Abstract:
The aim of this in vitro study was to assess the influence of microabrasive blasting on the physicochemical properties of dentine and shear bond strength (SBS) of self-adhesive resin cement (Maxcem Elite, Kerr, Orange, CA, USA) bonded to the dentine surface. Ninety cylindrical specimens with exposed dentine of human teeth were prepared and divided into three randomized, parallel sample sets A, B, and C. Groups B and C were subjected to abrasive blasting using a micro-sandblasting device (Microetcher IIa, Danville Materials, Carlsbad, CA, USA) with two gradations of Al2O3 abrasives (Group B, abrasion with a gradation of 50 μm; group C, abrasion with a gradation of 27 μm). SEM imaging, profilometry, chemical composition analysis, contact angle measurements, surface free energy, and SBS tests were performed. The resulting data were statistically analyzed using the Statistica software (ver. 13.3, Tibco Software Inc., Palo Alto, CA, USA). Microabrasive blasting caused changes in surface topography, structural features, and the connection strength between the dentin surface and self-adhesive prosthetic cement. Air microabrasion through the multifactorial positive reorganization of the treated surface of dentine is recommended as a pretreatment method in fixed prosthodontics adhesive cementation protocols.

Abstract:
Spent coffee grounds (SCGs) generated in coffee processing for beverages and other products are a very significant organic residue that needs to be properly treated. Waste valorization via oil extraction has the potential to obtain compounds that can be used for producing biodiesel or other high-value products, such as polymers. This work focuses on the ultrasound-assisted extraction of SCG oil using n-hexane as a solvent. Three key process parameters are analyzed: temperature, extraction time, and liquid/solid (L/S) rate of solvent, using a central composite rotatable design (CCRD), an analysis that, to the author’s knowledge, is not yet available in the literature. The data were analyzed using the software StatSoft STATISTICA 13.1 (TIBCO Software Inc., Palo Alto, CA, USA). Results show that all parameters have a statistical influence on the process performance (p < 0.05), being the L/S ratio the most significant, followed by extraction time and temperature. An analysis of variance (ANOVA) showed that the empirical model is a good fit to the experimental data at a 95% confidence level. For the range of conditions considered in this work, the optimal operating conditions for obtaining an oil extraction yield in the range of 12 to 13%wt are a solvent L/S ratio of around 16 mL g−1, for a temperature in the range of 50 to 60 °C, and the longest contact time, limited by the process economics and health and safety issues and also, by the n-hexane boiling temperature.

Abstract:
Introduction: The specialty training system in Poland is subject to constant changes on the formal and legal as well as qualitative and quantitative levels. These changes should be analysed on an ongoing basis to assess the directions of development of education for medical specialists. Objective of the paper: The study aimed to determine the quantitative changes taking place in the system of specialty training of physicians in general and in ten most numerous fields of medicine in the years 2011-2018. Material and methods: Data from the register of physicians undergoing specialty training conducted by the Centre of Postgraduate Medical Education from 2011 to 2018 constituted the material for analysis. The analysis of the material was of quantitative character. The data analysis was performed using the Statistica 12 Software (TIBCO Software Inc., Palo Alto, CA, the USA). Results: The analysis of data from the register of physicians undergoing specialty training indicates that the number of physicians who trained to be specialists and the percentage of women in the group increased in the years 2011-2018. In the period in question, the percentage of doctors in training within a residency, the number of specialty training providers the number of training places and the use of training places enabling them to pursue specialisation rose as well. The situation differed between 10 specialties analysed. Conclusions: The system of post-graduate education of physicians in Poland in the years 2011-2018 constantly developed and increasingly took advantage of most of its opportunities.

