Abstract:
Актуальність. Моделювання систем регулювання в середовищі розробки програмного забезпечення контролерів, наприклад в Step 7 TIA Portal, дозволяє провести налагодження параметрів функціональних блоків ПІД-регуляторів, перевірку грубості САР до варіацій параметрів об’єкта керування та безударність переходів між ручним та автоматичним режимом роботи. Мета. Показати приклади корекції програмного коду функціональних блоків бібліотеки “LSim”, які фірма Siemens рекомендує для моделювання об’єкта керування, що забезпечують встановлення довільних початкових умов в цифрових аналогах диференційних рівнянь динамічних ланок. Метод. Аналіз оригінального коду функціональних блоків бібліотеки “LSim” та його подальша корекція, моделювання скоригованих функціональних блоків в середовищі Step 7 TIA Portal. Результати. Наведені фрагменти кодів функціональних блоків, які дозволяють моделювати ланку запізнення, інерційну ланку першого порядку, інтегруючу ланку з довільними початковими умовами. Проведено моделювання скоригованих функціональних блоків в середовищі Step 7 TIA Portal та співставлення результатів із результатами моделювання аналогічних динамічних ланок в середовищі Simulink/MATLAB. Проведено моделювання САР із використанням зазначених блоків. Висновки. Запропоновані зміни коду функціональних блоків бібліотеки “LSim”, які реалізують ланку запізнення, інерційну ланку першого порядку, інтегруючу ланку, дозволяють врахувати довільні початкові умови в цих ланках, що забезпечує моделювання динамічно сталих режимів САР в середовищі Step 7 TIA Portal.

En este artículo se presenta el diseño de la estrategia de control para la reducción de ppm de ácido sulfhídrico, presente en el biogás crudo, extraído de unos biodigestores, mejorando así las condiciones del biogás para ser utilizado en generación eléctrica. Ante la falta de acceso a la planta, se aprovecha la disponibilidad de unos datos del funcionamiento de la torre de filtración, y utilizando redes neuronales artificiales se obtiene un modelo del proceso. Se construye con este modelo un lazo de control PID, en Simulink, para determinar los parámetros de sintonía del controlador que permitan obtener el desempeño adecuado del proceso. Para desarrollar el programa de control en un autómata programable, se utiliza la aplicación TIA Portal de Siemens, implementando la estrategia de control en un PLC serie S7-1500. Mediante intercambio de datos entre la aplicación TIA Portal y Simulink. a través de una aplicación OPC, se determina el comportamiento del proceso de acuerdo a las condiciones de diseño establecidas. Se diseña una interfaz gráfica de usuario, HMI, desde la cual el operario puede visualizar el estado del proceso o modificar algunas condiciones de su funcionamiento.

El proceso de lavado de garrafones de 20 litros basado en un sistema con diseños de control, permite mejorar el consumo del desinfectante determinando una cantidad estimada para cada garrafón en un tiempo promedio, esto se da mediante la ejecución realizada en una estructura diseñada para dos botellones, en donde descansan sobre dos soportes previamente construidos por sistema de impresión 3D que es lo ideal para este proceso ya que no existen en el mercado, la ubicación del garrafón será 90° en dirección hacia abajo sujeto por fricción que evita que salga expulsado por la fuerza del flujo, el proceso es manipulado mediante botones físicos de acción o botones digitales diseñados en una interfaz gráfica (HMI), obtener el flujo necesario para el proceso de lavado requiere de una identificación, es decir el modelo analítico, que será realizado en la etapa del bombeo del desinfectante, consiguiendo como resultado una ecuación denominada función de transferencia, esto implica una lectura de datos que obtendremos con un sensor de flujo, la misma que nos permitirá extraer una base de datos para el proceso de identificación obtenida mediante programación en el software TIA Portal, este registro de información del flujo será procesado en la plataforma de Matlab que es la que nos permitirá determinar dicha función, al obtener esta ecuación podemos diseñar el controlador en la misma planta con la cual podremos programar el controlador en TIA Portal con los valores obtenidos.

Masalah lain yang timbul dari sistem manual adalah industri yang masih menggunakan sistem manual. Maka industri sekarang dituntut untuk menghasilkan produk secara cepat agar dapat memenuhi permintaan konsumen, dengan kualitas dan proses industri sesuai standard perusahaan dan mencapai target yang lebih maksimal. Alternatif yang dapat dilakukan melihat masalah yang masih banyak di industri saat ini, yaitu dengan dibangunnya proses produksi yang menggunakan sistem otomatis. Dari tuntutan industri seperti ini maka dikembangkan suatu sistem kontrol otomasi modern menggunakan PLC. Dimana saat sekarang ini PLC sangat banyak dijadikan sebagai kontrol otomasi utama dalam sebuah industri. Selain itu juga menggandengkan PLC dengan HMI. Salah satu fungsi utama HMI untuk menghubungkan manusia dengan teknologi dan meningkatkan interaksi antara mesin dengan operator melalui tampilan layar. HMI sendiri berfungsi untuk menampilkan kesalahan mesin, proses sistem, status sistem yang sedang berjalan dan melakukan maintenance terhadap kinerja mesin. Untuk mengantisipasi semua itu, sistem mounting device ini didesain menggunakan PLC sebagai kontrol dan menggunakan HMI untuk memonitoring keadaan alat yang dikontrol semua oleh PLC SIEMENS Type S7 300 dan ditampilkan di HMI menggunakan software TIA Portal versi 13. Dengan adanya alat ini, setidaknya dapat meningkatkan penguasaan dan kemampuan menggunakan PLC sebagai sistem kontrol dan mampu menggunakan HMI sebagai monitoring alat. Juga dapat menguasai program dengan menggunakan Software TIA Portal v 13.

