Abstract:

The Thiersch procedure is a simple and safe surgical management option for rectal prolapse. Herein, experiences—the perioperative factor, functional outcome, recurrence, and complications—from a single institution are reported. Medical records were reviewed to identify patients diagnosed with rectal prolapse, and only patients who underwent the Thiersch procedure. The clinical presentations, perioperative findings, complications, recurrences, and long-term functional outcomes were collated. Twenty-four patients underwent the Thiersch procedure for rectal prolapse between 1995 and 2014. The mean operative time was 55.5 ± 25.6 minutes and postoperative wound pain was minor. The major complications were urine retention (29.2%) and wound infection (16.7%). All patients experienced symptom improvement after the operation and recovered their anal tone. Although the Thiersch procedure entails certain recurrence and infection rates, it provides improved functional outcomes and low perioperative risks.

Abstract:

Introduction Rectal prolapse is a debilitating condition usually affecting elderly women. The management is generally surgical but the optimal operation remains unclear. The recurrence rate after Delorme’s procedure has been found to be similar to that for an abdominal approach. Thiersch sutures have been associated with high rates of complications and recurrence. The aim of this study was to compare the outcomes of Delorme’s procedure with those of a combined Delorme–Thiersch procedure. Methods A retrospective case note review was performed of all patients who underwent Delorme’s procedure for rectal prolapse between 2008 and 2014 in a single centre. Results Sixty-seven patients (63 women) underwent Delorme’s procedure during the study period. The majority (85%) of patients were over 75 years old. Twelve patients had a Thiersch suture placed at the time of the procedure. The median length of stay was 3 days (range: 0–19 days). Postoperative bleeding requiring either transfusion or readmission occurred in five patients (7.5%) and two patients (3.0%) were readmitted with pain. There was no difference in the rate of complications regardless of whether a Thiersch suture had been placed. Recurrence occurred in 8.3% of those who had a Thiersch suture compared with 21.8% of those who did not (p=0.26). There was no difference in the median time to recurrence between the groups. Conclusions There was no increase in complications after placement of a Thiersch suture with Delorme’s procedure. The rate of recurrence could potentially be decreased with this combined technique. Additional studies are required to investigate this further.

Abstract:

L'intérêt des Allemands pour la Grèce moderne, fondé sur le sentiment d'une affinité culturelle avec les Grecs anciens, est lié à un contexte européen plus large et en particulier aux relations franco-allemandes. L'instrumentalisation politique et identitaire de la référence grecque antique fournit les bases du philhellénisme qui culmina durant la guerre d'indépendance grecque et trouva un prolongement dans la formation de l'État grec et de son idéologie nationale. Trois cas éclairent les différentes modalités du philhellénisme franco-allemand et l'articulation entre ses manifestations esthétiques, scientifiques et politiques. Karl Benedikt Hase, helléniste et médiateur culturel franco-allemand incontournable, associa l'aide quotidienne aux Grecs au philhellénisme culturel manifesté dans son enseignement à l'École des Langues Orientales et dans sa participation à des entreprises scientifiques collectives visant à faire progresser les études helléniques et l'éducation du peuple grec. Les recueils de chants populaires néogrecs, dont celui de Claude Fauriel joua un rôle fondateur, contribuèrent à la formation d'un discours sur la régénération de la Grèce et à la genèse des études néohelléniques et folkloriques. Friedrich Thiersch servit la cause des Grecs par des articles publiés en collaboration avec J. -G. Eynard et J. F. Cotta, par la rédaction d'ouvrages traduits ou rédigés directement en français et par son action pédagogique d'inspiration néohumaniste. Par leurs discours et leurs actions philhellènes, les hommes de lettres reportaient sur la Grèce des idéaux concernant leur propre pays et préservaient l'espoir d'un renouveau de la civilisation européenne
German interest for modern Greece, grounded in a sense of cultural affinity with ancient Greeks, is linked to a wider European context and in particular to Franco-German relations. The instrumentalisation of the reference to ancient Greece for political and identitary purposes lies at the root of the philhellenic movement which culminated during the Greek War of Independence and further developed during the formation of the Greek state and of its national ideology. Three cases account for the different modalities of Franco-German philhellenism and for the articulation of its aesthetic, scientific and political manifestations. Karl Benedikt Hase, a prominent Hellenist and cultural mediator between France and Germany, associated regular help to Greece to cultural philhellenism evinced in his lectures at the Ecole des Langues Orientales and in his participation in collective scientific projects meant to foster Hellenic studies and the education of the Greek people. The collections of Neo-Greek popular songs, among which that of Claude Fauriel played a founding role, contributed to the formation of a discourse on the regeneration of Greece and to the emergence of Neo-Hellenic and folkloric studies. Last but not least, Friedrich Thiersch served the Greek cause with his articles published in collaboration with J. -G. Eynard and J. F. Cotta, his books that were translated or written directly in French, and his neohumanistic pedagogical action. Through their discourses and philhellenic actions, men of letters projected on Greece ideals that touched their own countries, keeping alive the hope of a renewal of the European civilisation