Wprowadzenie i cel: Rak jelita grubego (C18-C21) stanowi obecnie w Polsce drugą wśród mężczyzn i trzecią wśród kobiet przyczynę zgonów nowotworowych. Celem pracy była ocena wiedzy oraz postaw mieszkańców gminy Czaplinek w zakresie profilaktyki raka jelita grubego. Materiał i metoda: Badanie przeprowadzono w okresie od października do grudnia 2020 roku wśród 238 mieszkańców gminy Czaplinek, położonej w województwie zachodniopomorskim, w powiecie drawskim. Narzędzie badawcze stanowił autorski kwestionariusz ankiety, składający się z 22 pytań o charakterze zamkniętym. Otrzymane wyniki opracowano w arkuszu kalkulacyjnym Microsoft® Excel® 2016 MSO i poddano analizie statystycznej przy użyciu oprogramowania Statistica® (TIBCO Software Inc.). Wyniki: Przeważająca część ankietowanych (74,8%) wiedziała, że wczesne zdiagnozowanie raka jelita grubego znacząco zwiększa szanse na jego wyleczenie. Tylko połowa respondentów (50,8%) miała świadomość tego, że nowotwór ten może rozwijać się bezobjawowo. Znajomość czynników ryzyka oraz objawów raka jelita grubego w grupie badanej była niska. Ogólny poziom wiedzy respondentów na temat raka jelita grubego i jego profilaktyki był najczęściej dostateczny (62,5%). Niespełna jedna trzecia ankietowanych (31,5%) słyszała o Programie Badań Przesiewowych raka jelita grubego. Wnioski: Poziom wiedzy oraz postaw mieszkańców gminy Czaplinek w zakresie raka jelita grubego i jego profilaktyki jest zdecydowanie niezadowalający. Istnieje pilna potrzeba prowadzenia intensywnych działań edukacyjnych, skupiających się przede wszystkim na promowaniu badań przesiewowych w kierunku raka jelita grubego.

Abstract:
В работе описаны два статистических эксперимента, целью которых стало выявление корреляционной близости / удаленности 12 текстов, дошедших до нас в русских списках XI века, и сопоставление с ними произведений автора XII века Кирилла Туровского (РНБ, F.п.I. 39, XIII в.; лл. 1–48), приведены результаты сопоставительного анализа: а) различных способов извлечения лингвистических единиц из текстов и б) выборок разного объема, а также лингвистической интерпретации основных закономерностей группировки рукописей. Степень лингвостатистической тесноты рукописей вычисляется в два этапа: на первом сопоставляются перечни наиболее частотных слов каждой пары текстов (вычисляется коэффициент ранговой корреляции Спирмена), на втором тексты группируются на основе полученных значений корреляции, которые принимаются за расстояния между рукописями (используется кластерный анализ и строится дендрограмма). Извлечение наиболее частотных слов рукописей, построение ранжированных перечней, получение сведений о количестве (а соответственно, о ранге) каждой из форм в других кодексах выполнено с помощью модуля статистики исторического корпуса «Манускрипт». Вычисление коэффициентов корреляции текстов и кластеризация текстов осуществлены с помощью программного пакета «Статистика» (TIBCO Software Inc.). Проанализированы перечни разного объема (от 50 до 300 словоформ), состоящие из единиц разной степени унификации относительно текстовых форм.Результатом первого эксперимента стало выявление трех основных устойчивых кластеров подкорпуса – группы Евангелий, группы миней и группы сборников разного содержания. Второй эксперимент дал возможность увидеть зависимость близости проповедей Кирилла Туровского разным кластерам от степени унификации форм в выборках и объема последних.Лингвистический анализ результатов позволил выявить лексико-грамматические и лексико-семантические факторы, определяющие вхождение текстов Кирилла Туровского при различных исходных условиях выборки в разные кластеры – в группу Евангельских списков (при объеме выборки 50 или 100 слов), в подгруппу сборников (при выборке в 200 слов), в подгруппу Изборника 1073 г. и Пандектов Антиоха (выборка – 300 слов).

