academic course equipping us with a certain level of theory and practical skills. However, what makes us individual is how we intend to use what we have learned throughout our lives and the philosophy that underpins our work. I began this course with the belief that psychiatric nursing was grounded in science. I now feel that while science can hypothesize about the causes of mental illness, understanding and accepting a client’s life experience is central to supporting them in times of mental distress. As I progress through my career, my approach may develop or change direction but for the moment, I feel that it will be vital to be able to communicate my own perspective to future employers. Developing your own approach to mental health nursing, along with an audit of what you can offer an employer in terms of practical skills, knowledge, and experience, establishes your own ‘unique selling point’ in a competitive jobs market.

COVID-19 impacted in person learning, particularly for the health sciences. Nursing students learn valuable clinical skills in simulation labs on campus. When one university campus stopped in person instruction during the 2020 spring semester, two librarians worked together to identify resources to support a nursing course that quickly switched to remote learning. These resources ranged from library licensed content to free virtual reality simulations. In order to identify materials, the librarians first defined visual literacy within nursing, as well as met with various constituents to understand curriculum goals and needs. Making connections with both the faculty and the curriculum was the impetus for examining similarities between the Association of College and Research Libraries Visual Literacy Competency Standards and the American Association of Colleges of Nursing Clinical Resources Essentials for Baccalaureate Nursing Education. Both librarians are eager to continue working on strategically and systematically incorporating visual literacy library instruction into the nursing curriculum.

The overall aim of this chapter is to introduce different competencies that are considered as pivotal for good nursing care. This is motivated as skilfulness in such competences can contribute to how professionals manage their responsibilities and take actions as professionals. Therefore, the chapter is not presented as a study. Rather, it has a theoretical approach focusing on the description of six core competencies in nursing, i.e., patient-centred care, teamwork and collaboration, evidence-based practice, quality improvement, safety, and informatics. These core competencies have been developed and described by the QSEN Institute (Quality and Safety Education for Nurses). Therefore, they are reflected on in relation to an understanding of nursing as an autonomous profession with a theoretical basis in caring science. This will not only shed light on nurses’ competencies but also contribute insights into the understanding of challenges and opportunities related to nurses’ domains of actions.

The innovative impact of advancing e-Health technologies is more frequently being discussed in nursing science. Nurses play an important role in collecting data and giving support to other users, especially in home care. Since issues of acceptance play a major role, the following article, which presents findings based on a focus group consisting of the elderly, caretaking relatives and professional nurses, is discussed from an ethical point of view in the context of international debate. It is interesting to note, that to some extent there is substantial ambivalence in the willingness to integrate such technologies into daily care. A need for technical assistance is clearly recognizable, however, limits as well. The authors’ findings indicate that a fundamental discussion on the relevance of e-Health methods in professional nursing needs to be held. It should address the ethical questions of often conflicting interests and rights (protection of identity, privacy and safety) in situations of high vulnerability.

The Skilled Nursing Facility (SNF) is largely a creation of Medicare: the institution of prospective payment for hospital care in 1983 led to early discharge of patients from the hospital. For frail, old people who were not ready to go home so quickly, the SNF evolved as an intermediate site of care. Social trends and scientific developments also affected the development of the SNF over the past fifty years. Changes in family structure as well as unprecedented geographic mobility weakened the ability of families to take care of older people after a hospital stay. The growth of mega-corporations and of mergers and acquisitions led to the development of SNF chains. In medical science, the development of hip and knee joint replacement surgery, along with the aging of the population, led to the rise in popularity of these procedures, with a concomitant need for post-surgical rehabilitation. The development of the intensive care unit and advances in anaesthesia resulted in a steady rise in the age of patients undergoing complex surgery, further stimulating the need for skilled nursing facility care.

Concern for relationships among ethics, values, policy, and science and engineering is prominent in modern society. The existence of a program called Ethics and Values Studies in an agency of the U.S. government, the National Science Foundation, provides some evidence of this (Hollander 1987a, 1987b; Hollander and Steneck 1990). The bills introduced in the U.S. Congress to support bio(medical) ethics centers through the National Institutes of Health also provide evidence (U.S. Senate 1988). New initiatives support research and related activities in areas of biomedical ethics in the National Center for Nursing Research and the Office of Human Genome Research in the National Institutes of Health. In July 1988, the Board of Radioactive Waste Management of the National Research Council devoted one day of a four-day retreat to considering the ethical and value aspects of that issue (BRWM 1988). In this chapter I shall attempt to show why such issues occupy particular attention now. My thesis is that a new acknowledgment of our collective moral responsibility is needed because of the political and social context in which science now operates. This context requires more sophisticated scientific and ethical analysis, as well as scientists, engineers, policymakers, interested scholars, and others working together to determine not just acceptable risk but also acceptable evidence. To provide perspective on these matters, we should note that interactions of science, technology, and society have raised these kinds of problems for a long time. A play by Henrik Ibsen, An Enemy of the People, written in 1882, raises all these concerns. An Enemy of the People is a story about the possibility of contamination in the water supply that feeds a town's new mineral baths. The baths attract the summer visitors that have rejuvenated the community. A Dr. Thomas Stockmann has investigated and discovered the problem; he has documented it, and he is delighted to have made the discovery. He, after all, had warned the town fathers about the problem when they designed the water supply, and they did not listen. Now he presents the truth as he sees it—and he sees it in the worst possible light—to his brother Peter, the mayor, who had organized the efforts to construct the baths.
