Joseph Bromer, Head of IT Solutions Delivery at LifeInsure Emerging Markets, deliberated on a challenge that he had just been given. TIBCO, a vendor supporting ObjectStar, notified LifeInsure that support for the application would cease at the end of 2017. ObjectStar was the Product Administration System (PAS) system used by LifeInsure. The ObjectStar platform was called Alpha and was used in over 200 applications. There were a number of risks, which added more pressure including the scarcity of skills and an aging workforce, but the key risk related to this was the lack of ongoing support.

Abstract:
TiCo alloys are important biomedical materials for both scientific and technological interests due to their potential applications as hard tissue replacement (HTR). The microstructural characteristics of the alloys in this system are of both scientific and technological interests. This work presents the results of microstructural characterization of as-cast TiCo alloys using a combination of various experimental methods. The results confirm that there are two variants of the Laves phase TiCo2, i.e. h-TiCo2 and c-TiCo2 exist in this system of as cast state. The eutectic reaction L(β-Ti)+ Ti2Co is determined to be occur at at 23 at.% Co, 1020 °C. Four peritectic reactions, i.e. L+ TiCo Ti2Co, L+ TiCo (c-TiCo2), L+(c-TiCo2) (h-TiCo2) and L+Co TiCo3 occur at 1058 °C, 1235 °C, 1210 °C, and 1190 °C, respectively. Eutectoid reaction (βTi) (αTi)+ Ti2Co occurs at 9 at.% Co, 685 °C.

Abstract:
Radiographic outcome assessment of ankle arthrodesis (AA) requires accurate measurement techniques. This study aimed to identify the most reliable methods for sagittal tibio-talar alignment measurements with regard to the tibio-talar offset after AA. Lateral weight-bearing radiographs of 38 fused ankles were selected for retrospective review. The sagittal tibio-talar angle (STTA), the modified tibio-talar ratio (mT-T ratio) and the sagittal tibio-talar offset (tibCOR, procLAT) were measured by three independent observers. Intra- and interobserver correlation coefficients (ICC) and mean measurement differences were calculated to assess measurement reliability and accuracy. By defining the talar longitudinal axis as a line from the inferior aspect of the posterior tubercle of the talus to the most inferior aspect of the talar neck, STTA showed excellent (ICC 0.924; CI 95% 0.862–0.959) and mTT-ratio provided high (ICC 0.836; CI 95% 0.721–0.909) interobserver reliability, respectively. For tibio-talar offset measurement the tibCOR method showed superior reliability and better interobserver agreement compared to the procLAT technique. The STTA and a modified T-T ratio are recommended for future scientific radiographic measurements in AA.

Abstract:
У грудні 2019 року з міста Ухань (провінція Хубей, Китай) надійшли перші повідомлення про COVID-19 – захворювання, спричинене новим тяжким гострим респіраторним синдромом коронавірусної етіології (SARS-CoV-2). З цього часу ця хвороба стала світовою пандемією. На сьогоднішній день загинуло понад 2,8 мільйона осіб (ВООЗ, 2021). Вірус SARS-CoV-2 став сьомим відомим варіантом коронавірусу, патогенним для людини. Метою огляду була оцінка впливу інфекції SARS-CoV-2 на поширеність ГПМК у країнах світу. Матеріал і методи. Ретроспективне дослідження проведене за матеріалами міжнародних баз даних (CDC, ВООЗ), а також реєстрів ГПМК (SITS, resQ, DAP, GSR, SSR, LASE). Проведене зіставлення інкременту захворюваності на SARS-CoV-2 та ГПМК у динаміці з моменту виявлення перших випадків COVID-19. Статистичну обробку проводили за допомогою програмного забезпечення Statistica 13.0 (TIBCO Inc., США). Результати. Частота ГПМК у світі корелює з поширеністю SARS-CoV-2. На ранніх стадіях пандемії COVID-19 відбувалося меншення захворюваності на ГПМК у розвинутих країнах. Існуючі міжнародні реєстри ГПМК відображають збереження цієї тенденції. Висновки. 1. Пандемія COVID-19 пов᾽язана із глобальним зменшенням звернення за спеціалізованою медичною допомогою при ГПМК, що впливає на загальну кількість госпіталізацій та число процедур механічної тромбектомії. 2. Незважаючи на географічні розбіжності, це зменшення обсягу спостерігалося незалежно від тягаря госпіталізації COVID-19 та обсягів госпіталізації хворих з ГПМК у допандемічний період.

Abstract:
Accumulating evidence has suggested a correlation of tumor infiltrating B cells (TiBcs) and a good prognosis of cancer diseases. In some cases, TiBcs appear to have experienced antigen stimulation since they have undergone class-switching and somatic hypermutation and formed tertiary lymphoid structures around tumors together with T cells. Assuming TiBcs include those that recognize some tumor antigens, we sought to investigate their possible usefulness for cell-mediated immunotherapies. To expand usually small number of TiBcs in vitro, we modified our B cell culture system: we transduced B cells with ERT2-Bach2 so that they grow unlimitedly provided with tamoxifen, IL-21 and our original feeder cells. Such cells differentiate into plasma cells and produce antibodies upon withdrawal of tamoxifen, and further by addition of a Bach2-inhibitor in vitro. As a preliminary experiment, thus expanded splenic B cells expressing a transgenic antigen receptor/antibody against hen egg lysozyme were intravenously injected into mice pre-implanted with B16 melanoma cells expressing membrane-bound HEL in the skin, which resulted in suppression of the growth of B16 tumors and prolonged survival of the recipient mice. To test the usefulness of TiBcs for the immunotherapy, we next used APCmin/+ mice as a model that spontaneously develop intestinal tumors. We cultured TiBcs separated from the tumors of APCmin/+ mice as above and confirmed that the antibodies they produce recognize the APCmin/+ tumor. Repeated injection of such TiBcs into adult APCmin/+ mice resulted in suppression of intestinal tumor growth and elongation of the survival of the recipient mice. Serum antibody from the TiBc-recipient mice selectively bound to an antigen expressed in the tumor of APCmin/+ mice. These data suggest a possibility of the novel individualized cancer immunotherapy, in which TiBcs from surgically excised tumor tissues are expanded and infused into the donor patients.