Abstract:
Background: Fenestrations are rare, but well-known, vascular variations of the cerebral arteries. They are mostly incidental, asymptomatic angiographic findings and might precipitate vascular lesions such as AVM, aneurysmal dilatation, or even ischemic symptoms. However, association between arterial fenestration and brain aneurysms has not been clearly established. Objective: To evaluate whether incidence of arterial fenestrations are associated with brain aneurysm development and investigate the prevalence and most-common localizations of arterial fenestrations of the human brain. Design: Case–control study. Setting: All patients examined by CT angiography in University Hospital No. 4 in Lublin from 2009 to 2019. Patients: Each patient showing at least one cerebral aneurysm was included in the case group and each patient without cerebral aneurysm on CT angiography was included in the control group. Measurements: CT angiography examinations were conducted using the standard protocol used in the 1st Department of Radiology, Medical University of Lublin, Poland. The database and statistical research were conducted by use of the Statistica software (ver. 13.3, Tibco Software Inc., Palo Alto, CA, USA). Results: A total of 6545 CTA examinations were included in the study. Most of the aneurysms were located on the MCA: 629 (38.59%), ICA: 466 (28.59%) and AComA: 192 (11.78%). Cerebral arterial fenestration showed a non-statistically significant elevated risk for brain aneurysms in the entire study population (OR: 1.157; 95% CI: 0.826–1.621; p = 0.39). Among 6545 cranial CTA examinations, cerebral vessel fenestration was found in 49 of them, which constituted 0.75%. The most common vascular fenestrations were those located in the ACA (30.61%), BA (30.61%) and AComA (22.45%), while other fenestrations occurred infrequently. There were no significant differences in the age of patients in the individuals with vascular fenestration (p > 0.05). VA fenestration was slightly more common in men (16.67%) than in women (5.41%). However, these differences were not statistically significant (p = 0.216). Limitations: Our study has several limitations, including selection bias regarding examined population. Second, we assume that the total number of fenestrations detected in our study was underestimated due to the limitations of the CT method in comparison to other radiologic modalities. Conclusions: Cerebral arterial fenestrations are rare vascular malformations. The ACA is the most common localization of fenestrations, followed by BA and AComA. Fenestrations of cerebral arteries insignificantly increase the risk of cerebral aneurysm formation. Further prospective studies are necessary to make this association more precise.

Abstract:
Purpose. To determine the crucial fluorescence-angiographic features of favorable and unfavorable types of active retinopathy of prematurity and evaluate their prognostic informational value. Material and methods. Fundus fluorescence angiography (FFA) was performed by 180 premature infants (180 eyes). The children had 1st–3rd stage of active ROP. The age of gestation of the children was 24–34 weeks. A birth weight of the children was 560–2150 g. By the time of the study the age of the children was 3–14 weeks. This age corresponded to 30–38 weeks of postconceptual age (PCA). The stage and type of the pathological process was determined by using a comprehensive ophthalmological examination, which included reverse ophthalmoscopy, digital retinoscopy and morphometry. Children were admitted to FFA procedure in a stable somatic state, without pathology of renal and hepatic metabolism, after setting a test to determine the sensitivity to the drug. The evaluation of the prognostic information content of the revealed features of FFA in relation to the type of disease course was performed. Statistical processing of the research results was carried out by using the Statistica 13.3 program (Tibco Software Inc., USA). Results. During of the study, pathological fluorescence-angiographic features were identified and studied. These features were conditionally divided into 3 groups depending on the location on the fundus: near the border with the avascular retina, in the avascular retina, in the vascularized retina. It has been shown that the unfavorable course of the 1st–3rd stage of active ROP is distinguished by a much wider variety of fluorescence-angiographic phenomena and their more pronounced intensity (frequency of occurrence) in comparison with the favorable course of active ROP. On the basis of a comprehensive statistical analysis, the most informative fluorescence-angiographic features for the prognosis of the type of active ROP were determined: increased vascular tortuosity near the border with the avascular retina, zones of capillary nonperfusion in the vascularized retina, multiple foci of local neovascularization in the vascularized retina. Conclusion. Clinical use of a fluorescence angiographic study of the fundus of patients with ROP with the determination of crucial prognostic features of favorable and unfavorable types in the early stages of the disease will contribute to early treatment of this severe retinal pathology in premature infants and consequently, its favorable outcome. Abstract:
