Abstract:
Pharyngitis typically presents with sore throat, fever, and pharyngeal inflammation. More serious cases may progress to peritonsillar abscess (PTA). Infection or inflammation of the throat that primarily affects the palantine tonsils is considered tonsillitis, whereas infections involving the posterior pharynx are considered pharyngitis. Weber glands (a group of mucous salivary glands superior to the tonsil in the soft palate) have been implicated in the formation of PTA; one must consider their proximity to the trachea and to the carotid artery when pursuing fine needle aspiration. Treatment for all cases includes supportive care with antipyretics, analgesics, and fluids. Despite the much lower prevalence of group A beta-hemolytic Streptococcus infection (for which antibiotics is indicated), a staggering 49% to 57% of children and 64% of adults evaluated for pharyngitis receive an antibiotic prescription. Up to 90% of primary infections with HIV-1 are associated with acute retroviral syndrome, which can include pharyngitis 2 to 4 weeks after exposure.

Abstract:
Infections of the nasal cavity, sinuses, pharynx, epiglottis, and larynx are termed upper respiratory tracts infections. These include acute coryza, pertussis, sinusitis, pharyngitis, tonsillitis, epiglottitis, laryngitis, laryngotracheobronchitis, and influenza. Rhinoviruses and coronaviruses account for the majority of acute coryzal illnesses. Acute sinusitis (<4 weeks duration) is also usually viral in origin. About 70% of pharyngitis and tonsillitis cases are viral in etiology. Haemophilus influenzae (Type B) is responsible for most cases of epiglottitis. Acute laryngitis and laryngotracheobronchitis are usually caused by human parainfluenza viruses. This chapter focuses on upper respiratory tract infections, including their etiology, symptoms, demographics, natural history, complications, diagnosis, prognosis, and treatment.

Abstract:
Post-tonsillectomy hemorrhage (PTH) is a very serious complication that can occur after a tonsillectomy. There are two broad categories of PTH, based on onset of bleeding: primary (less than 24 hours) or secondary (greater than 24 hours). Primary bleeding is often attributed to the surgical technique, and secondary bleeding is attributed to sloughing of healing surgical scar. Risk factors for PTH include male gender, age greater than 70 years (in adults), age greater than 5 years (in pediatrics), recurrent tonsillitis, use of nonsteroidal anti-inflammatory drugs (particularly ketorolac), among others. Rapid-sequence induction and intubation is advised to reduce the risk of aspiration for patients presenting with PTH.