Abstract:
Annotation. The aim of the study was to assess the characteristics of hemostasis and the formation of a scar on the uterus in women who underwent conservative myomectomy. Medical records of 426 women with a scar on the uterus were analyzed. The results of pathomorphological examination of biopsy materials obtained in the area of the “old” scar during myomectomy with recurrence of fibroids with a gestational age of 33 to 40 weeks. The micromorphological structure of postoperative scars and pericycatric region of the myometrium was determined and immunohistochemical examination was performed on serial paraffin sections of biopsy material using primary and secondary monoclonal antibodies of Dako kits (USA). To quantify the immunohistochemical results in the sections, the number of cells in which the immunoperoxidase label (positive staining) was detected in 10 fields of view randomly selected (>200 cells) was counted. FW expression was determined using a semi-quantitative method with H-score calculation. Statistical processing of the obtained results was performed by non-parametric methods using the software TIBCO Statistica 13.5 (USA). It was shown that 40.1% of uterine scars are due to conservative myomectomy. In the postoperative period, patients who underwent conservative myomectomy have a tendency to hypercoagulation, while the frequency of thromboembolic complications after conservative myomectomy does not exceed 3%. High FW expression indicates the presence of endothelial dysfunction and should be taken into account when assessing the risk of thromboembolic complications in patients after conservative myomectomy.

Abstract:
Introduction/Objective. Continuing Medical Education (CME) is a crucial element to keep the level of professionalism in the three key fields of medical education: pre-clinical, clinical, and public health. The profile of CME in Serbia has been analyzed for the 2011?2017 period. Methods. Between 2011 and 2017, 11,557 courses of CME have been submitted for accreditation, described by 26 variables. Due to the predominance of nominal data, we employed a principal component analysis (PCA) using the nonlinear iterative partial least squares algorithm (PCA/PLS) to arrange the 16 variables with complete information in such a way that most influential factors could be displayed and ranked. The analysis was done with TIBCO Statistical Software. Results. The Faculty of Medicine of Belgrade takes the top position among the medical faculties in Serbia with 569 courses or 47.9% (n = 1187; 2011?2017), whereas non-educational institutions with 86.2% of all courses (n = 11,514) are the most dominant providers. Clinical topics dominate the thematic spectrum with 59.7%. Between 2012 and 2017, the total number of courses offered diminished by 16.9%. A PCA of 16 potential determinants of CME reveals that the most relevant ones are duration, credit points, price, and number of lecturers. Conclusion. For the last decade, a standstill or even a regression in the development can be observed. Especially the faculties of medicine in Serbia, as well as other major providers, should reconsider the entire structure of their administrative organization and initiate innovative development.

Abstract:
Introduction: The specialty training system in Poland is subject to constant changes on the formal and legal as well as qualitative and quantitative levels. These changes should be analysed on an ongoing basis to assess the directions of development of education for medical specialists. Objective of the paper: The study aimed to determine the quantitative changes taking place in the system of specialty training of physicians in general and in ten most numerous fields of medicine in the years 2011-2018. Material and methods: Data from the register of physicians undergoing specialty training conducted by the Centre of Postgraduate Medical Education from 2011 to 2018 constituted the material for analysis. The analysis of the material was of quantitative character. The data analysis was performed using the Statistica 12 Software (TIBCO Software Inc., Palo Alto, CA, the USA). Results: The analysis of data from the register of physicians undergoing specialty training indicates that the number of physicians who trained to be specialists and the percentage of women in the group increased in the years 2011-2018. In the period in question, the percentage of doctors in training within a residency, the number of specialty training providers the number of training places and the use of training places enabling them to pursue specialisation rose as well. The situation differed between 10 specialties analysed. Conclusions: The system of post-graduate education of physicians in Poland in the years 2011-2018 constantly developed and increasingly took advantage of most of its opportunities.

В работе описаны два статистических эксперимента, целью которых стало выявление корреляционной близости / удаленности 12 текстов, дошедших до нас в русских списках XI века, и сопоставление с ними произведений автора XII века Кирилла Туровского (РНБ, F.п.I. 39, XIII в.; лл. 1–48), приведены результаты сопоставительного анализа: а) различных способов извлечения лингвистических единиц из текстов и б) выборок разного объема, а также лингвистической интерпретации основных закономерностей группировки рукописей. Степень лингвостатистической тесноты рукописей вычисляется в два этапа: на первом сопоставляются перечни наиболее частотных слов каждой пары текстов (вычисляется коэффициент ранговой корреляции Спирмена), на втором тексты группируются на основе полученных значений корреляции, которые принимаются за расстояния между рукописями (используется кластерный анализ и строится дендрограмма). Извлечение наиболее частотных слов рукописей, построение ранжированных перечней, получение сведений о количестве (а соответственно, о ранге) каждой из форм в других кодексах выполнено с помощью модуля статистики исторического корпуса «Манускрипт». Вычисление коэффициентов корреляции текстов и кластеризация текстов осуществлены с помощью программного пакета «Статистика» (TIBCO Software Inc.). Проанализированы перечни разного объема (от 50 до 300 словоформ), состоящие из единиц разной степени унификации относительно текстовых форм.Результатом первого эксперимента стало выявление трех основных устойчивых кластеров подкорпуса – группы Евангелий, группы миней и группы сборников разного содержания. Второй эксперимент дал возможность увидеть зависимость близости проповедей Кирилла Туровского разным кластерам от степени унификации форм в выборках и объема последних.Лингвистический анализ результатов позволил выявить лексико-грамматические и лексико-семантические факторы, определяющие вхождение текстов Кирилла Туровского при различных исходных условиях выборки в разные кластеры – в группу Евангельских списков (при объеме выборки 50 или 100 слов), в подгруппу сборников (при выборке в 200 слов), в подгруппу Изборника 1073 г. и Пандектов Антиоха (выборка – 300 слов).