Background. Diabetes mellitus (DM) is characterized by multiple risk factors for the combined development of disorders of phosphorus-calcium metabolism, due to more frequent overweight, decreased renal filtration function and vitamin D deficiency in this category of patients. Aim. To assess the level of calcium in blood serum and its correlations with parameters of carbohydrate metabolism, body mass index (BMI) and renal function in patients with type 1 and type 2 diabetes. Materials and methods. The object of the study: adult patients with type 1 diabetes (T1DM) and type 2 diabetes (T2DM) ≥18 years old who underwent examination in Diamodule (n=294) in 2019 in the Voronezh region, who were additionally tested for serum calcium. The examination at the mobile medical center includes: assessment of anthropometric data (height, weight, BMI), biochemical data of blood lipids and creatinine levels with calculation of glomerular filtration rate (GFR-EPI), albuminuria and the albumin/creatinine in a single portion of urine, measurement of glycated hemoglobin (HbA1c), blood pressure, electrocardiogram, consultation with a cardiologist, ophthalmologist, specialist of the Diabetic Foot office, diabetologist. All biochemical parameters was carried out using commercial kits on a biochemical express analyzer Spotchem EZ ArkraySP-4430. Determination of HbA1c, albuminuria, albumin/creatinine was performed by immunochemical method on a DCA Vantage analyzer. Data analysis was carried out using the Statistica v.13.3 software package (TIBCO Software Inc., USA). The results are presented as medians and quartiles [25; 75], the level of reliability is taken as p<0.05. Results. Hypocalcemia (serum calcium level less 2.15 mmol/L) was detected in 88.3% of patients with T1DM and 73.6% of patients with T2DM. The level of serum calcium in T1DM was 1.96 [1.83; 2.07], in T2DM – 2.04 [1.97; 2.16] (p<0.001) with significant differences in GFR: in T1DM 69.6 [57.8; 82.5], with T2DM 50.5 [44.1; 59.9] (p<0.001). We observed correlation between GFR and calcium level r=-0.3 (p<0.05,). GFR<60 ml/min/1.73 m² was observed in 30.5% of T1DM patients, in 75.0% of T2DM. The HbA1c in T1DM was 8.7% [7.7; 9.8], in T2DM – 8.3% [6.8; 9.4] (p=0.01). Obesity was revealed in T1DM in 12.3%, in T2DM in 71.4%; the median BMI in T1DM was 25.5 kg/m2 [21.9; 28.4], in T2DM – 33.2 kg/m2 [29.7; 37.9]. There was correlation between BMI and calcium level r=0.26 (p<0.05). Conclusion. The results of the study revealed a high prevalence of hypocalcemia among patients with diabetes, the most pronounced in type 1 diabetes. In the presence of a correlation between calcium and GFR, there was a high incidence of hypocalcemia in patients with T1DM, even in the absence of a significant decrease in GFR, which may indicate to additional risk factors. Since the assessment of the level of calcium is not included in the list of standard clinical examination, it is recommended to include this parameter and conduct regular screening in risk groups, which include patients with diabetes. More extensive research is needed to analyze the factors.

Abstract:
Introduction The pathogenesis of classic Hodgkin's lymphoma (cHL) is largely unsettled. Previous studies have provided limited support to the possible association between tobacco smoking and outcome in cHL with inadequate evidence of a dose-outcome relationship. Smoking can result in a multitude of preventable malignancies, thus we studied the association between smoking, the initial stage of cHL and survival outcomes. Methods This single-center study included patients with a diagnosis of cHL who were first seen at The University of Texas MD Anderson Cancer Center between January 1,2016 and May 28, 2020 for either newly diagnosed cHL or relapsed/refractory (R/R) cHL. Patients' charts were reviewed to assess the history of active smoking, former smoking and the amount of smoking (measured as pack-year). Former smoking was defined as history of smoking prior to the diagnosis of cHL and no active smoking at the time of diagnosis. The primary aims were to assess overall survival (OS), progression-free survival (PFS) and frequency of advanced disease at the time of diagnosis. Descriptive statistics including mean, standard deviation, median, and range for continuous variables such as age, amount of smoking and lab measurements, and frequency counts and percentages for categorical variables such as race, gender, history of current/prior smoking and response were analyzed. Kaplan-Meier method was used for time-to-event analysis including PFS and OS. Median time to event in months with 95% confidence interval (CI) was calculated. The Log-rank test was used to evaluate the difference in time-to-event endpoints between patient