The commissioning of new designs, technologies, production related changes and control codes and strategies has been always a risky milestone to overcome in manufacturing. It has been carried out, in many instances, in a (quasi) poka-yoke way often leading companies to incur extra costs affecting the overall efficiency of a given project. In this regard, the emergence of novel Virtual Commissioning tools and methods have become a great asset adding to the commissioning process the values of shorter times, flexibility and reduced risks. The present paper aims at giving an introductory insight into the early steps of “Virtual Commissioning” in the software Tecnomatix Plant Simulation in conjunction with other engineering tools like the S7-PLCSIM Advanced and STEP 7 TIA Portal. It generally exploressome of the capabilities of the software, and boards the benefits of using it prior to the implementation/real commissioning. The paper further lists key steps related to the realization of this process andmakes emphasis on the interconnection and functioning of all of these tools for Virtual Commissioning purposes. At the end, there is also a simple example that implements and illustrates some of the contents and knowledge addressed.

Today, raw material mixing systems are very popular in factories and manufacturing enterprises: cement, food processing, fertilizer,. . .The system of raw material mixers, in meeting that demand, must ensure the quality of the products. The objective of this article is to program the raw material mixing station using the S7-1200 PLC in combination with the control and supervising of the mixing and filling system by Webserver. The system performs the work of dividing the rate for each batch of mixing, material supplying, material mixing and extracting finished products. In addition, the author has rebuilt the plant model with a compact scale that suitable for the teaching or professional research purpose. The process of system programming is written on the TIA Portal V13 software ofSiemens agency and is operated on the HMI control screen as well as directly monitored on the Webserver. The results show that the stable running model produces good product, the system is operated and monitored accurately via webserver application designed by the author.

Abstract:
Battery energy storage system (BESS) is used in many practical applications including uninterruptible power supplies (UPS), portable devices, electrical vehicles and renewable energy systems. To utilize BESS effectively, an efficient control operation is required. Various controllers have been introduced in the open literature. However, they are not considered the best fit due to their limitations. This includes their incapability of handling high-power rating BESS, low noise immunity, short life cycle and limited number of input and output interfaces. In this paper, the programmable logic controller (PLC) is used to control and monitor a 158.8 kWh offline BESS for a typical Malaysian household. TIA Portal V13 software by siemens is used to program the proposed PLC control. Human machine interface(HMI) system is used to monitor and simulate the control performance. The results show that the PLC approach provides an efficient and reliable control of the BESS in which a compact protection against the battery overcharging, under-discharging and overheating is achieved.

Currently, Kenya supplies its energy demand predominantly through hydroelectric power, which fluctuates due to poor and unpredictable rainfall in particular years. Geothermal energy is proposed as a clean and reliable energy source in meeting Kenya’s increasing energy demand. During geothermal drilling operations, disruptions due to tool wear and breakages increases the cost of operation significantly. Some of these causes can be mitigated by real-time monitoring of the tool head during operations. This paper presents the design and implementation of a digital twin model of a drilling tool head, represented as a section of a mechatronic assembly system. The system was modelled in Siemens NX and programmed via the TIA portal using S7 1200 PLC. The digital model was programmed to exactly match the operations of the physical system using OPC (open platform communications) standards. These operations were verified through the motion study by simultaneous running of the assembly system and digital twin model. The study results substantiate that a digital twin model of a geothermal drilling operation can closely mimic the physical operation.

Abstract:
Abstract The design of control systems needs to ensure communication between multiple PLCs or external IoT devices. Furthermore, there are several ways of communication between them. This article dealt with the PLC communication based on TCP/IP protocol, and compared several communication options between two PLCs of the S7-300 and S7-1200 series connected using Profinet with a solution built in C# connected to the PLC via an OPC server. We used MODBUS TCP, TCON, and PUT/GET program blocks for comparison. We transmitted a digital Boolean data type and an analogue value of the integer data type via Profinet to read data from the S7-300 PLC and send data to the S7-1200. We compared the programming instructions standardly used as a client/server PLC programming with a Windows Forms application, and evaluated the advantages and disadvantages of this solution even when using external IoT devices. The solution was applied and successfully tested for communication between PLC S7-1200 and Nvidia Jetson Nano. We wrote program instructions for PLC in Siemens Tia Portal V15.