Abstract:
Purpose The purpose of this paper is to introduce coherent Industry 4.0 definition via a rigorous analysis framework, and provide a holistic view of technological, organizational and other key aspects (variables) of Industry 4.0 along with the identification of interdependencies that co-occur between them. Design/methodology/approach The study conducts a systematic literature review using Preferred Reporting Items for Systematic Review and Meta-Analysis methodology, and includes 675 papers analyzed both quantitatively and qualitatively. The former utilizes TIBCO Statistica. Furthermore, to define Industry 4.0, the authors reviewed 52 publications. Findings Industry 4.0 is a multidimensional system of value creation that includes 42 groups of terms in management, organizational and business-related variables, 30 technological and manufacturing-related variables – classified into seven categories – and several interdependencies that co-occur between them. Practical implications The analyses’ outcomes are of high importance both for academia and industry practitioners, as the findings elucidate the meaning of Industry 4.0 and may be used as the basis of future research in management, production management, industrial organizations and other Industry 4.0-related disciplines. Regarding industrial companies, the publication serves as a compendium, and should support industrial businesses in the transition from traditional manufacturing into the Industry 4.0 era. Originality/value This work’s novelty and value is threefold: first, the paper introduces an Industry 4.0 definition framework based on the most popular publications in the field. Second, the paper identifies and presents Industry 4.0’s common technologies and organizational variables via a systematic and current literature review. Finally, the paper extends the ongoing discourse on Industry 4.0. For the first time in this discipline, interdependences between identified Industry 4.0 variables are presented and discussed.

Introduction: to investigate the possibility of using clindamycin and butaconazole in local therapy of moderate and severe dysbiosis in patients during the menopausal transition (early and late).Materials and methods: 107 women were examined. They were divided into two groups depending on their complains: the patients of group 1 (49.5%) had an early transition to menopause (44.7 ± 2.3 years); the patients of group 2 (50.5%) corresponded to a late transition to menopause (49.1 ± 1.5 years). The complains were evaluated. The levels of hormones (FSH, AMH, Inhibin B, estradiol) were determined. A study of the vaginal biotope was conducted by PCR ‘Femoflor 8. Statistical studies were conducted in the environment of STATISTICA 10 package (Tibco, USA). The difference in average values was considered statistically significant for p<0.05. Results: vegetative-vascular disorders were typically for patient in group 2 and detected in 74.1%. Mucosal dryness was 5.2 times more common in patients 2 group, as was dyspareunia (5.5 times more often), dysuria (14 times more often), itching in the vagina (3.5 times more often), discomfort (3.4 times often). All patient in group 2, and 51% of patient in group 1 had moderate and severe dysbiosis on the background of hypoestrogenia.Conclusion: The period of menopausal transition is characterized as a abnormalities of the vaginal biotope due to the activation of aerobes, anaerobes, and fungi of the genus Candida, and estrogen deficiency, that is more pronounced in patients with a late transition to menopause. Combined approach to the therapy these disorders allows to level out the clinical manifestation due to the consistent use of Clindacin B prolong and Ovipol Clio, but also improve the quality of life by individually selecting the duration estradiol therapy.

Abstract:
Abstract BACKGROUND Glioblastoma multiforme is the most common and aggressive brain tumor in adults, with an average overall survival of 14 months. Current standard of care consists of tumor resection followed by radiotherapy with concomitant temozolomide and adjuvant temozolomide. However, glioblastoma recurs in all patients. The causes reside in the enhanced invasiveness and resistance to treatment, giving a clear indication that recurrent and resistant glioblastoma biology must be understood better in order to achieve future treatment strategies to benefit the patients. The complex nature of recurrent glioblastoma makes its understanding still a challenging achievement in the field. Nowadays multi-omics approaching is developing further and further and it may be used to unravel, by combining different layers of biological information, a comprehensive view of the changes occurring during the treatment. MATERIAL AND METHODS A discovery set of 13 primary patient-derived glioblastoma stem-like cultures were analysed, comprising selected resistant, induced resistant and with pre-existing resistance conditions. A characterization of transcriptome, proteome and phosphoproteome was performed using RNAseq and liquid chromatography mass spectrometry. Additional 10 paired primary and recurrent tumor tissues were utilized as a validation set. The data obtained was visualised, explored and integrated through TIBCO Spotfire, Ingenuity Pathway Analysis, STRING and COREMINE medical software. RESULTS Genetic regulatory processes such as DNA repair mechanism, mRNA splicing and chromatin assembly were shown to be common over-represented trends in resistant and recurrent glioblastomas as a result of increased genomic instability and stress deriving from acute an repeated temozolomide exposure. Due to the immense heterogeneity of glioblastomas, other proteins and genes here identified as differentially expressed need a further investigation as they also may play an important role in relevant biological processes in a patient-specific way. CONCLUSION This study provides further understanding of glioblastoma biology revealing an association with processes of recurrence and temozolomide resistance, moreover offering potential therapeutic targets for better treatment options for glioblastoma patients.