groups. Statistical software SAS 9.4 (SAS, Cary, NC) and S-Plus 8.2 (TIBCO Software Inc., Palo Alto, CA) were used for statistical analyses. Results Between 2016 and 2020, 644 patients met the inclusion criteria. The median age at diagnosis was 33 years (range: 9-85 years) with 52% males. Advanced stage (stages III and IV) occurred in 45% of patients. Ninety-seven percent of patients had 0-1 Eastern Cooperative Oncology Group (ECOG) performance status. Histological subtype was predominately nodular sclerosis (83%). International Prognostic Index was =&gt;4 in 13% of patients. Most of the patients received doxorubicin, bleomycin, vinblastine and dacarbazine (ABVD) based therapy (77%), brentuximab vedotin (BV) based therapy (13%)and checkpoint inhibitors (CPIs) based therapy (6%). Patient's demographics are outlined in Table.1. The percentage of active smoking was low at 6% (median age of active smokers: 33.5). History of prior smoking occurred in 21% of patients. The mean amount of smoking was 1.87 pack-year (rang :0. 05-54 pack-year). Active smoking was associated with advanced stage (9.5% vs. 4.9%, p-value: 0.033). PFS rate at 5 years was similar in smokers and non smokers (52% vs. 54%, p-value 0.9). OS rate at 10 years was better in never smokers when compared to former smokers (95% vs. 77%, p-value: 0.017). Univariable Cox proportional hazards model for OS showed a significant association between amount of smoking and OS with hazard ratio of 1.04 (95% CI: 1.005 1.07, p-value: 0.025). Conclusions We report the largest analysis of smoking status and impact on advanced stage and cHL clinical outcomes. Smoking history is associated with inferior 10 year OS (18% lower OS in patients with history of former smoking). Active smoking was associated with advanced stage however the frequency of active smoking in our patient database was low at 6% compared to previous reports of higher incidence of up to 20%. Patients should be counseled against smoking to avoid worse outcome associated with smoking in many clinical conditions including cHL. Aim. To analyze the effect of the immunomodulator azoximer bromide on serum immunoglobulins (IgE, A, M, G) in the complex treatment for children with newly diagnosed tuberculosis. Materials and methods. The study was included 51 with newly diagnosed tuberculosis children, which were divided into 2 groups: 26 children suffer from disease, who received immunomodulator azoximer bromide (main group) in combination treatment with antimycobacterial therapy (AMBT) and 25 children suffer from disease who received AMBT only (control group). The comparison group included 30 healthy children. The children from the main group on the background of standard AMBT additionally were used the azoximer bromide (immunomodulator) to correct immunological changes: in children under 10 years of age, 6 mg twice a day, in children over 10 years – 12 mg twice a day; course of treatment – 14 days. Research indicators such as IgE, A, M, G in serum were performed on the basis of the Immunological Department in the Training Medical and Laboratory center of ZSMU by solid-phase enzyme-linked immunosorbent assay (ELISA) using a device enzyme-linked immunosorbent reader Sirio S using reagent kits LTD RPL “Granum” (Kharkiv), units of measurement: IgE – IU/ml), IgA, M, G – g/l. The study was conducted at the beginning of intensive phase (IF) AMBT and at the end of the AMBT maintenance phase (MF). The study results were processed on a personal computer using the statistical package of the licensed program Statistica, version 13 (Copyright 1984–2018 TIBCO Software Inc. All rights reserved, License No. JPZ8041382130ARCN10-J). Results. Children with newly diagnosed tuberculosis, who received immunomodulator azoximer bromide in combination therapy, after the end of the AMBT MF was determined by the normalization of serum IgA levels (1.22 (0.75; 1.74) g/l against 1.70 (1.11; 2.01) g/l, P < 0.02), which was 1.4 times lower than children who received only AMBT. The levels of IgE and M throughout the course of treatment were within the age norm with decreasing at the end of treatment, and the level of IgG throughout the course of treatment remained significantly higher than that of the healthy children group. Children with newly detected tuberculosis who received only AMBT, after the end of AMBT MF on the background of normal levels of IgE and M, the level of IgA remained significantly higher in 1.4 times, and the level of IgG tended to increase. Conclusions. Considering the obtained results, children with newly detected tuberculosis, who received of the immunomodulator azoximer bromide in complex treatment, helped to normalize of IgA level in serum, after completion of treatment. Therefore, the effect of the immunomodulator azoximer bromide in the complex therapy of children with newly detected tuberculosis on immunoglobulin indicators were significant, which would improve the effectiveness of antimycobacterial therapy in this contingent of patients in general.