La industria 4.0 abarca distintas tecnologías, por lo que, se ha desarrollado un sistema que utiliza parte de estas técnicas, con el fin de emplearlas para el progreso y desarrollo de Pymes. OBJETIVO. Simular un sistema para el control y monitoreo de procesos, que permita planificar la producción en Pymes. MÉTODO. El sistema de control y adquisición de datos se realizó con el software Ignition SCADA de Inductive Automation, la base de datos utilizada fue MYSQL, para el control del PLC se utilizó el software TIA PORTAL de Siemens y la simulación de la planta se realizó con el software Factory IO. RESULTADOS. El sistema desarrollado permite adquirir las señales del controlador lógico programable y enviarlas al servidor OPC-UA de Ignition de manera que se comunica con el SCADA, a su vez los datos del sensor de altura y los identificadores de los productos son enviados a la base de datos de MySQL. DISCUSIÓN Y CONCLUSIONES. El desarrollo del sistema con tecnologías aplicables en la industria 4.0, ha permitido tener una mejor toma de decisiones, una trazabilidad, una gestión de stock y datos de almacenamiento para el desarrollo de pequeñas y medianas empresas.

Este trabajo tiene por objeto proponer una metodología para la programación de autómatas programables con TIA Portal a partir de Redes de Petri. Para esto se selecciona una aplicación industrial donde se requiere hacer una mezcla de pinturas, se presenta entonces un proceso simulado compuesto por tres tanques con sus correspondientes válvulas que se activarán por un tiempo determinado de acuerdo con el color requerido en la preparación, tres pulsadores seleccionarán el color de la pintura que se quiere producir. Inicialmente se plantea la red de Petri y luego por medio de matrices se despliega la ecuación de estados para los disparos de todas las transiciones hasta regresar al estado inicial. La metodología establece que luego se deben asociar las salidas a los diferentes estados y las transiciones se asocian a entradas. La red de Petri es entonces traducida al lenguaje de programación teniendo cuidado de activar el estado siguiente y desactivando el estado anterior según la transición que se dé. Finalmente, las salidas son activadas de acuerdo a los estados en que fueron ubicadas. Al realizar la simulación en el PLC, se pudo comprobar el comportamiento adecuado de la secuencia de acuerdo con lo que se tenía previsto, demostrándose así la efectividad de la metodología Presentada.

Abstract:
The paper concerns an alternative ways of object control using S7 1200 controller. Author’s solution is based on an external Matlab program with Simulink extension. Standard ways of controlling an object with the use of a controller to control and analyze the operation of this object use either the controller itself or a graphical representation of the system using an HMI operator panel. An alternative way of controlling a three-phase electrical motor using an external software Matlab with Simulink extension was invented and created in the university laboratory. Algorithm of controlling the object has been created in Matlab environment, through which it is possible to control the object and to read parameters of voltage, output current or even torque. The program is fully compatible with TIA Portal software, which uses TSEND_C and TRCV_C function blocks to exchange data with a PC running Matlab. The entire control process is based on the TCP protocol, while the object is connected through the inverter to the controller using the Modbus RTU protocol. A comparison of the presented solution with currently used solutions has been made. At the end of the article, all advantages and disadvantages of the discussed solution are presented, as well as personal comments on the solution.

Abstract Digital twins are rapidly establishing themselves as a necessary step in digital change also in the design of automated and robotic manufacturing systems. The digital twin serves as an interface between physical industrial products and digital worlds. This allows you to map the continuous availability of data throughout the lifecycle from product planning and development, production and commissioning to use and recycling. An important step towards this goal is to create digital twins of the components in order to create a database usable for the design of new production systems in a virtual environment. One of the possibilities of using the digital twins is testing and virtual commissioning of production systems. Instead of the classic CAD model, the digital twin replaces a detailed digital image of individual components, including their full functionality. The Digital Twin comprises the CAD model, kinematic behavior model a most important is the logical behavior model. Our goal is to create a digital twin component of the production system, integrate a CAD model, create a kinematic model and a logical behavior model. This digital twin will have signals assigned to it for logical behavior. The result is the verification and testing of the information exchange between the virtual control system and the virtual model, using the Software-in-the-Loop method and the Tecnomatix Process Simulate software environment on the digital twin side and the Siemens TIA Portal with PLC SIM Advanced on the controller side. The result of the knowledge is that the exchange of information is functional and usable for virtual commissioning.

El presente artículo propone el diseño e implementación de un sistema de monitoreo de actuadores eléctricos para la empresa AUMA Andina&Centroamérica con el n de supervisar todas las variables de control provenientes de una red de actuadores eléctricos diseñados con interfaces de comunicación Modbus RTU. Los enlaces de comunicación son entablados haciendo uso de buses de campo sobre los cuales se controla los procesos que intervienen en la automatización de actuadores eléctricos.El equipo principal utilizado para gestionar todos los enlaces de comunicación es el controlador SIMATIC S7-1200 de Siemens, al cual se le incluye las tarjetas de comunicación CM 1241 para interface Modbus, con lo cual se logra el intercambio de datos de forma serial a través del estándar RS-485. La programación del proceso y la con guración de los protocolos industriales se realiza a través de las herramientas y librerías del software TIA Portal V13, y como herramienta de visualización de resultados del proceso de control se utiliza la pantalla Red Lion G306A (HMI) que se programa mediante el software Crimson 3.0. Todos los equipos antes mencionados se integran en un tablero de control de baja potencia diseñado bajo las normas IEC 61439-1 e IEC 61439-2, el cual incluye todos los elementos de maniobra y protección necesarios para un proceso de control.