High levels of infiltrating cytotoxic (CD8+) T lymphocytes (CTLs) and low levels of CD68+ macrophages (MΦ) may correlate with prolonged survival but quantification methods are not standardized for clinical practice. HLA-DR is a marker of MΦ activation not expressed by suppressor myeloid cells. A novel pathology technique using mIHC allows for quantitative and spatial analysis of immune cell subsets. Methods: In a pilot set of stage II/III primary melanomas from Columbia University Medical Center (n = 94), clinical follow up is available for 51 cases. 32 had no evidence of recurrence at last follow up (minimum 2 years) while 19 died of melanoma. 5µm slides were stained using Opal multiplexed IHC (mIHC) for DAPI, CD3, CD8, CD68, SOX10, HLA-DR and Ki67. Tumor areas were pre-selected by a dermatopathologist, visualized using Mantra (Perkin Elmer) and analyzed using inForm (Perkin Elmer) and Spotfire (TIBCO). Results: In all patients (n = 94), CTLs are farther from tumor (SOX10+) cells when they are proliferating (Ki67+) (p < 0.0001***), while they are closer to MΦ when they are activated (HLA-DR+) (p = 0.0002***). Next, we evaluated impact on prognosis using disease specific survival (DSS) as an outcome based on median value (n = 51). In this exploratory study no correction for multiple comparisons was made. We find that CTL density correlates with prolonged DSS in tumor (p = 0.0185*) but not in stroma (p = 0.1630 ns). Ratio of density of CD8/CD68+HLA-DR- correlates with DSS in both tumor (p = 0.027*) and stroma (p = 0.017*). Finally, distance from CTLs to HLA-DR- MΦ was significantly greater in non-recurrent melanomas as compared to recurrent ones (p = 0.0167*). Conclusions: HLA-DR expression on MΦ and Ki67 expression on tumor cells correlate with position of CTLs in TME in primary melanoma. CTL density is a favorable prognostic marker while HLA-DR non-expressing MΦ may favor tumor progression. Quantitative mIHC allows for accurate spatial analysis of immune subsets within the TME and the development of novel, more accurate and potentially clinically relevant biomarkers.

The clinical significance of stem-like CTCs in OC remains to be studied. We aimed to assess the quantities of the stem, epithelial, mesenchymal CTCs and their relationships with the clinical parameters in the OC. Methods: Peripheral blood samples (7.5 ml) were obtained from patients with primary epithelial OC before treatment. CTCs were isolated by flow cytometry (Cytoflex S (Beckman Coulter, USA)) using antibodies to CD45 (BioLegend, USA); CD44 (BioLegend, USA), CD133 (Miltenyi biotec, Germany), ALDH (Stemcell, Canada) to detect the stem markers; EpCAM (BioLegend, USA), cytokeratins 8, 18 (Abcam plc., UK), vimentin (BioLegend, USA) for epithelial and mesenchymal markers. Blood samples from patients with benign ovarian tumors served as a control. Informed voluntary consent was obtained from all the women. Statistical processing included Mann-Whitney U-test, linear regression, Cox proportional hazards model for progression-free survival (PFS) (Statistica 13.0 (TIBCO, USA)). Results: The study included 30 patients, median age 64 (34-76) years. 15 patients had a FIGO stage IV, 12 - stage III, 1 – stage II and 1 – stage I. The content of CTCs populations is presented in the table. The CTCs counts did not differ depending on age, platelet count, and stage 3 or 4. The amount of CD45-CK+Vim- was higher in the presence of ascites (p = 0.035). We found a regression relationship between the serum CA-125 and the number of CD45-CD44+CD133+ (R2= 0.220, p = 0.016); the leukocyte count in blood and CD45-CD44+ALDHhigh (R2= 0.234, p = 0.017); the number of CD45-Vim+ and CD45-CD44+CD133+ALDH+ (R2= 0.305, p = 0.014); CD45-CK-Vim+ and CD45-EpCAM+CK+ (R2= 0.717, p < 0.001). The Cox regression model for PFS included the number of CD45-CD44+CD133+ALDH+ (HR 1.51 95% CI 1.01-2.24 p = 0.043) and the cytoreductive surgery performance (HR 0.09 95% CI 0.01-0.89 p = 0.039) during the first line of treatment. Conclusions: Various populations of circulating tumor cells coexist in ovarian cancer patients. Category: Ankle Introduction/Purpose: Tibiotalar arthrodesis is generally a successful treatment option for patients with end stage ankle arthritis. However, there is a 9% risk of nonunion in patients undergoing primary tibiotalar arthrodesis. To date, it is unclear whether concurrent distal tibio-fibular joint arthrodesis affects this nonunion rate as there have been no studies directly comparing patients with and without arthrodesis of the distal tibio-fibular joint. The purpose of this clinical study is to compare the rate of nonunion in patients with a distal tibio-fibular fusion to those without a distal tibio-fibular fusion in the setting of a primary, open ankle arthrodesis. The hypothesis of this study was that the addition of a distal tibio-fibular fusion would decrease the nonunion rate in patients undergoing open ankle arthrodesis. Methods: This is a retrospective review of 521 consecutive patients from October 2002 to April 2016. 366 ankles from 354 unique patients met inclusion criteria. All patients underwent primary, open tibiotalar arthrodesis. 250 patients underwent open tibiotalar arthrodesis with a distal tibio-fibular fusion and 116 patients underwent open tibiotalar arthrodesis without a distal tibio-fibular fusion. Age, gender, body mass index, smoking, and preoperative radiographic deformity were controlled. The primary outcome measure was nonunion rate of tibiotalar arthrodesis. Secondary outcome measures were time to union, rate of wound complications, and rate of development of post-operative deep vein thrombosis (DVT)/Pulmonary embolism (PE). Results: Average age of the patients was 56.2 +/- 14.2 years. Mean follow-up time was 33.8 months. Unions were assessed on routine post-operative radiographs and by clinical examination. If there was a concern for nonunion, computerized tomography scan was utilized for further assessment. Nonunion rate of patients who had the distal tibio-fibular joint included was 19/250 (8%) and nonunion rate of those who did not have the distal tibio-fibular joint fused was 14/116 (12%) (p=0.16). There was no significant difference between those who had the distal tibio-fibular joint included versus who did not in wound complication rate (27% vs 31%, p=0.40), time to union (4.9 weeks versus 5 weeks, p =0.54), and DVT/PE rate (5% vs 3%, p=0.41), respectively [Table 1]. There were no major complications. Conclusion: To our knowledge, this is the first study directly comparing nonunion rates and complication rates in patients who underwent primary, open ankle arthrodesis with and without distal tibio-fibular joint arthrodesis. In this study, inclusion of the distal tibio-fibular joint in tibiotalar arthrodesis does not affect nonunion rate in patients undergoing primary, open ankle arthrodesis. Additionally, inclusion of the distal tibio-fibular joint does not affect rate of wound complication, time to union, and DVT/PE rate.