Abstract:

Adolphe Thiers (1797 – 1877) acheva une carrière politique de cinquante ans par la présidence de la République. Il dirigea la France durant une courte mais dramatique époque : de février 1871 au 24 mai 1873. Thiers fut aussi un célèbre historien de son temps et il rassembla ses propres archives qui sont étudiées ici dans leur globalité. Le but de cette thèse est de comprendre son exercice du pouvoir ainsi que ses relations avec l'opinion grâce aux pétitions qu'il conserva. La première partie brosse le portrait d'un intellectuel devenu homme politique. La deuxième partie retrace les trois priorités de son action politique : la lutte contre l'insurrection parisienne, la libération du territoire, enfin la dénonciation des traités de libre-échange. La troisième partie s'intéresse à la pratique quotidienne du gouvernement : sont examinés les rapports avec les autorités locales, l'armée, le corps diplomatique. La dernière partie traite des relations entre l'opinion publique et l'action politique. Les pétitions reçues accentuèrent le caractère républicain conservateur de son action. Elles continuèrent après sa démission
When Adolphe Thiers (1797 – 1877) finished his fifty years in politics, he had been President of the Third Republic. He led France during a short but dramatic period, from February 1871 to May 24th 1873. Thiers was also a well known historian and he created his own State papers which are here wholly studied. The purpose of this thesis is first to demonstrate how he exercised power as President, then to go into his relationship with public opinion by examining the petitions he archived. The first part is a portrait of this intellectual political man of State. As chief of State, Thiers meets three main politics. This is the second part. First of all there was the fight against Paris' upheaval, then the German army withdrawal, at last he denounces free trade tariff. The third part's axis deals with all days practice : his relationship with local authorities, army and diplomatic corps. With the last part begins the study of public opinion and political action. These petitions increased his conservative republican politics. Despite his resignation, the petitions still went on

Abstract:

L'eloquence, et a fortiori l'eloquence parlementaire, n'est plus un objet d'etude litteraire. Pourtant, jusqu'au xixe siecle, l'eloquence a constitue l'essentiel de la formation intellectuelle : litterature et politique ne se pensent pas alors de maniere separee, et la rhetorique est le fondement de l'une comme de l'autre. D'ou la necessite de revenir a la rhetorique pour etudier les discours parlementaires et l'eloquence deliberative a laquelle ils se rattachent. A cet egard, la monarchie de juillet est une periode privilegiee qui permet pour la premiere fois a cette eloquence de se developper durablement dans un cadre reglementaire favorable. Le discours liberal majoritaire affronte alors les realites du pouvoir, et les orateurs doivent apprendre a utiliser la parole pour gouverner. Tres vite, guizot et thiers se revelent des maitres dans l'exercice de cette parole confrontee a la liberte d'expression. Leurs discours s'adressent a l'auditoire restreint des deputes et de leurs electeurs, et ils tentent de rallier au regime les << hommes de sens >> qu'effraient les passions revolutionnaires. Pour cela, il leur faut concilier ces valeurs a priori contradictoires que sont l'ordre et la liberte. Mais si guizot et thiers sont proches par les valeurs qu'ils defendent, c'est par des styles tres differents qu'ils contribuent l'un et l'autre a la creation d'une eloquence deliberative moderne. Tocqueville, entre a la chambre des deputes en 1839, n'a certes pas les memes facilites d'improvisation, et il se plie a grand peine aux compromissions du jeu politique. Mais sa voix, quoique isolee, n'en participe pas moins au renouveau du genre deliberatif en france. L'eloquence politique va neanmoins souffrir de la sclerose du regime qui contribue a separer les champs de la politique et de la litterature, sans que les orateurs aient pour autant demerite. Dans cette rencontre manquee, ce sont aussi bien les ecrivains romantiques qui se montrent incapables de s'adapter aux nouvelles formes de l'eloquence. Si la monarchie de juillet accuse la rupture entre l'ecrivain et l'homme politique, au moins l'eloquence parlementaire aura-t-elle fait la preuve de son efficacite politique, sans se departir de ce qu'elle a aujourd'hui definitivement perdu, le style.