En este documento se muestra el diseño e implementación de una plataforma de control para proceso Batch, dicha plataforma de control permite la elaboración y aplicación de secuencias programadas (recetas), así como la modificación y operación de los diferentes lazos de control a partir de las variables de proceso. Para esto, se integraron tres plantas en cascada de análisis de pH, térmica y nivel (flujo de la marca Amatrol que se encuentran en los Laboratorios de la Universidad Distrital, Facultad Tecnológica), la sensórica de campo (RTD, pH metro, medidor de caudal, ultrasonido, presión diferencial) empleada mide de manera continua las variables de la receta a través de los PLC S7-300, y procesa la variable medida en cada uno de los lazos de control. En caso de presentarse alguna variación en el proceso dada por la materia prima, cambios ambientales, etc., el controlador PID y sus variantes regulan el sistema de tal manera que se mantengan las cantidades requeridas de la mezcla, todo por medio del software TIA Portal 12 (Step 7 y WINCC). Asimismo, se puede verificar el histórico de las alarmas y un reporte que indica las recetas generadas con el PLC Siemens (SIMATIC CPU 313C) en el tablero de control, en el PC con visualización en 42¨, y en la interfaz hombre – máquina HMI. Esta investigación y su desarrollo fueron co-financiadas con los equipos existentes en los laboratorios de la Universidad Distrital, por un valor aproximado de $170.000.000 de pesos.

The paper is to deal with the analysis of Profinet IO networks based on the design of a watertight door control system on a passenger ship. The analysis was based on modernization of existing watertight door system installed on a ship. The currency system on ship it use old PLC like S7-200, receive and send date only serial communication. In laboratory on University conditions the same network using two controls company Siemens PLC S7-1214 DC/DC/DC and simulated HMI write on WinCC was recreated and modified with Profinet IO standard. The authors propose a novel watertight door control system using two controllers and a touch panel HMI. The control algorithms have been written in LAD language with use norm SOLAS about list alarm. Program it was write in TIA Portal with use Function blocks (FB) with Block Date (DB) and Function (FC). When designing the laboratory bench, the authors based their assumptions on the actual solutions used on the Balmoral passenger ship. Tests were carried out on parameters related to dimensions, including the length of cables between communication modules of controllers, giving in results the actual lengths of cables in the current system and the one proposed in this work. Then, the possibility of data exchange between the two controllers and the visualization of the simulated KTP 1200 panel was examined. The operation of the software was verified, including its optimization for the correct operation of the entire system. The final part of the paper, after verifying the performance of the system in three steps, an analysis of the current system and the novel solution based on Profinet IO network is given.

As Digital Twins gain more traction and their adoption in industry increases, there is a need to integrate such technology with machine learning features to enhance functionality and enable decision making tasks. This has lead to the emergence of a concept known as Digital Triplet; an enhancement of Digital Twin technology through the addition of an ’intelligent activity layer’. This is a relatively new technology in Industrie 4.0 and research efforts are geared towards exploring its applicability, development and testing of means for implementation and quick adoption. This paper presents the design and implementation of a Digital Triplet for a three-floor elevator system. It demonstrates the integration of a machine learning (ML) object detection model and the system Digital Twin. This was done to introduce an additional security feature that enabled the system to make a decision, based on objects detected and take preliminary security measures. The virtual model was designed in Siemens NX and programmed via Total Integrated Automation (TIA) portal software. The corresponding physical model was fabricated and controlled using a Programmable Logic Controller (PLC) S7 1200. A control program was developed to mimic the general operations of a typical elevator system used in a commercial building setting. Communication, between the physical and virtual models, was enabled using the OPC-Unified Architecture (OPC-UA) protocol. Object recognition using “You only look once” (YOLOV3) based machine learning algorithm was incorporated. The Digital Triplet’s functionality was tested, ensuring the virtual system duplicated actual operations of the physical counterpart through the use of sensor data. Performance testing was done to determine the impact of the ML module on the real-time functionality aspect of the system. Experiment results showed the object recognition contributed an average of 1.083 s to an overall signal travel time of 1.338 s.