Abstract Background: We previously reported the development of a fully automated assay for total iron-binding capacity (TIBC) in serum, using a multipurpose automated analyzer. However, this method requires four different reagents and is thus useful only with a limited number of available analyzers. We simplified our original assay and compared the analytical performance of the modified method with that of a commercial, fully automated TIBC assay (Dimension® TIBC assay). Methods: We simplified our original method to require only three reagents. Calibration was also altered and was performed with human transferrin standard solutions. An advantage of this method is that it does not require separation of excess unbound iron after the first step of transferrin saturation. Unbound iron is eliminated by formation of a complex with the chromogenic reagent ferrozine in the second step. Iron dissociated from transferrin by acidic pH reacts with ferrozine to form a colored complex in the final step, and the increase in absorbance at 570/660 nm is directly proportional to the TIBC measured. TIBC values were determined for 49 healthy individuals and 148 patients with this modified TIBC assay and with a commercial, fully automated TIBC method (Dimension clinical chemistry system), and calculation of TIBC based on the sum of the serum iron and unsaturated iron-binding capacity was performed for 97 patients. Results: The within-run CVs for the modified TIBC assay and the Dimension TIBC assay were &lt;4.8% and &lt;2.4%, and the between-run CVs were 1.2% and 1.7%, respectively. The dilution curves were linear for TIBC values up to at least 180 μmol/L with both methods. TIBC values obtained by our method were linearly correlated with serum transferrin concentrations (r = 0.984; Sy|x = 3.18 μmol/L; P &lt;0.001). The correlation between the values obtained with the present method (y) and those obtained with the Dimension TIBC method (x) was y = 1.04x + 1.19 μmol/L (r = 0.985; Sy|x = 2.47 μmol/L), and with the calculation method (x) was y = 1.18x + 2.62 μmol/L (r = 0.976; Sy|x = 3.27 μmol/L). Conclusions: Our modified, fully automated TIBC assay performed similarly to the Dimension TIBC assay and is adaptable for use with many multipurpose automated analyzers.

OBJECTIVE: To evaluate recent data that suggest that total iron binding capacity (TIBC) is not a reliable laboratory parameter in assessing acute iron overdose. DATA SOURCES: A MEDLINE search of the literature with a fan search of relevant articles. STUDY SELECTION AND DATA EXTRACTION: Laboratory, human, and animal studies on the measurement and relevance of TIBC in acute iron poisoning were reviewed. These data were analyzed in light of the current guidelines for use of TIBC in assessing iron poisoning. DATA SYNTHESIS: There are significant data to suggest that the sensitivity of TIBC in acute iron poisoning is unacceptable. This lack of sensitivity is also reflected in the poor positive predictive value of TIBC. The specificity and negative predictive value are higher but not without error. One survey of 500 laboratories reported a coefficient of variation for TIBC of 16 percent (95 percent confidence interval), suggesting unacceptable accuracy. CONCLUSIONS: These data suggest that TIBC may not be a reliable laboratory parameter in assessing acute iron poisoning. Studies are needed to develop a reliable analytical procedure for TIBC and to correlate TIBC with clinical outcomes in acute iron poisonings.

Total iron-binding capacity (TIBC) values are determined on Olympus AU2700 automated chemistry analyzer as the sum of serum iron and unsaturated iron-binding capacity (UIBC) - (calculated TIBC, TIBCcal). Considering that Olympus AU2700 automated analyzer was recently brought in function and TIBC values calculated from serum iron and UIBC values were significantly lower than those obtained by a direct and fully automated TIBC assay, it was necessary to determine the reference interval for TIBC, according to the recommendation that every laboratory should have its own reference ranges. The "calculation method" of TIBC determination showed satisfactory accuracy (p > 0.001) and precision, with CV values ranging from 0.91% to 1.63% within-run and from 2.30% to 2.80% day-to-day. The correlation between the TIBC values obtained with the "calculation method" using Olympus AU2700 analyzer (y) and those obtained with the direct method (x) was: y = 0.919x + 2.319 mmol/L (r = 0.980; Sy,x = 1.814; p < 0.001; N = 85). The reference interval for TIBC was determined using sera collected from 125 healthy individuals of both sexes, 15 to 80 years old. Since the values did not depend on sex, the reference interval calculated for the whole studied population ranged between 42.0 mmol/L and 64.3 mmol/L.

Recent survey work for planthoppers at La Selva Biological Station in Costa Rica found two new species allied with Cenchrea Westwood. The cytochrome c oxidase subunit I (COI) and 18S were sequenced for the new taxa and used these data to assess the genus-level standing of the new taxa. The new taxa do not cluster with Cenchrea dorsalis Westwood, the type species of Cenchrea. A new genus Tico gen. n. described for the reception of new species described as Tico emmettcarri sp. n. (the type species) and Tico pseudosororius sp. n. Cenchrea sororia Fennah is moved to Tico gen. n., to form the new combination Tico sororius (Fennah). Tico gen. n. is compared with allied genera, and review genus-level diagnostic features and the species composition of Cenchrea, which appears to be compositionally heterogenous, but additional data is needed to evaluate genus-level placement of most species.

