Abstract:
This issue of Critical Care and Resuscitation focuses on several key aspects of modern intensive care unit (ICU) practice in Australia and New Zealand. Organ donation is particularly prominent, with two original articles and a dedicatededitorial. 1, 2, 3 The first article provides strong evidence that, in more difficult organ donation family conversations (ie, when there is no evidence of organ donation registry presence and organ donation is not raised by the family), the likelihood of successful donation is significantly increased by such conversation being led by an organ donation specialist. As supported by the editorial, it is increasingly clear that in this field, like all other aspects of critical care practice, training and specialisation make a difference. The second article demonstrates that organ donation can be successfully achieved under time pressure, even for highly vasopressor-dependent donors, and with good long term outcome when applying an expedited organ donation process. The implications are clear.

Abstract:
What is delirium? How do we diagnose it? What is the difference between delirium and behavioural disturbance? Is delirium a useful clinical construct? Is behavioural disturbance a more useful clinical construct for intensivists? Can we do large-scale epidemiological assessment of behavioural disturbance given that it is not a binary state and that it cannot be diagnosed by numbers? These are fundamental questions in the practice of modern intensive care medicine given that such “states” appear to affect one-third or more of patients admitted to the intensive care unit (ICU). In this issue of Critical Care and Resuscitation, we present the first attempt to address this concept using the technique of natural language processing and applying it to electronic ICU notes by nurses, doctors and allied health staff. The findings may surprise you, fascinate you, and make you think about these concepts from a different perspective, as summarised in a thoughtful editorial by Professor Reade.

The Proton Pump Inhibitors versus Histamine-2 Receptor Blockers for Ulcer Prophylaxis Therapy in the Intensive Care Unit (PEPTIC) study1 has now been published.2 With 26 828 participants, the PEPTIC trial is the largest clinical trial ever conducted in intensive care medicine and provides very precise estimates of the likely range of possible treatment effects associated with proton pump inhibitors versus histamine-2 receptor blockers. In this issue of Critical Care and Resuscitation, Young and colleagues3 discuss the meaning and implications of this extraordinary trial in detail in a not-to-be-missed editorial.

In this issue of Critical Care and Resuscitation, inevitably, coronavirus disease 2019 (COVID-19) has started casting its dark shadow on the intensive care community. CCR and the editorial and publication team have reacted outstandingly by rapidly turning key articles and documents around within 48–72 hours, having them ready for electronic publication and getting them listed in PubMed. This has been made possible by the fact that CCR is now in electronic format and by the outstanding work of Liv Sullivan at the College of Intensive Care Medicine of Australia and New Zealand and by the extraordinary copy-editing effort of Laura Teruel. They make a formidable team. With electronic publishing and a growing media profile, CCR is now able to track the number of viewers and visits it receives. For our recent rapid electronic publications, we have seen thousands of page views over just a few days, more than any views that the College itself receives during a similar period, and most of them from mobile phones.

Introduction: Various techniques have been developed for the repair of femoral hernia. The technique with the Lichtenstein Plug since 1989 has allowed to obtain a lower rate of complications and recurrences. AIM: The aim of this work is to review the experience of our basic group of work in the surgical treatment of femoral hernia using this technique. Methods: A retrospective descriptive observational study was conducted in our basic work group from the surgery service of the General Teaching Hospital “Enrique Cabrera” between 2009 and 2020, to which this surgical technique was applied (Plug Lichtenstein). We study the anatomical variants of hernias as well as post-operative complications and clinical evolution. Results: The mean age of the patients was 58.7 years (19-92 years), being the female with the highest incidence 81,7%, as well as, the most frequent location the right, 64.7%. The prosthesis used in the hernioplasty was that of polypropylene. Local anesthesia was applied to 48 patients (67,6%) of them. The average surgical time was25 minutes, (15-65 minutes). Ambulation was early and the average hospital stay was lessthan 24 hours, in most patients. Only one infectionof the wound and one hernia recurrence in one patient was confirmed. Conclusion: Therefore, we believe that the Lichtenstein Plug technique should be considered among the techniques of choice in the treatment of femoral hernia

In this historical moment, when FNA seems to have lost its paramount importance, it’s necessary to publish the data of the case study carried out from 2007 to 2014 in the pathologic anatomy unit at Carlo Poma Hospital in Mantua. This work includes 5,586 FNAs performed in various parts of the body. In particular, we examined 583 breast FNAs performed by a pathologist without help from a radiologist. The data confirm that it is very important for a pathologist to be present when a sample is being taken as this results in a decrease in the number of inadequate preparations as well as in improved diagnostic quality.

In March 2017, it was revealed that current and former armed service members shared thousands of nude photos of their female counterparts over social media. Although some of these photos were taken with the women’s consent, almost none of them were distributed with the women’s consent. Victims have little legal recourse. Military law is silent on the matter of non-consensual distribution. Federal civilian law speaks only to interstate stalking, domestic violence, and harassment, while only thirty-four states have revenge porn laws that sufficiently criminalize nonconsensual distribution of illicit photographs. Further complicating matters, the perpetrator’s military status as active duty, reservist, or National Guardsman at the time of the crime determines which remaining punitive remedies are available to the victim, if any. Under the current legal framework, two recent developments in U.S. military policy risk opening female service members to additional exposure. First, in 2015, the Department of Defense opened all combat roles to women. Second, and concurrently, the Department began downscaling the military to a smaller, more flexible force increasingly supplemented by its reserve and National Guard (non-active-duty) forces. In light of these developments, the current state of the law poses an unacceptable risk that the growing number of female service members will be subjected to the dual horrors of seeing lewd photos of themselves plastered across the internet, and of seeing the perpetrator walk freely. This Note argues for expanding Article 2 subject matter jurisdiction of the Uniform Code of Military Justice to include all currently contracted members of the U.S. military—active duty, reservist, and guard—in order to bring peace of mind to those female service members willing to fight for peace abroad.