Соняшникова олія для України – продукт стратегічний. Вона є важливим джерелом валютних надходжень до країни та цінним харчовим продуктом для її громадян. Зростаюча конкуренція на світових ринках, зокрема на ринку соняшникової олії, вимагає впровадження технологій, що забезпечують підвищення якості та зниження собівартості готової продукції. А це вимагає підвищення вимог до точності керування процесами. У даній роботі для досягнення мети підвищення ефективності виробництва рафінованої соняшникової олії було означено задачу підвищення ефективності системи автоматичного регулювання (САР) параметрів процесу дистиляції місцели. Аналіз літературних джерел з розробки САР для процесу дистиляції місцели виявив значний резерв підвищення ефективності керування процесом. Для вирішення задачі побудови ефективної САР процес дистиляції місцели в трубчастому дистиляторі було проаналізовано як об’єкт керування (ОК), було отримано його концептуальну модель, проідентифіковано математичну модель, яку відтворено в середовищі імітаційного моделювання у вигляді імітаційної моделі. На першому етапі синтезу САР реалізовано, як двоканальну САР температури та рівню місцели. Надалі здійснювалося підвищувалася динамічна точність САР шляхом забезпечення автономності контуру регулювання рівню. Наступним етапом було досліджено вплив варіації параметрів ОК на якість керування. Запропоновано алгоритми керування на основі нечіткого та нейронного регулятора. Подальші дослідження пов’язані з розширенням моделі ОК за рахунок введення моделі концентрації гексану, що є безпосереднім показником якості олії. Запропоновано каскадний контур регулювання концентрації гексану з проміжною точкою: температурою дистиляції місцели. Подальші дослідження пов’язані з доповненням каскадної САР контуром гарантування, що забезпечує підвищення ефективності САР в умовах дії інтенсивних координатних та параметричних збурень. На всіх етапах синтезу варіанти САР було проаналізовано на відповідність гранично припустимим вимогам та на «грубість» або чутливість до варіації параметрів об’єкту керування. В роботі також опрацьовано питання програмної реалізації розроблених алгоритмів регулювання в середовищі TIA Portal та програмної взаємодії з оператором установки шляхом розробки графічного інтерфейсу взаємодії на основі SCADA. У майбутньому запропоновані рішення складуть основу модернізованої ефективної системи керування процесом дистиляції місцели, яка є інтелектуальним ядром автоматизованого робочого місця оператора.

Modern industrial technologies require raw materials of high purity from suppliers, especially due to the possible presence of inclusions of heterogeneous metals in them. The existence of metallic inclusions of heterogeneous metals in the raw materials leads to equipment failure, a decrease in the quality of output products, and, consequently, large financial losses. Since the main method of transporting raw materials in the industry is still a conveyor belt, this imposes additional conditions to control and remove metallic inclusions. For various reasons, current methods for removing metallic inclusions in the conveyor belt do not fully meet the needs of modern production. The main issue related to existing removal systems is the lack of intelligent interaction between these systems and the absence of information exchange between systems that detect and remove metallic inclusions. An alternative method for removing metallic inclusions of heterogeneous metals from loose medium has been proposed, which implies the tandem operation of the system that detects metallic inclusions and the system that removes them. The tandem operation of the two systems makes it possible to exchange information about a detected metallic inclusion and, as a result, to more flexibly use tools for the removal of metallic inclusion depending on the size and location of the metallic inclusion relative to the conveyor belt axis. At the same time, the control unit of the removal system makes it possible to control the conveyor belt itself, which allows the removal of complex metallic inclusions using the reverse of the electric drive of the belt, as well as enables a control check of the fact of removal. The developed algorithm of the removal system was implemented in the programming environment TIA-Portal. The introduction of this removal system could reduce the number of metallic inclusions in raw materials by 15‒20 %; moreover, its application is not limited to only one sector of the national economy

When upgrading the system MPS 500 and preparing this paper, we took into account both current sources – publications and papers dealing with the current state of Industry 4.0 and modular production systems as well as existing modular production systems based on Industry 4.0 platform. Uninvestigated parts of general matters defining. At this stage of the research, data acquisition from the system MPS 500 and interconnection with the cloud was not realized. The research objective. The purpose of this article is to modernize the MPS 500, which will allow focusing on Industry 4.0 research specifically for the deployment of Cyber-physical systems, Internet of Things, Big Data, Cloud Computing. The statement of basic materials. Effective research of the new technologies in the industry requires to use modern systems which meet the criteria of Industry 4.0 platform. So the original system Festo MTS 500 was upgraded by systems from Siemens. Conclusions. The main aim of this work was to modernize the transport system of the production system MPS 500. Elements of the system management were changed, and a new control program was created in the TIA Portal environment. The functionality of the MPS 500 was subsequently verified, where the full functionality of the system was confirmed. It makes the MPS 500 ready for further expansion in accordance with Industry 4.0.

Portal hypertension is the main complication of cirrhosis and represents a leading cause of death in patients with chronic liver disease. Therapeutic agents to improve portal hypertension should ameliorate the underlying mechanisms of portal hypertension: the elevated hepatic resistance and the hyperdynamic circulation. In the present issue of Clinical Science, Hsu and co-workers show the beneficial effects of GTPs (green tea polyphenols) in improving portal hypertension. Long-term administration of GTPs inhibited the development of cirrhosis and portal hypertension by decreasing both hepatic resistance and splanchnic hyperdynamic circulation. The main underlying mechanism of the benefits of GTPs appears related to the down-regulation of splanchnic angiogenesis. The present study adds further evidence supporting the potential of natural compounds for an effective nutriceutical approach to the treatment of patients with cirrhosis of the liver.