Pemerintah mulai ikut berpartisipasi mulai tahun 2012. Faktor pendukung yang menyebabkan pelaksanaan Tradisi Adat Pattaungeng masih tetap dilaksanakan oleh masyarakat di daerah tinco yaitu adanya niat dari dalam diri untuk membudayakan dan melestarikan tradisi adat pattaungeng, adanya penerus atau pergantian generasi yang akan melaksanakan sehingga budaya tersebut tetap terlaksana dan bertahan didalam masyarakat. Tradisi tersebut dilaksanakan antar bulan september-oktober.Berdasarkan hasil penelitian di atas dapat disimpulkan bahwa masyarakat di Tinco melakukan Tradisi Adat Pattaungeng dengan keyakinan bahwa tradisi tersebut di laksanakan untuk tola bala, untuk keselamatan dan kesejahteraan agar terhindar dari bencana. Background: The aim of the study is focused to measure TIBC by calculated method (TIBC using the magnesium carbonate- MgCO3 adsorption), measurement of TIBC by calculatory method from automated UIBC and automated Iron measurement. Latter these two values are to be added and would give an additive result of TIBC. The experiment would be performed by using the light MgCO3 powder and the heavy MgCO3 powder and check for any differences in their respective values. Regression analysis, Bland Altman analysis or histogram analysis of TIBC results obtained by calculated methods in patient samples to find correlation between the two methods is being performed.Methods: Samples received for analysis of tests, regardless of patient identification with sample volume serum were used for the study. After complete analysis and reporting of the sample, the leftover serum was used. Serum was kept at room temperature. Then the calculated TIBC from UIBC and the IRON were compared after doing batch calibration for UIBC and lot calibration of Iron.Results: It was found that, this study had positive bias by the usage of the heavy MgCO3 powder. Positive bias in unsaturated iron binding capacity observed by both methods is studied and further investigated using data obtained during the experiment.Conclusions: There is a higher yield of serum obtained with the heavy MgCO3 powder, TIBC calculated from iron and unsaturated iron binding capacity as compared to TIBC measured directly using the light MgCO3 powder.

Abstract We evaluated plasma iron (PI) and total iron-binding capacity (TIBC) or transferrin in normal individuals and in patients with iron imbalance. The standard colorimetric measurements of PI and TIBC and the standard isotope-dilution measurement of TIBC were compared with an immunoprecipitation method and also with immunoelectrophoresis of transferrin. PI concentrations as measured by the standard and immunoprecipitation methods agreed closely for all individuals except those with saturated transferrin, where nontransferrin iron increased the results in the standard assay. This excess iron in saturated plasma may be derived from either free iron or iron-bearing ferritin. There were also differences in TIBC between the two methods. Iron-deficient sera gave higher values for transferrin when measured by immunoelectrophoresis. Unsaturated iron-binding capacity was increased in the isotope-dilution method in some iron-saturated plasma, compounding errors when added to erroneously high PI values to compute TIBC. Perhaps some exchange of iron occurred between added iron and transferrin iron in the isotope-dilution method. These measurements confirm the accuracy of the standard colorimetric method of measuring PI and TIBC except in iron-saturated plasma. However, the greater specificity of a polyclonal immunoprecipitation method of measuring PI and TIBC makes it particularly useful in differentiating transferrin-bound iron from nontransferrin iron.

Abstract We surveyed 140 clinical chemistry laboratories in Australia to establish which laboratory methods they used to determine serum iron status: 125 measured serum iron (Fe), 85 measured transferrin (TRF), 47 measured total iron-binding capacity (TIBC), and 14 measured both TRF and TIBC. Of the 55 laboratories routinely reporting TRF saturation (TS), 16 calculated TS directly as (Fe/TIBC) x 100, and 9 used [Fe/(TRF x 2)] x 100. Thirty laboratories measured TRF and converted it to an equivalent TIBC concentration; the derived TIBC was then used to calculate TS. We measured iron, TIBC, and TRF concentrations in 94 control subjects, 59 patients with alcoholic liver disease (ALD), and 20 with proven genetic hemochromatosis (GH). TS was compared with a transferrin index (TI = Fe/TRF) to determine whether both methods were sensitive for GH screening and which method gave the fewest false-positive results with discrimination limits of &gt; 55% and &gt; 1.0, respectively. All GH patients were detected by both TS and TI at these limits. One control subject had a TI &gt; 1.0, whereas three control subjects had a TS &gt; 55%. Nine patients with ALD had a TI &gt; 1.0 and 11 ALD patients had a TS &gt; 55%. Some iron-overload patients had lower than expected TS values compared with TI, possibly because of ferritin interference in the TIBC assay. Also, the precision of the TRF assay was better than that of the TIBC assay: CVs of 1.85-3.68% vs 6.17%. We therefore recommend that calculated TI replace TS in screening for iron overload.

TiBCN coatings were prepared by magnetron sputtering installed with two kinds of target materials (Ti and TiB2) in a mixed Ar, N2 and C2H2 ambient. The results showed that the TiBCN coatings could be successfully obtained by reactive sputtering with Ti and TiB2 dual targets. Raman spectroscopy indicated that TiBCN had similar spectra with DLC, which confirmed with high C content, in the wave range of 1000 ~ 2000 cm-1. The hardness of the coatings would decrease with an increase of N and the sp3 bond in the coatings. The friction coefficient against soda-lime glass ball was decreased from 1.3 of blank WC-Co substrate to 0.35 of the coatings in dry sliding condition.

Background. Diabetes mellitus (DM) is characterized by multiple risk factors for the combined development of disorders of phosphorus-calcium metabolism, due to more frequent overweight, decreased renal filtration function and vitamin D deficiency in this category of patients. Aim. To assess the level of calcium in blood serum and its correlations with parameters of carbohydrate metabolism, body mass index (BMI) and renal function in patients with type 1 and type 2 diabetes. Materials and methods. The object of the study: adult patients with type 1 diabetes (T1DM) and type 2 diabetes (T2DM) ≥18 years old who underwent examination in Diamodule (n=294) in 2019 in the Voronezh region, who were additionally tested for serum calcium. The examination at the mobile medical center includes: assessment of anthropometric data (height, weight, BMI), biochemical data of blood lipids and creatinine levels with calculation of glomerular filtration rate (GFR-EPI), albuminuria and the albumin/creatinine in a single portion of urine, measurement of glycated hemoglobin (HbA1c), blood pressure, electrocardiogram, consultation with a cardiologist, ophthalmologist, specialist of the Diabetic Foot office, diabetologist. All biochemical parameters was carried out using commercial kits on a biochemical express analyzer Spotchem EZ ArkraySP-4430. Determination of HbA1c, albuminuria, albumin/creatinine was performed by immunochemical method on a DCA Vantage analyzer. Data analysis was carried out using the Statistica v.13.3 software package (TIBCO Software Inc., USA). The results are presented as medians and quartiles [25; 75], the level of reliability is taken as p<0.05. Results. Hypocalcemia (serum calcium level less 2.15 mmol/L) was detected in 88.3% of patients with T1DM and 73.6% of patients with T2DM. The level of serum calcium in T1DM was 1.96 [1.83; 2.07], in T2DM – 2.04 [1.97; 2.16] (p<0.001) with significant differences in GFR: in T1DM 69.6 [57.8; 82.5], with T2DM 50.5 [44.1; 59.9] (p<0.001). We observed correlation between GFR and calcium level r=-0.3 (p<0.05,). GFR<60 ml/min/1.73 m² was observed in 30.5% of T1DM patients, in 75.0% of T2DM. The HbA1c in T1DM was 8.7% [7.7; 9.8], in T2DM – 8.3% [6.8; 9.4] (p=0.01). Obesity was revealed in T1DM in 12.3%, in T2DM in 71.4%; the median BMI in T1DM was 25.5 kg/m2 [21.9; 28.4], in T2DM – 33.2 kg/m2 [29.7; 37.9]. There was correlation between BMI and calcium level r=0.26 (p<0.05). Conclusion. The results of the study revealed a high prevalence of hypocalcemia among patients with diabetes, the most pronounced in type 1 diabetes. In the presence of a correlation between calcium and GFR, there was a high incidence of hypocalcemia in patients with T1DM, even in the absence of a significant decrease in GFR, which may indicate to additional risk factors. Since the assessment of the level of calcium is not included in the list of standard clinical examination, it is recommended to include this parameter and conduct regular screening in risk groups, which include patients with diabetes. More extensive research is needed to analyze the factors.

e17536 Background: During the ovarian carcinogenesis, circulating neutrophils (Nph) phenotype switches to the pro-tumor, with an increase in the Nph C-C motif ligand 2 (CCL2) expression. Proinflammatory interleukin-17A (IL-17A) is able to induce the CCL2 expression in Nph. The study aimed to evaluate the IL-17A and CCL2 levels in serum and circulating Nph in patients with ovarian tumors. Methods: The study included 97 ovarian cancer (OC) patients, 30 patients with benign ovarian tumors (BT) and the control (n=22). The levels of IL-17A (LLC "Cytokine", Russia) and CCL2 (Vector-Best-Volga, Russia) in serum and Nph lysate were assessed by ELISA. The systemic inflammation was assessed by neutrophil to lymphocyte ratio (NLR). Informed voluntary consent was obtained from all women. Statistical processing was performed using one-way ANOVA, Spearman correlations (Statistica 13.0 (TIBCO, USA)). Results: We found an increase in NLR in BT (4.3 ± 1.0) compared with the control (2.2 ± 0.1) (p = 0.020), a decrease in stage I-II OC (2.4 ± 0.2) compared with BT (p = 0.053), and an increase in stage III (4.5 ± 0.9) and IV (4.6 ± 0.5) compared with stage I-II OC (p = 0.054 and p = 0.046, respectively). The cytokines levels are presented in Table. The serum level of IL-17A in BT was higher than in the control (p = 0.010). In stage I-II OC, the serum IL-17A level was decreased in comparison with the BT (p = 0.004). In stage IV OC, the serum IL-17A was higher than in stage I-II (p = 0.015) and the control (p = 0.017). The level of IL-17A in Nph in BT did not differ from the control. In all stages of the OC, the IL-17A Nph level was lower than in the control (p1, 2, 3 = 0.0001). In stage IV OC, the expression of IL-17A in Nph was lower compared with stage III (p = 0.031). The serum and Nph levels of IL-17A were correlated only in stage I-II OC (r = 0.679, p = 0.011). The CCL2 serum levels in BT (p = 0.0002) and OC stage I-II, III, IV were higher compared with the control (p1 = 0.031, p2 = 0.001, p3 = 0.0005, respectively). The Nph CCL2 level was higher in BT than in the control (p = 0.001). In stage I-II OC, the expression of CCL2 in Nph was higher than in the control (p = 0.034), but lower than in the BT (p = 0.003). In OC stages III and IV, the Nph CCL2 level was higher than in the control, but did not differ from that in BT and stages I-II. In stage III OC, the CCL2 level in Nph negatively correlated with the serum IL-17A level (r = -0.405, p = 0.049). Conclusions: The pro-tumor polarization of circulating Nph is not IL-17A-dependent in patients with ovarian tumors. The patterns of the systemic immune response differ in benign ovarian tumors, early, and advanced ovarian cancer.[Table: see text]