Because splenomegaly in humans is generally investigated at advanced stage, little is known about the relationship between the change in portal pressure and the pathological characteristics of splenomegaly or the pathological features of splenomegaly in the early stage. To clarify the relationship between the development of splenomegaly and portal hypertension, a morphological study on splenomegaly induced by portal hypertension was performed mainly by scanning electron microscopy (SEM) in liver cirrhotic rats.Male Wistar rats, weighing 200 to 250 g, were used in this study. Liver cirrhosis was produced by intraperitoneal injections of 4% thioacetamide (TAA) in saline according to the previous report. Rats were divided into 5 groups (n=5 for each): groups 1 to 5 corresponding to 20, 40, 60, 80, and 100 injections of TAA, respectively. As a control for each experimental group, rats were injected with saline instead of TAA. The injection needles were placed in the portal vein trunks and abdominal aorta.

Abstract Introduccion: Patients with essential thrombocythemia (ET) may have abnormalities of platelet function (PF), the most frecuently are reduced response to epinephrine and ADP-induced aggregation and spontaneous aggregation. Anagrelide (A) has a phosphodiesterase inhibitor activity and may inhibit PF. Therefore, we evaluated the follow-up of clinical manifestations (CM) and the PF in a group of 42 ET patients before treatment and on hematological remission. Materials and methods: 42 ET patients (33/9, F/M) who meet de PVSG criteria, were studied before treatment with A and on hematological remission. The CM were grouped into thrombosis (T), hemorrhages (H), microcirculation disturbances (MD) and combined (C), that included H plus MD or H plus T. Platelet function tests were perfomed with different agonists such as epinephrine, ADP, ristocetine, arachidonic acid and bovine vWF. Results: are shown in Table I. Abbreviations: T≠ (4 TIAs, 1 stroke, 1 AMI), H* (1 upper gastrointestinal bleeding, 4 minor hemorrhages) C± (1 mesenteric, portal and splenic vein thrombosis plus minor hemorrhage, 4 minor hemorrhages plus MCD), Tψ (1 TIA, 1 stroke), H§ (1 lower gastrointestinal bleeding) C† (1 AIT plus minor hemorrhage). During hematological remission spontaneous platelet aggregation disappeared in all patients except one. Abnormal epinephrine response desappeared in 16 patients, was not corrected in 10 and this abnormality appeared in 2 whose response were previously normal. Abnormal ADP response disappeared in 4 cases during treatment and this abnormality was found in 5 previously normal cases. No correlation was found between spontaneous platelet aggregation and clinical manifestations p= NS. No correlation was found between both epinephrine and ADP response and hemorrhages. No significant abnormalities were found in the response to arachidonic acid, ristocetin, and bovine vWF before or during treatment with anagrelide. Conclusions: In this group of ET patients platelet function was not affected by the phosphodiesterase inhibibitor activity of anagrelide. Similarly bleeding manifestations were not worsen during this therapy. As previously shown spontaneous platelet aggregation desappeared in all but one patient. We have not yet an explanation for the increased microvascular disturbances during treatment. Nagrinėjamos piliečių žurnalistikos skiltyse 2012–2014 m. pateiktos pu­blikacijos emigracijos tema. Tyrimo duomenys buvo renkami 2015-aisiais. Tyrime naudoti 269 tekstai emigracijos tema (178 publikacijos paskelbtos internetinio dienraščio Delfi.lt piliečių žurnalistikos skiltyje „Pilietis“, 91 – naujienų portalo Lrytas.lt skiltyje „Bendraukime“). Rezultatai parodė, kad naujienų portalų piliečių žurnalistikos skiltyse apie emigraciją dažniausia rašo patys emigrantai. Jiems tai galimybė ne tik pasidalyti savo istorija, bet ir paneigti viešojoje erdvėje įsitvirtinusią neigia­mą nuomonę apie emigraciją. Emigrantai sprendimą emigruoti ir gyvenimą išeivijoje daugiausia vertina teigiamai. Emigracijos nauda vertinama eko­nominiu aspektu. Piliečių žurnalistais dažniausiai tampa darbo emigrantai (emigravusieji dėl ekonominių sunkumų Lietuvoje; kritikuojantys Lietuvos valstybę ir kaltinantys ją privertus emigruoti). Būtent toks emigranto pa­veikslas vyrauja piliečių žurnalistikoje.

Abstract This study describes the clinicopathologic features of 5 patients who developed a fulminant Epstein-Barr virus (EBV)-positive clonal T-cell lymphoproliferative disorder (LPD) after acute EBV infection. One additional patient developed a similar disorder in the setting of long-standing chronic active EBV infection. Detailed immunophenotyping, in situ hybridization for EBV early RNA-1 (EBER1) and polymerase chain reaction (PCR) analyses for immunoglobulin (Ig) heavy chain and T-cell receptor (TCR)-gamma gene rearrangements were performed on paraffin-embedded tissue from all patients. In addition, EBV strain typing and detection of the characteristic 30-bp deletion of the latent membrane protein-1 (LMP-1) gene were performed by PCR. Controls included 8 cases of uncomplicated infectious mononucleosis (IM). Patients included 4 males and 2 females with a median age of 18 years (2-37 years). Three patients were Mexican, 2 were white, and 1 was of Asian descent. All presented with fever, hepatosplenomegaly, and pancytopenia; 5 were previously healthy, but had a clinical history of a recent viral-like upper respiratory illness (1 week to 2 months), and 1 patient had documented chronic active EBV infection for 7 years. Serologic data for EBV were incomplete but titers were either negative or only modestly elevated in 3 cases. In 1 case serology was consistent with severe chronic active EBV infection. In the remaining 2 cases serologic studies were not performed. All patients died within 7 days to 8 months of presentation with T-cell LPD. On histologic examination, the liver and spleen showed prominent sinusoidal and portal lymphoid infiltrates of CD3+, βF1+, EBER1+ T cells lacking significant cytologic atypia. Two cases were CD4+, 2 cases were CD8+, and 2 cases had admixed CD4+ and CD8+ cells without clear subset predominance. All were TIA-1+, CD56−. Only rare B cells were noted. Marked erythrophagocytosis was present. Molecular analysis revealed identical T-cell clones in 2 or more sites (liver, spleen, lymph node) in 5 cases. Abstract:
This study describes the clinicopathologic features of 5 patients who developed a fulminant Epstein-Barr virus (EBV)-positive clonal T-cell lymphoproliferative disorder (LPD) after acute EBV infection. One additional patient developed a similar disorder in the setting of long-standing chronic active EBV infection. Detailed immunophenotyping, in situ hybridization for EBV early RNA-1 (EBER1) and polymerase chain reaction (PCR) analyses for immunoglobulin (Ig) heavy chain and T-cell receptor (TCR)-gamma gene rearrangements were performed on paraffin-embedded tissue from all patients. In addition, EBV strain typing and detection of the characteristic 30-bp deletion of the latent membrane protein-1 (LMP-1) gene were performed by PCR. Controls included 8 cases of uncomplicated infectious mononucleosis (IM). Patients included 4 males and 2 females with a median age of 18 years (2-37 years). Three patients were Mexican, 2 were white, and 1 was of Asian descent. All presented with fever, hepatosplenomegaly, and pancytopenia; 5 were previously healthy, but had a clinical history of a recent viral-like upper respiratory illness (1 week to 2 months), and 1 patient had documented chronic active EBV infection for 7 years. Serologic data for EBV were incomplete but titers were either negative or only modestly elevated in 3 cases. In 1 case serology was consistent with severe chronic active EBV infection. In the remaining 2 cases serologic studies were not performed. All patients died within 7 days to 8 months of presentation with T-cell LPD. On histologic examination, the liver and spleen showed prominent sinusoidal and portal lymphoid infiltrates of CD3+, βF1+, EBER1+ T cells lacking significant cytologic atypia. Two cases were CD4+, 2 cases were CD8+, and 2 cases had admixed CD4+ and CD8+ cells without clear subset predominance. All were TIA-1+, CD56−. Only rare B cells were noted. Marked erythrophagocytosis was present. Molecular analysis revealed identical T-cell clones in 2 or more sites (liver, spleen, lymph node) in 5 cases. Liver cirrhosis is characterized by portal hypertension. However, the alteration of portal hypertension-related derangements during cirrhosis resolution is not well known. The present study aimed to establish animal models with cirrhosis resolution and to investigate the relevant changes during this process. Male Sprague–Dawley rats were applied. In reverse thioacetamide (rTAA) model, rats were randomly allocated into four groups with control, thioacetamide (TAA) cirrhosis and rTAA groups that discontinued TAA for 4 or 8 weeks after cirrhosis induction. In reverse bile duct ligation (rBDL) model, rats received choledochoduodenal shunt surgery upon the establishment of cirrhosis and 4, 8, or 16 weeks were allowed after the surgery. At the end, portal hypertension-related parameters were evaluated. Cirrhosis resolution was observed in rTAA groups. Portal pressure (PP) decreased after cirrhosis resolution but remained higher than control group (control, TAA, rTAA4, rTAA8 (mmHg): 5.4 ± 0.3, 12.9 ± 0.3, 8.6 ± 0.4, 7.6 ± 0.6). Further survey found the increased splanchnic blood flow did not reduce during cirrhosis resolution. The extrahepatic pathological angiogenesis was not ameliorated (% of mesenteric window area: 1.2 ± 0.3, 7.3 ± 1.1, 8.3 ± 1.0, 11.3 ± 2.7). In collateral system, the shunting degree reduced while the vessels structure remained. The vascular contractility of all systems and nitric oxide (NO) production were normalized. In rBDL series, PP decreased in rBDL16 groups but the extrahepatic angiogenesis persisted. In conclusion, cirrhosis resolution attenuates but not completely normalizes portal hypertension because of persistently high splanchnic inflow and angiogenesis. In clinical setting, vascular complications such as varices could persist after cirrhosis resolution and further investigation to define the follow-up and treatment strategies is anticipated.