Abstract Background: Present methods for measuring serum total iron-binding capacity (TIBC) involve manipulation of samples or performance of two assays on each sample. We developed a direct automated assay (DTIBC) for TIBC. Methods: We added to serum a saturating amount of iron bound to an excess of chelating dye at a low pH, recorded a blank reading that represented the sum of the saturating amount of iron plus the serum iron, and then added a strong neutral pH buffer. The decrease in absorbance (as transferrin extracts iron from the iron-dye complex) is directly proportional to the TIBC. TIBC values for 125 patients were determined by DTIBC, alumina column TIBC (AC), magnetic particle TIBC (MTIBC), and the UIBC method (UIBC) on Roche COBAS FARA and Mira chemistry analyzers. In a separate study, TIBC values for 128 patients were determined on an Olympus AU400 by the DTIBC and the MTIBC methods. Results: Methods comparisons on the COBAS analyzers yielded the following results: DTIBC = 1.05(MTIBC) − 1.0 μmol/L (r = 0.987; Sy|x = 2.6 μmol/L); DTIBC = 1.07(AC) − 1.0 μmol/L (r = 0.982; Sy|x = 3.0 μmol/L); and DTIBC = 1.14(UIBC) + 3.4 μmol/L (r = 0.982; Sy|x = 3.0 μmol/L). A similar correlation study using the Olympus AU400 yielded DTIBC = 1.00(MTIBC) − 0.1 μmol/L (r = 0.983; Sy|x = 2.7 μmol/L). The assay was linear from 12.5 to 125 μmol/L (70–700 μg/dL) TIBC on the COBAS FARA. Within- and between-run imprecision (CV) was ≤4.8% at two concentrations. Plasma samples were unsuitable for the method. No interference was seen with common interferants other than ascorbate, deferoxamine, and ferrous sulfate, and only at concentrations well above normal. Conclusion: The new DTIBC assay is suitable for routine use in clinical laboratories and may improve the quality of iron metabolism studies.

Data and information were obtained by observation and structured interviews to “Tibo-Tibo” fisherwomen, business partners and village governments. The data was analyzed qualitatively by using Miles Huberman perspective, through stages: data reduction, data presentation, conclusion drawing and verification. After going through the validity of experts, this paper produced a conceptual model of informal business development based on intellectual capital namely human capital, social capital and organizational capital. This paper recommends that through the intellectual capital approach "Tibo-Tibo" fisher women's business can be improved through empowerment. Empowerment in the form of human capital will increase the added value products with the availability of a variety of products for frozen food businesses and products for culinary businesses. Empowerment in the form of social capital will strengthen the values of relationships with business partners, trust to customers and employees and strengthen the work culture "mapalus" (local wisdom for the community) such as cooperation and helping each other. Empowerment in the form of organizational will strengthening the management and organizational professionalism as well as the acknowledgment of business license and product brands by consumers.

Fcc-TiBCN powder synthesized by boronizing of Ti was pressured into monolithic block by hot pressing (HP) method. Then the block material was machined by wire-cut EDM. The machinability and the material removal mechanisms were discussed of Fcc-TiBCN block in wire-cut EDM. Results show that the machinability of Fcc-TiBCN is satisfying. The discharge current has little relationship on surface roughness in the range of factory-adjusted optimum settings. The discharge duration and pulse interval time play an important role on the quality of machined surface and EDM machining efficiency. In order to obtain excellent quality of the machined surface, the number of power tube turned on can not exceed 6, and the pulse interval time must be as long as possible. The mini roughness of machined surface is 0.4μm~0.6μm as the number of power tube turned on is 5, the pulse interval time and discharge duration is set in the range of 20~22μs and 65~75μm, respectively. There are three material removal mechanisms of TiBCN conductive ceramic in the EDM machining process: spalling, melting and evaporation. Spalling is the main mechanism resulted by thermal stress superposing.

Introducción: La afección articular que lleva a la necesidad de una artrodesis tibio-calcánea puede estar acompañada de una segunda lesión en el mismo segmento óseo, y los clavos de diseño para artrodesis no tienen la longitud adecuada para cubrir ambas lesiones. No hallamos opciones terapéuticas para este tipo de lesiones en la bibliografía. Objetivo: Presentar una serie de casos en los que se utilizó un único clavo de tibia (de colocación anterógrada) de forma retrógrada, con el doble objetivo de efectuar una artrodesis tibio-talo-calcánea, sumada al tratamiento de una lesión asociada en un mismo tiempo quirúrgico. Materiales y Métodos: Se evaluó, en forma retrospectiva, desde septiembre de 2009 hasta junio de 2019, a un grupo de 12 pacientes que requirió una artrodesis tibio-talo-calcánea sumada a la necesidad de resolver simultáneamente un defecto secundario local. La edad promedio fue de 43.7 años, y el seguimiento global fue de 43.9 meses. Resultados: Todos los pacientes lograron una artrodesis tibio-calcánea constatada en radiografías de frente y de perfil, y el 83,3% recuperó el stock óseo de manera completa. Conclusión: Ante la ausencia en el mercado de osteosíntesis para resolver las patologías asociadas en un mismo paciente, proponemos el uso del clavo endomedular largo de tibia colocado de manera retrógrada como una opción de tratamiento, porque se ha demostrado que es eficaz para lograr la artrodesis tibio-calcánea. Además, se lo pudo utilizar como guía en el alargamiento y el transporte óseo, y como estabilización para tratar lesiones simultáneas.