SummaryPortal vein thromboses are frequent in cirrhotic patients and may be favoured by hypercoagulability in the splanchnic venous system. The coagulation balance and thrombin generation (TG) were evaluated in platelet-free plasma obtained from portal and systemic blood samples in 28 cirrhotic patients while undergoing transjugular intrahepatic porto-systemic shunt. TG assay (TGA) was performed with all samples from cirrhotic patients and with plasma samples from 14 healthy controls, with varying concentrations of tissue factor and phospholipids, with or without thrombomodulin. Screening tests and specific assays were also performed and activated partial thromboplastin time was shorter in portal plasma samples with higher FVIII and lower protein C levels, well correlated with Child-Pugh scores, and higher D-dimers and F1+2 levels However, all TGA parameters were similar in portal and jugular samples, possibly due in part to similar concentrations of factor II and antithrombin in these two sites of plasma sampling. TGA showed lower thrombin peaks and endogenous thrombin potential values in cirrhotic plasma compared to those of healthy controls. Importantly, a resistance to thrombomodulin that well correlated with factor VIII and PC levels, was evidenced in all samples from patients with cirrhosis, and was more significant in those from severely affected cases. This study therefore supports the existence of a relative hypercoagulability in the portal vein of cirrhotic patients that is likely due to protein C/S deficiency and to high FVIII levels.

Common bile duct ligation (CBDL) triggers a molecular cascade resulting in the hepatopulmonary syndrome (HPS). Both increased hepatic endothelin-1 (ET-1) production and pulmonary vascular ETB receptor expression with stimulation of endothelial nitric oxide synthase and TNF-α mediated inducible nitric oxide synthase and heme oxygenase-1 expression in pulmonary intravascular macrophages occur. Whether biliary cirrhosis is unique in triggering ET-1 and TNF-α alterations and HPS is unknown. We evaluated for HPS in rat prehepatic portal hypertension [partial portal vein ligation (PVL)], biliary (CBDL) and nonbiliary [thioacetamide treatment (TAA)] cirrhosis, and assessed ET-1 infusion in normal and PVL animals. Control, PVL, CBDL, TAA-treated, and ET-1-infused PVL animals had ET-1 and TNF-α levels measured and underwent molecular and physiological evaluation for HPS. HPS developed only in biliary cirrhosis in association with increased plasma ET-1 and TNF-α levels and the development of established molecular changes in the pulmonary microvasculature. In contrast, PVL did not increase ET-1 or TNF-α levels and TAA treatment increased TNF-α levels alone, and neither resulted in the full development of molecular or physiological changes of HPS despite portal pressure increases similar to those after CBDL. Exogenous ET-1 increased TNF-α levels and triggered HPS after PVL. Combination of ET-1 and TNF-α overproduction is unique to biliary cirrhosis and associated with experimental HPS. ET-1 infusion increases TNF-α levels and triggers HPS in prehepatic portal hypertension. ET-1 and TNF-α interact to trigger pulmonary microvascular changes in experimental HPS.

A 67-year-old woman had hepatocellular carcinoma (HCC) measuring 3.7 cm at S8 of the liver with hyperammonemia due to a spontaneous giant mesocaval shunt. Admission laboratory data revealed albumin, 2.9 g/dL; total bilirubin, 1.3 mg/dL; plasma ammonia level (NH3), 152 g/dL; total bile acid (TBA) 108.5 μmoL/L; indocyanine green retention rate at 15 min (ICG15), 63%. Superior mesenteric arterial portography revealed a hepatofugal giant mesocaval shunt, and the portal vein was not visualized. Before surgery, transjugular retrograde obliteration (TJO) for the mesocaval shunt was attempted to normalize the portal blood flow. Via the right internal jugular vein, a 6 F occlusive balloon catheter was inserted superselectively into the mesocaval shunt. The mesocaval shunt was successfully embolized using absolute ethanol and a 50% glucose solution. Eleven days after TJO, NH3, TBA, and ICG15 decreased to 56, 44, and 33, respectively. Superior mesenteric arterial portography after TJO revealed a hepatopetal portal flow. Partial hepatectomy of S8 was performed 25 days after TJO. The subsequent clinical course showed no complications, and the woman was discharged on postoperative day 14. We conclude that the combined therapy of surgery and TJO is an effective means of treating HCC with hyperammonemia due to a spontaneous portosystemic shunt.

Foram usados os descritores gifted e adhd, hyperactivity e attention deficit hyperactivity disorder em inglês, e também foram adotados “altas habilidades”, “superdotação”, “talento”, “dotação” e “precocidade”, juntamente com o descritor “TDAH”, “transtorno de déficit de atenção/hiperatividade” e “hiperatividade”. Assim, foram localizadas 33 produções. A categorização e análise dos trabalhos revelou a existência de diferenças no funcionamento de estudantes com comportamento superdotado quando em associação ao TDAH, as quais afetam aspectos educacionais, socioemocionais e requerem intervenção específica. Maiores investigações acerca dessa temática são necessárias para evitar erros diagnósticos..
